Abstract:PURPOSE: To evaluate the uveal and capsular biocompatibility of intraocular lenses (IOLs) with a hydrophilic anterior surface and a hydrophobic posterior surface in a rabbit model.SETTING: Eye Center, Affiliated Second Hospital, College of Medicine, Zhejiang University, Hangzhou, China.METHODS: Modified silicone IOLs were produced by grafting 2-methacryloyloxyethyl phosphorylcholine (MPC) onto the anterior IOL surface using a plasma technique. A contact-angle test characterized the hydrophilicity of the IOL surface; physical and optical properties were determined by State Food and Drug Administration (SFDA) standards. Rabbits had phacomulsification and implantation a modified silicone IOL, a control silicone IOL, or a hydrogel IOL. Postoperative inflammation was assessed by aqueous flare measurement, and PCO was evaluated by software analysis. Three months after surgery, attached cells on extracted IOLs were evaluated by light microscopy; PCO was evaluated by Miyake-Apple technique. Histologic sections of globes were used to assess lens epithelial cells (LECs) and extracellular matrix in the capsular bag.RESULTS: Contact angle data showed the MPC-modified IOL had a hydrophilic anterior surface and hydrophobic posterior surface. The properties of the modified IOLs met SFDA standards. There was no statistical difference in aqueous flare between the IOL groups at any time. The modified and control IOLs had less PCO than the hydrogel IOLs (P<.05). There were fewer cells on modified IOLs than on silicone IOLs (P<.05). The LECs and cortical remnants on modified IOLs had a rapid, fibroblastic appearance at the optic periphery; the center was clear.CONCLUSIONS: Results suggest that the MPC-modified IOL has excellent uveal and capsule biocompatibility from hydrophilic anterior surface and hydrophobic posterior surface properties, respectively.
Biocompatibility of membranous polymers in rabbit eyes

Abstract:Objective To investigate the biocompatibility of three membranous polymers,polyhydroxyalkanoates(PHA),polylacetic acid(PLA) and poly-ε-caprolactone(PCL) in the rabbit eyes.Methods Twenty four healthy New Zealand white rabbits were randomly divided into 4 groups,with 6 rabbits each.PHA,PLA and PCL polymers were implanted into subconjunctival space of the right eyes in rabbits.The sham-operation group underwent subconjunctival space blunt dissection,without polymers implantation.The reactions of the operated eyes at different time-points were scored according to the Experimental Eye Irritation criteria under slit lamp microscope.Absorption time of the polymers was also recorded.The eyeballs were enucleated at 4 weeks and 16 weeks after implantation,HE staining,Masson staining and Sirius red staining were used to examine the tissue structure and inflammatory cells,collagen and collagen fiber subtypes,respectively.Results Irritation scores of all experimental groups were less than the level of "mild irritant".The absorption time of PHA,PLA and PCL groups were 16 weeks,12 weeks and longer than16 weeks,respectively.Histological study revealed that,4 weeks after the implantation,a capsule was formed around the implanted membrane by fibroblasts and collagen fibers.Few capillaries and inflammation cells,neutrophils were also observed.The subtypes of collagen fibers showed no significant difference among groups.Sixteen weeks after the implantation,no PHA and PLA membranes could be observed at the implanted site,leaving an irregular cavity.PCL membrane was still present in situ.Capillaries were rare and a few lymphocytes were present.Conclusions Membranous PHA,PLA and PCL polymers show good biocompatibility in rabbit eyes,and their absorption time is 16 weeks,12 weeks and longer than 16 weeks,respectively.
[Experimental study of acellular xenogenic dermal matrix and allogeneic sclera as wrapping materials for hydroxy apatite implantation]

