Abstract:Long-term care (LTC) policy in China is in its infancy, and it is highly decentralised. Where policy structures exist, they are poorly resourced. Although China's demography is still young by developed country standards, it is ageing very rapidly, and by mid-century will have "caught up" with many countries in the developed world with respect to population ageing. LTC policy development, therefore, is becoming a priority in China. We argue that it should be formulated with population ageing as a framework.Policy designs, which take account of and encourage, informal care provision, will be critical to the fiscally sustainable delivery of LTC. In China, informal care is sometimes seen as very scarce because of the one child policy. With only one child, it is argued, there will be less informal care offered than in societies with larger families.This paper uses the recently developed China Health and Retirement Longitudinal Study (CHARLS) dataset to analyse the current patterns of disability and informal care availability. In particular, and contrary to expectation, we find that fertility change is not the main driver for reducing informal care. Education levels, living standards, urbanization and co-residency are much more important. This suggests that current policy, which targets those with one child families, may be misguided, and also that mechanical extrapolations of future demand for care may be misleading.

Abstract:Many countries face policy challenges related to the well-being of their aging populations, and China, with the world's largest aging population, is no different. Over the past 40 years, China has experienced demographic transition toward an “aging society.” According to the National Bureau of Statistics in China (NBSC), the number of people aged 60 years and older in China has risen to 185 million, with rural areas aging more rapidly during China's demographic transition (National Bureau of Statistics of China, 2013). In 1982, the proportion of population aged 60 years and older was 7.8% in rural China and 7.1% in urban China. However, by 2011 these proportions had risen to 12.6% and 15.4%, respectively, with a national average of 13.26% (National Bureau of Statistics of China, 2013). Difference between rural and urban areas in the proportion of the population aged 65 years and older in China has shown a similar trend over time. Rapid economic reforms since the 1980s, associated with increased rural-to-urban migration, especially the outflow of rural young population, have resulted in a larger proportion of elderly adults in rural areas (Cai et al ., 2012; National Bureau of Statistics of China, 2013). These same social forces have resulted in relatively fewer people available to take care of the growing population of older adults as well. The “elderly dependency ratio” (the ratio of older adults in China to those in the population of working age) has increased from 8.0% in 1982 to 12.7% in 2012 (National Bureau of Statistics of China, 2013). The economic and social impact on caregiving will be profound.
Factors associated with healthy ageing: a comparative study between China and the United States

Abstract:This study compares the associators of healthy ageing in China and the United States, using the 2005–2014 Chinese Longitudinal Healthy Longevity Survey (CLHLS) and the 2004–2014 Health and Retirement Study (HRS). Health ageing is measured using an overall healthy ageing index (HAI), defined according to five dimensions: no major chronic diseases, free of physical functional impairment, free of cognitive impairment, no depressive symptoms, and socially active engagement. Multilevel logistic regression analysis is adopted to explore the association of demographic characteristics, socioeconomic status, and healthy lifestyles with the odds ratios of healthy ageing in China and the United States. Results indicate that the proportion of individuals experiencing healthy ageing is slightly higher in US than Chinese older adults aged 65–100 (25.67% vs 23.27%). Nevertheless, Chinese oldest-old (80–100) have a higher proportion of healthy ageing than American oldest-old (15.05% vs. 12.19%). Our results indicate that the odds of healthy ageing decrease with age, whereas they increase with education, income, marriage, and health behaviours. The odds of healthy ageing are lower for older Chinese women than men, whereas the odds are higher for older US women. Moreover, non-white elders in the United States have significantly lower odds of healthy ageing than their white counterparts. These findings suggest certain similar patterns of healthy ageing across two countries, but distinct patterns do exist. Promoting gender and racial equalities in healthy ageing are crucial for Chinese and American policymakers, respectively.

