Abstract:Abstract BACKGROUND AND AIMS A major barrier to the routine adoption of total kidney volume (TKV) as a clinical biomarker of disease for autosomal dominant polycystic disease (ADPKD) is the significant human operator time required even by experienced analysts (typically, 45–90 min per patient). Several groups have investigated automated and semi-automated kidney segmentation methods to either reduce or eliminate the human interaction required. However, such tools have mostly been developed using data from single centers, which may not translate well to other centers. To date, there has been little attempt to develop or validate algorithms using multi-center and multi-scanner data. Here, we report an automated segmentation tool capable of high performance across different patient populations and scanner sequences using 1.5 T MRI data from four centers (the CYSTic consortium). The algorithm was subsequently tested in a separate clinical cohort to assess its likely performance during routine clinical use. METHOD All 1.5 T studies from the CYSTic trial were downloaded (acquired from Siemens Avanto, GE Optima and Siemens Aera, using different sequences). Cases with poor image quality or with sections of kidney missing from the field of view were excluded. A single, experienced operator selected the most appropriate image series for segmentation and manually segmented each patient's kidneys using a commercial software program (MIM Encore). There were 454 kidneys segmented from 227 scans. These data were used for algorithm training and validation. In addition, 48 routine clinical scans from the Sheffield 3D Lab were extracted from the archives along with their original segmentations (performed by six different analysts), to use as a test set. None of the patients in the clinical test set were included in the training set. An ensemble U-net algorithm was created using the nnUNet approach Isensee et al. (nnU-Net: a self-configuring method for deep learning-based biomedical image segmentation. Nat Meth 2021; 18(2): 203–211), whereby the CYSTic data were used in a 5-fold cross-validation, with stratification across the four centers (i.e. each center contributed 80% of the available data to algorithm training in each fold). Algorithm training proceeded according to the standard heuristic nnUnet functions, using a 3D architecture, for 100 epochs. Segmented kidneys were split into left and right sides during post-processing, through analysis of the position of the center of gravity of segmented regions. Once trained, the five algorithms from cross-validation were applied in an ensemble to the clinical test cohort. RESULTS In both cross-validation and clinical testing phases, the median DICE score was 0.96 for each kidney (IQR of 0.95–0.97 in cross-validation on both sides, And 0.95–0.97 on the left side for clinical testing and 0.96 for the right). The median total kidney volume error was −0.46% (−2.02 to 1.27) for the left side in cross-validation and −0.82% (−2.55 to 0.86) for the right. In the clinical testing phase, the median volume errors were −1.8% (−3.69 to 1.29), left and −1.79% (−3.95 to 0.65), right. The mean time taken to manually segment kidneys in the CYSTIc dataset was 54 min per scan (SD of 31 min). Use of the algorithm as a first pass segmentation, with subsequent checking and editing by an operator, would significantly reduce human input time to a few minutes per case CONCLUSION Our new algorithm demonstrates high accuracy compared to the gold standard of manual TKV segmentation and performs well in a wide range of patients with ADPKD imaged using different scanners at several European centers. Its high performance in a real-world clinical dataset demonstrates that such tools can provide a reliable means of measuring TKV in routine practice and reduces the previous barrier of analyst time and experience.

Abstract:Electron microscopic studies of rat virus and bluetongue vaccine virus (BTVV) infections of the subventricular zone (SVZ) of neonatal hamsters were carried out five days after inoculation. Although rat virus infected ventricular lining cells and BTVV did not, cell populations of the SVZ were infected by both viruses. At this time, when viruses had reached maximal multiplication and inflammation was not yet present, more cell destruction was evident in BTVV infected animals. In both infections blood vessels appeared normal without evidence of endothelial cell infection.
A Deep Learning Approach for Automated Segmentation of Kidneys and Exophytic Cysts in Individuals with Autosomal Dominant Polycystic Kidney Disease

