Abstract:A rare disorder called newborn hydronephrosis is mostly caused by the obstruction of the pyeloureteral junction. We describe a case study of a male neonate who underwent Anderson-Hynes pyeloplasty to effectively cure hydronephrosis in a single kidney that was further complicated by early renal failure. Considering kidney failure can result in progressive renal fibrosis, early management is essential. The gold standard is Anderson-Hynes pyeloplasty, which is usually advised for individuals who weigh over 10 kg. Percutaneous nephrostomy is frequently used in the early stages of care for newborns in order to reduce dilatation and restore renal elasticity. After surgery, the resolution of hydronephrosis may take up to 24 months. We note that pyeloureteral junction obstruction can appear as a single anomaly or a component of a multifactorial illness. This study aims to contribute to the discourse surrounding the optimal timing of Anderson-Hynes pyeloplasty in pediatric patients, providing insights into clinical management strategies and outcomes.

Abstract:Pyeloplasty is a standard therapy for ureteropelvic junction obstruction with a success rate of over 90% for all types of repair. In 1993 the first cases of laparoscopic repair of ureteropelvic junction obstruction via the transperitoneal approach were described.1 In the last few decades open pyeloplasty has been a standard for surgical treatment with a long-term success rate of more than 90%. The results have shown the feasibility of Anderson-Hynes dismembered pyeloplasty.2 In this report, we present our experience with transperitoneal laparoscopic dismembered pyeloplasty in 18 patients with ureteropelvic junction obstruction (UPJO) at our hospital between October 2001 and October 2002. All the operations were successful and the results satisfactory.

Abstract:Aim: We aimed to investigate the short-term efficacy and safety of laparoscopic pyeloplasty for treating newborns with severe hydronephrosis due to ureteropelvic junction obstruction (UPJO). Methods: A retrospective analysis was performed on 16 newborn patients with severe neonatal hydronephrosis who underwent laparoscopic pyeloplasty at our hospital from January 2021 to November 2022. All patients were regularly followed up. Laparoscopic pyeloplasty with double J stent placement was performed after the presence of severe hydronephrosis was confirmed. Results: Among the 16 pediatric patients (13 males, 3 females), the left side was affected in 13 cases and the right side in 3. The average age at surgery was 9.50 (8.50–12.00) days, with an average weight of 3.30 ± 0.95 kg. Laparoscopic pyeloplasty was performed in all cases without the need for open conversion. The mean surgery duration was 292.06 ± 73.60 min, with minimal blood loss (2.50, 2.00–5.00 ml). Postoperative hospital stays averaged 13.44 ± 4.70 days. No anastomotic leakage occurred, and follow-ups at 1, 3, 6, and 12 months showed no stent displacement, except for one case where the stent was removed at 1 month, and the others at 3 months. There were no cases of worsened hydronephrosis, except for one with renal atrophy at the 6-month follow-up. Changes in renal pelvis anteroposterior diameter exhibited a time effect ( F = 49.281, P < 0.001), with significant differences at 1, 3, 6, and 12 months postoperatively compared to preoperative values ( P < 0.05). Notably, differences were observed between 6 and 3 months, as well as between 12 and 3 months postoperatively ( P < 0.05). Similarly, renal parenchymal thickness changes showed a time effect Pediatric urology, Guangdong Women and Children Hospital, Guangzhou, China ( F = 49.281, P < 0.001), with significant differences at 1, 3, 6, and 12 months postoperatively compared to preoperative values ( P < 0.05). Significant differences were also noted between 6 and 1 month, as well as between 12 and 1 month postoperatively ( P < 0.05). There was one case of urinary tract infection after surgery, and no case of recurrence was observed. Conclusion: Severe neonatal hydronephrosis must be treated promptly. Laparoscopic pyeloplasty is a safe and feasible treatment with minimal complications for newborn patients with severe hydronephrosis due to UPJO.

