Abstract:Objective: To test the null hypothesis that dentine treatment with silver diamine fluoride (SDF), a potent antimicrobial agent, following use of proteolytic and chelating agents does not influence the wettability of an epoxy resin (AH Plus, Dentsply Sirona, Charlotte, NC, USA) and a tricalcium silicate sealer (BioRoot RCS, Septodont, Saint-Maur-des-Fossés, France). Methods: Seventy-two intraradicular dentine specimens were divided into six groups based on the final irrigation solutions used: 2.5% sodium hypochlorite (NaOCl) and 17% ethylenediaminetetraacetic acid (EDTA) (NaOCl-EDTA) (group 1); NaOCl-EDTA-NaOCl (group 2); NaOCl-EDTA followed by 3.8% SDF, NaOCl-EDTA-SDF (group 3); NaOCl-EDTA-NaOCl-SDF (group 4); SDF (group 5) and saline (group 6). After irrigation, the specimens were divided into subgroups according to the sealer used, AH Plus or BioRoot RCS. Contact angles were measured using a contact angle analyser. The data were analysed using an independent t test, one-way analysis of variance (ANOVA) and Tamhane T2 post hoc test, with the level of significance set at P < 0.05. Results: In the epoxy resin sealer group, dentine surfaces treated with only SDF showed the lowest contact angle. This was significantly less than the groups in which NaOCl was used as the final irrigant (P < 0.05). In the tricalcium silicate-based sealer group, the groups treated with SDF showed significantly greater contact angles when compared to the control group (P < 0.05). Conclusion: It was concluded that SDF conditioning of dentine favours the wettability of epoxy resin sealer but is detrimental to the wettability of tricalcium silicate sealer.
Pre-Sealing of Endodontic Access Cavities for the Preservation of Anterior Teeth Fracture Resistance

Abstract:Objective: Sodium hypochlorite solution (NaOCl) is an effective canal irrigant but interferes with the mechanical features of dentin and the bonding capability of adhesives when restoring endodontically treated teeth. This study evaluated whether access cavity resin sealing before using canal irrigant would augment the resistance of endodontically treated anterior teeth against fracture. Methods: Sixty maxillary incisors underwent endodontic treatment in five groups (n = 12). Irrigation with 5.25% NaOCl and 17% ethylenediaminetetraacetic acid (EDTA) was performed in all groups except for Group 5. After root canal obturation, in Group 1, the access cavity was kept unrestored. In Group 2, immediate restoration after obturation was achieved. For Group 3, delayed restoration after 1 week was provided. In Group 4 (pre-sealed), before canal irrigation, the dentin surface of access cavities was sealed using self-adhesive composite resin (Vertise Flow) and then restored after obturation. In Group 5, which was saline irrigated, immediate restoration was performed. After storage and thermal cycling for 5000 cycles at 5°C-55°C with a dwell time of 15 s and a transfer time of 5 s, teeth were statically loaded by a universal testing machine until a fracture occurred. Data were collected as the fracture resistance (FR) and analyzed using the one-way analysis of variance and Tukey's tests. Results: FR significantly differed between all groups (p < 0.001). The lowest FR was recorded in the unrestored group (284 ± 86 N), which was not statistically different from the immediately restored group (p = 0.065). The pre-sealed group exhibited the highest FR value (810 ± 127 N, p ≤ 0.02 vs. other groups). The FR of the saline-irrigated and delayed restored groups was almost similar (p = 0.13). Conclusions: NaOCl/EDTA irrigation resulted in an adverse effect on FR. Delayed restoration could reduce this adverse effect. Access cavity pre-sealing with flowable composites led to a higher FR than conventional methods and may be considered an effective step during treatment procedures.
Bonding Quality of Etch-and-Rinse Adhesives in Root Canals Upon Different Pretreatments.

