Abstract:Objectives Evaluating the diagnostic efficiency of deep learning models to diagnose vertical root fracture in vivo on cone-beam CT (CBCT) images. Materials and methods The CBCT images of 276 teeth (138 VRF teeth and 138 non-VRF teeth) were enrolled and analyzed retrospectively. The diagnostic results of these teeth were confirmed by two chief radiologists. There were two experimental groups: auto-selection group and manual selection group. A total of 552 regions of interest of teeth were cropped in manual selection group and 1118 regions of interest of teeth were cropped in auto-selection group. Three deep learning networks (ResNet50, VGG19 and DenseNet169) were used for diagnosis (3:1 for training and testing). The diagnostic efficiencies (accuracy, sensitivity, specificity, and area under the curve (AUC)) of three networks were calculated in two experiment groups. Meanwhile, 552 teeth images in manual selection group were diagnosed by a radiologist. The diagnostic efficiencies of the three deep learning network models in two experiment groups and the radiologist were calculated. Results In manual selection group, ResNet50 presented highest accuracy and sensitivity for diagnosing VRF teeth. The accuracy, sensitivity, specificity and AUC was 97.8%, 97.0%, 98.5%, and 0.99, the radiologist presented accuracy, sensitivity, and specificity as 95.3%, 96.4 and 94.2%. In auto-selection group, ResNet50 presented highest accuracy and sensitivity for diagnosing VRF teeth, the accuracy, sensitivity, specificity and AUC was 91.4%, 92.1%, 90.7% and 0.96. Conclusion In manual selection group, ResNet50 presented higher diagnostic efficiency in diagnosis of in vivo VRF teeth than VGG19, DensenNet169 and radiologist with 2 years of experience. In auto-selection group, Resnet50 also presented higher diagnostic efficiency in diagnosis of in vivo VRF teeth than VGG19 and DensenNet169. This makes it a promising auxiliary diagnostic technique to screen for VRF teeth.

Abstract:Abstract Objective: To establish a comprehensive and accurate assessment model of periodontal alveolar bone loss based on panoramic images.Methods:A total of 640 panoramic images were included, and 3 experienced periodontal physicians marked the key points needed to calculate the degree of periodontal alveolar bone loss and the specific location and shape of the alveolar bone loss. A deep learning architecture based on UNet and YOLO-v4 was proposed to localize the tooth and key points, and the percentage and stageof periodontal alveolar bone loss were accurately calculated. The ability of the model to recognize these features was evaluated and compared with that of general dental practitioners.Results: The overall classification accuracy of the model was 0.77, and the performance of the model varied for different tooth positions and categories;model classification was generally more accurate than that of general practitioners.Conclusion: It is feasible to establish deep learning model forassessmentand staging radiographicperiodontal alveolar bone loss using two-stage architecture based on UNet and YOLO-v4.

Abstract:ObjectivesThe aim of this study was to evaluate the use of a convolutional neural network (CNN) system for detecting vertical root fracture (VRF) on panoramic radiography.MethodsThree hundred panoramic images containing a total of 330 VRF teeth with clearly visible fracture lines were selected from our hospital imaging database. Confirmation of VRF lines was performed by two radiologists and one endodontist. Eighty percent (240 images) of the 300 images were assigned to a training set and 20% (60 images) to a test set. A CNN-based deep learning model for the detection of VRFs was built using DetectNet with DIGITS version 5.0. To defend test data selection bias and increase reliability, fivefold cross-validation was performed. Diagnostic performance was evaluated using recall, precision, and F measure.ResultsOf the 330 VRFs, 267 were detected. Twenty teeth without fractures were falsely detected. Recall was 0.75, precision 0.93, and F measure 0.83.ConclusionsThe CNN learning model has shown promise as a tool to detect VRFs on panoramic images and to function as a CAD tool.

