Abstract:We aims to assess the evidence of nasal continuous positive airway pressure (nCPAP) for treating stroke patients with sleep-disordered breathing (SDB). We conducted a systematic review encompassing both published randomized controlled trials that compared the use of nCPAP versus no treatment or sham CPAP. Two reviewers independently assessed eligibility, extracted characteristics and results, evaluated risk of bias, and synthesized findings using meta-analysis. Four studies (comprising five publications, with a total of 274 participants) met the inclusion criteria. The included studies exhibited a high risk of bias in at least one assessed domain, particularly regarding blinding of participants or personnel and other sources of bias. The impact of nCPAP on all-cause mortality and new stroke occurrence was inconclusive and statistically nonsignificant. However, a pooled analysis revealed an overall favorable effect of nCPAP on vascular mortality (odds ratio (OR) 0.16, 95% CI 0.03 to 0.93; P=0.04; n=196; 2 studies (3 reports); I2=0%). However, the analysis of adverse events, independence in activities of daily living (ADL), depression, and quality of life was hindered by insufficient data. Consequently, our review underscores the limited conclusion of nCPAP's efficacy in treating SDB post-stroke and underscores the need for further investigation.

Prevalence, incidence, and the time trends of sleep-disordered breathing among patients with stroke: a systematic review and meta-analysis

Abstract:Background: Recent studies have investigated the epidemiological burden of sleep-disordered breathing (SDB) in patients with stroke; however, the results have been inconsistent, and the temporal trends of SDB after stroke remain unclear. Objective: To perform a systematic review and meta-analysis of the prevalence and incidence of post-stroke SDB, evaluate demographic and clinical characteristic predictors of post-stroke SDB, and examine temporal trends in the overall burden of post-stroke SDB. Methods: We searched PubMed, MEDLINE, Embase, Web of Science, CINAHL, and the Cochrane Library for studies reporting the burden of SDB in stroke patients published between 1 January 2010 and 30 December 2023. Two researchers independently screened the records for eligibility, extracted the data, and assessed the quality of the studies. Data were analyzed using random effects meta-analyses, and sources of heterogeneity were explored using subgroup analyses and meta-regression analyses. Results: Out of the 8,799 references retrieved, none examined the incidence of SDB after stroke. However, 85 studies from 26 countries examined the prevalence of SDB and were included. The overall prevalence of SDB, mild SDB, and moderate to severe SDB were 60.0% (95% CI, 60.0–70.0%), 30.0% (95% CI, 23.0–37.0%), and 45.0% (95% CI, 33.0–57.0%), respectively. Meta-regression revealed that sex ( p < 0.0001) and sample size ( p < 0.01) were sources of heterogeneity among the studies. The pooled overall prevalence of SDB remained stable over time. Conclusion: SDB is common in patients with stroke, and no reduction in the high prevalence of SDB has been observed over time, suggesting that early screening and prevention of post-stroke SDB still have not received sufficient attention. Moreover, additional studies investigating the incidence of this disease are needed to inform clinical practice.

Abstract:To the Editor.— The article "Toxic Effects of Local Anesthetics" by Rudolph H. de Jong, MD (239:1166, 1978), is, as with the majority of the articles and books by Dr de Jong, excellent. However, the implication that diazepam (Valium) should be used prophylactically to prevent systemic toxic reactions in humans, as well as to treat them, is misleading. The studies cited by the author to support this contention were done in laboratory animals and in the research of Dr de Jong with lidocaine (Xylocaine) only. These data have been extrapolated to humans, but the author has no substantial data to support that, in fact, what applies in laboratory animals also applies in humans. To the best of my knowledge, he has not taken this valuable research into a clinical setting to confirm that in humans, diazepam is as effective as in animals, and, furthermore, that the dosage or the time
Effect of Continuous Positive Airway Pressure Ventilation on Nocturnal ST-segment Changes in Patients with Sleep-Disordered Breathing