Abstract:PURPOSE:To evaluate the effect of ethanol-wet/hydrophobic resin bonding (EWB) in a simplified dehydration protocol with ultrasonic agitation (UA) of etch-and-rinse adhesives on the bonding quality in root canals.MATERIALS AND METHODS:56 extracted human maxillary central incisors were divided into 4 groups (n = 14) according to the procedures of bonding fiber posts in root canals: A: conventional etch-and-rinse adhesive as control (All-Bond 3, Bisco); B: EWB in a simplified dehydration protocol, ie, after 100% ethanol dehydration (30 s, 3 times), ethanol/hydrophobic resin (50% bis-GMA + TEG-DMA, 50% ethanol) was applied into the root canal as a primer, followed by applying pure co-monomer adhesive; C: EWB in a standard dehydration protocol, ie, ascending concentrations of ethanol (50%, 70%, 80%, 90%) for 30 s and 100% ethanol (30 s, 3 times) for dehydration, with the remaining procedure conducted as in group B; D: EWB in a simplified dehydration protocol under ultrasonic agitation (UA), ie, 100% ethanol under ultrasonic agitation for 30 s, ethanol/hydrophobic resin as a primer applied under UA for 60 s, followed by the application of pure co-monomer adhesive. All the specimens were cemented with Duo-Link (Bisco) and immersed in artificial saliva for 24 h or thermocycled for 10,000 times. Bond strength, failure mode, and nanoleakage were evaluated. Hybrid layer and resin tags were also observed with SEM. Data were analyzed with two-way ANOVA (bond strength) and Kruskal-Wallis analysis (nanoleakage, resin tag score) (α = 0.05).RESULTS:Significant differences existed in bond strength (p = 0.02) and nanoleakage (p = 0.02) among groups. After 24 h and after thermocycling, group D exhibited higher bond strengths than did group B (p = 0.02 after 24 h, p = 0.03 after aging) and group A (p = 0.06 after 24 h, p = 0.04 after aging). Bond strength and nanoleakage showed no significant difference after aging (p > 0.05). There was no significant difference in resin tag scores between any of the groups, but the apical third of the roots exhibited the lowest score (p = 0.001).CONCLUSION:The synergistic effects of the two pretreatment strategies (EWB in a simplified dehydration protocol and UA) improved the bonding quality. UA had a positive effect on the adhesion of the etch-and-rinse adhesives, when EWB in a simplified procedure was applied into the root canal.
Effect of sodium hypochlorite and ethylenediaminotetraacetic acid activated by laser and ultrasonic energy on surface morphology and chemical composition of intracanal dentin

Abstract:The aim of the study was to evaluate the structural and chemical changes in intracanal dentin after root canal irrigation with sodium hypochlorite (NaOCl) and ethylenediaminetetraacetic acid (EDTA) using additional activation with a novel Er:YAG Shock Wave-Enhanced Emission Photoacoustic Streaming (SWEEPS), ultrasonic irrigation (UI), and Nd:YAG irradiation. Root canals of extracted human premolars were instrumented with distilled water and distributed into groups according to the irrigation protocol used: Group 1: conventional needle irrigation (CI) with 3% NaOCl; group 2: CI with 15% EDTA; and group 3: CI with NaOCl+EDTA. In group 4, root canals were irrigated with NaOCl+EDTA+NaOCl using the following techniques: CI (group 4A), SWEEPS (group 4B), UI (group 4C). In group 4D, root canals were finally irradiated with Nd:YAG laser. After the irrigation, the intracanal dentin sample was collected and analyzed using spectroscope with a Fourier transformation of infrared spectrum in Attenuated total reflection technique (FTIR-ATR) to calculate apatite/collagen ratio. Scanning electron microscopy (SEM) and energy dispersive x-ray spectrometry (EDS) were used to determine its chemical composition. FTIR-ATR showed no significant difference in the phosphate/amid I ratio between the control and the experimental groups (p > 0.05). SWEEPS and UI of NaOCl+EDTA+NaOCl caused the most pronounced decrease of Ca and P (p < 0.05) value compared to CI and control group, and canal wall erosion. SWEEPS and UI of NaOCl+EDTA+NaOCl caused the most pronounced decrease of Ca and P value and significant canal wall erosion. However, no difference in phosphate/amide ratio was reported among groups. RESEARCH HIGHLIGHTS: SWEEPS and UI of NaOCl+EDTA+NaOCl caused the most pronounced decrease of Ca and P values and significant canal wall erosion. There was no difference in phosphate/amide ratio among the irrigation protocols.
Evaluating the Efficacy of Certain Intermediate Irrigants in Preventing Precipitate Formed Due to Sodium Hypochlorite and Chlorhexidine Within the Root Canal System: An In Vitro Study