Abstract:Objective: To observe the experimental results and histopathological changes of acellular xenogenic dermal matrix (X-ADM) and allogeneic sclera used as wrapping materials of hydroxy apatite (HA) ocular implants in New Zealand white rabbits. Methods: Twenty-four rabbits received unilateral eye enucleating and the sockets were implanted with HA spherical implants wrapped with either acellular xenogenic dermal matrix or allogeneic sclera at random. The rabbits were examined for inflammation and implant exposure and sacrificed at 1, 2, 4, 6, 8 and 12 weeks after implantation. The sockets with the grafts were exenterated and the specimens were assessed histopathologically and ultrastructurally with light or transmission electron microscopy for the changes in inflammation reaction and vascularization. Results: Compared to allogeneic sclera at the same stage of implantation, acellular xenogenic dermal matrix demonstrated more active and earlier growth of fibroblasts and new vessels with abundant collagen deposition. There were few inflammatory cells and no rejection was found. Conclusion: This experiment showed that the acellular xenogenic dermal matrix, with fast neovascularization and low immunity, can be an ideal material of ocular implant and a good substitute for allogeneic sclera.
[Experimental study of using xenologous sclera as wrapping materials for hydroxy apatite(HA)]

Abstract:Objective: Use xenologous sclera (pig sclera) as wrapping materials for hydroxyapatite (HA) ocular implants in enucleated New Zealand white rabbits. Methods: Ten rabbits were unilaterally enucleated and the sockets implanted with 10 mm HA spherical implants. All implants were wrapped with pig sclera. The rabbits were killed 4 weeks later and the sockets were examined clinically for inflammation and implant exposure. Exenterated specimens were assessed histopathologically for inflammation and vascularization. Results: No clinical evidence of implant exposure or extrusion was detected in rabbits receiving pig sclera wrapped implants. Histopathologically the xenologous (pig) sclera displayed moderate inflammation, as well as maintain its lamellar arrangement without disruption. Fibrovascular ingrowth was complete in all implants. Conclusion: Xenologous (pig) sclera behaves similar clinically to homologous sclera as a wrapping material for spherical HA ocular implants in a rabbit model, and may represent a reasonable alternative to sclera in such procedures in humans.
In Vivo Study of the Biocompatibility of a Novel Compressed Collagen Hydrogel Scaffold for Artificial Corneas.

Abstract:The experiments were designed to evaluate the biocompatibility of a plastically compressed collagen scaffold (PCCS). The ultrastructure of the PCCS was observed via scanning electron microscopy. Twenty New Zealand white rabbits were randomly divided into experimental and control groups that received corneal pocket transplantation with PCCS and an amniotic membrane, respectively. And the contralateral eye of the implanted rabbit served as the normal group. On the 1st, 7th, 14th, 21st, 30th, 60th, 90th, and 120th postoperative day, the eyes were observed via a slit lamp. On the 120th postoperative day, the rabbit eyes were enucleated to examine the tissue compatibility of the implanted stroma. The PCCS was white and translucent. The scanning electron microscopy results showed that fibers within the PCCS were densely packed and evenly arranged. No edema, inflammation, or neovascularization was observed on ocular surface under a slit lamp and few lymphocytes were observed in the stroma of rabbit cornea after histological study. In conclusion, the PCCS has extremely high biocompatibility and is a promising corneal scaffold for an artificial cornea.
Efficacy, safety, and biodegradation of a degradable scleral buckle of chitosan-gelatin polymer in rabbits.

Abstract:Purpose: To evaluate the efficacy, safety, and biodegradation of chitosan-gelatin scleral buckles in rabbit eyes. Methods: Segmental scleral buckling was performed on the left eye of each rabbit. There were 40 rabbits in chitosan-gelatin group and 20 rabbits in silicone sponge group. B-scan ocular ultrasonography was used to evaluate buckling effect. Retinal function after implantation was evaluated by electroretinograms. Tissue reaction and biocompatibility was examined by histology. Scanning electron microscope was used to analyze structural changes after episcleral implantation. Degradation of chitosan-gelatin was assessed by mass loss. Results: Indentation of both implants decreased over time. However, the indentation created by chitosan-gelatin implant was lower than the silicone sponge one. Extrusion of implant was found in 20% eyes of silicone sponge group. Electroretinography revealed no detectable difference in retinal function between the two groups. Partially degraded chitosan-gelatin, newly formed capillaries, loose connective tissue, and some cell infiltration were found in chitosan-gelatin; however, only a slight cell infiltration was found in silicone sponge. Over a 9-month observation period, degradation rate of the chitosan-gelatin was about 36%. Conclusion: Chitosan-gelatin polymer was used effectively and safely as an implant for scleral buckling in a rabbit and showed good biodegradation. RETINA 33: 1062-1069, 2013

