Abstract:Background After tooth loss, alveolar bone atrophy and pneumatization of the maxillary sinus occur. It is difficult for implant placement to posterior maxilla because of the reduced residual bone height (RBH) available. Osteotome sinus floor elevation (OSFE) is a reliable technique and produced satisfied results. Compared with oral panoramic radiograph (OPG), the application of cone beam computed tomography (CBCT) that provides clinically valuable information is considered to be more accurate assessment. Aim/Hypothesis The purpose of this study was to compare radiographically the variances of measurements on OPG and CBCT, and to evaluate the effects of endo-sinus new bone formation after OSFE without grafting in a short time. Simultaneously, to analysis the potential related factors of endo-sinus bone gain (ESBG). Material and Methods A prospective study design was adopted. 87 implant placements were inserted in 80 patients. OPG and CBCT were used at preoperative, immediately in postoperative period and before stage II surgery (mean 5-month postoperative). The RBH was measured at preoperative and before stage II surgery, the implant protrusion length (IPL) was assessed at immediately postoperative and before stage II surgery, ESBG was reviewed before stage II surgery. Compare the RBH, IPL, ESBG between two groups from OPG and CBCT. Correlation analysis was conducted to explore the relationship between ESBG with IPL, RBH, age, gender and bone block lifted intraoperatively by osteotomes. Significance was set to 0.05. Results The mean RBH was 8.47 ± 1.78 mm on OPG and 8.08 ± 1.47 mm on CBCT preoperatively. After mean 5 months, it was found the averaged RBH was 9.58 ± 1.67 mm on OPG and 10.59 ± 1.51 mm on CBCT. The mean IPL was 2.44 ± 1.16 mm on OPG and 1.98 ± 1.09 mm on CBCT at immediately postoperative. The averaged IPL was 1.27 ± 1.30 mm on OPG and 0.05 ± 1.33 mm on CBCT before the stage II surgery, respectively. ESBG was 1.10 ± 0.76 mm on OPG and 1.90 ± 1.38 mm on CBCT. And there were all significant differences between two groups (P < 0.05). As shown by the numerical value, ESBG with bone block lifted were more than with no bone block lifted, but P value > 0.05 that indicating the difference was again not statistically significant difference between two groups. The relevant factors analysis detected that the final ESBG was positively correlated to IPL at immediately postoperative. Meanwhile, no other variables were detected showed to be associated with the final ESBG, including RBH, age, gender and bone block lifted. Conclusion and Clinical Implications In conclusion, compared with the OPG of 2D image, the errors of measurement could be reduced and the results could be assessed more accurately on CBCT. Approximate 2 mm endo-sinus bone augment could be acquired, and the final ESBG was explored to be positively correlated to IPL measured at immediately postoperative. Bone block lifted intraoperatively by osteotomes does not increase the amount of endo-sinus bone formation.

Abstract:PURPOSE Using panoramic radiograph and CBCT to compare the measurement errors of the residual bone height in the same site before implant placement in the posterior maxilla and endo-sinus bone gain after osteotome sinus floor elevation without grafting, and to evaluate endo-sinus bone augmentation before stage-two surgery with CBCT. Also, some related factors of new bone formation in the sinus were analyzed, such as small bone block elevated by osteotome at the implant apex intraoperatively. MATERIALS AND METHODS Patients were enrolled in the retrospective study; dental implants were placed in the maxillary posterior region using osteotome sinus floor elevation without grafting. The panoramic radiograph and CBCT were taken preoperatively, immediate postoperatively, and before the stage-two surgery. Endo-sinus bone augmentation was evaluated. A generalized linear model was made to explore the related factors of endo-sinus bone gain. Also, some clinical indexes, such as sinus membrane perforation rate, implant success, and failure criteria were assessed. RESULTS One hundred two dental implants were inserted in 91 patients. Preoperatively, the mean residual bone height was 8.53 ± 1.76 mm and 7.87 ± 1.45 mm measured using a panoramic radiograph and CBCT, respectively, with significant statistical differences (P < .05). Endo-sinus bone gains of 1.31 °æ 1.05 mm by panoramic radiograph and 1.80 ± 1.72 mm by CBCT were observed, with statistically significant differences (P < .05). The final endosinus bone gain was positively correlated with the implant protrusion length at baseline without any other relevant factors, such as elevated small bone block. CONCLUSION Measurement errors could be relatively minimized when using CBCT. Using the osteotome sinus floor elevation technique without grafting, approximately 2 mm of endo-sinus bone could be acquired. The final endo-sinus bone gain was positively correlated with implant protrusion length at baseline. Small bone block elevated intraoperatively did not gain more endo-sinus bone.