Abstract:Introduction: The purpose of this study was to evaluate the diagnostic accuracy of cone-beam computed tomographic (CBCT) imaging in the detection of subtle vertical root fractures (VRFs) in endodontically treated teeth in vivo and to discuss direct and indirect evidence for the diagnosis of subtle VRFs. Methods: Twenty-nine endodontically treated teeth with suspected VRFs from 29 patients were examined using CBCT imaging. CBCT images were scored based on evaluation of the fracture line and vertical buccopalatal (lingual) bone loss. VRF was diagnosed only when a definite fracture line was detected on CBCT images, and findings of periodontal exploration or surgical extraction were considered the gold standard. The diagnostic sensitivity, specificity, and accuracy were assessed. Results: Of the 29 teeth, 27 were positive and 2 were negative for VRF according to the gold standard. Sensitivity, specificity, and accuracy were 33.3%, 100%, and 37.9%, respectively, based on the direct finding of a fracture line on CBCT images. However, vertical buccopalatal (lingual) bone loss was found in 25 of 27 teeth with VRFs. Conclusions: Although the accuracy of CBCT imaging for the diagnosis of subtle VRFs in endodontically treated teeth in vivo was poor, vertical buccopalatal (lingual) bone loss is an important indirect sign for the diagnosis of VRFs, which can be found on CBCT images.
Detection and classification of mandibular fracture on CT scan using deep convolutional neural network

Abstract:Objectives: This study aimed to evaluate the accuracy and reliability of convolutional neural networks (CNNs) for the detection and classification of mandibular fracture on spiral computed tomography (CT). Materials and methods: Between January 2013 and July 2020, 686 patients with mandibular fractures who underwent CT scan were classified and annotated by three experienced maxillofacial surgeons serving as the ground truth. An algorithm including two convolutional neural networks (U-Net and ResNet) was trained, validated, and tested using 222, 56, and 408 CT scans, respectively. The diagnostic performance of the algorithm was compared with the ground truth and evaluated by DICE, accuracy, sensitivity, specificity, and area under the ROC curve (AUC). Results: One thousand five hundred six mandibular fractures in nine subregions of 686 patients were diagnosed. The DICE of mandible segmentation using U-Net was 0.943. The accuracies of nine subregions were all above 90%, with a mean AUC of 0.956. Conclusions: CNNs showed comparable reliability and accuracy in detecting and classifying mandibular fractures on CT. Clinical relevance: The algorithm for automatic detection and classification of mandibular fractures will help improve diagnostic efficiency and provide expertise to areas with lower medical levels.
A Radiocarbon Dated Pollen Sequence from the Wells Mastodon Site Near Rochester

