Abstract:Background After tooth loss, alveolar bone atrophy and pneumatization of the maxillary sinus occur. It is difficult for implant placement to posterior maxilla because of the reduced residual bone height (RBH) available. Osteotome sinus floor elevation (OSFE) is a reliable technique and produced satisfied results. Compared with oral panoramic radiograph (OPG), the application of cone beam computed tomography (CBCT) that provides clinically valuable information is considered to be more accurate assessment. Aim/Hypothesis The purpose of this study was to compare radiographically the variances of measurements on OPG and CBCT, and to evaluate the effects of endo-sinus new bone formation after OSFE without grafting in a short time. Simultaneously, to analysis the potential related factors of endo-sinus bone gain (ESBG). Material and Methods A prospective study design was adopted. 87 implant placements were inserted in 80 patients. OPG and CBCT were used at preoperative, immediately in postoperative period and before stage II surgery (mean 5-month postoperative). The RBH was measured at preoperative and before stage II surgery, the implant protrusion length (IPL) was assessed at immediately postoperative and before stage II surgery, ESBG was reviewed before stage II surgery. Compare the RBH, IPL, ESBG between two groups from OPG and CBCT. Correlation analysis was conducted to explore the relationship between ESBG with IPL, RBH, age, gender and bone block lifted intraoperatively by osteotomes. Significance was set to 0.05. Results The mean RBH was 8.47 ± 1.78 mm on OPG and 8.08 ± 1.47 mm on CBCT preoperatively. After mean 5 months, it was found the averaged RBH was 9.58 ± 1.67 mm on OPG and 10.59 ± 1.51 mm on CBCT. The mean IPL was 2.44 ± 1.16 mm on OPG and 1.98 ± 1.09 mm on CBCT at immediately postoperative. The averaged IPL was 1.27 ± 1.30 mm on OPG and 0.05 ± 1.33 mm on CBCT before the stage II surgery, respectively. ESBG was 1.10 ± 0.76 mm on OPG and 1.90 ± 1.38 mm on CBCT. And there were all significant differences between two groups (P < 0.05). As shown by the numerical value, ESBG with bone block lifted were more than with no bone block lifted, but P value > 0.05 that indicating the difference was again not statistically significant difference between two groups. The relevant factors analysis detected that the final ESBG was positively correlated to IPL at immediately postoperative. Meanwhile, no other variables were detected showed to be associated with the final ESBG, including RBH, age, gender and bone block lifted. Conclusion and Clinical Implications In conclusion, compared with the OPG of 2D image, the errors of measurement could be reduced and the results could be assessed more accurately on CBCT. Approximate 2 mm endo-sinus bone augment could be acquired, and the final ESBG was explored to be positively correlated to IPL measured at immediately postoperative. Bone block lifted intraoperatively by osteotomes does not increase the amount of endo-sinus bone formation.
Effect of Initial Biologic Width on Crestal Bone Loss‐ a Clinical Retrospective Study of 1–5 Years

Abstract:Background Limiting the marginal bone loss is a prerequisite for long-term stability of implants. Previous studies have evaluated the factors affecting marginal bone loss, including the mucous thickness, the depth of placement. The biologic width will form when the implants are uncovered, which unvaryingly exposes the bone around the implant neck to the oral cavity and starts bone loss, and the biologic width is formed by dissected mucous and the depth of placement of implant at initial stage. Aim/Hypothesis The purpose of this study was to evaluate the short-term effect of the mucous thickness, the depth of placement and the initial biologic width on the marginal bone loss. Material and Methods The clinical and radiographic data of 181 delayed-loaded Ankylos Implants in 140 patients were collected after 1–5 years follow-up. The radiographic examination was taken after implant placement, before the second stage surgery, and after 1–5 years follow-up. The mucous thickness, the depth of placement and crestal bone loss were calculated medially and distally. The initial biologic width was defined as either the thickness of the mucous when the implant was positioned supracrestally, or the sum of the thickness of the mucous and the depth of the implant when the implant was placed below the bone level. Implants were divided into 3 groups based on the initial biologic width, Group A (width less than 3 mm, include 3 mm), Group B (between 3 mm and 4 mm, include 4 mm), and Group C (width more than 4 mm). Significance was set to 0.05. Results Implants in Group A had 0.4(0.1.0) mm bone loss mesially and 0.3(0.0.8) mm distally. For Group B, the bone loss values were m- 0.3(0.0.8) mm and d- 0.3(0.0.7) mm. Group C implants experienced bone loss of m- 0.7(0.2.1.1) mm and d- 0.6(0.1.1.0) mm. Kruskal-Wallis test was applied to find differences between groups. There was significant difference between groups (P = 0.011). Kruskal-Wallis test for implants placed below, at, or above the bone level yielded a statistically significant difference (P = 0.000). The calculation of Spearman's correlation coefficient between the mucous thickness and the marginal bone loss shows negative correlation (rs=−0.243, P = 0.001), while the depth of placement and the marginal bone loss show the positive correlation (rs=0.477, P = 0.000). Conclusions and Clinical Implications The crestal bone loss was found to be the positively correlated to the depth of placement and had negative correlated to the mucous thickness. The result may suggest that the closer the initial biologic width is to the final biologic width, the less bone loss is, and if the initial biologic width is not adequate, bone loss may occur to ensure the proper development of biologic width.