Abstract:Introduction: Accurate assessment of osseointegration in dental implants requires precise radiographic visualization of pathologic conditions as well as anatomical structures. The present study aimed to evaluate the formation of bony tissue (osseointegration) using digital orthopantomogram (OPG) and cone beam computed tomography (CBCT) immediately after implant insertion (within 7 days) and 3 months postinsertion. Materials and methods: Twenty single-implant sites on mandi-bular posterior regions were selected on patients irrespective of their gender. Both digital OPG and CBCT were done within a week and again after 3 months of implant insertion surgery, using the same exposure parameters. Results: Three of the 20 implants were submerged and were excluded as the crestal bone height could not be measured. The participants were recalled for radiographic measurements after 3 months of implant placement. On an average, there was 0.03 mm of osseointegration at the apical portion after 3 months of implant insertion on digital OPG; 0.04 mm of osseointegration at the crestal bone height after 3 months on digital OPG; and 0.01 mm of osseointegration at the apical portion after 3 months on CBCT. No change or ≤0.02 mm of osseointegration at crestal bone height after 3 months on CBCT. Conclusion: Both digital OPG and CBCT are significant for the assessment of osseointegration in implants, and hence, endow definite benefit for accurate assessment in terms of the success of the implant placement. Clinical significance: However, CBCT is a better mode of evaluating dental implants but one should keep in mind that radiographic examination must be conducted to the benefit of the patient by application of the lowest achievable dose.
Comparative evaluation of circumferential crestal bone loss after 1 year of implant placement with flapless versus flap surgery using surgical template after immediate loading in the posterior mandibular region using cone-beam computed tomography: A randomized controlled trial

Abstract:Aim: The study was aimed to evaluate circumferential crestal bone level after one year of implant placement with flapless versus flap surgery using surgical template after immediate loading in the posterior mandibular region using CBCT. Setting and design: The study was designed as a Randomized controlled trial. Material and methods: 32 implants were placed in single edentulous spaces in the mandibular posterior region after random allocation into two groups: Flap surgery (Group A) and Flapless surgery (Group B). Virtual implant planning was performed using Blue Sky Bio software, and static CBCT guided 3D printed surgical templates were fabricated for all participants of both the groups. Immediate non-functional temporization was performed. Circumferential crestal bone levels were assessed after surgery and one-year follow-up using CBCT and XELIS software. Vertical bone loss (VBL) and horizontal bone loss (HBL) was assessed on four sides: buccal, lingual, mesial and distal. Statistical analysis used: Data was analyzed using Statistical Package for Social Sciences IBM Corp. Released 2017, IBM SPSS Statistics for Windows, Version 25.0. (Armonk, NY: IBM Corp.) and Graph Pad Prism 7.0 version. The level of significance was chosen <0.05. Chi square test was performed to assess the difference in the age in the two groups. Mann-Whitney U test was performed to compare the two groups for outcome measure. Graphically, quantile-quantile (Q-Q) plot was made using mean and standard deviation for normality verification of data. Results: 100% survival rate and patient compliance was observed along the one-year follow-up duration. By using Mann-Whitney U test, statistically significant difference was found in the vertical bone loss among participants of Flap surgery (Group A) and Flapless surgery (Group B) on all the four sides after one year of implant placement. However, significant results were not obtained for the difference in the horizontal bone level. Conclusion: Within the limitations of this study, vertical bone loss measured circumferentially was more positively correlated with the implants placed with flap surgery compared to flapless surgery after immediate loading in the posterior mandibular region after one year.