Abstract:Authors describe a clinical case of a combination of a horseshoe kidney and hydronephrosis of its left half in a 3-year-old male patient and laparoscopic pyeloplasty of the identified pathology. Based on the results of preoperative CT, the diagnosis was confirmed and obstruction of the pyeloureteral segment was found as a source of hydronephrosis. Laparoscopic surgery made it possible to clarify the anatomy of the horseshoe kidney and the reason for the pyeloureteral segment obstruction in the form of a high discharge of the ureter from the pelvis. The Anderson-Hynes pyeloplasty with pelvic stenting was performed. The duration of the surgical intervention was 70 minutes. The need for analgesia persisted for 1 day (paracetamol 15 mg/kg/8 h). The drainage tube was removed after a follow-up ultrasound examination on the 3rd day, after which the patient was discharged for outpatient follow-up. The duration of follow-up was 6 months. The patient was asymptomatic throughout the control period. Repeated ultrasound performed 6 months after the surgical intervention revealed a decrease in the anteroposterior diameter of the pelvis to 8 mm. Conclusion: laparoscopic pyeloplasty is a feasible and safe procedure in patients with complex renal anatomy accompanied by the pyeloureteral segment obstruction. Accurate preoperative planning and standardized technique are the keys to successful treatment.

Abstract:Summary Ureteropelvic junction obstruction (UPJO) can be treated by various pyeloplasty techniques. We present a hybrid technique incorporating elements of laparoendoscopic single-site surgery and open pyeloplasty through a single umbilical incision. As a result, seven infants with UPJO underwent the hybrid pyeloplasty smoothly. The mean operative time was 131.9 min. At a follow-up of 11.8–50.0 months, all infants showed significant improvement and no symptoms except for one febrile urinary tract infection. The cosmetic results were very satisfactory without obvious visible scars. Therefore, the hybrid pyeloplasty appears to be a simple and effective minimally invasive surgery for treating infant UPJO.

Abstract:Introduction: In children, the association of ipsilateral pyeloureteral junction obstruction (PUJO) and ureterovesical junction obstruction (UVJO) is a rare malformation with a non-standardized treatment. We report a case of PUJO and UVJO treated by a combined minimally invasive surgical treatment to resolve the double urinary obstruction. The current literature was also reviewed. Case report and review: A two-month-old boy, without antenatal and postnatal signs of urinary tract anomalies, was hospitalized presenting right hydronephrosis, perirenal fluid effusion, and ascites. An acute pelvic rupture was suspected, and a retrograde pyelogram was performed, showing a primary obstructive megaureter (POM) associated with a corkscrew pyeloureteral junction. The impossibility to place a double J catheter through the pyeloureteral junction led us to achieve percutaneous nephrostomy and an abdominal drain placement. Three months later, the patient underwent a combined high-pressure balloon ureterovesical junction dilation and retroperitoneoscopic Anderson Hynes one-trocar-assisted pyeloplasty (OTAP). The literature search identified 110 children experiencing double urinary tract obstruction. All authors agreed on the difficulty to diagnose both obstructions preoperatively, but there is still no consensus on which obstruction should be relieved earlier, because the alteration in urinary vascularity during a double surgery could damage the ureter. Conclusions: The simultaneous occurrence of UPJO and UVJO is rare, with a challenging diagnosis. Prompt identification and timely surgical intervention are crucial to mitigate the risk of renal function loss attributable to obstruction and infection. Drawing from our expertise and the analysis of the existing literature, we propose employing a simultaneous double minimally invasive strategy in order to optimize the preservation of ureteral vascularity. This approach entails performing a minimally invasive pyeloplasty for the PUJ and utilizing high-pressure balloon dilatation for the UVJ.

Abstract:Objective: To present our primary experience of robotic-assisted laparoscopic pyeloplasty (RALP) for severe ureteropelvis junction obstruction (UPJO) infants under 3 months. Methods: We performed a retrospective study of 9 infants under 3 months who underwent RALP for severe UPJO between April 2017 and March 2019 in our center. The severe UPJO was defined as infants with severe hydronephrosis (Society of Fetal Urology grades III or IV, anteroposterior diameter >3 cm or split renal function <40% or T 1/2 >20 min) involving bilateral, solitary kidney, or contralateral renal hypoplasia UPJO at the same time. All clinical, perioperative, and postoperative information was collected. Results: There were four bilateral UPJO cases, two solitary kidney UPJO cases and three unilateral UPJO with contralateral renal hypoplasia cases included. One single surgeon performed RALP on all of the infants. The mean age of the infants was 1.62 ± 0.54 months. The mean operative time was 109.55 ± 10.47 min. The mean estimated blood loss was 19.29 ± 3.19 ml, and the mean length of hospital stay was 5.57 ± 0.73 days. According to the ultrasonography results, all patients had a significant recovery of renal function at 12 months after the operation. Conclusions: To maximize the protection of renal function, early RALP is a safe and feasible option for the treatment of severe UPJO in infants under 3 months.