Abstract:Objective To determine whether sleep-disordered breathing (SDB) may lead to nocturnal myocardial ischemia and whether the severity of this ischemia may be relieved by nasal continuous positive airway pressure (CPAP). Methods Overnight polysomnogram examination and simultaneous 3-channel Holter monitoring were performed on 76 patients with moderate to severe SDB and no history of coronary heart disease. All the cases were treated with CPAP for one night. ST depression was defined as a ST segment decrease of more than 1 mm from baseline and lasting 1 min or more. The total duration (minutes) of ST depression was indexed to the total sleep time (minutes per hour of sleep). Results Twenty-eight patients (37%) showed ST segment depression during their sleep. Before CPAP treatment, the respiratory disturbance index (RDI) and arousal index were significantly higher during periods of ST depression than when ST segments were isoelectric, whereas no significant difference was found in blood oxygen saturation (SaO2). After the CPAP treatment of patients with ST depression, the duration of ST depression was significantly reduced from 36.8±18.9 to 11.4±13.2 min/h (P＜0.05). ST depression-related indexes, including RDI, arousal index and the percentage of sleep time spent at SaO2 below 90% (TS90/ TST), were all significantly decreased, with RDI from 63.4±23.8 to 8.1±6.6 /h, arousal index from 51.2±18.9 to 9.6±5.4 /h, and TS90/ TST from 50.6±21.4 to 12.9±14.7% (P＜0.05). Conclusion ST-segment depression is rather common in patients with moderate to severe SDB, and CPAP treatment can significantly reduce the duration of ST depression. ST depression in these patients may reflect the myocardial ischemia that really exists and the non-ischemic changes associated with recurrent SDB.
Effects of continuous positive airway pressure (CPAP) therapy on neurological and functional rehabilitation in Basal Ganglia Stroke patients with obstructive sleep apnea: A prospective multicenter study

Abstract:Obstructive sleep apnea (OSA) adversely affects neurological recovery. This study aimed to determine the impact of continuous positive airway pressure (CPAP) and/or rehabilitation in basal ganglia stroke patients with OSA.A prospective controlled trial was conducted in 2015-2018. The subjects received routine rehabilitation training for up to 2 years and were assigned to the intervention and control groups treated with CPAP or without, respectively. Then, treatment effects on sleep parameters, motor function, stroke severity, daily life activities, cognitive function, and psychological states were assessed at different time points.At 6 months, the CPAP group showed significantly lower mean apnea-hypopnea index (AHI), percentage of time with SpO2 at <90% (TS90%), micro-arousal index, and percentages of time in non-rapid eye movement (non-REM) stages 1-2 and REM stage in total sleeping time compared with the control group, and significantly higher mean minimum of peripheral oxygen saturation (L-SaO2%) and percentage of time in stage 3 (P < .001) sleep. The CPAP group showed significant improvements in average the National Institutes of Health Stroke Scale (NIHSS), Fugl-Meyer assessment scale (FMA), Barthel index (BI), Minimental state examination (MMSE), Hamilton anxiety scale (HAMA) and Hamilton depression rating scale for depression (HRSD) scores at different times versus the control group, respectively (P < .05). However, no difference in body mass index (BMI) management was observed (P > .05). Repeated-measures ANOVA revealed significant interactions between the two groups for change in FMA, MMSE, BI, HAMA, and HRSD scores from admission to 24 months (P < .001), but no significant was found for BMI (P = .582).Basal ganglia stroke patients with OSA tend to have significantly greater sleeping, neurological and functional recovery after CPAP, and rehabilitation over 2 years.
Association of Positive Airway Pressure With Cardiovascular Events and Death in Adults With Sleep Apnea: A Systematic Review and Meta-analysis.