Abstract:Zwitterionic polymers have shown ultra-low biofouling properties at surfaces and excellent biocompatibility as implant. In this study, an in situ-forming zwitterionic hydrogel was designed and evaluated, both in vitro and in vivo, as a long-term vitreous substitute. The zwitterionic polymer poly(MPDSA-co-AC) was designed as a copolymer of the sulfobetaine methacrylamide and acryloyl cystamine monomers, providing the zwitterionic components and the thiol functional groups, respectively. The in situ gelation was via the thiol-ene Michael addition reaction with α-PEG-MA as the crosslinker. The gelation time, rheological properties, swelling profiles, and the transparency of zwitterionic hydrogels were studied in detail. Two systems with different crosslinker concentrations were tested in a rabbit model, and the one with the thiol-ene ratio of 2 : 1 showed excellent biocompatibility in vivo, formed space-filling hydrogels and remained transparent in the vitreous cavity for the 2 month implantation period. Therefore, in situ-forming zwitterionic hydrogels represent a promising material system as a vitreous substitute and possibly for other soft tissue replacements.

Abstract:A nano-hydroxyapatite/poly(vinyl alcohol) (n-HA/PVA) composite hydrogel was employed as artificial cornea fringe to improve biocompatibility for the firm fixation between material and surrounding host tissues. The morphology and swelling behavior, as well as mechanical strength of the fringes were characterized. The results showed that the n-HA/PVA fringes had interconnective porous structure, high water content and good mechanical properties. With the aid of cell culture observed by inverted microscopy, scanning electron microscopy (SEM) and MTT test, it was concluded that PVA hydrogel modified with n-HA can improve biocompatibility and has no negative effects on the corneal fibroblasts in vitro. These findings indicate that the porous n-HA/PVA fringe can allow invasion and proliferation of cells, and can function as a fringe for artificial cornea.
Study on biocompatibility of complexes of collagen-chitosan-sodium hyaluronate and cornea.

Abstract:In this study we investigated the biocompatibility of collagen-chitosan-sodium hyaluronate (Col-Chi-NaHA) complexes and cornea tissue, and the feasibility of Col-Chi-NaHA complexes as substrates for cultivating rabbit corneal cells. Different components of Col-Chi-NaHA complexes were prepared and tested. A circular complex film with a diameter of 6 mm was inserted into rabbit stomal pocket and traced for a period of 5 months. Clinical examination was made. Rabbit limbal corneal epithelial cells, corneal endothelial cells, and keratocytes were cultured primarily on complexes. Phase contrast microscope examination was made daily. Histological, immunohistochemical, and scanning electron microscopic examinations were carried out. The complexes of 20% collagen, 10% chitosan, and 0.5% sodium hyaluronate showed rather weak corneal edema and other responses. The degradation of materials was obvious after 5 months. Corneas were transparent and translucent. Cells seeded on Col-Chi-NaHA were allowed to proliferate and partly form confluent monolayer after 9 days in culture. Cultured cells were well attached to the complexes of 20% collagen, 10% chitosan, and 0.5% sodium hyaluronate, or 10% chitosan and 0.5% sodium hyaluronate. The results showed that Col-Chi-NaHA complexes had good biocompatibility with cornea. The complexes can degrade and be absorbed in cornea. Col-Chi-NaHA complex may be a suitable substrate for cultivating corneal cells and a feasible material as a scaffold of tissue-engineered cornea.

UV-assisted Treatment on Hydrophobic Acrylic IOLs Anterior Surface with Methacryloyloxyethyl Phosphorylcholine: Reducing Inflammation and Maintaining Low Posterior Capsular Opacification Properties.

