Abstract:Over the past couple of decades, the incidence of breast cancer (BC) has significantly increased among females in comparison to other cancer types. In medicinal terminology, the susceptibility to BC is mainly centered around three hormonal receptors: estrogen (ER), progesterone (PR), and human epidermal growth receptor (HER2). Notably, estrogen- dependent breast cancer has a considerable female demographic, making it treatable with hormonal drugs and less intensive immunotherapy. Conversely, the narrative delves into the ominous type of cancer known as triple-negative breast cancer (TNBC). The orientation of all three receptors falls in a negative direction, which is ineffective for treatments that rely on hormonal or antagonist medicaments. Therefore, the only option available to tackle this type of cancer is chemotherapy, which causes toxicity within the body, is highly expensive, and is non-targeted. To counter this challenge, researchers have pioneered nano-based drug delivery systems (NDDS) owing to their innumerable merits and scientific development. NDDS mainly involves polymeric nanoparticles, liposomes, and dendrimers. This review comprehensively details the advancements in nanoinduced targeted drug delivery systems, with a focus on surface modification techniques for active targeting, enhanced drug release, and improved pharmacokinetics. Critical analysis extends to preclinical and clinical studies, revealing the potential of nano-drug delivery systems in TNBC to surpass traditional therapies, with promising heightened efficacy and reduced side effects.

Abstract:Cancer is a major public health concern worldwide; it is the second-highest cause of death in the United States. According to projections of cancer incidence and mortality rates throughout the world for the year 2023, triple-negative breast cancer (TNBC) is expected to be the leading cause of cancer related death among women worldwide. Traditional strategies for the treatment of TNBC have many drawbacks, such as drug resistance, toxicity etc. Discovering novel treatment delivery techniques and researching innovative, efficient drug delivery methods is important. This review discusses the types and subtypes of TNBC. The problems associated with standard therapies, mechanism of drug resistance for TNBC and highlights the need to develop novel therapeutic strategies. It provides information on the relative prevalence and severity of TNBC in the world of cancer. Several treatment approaches viz. targeted therapy, gene therapy, bacterial-mediated therapy, nanomedicine, immune checkpoint inhibitors, theranostic, radiotherapy, chemotherapy, immunotherapy, herbal therapy, and AI-based treatment for TNBC, are discussed in detail. Additionally, diagnostic techniques, including imaging and biopsy, gene expression profiling, mammography, magnetic resonance imaging, ultrasound, computed tomography scan, positron emission tomography scan, and immunohistochemistry, have been discussed in the review for effective cancer treatment. This in-depth analysis highlights the need for innovative and individualized TNBC care methods to serve patients better. Abstract:The therapeutic effect of highly malignant triple negative breast cancer (TNBC) is negatively affected by the formation of tumor cell resistant clone and the severe toxicity of chemotherapy drugs to normal tissues. In accordance with research findings, in the comprehensive targeted therapy of TNBC, the nano delivery system effectively suppresses and kills tumor cells on the basis of its unique targeting properties as well as the ability to co-load, deliver, and release chemotherapeutic drugs, active gene fragments and immune enhancing factors, etc. When combined with photothermal ablation therapy, the synergism and toxicity reduction effect of chemotherapy drugs, the inhibition of tumor proliferation related genes as well as activation of immune system can be achieved. Nanoparticle delivery systems present a totally new way of drug design and usage, and change the pharmacokinetic characteristics of conventional chemotherapeutic drugs with significantly reduced adverse effects of some otherwise very toxic chemodrugs via targeted delivery. In this paper, a detailed review was carried out focusing on the research progress of nanoparticle delivery system in the comprehensive targeted therapy of TNBC. We also summarize the merits, shortcomings, perspectives and future developments of nanoparticle-drug delivery systems.