Abstract:Objective To report a retroperitoneal laparoscopic surgery for ureteropelvic junction obstruction (UPJO) by double renal veins.Methods A 28-year-old male patient with left low back pain for 6 months was diagnosed as left hydronephrosis and UPJO.A ureteral stent had been placed 3 months before and failed to improve hydronephrosis,so the ureteral stent was pulled out.CT scan showed that left UPJ went through the two renal veins,suggesting UPJO.Nephrogram showed that left GFR and right GFR were 35 ml/min and 34 ml/min,respectively.These results indicated mechanical obstruction of left upper urinary tract.The patient underwent retroperitoneal laparoscopic Anderson-Hynes dismembered pyeloplasty.Results The surgical procedure was successful.Two left renal veins were found,and the ventral one was in front of ureter,compressing the ureter.An aberrant renal artery went into left kidney with the ureter.0.5 cm stenosis of ureteropelvic junction was excised,and pyeloplasty was performed.A ureteral stent was placed into the ureter,then the pelvis and the ureter were sew up in front of the ventral renal vein.The surgical time was 240 min,and blood loss was 50 ml.Postoperative hospital stay time was 4 d.During 4 months' follow up,hydronephrosis was attenuated significantly.Conclusion Retroperitoneal laparoscopic surgery for UPJO with aberrant two renal vein might be a minimally-invasive and effective procedure.
Outcomes of Robot-Assisted Laparoscopic Pyeloplasty for Ureteropelvic Junction Obstruction with Small Pelvis in Children

Abstract:Objective:To summarize the experience of robotic-assistant laparoscopic pyeloplasty (RALP) for ureteropelvic junction obstruction (UPJO) with small pelvis in children.Methods:A retrospective analysis was performed on clinical data of 12 UPJO children (small pelvis) undergoing RALP from March 2018 to October 2019. Their clinical presentations, operative approaches and follow-up data were analyzed. There were 7 boys and 5 girls with a mean age of 7(4-15) years. The involved side was left ( n=9) and right ( n=3). Ultrasound at 1-7 days preoperatively indicated anterioposterior diameter (APD)<2.5 cm and magnetic resonance urography or computed tomography urogram (MRU/CTU) indicated obstruction at ureteropelvic junction. Ultrasound indicated APD>3.5 cm during an on set of abdominal pain. And radionuclide examination hinted at different declining levels of renal function and the function of affected kidney ranged from 6% to 43%. Results:RALP was performed all successfully without a conversion into open surgery. The pathogenesis was simple stricture or torsion ( n=3), polyps ( n=3), ectopic vascular compression ( n=4), calculus incarceration with infection ( n=1) and postcaval ureter ( n=1). The average operative duration was 165(115-225) min and the average ureteropelvic anastomosis time 50(40-60) min. There were no obvious intraoperative complications and the mean volume of blood loss was <10 ml. The average follow-up period was 12(2-22) months. The mean preoperative value of APD was (1.87±0.57)cm and the mean postoperative value of APD (1.16±0.82)cm ( P<0.05). Conclusions:Robot-assisted laparoscopic pyeloplasty may be employed for pediatric UPJO with small renal pelvis (APD<2.5 cm). Such a technique is easy to learn and clinical application prospects are excellent.

