Abstract:Introduction: Traditionally, elderly care organizations play a pivotal role in integrating care for an ageing population, but recently this role has been challenged due to competition from other service providers, such as for-profit organizations, hospitals, home- and primary care organizations. In addition, the increasing complexity of current health care, combined with ageing and housing issues as well as rising co-morbidity in Belgium leads to a rising demand for collaboration. Not only do elderly care organizations have to deal with more and more tasks that are unique, but also are resources distributed across various service providers. As such, elderly care organizations are moving slowly from managing mainly physical assets such as housing facilities towards more intellectual and relational capacities and treatment services resulting into shared service centers and tailor-made health and social care solutions.Theory/methods: Although a number of factors have been proposed how to integrate health- and social care service delivery, collaboration pervades most discussions. We argue in this article that elderly care organizations follow a combination of three underlying strategic motives based on effectiveness, efficiency and legitimacy criteria to make a conscious, intentional decision to establish a relationship with other organizations for explicitly formulated purposes. We conceptualize these relationships as three ideal types: solitary, merger or collaboration and propose that the presence or absence of these types is explained by a combination of the changing nature in the task, the organizational configuration and its interdependency on others in the external environment.The article presents a case survey approach of 18 elderly care organizations. Data were collected in a five months period of time by using semi-structured interviews of key representatives and a survey. In addition, five workshops were organized and secondary data on elderly care organizations was collected. We applied Fuzzy-Set Qualitative Comparative analysis to analyze what, if any, combination of determinants explains whether elderly care organizations act solitary, merge or collaborate.Results, conclusion and discussion: Implications of the findings for elderly care organizations are discussed in light of the rising health care issues and increasing competition in Flanders Belgium. In particular, we are interested in which combinatory recipes, if any, of task, organizational configuration, and interdependency explain the presence or absence of a particular ideal type of collaboration. Furthermore, future recommendations are presented to further the development of how elderly care organizations can deal with their changing role in the healthcare system.

Interprofessional collaboration regarding patients' care plans in primary care: a focus group study into influential factors

Abstract:Background: The number of people with multiple chronic conditions demanding primary care services is increasing. To deal with the complex health care demands of these people, professionals from different disciplines collaborate. This study aims to explore influential factors regarding interprofessional collaboration related to care plan development in primary care. Methods: A qualitative study, including four semi-structured focus group interviews (n = 4). In total, a heterogeneous group of experts (n = 16) and health care professionals (n = 15) participated. Participants discussed viewpoints, barriers, and facilitators regarding interprofessional collaboration related to care plan development. The data were analysed by means of inductive content analysis. Results: The findings show a variety of factors influencing the interprofessional collaboration in developing a care plan. Factors can be divided into 5 key categories: (1) patient-related factors: active role, self-management, goals and wishes, membership of the team; (2) professional-related factors: individual competences, domain thinking, motivation; (3) interpersonal factors: language differences, knowing each other, trust and respect, and motivation; (4) organisational factors: structure, composition, time, shared vision, leadership and administrative support; and (5) external factors: education, culture, hierarchy, domain thinking, law and regulations, finance, technology and ICT. Conclusions: Improving interprofessional collaboration regarding care plan development calls for an integral approach including patient- and professional related factors, interpersonal, organisational, and external factors. Further, the leader of the team seems to play a key role in watching the patient perspective, organising and coordinating interprofessional collaborations, and guiding the team through developments. The results of this study can be used as input for developing tools and interventions targeted at executing and improving interprofessional collaboration related to care plan development.