Abstract:Background: Total kidney volume (TKV) is an important imaging biomarker in autosomal dominant polycystic kidney disease (ADPKD). Manual computation of TKV, particularly with the exclusion of exophytic cysts, is laborious and time consuming. Methods: We developed a fully automated segmentation method for TKV using a deep learning network to selectively segment kidney regions while excluding exophytic cysts. We used abdominal T2 -weighted magnetic resonance images from 210 individuals with ADPKD who were divided into two groups: one group of 157 to train the network and a second group of 53 to test it. With a 3D U-Net architecture using dataset fingerprints, the network was trained by K-fold cross-validation, in that 80% of 157 cases were for training and the remaining 20% were for validation. We used Dice similarity coefficient, intraclass correlation coefficient, and Bland-Altman analysis to assess the performance of the automated segmentation method compared with the manual method. Results: The automated and manual reference methods exhibited excellent geometric concordance (Dice similarity coefficient: mean±SD, 0.962±0.018) on the test datasets, with kidney volumes ranging from 178.9 to 2776.0 ml (mean±SD, 1058.5±706.8 ml) and exophytic cysts ranging from 113.4 to 2497.6 ml (mean±SD, 549.0±559.1 ml). The intraclass correlation coefficient was 0.9994 (95% confidence interval, 0.9991 to 0.9996; P<0.001) with a minimum bias of -2.424 ml (95% limits of agreement, -49.80 to 44.95). Conclusions: We developed a fully automated segmentation method to measure TKV that excludes exophytic cysts and has an accuracy similar to that of a human expert. This technique may be useful in clinical studies that require automated computation of TKV to evaluate progression of ADPKD and response to treatment.
Novel semi-automated kidney volume measurements in autosomal dominant polycystic kidney disease

Abstract:Background: We assessed the effectiveness and convenience of a novel semi-automatic kidney volume (KV) measuring high-speed 3D-image analysis system SYNAPSE VINCENT® (Fuji Medical Systems, Tokyo, Japan) for autosomal dominant polycystic kidney disease (ADPKD) patients. Methods: We developed a novel semi-automated KV measurement software for patients with ADPKD to be included in the imaging analysis software SYNAPSE VINCENT®. The software extracts renal regions using image recognition software and measures KV (VINCENT KV). The algorithm was designed to work with the manual designation of a long axis of a kidney including cysts. After using the software to assess the predictive accuracy of the VINCENT method, we performed an external validation study and compared accurate KV and ellipsoid KV based on geometric modeling by linear regression analysis and Bland-Altman analysis. Results: Median eGFR was 46.9 ml/min/1.73 m2. Median accurate KV, Vincent KV and ellipsoid KV were 627.7, 619.4 ml (IQR 431.5-947.0) and 694.0 ml (IQR 488.1-1107.4), respectively. Compared with ellipsoid KV (r = 0.9504), Vincent KV correlated strongly with accurate KV (r = 0.9968), without systematic underestimation or overestimation (ellipsoid KV; 14.2 ± 22.0%, Vincent KV; - 0.6 ± 6.0%). There were no significant slice thickness-specific differences (p = 0.2980). Conclusions: The VINCENT method is an accurate and convenient semi-automatic method to measure KV in patients with ADPKD compared with the conventional ellipsoid method.
Feasibility of artificial intelligence-based decision supporting system in tolvaptan prescription for autosomal dominant polycystic kidney disease

Abstract:Purpose: Total kidney volume (TKV) measurement is crucial for selecting treatment candidates in autosomal dominant polycystic kidney disease (ADPKD). We developed and investigated the performance of fully-automated 3D-volumetry model and applied it to software as a service (SaaS) for clinical support on tolvaptan prescription in ADPKD patients. Materials and methods: Computed tomography scans of ADPKD patients taken between January 2000 and June 2022 were acquired from seven institutions. The quality of the images was manually reviewed in advance. The acquired dataset was split into training, validation, and test datasets at a ratio of 8.5:1:0.5. Convolutional, neural network-based automatic segmentation model was trained to obtain 3D segment mask for TKV measurement. The algorithm consisted of three steps: data preprocessing, ADPKD area extraction, and post-processing. After performance validation with the Dice score, 3D-volumetry model was applied to SaaS which is based on Mayo imaging classification for ADPKD. Results: A total of 753 cases with 95,117 slices were included. The differences between the ground-truth ADPKD kidney mask and the predicted ADPKD kidney mask were negligible, with intersection over union >0.95. The post-process filter successfully removed false alarms. The test-set performance was homogeneously equal and the Dice score of the model was 0.971; after post-processing, it improved to 0.979. The SaaS calculated TKV from uploaded Digital Imaging and Communications in Medicine images and classified patients according to height-adjusted TKV for age. Conclusions: Our artificial intelligence-3D volumetry model exhibited effective, feasible, and non-inferior performance compared with that of human experts and successfully predicted the rapid ADPKD progressor.