Abstract:The conventional therapeutic treatment of triple-negative breast cancer (TNBC) is negatively influenced by the development of tumor cell drug resistant, and systemic toxicity of therapeutic agents due to off-target activity. In accordance with research findings, nanoparticles (NPs) responsive to the tumor microenvironment (TME) have been discovered for providing opportunities to selectively target tumor cells via active targeting or Enhanced Permeability and Retention (EPR) effect. The combination of the TME control and therapeutic NPs offers promising solutions for improving the prognosis of the TNBC because the TME actively participates in tumor growth, metastasis, and drug resistance. The NP-based systems leverage stimulus-responsive mechanisms, such as low pH value, hypoxic, excessive secretion enzyme, concentration of glutathione (GSH)/reactive oxygen species (ROS), and high concentration of Adenosine triphosphate (ATP) to combat TNBC progression. Concurrently, NP-based stimulus-responsive introduces a novel approach for drug dosage design, administration, and modification of the pharmacokinetics of conventional chemotherapy and immunotherapy drugs. This review provides a comprehensive examination of the strengths, limitations, applications, perspectives, and future expectations of both novel and traditional stimulus-responsive NP-based drug delivery systems for improving outcomes in the medical practice of TNBC. Abstract:Triple-negative breast cancer (TNBC) is characterised by the lack or low expression of estrogen, progesterone, and human epidermal growth factor receptor 2 receptors. TNBC has a high recurrence rate, swiftly metastasizes, and has a high mortality rate. Subsequently, the increase in cases of TNBC has signaled the need for treatment strategies with improved drug delivery systems. New diagnostic approaches, chemical entities, formulations particular those in the nanometric range have emerged after extensive scientific research as alternative strategies for TNBC treatment. As compared to contemporary cancer therapy, nanoparticles offer peculiar tunable features namely small size, shape, electrical charge, magnetic and fluorescent properties. Specifically in targeted drug delivery, nanoparticles have been demonstrated to be highly efficient in encapsulating, functionalization, and conjugation. Presently, nanoparticles have ignited and transformed the approach in photodynamic therapy, bioimaging, use of theranostics and precision medicine delivery in breast cancer. Correspondingly, recent years have witnessed a drastic rise in literature pertaining to treatment of TNBC using nanomaterials. Subsequently, this manuscript aims to present a state-of-the-art of nanomaterials advance on TNBC treatment; the ubiquitous utility use of nanomaterials such as liposomes, dendrimers, solid lipid nanomaterials, gold nanomaterials and quantum dots as anticancer agents and drug delivery systems in TNBC.
Theranostics for Triple-Negative Breast Cancer

Abstract:Triple-negative breast cancer (TNBC) is an aggressive subtype of breast cancer with poor prognosis. Current endocrine therapy or anti HER-2 therapy is not available for these patients. Chemotherapeutic treatment response varies among patients due to the disease heterogeneity. To overcome these challenges, theranostics for treating TNBC have been widely investigated. Anticancer material conjugated nanoparticles with target-binding ligand and tracer agents enable simultaneous drug delivery and visualization of the lesion with minimal off-target toxicity. In this review, we summarize recently FDA-approved targeted therapies for TNBC, such as poly-ADP-ribose polymerase (PARP) inhibitors, check point inhibitors, and antibody-drug conjugates. Particularly, novel theranostic approaches including lipid-based, polymer-based, and carbon-based nanocarriers are discussed, which can provide basic overview of nano-therapeutic modalities in TNBC diagnosis and treatment.
Coeliac plexus block using computed tomography guidance

Abstract:Coeliac plexus blocks have been used successfully in the treatment of abdominal pain in advanced cancer and in benign chronic abdominal pain. However, concern remains about occasional potentially serious complications. One possible way to reduce the risks of this procedure may be to improve imaging during the procedure. We report a series of 38 coeliac plexus blocks carried out under computer tomographic (CT) guidance, mostly using the anterior approach. The technique is described. Effectiveness and side-effect rates were similar to other reported series. There were no major complications. Analysis of contrast spread would indicate that anterior preaortic or bilateral contrast spread is necessary to obtain pain relief. Our experience would indicate that routine CT guidance can be a simple aid to coeliac plexus block, and can be achieved easily in a district general hospital. Improved imaging allows accurate needle placement, while avoiding vital structures such as the aorta and pleura. Accurate placement may also allow the use of reduced volumes of neurolytic drugs.