Abstract:Fresh, unprocessed bone is ground to sections 75-100 μ thick, stained in an aqueous solution composed of fast green FCF, 0.1 gm; orange G, 2.0 gm; distilled water, 100.0 ml; and adjusted to pH 6.65, then in a mixture of 1 part alcoholic solution of 0.25% celestine blue B and 9 parts of alcoholic solution of 0.1% basic fuchsin. Surface stain is removed by grinding sections to 50 μ and washing them in 1% invert soap (Zephiran) to remove adherent debris. (Commercial detergents and alkaline soaps may interfere with chromophore groups of the dyes.) Wash in tap water; rinse in distilled water and differentiate in 1% acetic alcohol. Dehydrate in ascending alcohols, clear in xylene and mount permanently in a neutral, synthetic resin. Abstract:Rare earth (RE) materials, which are excited in the ultraviolet and emit in the visible light spectrum, are widely used as phosphors for lamps and displays. In the 1960's, researchers reported an abnormal emission phenomenon where photons emitted from a RE element carried more energy than those absorbed, owing to the sequential energy transfer between two RE ions--Yb(3+)-sensitized Er(3+) or Tm(3+)--in the solid state. After further study, researchers named this abnormal emission phenomenon upconversion (UC) emission. More recent approaches take advantage of solution-based synthesis, which allows creation of homogenous RE nanoparticles (NPs) with controlled size and structure that are capable of UC emission. Such nanoparticles are useful for many applications, especially in biology. For these applications, researchers seek small NPs with high upconversion emission intensity. These UCNPs have the potential to have multicolor and tunable emissions via various activators. A vast potential for future development remains by developing molecular antennas and energy transfer within RE ions. We expect UCNPs with optimized spectra behavior to meet the increasing demand of potential applications in bioimaging, biological detection, and light conversion. This Account focuses on efforts to control the size and modulate the spectra of UCNPs. We first review efforts in size control. One method is careful control of the synthesis conditions to manipulate particle nucleation and growth, but more recently researchers have learned that the doping conditions can affect the size of UCNPs. In addition, constructing homogeneous core/shell structures can control nanoparticle size by adjusting the shell thickness. After reviewing size control, we consider how diverse applications impose different requirements on excitation and/or emission photons and review recent developments on tuning of UC spectral profiles, especially the extension of excitation/emission wavelengths and the adjustment and purification of emission colors. We describe strategies that employ various dopants and others that build rationally designed nanostructures and nanocomposites to meet these goals. As the understanding of the energy transfer in the UC process has improved, core/shell structures have been proved useful for simultaneous tuning of excitation and emission wavelengths. Finally, we present a number of typical examples to highlight the upconverted emission in various applications, including imaging, detection, and sensing. We believe that with deeper understanding of emission phenomena and the ability to tune spectral profiles, UCNPs could play an important role in light conversion studies and applications.
Cone-beam CT evaluation of alveolar ridge modification subject to immediate implant with immediate loading and delayed loading

Abstract:Objective To assess and compare the radiographic outcomes of immediate implant placement with two different loading timing. Methods The involved cases were collected from Department of Implantology, Peking University School and Hospital of Stomatology. Patients recieved immediate implant placement during 2010 Aug to 2012 Dec. The cases admitted delayed restoration program were assigned to Group Ⅰ(18 patients with 21 implants), the cases admitted immediate restoration program were assigned to Group Ⅱ(16 patients with 22 implants). Cone-beam CT(CBCT) was taken on the day after surgery and 6 months later. The thickness of the alveolar and the vertical alteration of marginal bone level were measured using the computer software subject to the CBCT. Wound closure was performed by single sutures in Group Ⅰ. Provisional crown was delivered on the same day of implant insertion or the day after for Group Ⅱ. Comparisons between baseline data and data from 6-month follow-up in both groups were conducted using paired t test(α =0.05). Results The success rates of two groups were both 100% . The thickness got alteration after 6 months. The largest alteration position was at the 6 mm apical to the implant platform in Group Ⅰ, which was (-0.75±1.31) mm. In Group Ⅱ for each platform got similar alterations as -0.3 mm. There was no significant differences. Vertical alteration in two Groups showed significant difference, with (- 0.73 ± 1.88) mm(Group Ⅰ) and (0.39 ± 1.37) mm(Group Ⅱ) respectively. Conclusions Immediate restoration of dental implants which are placed in fresh extraction sockets has more advantages than delayed restoration protocol, for it shortens the course of treatment and preserves the alveolar bone.