Abstract:Fresh, unprocessed bone is ground to sections 75-100 μ thick, stained in an aqueous solution composed of fast green FCF, 0.1 gm; orange G, 2.0 gm; distilled water, 100.0 ml; and adjusted to pH 6.65, then in a mixture of 1 part alcoholic solution of 0.25% celestine blue B and 9 parts of alcoholic solution of 0.1% basic fuchsin. Surface stain is removed by grinding sections to 50 μ and washing them in 1% invert soap (Zephiran) to remove adherent debris. (Commercial detergents and alkaline soaps may interfere with chromophore groups of the dyes.) Wash in tap water; rinse in distilled water and differentiate in 1% acetic alcohol. Abstract:The aim of the study was the evaluation of plasma osteocalcin (O.), alkaline phosphatases (A.P.) and urinary hydroxyproline excretion (uOHPr) in subjects 3-20 years aged. O., A.F. and uOHPr were increased in patients less than 15 years aged; thereafter the values were similar to normal adult values. O. correlates with age and body surface area in children aged less than 15 years; the highest values were observed at the expected age of puberal spurt. This behaviour was not evident for the other bone turnover markers studied. In conclusion, it could be suggested that O. is a useful index of bone peak mass reached at puberty.

Abstract:Aim: The study was aimed to evaluate circumferential crestal bone level after one year of implant placement with flapless versus flap surgery using surgical template after immediate loading in the posterior mandibular region using CBCT. Setting and design: The study was designed as a Randomized controlled trial. Material and methods: 32 implants were placed in single edentulous spaces in the mandibular posterior region after random allocation into two groups: Flap surgery (Group A) and Flapless surgery (Group B). Virtual implant planning was performed using Blue Sky Bio software, and static CBCT guided 3D printed surgical templates were fabricated for all participants of both the groups. Immediate non-functional temporization was performed. Circumferential crestal bone levels were assessed after surgery and one-year follow-up using CBCT and XELIS software. Vertical bone loss (VBL) and horizontal bone loss (HBL) was assessed on four sides: buccal, lingual, mesial and distal. Statistical analysis used: Data was analyzed using Statistical Package for Social Sciences IBM Corp. Released 2017, IBM SPSS Statistics for Windows, Version 25.0. (Armonk, NY: IBM Corp.) and Graph Pad Prism 7.0 version. The level of significance was chosen <0.05. Chi square test was performed to assess the difference in the age in the two groups. Mann-Whitney U test was performed to compare the two groups for outcome measure. Graphically, quantile-quantile (Q-Q) plot was made using mean and standard deviation for normality verification of data. Results: 100% survival rate and patient compliance was observed along the one-year follow-up duration. By using Mann-Whitney U test, statistically significant difference was found in the vertical bone loss among participants of Flap surgery (Group A) and Flapless surgery (Group B) on all the four sides after one year of implant placement. However, significant results were not obtained for the difference in the horizontal bone level. Conclusion: Within the limitations of this study, vertical bone loss measured circumferentially was more positively correlated with the implants placed with flap surgery compared to flapless surgery after immediate loading in the posterior mandibular region after one year.
Assessment of Bone Loss Around Platform-Switching Subcrestal Implants: A Quasi-Experimental Study