Abstract:Breast cancer has remained a serious health concern globally for women despite the healthcare advances which have enabled early diagnosis and treatment. Due to its metastatic ability and development of resistance to chemotherapies, triple-negative breast cancer is an extremely challenging subtype to treat. Targeted and optimized therapy is imperative as these tumors have higher recurrence rates than other types of breast cancers. This review is focused on the novel therapeutic strategies that have been proposed for the treatment of these aggressive cancers including alternative approaches like patient selection using biomarkers, metabolic reprogramming and development of smart drug delivery systems (SDDS) using targeted nanoparticles to treat the tumors as well as ensure prevention of recurrence. All of these approaches are aimed towards removing and treating the malignancies of triple-negative breast cancer (TNBC) and optimizing the therapies according to the patient cohorts. Further research is, however, necessary for the designing of an effective therapeutic regimen for patient sub-groups suffering from TNBC.

Abstract:Abstract: Approximately 1,70,000 of the 1 million cases of Breast Cancer (BC) identified every year globally are triple-negative. Triple-Negative Breast Cancer (TNBC) has different clinical and pathologic features. Because of its aggressive attitude, typically poor prognosis, and non-existence of targeted medicines, chemotherapy is the only treatment available, making it a clinical problem. This subgroup is responsible for 15% of all types of BC cases and a larger proportion of BC cases in African- American females. It can be treated with conventional therapy because there are no special treatment recommendations for this subtype; nonetheless, this therapy leaves patients with a high incidence of local and systemic recurrence. Clinically, they manifest as interval cancer in younger women and have a higher chance of recurrence in the first three years. Epidermal Growth Factor Receptor (EGFR), VEGF, basal cytokeratins, poly (ADP-ribose) polymerase-1, p53, tyrosinase kinases, mTOR, heat and stress proteins, and TOP-2A are only a few of the biomarkers examined in research on TNBC. This study aims to concentrate on its characteristics, definition, and available treatments now and in the future. Additionally, we looked for angiogenesis, growth, and survival pathway blockade, as well as synthetic lethality. Moreover, nanomolecular therapeutic therapy options, the role of biomarkers, and various clinical trials are discussed briefly. The successful development of targeted therapy for TNBC is still limited because of its heterogeneity. In this article, we outline the present and potential treatment landscape for TNBC and discuss how a thorough knowledge of the ecosystem around TNBC could aid in categorizing risk levels and improving the likelihood of therapy personalization.

Abstract:Triple-negative breast cancer (TNBC), lacking specific receptors found in other breast cancer subtypes, poses significant treatment challenges due to limited therapeutic options. Therefore, it is necessary to develop novel treatment approaches for TNBC. In the last few decades, many attempts have been reported for alternative tools for TNBC treatment: immunotherapy, radiotherapy, targeted therapy, combination therapy, and nanotechnology-based therapy. Among them, combination therapy and nanotechnology-based therapy show the most promise for TNBC treatment. This review outlines recent advancements in these areas, highlighting the efficacy of combination therapy (immunotherapy paired with chemotherapy, targeted therapy, or radiotherapy) in both preclinical and clinical stages and nanotechnology-based therapies utilizing various nanoparticles loaded with anticancer agents, nucleic acids, immunotherapeutics, or CRISPRs in preclinical stages for TNBC treatment.