Abstract:PurposeIn ADPKD patients total kidney volume (TKV) measurement using MRI is performed to predict rate of disease progression. Historically T1 weighted images (T1) were used, but the methodology of T2 weighted imaging (T2) has evolved. We compared the performance of both sequences.Methods40 ADPKD patients underwent an abdominal MRI at baseline and follow-up. TKV was measured by manual tracing with Analyze Direct 11.0 software. Three readers established intra- and interreader coefficients of variation (CV). T1 and T2 measured kidney volumes and growth rates were compared with ICC and Bland–Altman analyses.ResultsParticipants were 49.7 ± 7.0 years of age, 55.0% female, with estimated GFR of 50.1 ± 11.5 mL/min/1.73 m2. CVs were low and comparable for T2 and T1 (intrareader: 0.83% [0.48–1.79] vs. 1.15% [0.34–1.77], P = 0.9, interreader: 2.18% [1.59–2.61] vs. 1.69% [1.07–3.87], P = 0.9). TKV was clinically similar, but statistically significantly different between T2 and T1: 1867 [1172–2721] vs. 1932 [1180–2551] mL, respectively (P = 0.006), with a bias of only 0.8% and high agreement (ICC 0.997). Percentage kidney growth during 2.2 ± 0.3 years was similar for T2 and T1 (9.3 ± 10.6% vs. 7.8 ± 9.9%, P = 0.1, respectively), with a bias of 1.5% and high agreement (ICC 0.843). T2 was more often of sufficient quality for volume measurement (86.7% vs. 71.1%, P < 0.001).ConclusionsIn patients with ADPKD, measurement of kidney volume and growth rate performs similarly when using T2 compared to T1 weighted images, although T2 performs better on secondary outcome parameters; they are more often of sufficient quality for volume measurement and result in slightly lower intra- and interreader variability.

Abstract:Polycystic kidney disease (Autosomal Dominant Polycystic Kidney Disease, ADPKD) is the most common genetic disease of the kidney, and the measurement of Total Kidney Volume (TKV) in clinical research of this disease is essential to study the progression of ADPKD. At present, the volume segmentation of polycystic kidneys mainly relies on doctors to manually outline the kidney boundary on the radiological image. This process is time-consuming, labor-intensive, inefficient, subjective, and difficult to guarantee consistency. In the research of this paper, A multi-module hybrid U-shape segmentation method is proposed (HUNet), which introduces wavelet pooling, cascade residual, and efficient multi-head self-attention into the U-shape structure. We use wavelet pooling instead of traditional down-sampling to reduce the loss of detailed features, the use of cascaded residual modules can improve the ability of model feature reuse, and the use of efficient multi-head self-attention modules can effectively capture global multi-scale information. In the decoding process of the U-shape, the corresponding loss value of each decoder will be calculated, and finally, the total loss value of the model will be obtained by weighted average. The method was trained and tested on the polycystic kidney dataset provided by Shanghai Changzheng Hospital. We automatically segmented the ADPKD in MRI images using the proposed method with a remarkably high Dice similarity coefficient relative to the manual segmentation (mean=0.915). The percentage difference between the total kidney volume values using manual and HUNet methods was only 0.4%. The proposed approach enables fast and accurate TKV measurement.

Abstract:Background:: Autosomal Dominant Polycystic Kidney Disease (ADPKD) is a genetic disorder that causes uncontrolled kidney cyst growth, leading to kidney volume enlargement and renal function loss over time. Total kidney volume (TKV) and cyst burdens have been used as prognostic imaging biomarkers for ADPKD. Objective:: This study aimed to evaluate nnUNet for automatic kidney and cyst segmentation in T2-weighted (T2W) MRI images of ADPKD patients. Methods:: 756 kidney images were retrieved from 95 patients in the Consortium for Radiologic Imaging Studies of Polycystic Kidney Disease (CRISP) cohort (95 patients × 2 kidneys × 4 follow-up scans). The nnUNet model was trained, validated, and tested on 604, 76, and 76 images, respectively. In contrast, all images of each patient were exclusively assigned to either the training, validation, or test sets to minimize evaluation bias. The kidney and cyst regions defined using a semi-automatic method were employed as ground truth. The model performance was assessed using the Dice Similarity Coefficient (DSC), the intersection over union (IoU) score, and the Hausdorff distance (HD). Results:: The test DSC values were 0.96±0.01 (mean±SD) and 0.90±0.05 for kidney and cysts, respectively. Similarly, the IoU scores were 0.91± 0.09 and 0.81±0.06, and the HD values were 12.49±8.71 mm and 12.04±10.41 mm, respectively, for kidney and cyst segmentation. Conclusion:: The nnUNet model is a reliable tool to automatically determine kidney and cyst volumes in T2W MRI images for ADPKD prognosis and therapy monitoring.