Abstract:Introduction: Replacement of lost teeth with implants is a well-known and accepted worldwide treatment. A healthy amount of bone surrounding the implant plays a vital role in osseointegration and is required for implant success. This study aimed to evaluate the efficacy of single-crown rehabilitation of subcrestal implants in terms of bone loss (BL) and overall success. Materials and methods: Twenty healthy patients requiring implant placement to replace single hopeless teeth or extracted teeth were recruited for this prospective study. In these 20 patients, the implants were placed at the subcrestal level. Bone levels around the implant placement (T0) and one year after loading (T1) were estimated in this study. A paired t-test was used for intra-group comparisons, multivariate linear regression analysis was conducted to analyze the effect of independent variables on BL, and a correlation test was used to correlate various variables. Statistical significance was maintained at a p value of 0.05. Results: The outcomes demonstrated statistically significant BL at the mesial and distal sides of the subcrestal implants at one-year follow-up (p<0.05). Age and sex were not significantly correlated with BL in any region (p>0.05). Brushing frequency, probing depth (PD), and bleeding index (BI) showed statistically significant effects on BL (p<0.05). A weak correlation was observed between age and other variables, with age and mesial BL showing a low correlation of 0.016. Conclusion: Within the parameters of this prospective study, it could be proposed that subcrestal implants caused significant BL. The PD, BI, and brushing frequency were significant predictors of BL.
Immediate Loading Using the Digitalized Customized Restoration of Single-tooth Implants Placed in Fresh Extraction Sockets in the Aesthetic Anterior Maxilla: A 10-Year Prospective Study of Marginal Bone Level

Abstract:Aim: The objective of this study was to assess marginal bone level around single implants inserted in fresh extraction sockets in the anterior maxillary region and instantly restored with computer-aided design/computer-aided manufacturing customized temporary crowns cemented on the final abutment. Materials and methods: A total of 20 patients (15 females and 5 males, with a mean age of 30 years), where 20 were placed in fresh extraction sockets. After raising a full-thickness flap, atraumatic extraction was performed the implant site was prepared and fixtures were stabilized on the palatal bone wall. The implant location was immediately transmitted to the prepared master model using the pick-up impression coping seated in the surgical guide template. Prefabricated abutments were used as the final abutment on the master model, scanned and the crown was planned using computer-aided manufacturing customized software. Later on 8th weeks, abutments were torqued as per the manufacturer's recommendation, and the final crowns were cemented. Using personalized intraoral radiographs marginal bone level was evaluated mesially and distally to the implant shoulder as a reference at implant placement, 8 weeks, 1, 3, 5, and 10 years after loading. Results: Wholly implants were osteo-integrated positively after 10 years of practical loading, but only 18 were available for clinical and radiological follow-up, and 2 patients with two implants were excluded from the study due to relocation abroad without any implant failure. The average marginal bone loss (MBL) in the current report was 0.16 ± 0.167 mm at crown cementation, 0.275 ± 0.171 mm after 1 year, 0.265 ± 0.171 mm after 3 years, 0.213 ± 0.185 mm after 5 years, and 0.217 ± 0.194 mm at 10 years. Conclusion: The strategy of inserting and not removing the final abutment at the time of implant placement facilitates the establishment of adequate attachment of both soft and hard tissues to the abutment surface, ensuring uninterrupted organization of tissue architecture and offers advantages in helping maintain soft tissue maturation and preventing marginal bone level. Clinical significance: Immediately loaded implants in freshly extracted sockets lead to a significant reduction in marginal ridge resorption. The use of a temporary crown on a prefabricated abutment, exclusive of successive abutment manipulation, proved effective in preserving the primarily founding blood clot and served as a prototype for shaping the soft tissue around the previously wounded gum. Evaluation of Osseointegration in Implants using Digital Orthopantomogram and Cone Beam Computed Tomography

