Abstract:AIM: To investigate the performance of magnifying endoscopy with narrow-band imaging (ME-NBI) in the diagnosis of early gastric cancer (EGC). METHODS: Systematic literature searches were conducted until February 2014 in PubMed, Embase, Web of Science, Ovid, Scopus and the Cochrane Library databases by two independent reviewers. Meta-analysis was performed to calculate the pooled sensitivity, specificity and diagnostic odds ratio and to construct a summary receiver operating characteristic (ROC) curve. Subgroup analyses were performed based on the morphology type of lesions, diagnostic standard, the size of lesions, type of assessment, country and sample size to explore possible sources of heterogeneity. A Deeks' asymmetry test was used to evaluate the publication bias. RESULTS: Fourteen studies enrolling 2171 patients were included. The pooled sensitivity, specificity and diagnostic odds ratio for ME-NBI diagnosis of EGC were 0.86 (95% CI: 0.83-0.89), 0.96 (95% CI: 0.95-0.97) and 102.75 (95% CI: 48.14-219.32), respectively, with the area under ROC curve being 0.9623. Among the 14 studies, six also evaluated the diagnostic value of conventional white-light imaging, with a sensitivity of 0.57 (95% CI: 0.50-0.64) and a specificity of 0.79 (95% CI: 0.76-0.81). When using "VS" (vessel plus surface) ME-NBI diagnostic systems in gastric lesions of depressed macroscopic type, the pooled sensitivity and specificity were 0.64 (95% CI: 0.52-0.75) and 0.96 (95% CI: 0.95-0.98). For the lesions with a diameter less than 10 mm, the sensitivity and specificity were 0.74 (95% CI: 0.65-0.82) and 0.98 (95% CI: 0.97-0.98). CONCLUSION: ME-NBI is a promising endoscopic tool in the diagnosis of early gastric cancer and might be helpful in further target biopsy.
The Application of Magnifying Endoscopy in the Diagnosis of Cervical Lesions

Abstract:Objective This study aimed to assess the sensitivity and specificity of magnifying endoscopy in the examination of patients with high-risk factors for cervical lesions. Methods Two equally skilled colposcopy specialists examined 59 patients admitted to the Obstetrics and Gynecology Hospital of Fudan University between March 14, 2023, and April 15, 2023. These patients underwent both colposcopy and magnifying endoscopy examinations. Preliminary diagnoses were then made by the two colposcopy specialists using colposcopy and magnifying endoscopy. The sensitivity and specificity of these methods were then evaluated compared to the postoperative pathology diagnosis for a better understanding of their efficacy. Results Colposcopy exhibited sensitivity rates of 23.08% for the diagnosis of normal tissue, 10% for low-grade squamous intraepithelial lesions (LSIL), 90% for high-grade squamous intraepithelial lesions (HSIL), and 84.62% for cervical cancer, with specificity rates of 93.94%, 67.35%, 71.43%, and 100%, respectively. In contrast, magnifying endoscopy demonstrated sensitivity rates of 42.31%, 40%, 90%, and 92.31% for normal tissue, LSIL, HSIL, and cervical cancer, respectively. The corresponding specificity rates for these categories were 87.88%, 77.55%, 83.67%, and 100%. Conclusion Magnifying endoscopy exhibits higher sensitivity and specificity compared to colposcopy in detecting cervical lesions. With the ability to magnify lesions up to 520 times, magnifying endoscopy facilitates precise visualization of cellular-level lesions, as well as associated anatomical features and vascular signals.
Diagnostic Ability of Magnifying Narrow-Band Imaging for the Extent of Early Gastric Cancer: A Systematic Review and Meta-Analysis