Abstract:In line with the worldwide trend in population aging, China has stepped into an aging society since 2000. The outstanding features of aging, including a large proportion of the older population, rapid growth, dramatic expansion of the oldest-old, and uneven aging distribution, have put China in a unique position. Besides, older population is expanding in parallel with the escalating burden of disease, high prevalence of disability, and low social involvement. However, China is not prepared to solve these problems in terms of the economy, awareness, geriatric care system, geriatric team, social security, or age-friendly environment. From the perspective of public health, we summarized the major challenges and proposed the following policy recommendations: (1) strengthening the top-level design and building a “government-leading, multi-sectoral-cooperating, and society-participating” pattern; (2) enhancing health services by implementing the “comprehensive health” strategy; (3) developing home and community care, coordinately enhance institutional care, promote integration of medical and care systems, and establish a multidimensional tailored care system; (4) optimizing geriatric the supporting system, included the construction of geriatric team and the long-term care insurance system; and (5) establishing a physical and socially age-friendly environment.

Abstract:China has transitioned from being one of the fastest-growing populations to among the most rapidly aging countries worldwide. In particular, the population of oldest-old individuals, those aged 80+, is projected to quadruple by 2050. The oldest-old represent a uniquely important group—they have high demand for personal assistance and the highest healthcare costs of any age group. Understanding trends in disability and longevity among the oldest-old—that is, whether successive generations are living longer and with less disability—is of great importance for policy and planning purposes. We utilized data from successive birth cohorts (n = 20,520) of the Chinese oldest-old born 10 years apart (the earlier cohort was interviewed in 1998 and the later cohort in 2008). Disability was defined as needing personal assistance in performing one or more of five essential activities (bathing, transferring, dressing, eating, and toileting) or being incontinent. Participants were followed for age-specific disability transitions and mortality (in 2000 and 2002 for the earlier cohort and 2011 and 2014 for the later cohort), which were then used to generate microsimulation-based multistate life tables to estimate partial life expectancy (LE) and disability-free LE (DFLE), stratified by sex and age groups (octogenarians, nonagenarians, and centenarians). We additionally explored sociodemographic heterogeneity in LE and DFLE by urban/rural residence and educational attainment. More recently born Chinese octogenarians (born 1919–1928) had a longer partial LE between ages 80 and 89 than octogenarians born 1909–1918, and octogenarian women experienced an increase in partial DFLE of 0.32 years (P = 0.004) across the two birth cohorts. Although no increases in partial LE were observed among nonagenarians or centenarians, partial DFLE increased across birth cohorts, with a gain of 0.41 years (P < 0.001) among nonagenarians and 0.07 years (P = 0.050) among centenarians. Subgroup analyses revealed that gains in partial LE and DFLE primarily occurred among the urban resident population. Successive generations of China’s oldest-old are living with less disability as a whole, and LE is expanding among octogenarians. However, we found a widening urban-rural disparity in longevity and disability, highlighting the need to improve policies to alleviate health inequality throughout the population.

Abstract:Abstract Background From 2020 to 2050, China’s population aged ≥65 years old is estimated to more than double from 172 million (12·0%) to 366 million (26·0%). Some 10 million have Alzheimer’s disease and related dementias, to approach 40 million by 2050. Critically, the population is ageing fast while China is still a middle-income country. Methods Using official and population-level statistics, we summarise China’s demographic and epidemiological trends relevant to ageing and health from 1970 to present, before examining key determinants of China’s improving population health in a socioecological framework. We then explore how China is responding to the care needs of its older population by carrying out a systematic review to answer the question: ‘what are the key policy challenges to China achieving an equitable nationwide long-term care system for older people?’. Databases were screened for records published between 1st June 2020 and 1st June 2022 in Mandarin Chinese or English, reflecting our focus on evidence published since introduction of China’s second long-term care insurance pilot phase in 2020. Results Rapid economic development and improved access to education has led to widescale internal migration. Changing fertility policies and household structures also pose considerable challenges to the traditional family care model. To deal with increasing need, China has piloted 49 alternative long-term care insurance systems. Our findings from 42 studies (n = 16 in Mandarin) highlight significant challenges in the provision of quality and quantity of care which suits the preference of users, varying eligibility for long-term care insurance and an inequitable distribution of cost burden. Key recommendations include increasing salaries to attract and retain staff, introduction of mandatory financial contributions from employees and a unified standard of disability with regular assessment. Strengthening support for family caregivers and improving smart old age care capacity can also support preferences to age at home. Conclusions China has yet to establish a sustainable funding mechanism, standardised eligibility criteria and a high-quality service delivery system. Its long-term care insurance pilot studies provide useful lessons for other middle-income countries facing similar challenges in terms of meeting the long-term care needs of their rapidly growing older populations.