Abstract:Introduction: Accurate assessment of osseointegration in dental implants requires precise radiographic visualization of pathologic conditions as well as anatomical structures. The present study aimed to evaluate the formation of bony tissue (osseointegration) using digital orthopantomogram (OPG) and cone beam computed tomography (CBCT) immediately after implant insertion (within 7 days) and 3 months postinsertion. Materials and methods: Twenty single-implant sites on mandi-bular posterior regions were selected on patients irrespective of their gender. Both digital OPG and CBCT were done within a week and again after 3 months of implant insertion surgery, using the same exposure parameters. Results: Three of the 20 implants were submerged and were excluded as the crestal bone height could not be measured. The participants were recalled for radiographic measurements after 3 months of implant placement. On an average, there was 0.03 mm of osseointegration at the apical portion after 3 months of implant insertion on digital OPG; 0.04 mm of osseointegration at the crestal bone height after 3 months on digital OPG; and 0.01 mm of osseointegration at the apical portion after 3 months on CBCT. No change or ≤0.02 mm of osseointegration at crestal bone height after 3 months on CBCT. Conclusion: Both digital OPG and CBCT are significant for the assessment of osseointegration in implants, and hence, endow definite benefit for accurate assessment in terms of the success of the implant placement. Clinical significance: However, CBCT is a better mode of evaluating dental implants but one should keep in mind that radiographic examination must be conducted to the benefit of the patient by application of the lowest achievable dose.
Cone-beam CT evaluation of alveolar ridge modification subject to immediate implant with immediate loading and delayed loading

Abstract:Objective To assess and compare the radiographic outcomes of immediate implant placement with two different loading timing. Methods The involved cases were collected from Department of Implantology, Peking University School and Hospital of Stomatology. Patients recieved immediate implant placement during 2010 Aug to 2012 Dec. The cases admitted delayed restoration program were assigned to Group Ⅰ(18 patients with 21 implants), the cases admitted immediate restoration program were assigned to Group Ⅱ(16 patients with 22 implants). Cone-beam CT(CBCT) was taken on the day after surgery and 6 months later. The thickness of the alveolar and the vertical alteration of marginal bone level were measured using the computer software subject to the CBCT. Wound closure was performed by single sutures in Group Ⅰ. Provisional crown was delivered on the same day of implant insertion or the day after for Group Ⅱ. Comparisons between baseline data and data from 6-month follow-up in both groups were conducted using paired t test(α =0.05). Results The success rates of two groups were both 100% . The thickness got alteration after 6 months. The largest alteration position was at the 6 mm apical to the implant platform in Group Ⅰ, which was (-0.75±1.31) mm. In Group Ⅱ for each platform got similar alterations as -0.3 mm. There was no significant differences. Vertical alteration in two Groups showed significant difference, with (- 0.73 ± 1.88) mm(Group Ⅰ) and (0.39 ± 1.37) mm(Group Ⅱ) respectively. Conclusions Immediate restoration of dental implants which are placed in fresh extraction sockets has more advantages than delayed restoration protocol, for it shortens the course of treatment and preserves the alveolar bone.
Effect of different implant placement depths on crestal bone levels and soft tissue behavior: a randomized clinical trial

Abstract:ObjectivesThis randomized clinical trial analyzed crestal bone changes and soft tissue dimensions surrounding implants with an internal tapered connection placed in the mandible anterior region at different depths (equicrestal and subcrestal).Materials and methodsEleven edentulous patients (five implants per patient) were randomly divided in a split-mouth design: G1, 28 equicrestal implants; and G2, 27 subcrestal implants. All implants were immediately loaded. Correlation between keratinized tissue width (KTW) and vertical mucosa thickness (MT) with soft tissue recession was analyzed. Intraoral radiographs were used to evaluate crestal bone changes. Patients were assessed immediately, 4-, and 8-months after implant placement. Rank-based ANOVA-type statistical test was used for comparison between groups (α = 0.05).ResultsFifty-five implants (G1 = 28 and G2 = 27) were assessed in 11 patients. Implant survival rate was 100% for both groups. Both tested implant placement depths presented similar crestal bone loss (P > 0.05). Significant crestal bone loss for each group was found in the different measurement times (T4 and T8) (P 0.05).ConclusionsDifferent implant placement depths do not influence crestal bone changes. Soft tissue behavior is not influenced by different implant placement depths or by the amount of keratinized tissue.