Abstract:The truancy of representation of the estrogen, progesterone, and human epidermal growth factor receptors occurs during TNBC. TNBC is recognized for the upper reappearance and has a poorer diagnosis compared with rest breast cancer (BC) types. Presently, as such, no targeted therapy is approved for TNBC and treatment options are subjected to chemotherapy and surgery, which have high mortality rates. Hence, the current article focuses on the scenario of TNBC vital pathways and discusses the latest advances in TNBC treatment, including immune checkpoint inhibitors (ICIs), PARP suppressors, and cancer vaccines. Immunotherapy and ICIs, like PD 1 and PD L1 suppressors, displayed potential in clinical trials (CTs). These suppressors obstruct the mechanisms which allow tumor cells to evade the system thereby boosting the body's defense against TNBC. Immunotherapy, either alone or combined with chemotherapy has demonstrated patient outcomes such as increased survival rates and reduced treatment-related side effects. Additionally, targeted therapy approaches include BRCA/2 mutation poly ribose polymerase inhibitors, Vascular Endothelial Growth Factor Receptor (VEGFR) inhibitors, Epidermal growth factor receptor inhibitors, Fibroblast growth factor inhibitors, Androgen Receptor inhibitors, PIK3/AKT/mTOR pathway inhibitors, Cyclin-dependent kinase (CDK) inhibitors, Notch signaling pathway inhibitors, Signal transducer and activator of transcription 3 (STAT3) signaling pathway inhibitors, Chimeric antigen receptor T (CAR-T) cell therapy, Transforming growth factor (TGF) -β inhibitors, Epigenetic modifications (EPM), Aurora Kinase inhibitors and antibody-drug conjugates. We also highlight ongoing clinical trials and potential future directions for TNBC therapy. Despite the challenges in treating TNBC, recent developments in understanding the molecular and immune characteristics of TNBC have opened up new opportunities for targeted therapies, which hold promise for improving outcomes in this aggressive disease.

Abstract:Triple-negative breast cancer (TNBC) cells are deficient in estrogen, progesterone and ERBB2 receptor expression, presenting a particularly challenging therapeutic target due to their highly invasive nature and relatively low response to therapeutics. There is an absence of specific treatment strategies for this tumor subgroup, and hence TNBC is managed with conventional therapeutics, often leading to systemic relapse. In terms of histology and transcription profile these cancers have similarities to BRCA-1-linked breast cancers, and it is hypothesized that BRCA1 pathway is non-functional in this type of breast cancer. In this review article, we discuss the different receptors expressed by TNBC as well as the diversity of different signaling pathways targeted by TNBC therapeutics, for example, Notch, Hedgehog, Wnt/b-Catenin as well as TGF-beta signaling pathways. Additionally, many epidermal growth factor receptor (EGFR), poly (ADP-ribose) polymerase (PARP) and mammalian target of rapamycin (mTOR) inhibitors effectively inhibit the TNBCs, but they face challenges of either resistance to drugs or relapse. The resistance of TNBC to conventional therapeutic agents has helped in the advancement of advanced TNBC therapeutic approaches including hyperthermia, photodynamic therapy, as well as nanomedicine-based targeted therapeutics of drugs, miRNA, siRNA, and aptamers, which will also be discussed. Artificial intelligence is another tool that is presented to enhance the diagnosis of TNBC.

Abstract:Triple positive breast cancer (TPBC) is one of the most aggressive breast cancer. Due to the unique cell phenotype, aggressiveness, metastatic potential and lack of receptors or targets, chemotherapy is the choice of treatment for TNBC. Doxorubicin (DOX), one of the representative agents of anthracycline chemotherapy, has better efficacy in patients with metastatic TNBC (mTNBC). DOX in anthracycline-based chemotherapy regimens have higher response rates. Nano-drug delivery systems possess unique targeting and ability of co-load, deliver and release chemotherapeutic drugs, active gene fragments and immune enhancing factors to effectively inhibit or kill tumor cells. Therefore, advances in nano-drug delivery systems for DOX therapy have attracted a considerable amount of attention from researchers. In this article, we have reviewed the progress of nano-drug delivery systems (e.g., Nanoparticles, Liposomes, Micelles, Nanogels, Dendrimers, Exosomes, etc.) applied to DOX in the treatment of TNBC. We also summarize the current progress of clinical trials of DOX combined with immune checkpoint inhibitors (ICIS) for the treatment of TNBC. The merits, demerits and future development of nanomedicine delivery systems in the treatment of TNBC are also envisioned, with the aim of providing a new class of safe and efficient thoughts for the treatment of TNBC.

Abstract:Triple-negative breast cancer (TNBC) is the most aggressive subtype of breast cancer, which is characterized by the absence of estrogen receptor (ER) and progesterone receptor (PR) expression and the absence of human epidermal growth factor receptor 2 (HER2) expression/amplification. Conventional chemotherapy is the mainstay of systemic treatment for TNBC. However, lack of molecular targeted therapies and poor prognosis of TNBC patients have prompted a great effort to discover effective targets for improving the clinical outcomes. For now, poly (ADP-ribose) polymerase (PARP) inhibitors (PARPi’s) and immune checkpoint inhibitors have been approved for the treatment of TNBC. Moreover, agents that target signal transduction, angiogenesis, epigenetic modifications, and cell cycle are under active preclinical or clinical investigations. In this review, we highlight the current major developments in targeted therapies of TNBC, with some descriptions about their (dis)advantages and future perspectives.