Abstract:Abdominal imaging of autosomal dominant polycystic kidney disease (ADPKD) has historically focused on detecting complications such as cyst rupture, cyst infection, obstructing renal calculi, and pyelonephritis; discriminating complex cysts from renal cell carcinoma; and identifying sources of abdominal pain. Many imaging features of ADPKD are incompletely evaluated or not deemed to be clinically significant, and because of this, treatment options are limited. However, total kidney volume (TKV) measurement has become important for assessing the risk of disease progression (i.e., Mayo Imaging Classification) and predicting tolvaptan treatment's efficacy. Deep learning for segmenting the kidneys has improved these measurements' speed, accuracy, and reproducibility. Deep learning models can also segment other organs and tissues, extracting additional biomarkers to characterize the extent to which extrarenal manifestations complicate ADPKD. In this concept paper, we demonstrate how deep learning may be applied to measure the TKV and how it can be extended to measure additional features of this disease.

SAM-based Automatic Workflow for Histology Cyst Segmentation in Autosomal Dominant Polycystic Kidney Disease

Abstract:Autosomal Dominant Polycystic Kidney Disease (ADPKD) is a genetic disorder characterized by the development of numerous cysts in the kidneys, ultimately leading to significant structural alterations and renal failure. Detailed investigations of this disease frequently utilize histological analyses of kidney sections across various stages of ADPKD progression. In this paper, we introduce an automated workflow leveraging the Segment Anything Model (SAM) neural network, complemented by a series of post-processing steps, to autonomously segment cysts in histological images. This approach eliminates the need for manual annotations or preliminary training phases and enables precise quantification of cystic changes over entire kidney sections. Application of this method to sequential histology images across the development timeline of ADPKD in mice demonstrated a notable increase in the proportion of diseased tissue from 8 to 12 weeks and from 12 to 16 weeks, with the cysts appearing progressively lighter. Our workflow not only surpasses the performance of the existing Cystanalyser tool but also offers enhanced flexibility and accuracy in full-image segmentation. The developed workflow is made publicly accessible to facilitate its adoption as an efficient tool for rapid and reliable cyst segmentation in histological studies.

Abstract:Autosomal Dominant Polycystic Kidney Disease (ADPKD) is the most prevalent kidney genetic disorder, producing structural abnormalities and impaired function. This research investigates its evolution on mouse models, utilizing a combination of histology imaging, Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) to evaluate its progression thoroughly. ADPKD has been induced in mice via PKD2 gene knockout, followed by image acquisition at different stages. Histology data provides two-dimensional details, like the cystic area ratio, whereas CT and MRI facilitate three-dimensional temporal monitoring. Our approach allows to quantify the affected tissue at different disease stages through multiple quantitative metrics. A pivotal point is shown at approximately ten weeks after induction, marked by a swift acceleration in disease advancement, and leading to a notable increase in cyst formation. This multimodal strategy augments our comprehension of ADPKD dynamics and suggests the possibility of employing higher-resolution imaging in the future for more accurate volumetric analyses.

Abstract:Mayo Imaging Classification (MIC) for predicting future kidney growth in autosomal dominant polycystic kidney disease (ADPKD) patients is calculated from a single MRI/CT scan assuming exponential kidney volume growth and height-adjusted total kidney volume at birth to be 150 mL/m. However, when multiple scans are available, how this information should be combined to improve prediction accuracy is unclear. Herein, we studied ADPKD subjects ( ) with 8+ years imaging follow-up (mean = 11 years) to establish ground truth kidney growth trajectory. MIC annual kidney growth rate predictions were compared to ground truth as well as 1- and 2-parameter least squares fitting. The annualized mean absolute error in MIC for predicting total kidney volume growth rate was compared to ( ) for a 2-parameter fit to the same exponential growth curve used for MIC when 4 measurements were available or ( ) with 3 measurements averaging together with MIC. On univariate analysis, male sex ( ) and PKD2 mutation ( ) were associated with poorer MIC performance. In ADPKD patients with 3 or more CT/MRI scans, 2-parameter least squares fitting predicted kidney volume growth rate better than MIC, especially in males and with PKD2 mutations where MIC was less accurate.