Abstract:<p>One of the key issues facing public healthcare is the global trend of an increasingly ageing society which continues to present policy makers and caregivers with formidable healthcare and socio-economic challenges. Ageing is the primary contributor to a broad spectrum of chronic disorders all associated with a lower quality of life in the elderly. In 2019, the Chinese population constituted 18 % of the world population, with 164.5 million Chinese citizens aged 65 and above (65+), and 26 million aged 80 or above (80+). China has become an ageing society, and as it continues to age it will continue to exacerbate the burden borne by current family and public healthcare systems. Major healthcare challenges involved with caring for the elderly in China include the management of chronic non-communicable diseases (CNCDs), physical frailty, neurodegenerative diseases, cardiovascular diseases, with emerging challenges such as providing sufficient dental care, combating the rising prevalence of sexually transmitted diseases among nursing home communities, providing support for increased incidences of immune diseases, and the growing necessity to provide palliative care for the elderly. At the governmental level, it is necessary to make long-term strategic plans to respond to the pressures of an ageing society, especially to establish a nationwide, affordable, annual health check system to facilitate early diagnosis and provide access to affordable treatments. China has begun work on several activities to address these issues including the recent completion of the of the Ten-year Health-Care Reform project, the implementation of the Healthy China 2030 Action Plan, and the opening of the National Clinical Research Center for Geriatric Disorders. There are also societal challenges, namely the shift from an extended family system in which the younger provide home care for their elderly family members, to the current trend in which young people are increasingly migrating towards major cities for work, increasing reliance on nursing homes to compensate, especially following the outcomes of the 'one child policy' and the 'empty-nest elderly' phenomenon. At the individual level, it is important to provide avenues for people to seek and improve their own knowledge of health and disease, to encourage them to seek medical check-ups to prevent/manage illness, and to find ways to promote modifiable health-related behaviors (social activity, exercise, healthy diets, reasonable diet supplements) to enable healthier, happier, longer, and more productive lives in the elderly. Finally, at the technological or treatment level, there is a focus on modern technologies to counteract the negative effects of ageing. Researchers are striving to produce drugs that can mimic the effects of 'exercising more, eating less', while other anti-ageing molecules from molecular gerontologists could help to improve 'healthspan' in the elderly. Machine learning, 'Big Data', and other novel technologies can also be used to monitor disease patterns at the population level and may be used to inform policy design in the future. Collectively, synergies across disciplines on policies, geriatric care, drug development, personal awareness, the use of big data, machine learning and personalized medicine will transform China into a country that enables the most for its elderly, maximizing and celebrating their longevity in the coming decades. This is the 2nd edition of the review paper (Fang EF et al., Ageing Re. Rev. 2015).</p>

Abstract:China's young old in their 60s are healthier today than two decades ago. The picture is more complex for the older age group (70 years and above). Although there is a substantial decline in leading disease burdens such as stroke and chronic obstructive pulmonary disease, diseases such as Alzheimer's and other dementia are on the rise. In 2021, China became an aged society when the share of its elderly population (age 65 years and above) exceeded 14%. In China, as in other upper‐middle‐income and high‐income countries, life expectancy gains are increasingly concentrated at older ages with below‐average health and economic activity. Governments worldwide are hence actively pursuing healthy and productive aging. Healthy aging by some measures is taking place. Similar with other upper‐middle‐income countries, China's young old in their 60s are healthier than the same age group two decades ago. The picture is more complex for the older age group (70 years and above). Although there is a substantial decline in leading disease burdens such as stroke and chronic obstructive pulmonary disease, diseases such as Alzheimer's and other dementia are on the rise. To meet the challenge of a rapidly aging population, China's healthy aging agenda should improve its multipronged approach that seeks to promote healthy lifestyles, enhance the healthcare system, create age‐friendly communities, and tackle socioeconomic and health disparities.

