Abstract:By 2050, the proportion of people over 60 years of age worldwide is expected to be 22 %, and the number of people over 80 is expected to triple by that time, reaching 426 million. Population aging threatens the sustainable functioning of national health care systems, since the increase in the proportion of the elderly population is accompanied by increased costs for the health care system and a decrease in the availability of medical care for other age groups. Based on the analysis and synthesis of data from foreign publications on the impact of population aging on the sustainability of the functioning of national healthcare systems, proposed measures and models of medical care that take into account changes in the age structure of the population are systematized. Analysis of foreign publications made it possible to identify the following challenges for the healthcare system associated with the aging population: rising costs of providing medical care, the need to increase emphasis on preventive measures, increasing prevalence and severity of chronic non-communicable diseases and comorbidity, inequality in access to medical care, expressed in territorial (regional), age and gender aspects, as well as determined by the socio-economic status of a person. Experts suggest that the most obvious and realistic solution to the problem is active prevention of chronic diseases and more intensive monitoring of the health status of the population. Digital solutions that provide disease prevention and remote monitoring of the health status of the elderly population, based on the use of machine learning technologies, artificial intelligence and telemedicine technologies, are identified as promising areas for limiting healthcare costs.

Abstract:Population ageing is both an achievement and challenge, an achievement as longevity is the result of successful prevention and control of diseases, decreasing fertility rates and overall socio-economic development. It is at the same time a challenge as the increasing number of older people and the resultant demographic shift are accompanied by the need to adjust and scale up the social and health care systems. The challenges are of particular relevance to the developing world where the demographic shift is occurring much faster. Comprehensive efforts based on country contexts are required in the following areas: (a) older persons and development, (b) health and well-being and (c) enabling and supportive environments to address population ageing needs. This article, however, focuses only on three most crucial issues, that is, livelihood, health care systems and care of the older dependent people. Measures to sustain the livelihood of older people, to align the health systems to provide care and to develop long-term care systems are highlighted. Person-centred care, integration and functional capacity are advocated. Further, ageing in place or living in one’s own home, community or a place with the closest fit with the person’s needs and preferences is considered very important for healthy ageing. In terms of enhancing livelihood, major policy changes and reforms to improve the social security systems and expanding coverage as well as increasing the amounts to minimum subsistence levels are highlighted. Another area which needs to be strengthened is the tradition of existing family support systems. The health systems alignment required are reflected for each health system building block, and focuses mainly on (a) developing and ensuring access to services that provide older-person-centred care; (b) shifting the clinical focus from disease to intrinsic capacity; and (c) developing or reorienting the health workforce to provide care as per alignment. Long-term care systems would best meet the needs of dependent older people if families, communities, civil society organisations and private sector are equally involved while governments play leadership roles in setting up and monitoring quality.

Abstract:Population aging is a significant medical and sociodemographic issue facing the world today. While the financial circumstances of older adults have seen some progress in recent decades, a significant number of senior citizens in the United States still grapple with the challenges of affording daily necessities and encounter inadequacies in their health insurance coverage. Current research has explored the challenges in elderly care, including shortages of medical staff and the need for integrating medical care with nursing care. However, there is still a lack of comprehensive policy approaches to address these issues. This article analyzes the current landscape of elderly care policies and proposes strategies to improve the independence and caregiving abilities of older people. The key findings suggest that increasing fertility rates through government policies and promoting the integration of nursing and medical care can significantly improve the quality of life for the elderly. The proposed "medical care and elderly care" model combines medical treatment with principles of elderly care, offering a more comprehensive approach to meet the health and well-being needs of older adults. The insights from this article provide valuable references for future policymaking and research in elderly care. However, further studies are needed to explore the implementation and effectiveness of these proposed strategies in various contexts. Future research should also focus on developing innovative solutions to address the growing challenges in elderly care as the global population continues to age.
Health and socioeconomic resource provision for older people in South Asian countries: Bangladesh, India, Nepal, Pakistan and Sri Lanka