Abstract:Accurate delineation of tumor margin is essential for complete resection of early gastric cancer (EGC). The objective of this study is to assess the performance of magnifying endoscopy with narrow-band imaging (ME-NBI) for the accurate demarcation of EGC margins.We searched PubMed, EMBASE, Web of Science, and Cochrane Library databases up to March 2020 to identify eligible studies. The diagnostic accuracy of ME-NBI for EGC margins was calculated, and subgroup analyses were performed based on tumor size, depth of tumor invasion, tumor-occupied site, macroscopic type, histological type, Helicobacter pylori (H. pylori), and endoscopists' experience. Besides, we also evaluated the negative and positive resection rates of the horizontal margin (HM) of EGC after endoscopic submucosal dissection (ESD) and surgery.Ten studies comprising 1018 lesions were eligible in the databases. The diagnostic accuracy of ME-NBI for the demarcation of EGC margins was 92.4% (95% confidence interval (CI): 86.7%-96.8%). According to ME-NBI subgroup analyses, the rate of accurate evaluation of EGC margins was not associated with H. pylori infection status, tumor size, depth of tumor invasion, tumor-occupied site, macroscopic type, histological type, and endoscopists' experience, and no statistical differences were found in subgroup analyses. Moreover, the negative and positive resection rates of HM after ESD and surgery were 97.4% (95% CI: 92.1%-100%) and 2.6% (95% CI: 0.02%-7.9%), respectively.ME-NBI enables a reliable delineation of the extent of EGC.
Magnifying endoscopy in diagnosing early colorectal cancers and precancerous lesions

Abstract:[Objectives] To investigate the features of pit patterns by magnifying endoscopy on colorectal lesions especially precancerous lesions and early colorectal cancers and the relationship between pit pattern and invasive depth, and evaluate the accuracy of diagnosing neoplastic and nonneoplastic colorectal lesions by magnifying endoscopy. [Methods] The pit patterns of lesions observed by magnifying endoscopy were compared with pathological results. [Results] The diagnostic accuracy of nonneoplastic lesions by magnifying endoscopy was 82.19% and 96.05% to neoplastic lesions, the overall accuracy were 84.21%. Of 11 cancerous lesions (8 intramucosal carcinomas and 2 submucosa carcinomas and 1 advanced carcinomas), 10 showed Ⅴ pit pattern. 30 lesions were observed by stereomicroscopey, the results were similar to that of magnifying endoscopy. [Conclusions] Magnifying endoscopy can well distinguish nonneoplastic lesions from neoplastic lesions. Most ⅤA pit pattern correspond to intramucosal carcinoma and it may be submucosa carcinoma or advanced carcinoma whenⅤN pit pattern appeared. The images of pit pattern obtained by magnifying endoscopy are similar to those provided by stereomicroscopy.
A Multicenter Prospective Evaluation of Four Enhanced Magnifying Endoscopy for Diagnosis of Precancerous Lesions and Early Gastric Cancers

Abstract:Objective To evaluate the diagnostic accuracy of four enhanced magnifying endoscopy methods for precancerous lesions and early gastric cancers,i.e.,magnifying endoscopy enhanced by epinephrine(ME-EPI),magnifying endoscopy enhanced by acetic acid-indigo carmine mixture (ME-AIM),magnifying endoscopy enhanced by indigo carmine(ME-IDC) and magnifying endoscopy enhanced with narrow-band imaging(ME-NBI).Methods A total of 3 059 patients (3 097 focal gastric lesions) detected with conventional white light endoscopy(WLE) at four hospitals from January 2010 to December 2011 were recruited and divided into three groups according to different chromoendoscopic procedures they underwent:1 094 ME-EPI cases,873 ME-AIM cases and 1 130 ME-IDC cases.The pathological results were used as golden standard,and diagnosis accuracy rates of precancerous lesions or early cancers were determined according to the VS criteria.At the mean time,in each group,a paired design was used to compare the accuracy between the above chromoendoscopies and ME-NBI respectively.Results A total of 3 097 lesions of 3 059 patients were evaluated,and 205 lesions were diagnosed as early gastric cancer according to pathological results.The diagnostic accuracy rates of ME-EPI,ME-AIM and ME-IDC for early gastric cancer were 98.2％,99.0％ and 98.4％,respectively.In each group,the diagnostic accuracy rates of paired ME-NBI were 98.0％,99.0％ and 97.4％,respectively.In contrast to ME-NBI,the diagnostic accuracy of ME-EPI and ME-AIM were comparable or lower,but the differences were not statistically significant (P ＞ 0.05).However,when it came to ME-IDC,the diagnostic accuracy declined and the differences were statistically significant (P ＜ 0.05).Conclusion Chromoendoscopy can be used to identify the early gastric cancer accurately,easily,safely and inexpensively.And the diagnostic effectiveness of ME-NBI is not superior to chromoendoscopy in diagnosing early gastric cancer.
[Clinical Value of Different Magnifying Chromoendoscopy Methods in Screening Gastric Precancerous Lesions and Early Cancers].

