Abstract:OBJECTIVE:To confirm the diagnostic performance of computed tomography (CT)-based texture analysis (CTTA) and magnetic resonance imaging (MRI)-based texture analysis for grading cartilaginous tumors in long bones and to compare these findings to radiological features.MATERIALS AND METHODS:Twenty-nine patients with enchondromas, 20 with low-grade chondrosarcomas and 16 with high-grade chondrosarcomas were included retrospectively. Clinical and radiological information and 9 histogram features extracted from CT, T1WI, and T2WI were evaluated. Binary logistic regression analysis was performed to determine predictive factors for grading cartilaginous tumors and to establish diagnostic models. Another 26 patients were included to validate each model. Receiver operating characteristic (ROC) curves were generated, and accuracy rate, sensitivity, specificity and positive/negative predictive values (PPV/NPV) were calculated.RESULTS:On imaging, endosteal scalloping, cortical destruction and calcification shape were predictive for grading cartilaginous tumors. For texture analysis, variance, mean, perc.01%, perc.10%, perc.99% and kurtosis were extracted after multivariate analysis. To differentiate benign cartilaginous tumors from low-grade chondrosarcomas, the imaging features model reached the highest accuracy rate (83.7%) and AUC (0.841), with a sensitivity of 75% and specificity of 93.1%. The CTTA feature model best distinguished low-grade and high-grade chondrosarcomas, with accuracies of 71.9%, and 80% in the training and validation groups, respectively; T1-TA and T2-TA could not distinguish them well. We found that the imaging feature model best differentiated benign and malignant cartilaginous tumors, with an accuracy rate of 89.2%, followed by the T1-TA feature model (80.4%).CONCLUSIONS:The imaging feature model and CTTA- or MRI-based texture analysis have the potential to differentiate cartilaginous tumors in long bones by grade. MRI-based texture analysis failed to grade chondrosarcomas.
Soft Tissue Sarcomas: Preoperative Predictive Histopathological Grading Based on Radiomics of MRI

Abstract:Rationale and objectives: The purpose of this study is to develop a radiomics model for predicting the histopathological grades of soft tissue sarcomas preoperatively through magnetic resonance imaging (MRI). Materials and methods: Thirty-five patients who were pathologically diagnosed with soft tissue sarcomas and their histological grades were recruited. All patients had undergone MRI before surgery on a 3.0T MRI scanner. Radiomics features were extracted from fat-suppressed T2-weighted imaging. We used the least absolute shrinkage and selection operator (LASSO) regression method to select features. Then three machine learning classification methods, including random forests, k-nearest neighbor, and support vector machine algorithm were trained using the 5-fold cross validation strategy to separate the soft tissue sarcomas with low- and high-histopathological grades. Results: The radiomics features were significantly associated with the histopathological grades. Quantitative imaging features (n = 1049) were extracted from fat-suppressed T2-weighted imaging, and five features were selected to construct the radiomics model. The model that used support vector machine classification method achieved the best performance among the three methods, with areas under the receiver operating characteristic curves Area Under Curve (AUC) values of 0.92 ± 0.07, accuracy of 0.88. Conclusion: Good accuracy and AUC could be obtained using only five radiomic features. Therefore, we proposed that three-dimensional imaging features from fat-suppressed T2-weighted imaging could be used as candidate biomarkers for preoperative prediction of histopathological grades of soft tissue sarcomas noninvasively.

Model performances (discrimination, calibration, and clinical usefulness) were validated in the external validation set. The RS‐T1 model, RS‐FST2 model, and RS‐Combined model had an area under curves (AUCs) of 0.645, 0.641, and 0.829, respectively, in the external validation set. Predicting the Malignancy Grade of Soft Tissue Sarcomas on MRI Using Conventional Image Reading and Radiomics