Abstract:IMPORTANCE Sleep apnea (obstructive and central) is associated with adverse cardiovascular risk factors and increased risks of cardiovascular disease. Positive airway pressure (PAP) provides symptomatic relief, whether delivered continuously (CPAP) or as adaptive servo-ventilation (ASV), but the associations with cardiovascular outcomes and death are unclear. OBJECTIVE To assess the association of PAP vs control with cardiovascular events and death in patients with sleep apnea. DATA SOURCES AND STUDY SELECTION MEDLINE, EMBASE, and the Cochrane Library were systematically searched from inception date to March 2017 for randomized clinical trials that included reporting of major adverse cardiovascular events or deaths. DATA EXTRACTION AND SYNTHESIS Two authors independently extracted data using standardized forms. Summary relative risks (RRs), risk differences (RDs) and 95% CIs were obtained using random-effects meta-analysis. MAIN OUTCOMES AND MEASURES The main outcomes were a composite of acute coronary syndrome (ACS) events, stroke, or vascular death (major adverse cardiovascular events); cause-specific vascular events; and death. RESULTS The analyses included data from 10 trials (9 CPAP; 1 ASV) of patients with sleep apnea (N = 7266; mean age, 60.9 [range, 51.5 to 71.1] years; 5847 [80.5%] men; mean [SD] body mass index, 30.0 [5.2]. Among 356 major adverse cardiovascular events and 613 deaths recorded, there was no significant association of PAP with major adverse cardiovascular events (RR, 0.77 [95% CI, 0.53 to 1.13]; P=.19 and RD, -0.01 [95% CI, -0.03 to 0.01]; P=.23), cardiovascular death (RR, 1.15 [95% CI, 0.88 to 1.50]; P=.30 and RD -0.00 [95% CI, -0.02 to 0.02]; P=.87), or all-cause death (RR, 1.13 [95% CI, 0.99 to 1.29]; P=.08 and RD, 0.00 [95% CI, -0.01 to 0.01]; P=.51). The same was true for ACS, stroke, and heart failure. There was no evidence of different associations for CPAP vs ASV (all P value homogeneity >.24), and meta-regressions identified no associations of PAP with outcomes for different levels of apnea severity, follow-up duration, or adherence to PAP (all P values >.13). CONCLUSIONS AND RELEVANCE The use of PAP, compared with no treatment or sham, was not associated with reduced risks of cardiovascular outcomes or death for patients with sleep apnea. Although there are other benefits of treatment with PAP for sleep apnea, these findings do not support treatment with PAP with a goal of prevention of these outcomes.
Proactive telemedicine monitoring of sleep apnea treatment improves adherence in people with stroke- a randomized controlled trial (HOPES study)

Abstract:Objective: Obstructive sleep apnea (OSA) impacts stroke recovery and outcome negatively. Although its identification and treatment are part of the current stroke guidelines, standard management with positive airway pressure (PAP) therapy is not routinely performed and adherence rates are very low. The purpose of this study was to determine whether PAP adherence can be improved by a PAP training strategy during in-hospital rehabilitation combined with a telemedicine monitoring system after discharge. Methods: In this study, we performed a controlled trial (RCT) on standard PAP treatment (SG) as compared with proactive telemonitored PAP treatment (TG). After three months and one year, PAP adherence (min of use per day) and clinical outcome variables were compared. Results: In 33 (47.1%) out of 70 patients diagnosed with therapy-relevant OSA [70% male, 62 (5) years, body mass index (BMI) 30 (4) kg/m2, Barthel Index 90 (20), NIHSS 3 (3)] in-hospital PAP titration was performed. Subsequently, they were randomized to SG or TG. Drop-out rates after three months and after one year were 12% and 30%, respectively, with no differences between the groups. After three months, telemonitored patients used the PAP device 76 min longer per night (SG: 299 (76), TG: 375 (86) minutes per night; p = 0.017), after one year there was no significant difference. Conclusion: People with stroke and therapy-relevant OSA who accept PAP therapy should receive additional telemedicine monitoring at least for three months. Abstract:Adherence to continuous positive airway pressure (CPAP) in patients with obstructive sleep apnea (OSA) post-stroke is often problematic, despite potential benefits. This study aimed to evaluate CPAP adherence in patients with OSA post-stroke based on the Andersen behavioral model of health services utilization. A total of 227 eligible participants were recruited from a Chinese hospital. After baseline assessment, participants were followed for 6 months to determine short-term CPAP adherence. Those with good short-term adherence were followed for an additional 6 months to explore long-term adherence and influencing factors. Short-term CPAP adherence rate was 33%. Being married or living with a partner, having an associate degree or baccalaureate degree or higher, and stronger health beliefs independently predicted short-term CPAP adherence. Only 25% of participants from the adherent group showed good long-term adherence. The factor associated with long-term CPAP adherence was participants not using alcohol. Adherence to CPAP is suboptimal among patients having OSA post-stroke. Addressing unfavorable predisposing factors and modifying health beliefs are suggested.