Abstract:OBJECTIVE:To prospectively evaluate the clinical value of different magnifying chromoendoscopy(MCE) methods in screening gastric precancerous lesions and early cancers. METHODS:Between March 2010 and October 2011, among all the patients aged over 40 who received esophagogastroduodenoscopy at Zhongshan hospital, Fudan University, suspicious lesion was detected in 699 patients, who were randomly assigned to three groups: epinephrine dye(n=240), indigo carmine dye(n=246), and acetic acid-indigo carmine mixture dye(n=213). Diagnosis was made according to surface patterns and microvessels of the lesion. Pathological diagnosis was used as the gold standard. The concordance between endoscopic diagnosis and pathological diagnosis was evaluated through the agreement(Kappa) test. McNemar Paired chi-square test was used to compare the concordance of three MCE methods, regular white light, magnification alone, and NBI magnifier before and after MCE. RESULTS:Pathological examination showed inflammatory lesions in 415 patients, intestinal metaplasia in 190, low grade intra-epithelial neoplasia in 17, and high grade intra-epithelial neoplasia or early cancer in 77. The percentage of patients with consistent endoscopic and pathological diagnosis was 77.1%(185/240) for epinephrine dye, 80.5%(198/246) for indigo carmine dye, and 81.2%(173/213) for acetic acid-indigo carmine mixture dye. Kappa values were 0.579, 0.502, and 0.667 respectively(all P<0.01). For the screening of high grade intra-epithelial neoplasia or early cancer, the diagnostic sensitivities were 84.0%, 83.3%, and 92.9%, respectively, and the specificities were 98.6%, 97.3%, and 98.4%. All the three chromoendoscopy methods improved the diagnostic accuracy for precancerous lesions compared with conventional gastroscopic observation with white light(all P<0.01). Indigo carmine and acid-indigo carmine mixture dye improved the diagnostic accuracy of magnification alone(both P<0.05). There was no significant difference in diagnostic accuracy between each MCE method and magnifying NBI observation(all P>0.05). CONCLUSION:NBI magnification and all the three MCE methods may improve the diagnostic accuracy of early gastric cancer and precancerous lesions.

Abstract:Background and Aims Magnifying image-enhanced endoscopy (MIEE) is an advanced endoscopy with image-enhancement and magnification used in preoperative examination. However, its impact on the detection rate is unknown. Methods We conducted an open-label, randomized, parallel (1:1:1) controlled trial in six hospitals in China. Patients were recruited between February 14, 2022, and July 30, 2022. Eligible patients were ≥ 18, undergoing gastroscopy in outpatient departments. Participants were randomly assigned to the MIEE-only mode (o-MIEE) group, white-light endoscopy-only mode (o-WLE) group, and MIEE when necessary mode (n-MIEE) group (initial white-light endoscopy, followed by the switch to another endoscope with MIEE if necessary). Biopsies of suspicious lesions or of the lesser curvature of the gastric antrum were performed. The primary and secondary aims were to compare detection rates and positive predictive value (PPV) of early cancer and precancerous lesions in these three modes, respectively. Results A total of 5100 recruited patients were randomly assigned to the o-MIEE (n=1700), o-WLE (n=1700), and n-MIEE (n=1700) groups. In the o-MIEE, o-WLE, and n-MIEE groups, 29 (1.51%, 95% CI 1.05-2.16), 4 (0.21%, 0.08-0.54), and 8 (0.43%, 0.22-0.85) early cancers were found, respectively (p<0.001). The PPV for early cancer was higher in the o-MIEE group compared to the o-WLE and n-MIEE groups (63.04%, 33.33%, and 38.1% respectively, p=0.062). The same trend was seen for precancerous lesions (36.67%, 10.00%, and 21.74% respectively). Conclusions The o-MIEE mode resulted in a significant improvement in early upper gastrointestinal (UGI) cancer and precancerous lesions diagnosing; thus, it could be used for opportunistic screening.