Abstract:The aim of the study was the evaluation of plasma osteocalcin (O.), alkaline phosphatases (A.P.) and urinary hydroxyproline excretion (uOHPr) in subjects 3-20 years aged. O., A.F. and uOHPr were increased in patients less than 15 years aged; thereafter the values were similar to normal adult values. O. correlates with age and body surface area in children aged less than 15 years; the highest values were observed at the expected age of puberal spurt. This behaviour was not evident for the other bone turnover markers studied. In conclusion, it could be suggested that O. is a useful index of bone peak mass reached at puberty.

Abstract:Identifying landscape elements that influence gene flow and migration in wild species is the current main topic of landscape genetics. Most landscape genetic studies infer gene flow and migration from genetic distances among populations or individuals and statistically relate these measurements to landscape composition and configuration. This approach assumes symmetrical gene flow between pairs of populations. Such an assumption, however, will often be violated, especially in source–sink systems. Source populations provide more emigrants than they receive immigrants, and sink populations get many immigrants, but release few emigrants. Source–sink dynamics cannot be explored using common landscape genetic approaches relying on genetic distances. In this issue of Molecular Ecology, Andreasen et al. ( ) apply an alternative approach allowing them to infer asymmetrical migration. They use a Bayesian assignment test among objectively defined populations of mountain lions (Puma concolor) in western USA to estimate recent and directional migration rates. The study shows that an area with a high amount of wildlife refuges and low hunting pressure harbours a source population for mountain lion dispersal, while areas with high hunting pressures form sink populations; a result helpful in making informed decisions in conservation management.

Abstract:OBJECTIVE To evaluate the impact of a voluntary termination of pregnancy (TOP) on contraceptive practice and sexual well-being in women. STUDY DESIGN AND METHODS Comparative evaluation of a retrospective study conducted in Lugano in 1995 with 85 women interviewed 3-6 months post-TOP, and a prospective study carried out in Geneva in 1998 with 103 women interviewed pre-TOP and 6 months post-TOP. In both studies TOP was performed during the first 12 weeks of pregnancy by vacuum aspiration and curettage. RESULTS Oral contraceptive use in the Lugano group increased from 21% pre-TOP to 47% post-TOP, and in the Geneva group from 19% pre-TOP to 69% post-TOP. Condom use decreased in both groups. Those practising unprotected intercourse also decreased by half (around one in five). No effect on sexual well-being was reported by almost two-thirds of both groups. CONCLUSIONS Despite large differences between these two populations, the two studies show a similar evolution. The majority of women appeared to be able to cope well with a TOP. About one third presented sexual dysfunction.

A new case of Finnish-type congenital nephrotic syndrome, neuromuscular symptoms and early death