Abstract:Rare earth (RE) materials, which are excited in the ultraviolet and emit in the visible light spectrum, are widely used as phosphors for lamps and displays. In the 1960's, researchers reported an abnormal emission phenomenon where photons emitted from a RE element carried more energy than those absorbed, owing to the sequential energy transfer between two RE ions--Yb(3+)-sensitized Er(3+) or Tm(3+)--in the solid state. After further study, researchers named this abnormal emission phenomenon upconversion (UC) emission. More recent approaches take advantage of solution-based synthesis, which allows creation of homogenous RE nanoparticles (NPs) with controlled size and structure that are capable of UC emission. Such nanoparticles are useful for many applications, especially in biology. For these applications, researchers seek small NPs with high upconversion emission intensity. These UCNPs have the potential to have multicolor and tunable emissions via various activators. A vast potential for future development remains by developing molecular antennas and energy transfer within RE ions. We expect UCNPs with optimized spectra behavior to meet the increasing demand of potential applications in bioimaging, biological detection, and light conversion. This Account focuses on efforts to control the size and modulate the spectra of UCNPs. We first review efforts in size control. One method is careful control of the synthesis conditions to manipulate particle nucleation and growth, but more recently researchers have learned that the doping conditions can affect the size of UCNPs. In addition, constructing homogeneous core/shell structures can control nanoparticle size by adjusting the shell thickness. After reviewing size control, we consider how diverse applications impose different requirements on excitation and/or emission photons and review recent developments on tuning of UC spectral profiles, especially the extension of excitation/emission wavelengths and the adjustment and purification of emission colors. We describe strategies that employ various dopants and others that build rationally designed nanostructures and nanocomposites to meet these goals. As the understanding of the energy transfer in the UC process has improved, core/shell structures have been proved useful for simultaneous tuning of excitation and emission wavelengths. Finally, we present a number of typical examples to highlight the upconverted emission in various applications, including imaging, detection, and sensing. We believe that with deeper understanding of emission phenomena and the ability to tune spectral profiles, UCNPs could play an important role in light conversion studies and applications.
Evaluation of Maxillary Alveolar Ridge Morphology and Residual Bone for Implant Placement by Cone Beam Computed Tomography (CBCT)

Abstract:The aim of this study was to assess the morphology and dimension of maxillary alveolar ridge at anterior and posterior regions using cone beam computed tomography (CBCT) prior to the placement of immediate implants. CBCT scans of 192 patients were evaluated. Measurements were taken at the cross sectional views in the maxillary central incisor, lateral incisor, canine incisor, first and second premolars and first and second molars regions. Alveolar ridge width and height, buccal plate thickness, buccal undercut, inter-root septum height, and root penetration to sinus floor were measured. Independent T-test was used for statistical analysis. The mean value of alveolar height in the maxillary incisors were higher than the posterior teeth with central incisors having the highest value of 19.23 ± 8.74 mm. At anterior maxilla, the lateral incisor with the mean value of 1.07 ± 0.64 mm exhibits the highest buccal undercut and the thinnest ridge width. At the site of posterior regions premolars had higher buccal undercut than molars. Canine with the mean value of 0.19 ± 0.31 mm thickness at the mid-alveolar of buccal plate, had the thinnest plate among other maxillary incisors. Sinus penetration and inter-root septum height were higher in premolar teeth than molars. The mean value of ridge width at all levels was significantly different between men and women, except at the level of crest in central incisor. CBCT evaluation presents accurate measurements for ridge assessment prior to immediate implant insertion. The recommended appropriate size of implant platform would be 5 mm in the incisor and almost 6 mm in premolar region.