Abstract:Abstract Background The global population is ageing rapidly, with low- and middle-income countries (LMICs) undergoing a fast demographic transition. This necessitates effective services to address the increasing physical and mental health needs of multimorbid and frail older people in LMICs. We review the current provision of health and socioeconomic resources for older people in South Asian countries: Bangladesh, India, Nepal, Pakistan and Sri Lanka, to identify gaps in available resources and assess areas for improvement. Methods We conducted a search of grey and published literature via Google Search and Compendex, EBSCO, JSTOR, Medline, Ovid, ProQuest databases, Scopus and Web of Science to extract data on population demographics, human resources, health funding and social security provision for older people. Local informants were consulted to supplement and verify the data. Results In most study countries, care of the elderly is provided by general physicians or primary care doctors, nurses and community health workers, though in very low numbers per capita. The number of geriatricians and specialist allied health professionals were largely unknown, with minimal postgraduate programmes available for specific training in geriatric medicine or psychiatry. Total average healthcare expenditure as a percentage of GDP was between 2.5-5.5%, markedly below the global average of 10.0%, with between 48.1-72.0% of healthcare costs covered by out-of-pocket payments. All countries, except Pakistan, had a social pension offering a minimum level of protection against financial constraints, but with varying coverage of those eligible. Financial provision for people with dementia was only available in India and Nepal. A modest number of state and charity-run care homes were established in all countries, though numbers were disproportionate to the growing number of older adults requiring long term care. Conclusions Inadequate health funding, a lack of specific healthcare and training and insufficient government pension and social security schemes are a current threat to achieving universal health coverage in LMICs. Governing bodies must establish geriatrics and geriatric psychiatry as specialities and scale up training programmes for specific healthcare providers for the elderly. This needs to occur alongside increased social protection provision to improve access to those in need and prevent catastrophic health expenditure.
Navigating the Health Care Landscape for an Ageing Population: An International Survey of Strategies and Priorities

Abstract:Objectives: The global increase in the older population, which is expected to reach 1.5 billion by 2050, poses significant challenges for publicly funded health care systems. Life expectancy, although positive, is leading to an increase in chronic diseases requiring complex and costly health and social solutions. This study explores key strategies to address these challenges. Design: Qualitative interviews followed by a survey. Setting and participants: The study involved experts, students, artificial intelligence, and participants at a congress. Methods: We first interviewed 5 experts from different countries representing health care management and psychology from Belgium, health economics from Canada, sociology from France, and geriatrics from Switzerland. In addition, a focus group session with medical students in physical therapy and queries to ChatGPT increased the range of perspectives. A synthesis of all opinions or insights was used to formulate concrete strategies. These strategies were incorporated into an online survey that was distributed to 215 participants of the Geriatric and Gerontologic Congress in Montreal, Canada, in September 2023. Results: All 20 potential solutions were duly acknowledged, with particular attention paid to the following 5 priorities: the urgent need to integrate geriatric training into the education of future health professionals, the promotion of home-based care models, the establishment of comprehensive and integrated care systems, the strengthening of primary care services, and the emphasis on primary prevention strategies. Conclusion and implications: This study highlights key priorities for addressing the health needs of the older population. By emphasizing education, home-based care, and integrated services and strengthening primary care and prevention, health systems can respond effectively to the challenges of an ageing population. Although these needs may not be entirely unmet, they indicate areas where existing services are insufficient in providing adequate coverage and support to ensure tailored and sustainable health care solutions for older people.

Abstract:Concern about the rapid ageing of all societies reaches alarming proportions as healthcare inequities are steeply rising, prompting the elderly to live longer but subject to insufficient social protection and healthcare in the wake of dwindling public resources. The aged population of developing nations are facing additional hardships due to the growing gap between needs and the financial reductions of public institutions, retirement funds, and the trend towards privatization of essential services turned into commodities. Current approaches to allocation of insufficient resources without ageist discrimination are briefly discussed: individual self-care aimed at successful, active and healthy ageing based on resourcefulness of the privileged elderly; utilitarian approaches founded on QALY and fair innings, and human rights focused on the plights of the elderly. These approaches cannot apply to resources poor nations, who need to engage in context-bound bioethics dealing with the realities of their exposed ageing population. A developing world bioethics is needed to face the plights of the elderly in countries with low and middle-income and insufficient social capital. Suggested are: 1) a phenomenological approach based on the interaction of bioethics and ethnology, furthering grass-roots input from the elderly; 2) Create small communities -campus-like boroughs- to simplify accessibility to social services and healthcare facilities, as an alternative to the high-cost WHO proposal of age-friendly large cities.