Abstract:Sir, The children affected by Finnish-type congenital nephrotic syndrome (FCNS) frequently show muscular hypotonia and neurological development retardation, which usually ameliorate after kidney transplantation [1,2,3,4]. However, Laakkonen H et al. recently described six FCNS patients with a severe dyskinetic cerebral palsy-like syndrome with dystonic features, athetosis and a hearing defect. Four of the patients died between 1 and 4 years of age in different circumstances; the others survived with severe neurological deficits. In spite of normal metabolic investigations, Laakkonen et al. hypothesize that the complex neurological picture was due to mitochondrial dysfunction [5]. We observed a child with FCNS and similar dyskinetic symptoms who died suddenly when he was 4.5 years. He was born at 34 weeks; elevated α-feto-protein was detected in maternal serum (1100 ng/ml) and amniotic fluid; the placenta was 1200 g with a placenta weight/newborn weight ratio of 0.44. Neonatal hyperbilirubinaemia, max 5.5 mg/dl (5.3 mg/dl non-conjugated and 0.2 mg/dl conjugated), was observed. At the age of 3 weeks, secondary hypothyroidism (T4 0.63 ng/dl and TSH 13.1 μU/ml; nv: T4 0.8–1.9 ng/dl, TSH 0.4–4 μU/ml) was diagnosed; thyroxine 37.5 μg/day normalized thyroid hormones (T4 1.2 ng/dl and TSH 3.4 μU/ml). Laboratory parameters also showed heavy proteinuria (>300 mg/Kg/day) and severe hypoalbuminaemia (0.78 g/dl). The clinical diagnosis of FCNS was later confirmed by molecular study for NPHS1 locus. At the age of 3 the patient developed ESRF and began peritoneal dialysis. When he was 4 years, in spite of a satisfactory cognitive and emotional development, the child showed severe neuromuscular deficit with focal hand dystonia and athetosis, inability to keep his head and trunk upright, or walk. He had no hearing defects. No metabolic investigations were performed. At the age of 4.4, after 6 months of treatment with growth hormone, the child weighed 14 kg (10°C) and was 96 cm tall (10°C). During a febrile illness, he manifested apnoea, followed by cardiac arrest, prolonged brain ischaemia and irreversible coma. An MRI performed in the first 24 h showed, besides brain atrophy, T2 hyperintensity on basal ganglia. Diffusion-weighted images showed an extensive signal alteration indicating water diffusion restriction in the lenticulostriate nuclei and the caudate nuclei head with low ADC values (4.1 ± 0.15 SD) in the posterior portion of the lenticulostriate nuclei. The anterior portions of lenticulostriate nuclei showed higher ADC values (7.3 ± 0.43 SD) in the same regions of hyperintensity on T2 weighted images (Figure (Figure1).1). Therefore the hyperintensity might be related to abnormalities of metabolic mechanisms existing before the hypoxic injury [6]. The child died after 2 months of coma. Axial apparent diffusion coefficient map shows a reduced ADC value (0.41) in posterior putamen and globus pallidus, and a high ADC value in anterior putamen and head of caudate ... Like the cases described by Laakkonen H et al., we can exclude prenatal or neonatal complications causing the neurological impairment in our patient. However, our case showed several different features, such as the more severe muscular hypotonia, adequate growth, growth hormone therapy and normal hearing. In spite of these differences, the MRI findings and the unexplained sudden event causing early death strongly suggest that our patient may have been suffering from the new clinical entity that Laakkonen et al. call NPHS1 with muscular dystonia and athetosis.
Evaluation of Maxillary Alveolar Ridge Morphology and Residual Bone for Implant Placement by Cone Beam Computed Tomography (CBCT)

Abstract:The aim of this study was to assess the morphology and dimension of maxillary alveolar ridge at anterior and posterior regions using cone beam computed tomography (CBCT) prior to the placement of immediate implants. CBCT scans of 192 patients were evaluated. Measurements were taken at the cross sectional views in the maxillary central incisor, lateral incisor, canine incisor, first and second premolars and first and second molars regions. Alveolar ridge width and height, buccal plate thickness, buccal undercut, inter-root septum height, and root penetration to sinus floor were measured. Independent T-test was used for statistical analysis. The mean value of alveolar height in the maxillary incisors were higher than the posterior teeth with central incisors having the highest value of 19.23 ± 8.74 mm. At anterior maxilla, the lateral incisor with the mean value of 1.07 ± 0.64 mm exhibits the highest buccal undercut and the thinnest ridge width. At the site of posterior regions premolars had higher buccal undercut than molars. Canine with the mean value of 0.19 ± 0.31 mm thickness at the mid-alveolar of buccal plate, had the thinnest plate among other maxillary incisors. Sinus penetration and inter-root septum height were higher in premolar teeth than molars. The mean value of ridge width at all levels was significantly different between men and women, except at the level of crest in central incisor. CBCT evaluation presents accurate measurements for ridge assessment prior to immediate implant insertion. The recommended appropriate size of implant platform would be 5 mm in the incisor and almost 6 mm in premolar region.