Abstract:Background Informal care features high on the policy agenda of many countries to deal with workforce shortages. As a consequence, care provision increasingly takes place in the care triad of care recipients, informal caregivers and care professionals. How collaboration between care partners takes shape depends on how the different partners perceive this collaboration. This paper aims to investigate the relative importance of the different aspects of collaboration from the perspectives of care recipients, informal caregivers and care professionals in the context of the care for older persons in The Netherlands. Methods Using Q‐methodology, 32 participants ranked 28 statements that reflect different aspects of collaboration in the care triad and explained their ranking during a follow‐up interview. Participants comprised 9 older persons, 10 informal caregivers and 13 care professionals. Data were analysed using by‐person factor analysis to identify common patterns in the rankings of the statements. Emerging patterns were interpreted and described as views on collaboration using aggregated rankings and qualitative data from the interviews. Results Five distinct views on collaboration were found: (1) Emphasizing warm collaboration, (2) trusting care professional's expertise, (3) open and compassionate care professionals, (4) responsive decision‐making by autonomous care professionals and (5) prioritizing care recipient's and informal caregiver's interests. Care recipients and/or informal caregivers were associated with views 1, 3 and, 5, whereas care professionals were associated with all five views. Conclusions Our study highlights the importance of recognizing the potential diversity of views between and within different partner groups in care triads. Governmental and organizational policy makers, as well as healthcare professionals who aim to increase or support the involvement of informal caregivers, should take this heterogeneity into consideration. Patient or Public Contribution An advisory board of older persons (care recipients and informal caregivers) was involved in the recruitment of the participants, the formulation of the statements and the reflection on the findings of the study and potential implications.

Abstract:IntroductionUtrecht Overvecht is a multicultural deprived neighbourhood in the Netherlands. Residents often have multiple problems, both in the field of health and wellbeing. More than 15% of the residents are above 65 years. Since older people live longer at home independently, they need increasingly complex care.To address the care needs of these elderly, professionals from medical and social care work together in an integrated manner. Underlying idea is that non-medical aspects of a patient's life situation are essential to take into account in the risk analysis and guidance of health problems. Professionals use the 4-Domains model (4D-model) as a tool to display and analyse the problems of their patients in a structured manner. The 4D-model provides a mutual framework of analysis and language for professionals from different domains.AimIn this study we evaluate promoting and impeding factors for cooperation in the collaboration between professionals from medical and social care in Overvecht.MethodFace to face interviews (25) and a focusgroup discussion with general practitioners, practice nurse for elderly, community nurses, and social workers.ResultsPromoting factors for collaboration are that professionals from medical and social care experience a shared vision on care for elderly in their neighbourhood, as well as large willingness to collaborate and share responsibility. Furthermore, professionals know each other personally, through formal and informal meetings and proactively seek collaboration. The 4D model provides a useful tool in joint patient discussions. The practice nurse is seen as a very important and accessible link in the collaboration between general practitioners and other health- and care providers.There are also impeding factors for collaboration. Compliance with working agreements varies between professionals, and also due to the large number of home care organisations and the large turn over in professionals, collaboration is sometimes difficult.DiscussionAlthough collaboration in the care for elderly in Overvecht generally goes well, it is difficult to equally involve all professionals in the neighbourhood. Personal factors as well as time and financial barriers play a role, but also the fact that a large number of professionals and organisations are involved in elderly care. It seems important to keep partnerships relatively small and well-arranged. It is better to have a few good ‘key-persons’ in the right place who can each reach their own network, so that necessary connections can be made through them.ConclusionsKnowing each other personally is an important prerequisite for good collaboration. A central ‘key-person’ who coordinates collaboration between professionals from different domains is indispensable. A mutual framework of analysis and language for professionals from medical and social domain, can be very useful in collaboration. Lessons learnedCollaboration is essential for good healthcare, but requires continuous investment.Limitations We interviewed a selective sample of professionals, who may be expected to be the most motivated persons. Their opinions may therefore not be entirely representative.Suggestions for future research In future research it is important to evaluate the experiences of elderly themselves with this type of integrated care.