Abstract:Abstract Background From 2020 to 2050, China’s population aged ≥65 years old is estimated to more than double from 172 million (12·0%) to 366 million (26·0%). Some 10 million have Alzheimer’s disease and related dementias, to approach 40 million by 2050. Critically, the population is ageing fast while China is still a middle-income country. Methods Using official and population-level statistics, we summarise China’s demographic and epidemiological trends relevant to ageing and health from 1970 to present, before examining key determinants of China’s improving population health in a socioecological framework. We then explore how China is responding to the care needs of its older population by carrying out a systematic review to answer the question: ‘what are the key policy challenges to China achieving an equitable nationwide long-term care system for older people?’. Databases were screened for records published between 1st June 2020 and 1st June 2022 in Mandarin Chinese or English, reflecting our focus on evidence published since introduction of China’s second long-term care insurance pilot phase in 2020. Results Rapid economic development and improved access to education has led to widescale internal migration. Changing fertility policies and household structures also pose considerable challenges to the traditional family care model. To deal with increasing need, China has piloted 49 alternative long-term care insurance systems. Our findings from 42 studies (n = 16 in Mandarin) highlight significant challenges in the provision of quality and quantity of care which suits the preference of users, varying eligibility for long-term care insurance and an inequitable distribution of cost burden. Key recommendations include increasing salaries to attract and retain staff, introduction of mandatory financial contributions from employees and a unified standard of disability with regular assessment. Strengthening support for family caregivers and improving smart old age care capacity can also support preferences to age at home. Conclusions China has yet to establish a sustainable funding mechanism, standardised eligibility criteria and a high-quality service delivery system. Its long-term care insurance pilot studies provide useful lessons for other middle-income countries facing similar challenges in terms of meeting the long-term care needs of their rapidly growing older populations.

Abstract:The global population is undergoing a significant surge in aging leading to increased susceptibility to various forms of progressive illnesses. This phenomenon significantly impacts both individual health and healthcare systems. Low and Middle Income Countries face particular challenges, as their Primary Health Care (PHC) settings often lack the necessary human and material resources to effectively address the escalating healthcare demands of the older people. This study set out to explore the experiences of older people living with progressive multimorbidity in accessing PHC services in Malawi. Between July 2022 and January 2023, a total of sixty in-depth interviews were conducted with dyads of individuals aged ≥50 years and their caregivers, and twelve healthcare workers in three public hospitals across Malawi’s three administrative regions. The study employed a stratified selection of sites, ensuring representation from rural, peri-urban, and urban settings, allowing for a comprehensive comparison of diverse perspectives. Guided by the Andersen-Newman theoretical framework, the study assessed the barriers, facilitators, and need factors influencing PHC service utilization by the older people. Three themes, consistent across all sites emerged, encompassing barriers, facilitators, and need factors respectively. The themes include: (1) clinic environment: inconvenient clinic setup, reliable PHC services and research in diabetic foods; (2) geographical factors: bad road conditions, lack of comprehensive PHC at local health facility and need for community approaches; and (3) social factors: encompassing use of alternative medicine, transport support, perceived health care benefit and support with startup capital for small-scale businesses. This research highlights the impact of multiple factors on the access to and utilization of PHC services among older individuals, emphasizing the urgent need for enhanced nationwide availability of such services. It strongly recommends a thorough investigation into successful practices implemented in diverse health facilities in Malawi, with a specific focus on addressing the unique healthcare needs of the older population.