Abstract:Purpose: Untreated obstructive sleep apnea (OSA) in patients with acute ischemic stroke (AIS) is associated with increased morbidity and mortality. However, diagnosing and treating OSA in AIS is challenging. We aimed to determine the feasibility of portable monitoring (PM) for diagnosis and positive airway pressure therapy for treatment of OSA in an inpatient stroke population. Methods: We recruited inpatients with AIS from Cleveland Clinic. Those who consented underwent PM; participants with a respiratory event index (REI) ≥ 10 were offered auto-titrating positive airway pressure therapy (APAP). Ease-of-use questionnaires were completed. We summarized categorical variables using n(%) and continuous variables using mean ± SD or median [IQR]. Results: 27 participants (age 59.8 ± 11.8, 51.9% female, 51.9% Black, BMI 33.4 ± 8.5) enrolled. The study ended early due to Medicare contracting that forced most patients to complete stroke rehabilitation outside the Cleveland Clinic health system. 59.3% had large vessel occlusions and 53.8% had moderate/severe disability (Modified Rankin score ≥ 2). PM was attempted in 21 participants, successful in 18. Nurses and patients rated the PM device as highly easy to use. 13 of 18 (72%) patients who had an REI ≥ 10 consented to APAP titration, but only eight (61.5%) of those 13 used APAP for more than one night, and only five (27.8%) used APAP up to 90 days with data captured for only one participant. Five required troubleshooting at titration, and only one had adherent APAP usage by objective assessment after discharge. Conclusions: This study demonstrates the real-world challenges of assessing and treating OSA in an AIS population, highlighting the necessity for further research into timely and feasible screening and treatment.

Abstract:Summary Sleep‐disordered breathing (SDB) is linked to cognitive dysfunction. Although SDB is common in stroke patients, the impact of SDB and its early treatment on cognitive functioning after stroke remains poorly investigated. Therefore, we explored the association between SDB and post‐stroke cognitive functioning, including the impact of early SDB treatment with adaptive servo‐ventilation (ASV) on cognitive recovery from acute event to 3 months post‐stroke. We used data from two studies, which included ischaemic stroke patients (n = 131) and no‐stroke controls (n = 37) without SDB (apnea–hypopnea index, AHI <5/h) and with SDB (AHI≥20/h). Cognitive functioning was assessed within 7 days and 3 months post‐stroke in stroke patients, or at study inclusion in no‐stroke control group, respectively. Stroke patients with SDB were randomized to ASV treatment (ASV+) or usual care (ASV‐). Linear regression adjusted for main confounders assessed the impact of SDB and its treatment on cognitive recovery. The intention‐to‐treat analysis did not show significant associations of SDB ASV+ (n = 30) versus SDB ASV‐ (n = 29) with cognitive recovery. In an exploratory subanalysis, compliant SDB ASV+ (n = 14) versus SDB ASV‐ showed improvements with ASV in visual memory and cognitive flexibility. Combining the stroke and non‐stroke datasets, SDB (n = 85) versus no‐SDB (n = 83) was associated with deficits in visual memory and response inhibition independently of stroke. SDB ASV‐ versus no‐SDB (n = 51) was associated with less improvement in visual memory. There was no substantial evidence for benefits of intention‐to‐treat ASV on cognitive recovery. Exploratory analysis indicated that compliant ASV treatment could benefit visual memory and cognitive flexibility, whereas untreated SDB could contribute to a poor recovery of visual memory.