Abstract:OBJECTIVES To assess the feasibility and efficacy of retroperitoneal laparoscopic dismembered Anderson-Hynes pyeloplasty in the treatment Of ureteropelvic junction obstruction (UPJO). METHODS Between August 2003 and June 2007, retroperitoneal laparoscopic dismembered Anderson-Hynes pyeloplasty was performed in 150 patients diagnosed with UPJO. All patients were assessed preoperatively by imaging examinations (ultrasonography, computerized tomography, and intravenous or retrograde urography) and isotope diuretic renography. Additionally, of the 150 cases, 16 patients with renal calculi underwent concomitant pyelolithotomy. RESULTS All surgeries were successful. No conversion to open surgery was observed. The average operating time, blood loss, and postoperative hospital stay was 105 minutes (range 95-190), 35 mL (range 20-80), and 7.4 days (range 6-12), respectively. A crossing vessel was encountered in 70 (46.7%) patients. Concomitant renal calculi were successfully removed in 11 patients. Two cases presented with urinary leakage that disappeared on the 11th and 12th postoperative day, respectively. Success rate was 98% at a mean follow-up of 16 months (range 12-24). CONCLUSIONS Retroperitoneal laparoscopic pyeloplasty provides a minimal invasive treatment modality with low morbidity, short hospital stay, and quick postoperative recovery for patients with UPJO. Robot-assisted modified bilateral dismembered V-shaped flap pyeloplasty for ureteropelvic junction obstruction in horseshoe kidney using KangDuo-Surgical-Robot-01 system

Abstract:Purpose: Horseshoe kidney (HSK) is the most common renal fusion anomaly, occurring in 0.25% of the population (1). It presents technical obstacles to pyeloplasty for ureteropelvic junction obstruction (UPJO) despite robotic assistance (2, 3). KangDuo-Surgical-Robot-01 (KD-SR-01), an emerging robotic platform in China, has yielded satisfactory outcomes in pyeloplasty (4, 5). We first describe our modified technique of robotic bilateral pyeloplasty for UPJO in HSK using KD-SR-01 system in the Lithotomy Trendelenburg position. Materials and methods: A 36-year-old man with HSK and bilateral UPJO suffered right flank pain due to renal calculi (Figure-1). Repeated double-J stent insertion and ureteroscopy lithotripsy did not relieve his symptoms. A robot-assisted modified bilateral dismembered V-shaped flap pyeloplasty was performed using KD-SR-01 system in the Lithotomy Trendelenburg position. Results: Total operative time was 298 minutes with 50 ml estimated blood loss. There was no conversion to laparoscopic or open surgery. A follow-up of 14 months showed relieving symptoms and stable renal function. Cine magnetic resonance urography and computed tomography urography revealed improved hydronephrosis and good drainage. No intraoperative or postoperative complications occurred. Conclusions: It is technically feasible to perform a KD-SR-01-assisted modified bilateral dismembered V-shaped flap pyeloplasty in the Lithotomy Trendelenburg position for HSK. This procedure achieves managing UPJO on both sides without redocking the system and provides a wider operative field. In addition, it may be associated with better ergonomics, better cosmetic outcomes, and less possibility of postoperative bowel adhesion. However, further investigation is still warranted to confirm its safety, efficacy, and advantages over traditional procedures.

Abstract:This paper discusses “The Cybernetics Thought Collective: A History of Science and Technology Portal Project,” a collaborative effort among four institutions that maintain archival records vital to the exploration of cybernetic history — the University of Illinois at Urbana-Champaign, the American Philosophical Society, the British Library, and MIT. With recent grant funding from the NEH, the multi-institutional team is developing a prototype web-portal and analysis-engine to provide access to archival material related to the development of the field of cybernetics, which influenced the development of modern computing and provided a common language to articulate similar questions about behavior across disciplines — regardless of whether the subject of study was animal, machine, or social group. The project is also enabling the digitization of the personal archives of four founders of cybernetics — Heinz von Foerster, W. Ross Ashby, Warren S. McCulloch, and Norbert Wiener. Using computational methods based on advanced machine-learning algorithms to yield network and entity relationships maps from the digitized texts, this project seeks to create access to archival material that enables humanities scholars to better understand the development of cybernetic ideas and to enable scientists and engineers to reuse and access cybernetic data.
Efficacy of laparoscopic pyeloplasty for pediatric hydronephrosis caused by symptomatic versus asymptomatic endogenous ureteropelvic junction obstruction: a retrospective analysis