Abstract:A radiocarbon date (1-586, 12,000=450) has been obtained from wood beneath a mastodon (Mastodon americanus) in north-central Indiana. The sediment sequence includes 72 inches of marl at the base, overlain by 12 inches of sand, capped by 34 inches of peat. The peat is truncated at the top by fire and cultivation. Pollen spectra from the marl associated with the mastodon burial indicate a predominantly coniferous forest consisting of spruce (Picea), fir (Abies), birch (Betula), and ash (Fraxinus) with small amounts of oak (Quercus), Hickory (Carya), and elm (Ulmus) pollen. Field evidence and pollen data indicate that the animal died prior to the formation of the peat. Correlation of this sequence with other radiocarbon-dated pollen sequences indicates that the mastodon died prior to the replacement of spruce pollen by oak pollen approximately 10,500 years ago. Molluscan data support the inferences derived from the pollen analysis. A vertebra and the right lower jaw of a Mastodon americanus were uncovered in 1959 in the excavation of a drainage ditch on a farm five miles west of Rochester, Indiana (fig. 1). Cursory probing by Mr. James Wells, owner of the farm, and James Cope and James Thorp of Earlham College revealed more bones a few feet below the bottom of the ditch. About 25 persons, under the direction of James Cope and Gertrude Ward of Earlham's Joseph Moore Museum, assisted during the summer of 1960 in the recovery of most of the mastodon skeleton. A. M. Gooding assisted with the dig, studied the geological setting and stratigraphy, and collected samples for pollen, molluscan, and radiocarbon analysis. John O'Brien, an Earlham senior geology major, studied and prepared the molluscan data, and J. Gordon Ogden, III, with technical assistance by Barbara Spross, prepared and analyzed the pollen samples. A pollen diagram and discussion of the conditions of burial are included in this report. GEOLOGICAL SETTING The Wells Mastodon Site is located in a broad area of valley train and outwash deposits between the Maxinkuckee and Packerton Moraines of Cary age (fig. 2). Outwash sands around the mastodon site have been worked by the wind in places, and groups of low dune hills interrupt the otherwise flat topography of the area. The water table is high, natural drainage poor, and extensive areas are covered with peat (Carlisle muck soil; Rogers, 1956) which lap onto the dune hills. The peat is commonly underlain with marl of variable thickness. The sluggish Mud Creek drainage system (fig. 1), along one tributary of which the Wells mastodon site is located, probably originally connected a series of small lakes and ponds in the area. Excavation for the skeleton extended across the drainage ditch, and a pit was opened which measured about 30 ft across and 8 ft deep (fig. 3). The following stratigraphic section on the south side of the pit was measured, described, and sampled for pollen and radiocarbon analysis: Unit Thickness Description 3 34 in. Peat—dense, black to brown. Upper few inches showed effects of burning of surface vegetation. 2 12 in. Sand—well-sorted quartz sand, probably largely windblown, containing numerous fine streaks of finely divided organic matter. 1 72 in. Marl—gray, consisting almost entirely of fragments of gastropod and pelecypod shells. An occasional piece of wood was encountered in this unit, but no concentration of plant remains. Radiocarbon-dated wood sample collected at 90 in. from surface (1-586,12,000 ±450). Bottom 28 in. determined by auger. Bottom of marl probably on till. Later extension of the pit to the north showed the same stratigraphy, although units varied slightly in thickness. INTERPRETATION OF FIELD DATA The Wells mastodon is exceptionally well preserved and few of the bones are broken or fragmented. Except for the left legs and the pelvis, which probably FIGURE 1. None of the bones show rodent teeth marks or other evidence of surface exposure after death. All of the skeletal remains were found in marl (unit 1), mostly in a zone between 5 and 7 ft below the surface, although some bones extended vertically downward to greater depths. The skull, with the lower jaws and the right tusk in place, was overturned to the left about 130 degrees. Most of the rest of the bones were found approximately in their proper positions with respect to this orientation of the skull. Although, obviously, the marl sediment around the skeleton must have been disturbed by the animal in his death struggles, and by later oozing into skeletal cavities, the field evidence suggests that burial occurred before the completion of

Abstract:Objective:To evaluate the value of cone-beam computed tomography(CBCT) in the early diagnosis of vertical root fracture(VRF).Methods:The study was conducted on 23 cases with clinically suspected VRF which were not confirmed by conventional periapical radiograph.All the cases were examined by CBCT.The CBCT images were evaluated and recorded in 1 of the following 3 categories:definite fracture,probable fracture,and no-fracture.The CBCT criteria of VRFs were set as detection of a hypoattenuated line on at least 2 continuous axial sections,or separation of the fracture segments.Results:In CBCT images,17 teeth were diagnosed as definite fracture,4 probable fracture and 2 no-fracture.The 17 teeth with definite fracture on CBCT images were proved after extraction and the 4 teeth with probable fracture on CBCT images were also evidenced to have VRFs during 4-6 months follow-up.The other 2 cases without CBCT detectable fracture were finally confirmed to be non-fractured.Among the 21 teeth with surgery-evidenced root fracture,10 were nonendodontically treated and 11 were endodontically treated.The Sensitivity for VRF detection of CBCT were 81%.Conclusion:CBCT was valuable for the early detection of VRFs.

Abstract:A definitive diagonosis of vertical root fracture (VRFs) is often a challenging task for clinicians. Two-dimensional periapical radiographs (PRs) may be not helpful in such a diagnosis when the x-ray beam is not parallel to the plane of the fracture line. This report presents a set of 3 cases in which 1 endodontically treated and 2 nonendodontically treated mandibular molars were diagnosed with VRFs based on findings from clinical, radiographic, and cone-beam computerized tomographic (CBCT) examinations. After extraction, VRFs were confirmed in all of the teeth. Deep and narrow periodontal pockets were detected in 2 molars. A widening of the root canal space was observed in the PR of 1 molar only, and crown cracks were detected in none of these cases. However, in all 3 molars, fracture lines were visible on the CBCT images. Thus, CBCT provided useful information in diagnosing VRFs in both endodontically treated and nonendodontically treated teeth, especially when VRFs could not be confirmed by clinical findings and PRs. Detection of Simulated Vertical Root Fractures: Which Cone-beam Computed Tomographic System Is the Most Accurate?