Abstract:Purpose: To evaluate outcomes of repeat pars plana vitrectomy for proliferative vitreoretinopathy after previous failed pars plana vitrectomy. Methods: This is a retrospective case series including 51 eyes of 50 patients who underwent repeat surgery after failed previous pars plana vitrectomy for proliferative vitreoretinopathy from 2000 to 2015 at the Kresge Eye Institute, Detroit, MI. Patients were classified into successful and unsuccessful groups. Success was defined as retinal reattachment, silicone oil removed, and best-corrected visual acuity (BCVA) ≥5/200 at the final follow-up visit. Results: Forty-three eyes (84.3%) were successfully reattached at the last follow-up. Seventeen (33.3%) eyes were deemed successful and 34 (66.7%) eyes unsuccessful according to our criteria. Compared with the successful group, eyes in the unsuccessful group had more eyes with preoperative BCVA <5/200 (P < 0.001), preoperative BCVA of hand motion or worse (P = 0.002), preoperative flare ≥Grade 2+ (P = 0.03), preoperative posterior breaks (P = 0.02), previous retinectomy (P = 0.04), and final postoperative hypotony (intraocular pressure ⩽ 5 mmHg) (P = 0.005). Eyes with silicone oil removed were more likely to have BCVA ≥5/200 (P < 0.001) at the final follow-up visit. Location of patients >100 miles (P = 0.04) from Detroit and preoperative BCVA of hand motion or worse (P = 0.01) were significantly associated with failure in the logistic regression analysis. Conclusion: Success after repeat surgery for proliferative vitreoretinopathy should include ambulatory vision, retinal reattachment, and silicone oil removal. We identified several preoperative and perioperative factors that were associated with success in the bivariate and logistic analyses. The decision to perform surgical reoperation in these patients should be based on multiple factors, most importantly preoperative BCVA.

Abstract:BACKGROUND:Obstructive sleep apnea (OSA) is associated with cognitive impairment and increased risks of dementia. However, the effect of continuous positive airway pressure (CPAP) on cognitive function in patients with OSA is still controversial.OBJECTIVE:To evaluate the cognitive effects of CPAP treatment on OSA.METHODS:We systematically searched PubMed, EMBASE, and Cochrane Library for randomized controlled trials (RCT) in the corresponding fields.RESULTS:Totally 14 studies and 1,926 participants were included in our meta-analysis. Standardized mean difference (SMD) or weighted mean difference (WMD) were calculated for subjective sleepiness and cognitive domains including attention and speed of information processing, executive function, and memory. Individual cognitive scale and subgroup analyses according to OSA severity, length of trial, and RCT design type were further conducted. Significant treatment effect on attention and speed of information processing was only observed in severe OSA patients (SMD, 0.17; 95% CI, 0.02 to 0.31; p = 0.025; I2 = 0%).CONCLUSIONS:Therefore, our meta-analysis indicates that CPAP treatment can partially improve cognitive impairment in the population of severe OSA.

Abstract:Objective: Evidence has shown that obstructive sleep apnea (OSA) is associated with the development of stroke. However, there is limited data on how OSA can influence the development and severity of post stroke complications. This study was done to investigate the potential relationship between OSA and post stroke complications and systematically review the current literature available on the topic. Methods: PubMed, Web of Science and Scopus Databases were used to collect data from the published literature up to December 2022. The studies that met the inclusion criteria were selected and Review Manager 5.4.1 was used for statistical analysis of data. Pooling of data with heterogeneity was done using a random-effects model. The findings were presented by using the standard mean differences within their 95% confidence intervals. Results: The analysis showed that stroke patients experiencing OSA do not significantly have higher post-stroke complications (i.e mild cognitive impairment, dementia, insomnia, fatigue, sleep quality, post-stroke depression, anxiety, recurrent strokes) compared to those stroke patients who do not have OSA (RR=1.05, 95% CI 0.97 to 1.13). Similarly, stroke patients experiencing OSA did not have any statistically significant risk of depression (RR=0.92, 95% CI = 0.58 to 1.48) or cognitive impairment (RR=1.05, 95% CI –0.90 to 1.23). Among patients with high stroke severity, those who have OSA have higher risk of post-stroke complications than those patients without OSA (RR=1.06, 95% CI –1.01 to 1.12) Conclusion: This systematic review and meta-analysis attempted to consolidate the literature and potentially suggested a possible positive relationship between OSA and post-stroke complications mainly with neuropsychiatric comorbidities including cognitive impairment, depression and anxiety. Further studies are required to explore the impact of OSA on post stroke complications.