Abstract:Women's reproductive, maternal, newborn, child, and adolescent health (RMNCAH) is a cornerstone for the healthy development of the next generation and a driving force for the progress of population and society in the future, especially in this era of population ageing and low fertility in China. In past decades, China's RMNCAH has made remarkable achievements in development goals related to reducing maternal and child mortality. However, as China moves towards a thriving goal—namely, towards ensuring health and wellbeing—the country faces emerging problems and new challenges in RMNCAH. These problems and challenges relate to the rapid and continuous improvement in social and economic conditions along with changes in demographics, lifestyles, the environment, and innovations in medical and therapeutic technologies, as well as an increasing threat of emerging infectious diseases such as COVID-19. The UN Sustainable Development Goals (SDGs) have also extended the focus from maternal and child survival to improvement of health across the life span. This focus is integrated in the visions of both the SDGs and the Healthy China 2030 agenda. The years 2021–30 are crucial in ensuring that China is able to meet the goal of universal RMNCAH coverage, which is embodied by access, quality, and equity. This Lancet RMNCAH Commission aims to review past achievements and lessons, to analyse current problems and challenges, and then to prioritise steps towards the achievements of the SDGs and Healthy China 2030. Abstract:The global population aged 60 and older is likely to grow to historically unprecedented levels. According to the latest estimation, 2 billion people will be aged 60 and over by 2050—a number equivalent to 22% of the world’s population. China’s aging population also continues to grow rapidly; the Chinese population aged 60 and older reached 14.8% by the end of 2013. This percentage is estimated to reach 17.17% by 2020, and may be as high as 30% by 2050. The roots of population aging in China are the same as elsewhere: a low fertility rate, a longer life expectancy, and the effective control of infectious diseases. However, China faces other challenges as well, as it possesses the largest population in the developing world. These challenges are: 1 a low GDP per capita (announced at #80 globally, in 2014 by International Monetary Fund), 2 an economic imbalance between coastal and inland regions, 3 a rapidly increasing ratio of urban to rural population, and 4 an increasing amount of medical expenses for the aging population. According to statistics from the National Health and Family Planning Commission of the People’s Republic of China (NHFPC), almost 50% of people over 60 suffer from chronic diseases; of these, hypertension, diabetes, Alzheimer’s disease, stroke, and coronary disease are the top five. Cancer is also a heavy burden for people and for society. From an economic point of view, the health expenditure for China’s aging population was 2.0456 trillion CNY in 2014, accounting for 3% of the GDP; and medical expenses for seniors accounted for 50% of existing healthcare resources.

Abstract:China is witnessing a profound demographic transformation marked by a swift rise in its elderly population, posing significant challenges to healthcare and retirement systems. This study investigates the multifaceted impact of this demographic shift, evaluating the effectiveness of government policies in mitigating these challenges. Driven by increased life expectancy and declining fertility rates, this demographic transition necessitates strategic policy responses. The central argument revolves around scrutinizing the efficacy of China's policies in addressing healthcare and retirement hurdles arising from this demographic change. The hypothesis posits that governmental initiatives, such as healthcare infrastructure investments, pension system reforms, and expanded healthcare coverage, will partially alleviate existing challenges. Moreover, the paper hypothesizes on the societal impact of these policies, aiming to enhance elderly individuals' quality of life, alleviate healthcare-related financial burdens, and stimulate economic growth while relieving social welfare pressures. Employing a literature analysis methodology, this study critically evaluates existing scholarly works, policy documents, and empirical studies. While acknowledging the present challenges, it anticipates potential disparities in medical resources, sustainability concerns in pension systems, and issues surrounding the social integration of the elderly as the aging population expands. Recommendations underscore the necessity for adaptive policies to sustainably address these challenges. Through a comprehensive approach, this paper analyzes current policies, forecasts their impact, and conjectures future challenges, aiming to provide holistic insights into the effectiveness of policies aimed at managing the intricacies of an aging population in China.