Abstract:INTRODUCTION: We aimed to compare the diagnostic accuracy of 5 cone-beam computed tomographic (CBCT) systems in detecting vertical root fractures (VRFs) and to assess whether fracture identification is affected by the presence of root canal filling.METHODS: Eighty extracted posterior teeth were included in this study. They were grouped according to the presence/absence of VRFs and the presence/absence of endodontic treatment. The teeth were then inserted in 5 dry skull/mandible assemblies. CBCT scans were performed using 5 different commercially available systems. Two observers evaluated the resultant multiplanar images twice for VRFs using a 3-point scale.RESULTS: i-CAT (Imaging Sciences International, Hatfield, PA) showed the highest diagnostic accuracy in the detection of VRFs among the 5 investigated CBCT systems. The presence of root canal filling did not significantly decrease the ability to detect VRFs by all the studied systems. In the detection of VRFs in endodontically treated teeth, i-CAT was the most accurate, whereas 3D Accuitomo (J. Morita, Kyoto, Japan) was the least. Interobserver agreement was moderate for the i-CAT and fair for the rest of the studied modalities, whereas intraobserver agreement was good for the Scanora 3D (Soredex, Tuusula, Finland) and moderate for images from the other CBCT machines.CONCLUSIONS: At the specified exposure parameters in the detection of VRFs in non-root canal-filled teeth, i-CAT showed the highest diagnostic accuracy followed by Planmeca Promax 3D (Planmeca, Helsinki, Finland), Scanora 3D, Accuitomo 3D, and Galileos 3D (Sirona Dental Systems, Bensheim, Germany), respectively. In the detection of VRFs in root canal-filled teeth, i-CAT showed the highest diagnostic accuracy followed by Planmeca Promax 3D, Scanora 3D, Galileos Comfort (Sirona Dental Systems), and Accuitomo 3D.

Exploring the Use of Enhanced Cone-Beam CT Technique to Diagnose Vertical Root Fracture.

Abstract:OBJECTIVES:To explore the feasibility of using sodium iodide (NaI)+dimethyl sulfoxide (DMSO)+ethyl alcohol+ethyl acetate as a cone-beam CT (CBCT) contrast agent in the diagnosis of vertical root fracture (VRF). METHODS:21 endodontically treated VRF teeth of 21 patients were collected in this study. All these 21 teeth were confirmed subtle fracture lines under transillumination, the number and position of fracture lines were recorded. All these patients had CBCT routine scanning (RS1) before extraction. After extraction, the teeth was performed micro-CT scanning and 3 in vitro CBCT scanning: CBCT routine scanning in vitro(RS2), CBCT enhanced scanning using meglumine diatrizoate (MD) as contrast agent(ES1); and CBCT enhanced scanning using NaI+DMSO+ethyl alcohol+ethyl acetate as contrast agent(ES2). The number of fracture lines was evaluated on all the 5 scanning modes and the accuracy of diagnosis was calculated. RESULTS:In all, there were 43 fracture lines on the 21 teeth. The accuracy of detection of fracture lines of CBCT RS1, RS2, ES1, ES2 and micro-CT was 0%, 20.9% (9/43), 11.6% (5/43), 93% (40/43) and 95.3% (41/43) respectively. Significant differences were found between ES2 vs. RS2, ES2 vs. ES1 (p < 0.01); however, no significant difference was found between ES2 vs. micro-CT (p > 0.05). CONCLUSION:CBCT enhanced scanning using NaI+DMSO+ethyl alcohol+ethyl acetate as contrast agent could be a prospective technique in the diagnosis of VRF.
Automated detection and classification of mandibular fractures on multislice spiral computed tomography using modified convolutional neural networks