Abstract:The authors studied 2,494 homicides that occurred in Harris County, Tex. (Houston), over a 14-year period in an effort to confirm a recent study suggesting that homicides are significantly related to phases of the moon. They found no significant relationship, although homicides did show strong day-of-week and hour-of-day cycles.

Abstract:Rationale: Obstructive sleep apnea is common among patients with acute ischemic stroke and is associated with reduced functional recovery and an increased risk for recurrent vascular events. Aims and/or hypothesis: The Sleep for Stroke Management and Recovery Trial (Sleep SMART) aims to determine whether automatically adjusting continuous positive airway pressure (aCPAP) treatment for obstructive sleep apnea improves clinical outcomes after acute ischemic stroke or high-risk transient ischemic attack. Sample size estimate: A total of 3062 randomized subjects for the prevention of recurrent serious vascular events, and among these, 1362 stroke survivors for the recovery outcome. Methods and design: Sleep SMART is a phase III, multicenter, prospective randomized, open, blinded outcome event assessed controlled trial. Adults with recent acute ischemic stroke/transient ischemic attack and no contraindication to aCPAP are screened for obstructive sleep apnea with a portable sleep apnea test. Subjects with confirmed obstructive sleep apnea but without predominant central sleep apnea proceed to a run-in night of aCPAP. Subjects with use (≥4 h) of aCPAP and without development of significant central apneas are randomized to aCPAP plus usual care or care-as-usual for six months. Telemedicine is used to monitor and facilitate aCPAP adherence remotely. Study outcomes: Two separate primary outcomes: (1) the composite of recurrent acute ischemic stroke, acute coronary syndrome, and all-cause mortality (prevention) and (2) the modified Rankin scale scores (recovery) at six- and three-month post-randomization, respectively. Discussion: Sleep SMART represents the first large trial to test whether aCPAP for obstructive sleep apnea after stroke/transient ischemic attack reduces recurrent vascular events or death, and improves functional recovery.

Abstract:Radiographic findings of the 2 new cases of oxyphilic renal adenoma, so-called renal oncocytoma, are reported and compared to previous descriptions. Ultrasonography shows no typical findings, but the vascular capsule of this tumor may show enhancement on computed tomography. Angiography contributes the most specific findings, because of the vascularity of these tumors: In the larger lesions a typical 'spoke wheel pattern' can be expected which may not be as prominent in smaller tumors. Notable is the absence of abrupt arterial caliber changes, AV shunting and contrast puddling which sets this tumor apart from the great majority of vascular renal cell carcinomas. The incidence of renal oncocytomas appears to be rising.