Efficacy of magnifying endoscopy combined with narrowband imaging in detection of early gastric cancer: a systematic evaluation and meta-analysis

Abstract:Objective: To systematically evaluate the diagnostic accuracy of magnifying endoscopy combined with narrowband imaging (ME-NBI) in detecting early gastric cancer (EGC) and to provide a scientific basis for its clinical utility. Methods: Literature published before May 2024 that utilized ME-NBI for diagnosing EGC was searched across PubMed, EMBASE, The Cochrane Library, Web of Science, and major Chinese databases. Included studies were cohort studies or randomized controlled trials, and their quality was assessed using the QUADAS-2 framework. Meta-analysis was conducted using Stata 17 software to calculate diagnostic indicators such as sensitivity, specificity, and area under the curve (AUC). Heterogeneity was explored through Spearman's correlation coefficient, I2 statistics, subgroup analysis, and meta-regression analysis. Publication bias was assessed with Deeks' funnel plot. Results: Twenty studies involving 7,770 patients and 7,917 lesions were included. The pooled sensitivity of ME-NBI for diagnosing EGC was 0.86 (95% CI: 0.80-0.90), specificity was 0.92 (95% CI: 0.86-0.96), and the AUC was 0.94 (95% CI: 0.91-0.96), demonstrating high diagnostic accuracy. Subgroup analysis revealed lower sensitivity in multicenter studies. Excised samples had similar sensitivity to biopsy samples but differed in specificity. Publication bias was detected (P=0.01), but sensitivity analysis corrected for this, maintaining high combined sensitivity, specificity, and AUC. Conclusion: ME-NBI is a highly accurate and reliable diagnostic tool for EGC. Despite have some bias and heterogeneity, this was effectively addressed through sensitivity and subgroup analyses. ME-NBI should be considered a preferred method for EGC screening and diagnosis in clinical practice.
A comparative study of magnifying endoscopy with narrow-band image and endocytoscopy in the diagnosis of gastric neoplasm: a pilot study

Abstract:Background/aims: Endoscopic technologies have recently advanced to optimize the detection and diagnosis of gastric lesions. Endocytoscopy aids in the virtual realization of histology. Herein, we aimed to investigate gastric lesions using single-stain endocytoscopy and compare them using magnifying endoscopy with narrow-band imaging (ME-NBI) in terms of diagnostic yield in vivo. Methods and methods: In the present prospective study, we registered 24 patients with gastric neoplasms and retrospectively reviewed their images. Three endoscopists reviewed the images of gastric neoplasms using white light, ME-NBI, and endocytoscopy. The diagnostic yield of endocytoscopy in early gastric cancer (EGC) was assessed using histopathology as the gold standard. Results: Endocytoscopy was performed in 24 patients with gastric neoplasms. Of these, 15 patients had adenocarcinomas, while nine patients had low-grade dysplasia. The sensitivity, specificity, and accuracy of endocytoscopy for EGC detection were reported as 80.0% [95% confidence interval (CI), 51.9-95.7], 66.7% (95% CI, 58.4-91.9), and 75.0% (95% CI, 53.3-90.2) by endoscopist A; 80.0% (95% CI, 51.9-95.7), 44.4% (95% CI, 13.7-78.8), and 66.7% (95% CI, 44.7-84.4) by endoscopist B; and 93.3% (95% CI, 68.1-99.8), 55.6% (95% CI, 21.2-86.3), and 79.2% (95% CI, 57.9-92.8) by endoscopist C; these findings were not inferior to NBI. The inter-observer agreement, κ statistic = 0.67 (95% CI, 0.43-0.90) was favorable. Conclusion: Endocytoscopy aid in the diagnosis of EGC because of its better sensitivity and accuracy compared to NBI or white-light imaging. However, further large-scale studies are required to confirm our findings.
AI-assisted Identification of Intrapapillary Capillary Loops in Magnification Endoscopy for Diagnosing Early-Stage Esophageal Squamous Cell Carcinoma: a Preliminary Study