Abstract:In order to enhance the surface biocompatibility of hydrophobic acrylic intraocular lenses (IOLs), the anterior surface was modified through graft polymerization with methacryloyloxyethyl phosphorylcholine (MPC) initiated by ultraviolet radiation. Chemical changes on the IOL surface were characterised by X-ray photoelectron spectroscopy (XPS) to confirm the covalent binding of MPC. Field emission scanning electron microscopy (FESEM) and atomic force microscopy (AFM) analysis were used to characterise the morphological changes to the IOL surface. Static water contact angle (WCA) measurements showed that the hydrophilicity of the anterior surface was enhanced after grafting with MPC. The surface biocompatibility of the untreated and Anterior UV-MPC IOLs was compared with the adhesion behaviour of macrophages in vitro. The degree of posterior capsule opacification (PCO) of untreated, anterior treated, and posterior treated IOLs was evaluated by observing the behaviour of lens epithelial cells (LECs) between the IOL optic portion and type IV collagen. Results suggest that the MPC modified anterior surface resisted the attachment of macrophages. However, MPC grafted onto the posterior surface enhanced the migration of LECs between the IOL optic and type IV collagen, which may result in severe PCO. In conclusion, our Anterior UV-MPC IOLs may reduce postoperative inflammation while maintaining the low PCO rates of hydrophobic acrylic IOLs.
The influence of the plasma modification on properties of PCL films as corneal implants

Abstract:The purpose of this research is the study the influence of the plasma modification on PCL films properties and the influence PCL films implantation on the morphology of the cornea in vivo experiment. The feedstock for films was obtained by dissolving PCL in the chloroform. Modification of the films was carried out using the low-temperature plasma. Studies were performed on 4 pubescent female Sylvilagus bachmani rabbits. The PCL films were implanted into the cornea of one animal eye. Sampling (animal's eyes) was performed on day 21 after the start of the in vivo experiment for morphology studying. Results showed that the plasma modification increases a roughness (1.5 times) of PCL films, the hydrophilicity of films, reducing the contact angle of the inner side by 9°–10° (12%–14%), and increases the surface energy values mainly due to the polar component to 20.6±0.3 mJ/m<sup>2</sup>. The implantation of the PLA films contributes to the development of the inflammatory response as a result of the surgery and implantation of the polymer material. The study showed the possibility of the plasma-modified PLA films using as a corneal implant in the future.
Intraocular pressure control efficacy and safety of HA-Mg glaucoma drainage plate implantation in the anterior chamber of rabbit eyes

Abstract:The current clinical application of glaucoma drainage devices is made of non-degradable materials. These non-degradable drainage devices often trigger inflammatory responses and scar proliferation, possibly leading to surgical failure. We developed a biodegradable material hydroxyapatite-coated magnesium (HA-Mg) as a glaucoma drainage device. Twelve New Zealand white rabbits were randomly assigned to three groups: HA-Mg drainage plate group (6 right eyes), trabeculectomy group (6 right eyes), and control group (12 left eyes). Results showed that all HA-Mg drainage plates were completely degraded ~4 months postoperatively. At the 5th month postoperatively, there was no statistical difference in the corneal endothelium density between the HA-Mg drainage plate group and the control group ( p = 0.857). The intraocular pressure (IOP) level in the HA-Mg drainage plate implantation group was lower than in the other two groups. The trypan blue dye still drained from the anterior chamber to the subconjunctiva 5 months after HA-Mg drainage plate implantation. HE staining revealed the scleral linear aqueous humor drainage channel and anterior synechia were observed after drainage plate completely degraded, with no obvious infiltration with the inflammatory cells. This study showed the safety and efficacy of HA-Mg glaucoma drainage plate in controlling IOP after implantation into the anterior chamber of rabbit eyes.

Exploration and application of polyhydroxyalkanoates and polylactic acid membranes during glaucoma filtering surgery and interlaminar transplantation in rabbits

Abstract:Objective To evaluate the effect of two polymer membranes, polyhydroxyalkanoates(PHA)and polylactic acid(PLA)during glaucoma filtration surgery(GFS),and to evaluate the morphology of membranous PHA after interlamellar implantation. Methods Twenty-eight New Zealand white rabbits were chosen and twenty-four of them were randomly divided into 6 groups(n=4):the PHA-low group,PHA-high group,PLA-low group,PLA-high group,positive control group(MMC group)and blank control group. The rabbits in each group received GFS. The corresponding polymer membranes were implanted under the scleral flap,while the MMC group was treated with 0.2 mg/mL mitomycin C(MMC) for 3 minutes,and the blank control group was treated without extra drugs. The intraocular pressure(IOP)was examined at 0 d,1 d, 3 d, 7 d, 14 d, 28 d and 84 d after GFS. The corneal layers of four rabbits were implanted with PHA membranes and the corneal morphological changes were observed after 84 d. Results The IOP of the PHA-low and PLA-high groups was lower than that of the blank control group at 84 d after GFS(P < 0.05),and was similar with that of the MMC group(P> 0.05). Morphological studies showed that there were no collagenous fibers filling in the duct, and the collagenous fibers around the membranes were generally arranged in parallel. There were no obvious changes in the peripheral collagen structure after implantation of PHA membranes between the corneal layers. Conclusions Application of PHA and PLA membranes during GFS in rabbits may maintain the level of IOP,and the effect is similar with MMC. The mechanism may be achieved through the mechanical blocking of fibrous tissue.
Observation of Cyclosporin A: Sustained Release Intraocular Lens Implantation in Rabbit Eyes