Abstract:Objective: During the immediate dental implant (IMI) procedure in the mandibular posterior region, some limitations caused by anatomical structures that may affect the success of implant treatment and increase the risk of complications may be encountered. Socket size, distance from root apices to the inferior alveolar canal (IAC), and lingual concavity are some of the critical conditions. This study aimed to examine the alveolar bone of mandibular first molars using cone beam computed tomography (CBCT) and to evaluate its prevalence on a subpopulation basis. Material and Methods: A total of 153 mandibular first molar teeth in 100 patients who met the evaluation criteria were evaluated for cross-sectional classification of the alveolar bone, distance from root apices to IAC, and socket size using CBCT scans. Results: In this study, which included 42 females and 58 males, the age range was 19-70 years (38.13±13.74 years). Of the 153 mandibular first molars analyzed, 53.6% were on the left, while 46.4% were on the right. The distances from the apices of the roots to the IAC were the least in females (p<0.05) and U-type ridges. It was also found that this distance was positively correlated with age. The mean crestal socket width measured in the current study was suitable for choosing a dental implant with an appropriate diameter for IMI surgery. Conclusion: Cross-sectional analysis of the relevant regions before surgery is important for IMI placement. This will allow clinicians to take precautions against possible complications.
Accuracy of peri-implant bone evaluation using cone beam CT, digital intra-oral radiographs and histology

Abstract:Objectives: The present study assesses the accuracy of three-dimensional (3D) cone beam CT (CBCT) and intra-oral radiography (CR) in visualizing peri-implant bone compared with histology. Methods: 26 titanium dental implants were placed in dog jaws with chronic type vestibular defects. After a healing period of 2 and 8 weeks (n = 12 dogs) the animals were sacrificed. CBCT scans and CR of the specimen were recorded. Dissected blocks were prepared, and histomorphometric analysis was performed. Both modalities were measured twice by two observers and compared with histomorphometry regarding bone levels and thickness around implants as well as length and diameter of implants. Results: Measurements of CBCT correlated well with histomorphometry of the vestibular bone level, oral bone thickness and implant length (all p-values <0.05). Compared with histomorphometry, the mean differences between CBCT and histomorphometry were between 0.06 and 2.61 mm. Mesial bone level (MBL) and distal bone level (DBL) were underestimated by both CR and CBCT. CR and histology measurements were only significantly correlated for implant length measurements. All intraclass correlations were highly significant. Conclusions: 3D CBCT provides usable information about bone in all dimensions around implants with varying accuracy. CR and CBCT perform similar in assessing MBL and DBL, but, within its limits, the CBCT can assess oral and buccal bone. Metallic artefacts limit the visualization quality of bone around implants and further research could elucidate the value of post-processing algorithms. When information about osseous perforation of implants is needed, CBCT may still provide clinically valuable information.

Immediate implant placement in the posterior mandible: A cone beam computed tomography study

Abstract:Objective: To determine the longest upright and tilted implant that can be placed in posterior mandibular teeth (second premolars [2PM], first molars [1M], and second molars [2M]) in case of immediate implant placement (IIP), respecting a safe distance from the inferior alveolar canal (IAC) and the submandibular fossa, using cone beam computed tomography (CBCT) scans. Method and materials: In CBCT scans of 118 subjects, the following were measured: distance from the tooth apex to the IAC (DTC), lateromedial width of the mandible, classification of mandibular cross-sectional morphology, upright and tilted implant placement simulation, tilting angle that allowed placement of the longest implant. Results: The DTC has shown to be greater for 1M, shorter for 2PM, and even shorter for 2M. Between 70% and 88% of all 2PM, 1M, and 2M present less than 6 mm of DTC. The cross-sectional morphology of the mandible had a tendency to be classified as "undercut" in more posterior regions. For both 4.0- and 5.0-mm implants, more posterior regions resulted in a greater difference in length between the upright and tilted implants. The tilting angle was greater for the more posterior regions. Most of the relationships between age/sex and the morphometric parameters were weak. Conclusion: Lingual concavity is a common clinical finding at the posterior mandibular region, and its occurrence varies according to the tooth type. The great majority of the 2PM, 1M, and 2M present limited available bone between the root apex and the IAC, which stresses the need for careful attention when IIP is planned in this area.