Abstract:In line with the worldwide trend in population aging, China has stepped into an aging society since 2000. The outstanding features of aging, including a large proportion of the older population, rapid growth, dramatic expansion of the oldest-old, and uneven aging distribution, have put China in a unique position. Besides, older population is expanding in parallel with the escalating burden of disease, high prevalence of disability, and low social involvement. However, China is not prepared to solve these problems in terms of the economy, awareness, geriatric care system, geriatric team, social security, or age-friendly environment. From the perspective of public health, we summarized the major challenges and proposed the following policy recommendations: (1) strengthening the top-level design and building a “government-leading, multi-sectoral-cooperating, and society-participating” pattern; (2) enhancing health services by implementing the “comprehensive health” strategy; (3) developing home and community care, coordinately enhance institutional care, promote integration of medical and care systems, and establish a multidimensional tailored care system; (4) optimizing geriatric the supporting system, included the construction of geriatric team and the long-term care insurance system; and (5) establishing a physical and socially age-friendly environment.

Abstract:The global population aged 60 and older is likely to grow to historically unprecedented levels. According to the latest estimation, 2 billion people will be aged 60 and over by 2050—a number equivalent to 22% of the world’s population. China’s aging population also continues to grow rapidly; the Chinese population aged 60 and older reached 14.8% by the end of 2013. This percentage is estimated to reach 17.17% by 2020, and may be as high as 30% by 2050. The roots of population aging in China are the same as elsewhere: a low fertility rate, a longer life expectancy, and the effective control of infectious diseases. However, China faces other challenges as well, as it possesses the largest population in the developing world. These challenges are: 1 a low GDP per capita (announced at #80 globally, in 2014 by International Monetary Fund), 2 an economic imbalance between coastal and inland regions, 3 a rapidly increasing ratio of urban to rural population, and 4 an increasing amount of medical expenses for the aging population. According to statistics from the National Health and Family Planning Commission of the People’s Republic of China (NHFPC), almost 50% of people over 60 suffer from chronic diseases; of these, hypertension, diabetes, Alzheimer’s disease, stroke, and coronary disease are the top five. Cancer is also a heavy burden for people and for society. From an economic point of view, the health expenditure for China’s aging population was 2.0456 trillion CNY in 2014, accounting for 3% of the GDP; and medical expenses for seniors accounted for 50% of existing healthcare resources.

Abstract:China is witnessing a profound demographic transformation marked by a swift rise in its elderly population, posing significant challenges to healthcare and retirement systems. This study investigates the multifaceted impact of this demographic shift, evaluating the effectiveness of government policies in mitigating these challenges. Driven by increased life expectancy and declining fertility rates, this demographic transition necessitates strategic policy responses. The central argument revolves around scrutinizing the efficacy of China's policies in addressing healthcare and retirement hurdles arising from this demographic change. The hypothesis posits that governmental initiatives, such as healthcare infrastructure investments, pension system reforms, and expanded healthcare coverage, will partially alleviate existing challenges. Moreover, the paper hypothesizes on the societal impact of these policies, aiming to enhance elderly individuals' quality of life, alleviate healthcare-related financial burdens, and stimulate economic growth while relieving social welfare pressures. Employing a literature analysis methodology, this study critically evaluates existing scholarly works, policy documents, and empirical studies. While acknowledging the present challenges, it anticipates potential disparities in medical resources, sustainability concerns in pension systems, and issues surrounding the social integration of the elderly as the aging population expands. Recommendations underscore the necessity for adaptive policies to sustainably address these challenges. Through a comprehensive approach, this paper analyzes current policies, forecasts their impact, and conjectures future challenges, aiming to provide holistic insights into the effectiveness of policies aimed at managing the intricacies of an aging population in China.

Abstract:Populations globally are ageing, in part due to dramatic increases in life expectancies, forcing a reconsideration of what constitutes being "elderly" and "old." The proportion of older adults living with disability may be decreasing, yet older individuals are living with a significant burden of chronic disease, geriatric impairments in cognition, vision and hearing and reduced physiological reserve (frailty). Caring for a growing number of medically complex individuals has implications for medical workforce size and composition, health programmes and expenditure. Future responses to an ageing population will require further innovation in health-care delivery models, and increasing representation of older adults in clinical trials.