Abstract:Journal of International Medical Research, Volume 52, Issue 3, March 2024. ObjectiveTo retrospectively compare the differences in the surgical efficacy and prognosis of laparoscopic pyeloplasty for hydronephrosis caused by symptomatic versus asymptomatic ureteropelvic junction obstruction (UPJO) in children and determine whether clinical symptoms affect the surgical outcome and prognosis.MethodsChildren who underwent laparoscopic pyeloplasty in our hospital from January 2018 to December 2022 were retrospectively analyzed. The children were divided into symptomatic and asymptomatic groups according to their main symptoms. The primary outcomes were the surgical success rate, change in renal parenchymal thickness, and change in renal pelvis anteroposterior diameter. The secondary outcomes were postoperative complications, reoperation rate, operative duration, intraoperative blood loss, and drainage tube indwelling time.ResultsIn total, 224 children with UPJO were enrolled; 148 (66.1%) were symptomatic and 76 (33.9%) were asymptomatic. The symptomatic group showed a significantly greater mean change in renal parenchymal thickness, significantly higher surgical success rate, and significantly lower postoperative complication rate.ConclusionsIn the present study, asymptomatic children had a lower surgical success rate, less postoperative imaging improvement, and more postoperative complications than symptomatic children. The presence or absence of clinical symptoms may affect the surgical outcome and prognosis.

Abstract:ABSTRACT:Over the last two decades ,minimally invasive treatment options for ureteropelvic junction obstruction (UPJO) have been developed and popularized .Multiple series of laparoscopic pyeloplasty have demonstrated high success rates and low perioperative morbidity in pediatric and adult populations ,with both transperitoneal and retroperitoneal approaches .The evo‐lution of laparoscopy has been limited by the challenges of intracorporeal suturing . Robotic surgical systems may offer the means to overcome this major impediment of laparoscopic surgery .The gold standard surgical technique for the laparoscopic management of UPJO is dismembered pyeloplasty ,as described by Anderson and Hynes .However ,other techniques such as Fenger plasty ,Y‐V pyeloplasty ,or“Vascular Hitch” technique for UPJO repair can be considered whenever possible because they offers more advantages at no increased risk ,given a careful case selection ,mostly based on intraoperative findings .

Abstract:Objective To evaluate the efficacy of Anderson-Hynes pyeloplasty in unilateral ureteropelvic junction obstruction(UPJO)patients using diuretic renography,and to analyze the possible relative factors to postoperative renal function improvement.Methods Forty-two patients who underwent Anderson-Hynes pyeloplasty for unilateral UPJO were retrospectively collected.All patients underwent diuretic renography before and after operation.The contralateral kidney had neither morphological nor functional abnormalities.The relative renal function(RRF)of affected kidney was expressed as its uptake rate in second to third minute post administration of imaging agent.Obstructive or non-obstructive hydronephrosis were classified according to the response to furosemide stimulation(RFS).Improvement of postoperative renal function and drainage were recorded.The correlation between postoperative renal function improvement with drainage improvement,preoperative renal function,and preoperative obstructive types,were analyzed respectively.Results RRF of affected kidney before operation was(40.40±13.45)%,post operation was(44.26±13.35)%,indicating a statistically significant renal function improvement after surgery(P=0.01).Patients with postoperative drainage improvement were 53.33%(8/15),56.00%(14/25)and 50.00%(1/2)in the group of renal function improved,unchanged and deteriorated,respectively(P=0.21).There was no significant correlation between postoperative renal function improvement with preoperative renal function(P=0.40),neither with preoperative obstructive types(P=0.60).Conclusion Anderson-Hynes pyeloplasty could improve the involved renal function for UPJO patients,and improve some patients' drainage conditions.However,there is no significant correlation between postoperative renal function improvement with drainage improvement.Preoperative renal function and obstructive types could not be used to predict the improvement of renal function post operation.
Significant improvement in hydronephrosis with pyeloplasty prior to 3 months of age in patients with antenatal severe hydronephrosis