Abstract:In caring for clients with combined problems, various professionals are encouraged to work together in new ways. Collaboration is often fluid, and professionals are expected to seek other professionals and organizations to solve complex problems. This type of collaboration is not institutionalized; it may therefore be hard to develop routines compared to fixed teams. Knowledge about how frontline professionals work together in non-institutionalized forms of fluid collaboration is lacking. This article addresses this gap by studying how professionals from various disciplines work together in fluid collaborative contexts when caring for clients with combined problems. To this end, this empirical research has an iterative design and uses ethnographic fieldwork in studying these hard-to-grasp contexts. In the analysis, we explore whether and how interprofessional collaboration manifests in fluid teams in general practice, mental healthcare and social welfare a Dutch city and how team fluidity plays a role.

Abstract:Interprofessional collaboration is essential to maintain high-quality care in long-term care and geriatric rehabilitation. However, little is known regarding perceived factors influencing interprofessional collaboration by people involved in care. This concerns both long-term care and geriatric rehabilitation. Moreover, knowledge of using patient outcome measures to enhance interprofessional collaboration during multidisciplinary team meetings is insufficient. This study examined the perceived facilitators of and barriers to interprofessional collaboration in general and during multidisciplinary team meetings, specifically according to healthcare professionals, patients, and informal caregivers. Differences between long-term care and geriatric rehabilitation were also investigated. Finally, it was examined which patient outcome measures were used in multidisciplinary team meetings.

Do project management and network governance contribute to inter-organisational collaboration in primary care? A mixed methods study

Abstract:Background: The need for organisational development in primary care has increased as it is accepted as a means of curbing rising costs and responding to demographic transitions. It is only within such inter-organisational networks that small-scale practices can offer treatment to complex patients and continuity of care. The aim of this paper is to explore, through the experience of professionals and patients, whether, and how, project management and network governance can improve the outcomes of projects which promote inter-organisational collaboration in primary care. Methods: This paper describes a study of projects aimed at improving inter-organisational collaboration in Dutch primary care. The projects' success in project management and network governance was monitored by interviewing project leaders and board members on the one hand, and improvement in the collaboration by surveying professionals and patients on the other. Both qualitative and quantitative methods were applied to assess the projects. These were analysed, finally, using multi-level models in order to account for the variation in the projects, professionals and patients. Results: Successful network governance was associated positively with the professionals' satisfaction with the collaboration; but not with improvements in the quality of care as experienced by patients. Neither patients nor professionals perceived successful project management as associated with the outcomes of the collaboration projects. Conclusions: This study shows that network governance in particular makes a difference to the outcomes of inter-organisational collaboration in primary care. However, project management is not a predictor for successful inter-organisational collaboration in primary care.
Governing Integrated Health and Social Care: An Analysis of Experiences in Three European Countries

Abstract:Purpose: Achieving greater health and social care integration is a policy priority in many countries, but challenges remain. We focused on governance and accountability for integrated care and explored arrangements that shape more integrated delivery models or systems in Italy, the Netherlands and Scotland. We also examined how the COVID-19 pandemic affected existing governance arrangements. Design/methodology/approach: A case study approach involving document review and semi-structured interviews with 35 stakeholders in 10 study sites between February 2021 and April 2022. We used the Transparency, Accountability, Participation, Integrity and Capability (TAPIC) framework to guide our analytical enquiry. Findings: Study sites ranged from bottom-up voluntary agreements in the Netherlands to top-down mandated integration in Scotland. Interviews identified seven themes that were seen to have helped or hindered integration efforts locally. Participants described a disconnect between what national or regional governments aspire to achieve and their own efforts to implement this vision. This resulted in blurred, and sometimes contradictory, lines of accountability between the centre and local sites. Flexibility and time to allow for national policies to be adapted to local contexts, and engaged local leaders, were seen to be key to delivering the integration agenda. Health care, and in particular acute hospital care, was reported to dominate social care in terms of policies, resource allocation and national monitoring systems, thereby undermining better collaboration locally. The pandemic highlighted and exacerbated existing strengths and weaknesses but was not seen as a major disruptor to the overall vision for the health and social care system. Research limitations: We included a relatively small number of interviews per study site, limiting our ability to explore complexities within sites. Originality: This study highlights that governance is relatively neglected as a focus of attention in this context but addressing governance challenges is key for successful collaboration.