Abstract:With population ageing and increased longevity, Australia faces pressing policy issues on caring for older people. As Medicare turns 40, it is timely to assess how the system has been performing, and what changes are needed to modernise Medicare. This article examines the quality and efficiency of care using hospital administrative data. We find that the current system provides good quality and efficient care for older patients, except for those living in residential aged care homes. These patients receive lower quality care yet stay longer in hospitals. We suggest alternative approaches designed around bundled payments and a single budget holder to provide incentives for integrated team‐based care.

Abstract:Introduction. The article is devoted to the topical issues in the area of ensuring the right of the older persons to health, taking into account modern realities. In particular, it focuses on the existing problematic aspects through the prism of the right to decent treatment and autonomy. The issue of abuse of the elderly, which negatively affects their physical and mental health and providing palliative care, which helps to improve the quality of life of patients with diseases that limit life expectancy have been analyzed; it focuses on the problem of dementia, which is one of the leading causes of disability and dependence among older people around the world; given the fact that older people are at high risk during COVID-19, the focus is on protecting this category of people during a pandemic. The main results of the study. An analysis of the existing legal framework for cooperation between states to overcome the negative trends in the provision of health care to the elderly, in particular: the Toronto Declaration on the Global Prevention of Elder Abuse, the Council of Europe Convention on preventing and combating violence against women and domestic violence, the European Social Charter (revised), the Inter-American Convention on Protecting the Human Rights of Older Persons, the Global action plan on the public health response to dementia 2017-2025, the “Towards a Dementia Plan” Guide , World Health Organization Guidelines for Reducing the Risk of Cognitive Function and Dementia. In addition, attention is paid to the recommendations of the main bodies of international intergovernmental organizations, in particular the Committee of Ministers of the Council of Europe, and international treaty bodies in this field - the Committee on Economic, Social and Cultural Rights, the Committee on the Elimination of Discrimination against Women (General recommendation No 24, General recommendation No 27) etc. Relevant conclusions and recommendations have been made. Conclusions. It should be noted that the existing tendency of the population to age is a reality of today and encourages society to seek new ways of life in these circumstances. Aging is no longer seen as a problem, but rather as a challenge and an opportunity for society to respond and take timely adaptation measures. The international community is making significant efforts to create a legal framework, develop standards, and develop international cooperation to ensure a decent old age for the elderly. The task for governments is to implement international norms into national legislation and to optimize national measures in this area. The field of healthcare for the older persons is the most sensitive, as it is a question of a decent standard of living, so it needs a balanced strategy, which should include: disease prevention (diagnosis, medical care, etc.); development, improvement, introduction of an affordable and inexpensive system of long-term care with the involvement of the elderly themselves in this work and training of specialists who will work in this field; training of persons who informally care for the elderly; regular independent monitoring of long-term care services, based on clear principles and rights that older people can exercise on their own; giving preference to home care; creation of appropriate services to provide advice, psychological, legal assistance to the elderly. It is also important for society to be aware of aging in order to understand the problems and not to cause repeated psychological trauma to the older persons so they don’t feel like a “burden” for society, rather than full members.

Abstract:The increase in the aging population in the ages of above 60 years old has given huge impact on the socioeconomic, growth and human development of a country. Meanwhile, poor well-being initiatives in a nation contribute more on neglect of elderly welfare, and mental and physical abuse affecting better life quality and satisfaction. Approximately 703 million persons aged 65 years old in the world in year 2019, and it is projected to double to 1.5 billion in 2050. Millions of senior citizens are exposed to two main problems, physical and mental health, and financial capacity. Financial poverty, particularly, is the core in an ageing society due to lack of regular income, the death of a spouse, mounting medical bills, and poor planning for retirement among elderly people. Thus, the main aim of this article is to review the role of financial support in improving elderly well-being. The main implication of this article reacts to government authorities in planning and providing initiatives or assistance for the elderly financially. Therefore, proper welfare, policy and legal services for the elderly require a deeper understanding of the relationship between the individual characteristics of adults and preferred financial components to improve elderly well-being and life satisfaction. For future recommendations, studies through observations, interviews, and empirical investigations would provide evidence the real demand of financial support and its components towards improving elderly well-being.