Abstract:Esophageal squamous cell carcinoma (ESCC) is one of the most common histological types of esophageal cancers. It can seriously affect public health, particularly in Eastern Asia. Early diagnosis and effective therapy of ESCC can significantly help improve patient prognoses. The visualization of intrapapillary capillary loops (IPCLs) under magnification endoscopy (ME) can greatly support the identification of ESCC occurrences by endoscopists. This paper proposes an artificial-intelligence-assisted endoscopic diagnosis approach using deep learning for localizing and identifying IPCLs to diagnose early-stage ESCC. An improved Faster region-based convolutional network (R-CNN) with a polarized self-attention (PSA)-HRNetV2p backbone was employed to automatically detect IPCLs in ME images. In our study, 2887 ME with blue laser imaging (ME-BLI) images of 246 patients and 493 ME with narrow-band imaging (ME-NBI) images of 81 patients were collected from multiple hospitals and used to train and test our detection model. The ME-NBI images were used as the external testing set to verify the generalizability of the model. The experimental evaluation revealed that the proposed method achieved a recall of 79.25%, precision of 75.54%, F1-score of 0.764 and mean average precision (mAP) of 74.95%. Our method outperformed other existing approaches in our evaluation. It can effectively improve the accuracy of ESCC detection and provide a useful adjunct to the assessment of early-stage ESCC for endoscopists.
Mucin phenotype and microvessels in early gastic cancer: Magnifying endoscopy with narrow band imaging

Abstract:Backgrounds: In order to detect early gastric cancer (EGC), this research sought to assess the diagnostic utility of magnifying endoscopy (ME) as well as the significance of mucin phenotype and microvessel features. Methods: 402 individuals with an EGC diagnosis underwent endoscopic submucosal dissection (ESD) at the Department of ME between 2012 and 2020. After adjusting for image distortion, high-magnification endoscopic pictures were taken and examined to find microvessels in the area of interest. The microvessel density was measured as counts per square millimeter (counts/mm2) after segmentation, and the vascular bed's size was computed as a percentage of the area of interest. To identify certain properties of the microvessels, such as end-points, crossing points, branching sites, and connection points, further processing was done using skeletonized pixels. Results: According to the research, undifferentiated tumors often lacked the MS pattern and showed an oval and tubular microsurface (MS) pattern, but differentiated EGC tumors usually lacked the MS pattern and presented a corkscrew MV pattern. Submucosal invasion was shown to be more strongly associated with the destructive MS pattern in differentiated tumors as opposed to undifferentiated tumors. While lesions with a corkscrew MV pattern and an antrum or body MS pattern revealed greater MUC5AC expression, lesions with a loop MV pattern indicated higher MUC2 expression. Furthermore, CD10 expression was higher in lesions with a papillary pattern and an antrum or body MS pattern. Conclusion: These results imply that evaluating mucin phenotype and microvessel features in conjunction with magnifying endoscopy (ME) may be a useful diagnostic strategy for early gastric cancer (EGC) detection. Nevertheless, further investigation is required to confirm these findings and identify the best course of action for EGC diagnosis.

Abstract:Objective To explore the diagnostic value of white light gastroscopy and magnifying endoscopy combined with narrow - band imaging (ME - NBI)in patients with atrophic gastritis and early gastric cancer. Methods A total of 243 patients were selected in this hospital in March 2016. After patients agreed to examine with white light endoscopy,then they underwent magnifying endoscopy combined with narrow band imaging examination,and the samples of suspicious tissue were sent for pathological examination. In comparison with the difference in test sensitiv-ity,specificity,positive predictive value,negative predictive value and accuracy of these 2 kinds of examination. Results Among 243 test sub-jects,routine gastroscopy showed that there were suspicious lesions under endoscopy,and biopsy specimens were obtained by gastroscopy. In 226 cases were diagnosed as atrophic gastritis,and 17 cases as early gastric cancer. The detection rate of white atrophic gastritis was 94. 7％ (214 /226)and early gastric cancer was 29. 4％ (5 / 17);magnifying endoscopy with narrow band imaging group of atrophic gastritis,the detection rate was 99. 6％,the detection rate of early gastric cancer was 88. 2％ (15 / 17). The difference in these two sets of inspection methods was significant (P < 0. 05). In ordinary white light endoscopy group on diagnosis of early gastric cancer,the sensitivity,specificity and positive predictive val-ue,negative predictive value and accuracy in diagnosis of early gastric cancer with white light endoscopy were 93. 2％,53. 5％,25. 3％,97. 4％and 54. 0％ respectively. Conclusion Magnifying endoscopy combined with narrow - band imaging can significantly raise the detection rate of patients with atrophic gastritis and gastric cancer,and this combi-nation examination in patients with early gastric cancer has high accuracy,simple operation procedure and strong enforce ability,which is helpful for early screening and diagnosis of patients.