Abstract:Objectives: This study aims to investigate MRI features predicting the grade of STS malignancy using conventional image reading and radiomics. Methods: Pretherapeutic imaging data regarding size, tissue heterogeneity, peritumoral changes, necrosis, hemorrhage, and cystic degeneration were evaluated in conventional image reading. Furthermore, the tumors' apparent diffusion coefficient (ADC) values and radiomics features were extracted and analyzed. A random forest machine learning algorithm was trained and evaluated based on the extracted features. Results: A total of 139 STS cases were included in this study. The mean tumor ADC and the ratio between tumor ADC to healthy muscle ADC were significantly lower in high-grade tumors (p = 0.001 and 0.005, respectively). Peritumoral edema (p < 0.001) and peritumoral contrast enhancement (p < 0.001) were significantly more extensive in high-grade tumors. Tumor heterogeneity was significantly increased in high-grade sarcomas, particularly in T2w- and contrast-enhanced sequences using conventional image reading (p < 0.001) as well as in the radiomics analysis (p < 0.001). Our trained random forest machine learning model predicted high-grade status with an area under the curve (AUC) of 0.97 and an F1 score of 0.93. Biopsy-underestimated tumors exhibited differences in tumor heterogeneity and peritumoral changes. Conclusions: Tumor heterogeneity is a key characteristic of high-grade STSs, which is discernible through conventional imaging reading and radiomics analysis. Higher STS grades are also associated with low ADC values, peritumoral edema, and peritumoral contrast enhancement.

Abstract:Background Soft tissue sarcomas are rare malignant tumors with significant heterogeneity. The importance of classifying histological grades is fundamental to defining the treatment approach. Objective To evaluate magnetic resonance imaging (MRI) in predicting the histological grade of soft tissue sarcomas. Methods A retrospective observational study included patients over 18 years undergoing MRI and primary tumor surgery at AC Camargo Cancer Center from January 2015 to June 2022. Abstract:Purpose To create a prognostic prediction radiomics model for soft tissue sarcoma (STS) of the extremities and trunk treated with neoadjuvant radiotherapy. Methods This study included 62 patients with STS of the extremities and trunk who underwent magnetic resonance imaging (MRI) before neoadjuvant radiotherapy. After tumour segmentation and preprocessing, 851 radiomics features were extracted. The radiomics score was constructed according to the least absolute shrinkage and selection operator (LASSO) method. Survival analysis (disease-free survival; DFS) was performed using the log-rank test and Cox’s proportional hazards regression model. The nomogram model was established based on the log-rank test and Cox regression model. Harrell’s concordance index (C-index), calibration curve and receiver operating characteristic (ROC) curve analysis were used to evaluate the prognostic factors. The clinical utility of the model was assessed by decision curve analysis (DCA). Results The univariate survival analysis showed that tumour location (p = 0.032), clinical stage (p = 0.022), tumour size (p = 0.005) and the radiomics score were correlated with DFS (p < 0.05). The multivariate analysis showed that tumour location, tumour size, and the radiomics score were independent prognostic factors for DFS (p < 0.05). The combined clinical-radiomics model based on the multivariate analysis showed the best predictive ability for DFS (C-index: 0.781; Area Under Curve: 0.791). DCA revealed that the use of the radiomics score-based nomogram was associated with better benefit gains relative to the prediction of 2-year DFS events than other models in the threshold probability range between 0.12 and 0.38. Conclusion The radiomics score from pretreatment MRI is an independent prognostic factor for DFS in patients with STS of the extremities and trunk. The radiomics score-based nomogram could improve prognostic stratification ability and thus contribute to individualized therapy for STS patients.

Abstract:Soft tissue sarcomas (STS) are a heterogeneous group of rare malignant tumors. Tumor grade might be underestimated in biopsy due to intratumoral heterogeneity. This mini-review aims to present the current state of predicting malignancy grades of STS through radiomics, machine learning, and deep learning on magnetic resonance imaging (MRI). Several studies investigated various machine-learning and deep-learning approaches in T2-weighted (w) images, contrast-enhanced (CE) T1w images, and DWI/ADC maps with promising results. Combining semantic imaging features, radiomics features, and deep-learning signatures in machine-learning models has demonstrated superior predictive performances compared to individual feature sources. Furthermore, incorporating features from both tumor volume and peritumor region is beneficial. Especially random forest and support vector machine classifiers, often combined with the least absolute shrinkage and selection operator (LASSO) and/or synthetic minority oversampling technique (SMOTE), did show high area under the curve (AUC) values and accuracies in existing studies.
Prediction of Treatment Response and Metastatic Disease in Soft Tissue Sarcoma