Abstract:Autosomal Dominant Polycystic Kidney Disease (ADPKD) characterized by progressive growth of renal cysts is the most prevalent and potentially lethal monogenic renal disease, affecting one in every 500-100 people. Total Kidney Volume (TKV) and its growth computed from Computed Tomography images has been accepted as an essential prognostic marker for renal function loss. Due to large variation in shape and size of kidney in ADPKD, existing methods to compute TKV (i.e. to segment ADKP) including those based on 2D convolutional neural networks are not accurate enough to be directly useful in clinical practice. In this work, we propose multi-task 3D Convolutional Neural Networks to segment ADPK and achieve a mean DICE score of 0.95 and mean absolute percentage TKV error of 3.86. Additionally, to solve the challenge of class imbalance, we propose to simply bootstrap cross entropy loss and compare results with recently prevalent dice loss in medical image segmentation community.

Abstract:Purpose The increasing incidence of kidney diseases is a global concern, and current biomarkers and treatments are inadequate. Changes in renal tubule luminal volume fraction (TVF) serve as a rapid biomarker for kidney disease and improve understanding of renal (patho)physiology. This study uses the amplitude of the long T2 component as a surrogate for TVF in rats, by applying multiexponential analysis of the T2‐driven signal decay to examine micromorphological changes in renal tissue. Methods Simulations were conducted to identify a low mean absolute error (MAE) protocol and an accelerated protocol customized for the in vivo study of T2 mapping of the rat kidney at 9.4 T. Abstract:Introduction: In the pediatric context, most children with autosomal dominant polycystic kidney disease (ADPKD) maintain a normal glomerular filtration rate (GFR) despite underlying structural kidney damage, highlighting the critical need for early intervention and predictive markers. Due to the inverse relationship between kidney volume and kidney function, risk assessments have been presented on the basis of kidney volume. The aim of this study was to use magnetic resonance imaging (MRI)-based kidney volume assessment for risk stratification in pediatric ADPKD and to investigate clinical and genetic differences among risk groups. Methods: This multicenter, cross-sectional, and case-control study included 75 genetically confirmed pediatric ADPKD patients (5–18 years) and 27 controls. Kidney function was assessed by eGFR calculated from serum creatinine and cystatin C using the CKiD-U25 equation. Blood pressure was assessed by both office and 24-hour ambulatory measurements. Kidney volume was calculated from MRI using the stereological method. Total kidney volume was adjusted for the height (htTKV). Patients were stratified from A to E classes according to the Leuven Imaging Classification (LIC) using MRI-derived htTKV. Results: Median (Q1-Q3) age of the patients was 6.0 (2.0–10.0) years, 56% were male. There were no differences in sex, age, height-SDS, or GFR between the patient and control groups. Of the patients, 89% had PKD1 and 11% had PKD2 mutations. Non-missense mutations were 73% in PKD1 and 75% in PKD2. Twenty patients (27%) had hypertension based on ABPM. Median htTKV of the patients was significantly higher than controls (141 vs. 117 ml/m, p = 0.0003). LIC stratification revealed Classes A (38.7%), B (28%), C (24%), and D + E (9.3%). All children in class D + E and 94% in class C had PKD1 variants. Class D + E patients had significantly higher blood pressure values and hypertension compared to other classes ( p > 0.05 for all). Discussion: This study distinguishes itself by using MRI-based measurements of kidney volume to stratify pediatric ADPKD patients into specific risk groups. It is important to note that PKD1 mutation and elevated blood pressure were higher in the high-risk groups stratified by age and kidney volume. Our results need to be confirmed in further studies.

Abstract:Background: Kidney volume provides important information for the diagnosis and prognosis of autosomal dominant polycystic kidney disease (ADPKD), as well as for the evaluation of the effects of drugs such as tolvaptan. Non-contrast computed tomography (CT) is commonly used for volumetry, and this study examined the correspondence and correlation of kidney volume measured by standard-dose or low-dose CT. Methods: Axial standard-dose and low-dose CT images with 1-mm slices were obtained from 24 ADPKD patients. The kidney was segmented in the Synapse 3D software and the kidney volume was calculated using stereology. The kidney volume was compared between the two sets of images using R2, Bland-Altman plots, coefficient of variation, and intra-class correlation coefficients (ICCs). Results: The mean age of the 24 patients was 48.4 ± 10.9 years, and 45.8% were men (n = 11). The mean total kidney volume on standard-dose CT was 1501 ± 838.2 mL. The R2 of volume between standard-dose and low-dose CT was 0.995. In the Bland-Altman plot, except for one case with a large kidney volume, the two measurements were consistent, and the coefficient of variation and ICC were also good (0.02, 0.998). The CT radiation dose (dose-length product) was 229 ± 68 mGy·cm for standard-dose CT and 50 ± 19 mGy·cm for low-dose CT. A comparable volume was obtained with 20% of the radiation dose of standard-dose CT. Conclusions: Standard-dose and low-dose CT showed comparable kidney volume in ADPKD. Therefore, low-dose CT can substitute for ADPKD volumetry while minimizing radiation exposure.