Abstract:Objective: To evaluate the performance of convolutional neural networks (CNNs) for the automated detection and classification of mandibular fractures on multislice spiral computed tomography (MSCT). Study design: MSCT data from 361 patients with mandibular fractures were retrospectively collected. Two experienced maxillofacial surgeons annotated the images as ground truth. Fractures were detected utilizing the following models: YOLOv3, YOLOv4, Faster R-CNN, CenterNet, and YOLOv5-TRS. Fracture sites were classified by the following models: AlexNet, GoogLeNet, ResNet50, original DenseNet-121, and modified DenseNet-121. The performance was evaluated for accuracy, sensitivity, specificity, and area under the curve (AUC). AUC values were compared using the Z-test and P values <.05 were considered to be statistically significant. Results: Of all of the detection models, YOLOv5-TRS obtained the greatest mean accuracy (96.68%). Among all of the fracture subregions, body fractures were the most reliably detected (with accuracies of 88.59%-99.01%). For classification models, the AUCs for body fractures were higher than those of condyle and angle fractures, and they were all above 0.75, with the highest AUC at 0.903. Modified DenseNet-121 had the best overall classification performance with a mean AUC of 0.814. Conclusions: The modified CNN-based models demonstrated high reliability for the diagnosis of mandibular fractures on MSCT.

Abstract:Individual-level health behaviors and outcomes have multi-level determinants (individual and environmental). Multi-level analysis seeks to explain individual outcomes in terms of both individual and environmental or aggregate variables. Ecologic fallacy (improper inference about individual-level associations based on associations measured only at the aggregate level) can result from confusion about the level of inference that is of ultimate interest. The perspective of multi-level analysis acknowledges the importance of both individual and environmental variables in determining health behaviors and outcomes at the level of the indivisible unit--the individual. The authors review concepts and methods of multi-level analysis and their application to epidemiologic research on health behavior and health outcomes.
A hierarchical deep learning approach for diagnosing impacted canine-induced root resorption via cone-beam computed tomography

Abstract:Objectives: Canine-induced root resorption (CIRR) is caused by impacted canines and CBCT images have shown to be more accurate in diagnosing CIRR than panoramic and periapical radiographs with the reported AUCs being 0.95, 0.49, and 0.57, respectively. The aim of this study was to use deep learning to automatically evaluate the diagnosis of CIRR in maxillary incisors using CBCT images. Methods: A total of 50 cone beam computed tomography (CBCT) images and 176 incisors were selected for the present study. The maxillary incisors were manually segmented and labeled from the CBCT images by two independent radiologists as either healthy or affected by root resorption induced by the impacted canines. We used five different strategies for training the model: (A) classification using 3D ResNet50 (Baseline), (B) classification of the segmented masks using the outcome of a 3D U-Net pretrained on the 3D MNIST, (C) training a 3D U-Net for the segmentation task and use its outputs for classification, (D) pretraining a 3D U-Net for the segmentation and transfer of the model, and (E) pretraining a 3D U-Net for the segmentation and fine-tuning the model with only the model encoder. The segmentation models were evaluated using the mean intersection over union (mIoU) and Dice coefficient (DSC). The classification models were evaluated in terms of classification accuracy, precision, recall, and F1 score. Results: The segmentation model achieved a mean intersection over union (mIoU) of 0.641 and a DSC of 0.901, indicating good performance in segmenting the tooth structures from the CBCT images. For the main classification task of detecting CIRR, Model C (classification of the segmented masks using 3D ResNet) and Model E (pretraining on segmentation followed by fine-tuning for classification) performed the best, both achieving 82% classification accuracy and 0.62 F1-scores on the test set. These results demonstrate the effectiveness of the proposed hierarchical, data-efficient deep learning approaches in improving the accuracy of automated CIRR diagnosis from limited CBCT data compared to the 3D ResNet baseline model. Conclusion: The proposed approaches are effective at improving the accuracy of classification tasks and are helpful when the diagnosis is based on the volume and boundaries of an object. While the study demonstrated promising results, future studies with larger sample size are required to validate the effectiveness of the proposed method in enhancing the medical image classification tasks.
Development of a Predictive Model for Identifying Previously Undetected Vertical Root Fractures.

Abstract:This study aimed to develop a predictive model to screen for undetected vertical root fractures (VRFs) in root canal treated teeth. We included 95 root canal treated teeth with suspected VRFs; 77 for training and 18 for validation. Following clinical and cone-beam CT parameters were recorded: sex, tooth type, coronal restoration, time interval from completion of endodontic treatment to definitive diagnosis (TI), type of bone loss (BL), apical extent of root filling (AR) and the ratio of root filling diameter to the actual diameter in the coronal (1/3TA) and middle (2/3TA) root thirds. A predictive model p = 1/(1 - e-x ) was generated, where x = -7.433 + 1.977BL + 1.479 (2/3TA) + 1.102 AR; the sensitivity and specificity were 0.852 and 0.875 for training and 0.917 and 0.833 for validation. VRF teeth were more likely to have vertical bone loss and overfilled root canals. This model had a high diagnostic efficacy for VRFs.