Abstract:Soft tissue sarcomas (STS) are a heterogenous group of malignant tumors comprised of more than 50 histologic subtypes. Based on spatial variations of the tumor, predictions of the development of necrosis in response to therapy as well as eventual progression to metastatic disease are made. Optimization of treatment, as well as management of therapy-related side effects, may be improved using progression information earlier in the course of therapy. Multi-modality pre- and post-gadolinium enhanced magnetic resonance images (MRI) were taken before and after treatment for 30 patients. Regional variations in the tumor bed were measured quantitatively. The voxel values from the tumor region were used as features and a fuzzy clustering algorithm was used to segment the tumor into three spatial regions. The regions were given labels of high, intermediate and low based on the average signal intensity of pixels from the post-contrast T1 modality. These spatially distinct regions were viewed as essential meta-features to predict the response of the tumor to therapy based on necrosis (dead tissue in tumor bed) and metastatic disease (spread of tumor to sites other than primary). The best feature was the difference in the number of pixels in the highest intensity regions of tumors before and after treatment. This enabled prediction of patients with metastatic disease and lack of positive treatment response (i.e. less necrosis). The best accuracy, 73.33%, was achieved by a Support Vector Machine in a leave-one-out cross validation on 30 cases predicting necrosis < 90% post treatment and metastasis.
Radiomics model for predicting distant metastasis in soft tissue sarcoma of the extremities and trunk treated with surgery

Abstract:Purpose: The aim of this study was to develop a radiomics model based on magnetic resonance imaging (MRI) for predicting metastasis in soft tissue sarcomas (STSs) treated with surgery. Methods/patients: MRI and clinical data of 73 patients with STSs of the extremities and trunk were obtained from TCIA database and Jiangsu Cancer Hospital as the training set, data of other 40 patients were retrospectively collected at our institution as the external validation set. Radiomics features were extracted from both intratumoral and peritumoral regions of fat-suppressed T2-weighted images (FS-T2WIs) of patients, and 3D ResNet10 was used to extract deep learning features. Recursive feature elimination (RFE) and least absolute shrinkage and selection operator (LASSO) algorithms were used for the selection of features. Based on 4 different sets of features, 5 machine learning algorithms were used to construct intratumor, peritumor, combined intratumor and peritumor radiomics models and deep learning radiomics (DLR) model. The area under the ROC curve (AUC) and Decision curve analysis (DCA) were used to evaluate the ability of models to predict metastasis. Results and conclusions: Based on 20 selected features from the deep-learning and radiomics features set, the DLR model was able to predict metastasis in the validation dataset, with an AUC of 0.9770. The DCA and Hosmer-Lemeshow test revealed that the DLR model had good clinical benefit and consistency. By getting richer information from MRI, The DLR model is a noninvasive, low-cost method for predicting the risk of metastasis in STSs, and can help develop appropriate treatment programs.

Abstract:Objective To assess sarcoma margins with more accuracy and aid surgical planning, we constructed three-dimensional (3D) digital models with computed tomography(CT) and magnetic resonance imaging (MRI) image fusion data and validated the preciseness of the models by comparing them with 3D models constructed with CT only data. Materials and Methods We retrospectively reviewed a consecutive set of patients treated in our center who were preoperatively evaluated with the fusion image model. Models based on fusion images or CT-only data were constructed. Volumes of both tumors were calculated and the tumors were overlapped to see the location of differences between the two models. Results A consecutive 12 cases (4 male vs. 8 female) were included in this study. Most of the tumors were located in the pelvic bone or spine. The volume of the two tumor models was different and the differences were mainly in the peripheral region of the tumor. Conclusion CT and MRI fusion image 3D models are more accurate than models with CT-only data and can be very helpful in preoperative planning of sarcoma patients.
Development and External Validation of Deep-Learning-Based Tumor Grading Models in Soft-Tissue Sarcoma Patients Using MR Imaging