Abstract:To assess the accuracy of CBCT in implant-supported prostheses and to evaluate metal artifacts with and without implants or implant prostheses. Accuracy and artifacts were assessed in the dried mandible at three points on the buccal and lingual cortical plates on the mandible's body near the crest and the base. On the buccal cortical plate, these points were labelled as A, B and C near the crest and D, E and F near the base of the body of the mandible. Similarly, points a to f were marked on the lingual cortical plate corresponding to points A to F. The study had two control groups, C0 for physical linear measurement (PLM) and C1 for radiographic linear measurement (RLM) and artifact assessment. There were seven test groups, TG 1 to 7, progressing from a single implant to implant full-arch prosthesis. For accuracy assessment, PLM was compared to RLM. CBCT artifacts were investigated in images integrated at 0.25 mm, 10 mm, and 20 mm at regions of interest on concentric circles at different intersecting angles by comparing grayscale values at C1 and TG1 to 7. The data were collected and statistically analyzed. A significant difference was observed between C0 and C1, and RLM in test groups at the superior axial plane. Similarly, PLM and test RLM in the sagittal plane at A-B, B-C, and D-E were statistically significant. A significant difference between PLM and RLM was also observed in the vertical plane at A-D, B-E, and C-F. Quantification of CBCT artifacts in the presence of implants or prostheses revealed that full-arch prostheses had the highest mean grayscale value, whereas single implants with a prosthesis had the lowest. The mean grayscale change was greatest around the implant and implant prosthesis. The mean grayscale value was maximum at 20 mm voxel integration scales (VIS) and lowest at 0.25 mm. CBCT is a clinically reliable device. Metal in implants or implant-supported prostheses prevents true assessment of the peri-implant area; therefore, lower VIS is suggested in the presence of implants or implant prostheses.

Abstract:Aim: The objective of this study was to assess marginal bone level around single implants inserted in fresh extraction sockets in the anterior maxillary region and instantly restored with computer-aided design/computer-aided manufacturing customized temporary crowns cemented on the final abutment. Materials and methods: A total of 20 patients (15 females and 5 males, with a mean age of 30 years), where 20 were placed in fresh extraction sockets. After raising a full-thickness flap, atraumatic extraction was performed the implant site was prepared and fixtures were stabilized on the palatal bone wall. The implant location was immediately transmitted to the prepared master model using the pick-up impression coping seated in the surgical guide template. Prefabricated abutments were used as the final abutment on the master model, scanned and the crown was planned using computer-aided manufacturing customized software. Later on 8th weeks, abutments were torqued as per the manufacturer's recommendation, and the final crowns were cemented. Using personalized intraoral radiographs marginal bone level was evaluated mesially and distally to the implant shoulder as a reference at implant placement, 8 weeks, 1, 3, 5, and 10 years after loading. Results: Wholly implants were osteo-integrated positively after 10 years of practical loading, but only 18 were available for clinical and radiological follow-up, and 2 patients with two implants were excluded from the study due to relocation abroad without any implant failure. The average marginal bone loss (MBL) in the current report was 0.16 ± 0.167 mm at crown cementation, 0.275 ± 0.171 mm after 1 year, 0.265 ± 0.171 mm after 3 years, 0.213 ± 0.185 mm after 5 years, and 0.217 ± 0.194 mm at 10 years. Conclusion: The strategy of inserting and not removing the final abutment at the time of implant placement facilitates the establishment of adequate attachment of both soft and hard tissues to the abutment surface, ensuring uninterrupted organization of tissue architecture and offers advantages in helping maintain soft tissue maturation and preventing marginal bone level. Clinical significance: Immediately loaded implants in freshly extracted sockets lead to a significant reduction in marginal ridge resorption. The use of a temporary crown on a prefabricated abutment, exclusive of successive abutment manipulation, proved effective in preserving the primarily founding blood clot and served as a prototype for shaping the soft tissue around the previously wounded gum.