Abstract:BackgroundWith an increasing number of people dying in old age, collaboration between palliative care and geriatric medicine is increasingly being advocated in order to promote better health and health care for the increasing number of older people. The aim of this study is to identify barriers and facilitators and good practice examples of collaboration and integration between palliative care and geriatric medicine from a European perspective.MethodsFour semi-structured group interviews were undertaken with 32 participants from 18 countries worldwide. Participants were both clinicians (geriatricians, GPs, palliative care specialists) and academic researchers. The interviews were transcribed and independent analyses performed by two researchers who then reached consensus.ResultsLimited knowledge and understanding of what the other discipline offers, a lack of common practice and a lack of communication between disciplines and settings were considered as barriers for collaboration between palliative care and geriatric medicine. Multidisciplinary team working, integration, strong leadership and recognition of both disciplines as specialties were considered as facilitators of collaborative working. Whilst there are instances of close clinical working between disciplines, examples of strategic collaboration in education and policy were more limited.ConclusionsImproving knowledge about its principles and acquainting basic palliative care skills appears mandatory for geriatricians and other health care professionals. In addition, establishing more academic chairs is seen as a priority in order to develop more education and development at the intersection of palliative care and geriatric medicine.

Abstract:Background Across OECD countries, integration between healthcare organisations has become an indispensable part of contemporary healthcare provision. In recent years, inter‐organisational collaboration has increasingly been encouraged in health and competition policy at the expense of mergers. Yet, understanding of whether healthcare organisations make an active choice between merging and collaborating is lacking. Hence, this study systematically examines (i) healthcare executives' motives for integration, (ii) their potential trade‐offs between collaborating or merging, and (iii) the barriers to collaborating perceived by them. Methods Early 2019, an online questionnaire was conducted among a nationwide panel of 714 healthcare executives in the Netherlands. Because of their strategic position within healthcare organisations as end‐responsible managers, healthcare executives are especially suited to provide broad and in‐depth knowledge on the internal and external processes and decisions. Three hundred thirty‐seven Dutch healthcare executives completed the questionnaire (response rate 47%). This study sample was representative of the largest healthcare sectors in the Netherlands. In total, 137 mergers and 235 inter‐organisational collaborations were reported. Both closed questions and open‐ended questions were systematically analysed. Results Improving or broadening healthcare provision is the foremost motive for mergers as well as inter‐organisational collaborations. When considering both types, reducing governance complexity is one of the decisive reasons to opt for a merger, whereas aversion towards a full merger and lack of support base within the own organisation convinced healthcare executives to choose for a collaboration. When comparing specific healthcare sectors, the overlap in pursued motives and sub‐motives indicates that inter‐organisational collaborations and mergers are used for comparable objectives. Only a small minority of the responding executives switched between both types of integration. Institutional barriers, such as laws, regulations and financing regimes, appear to be the most restricting for healthcare executives to engage in inter‐organisational collaborations. Conclusions Our integral approach and systematic comparison across sectors could serve policymakers, regulators and healthcare providers in aligning organisational objectives and societal objectives in decision‐making on collaborations and mergers. Future research is recommended to study multiple collaboration and merger cases qualitatively for a detailed examination of decision‐making by healthcare executives, and develop an integral assessment framework for balancing collaborations and mergers based on their effects in the medium to long term.

Integrated Palliative Care for Nursing Home Residents: Exploring the Challenges in the Collaboration between Nursing Homes, Home Care and Hospitals