Abstract:Background: In patients with soft-tissue sarcomas, tumor grading constitutes a decisive factor to determine the best treatment decision. Tumor grading is obtained by pathological work-up after focal biopsies. Deep learning (DL)-based imaging analysis may pose an alternative way to characterize STS tissue. In this work, we sought to non-invasively differentiate tumor grading into low-grade (G1) and high-grade (G2/G3) STS using DL techniques based on MR-imaging. Methods: Contrast-enhanced T1-weighted fat-saturated (T1FSGd) MRI sequences and fat-saturated T2-weighted (T2FS) sequences were collected from two independent retrospective cohorts (training: 148 patients, testing: 158 patients). Tumor grading was determined following the French Federation of Cancer Centers Sarcoma Group in pre-therapeutic biopsies. DL models were developed using transfer learning based on the DenseNet 161 architecture. Results: The T1FSGd and T2FS-based DL models achieved area under the receiver operator characteristic curve (AUC) values of 0.75 and 0.76 on the test cohort, respectively. T1FSGd achieved the best F1-score of all models (0.90). The T2FS-based DL model was able to significantly risk-stratify for overall survival. Attention maps revealed relevant features within the tumor volume and in border regions. Conclusions: MRI-based DL models are capable of predicting tumor grading with good reproducibility in external validation.

Abstract:ObjectiveTo systematically review the literature assessing the role of Dynamic Contrast-Enhanced Magnetic Resonance Imaging (DCE-MRI) in the differentiation of soft tissue sarcomas from benign lesions.Materials and methodsA comprehensive literature search was performed with the following keywords: multiparametric magnetic resonance imaging, DCE-MR perfusion, soft tissue, sarcoma, and neoplasm. Original studies evaluating the role of DCE-MRI for differentiating benign soft-tissue lesions from soft-tissue sarcomas were included.ResultsEighteen studies with a total of 965 imaging examinations were identified. Ten of twelve studies evaluating qualitative parameters reported improvement in discriminative power. One of the evaluated qualitative parameters was time-intensity curves (TIC), and malignant curves (TIC III, IV) were found in 74% of sarcomas versus 26.5% benign lesions. Six of seven studies that used the semiquantitative approach found it relatively beneficial. Four studies assessed quantitative parameters including Ktrans (contrast transit from the vascular compartment to the interstitial compartment), Kep (contrast return to the vascular compartment), and Ve (the volume fraction of the extracellular extravascular space) in addition to other parameters. All found Ktrans, and 3 studies found Kep to be significantly different between sarcomas and benign lesions. The values for Ve were variable. Additionally, eight studies assessed diffusion-weighted imaging (DWI), and 6 of them found it useful.ConclusionOf different DCE-MRI approaches, qualitative parameters showed the best evidence in increasing the diagnostic performance of MRI. Semiquantitative and quantitative approaches seemed to improve the discriminative power of MRI, but which parameters and to what extent is still unclear and needs further investigation.

Abstract:Abstract Objectives To construct effective prediction models for neoadjuvant radiotherapy (RT) and targeted therapy based on whole-tumor texture analysis of multisequence MRI for soft tissue sarcoma (STS) patients. Methods Thirty patients with STS of the extremities or trunk from a prospective phase II trial were enrolled for this analysis. All patients underwent pre- and post-neoadjuvant RT MRI examinations from which whole-tumor texture features were extracted, including T 1 -weighted with fat saturation and contrast enhancement (T 1 FSGd), T 2 -weighted with fat saturation (T 2 FS), and diffusion-weighted imaging (DWI) sequences and their corresponding apparent diffusion coefficient (ADC) maps. According to the postoperative pathological results, the patients were divided into pathological complete response (pCR) and non-pCR (N-pCR) groups. pCR was defined as less than 5% of residual tumor cells by postoperative pathology. Delta features were defined as the percentage change in a texture feature from pre- to post-neoadjuvant RT MRI. After data reduction and feature selection, logistic regression was used to build prediction models. ROC analysis was performed to assess the diagnostic performance. Results Five of 30 patients (16.7%) achieved pCR. The Delta_Model (AUC 0.92) had a better predictive ability than the Pre_Model (AUC 0.78) and Post_Model (AUC 0.76) and was better than AJCC staging (AUC 0.52) and RECIST 1.1 criteria (AUC 0.52). The Combined_Model (pre, post, and delta features) had the best predictive performance (AUC 0.95). Conclusion Whole-tumor texture analysis of multisequence MRI can well predict pCR status after neoadjuvant RT and targeted therapy in STS patients, with better performance than RECIST 1.1 and AJCC staging. Key points • MRI multisequence texture analysis could predict the efficacy of neoadjuvant RT and targeted therapy for STS patients. • Texture features showed incremental value beyond routine clinical factors. • The Combined_Model with features at multiple time points showed the best performance.