Abstract:Aim: Root canal irrigation using a mixture of sodium hypochlorite (NaOCL) and chlorhexidine gluconate (CHX) might cause a precipitate to develop. This study aims to check the efficacy of sodium thiosulfate and normal saline as irrigating solutions. Materials and methods: Roots of 45 teeth were biomechanically prepared, and the teeth were then tested. A size 15k file was introduced to the point where the apical foramen could be visible in order to provide an accurate reading of the working length. As a precaution against irrigating solutions leaking out, the specimens' tips were sealed with modeling wax before being instrumented. Each group's root canals were instrumented with #F4 hand Protaper (Dentsply Sirona, USA) as recommended by the manufacturer. Between instrumentation, The canals were lubricated with ethylenediamine tetraacetic acid (EDTA) and irrigated with 2.5% sodium hypochlorite (RC Help, Prime Dental, Mumbai, India). Fifteen samples were randomly assigned to one of three experimental categories based on the middle watering arrangement used: Group 1 (control), Group 2 (saline irrigant), and Group 3 (3.86% sodium thiosulfate). The jewel plate was submerged in water to cool it down, while two longitudinal scores were made on the root's buccal and lingual surfaces. We used a stereomicroscope to examine the exposed surfaces of the root trench in the coronal, middle, and apical thirds for the orange-earthy colored material (Stereozoom Nikon magnifying lens under 20X amplification), and we used the Mann-Whitney U test and the Kruskal Wallis test during our thorough analysis. Results: The precipitation generated in the coronal, middle, and apical thirds had significantly different thicknesses. While precipitation did occur in all three regions, it was at a much lower rate in the apical third compared to the coronal and middle regions. In Group 1, the control group, the precipitate was thicker than in Groups 2 (saline irrigant) and 3 (3.86% sodium thiosulfate). Conclusion: Sodium thiosulfate, which is a biocompatible solution, can be used as an intermediate irrigant as it shows less precipitate as compared to saline.
Can the addition of surfactants to NaOCl irrigation impact on the percentage of voids of root canal filling?

Abstract:This study evaluated the impact of canal irrigation using surfactants associated to NaOCl on the percentage of voids of root canal filling through micro‐computed tomographic (micro‐CT) analysis. 27 mandibular mesial roots with Vertucci type II canal configuration with isthmus were selected and scanned in a micro‐CT device. The specimens were assigned to three groups (n = 9), according to the irrigation solution during instrumentation: 2.5% NaOCl without additives (control), 2.5% NaOCl with 0.1% Benzalkonium Chloride and 2.5% NaOCl with 0.1% Tween 80. After obturation, the specimens were rescanned, and the percentage of voids were calculated. Data were analysed using one‐way ANOVA with a significance level of 5%. The percentage of voids were similar among the groups (p > 0.05). None of the specimens showed completely voids‐free areas. The addition of surfactants to NaOCl irrigation did not improve the quality of root canal filling of mandibular mesial root canals.

Effect of Laser Activated Sodium Hypochlorite on Sealing Ability of Two Endodontic Sealers Using Stereomicroscope-An In vitro Study

Abstract:Introduction: The combination of various irrigants, irrigation techniques along with root canal sealers helps in achieving a three-dimensional seal in root canals, which is of clinical relevance. The present in-vitro study aims to assess and compare the efficacy of laser-activated NaOCl on the sealing ability of two endodontic sealers. Materials and Methods: Forty extracted human single-rooted permanent teeth were collected and decoronated 12 mm from their apices. Instrumentation did with an 11 mm working length using Rotary Pro taper files to F3. All the teeth grouped into four (n= 10) based on irrigation protocol and sealer used as follows: Group A1 – Laser activated NaOCl + AH Plus Group A2 – Laser activated NaOCl +nano seal, Group B1 – NaOCl + AH Plus, Group B2 – NaOCl + nano seal. The lateral condensation technique did obturation. Roots stored in freshly prepared 1% methylene blue for 72 hours. The roots were split longitudinally along the long axis using a diamond disc underwater coolant, and markings made at 2, 4, and 6 mm from the apex. The depth of dye penetration and microleakage examined under a stereomicroscope (Magnus) at 30× magnification. Results: Statistical analysis carried out by using the software, statistical package for social sciences [SPSS]. Results represented in the form of minimum, mean, and standard deviation. Conclusion: The laser-activated NaOCl group exhibited better sealing ability than the control group. Among the two sealers, the Bio root RCS sealer showed less leakage & better sealing ability than AH plus Clinical Significance: Increasing the temperature of low concentration sodium hypochlorite solution by the action of the diode laser, not only enhances its immediate tissue dissolution capacity but also increased the dentin permeability by removing the smear layer. Further leading to better adhesion between the root canal sealer and the gutta-percha with the radicular dentin.