Abstract:Background: stroke patients often have a combination of sleep apnea syndrome, which is an important and modifiable risk factor for stroke prognosis. Acupuncture is one of the measures for sleep apnea syndrome, and it is also widely used in stroke. However, we are concerned that its efficacy and safety in the treatment of stroke with sleep apnea syndrome are not yet clear. Methods: This systematic review and meta-analysis was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses schema and was registered with INPLASY (registration number: INPLASY202250113). The following 8 databases were searched: PubMed, Cochrane Library (CENTRAL), Embase, Web of Science, China National Knowledge Infrastructure, Chongqing VIP Information, WanFang Data, and China Biomedical Literature Database limited from the establishment of each database to May 4, 2022. Subject headings, free words, and keywords were used for retrieval. Relevant literature was supplemented by consulting other resources. We assessed the risk of bias in the included studies using the Cochrane risk of bias tool. RevMan 5.4 software (The Cochrane Collaboration, 2020) was used to perform the meta-analysis. Results: Six records were included, including a total of 513 participants: 256 in the experimental group and 257 in the control group. The results showed that the total effective rate (relative risk = 1.23, 95% confidence interval (CI): 1.13, 1.34, P < .00001), apnea-hypopnea index (mean difference (MD) = -8.39, 95% CI: -9.19, -7.59, P < .00001), Epworth Sleepiness Scale score (MD = -1.59, 95% CI: -2.66, -0.52, P = .004), minimal oxygen saturation (MD = 4.99, 95% CI: 3.5, 6.47, P < .00001), longest duration of apnea (MD = -7.47, 95% CI: -8.97, -5.97, P < .00001), longest duration of apnea (MD = -6.48, 95% CI: -8.60, -4.35, P < .00001), and S100β levels (standard mean difference = -1.52, 95% CI: -1.87, -1.18, P < .00001) were better in the experimental group than in the control group. Simultaneously, the effect of reducing the neuron-specific enolase level in the experimental group was comparable to that in the control group (MD = -3.40, 95% CI: -9.08, 2.29, P = .24). Conclusions: Acupuncture can improve the clinical symptoms and related laboratory indicators for sleep apnea syndrome in patients with stroke. More high-quality trials remain urgently needed.
Investigation of Ischemic Stroke Patients with Mild or Moderate Obstructive Sleep Apnea-hypopnea Syndrome Treated by Nasal Continuous Positive Airway Pressure

Abstract:Objective To observe the clinical results of ischemic stroke patients with mild and moderate obstructive sleep apnea-hypopnea syndrome treated by nasal continuous positive airway pressure (nCPAP). Methods Sixty-two cases were selected from March 2008 to March 2010 who suffered acute ischemic stroke. After 90 days, thirty patients with mild or moderate obstructive sleep apnea-hypopnea syndrome were enrolled into the study by using polysomnography. Then they were divided into two groups by whether using nCPAP or not:one group (14 cases) accepted treatment with nCPAP, while the other group (16 cases) rejected it. The recurrence rate of stroke, apnea hypopnea index (AHI), blood pressure, body mass index (BMI), Epworth Sleepiness Score (ESS), Hospital Anxiety and Depression Scale (HAD) at the 6th, 12th, 18th, 24th month were measured. Results There were no significant differences between two groups in age, sex, diabetes mellitus,hypertension disease, AHI, systolic pressure, diastolic pressure, BMI, modified Rankin Scale score, ESS, HAD (P value>0.05). There are no recurrent ischemic strokes through two years of observation. The nCPAP group performs better than non-nCPAP group in reducing AHI (nCPAP group AHI:3.9±0.6, 3.8±0.5, 3.9±0.5, 3.8±0.5, non-CPAP group AHI:20.8±4.1, 21.7±4.5, 22.6±4.2, 26.8±6.1, t value 16.2, 15.9, 17.8, 15.0, P value <0.001) and ESS scores (nCPAP group 3.5±1.7, 2.6±1.5, 2.2±1.4, 2.1±1.1, t value 6.1, 8.8, 9.9, 10.0, all P values <0.001), improving the ratio of anxiety in 24th month (P=0.039), while there is subtle difference in the ratio of depression level in the 6th, 12th, 18th, 24th month (nCPAP group:14.3%, 14.3%, 14.3%,14.3%, non-CPAP group:18.8%, 18.8%, 18.8%, 18.8%, P value 1.0, 1.0, 1.0, 1.0) and there is subtle difference in BMI in the 6th, 12th, 18th, 24th month (nCPAP group:22.8±1.4, 23.1±1.4, 22.7±1.4, 22.6±1.4, non-CPAP group:23.3±1.4, 23.7±1.6, 23.5±1.6, 23.0±1.3, P value>0.05). Conclusion It proves that treatment with nCPAP can reduce AHI, decrease ESS scores, and improve the level of anxiety.