Abstract:Objectives: To construct an optimal magnetic resonance imaging (MRI) texture model to evaluate histological patterns and predict prognosis in patients with osteosarcoma (OS). Methods: Thirty-four patients underwent pretreatment MRI and were diagnosed as having OS by surgical resection or biopsy between September 2008 and June 2018. Histological patterns and 3-year survival were recorded. Manual segmentation was performed in intraosseous, extraosseous, and entire lesions on T1-weighted, T2-weighted, and contrast-enhanced T1-weighted images to extract texture features and perform principal component analysis. A support vector machine algorithm with 3-fold cross-validation was used to construct and validate the models. The area under the receiver operating characteristic curve (AUC) was calculated to evaluate diagnostic performance in evaluating histological patterns and 3-year survival. Results: Eight patients were chondroblastic and the remaining twenty-six patients were non-chondroblastic patterns. Twenty-seven patients were 3-year survivors, and the remaining seven patients were non-survivors. In discriminating chondroblastic from non-chondroblastic patterns, the model from extraosseous lesions on the T2-weighted images showed the highest diagnostic performance (AUCs of 0.94 and 0.89 in the training and validation sets). The model from intraosseous lesions on the T1-weighted images showed the highest diagnostic performance in discriminating 3-year non-survivors from survivors (AUCs of 0.99 and 0.88 in the training and validation sets) with a sensitivity, specificity, positive predictive value, and negative predictive value of 85.7%, 92.6%, 75.0%, and 96.2%, respectively. Conclusions: The texture models of extraosseous lesions on T2-weighted images can discriminate the chondroblastic pattern from non-chondroblastic patterns, while the texture models of intraosseous lesions on T1-weighted images can discriminate 3-year non-survivors from survivors.

Abstract:Background Ki‐67 expression has been shown to be an important risk factor associated with prognosis in patients with soft tissue sarcomas (STSs). Its assessment requires fine‐needle biopsy and its accuracy can be influenced by tumor heterogeneity. Purpose To develop and test an MRI‐based radiomics nomogram for identifying the Ki‐67 status of STSs. Study type Retrospective. Population A total of 149 patients at two independent institutions (training cohort [high Ki‐67/low ki‐67]: 102 [52/50], external validation cohort [high Ki‐67/low ki‐67]: 47 [28/19]) with STSs. Field Strength/Sequence Fat‐saturated T2‐weighted imaging (FS‐T2WI) with a fat‐suppressed fast spin/turbo spin echo sequence at 1.5 T or 3 T. Assessment After radiomics feature extraction, logistic regression (LR), random forest (RF), support vector machine (SVM), and k‐nearest neighbor (KNN) were used to construct radiomics models to distinguish between high and low Ki‐67 status. Clinical‐MRI characteristics included age, gender, location, size, margin, and MRI morphological features (size, margin, signal intensity, and peritumoral hyperintensity) were assessed. Univariate and multivariate logistic regression analysis were applied for screening significant risk factors. Radiomics nomogram was constructed by radiomics signature and risk factors. Statistical Tests Model performances (discrimination, calibration, and clinical usefulness) were validated in the validation cohort. The nomogram was assessed using the Harrell index of concordance (C‐index), calibration curve analysis. The clinical utility of the model was assessed by decision curve analysis (DCA). Results LR, RF, SVM, and KNN models represented AUCs of 0.789, 0.755, 0.726, and 0.701 in the validation cohort (P > 0.05). The nomogram had a C‐index of 0.895 (95% CI: 0.837–0.953) in the training cohort and 0.852 (95% CI: 0.796–0.957) in the validation cohort and it demonstrated good calibration and clinical utility (P = 0.972 for the training cohort and P = 0.727 for the validation cohort). Data Conclusion This MRI‐based radiomics nomogram developed showed good performance in identifying Ki‐67 expression status in STSs. Level of Evidence 3. Technical Efficacy Stage 2.