Abstract:Autosomal dominant polycystic kidney disease (ADPKD) is a monogenic, rare disease, characterized by the formation of multiple cysts that grow out of the renal tubules. Despite intensive attempts to develop new drugs or repurpose existing ones, there is currently no definitive cure for ADPKD. This is primarily due to the complex and variable pathogenesis of the disease and the lack of models that can faithfully reproduce the human phenotype. Therefore, the development of models that allow automated detection of cysts' growth directly on human kidney tissue is a crucial step in the search for efficient therapeutic solutions. Artificial Intelligence methods, and deep learning algorithms in particular, can provide powerful and effective solutions to such tasks, and indeed various architectures have been proposed in the literature in recent years. Here, we comparatively review state-of-the-art deep learning segmentation models, using as a testbed a set of sequential RGB immunofluorescence images from 4 in vitro experiments with 32 engineered polycystic kidney tubules. To gain a deeper understanding of the detection process, we implemented both pixel-wise and cyst-wise performance metrics to evaluate the algorithms. Overall, two models stand out as the best performing, namely UNet++ and UACANet: the latter uses a self-attention mechanism introducing some explainability aspects that can be further exploited in future developments, thus making it the most promising algorithm to build upon towards a more refined cyst-detection platform. UACANet model achieves a cyst-wise Intersection over Union of 0.83, 0.91 for Recall, and 0.92 for Precision when applied to detect large-size cysts. On all-size cysts, UACANet averages at 0.624 pixel-wise Intersection over Union. The code to reproduce all results is freely available in a public GitHub repository.

Abstract:Abstract The E. & F. White Conference held in Sydney in December 1999 brought together expertise on a range of interference mitigation techniques from CSIRO, Australian and international industry and universities. Key goals were to enhance the understanding of techniques and their inter-relationship, to increase awareness of advanced technologies such as software radios and photonics, and to foster a cooperative approach to the development of interference mitigation techniques. The foremost application in mind was the square kilometre array (SKA) and the need to find ways to develop a hierarchical scheme for removing unwanted signals from astronomical data. This paper gives an overview of the topics discussed at the conference and summarises some of the key ideas and results that were presented.
Automatically Detecting Pancreatic Cysts in Autosomal Dominant Polycystic Kidney Disease on MRI Using Deep Learning

Abstract:Background: Pancreatic cysts in autosomal dominant polycystic kidney disease (ADPKD) correlate with PKD2 mutations, which have a different phenotype than PKD1 mutations. However, pancreatic cysts are commonly overlooked by radiologists. Here, we automate the detection of pancreatic cysts on abdominal MRI in ADPKD. Methods: Eight nnU-Net-based segmentation models with 2D or 3D configuration and various loss functions were trained on positive-only or positive-and-negative datasets, comprising axial and coronal T2-weighted MR images from 254 scans on 146 ADPKD patients with pancreatic cysts labeled independently by two radiologists. Model performance was evaluated on test subjects unseen in training, comprising 40 internal, 40 external, and 23 test–retest reproducibility ADPKD patients. Results: Two radiologists agreed on 52% of cysts labeled on training data, and 33%/25% on internal/external test datasets. The 2D model with a loss of combined dice similarity coefficient and cross-entropy trained with the dataset with both positive and negative cases produced an optimal dice score of 0.7 ± 0.5/0.8 ± 0.4 at the voxel level on internal/external validation and was thus used as the best-performing model. In the test–retest, the optimal model showed superior reproducibility (83% agreement between scan A and B) in segmenting pancreatic cysts compared to six expert observers (77% agreement). In the internal/external validation, the optimal model showed high specificity of 94%/100% but limited sensitivity of 20%/24%. Conclusions: Labeling pancreatic cysts on T2 images of the abdomen in patients with ADPKD is challenging, deep learning can help the automated detection of pancreatic cysts, and further image quality improvement is warranted.