Abstract:Aim: The aim of this study was to compare the effect of three irrigating solutions on the apical sealing ability of three endodontic sealers when the continuous wave obturation technique was used.Materials and Methods: 100 maxillary central incisors were decoronated and roots were divided into three experimental groups (n=30 each) according to the type of irrigating solution, and two control groups (n=5 each). Each experimental group was divided equally into three subgroups according to the type of obturation system. All canals were instrumented using ProTaper Universal rotary system and irrigated with 3% sodium hypochlorite (Group 1), 2% chlorohexidine (Group 2), and 3% hydrogen peroxide (Group 3), followed by 17% EDTA and distilled water. Continuous wave technique was used to obturate root canals with the following combinations: MTA Fillapex/Gutta-percha (Subgroup A), AH Plus/Gutta-percha (Subgroup B) and Realseal SE/Realseal (Subgroup C). The apical microleakage was assessed using linear dye penetration method and the data were statistically analyzed. Results: Significant differences were found between all groups (P<0.05). All root canal sealers after irrigation with 3% hydrogen peroxide showed the lowest amounts of apical leakage. Samples irrigated with 3% sodium hypochlorite or 2% chlorohexidine showed the highest amounts of apical leakage. Conclusions: The type of irrigating solution could affect the apical sealing ability of experimental sealers when used with the continuous wave obturation technique. Root canal irrigation with 3% hydrogen peroxide + 17% EDTA increased the sealing ability of all root canal sealers. Root canal irrigation with 3% sodium hypochlorite 2% chlorohexidine + 17% EDTA decreased the sealing ability of all root canal sealers.

Abstract:Background: The main factors that affect the success of an endodontic infection are effective cleaning and shaping of the root canal system including complete disinfection by using chemical irrigating solutions and obturation with an endodontic sealer to provide a fluid-tight seal. Using rotary and hand instruments for shaping and cleaning the root canal along with irrigants produces a smear layer on the surface of the root dentin affecting the penetration of the endodontic sealer into the dentinal tubules. This smear is difficult to remove with the use of only endodontic irrigants, hence, chelating agents were introduced in adjunct with irrigating solutions for irrigation protocol for effective removal of smear layer which affect the penetration of endodontic sealers into the dentinal tubules. Methods: 32 mandibular premolar teeth were used. The biomechanical preparation was done till Protaper F3 size. Irrigation was done with 2.5 mL sodium hypochlorite (NaOCl) solution after each instrumentation change for 1 min. Samples were then divided into 4 groups according to the irrigating solution used as the final rinse used with passive ultrasonic agitation. The groups were: Group I: 5 ml of saline, Group II: 5ml of 17% ethylenediaminetetraacetic acid, Group III: 5 ml of 10% citric acid, Group IV: 5 ml of 7% maleic acid each for one minute. All the canals were obturated with BioRoot tmRCS with gutta-percha using the ultrasonic condensation technique. For staining the samples for Confocal LASER microscopy, BioRoot tmRCS was mixed with Rhodamine B dye. Results: The maximum penetration of bio-ceramic sealer was observed in the coronal region. At the apical third, the maximum sealer penetration was seen with 7% maleic acid. Conclusions: Maximum sealer penetration was seen in the coronal section followed by the middle and apical section. Maximum sealer penetration was seen with 7 % maleic acid at the apical third.
A comparative evaluation of two irrigating solutions combination effectiveness on apical sealing of different root canal sealers