Abstract:Introduction In dental clinical practice, cone-beam computed tomography (CBCT) is commonly used to assist practitioners to recognize the complex morphology of root canal systems. But due to its resolution limitations, certain small anatomical structures still cannot be accurately recognized on CBCT. The purpose of this study was to perform image super-resolution (SR) processing on CBCT images of extracted human teeth with the help of a deep learning model, and to compare the differences among CBCT, SRCT, and micro-computed tomography (Micro-CT) images through three-dimensional reconstruction. Methods The deep learning model (Basicvsr++) was selected and modified. The dataset consisted of 171 extracted teeth that met inclusion criteria, with 40 maxillary first molars as training set and 40 maxillary first molars as well as 91 teeth from other tooth positions as external test set. The corresponding CBCT, SRCT, and Micro-CT images of each tooth in test sets were reconstructed using Mimics Research 17.0, and the root canal recognition rates in the three groups were recorded. The following parameters were measured: volume of hard tissue(V1), volume of pulp chamber and root canal system(V2), length of visible root canals under orifice(VL-X, where X represents the specific root canal), intersection angle between coronal axis of canal and long axis of tooth(∠X, where X represents the specific root canal). Data were statistically analyzed between CBCT and SRCT images using paired sample t-test and Wilcoxon test analysis, with the measurement from Micro-CT images as the gold standard. Results Images from all tested teeth were successfully processed with super-resolution program. In 4-canal maxillary first molar, identification of MB2 was 72% (18/25) in CBCT group, 92% (23/25) in SRCT group, and 100% (25/25) in Micro-CT group. The difference of hard tissue volume between SRCT and Micro-CT was significantly smaller than that between CBCT and Micro-CT in all tested teeth except 4-canal mandibular first molar (P < 0.05). Similar results were obtained in volume of pulp chamber and root canal system in all tested teeth (P < 0.05). As for length of visible root canals under orifice, the difference between SRCT and Micro-CT was significantly smaller than that between CBCT and Micro-CT (P < 0.05) in most root canals. Conclusions The deep learning model developed in this study helps to optimize the root canal morphology of extracted teeth in CBCT. And it may be helpful for the identification of MB2 in the maxillary first molar.

Abstract:Introduction Orthodontically induced root resorption (OIRR) is a common and undesirable consequence of orthodontic treatment. Traditionally, studies employ manual methods to conduct 3-dimensional quantitative analysis of OIRR via cone-beam computed tomography (CBCT), which is often subjective and time-consuming. With advancements in computer technology, deep learning-based approaches have gained traction in medical image processing. This study presents a deep learning-based model for the fully automatic extraction of root volume information and the localization of root resorption from CBCT images. Methods In this cross-sectional, retrospective study, 4534 teeth from 105 patients were used to train and validate an automatic model for OIRR quantification. The protocol encompassed several steps: preprocessing of CBCT images involving automatic tooth segmentation and conversion into point clouds, followed by segmentation of tooth crowns and roots via the Dynamic Graph Convolutional Neural Network. The root volume was subsequently calculated, and OIRR localization was performed. The intraclass correlation coefficient was employed to validate the consistency between the automatic model and manual measurements. Results The proposed method strongly correlated with manual measurements in terms of root volume and OIRR severity assessment. The intraclass correlation coefficient values for average volume measurements at each tooth position exceeded 0.95 ( P <0.001), with the accuracy of different OIRR severity classifications surpassing 0.8. Conclusions The proposed methodology provides automatic and reliable tools for OIRR assessment, offering potential improvements in orthodontic treatment planning and monitoring.