Abstract:IntroductionPyeloplasty is the definitive management of ureteropelvic junction obstruction (UPJO). One of the challenging questions is when to perform pyeloplasty. We studied if improvement post-pyeloplasty in the first 3 months of life could show greater improvement in hydronephrosis than surgery at an older age.Patients and MethodsPatients with postnatally diagnosed UPJO and underwent pyeloplasty in the first year of life were retrospectively reviewed. We excluded patients with concomitant vesicoureteral reflux, and patients who had pyeloplasty because of UTI or missed follow-up. Patients were divided into two groups, according to the age at pyeloplasty, before and after the age of 3 months. We collected patients’ demographics, anteroposterior diameter of the renal pelvis (APD), SFU grade, renogram data, perioperative data (surgery duration, hospital stay, and ureteral stent duration) and postoperative ultrasound changes. The percentage of change of APD (Δ%APD) was calculatedusing the formula: Δ%APD = [ (initial APD-last APD)/initial APD] *100.ResultsWe included 90 patients (93 renal units). 36 patients had pyeloplasty during the first 3 months of life and 57 patients at 3 -12 months. Patients’ characteristics were similar in both groups except APD which was higher when pyeloplasty was done < 3 months of age (p = 0.02). Both groups had comparable perioperative parameters. After almost similar follow-up period of both groups. The Δ%APD was 58% when pyeloplasty was done < 3 months compared to 33% when was performed > 3 months (p = 0.009). Using Kaplan–Meier analysis, APD significantly improved when pyeloplasty was performed before the age of 3 months (p = 0.001).ConclusionEarly pyeloplasty, in the first 3 months of life, showed a significant improvement of APD postoperatively than those had surgery later. It is unclear if this will relate to less loss of renal function yet certainly this would be suspected and feel this finding provides some evidence for early intervention.

The role of voiding cystourethrography in asymptomatic unilateral isolated ureteropelvic junction obstruction: A retrospective study

Abstract:Introduction: The presence of concomitant vesicoureteric reflux (VUR) and ureteropelvic junction obstruction (UPJO) is uncommon. Nevertheless, the reported VUR coexisting with asymptomatic unilateral isolated hydronephrosis (AUIH) requiring pyeloplasty for correction of UPJO was of low grade and mostly resolved during conservative follow-up. Therefore, VCUG may be not indicated in these children except if voiding symptoms, urinary tract infection (UTI), dilated ureters, or bladder and ureteric abnormalities are suspected. Objectives: The aim was to evaluate the need for VCUG in infants <1 year old with AUIH for whom a dismembered pyeloplasty was indicated for correction of UPJO. Methods: Ninety-six children <1 year old with pyeloplasty carried out from January 2012 to March 2014 were retrospectively included. Children with voiding symptoms or dilated ureter, duplex system, fused kidneys, bilateral dilatation, or any bladder abnormality on ultrasound were excluded. Anderson-Hynes pyeloplasty was performed through a flank incision. Preoperative VCUG was analyzed in relation to outcome and any UTI during follow-up. The Student t test, Mann-Whitney U test, or Fisher exact test were used to compare variables. Results: Five children had concomitant VUR with UPJO. Most of the children were circumcised during the first postnatal week. The remaining few children were circumcised at the time of pyeloplasty. Side, grade of detected VUR, and complications (18.75%) (postoperative or during follow-up) are presented in the Table. Outcomes in children with and without VUR were not different. Dismembered pyeloplasty was successful in children with VUR and with no complications except for non-febrile UTI in one child only. Ureters were still not dilated at the last follow-up. Discussion: The required imaging in infants with AUIH is still a subject of debate. As we expected, there was a low incidence of associated VUR in the present study. They were of low grade without any complications during follow-up and without affecting the outcome. The present study has its limitations, including the retrospective nature and short follow-up. However, as at least 2 years of follow-up were documented without any harm or ureteric dilation, VUR will mostly resolve. The present study is strengthened by inclusion of infants only. Conclusions: Our data suggest that VCUG is not indicated in infants with AUIH requiring pyeloplasty for correction of UPJO. VCUG will not affect the treatment decision, operative outcome, or postoperative complications. VCUG may be indicated in case of suspected voiding symptoms, UTI, dilated ureters, or bladder and ureteric abnormalities.