Abstract:Triple-negative breast cancer (TNBC) is an aggressive subtype with the worst prognosis and highest recurrence rates. The treatment choices are limited due to the scarcity of endocrine and HER2 targets, except for chemotherapy. However, the side effects of chemotherapy restrict its long-term usage. Immunotherapy shows potential as a promising therapeutic strategy, such as inducing immunogenic cell death, immune checkpoint therapy, and immune adjuvant therapy. Nanotechnology offers unique advantages in the field of immunotherapy, such as improved delivery and targeted release of immunotherapeutic agents and enhanced bioavailability of immunomodulators. As well as the potential for combination therapy synergistically enhanced by nanocarriers. Nanoparticles-based combined application of multiple immunotherapies is designed to take the tactics of enhancing immunogenicity and reversing immunosuppression. Moreover, the increasing abundance of biomedical materials holds more promise for the development of this field. This review summarizes the advances in the field of nanoparticle-mediated immunotherapy in terms of both immune strategies for treatment and the development of biomaterials and presents challenges and hopes for the future.

Abstract:Triple-negative breast cancer (TNBC) is one of the most aggressive subtypes of breast cancer, marked by poor outcomes and dismal prognosis. Due to the absence of targetable receptors, chemotherapy still represents the main therapeutic option. Therefore, current research is now focusing on understanding the specific molecular pathways implicated in TNBC, in order to identify novel biomarker signatures and develop targeted therapies able to improve its clinical management. With the aim of identifying novel molecular features characterizing TNBC, elucidating the mechanisms by which these molecular biomarkers are implicated in the tumor development and progression, and assessing the impact on cancerous cells following their inhibition or modulation, we conducted a literature search from the earliest works to December 2023 on PubMed, Scopus, and Web Of Science. A total of 146 studies were selected. The results obtained demonstrated that TNBC is characterized by a heterogeneous molecular profile. Several biomarkers have proven not only to be characteristic of TNBC but also to serve as potential effective therapeutic targets, holding the promise of a new era of personalized treatments able to improve its prognosis. The pre-clinical findings that have emerged from our systematic review set the stage for further investigation in forthcoming clinical trials.

Abstract:Triple-negative breast cancer (TNBC) is a highly aggressive subtype of breast cancer that lacks expression of estrogen, progesterone, and HER2 receptor targets for therapy. Polymeric nanoparticles help address the challenges in treating TNBC by enabling tailored and targeted drug delivery. Biocompatible polymeric nanoparticles leverage enhanced tumor permeability for site-specific accumulation and ligand-mediated active targeting to boost specificity. Controlled, sustained intratumorally release of encapsulated chemotherapies, such as paclitaxel and curcumin, improves antitumor efficacy as demonstrated through preclinical TNBC models. However, the practical application of these nanomedicines still has room for improvement. Advancing personalized nanoparticle platforms that align treatments to TNBC's expanding molecular subtypes shows promise. Expanding the polymer range through novel copolymers or drug conjugates may improve tumor penetration, stability, and drug encapsulation. Incorporating gene therapies, imaging agents, or triggering stimuli responsiveness into polymeric nanoparticles can also overcome innate and acquired drug resistance in TNBC while monitoring outcomes. This article reviews the different types of nanoparticles used to treat TNBC and the different mechanisms of nanoparticles that can deliver drugs to tumor cells. Collaboration across different disciplines aimed at developing combination therapies, immuno-oncology, tumor-targeting ligands, and translating preclinical safety/efficacy via scalable manufacturing practices is essential. Well-designed polymeric nanoparticles offer immense potential for patient-centric TNBC treatment, but continued optimization across bench to bedside efforts is critical for clinical realization and transforming patient outcomes.