Abstract:China has entered an aging society since 1999. By the end of 2017, the number of persons aged ≥60 years had increased to 240.90 million. At present, the average life expectancy of China's population is 76.4 years, and the healthy life expectancy is around 68.7 years, which means that the elderly will be accompanied by disease for about 8 years. The main health problems of the elderly in China include increased prevalence of chronic diseases, increased prevalence of disability, insufficient attention to older persons' mental health and area specific differences in the health level of the elderly. The disease status of the elderly cannot be ignored. Strengthening the medical services in western and rural areas, emphasizing the psychological and mental concern for the elderly, formulating unified health evaluation criteria, promoting a new model of medical and nursing care combination, establishing an elderly security system with equal responsibilities and rights, jointly managing under the cooperation of whole society and multi-dimensions to protect the health of the elderly are essential to the construction of an active ageing society in China and the realization of the strategic goal of healthy China.

Abstract:Summary Background An ageing population coupled with an increase in morbidity places a considerable burden on health and social care systems. The aim of our study was to estimate the trends in functional dependency and project future care needs for older people in China. Methods We analysed data from the China Health and Retirement Longitudinal Study, a nationally representative survey of a cohort of Chinese people (aged ≥45 years) from 150 counties or districts and 450 villages or urban communities across 28 provinces, who were selected by use of multistage stratified probability-proportionate-to-size sampling. The baseline survey was conducted in 2011 and follow-up surveys were conducted in 2013, 2015, 2018, and 2020. We excluded people younger than 60 years or people who had missing variables on dependency in the five follow-up interviews. Three dependency levels were determined on the basis of activities of daily living (ADLs) and instrumental activities of daily living (IADLs): any ADL items (level 1 dependency); any ADL items or difficulty cooking, shopping, or taking medications (level 2 dependency); and difficulty in any ADL or IADL items (level 3 dependency). The dependency rates were extrapolated to derive the number of people older than 60 years with dependency in China from 2011 to 2020. We used a regression model to project future changes and forecast the size of the older population with dependency between 2021 and 2030. Findings A total of 89 031 individuals across five waves completed the surveys, of whom 46 619 were eligible for inclusion. The prevalence of level 1 dependency among older Chinese adults declined from 11·7% (95% CI 10·6–12·8) in 2011 to 8·1% (7·5–8·7) in 2020. Level 2 and level 3 dependency also declined. The total number of older people requiring care in 2020 was 20·61 million (95% CI 19·01–22·20) with level 1 dependency, 36·33 million (34·27–38·40) with level 2 dependency, and 45·30 million (43·02–47·59) with level 3 dependency. Improved education, housing, and access to health care was associated with 41·84% of the decline in level 3 dependency prevalence between 2011 and 2020. By 2030, the projected dependency rates could decline to 8·04% for level 1 dependency, 13·28% for level 2 dependency, and 16·05% for level 3 dependency. Nonetheless, the cohort size will grow, resulting in more older Chinese people who need care (29·71 million [27·07–32·36] in level 1, 49·07 million [45·98–52·16] in level 2, and 59·32 million [55·94–62·70] in level 3) in 2030. By 2030, we estimate that 14·02 million more older Chinese people will need care than in 2020. Interpretation Rapid ageing of the population could offset the decline in dependency and result in a substantial increase in the population with complex care needs. Promoting healthy ageing and investing in an age-friendly environment are important in reducing care burdens in China. Abstract:Background: China is facing the challenge of an increasingly ageing population. The proportion of people aged 60 and above increased from 10% to 21.1% between 1999 and 2023 and is projected to reach 28% by 2040. In response to this demographic shift, the Chinese central government has implemented a series of policies that have evolved over time. The rapid advancement of information technology and science has accelerated these changes. However, there is still a research gap in understanding the digital service policies for elderly care. Methodology: Based on research literature and policy texts, this study specifically focuses on policy development between January 2019 and January 2024. Seventy traceable national policy texts were categorized and analyzed, gathered from the State Council's policy document database and the official websites of national ministries. Result: The 70 policies were interpreted and categorized into three major aspects. Firstly, the policies on the improvement of traditional elderly care aimed to meet diverse needs by focusing on health and medical care, the management and standards of serving institutions, service improvements and standardization, and the development of the service workforce. Secondly, the policies on system integration between elderly care and other industries included the integration of finance and elderly care, the construction of the insurance system, long-term care institutions and insurance, hospice care, and the silver economy. Thirdly, the policies on digital services comprised the smart healthcare industry, information accessibility, elderly-friendly digital technology and empowerment, and the creation of elderly-friendly communities and societies. Conclusion: The development concepts and contents of China's elderly care policy have evolved. The acceleration of transformation and promotion of the silver economy has become a new comprehensive and universal measure. Despite recent policy developments indicating a growing trend towards integration, there remains considerable room for improvement in actual implementation. This should focus on the uneven development of the "three pillars" of the pension system, insufficient innovation in financial products, and inadequate industry financing channels. It is important to be alert to the persistent problems and obstacles, which widen the gap in elderly welfare, create