Abstract:Purpose: Adequate staging of early rectal neoplasms is essential for organ-preserving treatments, but magnetic resonance imaging (MRI) frequently overestimates the stage of those lesions. We aimed to compare the ability of magnifying chromoendoscopy and MRI to select patients with early rectal neoplasms for local excision. Methods: This retrospective study in a tertiary Western cancer center included consecutive patients evaluated by magnifying chromoendoscopy and MRI who underwent en bloc resection of nonpedunculated sessile polyps larger than 20 mm, laterally spreading tumors (LSTs) [Formula: see text] 20 mm, or depressed-type lesions of any size (Paris 0-IIc). Sensitivity, specificity, accuracy, and positive and negative predictive values of magnifying chromoendoscopy and MRI to determine which lesions were amenable to local excision (i.e., [Formula: see text] T1sm1) were calculated. Results: Specificity of magnifying chromoendoscopy was 97.3% (95% CI 92.2-99.4), and accuracy was 92.7% (95% CI 86.7-96.6) for predicting invasion deeper than T1sm1 (not amenable to local excision). MRI had lower specificity (60.5%, 95% CI 43.4-76.0) and lower accuracy (58.3%, 95% CI 43.2-72.4). Magnifying chromoendoscopy incorrectly predicted invasion depth in 10.7% of the cases in which the MRI was correct, while magnifying chromoendoscopy provided a correct diagnosis in 90% of the cases in which the MRI was incorrect (p = 0.001). Overstaging occurred in 33.3% of the cases in which magnifying chromoendoscopy was incorrect and 75% of the cases in which MRI was incorrect. Conclusion: Magnifying chromoendoscopy is reliable for predicting invasion depth in early rectal neoplasms and selecting patients for local excision.

Our previous study demonstrated the advantages of magnifying endoscopy from general endoscopy for lesion detection, prompting further investigation into the variations in lesion detection rates across endoscopes with different resolutions.

Abstract:Objective: To investigate the diagnostic value of endoscopic narrow-band imaging technique in early gastric cancer and precancerous lesions. Methods: A total of 100 patients with recurrent upper gastrointestinal symptoms in our hospital from January 2017 to January 2022 were selected and divided into group A and group B according to the random number table method, with 50 cases in each group. Group A received white light endoscopy, and group B received narrow-band imaging technology combined with endoscopy. Narrow-band imaging combined with magnifying endoscopy was used to stain the area with suspicious mucosal lesions with indigo carmine and magnified observation. Results: The endoscopic image clarity of group B was significantly better than that of group A in terms of lesion outline, gastric pit, and microvascular morphology (P < 0.05). There were 10 cases of early gastric cancer, 18 cases of benign lesions, and 9 cases of gastric cancer (nonearly stage); 17 cases of precancerous lesions, 12 cases of early gastric cancer, 13 cases of benign lesions, and 6 cases of gastric cancer (nonearly stage) were diagnosed by ordinary white light endoscopy. Pathological results confirmed that among the 50 patients in group B, there were 15 cases of precancerous lesions, 11 cases of early gastric cancer, 17 cases of benign lesions, and 7 cases of gastric cancer (nonearly stage). Among the 50 patients in group A, 16 were precancerous lesions, 11 were early gastric cancer, 15 were benign lesions, and 8 were gastric cancer (non early stage). In the diagnosis of precancerous lesions and early gastric cancer, the diagnostic consistency, sensitivity, and specificity of group B were better than those of group A (P < 0.05); NBI combined with endoscopy in the diagnosis of precancerous lesions and early gastric cancer (kappa = 0.860, kappa = 0.883) was more consistent with pathological diagnosis than common white light endoscopy (kappa = 0.433, kappa = 0.535). Conclusion: The value of narrow-band imaging technology combined with endoscopy in the diagnosis of precancerous lesions and early gastric cancer is better than that of ordinary white light endoscopy, and it can be widely used in clinical practice.
Usefulness of magnifying narrow-band imaging endoscopy for the diagnosis of gastric and colorectal lesions