Abstract:Objective: CBCT was used to investigate the effect of buccal bone thickness on horizontal bone loss at implant neck following immediate implantation in the aesthetic zone of the anterior maxilla and guided bone regeneration (GBR). Methods: Selecting CBCT data from patients who received immediate implant placement in the aesthetic zone with simultaneous GBR between April 2014 and January 2018. 72 patients (with a total of 90 dental implants) aged 18 to 65 years were selected as subjects. They received CBCT examination on the postoperative day, at 6th month and at least 12 months after implantation to measure horizontal buccal bone thicknesses at implant neck. The patients were divided into Groups 1 to 5 according to different thicknesses on the postoperative day (< 2 mm, 2-2.5 mm, 2.5-3 mm, 3-3.5 mm, and > 3.5 mm). Data were statistically analyzed using paired t test, One-way ANOVA and multiple comparisons of LSD. Results: Comparing the horizontal buccal bone loss rate at 6 months postoperatively, Group 1 (10.32 +/- 29.18%) was significantly less than Groups 3 (28.43 +/- 20.98%), 4 (30.15 +/- 23.65%) and 5 (36.15 +/- 18.06%) (P = 0.021, 0.014 and 0.002, respectively). Greater than 12 months after implantation, Group 1(22.53 +/- 37.36%) was significantly less than Groups 3 (40.89 +/- 20.57%), 4 (42.50 +/- 25.73%) and 5 (49.53 +/- 15.96%) (P = 0.026, 0.018 and 0.002, respectively), while Group 2 (27.45 +/- 18.81%) was also less than Group 5 (P = 0.012). Bone loss was more significant in the first six months after implantation than that in the latter six months (P = 0.013). Conclusion: After immediate implantation in the aesthetic zone with simultaneous GBR, there were horizontal bone loss at implant neck when the buccal bone thickness was above 3.5 mm.

Abstract:Introduction There is evidence that the apico‐coronal implant position and the mucosal phenotype can affect the extent of peri‐implant bone loss. This clinical trial analyzes the bone remodeling and marginal bone loss that occur around conical‐connection implants placed equicrestally and subcrestally, assessing the effect of the peri‐implant soft‐tissue phenotype. Methods Fifty‐one patients received 56 implants of distinct diameters (3.5 mm Ø n = 6; 4.3 mm Ø n = 41; 5 mm Ø n = 9) in the posterior part of the maxilla or mandible. The implants were placed equicrestally, 1 mm subcrestally and >1 mm subcrestally, depending on the initial supracrestal tissue height (STH). After 3 months of non‐submerged healing, single metal‐ceramic screw‐retained implant‐supported crowns were placed. Longitudinal measurements of STH, mucosal thickness and keratinized mucosa width (KMW) were made at the time of implant placement (T0), crown placement (T1), and after 3 (T2) and 6 months (T3) of prosthetic loading. At each of these points, a radiographic evaluation of bone remodeling and marginal bone loss was also performed. Results STH was significantly greater for implants placed >1 mm subcrestally than for those placed 1 mm subcrestally. After 12 months of follow‐up, a very significant (p 1 mm subcrestally, respectively. After the multiple linear regression, marginal bone loss was found to depend primarily on KMW (β = −0.43), while also being affected by STH (β = 0.32) and implant diameter (β = −0.28). Conclusions Marginal bone loss may be influenced by the position with respect to the bone crest, as well as the KMW, STH, and implant diameter. However, more well‐controlled studies are needed to verify these above‐mentioned findings with different implant designs and connections.

Abstract:Identifying landscape elements that influence gene flow and migration in wild species is the current main topic of landscape genetics. Most landscape genetic studies infer gene flow and migration from genetic distances among populations or individuals and statistically relate these measurements to landscape composition and configuration. This approach assumes symmetrical gene flow between pairs of populations. Such an assumption, however, will often be violated, especially in source–sink systems. Source populations provide more emigrants than they receive immigrants, and sink populations get many immigrants, but release few emigrants. Source–sink dynamics cannot be explored using common landscape genetic approaches relying on genetic distances. In this issue of Molecular Ecology, Andreasen et al. ( ) apply an alternative approach allowing them to infer asymmetrical migration. They use a Bayesian assignment test among objectively defined populations of mountain lions (Puma concolor) in western USA to estimate recent and directional migration rates. The study shows that an area with a high amount of wildlife refuges and low hunting pressure harbours a source population for mountain lion dispersal, while areas with high hunting pressures form sink populations; a result helpful in making informed decisions in conservation management.