Abstract:INTRODUCTION: Nursing home residents are a vulnerable and frail segment of the population, characterised by their complex and palliative care needs. To ensure an integrated approach to palliative care for this target group, working on a collaborative basis with multiple providers across organisational boundaries is necessary. Considering that coordinators of palliative networks support and coordinate collaboration, the research question is: 'how do network coordinators perceive the process of collaboration between organisations in Flemish palliative networks?'METHODS: A dual-phase sequential mixed-methods design was applied. First, the coordinators of each of the fifteen palliative networks in Flanders completed a survey in which they evaluated ten aspects of collaboration for two types of cooperation: between nursing homes and home care, and between nursing homes and hospitals. Next, the survey results thus obtained were discussed to improve understanding in a focus group composed of the above coordinators, and which was analysed on the basis of content analysis.RESULTS: In both forms of cooperation, the 'formalisation' and 'governance' were the aspects that yielded the lowest mean scores. The coordinators in the focus group expressed a need for more formalised interaction among organisations with regard to palliative care, the establishment of formal channels of communication and the exchange of information, as well as the development of shared leadership.CONCLUSIONS: The perspectives of the coordinators on inter-organisational collaboration are a valuable starting point for interventions directed at the stronger integration of palliative care for residents of long term-care facilities.

Abstract:Health Informatics Journal, Volume 30, Issue 4, October-December 2024. ObjectivesThis study explores the governance intricacies in establishing a large-scale information infrastructure for integrated care within the National Health Service of England. By focusing on the initial 5 years of a regional interoperability program, we examine the challenges and dynamics of creating a unified system across organizational boundaries.MethodsA longitudinal multi-methods approach was utilized, collecting data through interviews with health and social care personnel involved in project governance, meeting observations, and document analysis. This provided an in-depth understanding of the processes and challenges of developing the regional interoperability program.ResultsThis study reveals a dynamic negotiation process and evolving governance structures that must continuously adapt to integrate diverse systems. Key findings include challenges in reconciling multiplicity and heterogeneity, the importance of social learning and stakeholder adaptation, and the role of ongoing negotiation and collaborative group construction in implementing interorganizational infrastructures.ConclusionThis study underscores the necessity of continuous adaptation and negotiation among stakeholders in establishing effective governance of integrated care infrastructures. Social learning and active participation play pivotal roles in overcoming the challenges associated with system integration. Strategic insights from this research can guide stakeholders in developing sociotechnical solutions for digital interorganizational infrastructures in integrated care.

Abstract:PURPOSE: To examine the barriers that hinder collaboration between health care and social care services and to report recommendations for effective collaboration to meet the growing support and care needs of our ageing population.METHODS: Data for this qualitative study were obtained from interviews with 7 key informants (n = 42) and 22 focus groups (n = 117) consisting of service providers who were from the health care or social care sectors and supporting elderly patients with multiple chronic diseases or long-term care needs. Data collection was conducted from 2015 to 2016. The data were analysed using an inductive approach on the basis of thematic analysis.FINDINGS: Qualitative analysis reviewed a number of factors that play a significant role in setting up barriers at the operational level, including fragmentation and lack of sustainability of discharge programmes provided by non-governmental organisations, lack of capacity of homes for the elderly, limitation of time and resources, and variation of roles in supporting end-of-life care decisions between the medical and social sectors. Other barriers are those of communication to be found at the structural level and perceptual ones that exist between professionals. Of these, perceptual barriers affect attitudes and create mistrust and interprofessional stereotypes and a hierarchy between the health care and social care sectors.CONCLUSION: Health care and social care service providers recognise the need for collaborative work to enhance continuity of care and ageing in place; however, their efforts are hindered by the identified barriers that need to be dealt with in practical terms and by a change of policy.