Abstract:Background: China is facing the challenge of an increasingly ageing population. The proportion of people aged 60 and above increased from 10% to 21.1% between 1999 and 2023 and is projected to reach 28% by 2040. In response to this demographic shift, the Chinese central government has implemented a series of policies that have evolved over time. The rapid advancement of information technology and science has accelerated these changes. However, there is still a research gap in understanding the digital service policies for elderly care. Methodology: Based on research literature and policy texts, this study specifically focuses on policy development between January 2019 and January 2024. Seventy traceable national policy texts were categorized and analyzed, gathered from the State Council's policy document database and the official websites of national ministries. Result: The 70 policies were interpreted and categorized into three major aspects. Firstly, the policies on the improvement of traditional elderly care aimed to meet diverse needs by focusing on health and medical care, the management and standards of serving institutions, service improvements and standardization, and the development of the service workforce. Secondly, the policies on system integration between elderly care and other industries included the integration of finance and elderly care, the construction of the insurance system, long-term care institutions and insurance, hospice care, and the silver economy. Thirdly, the policies on digital services comprised the smart healthcare industry, information accessibility, elderly-friendly digital technology and empowerment, and the creation of elderly-friendly communities and societies. Conclusion: The development concepts and contents of China's elderly care policy have evolved. The acceleration of transformation and promotion of the silver economy has become a new comprehensive and universal measure. Despite recent policy developments indicating a growing trend towards integration, there remains considerable room for improvement in actual implementation. This should focus on the uneven development of the "three pillars" of the pension system, insufficient innovation in financial products, and inadequate industry financing channels. It is important to be alert to the persistent problems and obstacles, which widen the gap in elderly welfare, create new forms of social inequality and exclusion, and hinder the development of a friendly information society. Abstract:Most older persons face two potentially serious economic problems: (a) declining earning power and (b) declining health that can be partly offset by increased utilization of health care. The decline in earning power is largely attributable to physiological changes and to obsolescence of skills and knowledge. These adverse effects are exacerbated by public and private policies that reduce the incentives of older persons to continue work and increase the cost to employers of employing older workers. The problems of earnings replacement and health care payment are usually discussed separately, but there are several reasons why they should be considered together. First, there are often tradeoffs between the two. Money is money, and for most people there is never enough to go around. This is self-evident where private funds are concerned. Low-income elderly, for instance, frequently must choose between expensive prescription drugs and an adequate diet. For middle-income elderly, the choice may be between saving on medigap insurance or forgoing an airplane trip to a grandchild's graduation. Difficult choices are also inherent in the allocation of public funds. The same tax receipts that could be used to maintain or increase retirement benefits could be used to fund additional care, and vice versa. In discussing these tradeoffs, some analysts assert that people will gladly give up other goods and services for medical care that cures illness, relieves pain, or restores function. Others believe that some people would forgo some health insurance in order to maintain access to other goods and services. A second reason for looking at the two problems together is that they pose similar questions for public policy. How much should each generation provide for its own needs in old age, and how much should be provided by the generations that follow? How much provision should be voluntary, how much compulsory? How much intra-generational redistribution is appropriate after age 65? How well can private markets serve the elderly's desire for annuities and health insurance, and when are public programs more efficient? Finally, the problem of health care payment is approaching that of earning replacement in economic importance and, by 2020, will far exceed it. Declining health after age 65 results in substantial increase in use of prescription drugs, hospital admissions, repair or replacement of parts of the body, rehabilitation and physical therapy, and assistance with activities of daily living. New technologies offer great promise for offsetting the health problems of aging, but often at considerable expense. Overall, per capita expenditures for health care after age 65 are between three and four times as great as for those under 65. This presentation will focus primarily on the (thus far) inexorable increase in consumption of health care by older Americans. If this increase continues and if the government's share of the total remains unchanged, the tax burden on younger cohorts could become intolerable. Concomitantly, if the private share remains unchanged, the ability of the elderly to obtain other goods and services would be sharply diminished. Although the emphasis of the session will be on aggregate and average results, levels and trends in income inequality among the elderly will also be examined and compared with inequality at younger ages. The session will conclude with a discussion of changes that might avert the economic and social crises foreshadowed in the data.