Abstract:Background: Root canal irrigants play an essential role in removing the smear layer, pulp tissue, and microorganisms in the root canal system. The irrigation solution can remove the smear layer, which can interfere with the bond between the sealers. As a result of sealer penetration into dentin tubules following smear layer removal, an apical seal was obtained. One of the most recent innovations in root canals consists of a combination of silver citrate (BioAKT, New Tech Solution, Brescia, Italy) and chlorhexidine-ethylenediaminetetraacetic acid (CHX-EDTA; SmearOFF, Vista Dental, Racine, USA). These irrigants were used as the final irrigant for root canal disinfection. Epoxy sealers (AH26, Dentsply Maillefer, Ballaigues, Switzerland) and bioceramic sealers (ceraseal, Meta Biomed Co., Cheongju, Korea) have excellent adhesive properties and sealing ability and are commonly used as obturation materials. Objective: The aim of this study was to compare the effectiveness of a root canal combination irrigant between silver citrate and CHX-EDTA on the apical sealing ability of epoxy sealer and bioceramic sealer. Materials and Methods: Forty extracted mandible single-root premolars were used in this study. The root canal preparation was done with a rotary instrument until no. 30.06. Irrigation was performed using 5.25% NaOCl between instrumentations. The final irrigant and sealers were Group I: BioAKT + ceraceal; Group II: BioAKT + AH26; Group III: SmearOFF + ceraceal; and Group IV: SmearOFF + AH26. Obturation was then performed using the warm vertical compaction technique. The sealing ability of endodontic sealers was evaluated using the dye penetration method under the magnification of the stereomicroscope. The data were statistically analyzed using a two-way analysis of variance (ANOVA) (SPSS, IBM, USA). Results: No significant difference (P > 0.05) was observed between the four experimental groups. Conclusion: Both combination irrigants and sealers showed comparable effectiveness in smear layer removal and a similar sealing ability.

Influence of Sapindus mukorossi extract in comparison to 17% EDTA as final root canal irrigant on the sealer penetration and microleakage of dentinal tubules

Abstract:Objective: The study evaluated the effect of Sapindus mukorossi (SM) extract as a final root canal irrigant on sealer penetration (SP) in dentinal tubules and microleakage. Materials and methods: Samples were selected based on inclusion and exclusion criteria. An access opening in all samples was performed and the working length was decided using pro taper for canal finishing along with constant irrigation. Specimens were randomly divided into 3 groups. Group 1 was irrigated with 3 ml of 17% EDTA; group 2 was irrigated with SM irrigant and group 3 samples were irrigated with 0.9% saline. After obturation, samples were vertically placed in 1% methylene blue dye cut in half longitudinally, and viewed under a stereomicroscope. Analysis of SP in the dentinal tubule was assessed using scanning electron microscopy (SEM). For microleakage assessment, mean and standard deviation were reported and One-Way ANOVA was applied. SP was compared using Kruskal-Wallis' test. For inspecting the interaction between SM/EDTA and NaOCl, Fisher's exact test was applied. No statistically significant difference between microleakage in any of the tested groups was observed. The control group showed minimum leakage as compared to EDTA and SM. Results: The results displayed that there was no significant difference, (p=0.67), between dentinal tubule SP at 2 mm. A significant difference between dentinal tubule SP among groups at 5 mm was observed (p<0.05). Conclusions: SM ethanolic extract showed comparable outcomes of smear layer removal and sealer penetration to 17% EDTA, as a final irrigant in root canal cleaning. Therefore, SM has the potential to be used as an adjuvant final irrigant in conjunction with NaOCl.
Effect of Calcium Hydroxide and Nano Calcium Hydroxide on Push-out Bond Strength of Epoxy Resin Sealer to Root Canal Dentin

Abstract:Introduction: The goal of this paper was to determine the impact of calcium hydroxide (CH) and nano-calcium hydroxide (NCH) on the push-out bond strength of an epoxy resin-based sealer. Materials and Methods: A total of 48 mandibular premolars were decoronated in vitro and instrumented by a ProTaper rotary system up to F4. The specimens then were randomly allocated into 3 groups (n=16). The two intervention groups were treated with either CH or NCH and one control group which did not receive any intracanal medicament. After one week of medicament placement, the specimens were irrigated by 10 mL 17% EDTA, followed by 10 mL 2.5% NaOCl and an ultimate flushing by 5 mL sterile saline. The samples were obturated using AH-Plus Jet sealer and gutta-percha by lateral condensation technique. Push-out test was done by a universal test machine to evaluate the bond strength among the sealer and the root canal dentin. Repeated measurement analysis followed by Tukey’s HSD and Bonferroni post-hoc tests were used for data analysis. Results: The control group showed a higher push-out bond strength in comparison to the specimens in the CH and NCH groups (P<0.05). In spite of the greater push-out bond strength of the samples treated with NCH in comparison to those in CH group, no statistically notable difference was found among them (P>0.05). Additionally, irrespective of the kind of medicament, the bonding at the apical level of the root canal was stronger compared to the coronal third (P<0.001). Conclusion: According to the present in vitro study, application of CH and NCH resulted in a significant reduction of the bond strength between the epoxy resin-based sealer and dentinal walls. Although, application of these medicaments resulted in reduction of bonding of epoxy resin-based sealers root canal dentin significantly.