Abstract:To observe the performance of cyclosporine A (CsA)-loaded intraocular lens (IOLs) implanted into rabbit eyes. To prepare a PLGA-based CsA-sustained release IOLs and study the in vitro drug release. Forty-two New Zealand white rabbits were randomly and equally divided into three groups, and all right eyes underwent phacoemulsification. In group A, a common polymethylmethacrylate (PMMA) IOLs was implanted, while polylactide-glycoli acid (PLGA-loaded)-PMMA-IOLs was implanted in group B, and CsA-PLGA-PMMA-IOLs was implanted in group C. All experimental eyes were examined by slit-lamp microscopy. In addition, fundoscopy and the number of corneal endothelial cells, anterior chamber flare grading, and the number of aqueous humor cells were assessed at different time points post-surgery. The wet lens capsule was weighed and histological examination was performed 6 months post-operation. In the early post-operative period, the inflammatory reaction of anterior chamber in groups A and B were more severe than group C. The initial appearance of PCO in group C was much later than the other two groups ( F = 68.91; p = 0.000), and PCO grade in group C was much lower than the other two groups (χ 2 = 36.07; p = 0.000). The mean weights of wet lens capsules in groups A and B were significantly heavier than group C ( F = 134.88; p = 0.00). Histological observation showed no obvious toxic reaction in the intraocular tissues of the CsA-PLGA-PMMA-IOLs group, and the proliferation and accumulation of lens epithelial cells in groups A and B were greater than in group C. CsA-sustained release IOLs can effectively prevent PCO in rabbit eyes without defined intraocular toxicity.

Towards improving the biocompatibility of prosthetic eyes

Abstract:Prosthetic eyes are currently manufactured using Poly(methyl methacrylate) (PMMA) which is not an ideal material because it is hydrophobic. While significant research has investigated the benefits of hydrophilic materials for contact lenses, no such research has been carried out on hydrophilic materials for prosthetic eyes until now. In this study, different derivatives of Poly(ethylene glycol) (PEG) monomer and methyl methacrylate (MMA) monomer were grafted to PMMA using copolymerisation. The resulting matrixes were evaluated by water contact angle measurement, 24 h water absorption testing, and colour-difference measurement when exposed to ultraviolet light. The contact angle and water absorption results indicated that ethylene glycol dimethacrylate (EGDMA) grafted PMMA matrix had a better hydrophilic performance than the other matrixes tested. EGDMA is already a minor constituent of the PMMA matrix currently used for manufacturing prosthetic eyes but when the proportion of EGDMA monomer to MMA monomer used in the manufacturing process was increased to 50/50 the hydrophilicity of the matrix was significantly improved. EGDMA-grafted PMMA is inexpensive and comes as a liquid monomer that is easily mixed with the PMMA monomer that ocular prosthetists are familiar with. The mixture requires no special handling beyond the normal safety precautions that apply when using PMMA monomers. In-vitro testing shows that EGDMA-grafted PMMA significantly improves the wettability of PMMA currently used for the manufacture of prosthetic eyes and has the potential to significantly improve wearing comfort and socket health.