new forms of social inequality and exclusion, and hinder the development of a friendly information society. Abstract:To the Editor: We read with great interest the article by Flaherty et al.,1 in which they provided an overview of the population demographics and healthcare statistics in the People's Republic of China focusing on the older Chinese population, its current care system, and geriatrics. We appreciate the authors and the Journal for the interest in introducing the largest aging population in world to the American geriatrics community. In addition to congratulating the authors' accomplishment of such an overview, we have the following comments. The figure of 130 million Chinese older adults and subsequent population figures provided in Table 1 of the article are rather conservative. On February 25, 2006, the China National Committee on Aging (CNCA), the most authoritative government agency on aging in China, announced that there were 143 million Chinese people aged 60 and older at that time,2 10% more than the above figure. According to the CNCA, China's aging population has been growing at a rate of 3.28% annually since 2001 and is expected to reach 437 million by 2051.2 This suggests an even greater challenge for the Chinese healthcare system and the need for developing quality geriatrics care for the largest and ever-growing older adult population in the world. Regarding geriatrics development as a medical subspecialty in China, a significant number of major medical centers in large cities have established geriatrics clinical services and research centers or laboratories on aging. There are also several Chinese geriatrics and aging journals, including the Chinese Journal of Geriatrics, a monthly subspecialty medical journal established by the Chinese Medical Association (China's equivalent to the American Medical Association) in 1982.3 However, as Flaherty et al. note, there are no formal geriatric fellowships or national board certifications.1 The quality of the geriatrics departments or programs at different medical institutions is highly variable. Because physicians and researchers in geriatrics and aging research are usually from other subspecialties, their programs mostly focus on specific organ systems rather than geriatric syndromes or comprehensive geriatric care of the whole person in the older adult population. In addition, most geriatric clinical programs or departments provide care for government officials, current or retired, and some of them are established in military hospitals. They usually have stable financial resources and do not need to compete for patients and scarce healthcare dollars. Furthermore, health information and clinical data for government officials and military personnel are usually kept confidential and not allowed in scientific investigation or reporting. All these are potential factors that hinder further development of geriatric medicine as a subspecialty, as well as scientific advancement in aging research in China. In addition to the language barrier, these may also contribute, at least to some extent, to the lack of international collaboration and scientific exchange. Given the sheer number of older adults and their urgent need for quality geriatrics care in China, it is important to think creatively about how to help promote further development of geriatric medicine and aging research in this “aging giant.” The significance not only is apparent in improving healthcare for older Chinese persons, but also lies in the potentially tremendous impact on the economy and society in China as well as around the world. Such efforts have recently started to emerge. For example, an international geriatrics collaborative project between the Johns Hopkins University School of Medicine and Peking Union Medical College (PUMC) Hospital in Beijing was initiated in 2006. Funded by the China Medical Board of New York, a private foundation, this 4-year project aims to establish a geriatric medicine program at PUMC Hospital. Geriatrics training of PUMC Hospital physicians and nurses is provided in the Division of Geriatric Medicine and Gerontology at Johns Hopkins as part of the “training the trainers” program. A geriatrics demonstration ward is now open in the Department of Medicine at PUMC Hospital. With subsequent funding, potentially from the Chinese government, private foundation(s), and other sources, the long-term goal of this project is to establish the PUMC geriatrics program as the flagship to promote geriatrics education, quality care, and aging research throughout China. As described in the article by Flaherty et al., there are many unique characteristics of the Chinese older population, the needs and support of Chinese older adults, and the overall Chinese healthcare system and financing mechanisms. Geriatrics program development in China, as such, should take these unique characteristics into account and be tailored to the Chinese culture and society. It is likely that the American geriatrics community will learn and benefit from such collaborative efforts as well. Abstract:China faces an aging tsunami. By the end of 2016, the number of older adults aged 60 and older reached 230 million. This number is projected to reach 418 million by 2035 and peak at 487 million by 2053. The number of individuals aged 80 and older, the oldest adults, reached 26 million and continues to grow by 1 million a year. The socioeconomic context, characterized by an inverted pyramid family structure and the number of "empty nesters," has greatly compromised traditional Chinese family support for older adults. This article aims to provide an overview of geriatrics development in China. It begins with a brief account of the dramatic demographic shift, major socioeconomic factors, China's healthcare system, and reform related to senior health with a historical perspective. It then describes recent advances in government policy and support and development of a geriatrics physician workforce, care models, primary care networks, and privately owned senior care facilities and support services. Although it is impossible to cover all aspects of the topic, it is hoped that this article provides readers an overall picture of Chinese geriatrics and senior healthcare development in a complex and evolving healthcare system. Geriatrics communities in the United States and around the world will undoubtedly learn and benefit from the unparalleled and continued efforts to address this unprecedented opportunity and challenge in China.