Abstract:Objectives The current guidelines recommend that immediate implants be placed in patients with thick (>1 mm) buccal bone due to the inevitable tissue remodeling that follows tooth extraction. The aim of the current study was to investigate the effect of buccal bone thickness on bone resorption in immediate implant placement and compare two measuring techniques of the aforementioned resorption. Materials The present study was designed as a prospective nonrandomized, controlled clinical trial. A total of 30 implants were split between the two study arms, thin buccal bone and thick buccal bone. The primary outcome was to assess vertical bone changes radiographically by cone beam scans preoperatively, at 2 months and 18 months after implant placement in patients with thin and thick buccal plate. Secondary outcomes included the change in the thickness of the buccal bony plate, marginal bone loss, and pink esthetic score. Results Only 26 implants were statistically analyzed as one early failure was observed in each group. Furthermore, 2 patients of the thick group withdrew from the study. Cone beam computed tomography measurements revealed that at 2 months the vertical bone loss was 1.09 for the thin group and 0.85 for the thick group. The buccal bone plate resorption of the thin group was 0.39 mm while it was 0.52 mm for the thick group. The buccal bone plate was 1.25 mm in the thin group and 1.88 mm in the thick group. The PES did not show any significant difference with very good esthetic results. Conclusion Within the limitations of the current study, the amount of buccal bone plate resorption and the subsequent thickness obtained after implantation in both groups suggest successful long‐term results. Influence of preservation of the alveolar ridge on delayed implants after extraction of teeth with different defects in the buccal bone

Abstract:Our aim was to evaluate the influence of preservation of the alveolar ridge on delayed implants with different defects in the buccal bone. We enrolled 60 patients who had one posterior mandibular tooth extracted. Cone-beam computed tomography (CT) was used to measure the buccal bone defects in the alveolar ridge before the tooth was extracted (level A=3 to 5 mm, and level B=more than 5 mm). After the tooth had been extracted, the socket either had the alveolar ridge preserved (trial group) or it was left to heal spontaneously (control group). The changes in the dimensions of the alveolar ridge from preoperatively to 6 months postoperatively were evaluated by cone-beam CT. Suitable implants were inserted 6 months later, and their length and diameter recorded. The implant stability quotient was evaluated for the following 3 months. The dimensions of the bone in the alveolar ridge in the trial group were significantly less than those in the control groups in both levels. Fifty-seven patients required implants (except 3 in level B in the control group). There were more longer and wider implants in the trial group than in the control group in Level B. 3 months after implantation, there were no significant differences in implant stability quotients between the groups, though in the control group, Level B, the mean (SD) value was 69.50 (1.00) while in the other groups values were all above 70 at 3 months. We conclude that when the defect in the buccal bone was more than 5mm, the alveolar ridge preservation demonstrated a remarkable effect in preserving the alveolar ridge dimension and delayed implantation.

Abstract:Aim: To explore the features of and potential landmarks that predict crestal ridge remodeling after immediate implant placement and provisionalization (IIPP) with thin buccal plate phenotype in the anterior maxilla. Materials and methods: Patients requiring IIPP of single maxillary incisor with thin buccal bone plate (<1 mm) were recruited. Cone-beam computed tomography (CBCT), performed before and 6 months after the intervention, were three-dimensionally superimposed. A virtual line connecting the bony emergence points of adjacent teeth was drawn as a reference. The facial-coronal alveolar crest of the extraction socket and healed ridge were located in mesial, middle and distal cross-sectional planes, and linear ridge reductions were measured. The correlation between ridge reduction and local factors was analyzed. Results: Fifty patients were included. From the extraction socket to healed ridge, the alveolar crest underwent linear bone reduction of 1.50 +/- 0.50, 1.54 +/- 0.66 and 1.65 +/- 0.73 mm in the mesial, middle, and distal regions in an apical-palatal direction. The middle region had significantly higher horizontal (facial-palatal) but lower vertical (coronal-apical) bone reduction than the mesial and distal areas (P < 0.01). The facial-coronal ridge crest of the healed ridge located close to the reference line. Ridge reduction positively correlated with the distance between the initial facial-coronal crest of the extraction socket and the reference line (P < 0.01). Conclusions: Alveolar crest of the socket lost its curvature and tended to attain a flat profile after IIPP due to inconsistent ridge reduction in middle, mesial and distal areas. The reference line may be a potential landmark for predicting the ridge crest after remodeling.