Abstract:Background/purpose Vertical root fracture (VRF) is a prevalent reason for tooth extraction following root canal treatment and even after crown placement. Predicting fractures is challenging due to multifactorial nature. The current study aimed to predict the likelihood of fracture following root canal treatment and crown placement by developing a deep learning (DL) model. Materials and methods DL techniques were employed to analyze a dataset comprising 145 clinical cases consisting of 97 fractured teeth and 48 non-fractured teeth. This dataset spanned a five-year period and encompassed cases involving root canal therapy and crown installation. The analysis identified several root fracture-related parameters, which were incorporated into the DL system. The dataset consisted of 17 features presented in a mixed-type tabular format. Results The deep neural network (DNN) model surpassed the support vector machine (SVM) model with a higher accuracy (80.7 % vs. 71.7 %) and F1-score value (0.857 vs. 0.817) for predicting root fracture. Furthermore, in determining root fracture occurrence, it was observed that 17 significant characteristics in the DNN model outperformed the 7 features by 11.7 % in accuracy and 10 % in F1-score. Conclusion DL shows promise in predicting root fracture post root canal therapy and prosthesis, and it may have the potential to aid clinicians in assessing fracture risk and improving decision-making.

The preliminary in vitro study and application of deep learning algorithm in cone beam computed tomography image implant recognition

Abstract:To properly repair and maintain implants, which are bone tissue implants that replace natural tooth roots, it is crucial to accurately identify their brand and specification. Deep learning has demonstrated outstanding capabilities in analysis, such as image identification and classification, by learning the inherent rules and degrees of representation of data models. The purpose of this study is to evaluate deep learning algorithms and their supporting application software for their ability to recognize and categorize three dimensional (3D) Cone Beam Computed Tomography (CBCT) images of dental implants. By using CBCT technology, the 3D imaging data of 27 implants of various sizes and brands were obtained. Following manual processing, the data were transformed into a data set that had 13,500 two-dimensional data. Nine deep learning algorithms including GoogleNet, InceptionResNetV2, InceptionV3, ResNet50, ResNet50V2, ResNet101, ResNet101V2, ResNet152 and ResNet152V2 were used to perform the data. Accuracy rates, confusion matrix, ROC curve, AUC, number of model parameters and training times were used to assess the efficacy of these algorithms. These 9 deep learning algorithms achieved training accuracy rates of 100%, 99.3%, 89.3%, 99.2%, 99.1%, 99.5%, 99.4%, 99.5%, 98.9%, test accuracy rates of 98.3%, 97.5%, 94.8%, 85.4%, 92.5%, 80.7%, 93.6%, 93.2%, 99.3%, area under the curve (AUC) values of 1.00, 1.00, 1.00, 1.00, 1.00, 1.00, 1.00, 1.00, 1.00. When used to identify implants, all nine algorithms perform satisfactorily, with ResNet152V2 achieving the highest test accuracy, classification accuracy, confusion matrix area under the curve, and receiver operating characteristic curve area under the curve area. The results showed that the ResNet152V2 has the best classification effect on identifying implants. The artificial intelligence identification system and application software based on this algorithm can efficiently and accurately identify the brands and specifications of 27 classified implants through processed 3D CBCT images in vitro, with high stability and low recognition cost.

Abstract:Rapid and accurate detection of a C-shaped root canal on mandibular second molars can assist dentists in diagnosis and treatment. Oral panoramic radiography is one of the most effective methods of determining the root canal of teeth. There are already some traditional methods based on deep learning to learn the characteristics of C-shaped root canal tooth images. However, previous studies have shown that the accuracy of detecting the C-shaped root canal still needs to be improved. And it is not suitable for implementing these network structures with limited hardware resources. In this paper, a new lightweight convolutional neural network is designed, which combined with receptive field block (RFB) for optimizing feature extraction. In order to optimize the hardware resource requirements of the model, a lightweight, multi-branch, convolutional neural network model was developed in this study. To improve the feature extraction ability of the model for C-shaped root canal tooth images, RFB has been merged with this model. RFB has achieved excellent results in target detection and classification. In the multiscale receptive field block, some small convolution kernels are used to replace the large convolution kernels, which allows the model to extract detailed features and reduce the computational complexity. Finally, the accuracy and area under receiver operating characteristics curve (AUC) values of C-shaped root canals on the image data of our mandibular second molars were 0.9838 and 0.996, respectively. The results show that the deep learning model proposed in this paper is more accurate and has lower computational complexity than many other similar studies. In addition, score-weighted class activation maps (Score-CAM) were generated to localize the internal structure that contributed to the predictions.