Abstract:Background:Accurately predicting the prognosis of patients with high-grade glioma (HGG) is potentially important for treatment. However, the predictive value of images of various magnetic resonance imaging (MRI) sequences for prognosis at different time points is unknown. We established predictive machine learning models of HGG disease progression and recurrence using MRI radiomics and explored the factors influencing prediction accuracy.Methods:Radiomics features were extracted from T1-weighted (T1WI), contrast-enhanced T1-weighted (CE-T1WI), T2-weighted (T2WI), and fluid-attenuated inversion recovery (FLAIR) images (postoperative radiotherapy planning MRI images) obtained from 162 patients with HGG. The Mann-Whitney U test and least absolute shrinkage and selection operator (LASSO) algorithm were used for feature selection. Machine learning models were used to build prediction models to estimate disease progression or recurrence. The influence of different MRI sequences, regions of interest (ROIs), and prediction time points was also explored. The receiver operating characteristic (ROC) curve was used to evaluate the discriminative performance of each model, and the DeLong test was employed to compare the ROC curves.Results:Radiomics features from T2WI and FLAIR demonstrated greater predictive value for disease progression compared with T1WI or CE-TIWI. The best predictive models, with areas under the ROC curves (AUCs) of 0.70, 0.68, 0.78, 0.78, and 0.78 for predicting disease progression at the 6th, 9th, 12th, 15th, and 18th month after radiotherapy, respectively, were obtained by combining clinical features with gross tumor volume (GTV) and clinical target volume (CTV) features extracted from T2WI and FLAIR.Conclusions:Structural MRI obtained before radiotherapy can be used to predict the disease progression or posttreatment recurrence of HGG. When using MRI radiomics to predict long-term outcomes as opposed to short-term outcomes, better predictive results may be obtained.
Machine Learning-Based Radiomics Predicting Tumor Grades and Expression of Multiple Pathologic Biomarkers in Gliomas

Abstract:<span><b>Background:</b> The grading and pathologic biomarkers of glioma has important guiding significance for the individual treatment. In clinical, it is often necessary to obtain tumor samples through invasive operation for pathological diagnosis. The present study aimed to use conventional machine learning algorithms to predict the tumor grades and pathologic biomarkers on magnetic resonance imaging (MRI) data.<b>Methods:</b> The present study retrospectively collected a dataset of 367 glioma patients, who had pathological reports and underwent MRI scans between October 2013 and March 2019. The radiomic features were extracted from enhanced MRI images, and three frequently-used machine-learning models of LC, Support Vector Machine (SVM), and Random Forests (RF) were built for four predictive tasks: (1) glioma grades, (2) Ki67 expression level, (3) GFAP expression level, and (4) S100 expression level in gliomas. Each sub dataset was split into training and testing sets at a ratio of 4:1. The training sets were used for training and tuning models. The testing sets were used for evaluating models. According to the area under curve (AUC) and accuracy, the best classifier was chosen for each task.<b>Results:</b> The RF algorithm was found to be stable and consistently performed better than Logistic Regression and SVM for all the tasks. The RF classifier on glioma grades achieved a predictive performance (AUC: 0.79, accuracy: 0.81). The RF classifier also achieved a predictive performance on the Ki67 expression (AUC: 0.85, accuracy: 0.80). The AUC and accuracy score for the GFAP classifier were 0.72 and 0.81. The AUC and accuracy score for S100 expression levels are 0.60 and 0.91.<b>Conclusion:</b> The machine-learning based radiomics approach can provide a non-invasive method for the prediction of glioma grades and expression levels of multiple pathologic biomarkers, preoperatively, with favorable predictive accuracy and stability.</span>

Abstract:Magnetic resonance imaging (MRI) is known for its accurate soft tissue delineation of tumors and normal tissues. This development has significantly impacted the imaging and treatment of cancers. Radiomics is the process of extracting high-dimensional features from medical images. Several studies have shown that these extracted features may be used to build machine-learning models for the prediction of treatment outcomes of cancer patients. Various feature selection techniques and machine models interrogate the relevant radiomics features for predicting cancer treatment outcomes. This study aims to provide an overview of MRI radiomics features used in predicting clinical treatment outcomes with machine learning techniques. The review includes examples from different disease sites. It will also discuss the impact of magnetic field strength, sample size, and other characteristics on outcome prediction performance.