Evaluation of the Sealing Ability and Bond Strength of Two Endodontic Root Canal Sealers: An In Vitro Study

Abstract:Background: Obturation represents a critical step in endodontic treatment, which relies on a core material and a sealer. This study aims to evaluate the sealing ability and bond strength to the root canal walls of an epoxy resin-based sealer (AH-Plus®, Dentsply Sirona, Johnson City, TN, USA) and a bioceramic sealer (GuttaFlow Bioseal®, Coltène/Whaledent, GmbH + Co. KG, Langenau, Germany). Methods: Thirty-eight maxillary anterior teeth with single roots and identical round sections were separated into two experimental groups according to the root canal sealers used, namely, G1 = AH-Plus® and G2 = GuttaFlow Bioseal®, and two control groups, specifically, G3 = the negative control and G4 = the positive control. The sealing capacity was measured by the penetration of the radioactive isotope 99mTc. For the push-out test, the compressive force test was performed in a universal machine and the force was applied by exerting pressure on the surface of the material to be tested in the apical to the coronal direction and using three test points with different diameters. Results: GuttaFlow Bioseal® exhibited superior sealing ability compared to AH-Plus® (p = 0.003). Regarding the bond strength, AH-Plus® provided higher adhesion values than GuttaFlow Bioseal® in the three sections of the tooth root (p = 0.001). Conclusions: GuttaFlow Bioseal® had significantly better sealing ability than AH-Plus® but lower adhesion values in the three zones of the root canal, with statistically significant differences between the groups. However, it is important to note that for the action of endodontic sealers to be maximized, the root-filling technique must be most appropriate.

Abstract:The purpose of this study was to evaluate the effect of different irrigating solutions on smear layer removal and dentinal tubule opening on root canal surfaces after post space preparation and to study whether additional ultrasonic irrigation has any effect on smear layer removal. Forty-eight anterior teeth were treated endodontically. After post space preparation, they were assigned to six groups: group 1, EDTA; group 2, EDTA with ultrasonic activation; group 3, sodium hypochlorite (NaOCl); group 4, NaOCl with ultrasonic activation; group 5, sodium chloride (NaCl); and group 6, NaCl with ultrasonic activation. Specimens were examined under a field-emission scanning electron micro;cope and scored for debris removal and dentinal tubule opening at the coronal, middle, and apical thirds of the root canal. The results showed that EDTA performed significantly better than NaCl and NaOCl in smear layer removal and dentinal tubule opening. Additional ultrasonic irrigation did not improve smear layer removal significantly. (J Endod 2009;35:583-586)

Evaluation of resin sealer penetration of dentin following different final rinses for endodontic irrigation using confocal laser scanning microscopy

Abstract:The aim of the study was to assess the depth of sealer penetration into dentinal tubules following different final rinses and indirectly evaluate precipitation of irrigating solutions. Methods: 52 extracted maxillary incisors were prepared to size 40/04 with 6% sodium hypochlorite irrigating solution. Following power analysis, teeth were randomly divided into 4 groups (n = 13) according to final rinse. Group 1 - 17% EDTA, Saline and 2% Chlorhexidine solution; Group 2 - SmearOFF™; Group 3 - Qmix® ; and Group 4 - saline control. Obturation was performed with gutta percha and resin sealer mixed with rhodamine-B-isothiocyanate dye using warm vertical compaction. Teeth were sectioned into apical, middle, and coronal thirds using Buehler diamond saw at 3, 6 and 9 mm from the apex. Sections were examined under confocal laser scanning microscopy, and images were exported to Image J software to measure the maximum depth of sealer penetration circumferentially. Data were analysed using a generalised linear model and post hoc multiple comparisons with significance set at P < 0.05. Group 3 had the highest depth of penetration. There were statistically significant differences between Groups 2 and 3 and control for all sections, Group 1 and control in apical section only. Conclusion: Recently developed final rinses produced higher sealer penetration and more patent dentinal tubules than using EDTA and chlorhexidine sequentially.
In Vitro Evaluation of Antibacterial Properties and Smear Layer Removal/Sealer Penetration of a Novel Silver-Citrate Root Canal Irrigant