Abstract:Health is the key to the aging problem, and “healthy aging” depicts the overall changing trends in the health of all elderly individuals in a society. Based on the Chinese Longitudinal Healthy Longevity Survey (CLHLS) data from the years 2002, 2005, 2008, 2011 and 2014, this article investigates whether there is a trend of “healthy aging” in China. A second-order factor model including four dimensions of physical health, functional status, mental health and social health was constructed to measure a latent variable, “Health_elders”. The further multigroup comparison results by structural equation modeling showed that, with the exception of 2008, the Health_elders in 2002, 2005, 2011 and 2014 displayed an upward trend, and the mean differences in Health_elders across five periods were significant. These findings indicate that on the whole, compared with older people in the past, older people in more recent periods are healthier, which supports the trend of “healthy aging” in China. In terms of cohorts, the average health levels of male, town-residing elderly populations are higher, while the healthy aging trends among female, rural and urban elderly populations are stronger. Moreover, the physical health levels of the 60–74 years-old cohort are decreasing, and the participation of elderly individuals in social activities is low, which are the weaknesses in the healthy aging process in China.

Abstract:Following the global trend of population aging, China became an aging society at the end of the 20th century. The ever-growing medical demands of the elderly, the lag in medical insurance policy, and the late development of geriatric services make the present situation of public health in China worrying. To meet these challenges, the Beijing municipal government has actively adjusted its development strategies and has been building up a medical service and healthcare system suitable for the elderly. The core of the system is a three-level management of geriatric diseases: prevention and treatment for chronic diseases, functional rehabilitation, long-term care, and family attendance at rural and urban community health service centers (stations); post-acute rehabilitation, long-term care, and palliative treatment in the specialized geriatric hospitals of every district (county); and rescue and treatment for acute and serious geriatric diseases in the geriatric sections of all general hospitals and Beijing Geriatric Hospital. By raising awareness and gaining support from all of society, the implementation of this system will benefit millions of elderly people and promote the sustainable financial development and social harmony of Beijing.

Abstract:The aim of the current study was to review the current state and characteristics of the elderly population in China in the context of aging, difficulties and challenges faced by older people, and efforts of the current Chinese Government in this area. The process of population aging in China began to accelerate in the late 1970s and has continued to increase at a rate of about 3.2% per year since then. This process took more than 45 years in developed countries, while it took only about 27 years in China, and aging may continue to increase for a long time. China is now moving toward a superannuated society due to declining fertility rates and increasing life expectancy. There is a great need for care due to the high disease burden among older people. However, more than 1 million "families have lost their only child", and this number is increasing annually by about 76,000; moreover, there are a large number of "deficient families [with an injured family member]" in China. These families face greater difficulties due to aging and need to rely on society for more support given the lack of care provided by their children or spouses. The current study has focused on improving the quality of life of older people, helping them achieve healthy aging, and to assist the country in further providing care for the elderly.