Abstract:Objective To assess polyvinyl alcohol hydrogel /polyvinyl pyrrolidine,an alternative repair material for articular cartilage injury in the future comprehensively by a series of in vivo and external tests,preliminarily explore its in vivo biocompatibility and safety and and provide the basis for further human trials.Methods According to national standards GB/T16886.1 and ISO10993-1 standard for medical equipment supplies requirements and evaluation criteria for articular cartilage repair materials:Cell toxicity test(MTT),blood compatibility test,acute systemic toxicity test,chronic systemic toxicity,skin irritation test,muscle implantation test were performed.Results The relative increasing rate for test material cell cytotoxicity test was 91.7%,showing no cytotoxicity;for blood compatibility test of PVA /PVAthe hemolysis rate was 0.51%,less than 5%,indicating that the material has no hemolysis action,agreement with medical materials and hemolysis test requirements.Acute systemic toxicity test based on the experimental animals Kunming mice at 24 h,48 h,72 h,no chills,fever,dys pnea,exercise less,diarrhea,weight loss and other symptoms.Meanwhile the experimental group and control group mice gained weight,but t test showed no significant difference(P>0.05).After 12 week's chronic systemic toxicity test,experimental animals of New Zealand white rabbits had no obvious hyperemia and edema for liver,kidney no nodules and hardened,non-cheese-type necrosis and other inflammatory response.Materials implanted in vivo animal test group compared with the control group,indicated no significant hepatic and renal function(P>0.05),the experimental group showed no significant difference before and after surgery for liver and kidney function(P> 0.05).Muscle and bone after implantation test histopathology biopsy confirmed that afterthe material was implanted in rabbits inflammatory cells gradually reduced and surrounding tissue fibers cyst gr adually thinned.Conclusion This kind of material has good biocompatibility and safety.

Abstract:The influence of micelle-drug solubilization on the dissolution rate has been investigated. The dissolution rates predicted by the diffusion layer model, the Danckwerts theory, and by the rotating disk theory were calculated and compared with experimental data obtained for the benzocaine-polysorbate 80 system. All the parameters involved were independently determined. The different conditions under which each of these theories is applicable have been discussed. Different kinds of dissolution experiments were designed to produce the conditions under which each theory would apply. The theoretical and experimental procedures involved in these studies provide a unique method for distinguishing mechanisms and should be useful in future studies, e.g. , in studies of the effects of agitation on mechanisms of interphase transport.

Abstract:Acrylates have been used in ophthalmic practice as a paradigmatic implant material for decades, especially as intraocular lens for their excellent transparency. A novel polymeric shape memory system of chemically cross-linked acrylate-methacrylate copolymer networks was developed and characterized in this study. The thermomechanical properties, shape memory properties, transparency, and surface wettability as well as cytotoxicity were systematically evaluated to mimic the in vivo situation by differential scanning calorimetry (DSC), tensile tests, spectrophotometer, Abbe refractometer, contact angle measurements, and MTT assay. It was found that the chemically cross-linked copolymer network behaves as an elastomer capable of arbitrary shaping above the glass-transition temperature. Transition temperatures of the networks were tunable through the change of the composition of monomers.

Abstract:Poly(lactic-co-glycolic acid) (PLGA) and/or poly(lactic-acid) (PLA) microspheres are important drug delivery systems. This study investigated eye biocompatibility and safety of PLGA/PLA microspheres through intravitreal injection in rabbits. Normal New Zealand rabbits were randomly selected and received intravitreal administration of different doses (low, medium, or high) of PLGA/PLA microspheres and erythropoietin-loaded PLGA/PLA microspheres. The animals were clinically examined and sacrificed at 1, 2, 4, 8, and 12 weeks postadministration, and retinal tissues were prepared for analysis. Retinal reactions to the microspheres were evaluated by terminal deoxynucleotidyl transferase-mediated dUTP nick end staining and glial fibrillary acidic protein immunohistochemistry. Retinal structure changes were assessed by hematoxylin and eosin staining and transmission electron microscopy. Finally, retinal function influences were explored by the electroretinography test. Terminal deoxynucleotidyl transferase-mediated dUTP nick end staining revealed no apoptotic cells in the injected retinas; immunohistochemistry did not detect any increased glial fibrillary acidic protein expression. Hematoxylin and eosin staining and transmission electron microscopy revealed no micro- or ultrastructure changes in the retinas at different time points postintravitreal injection. The electroretinography test showed no significant influence of scotopic or photopic amplitudes. The results demonstrated that PLGA/PLA microspheres did not cause retinal histological changes or functional damage and were biocompatible and safe enough for intravitreal injection in rabbits for controlled drug delivery.