Abstract:This study aimed at evaluating the efficacy of a novel silver-citrate root canal irrigation solution (BioAKT) on smear layer removal, sealer penetration after root canal instrumentation and antibacterial activity. Single-root teeth were endodontically treated, sealed with an epoxi-amine resin sealer and irrigated using: Group I: 5.25% sodium hypochlorite (NaOCl); Group II: silver-citrate solution (BioAKT); Group III: phosphate buffer solution (PBS); Group IV: 17% ethylenediaminetetraacetic acid (EDTA). Smear layer removal and silver deposition at the coronal, middle and apical portion of each canal were analyzed using scanning electron microscopy (SEM) and energy-dispersive x-ray spectroscopy (EDS). Sealer penetration into dentinal tubules at coronal, middle and apical portion was assessed through dye-assisted confocal microscopy (CSM). Both SEM and CSM micrographs were evaluated by two examiners (κ = 0.86), who were blind to the irrigation regimens; scores were given according to the degree of penetration of the sealer. Data analysis included Pearson's x2 and Sidak's multiple comparisons. Dentin discs were polished and sterilized. Enterococcus faecalis biofilms were grown using a continuous-flow bioreactor under anaerobic conditions for 72 h. Specimens were irrigated with the tested solutions, and bacterial viability was assessed using a tetrazolium salt assay (MTT). Statistical analysis included one-way ANOVA and Student's post-hoc t-test (p < 0.05). BioAKT and EDTA were the most efficient solutions both in removing the smear layer and allowing sealer penetration. However, at the apical portion BioAKT performed significantly better compared to EDTA both in smear layer removal and sealer penetration (p < 0.05). BioAKT and NaOCl showed comparable antibacterial effect (p = 0.53). In conclusion, BioAKT represents a suitable smear layer removal agent, which allows for reliable sealer penetration at the apical portion of the root canal system and offers significant antibacterial properties.

Abstract:The purpose of this study was to compare the efficacy of different chelating solutions (17% EDTA and 10% citric acid) on the smear layer removal, and their effect on tubular dentin sealer penetration. Sixty root canals were prepared and distributed into four groups (n = 15) according to the final irrigation protocol: G1, final irrigation with 2.5 mL of distilled water; G2, final irrigation with 2.5 mL of 2.5% sodium hypochlorite solution; G3, final irrigation with 2.5 mL of 17% EDTA; and G4, final irrigation with 2.5 mL of 10% citric acid. Five specimens from each group were not filled to assess smear layer removal by scanning electron microscopy. Ten specimens from each group were filled for analysis of sealer penetration into dentinal tubules by confocal laser scanning microscopy. Smear layer removal (Kruskal-Wallis and Dunn's tests) and sealer penetration (F and Tukey's tests) were statistically analyzed with 95% of significance level. G3 and G4 had greater smear layer removal rates in the cervical and middle thirds, in comparison with G1 and G2 (p < .05). G3 and G4 had the highest percentages of sealer penetration in all thirds, in comparison with G1 and G2 (p < .05). Smear layer removal was effective only at the cervical and middle thirds when the chelating solutions were used. Sealer penetration into the dentinal tubules significantly increased in all root thirds when the specimens were treated with both chelating solutions.

Abstract:The purposes of this study were to investigate the reliability and efficiency of a new evaluation method for resin bonding to root canal dentin, which measures both marginal adaptation and shear bond strength simultaneously, and to determine the effects of root canal irrigants on resin bonding. A wet bonding system (Single Bond) and a self-etching primer system (Clearfil Mega Bond) were employed; NaClO was used as a root canal irrigant. No gaps or changes in bond strength were observed despite the NaClO treatment when the wet bonding system was employed, while the gap formation ratio increased, and bond strength decreased with longer NaClO treatment time when the self-etching primer system was employed. These findings suggested that this new experimental method was effective for evaluating resin systems to the root canal wall dentin which is affected by irrigation with NaClO.