Abstract:A series of studies about the potential usefulness of magnifying endoscopy with narrow-band imaging (NBI) for the diagnosis of gastric and colonic lesion is reviewed. Concerning the magnifying NBI appearances of gastric lesions, a light blue crest is a highly accurate sign of the presence of histological intestinal metaplasia. Also, the degree of irregularity of the mucosal and vascular pattern is correlated with the histological severity of Helicobacter pylori-associated chronic gastritis. According to the 'VS classification', an irregular microvascular pattern and/or an irregular microsurface pattern together with a clear demarcation line are characteristic for early gastric carcinoma, and a multicenter prospective randomized controlled trial demonstrated that magnifying endoscopy with NBI is superior to ordinary white light endoscopy for making a differential diagnosis of a small depressed lesion between carcinoma and non-carcinoma. Concerning the magnifying NBI appearances of colonic tumor, the vague or invisible microvascular pattern is mostly observed in hyperplastic polyp. The regular meshed microvascular pattern is mostly observed in adenoma. The irregular meshed microvascular pattern is mostly observed in intramucosal or shallow submucosal-invasive carcinoma. The decreased or loose microvasucular pattern is mostly observed in deep submucosal-invasive carcinoma. Thus, magnifying NBI endoscopy is useful for the differentiation of colorectal non-adenomatous lesions from adenoma, the differentiation of adenoma from carcinoma, and the assessment of invasion depth of early colorectal carcinoma. At present, several magnifying NBI classifications for the diagnosis of early colorectal neoplasia have been proposed in Japan. Recently, the NICE classification based on NBI findings with/without magnification for colorectal tumor was established by an international group.

Abstract:Background: Minute gastric cancers (MGCs) have a favorable prognosis, but they are too small to be detected by endoscopy, with a maximum diameter ≤ 5 mm. Aim: To explore endoscopic detection and diagnostic strategies for MGCs. Methods: This was a real-world observational study. The endoscopic and clinicopathological parameters of 191 MGCs between January 2015 and December 2022 were retrospectively analyzed. Endoscopic discoverable opportunity and typical neoplastic features were emphatically reviewed. Results: All MGCs in our study were of a single pathological type, 97.38% (186/191) of which were differentiated-type tumors. White light endoscopy (WLE) detected 84.29% (161/191) of MGCs, and the most common morphology of MGCs found by WLE was protruding. Narrow-band imaging (NBI) secondary observation detected 14.14% (27/191) of MGCs, and the most common morphology of MGCs found by NBI was flat. Another three MGCs were detected by indigo carmine third observation. If a well-demarcated border lesion exhibited a typical neoplastic color, such as yellowish-red or whitish under WLE and brownish under NBI, MGCs should be diagnosed. The proportion with high diagnostic confidence by magnifying endoscopy with NBI (ME-NBI) was significantly higher than the proportion with low diagnostic confidence and the only visible groups (94.19% > 56.92% > 32.50%, P < 0.001). Conclusion: WLE combined with NBI and indigo carmine are helpful for detection of MGCs. A clear demarcation line combined with a typical neoplastic color using nonmagnifying observation is sufficient for diagnosis of MGCs. ME-NBI improves the endoscopic diagnostic confidence of MGCs.
Bully/victim problems in Northern Ireland's schools: data from the 2003 Young Persons' Behavior and Attitude Survey.

Abstract:McGuckin and Lewis (2003, 2006, 2008), Mc Guckin, Lewis and Cummins (under review b) have reported that little is known about the nature, incidence and correlates of bully/victim problems in the Northern Ireland school system. The aim of the present study was to examine the prevalence of self-reported experiences of bully/victim problems among a representative sample of 7,223 11- to 16-year-olds living in Northern Ireland who participated in the 2003 Young Persons' Behaviour and Attitude Survey (YPBAS: Northern Ireland Statistics and Research Agency [NISRA], 2003). Respondents were presented with three questions inquiring explicitly and three questions inquiring implicitly about bully/victim problems. Among other questions, respondents volunteered other salient information about personal experiences of bully/victim problems (i.e., through use of the "other" response option). Almost one-fifth of all respondents (17.2%, n = 1,026) reported being a victim of bullying behavior, and 8.1% (n = 492) reported that they had picked on or bullied another school pupil. Bully/victim problems also pervaded personal experiences of school meal times, sporting activities, and perceptions of personal safety. These findings are placed within the context of previous findings.