These transfers are common due to the complexity of palliative care, demographic changes increased prevalence of multimorbidity and higher life expectancy and social changes more people living alone. As a result, transfers require efficient and effective collaboration in order to provide good quality of care.This study is part of a larger research project on integrated care for palliative patients, in a regional palliative care network in Belgium.The study aim was:- to explore healthcare professionals’ experiences of interprofessional collaboration- to explore their perceived factors influencing the continuity of care during palliative patients’ transfer of care setting.Methods: Nine focus group discussions were held, with various professionals GPs, nurses, specialists, psychologists, dieticians, social workers, coordinators. N: 53 from different palliative care settings. Constant comparative analysis was used.Results: Knowing each other personally positively influenced the interprofessional collaboration. Perceptions of hierarchy negatively influenced the interprofessional collaboration, within and between the settings.Timely and sufficient information flow between all professionals involved was considered fundamental. A perceived barrier for interprofessional collaboration was the lack of a shared electronic health record. Efficient multi-disciplinary team meetings with shared care goals were regarded essential, though often lacking.Participants reported that specialists informed the patients on disease stage and prognosis late and insufficiently. This hampered open communication on end-of-life decisions and impeded a timely transfer to the home situation or the palliative care unit.The complexity of patient’s case interfered with the hospitalization length and timely organizing optimal home care the moment of discharge. When transfer to a nursing home was deemed necessary, waiting lists negatively influenced the continuity of careDiscussion: Our research shows that several clinical, organisational and service aspects of care at micro-, meso- and macro-level can be improved.At micro-level: insufficient open and timely communication towards the patient, hampers other healthcare professionals’ involvement. Together with absence of shared care goals, this negatively influences a timely referral to other settings.At meso-level: the current electronic health record system does not provide enough opportunities to share and exchange information for professionals involved, within and between the different care settings.At macro-level: continuity of care is hampered when appropriate supply of services lack: e.g., when referral to a nursing home is needed.Conclusions and lessons learned: Doctors should be trained more in bad news delivery and shared decision making. Interprofessional education modules may enhance collaboration and the discussing of shared care goals.Policy makers should invest in sufficient supply of palliative care services and the development of an electronic health record system to provide efficient and effective information transfers between settings.Limitations: We focused on only 1 palliative care network, therefor we are careful to generalize our results.Suggestions for future research: Interventions aiming to improve integrated care within the region will be evaluated.

Abstract:Globally, there is an urgency to address fragmented mental health systems, especially in low-to-middle income countries. State and non-state mental health service collaboration is a central strategy to strengthen care. The study was undertaken to analyse the power in governance processes of public mental health service provision. Semi-structured interviews were conducted with state and non-state actors in mental health care in a South African district. Transcriptions were thematically analysed using the Framework for Assessing Power in Collaborative Processes. Findings suggested that collaborative processes were significantly state-owned, in terms of funding models, administrative and legislative jurisdiction, and state hierarchical referral structure. No formal agreements were in place, elevating the importance of key network actors to bring less-endowed NGOs into the service network. Fragmentation between the Departments of Health and Social Development was telling in district forums. Resistance to power structures unfolded, some participants sidestepping traditional hierarchies to leverage funding and support. The paper highlights the complexities and different facets of power in integrated mental health care in a South African district, adding to growing literature on the social mechanisms that influence collaboration.

INTERVZP - Interprofessional collaboration in Advance Care Planning: examining support needs across disciplines and settings

Abstract:Background: Although advance care planning (ACP) can improve goal-oriented and goal-concordant care, it requires substantial interprofessional collaboration to ensure up-to-date information from patients and families is passed along across the care trajectory of the patient. The often-complex care trajectories of patients who would benefit most from ACP, as well as our fragmented care systems is hindering this. Aim: We aimed to explore how to better support a variety of professionals across care levels to improve advance care planning in their settings by exploring barriers related to ACP specifically, their support needs as well as potential strategies for improvement. Methods: We have carried out a targeted literature review in PubMed searching academic literature on advance care planning, chronic care or palliative care and interprofessional/multidisciplinary collaboration (and education); expert interviews with educators and trainers in interprofessional collaboration in healthcare (ie medicine, nursing, social work, obstetrics); and 3 semi-structured stakeholder workshops with a variety of professionals working in nursing homes, hospitals, and the home setting. Project progress and deliverables were safeguarded by an advisory group comprising users and stakeholders from various professional, policy, and umbrella organizations. Results: Fifteen published studies were included; 12 experts and 20 professionals participated. Various factors were found to hinder interprofessional collaboration in ACP: personal factors (eg. discipline: support staff, as opposed to registered nursing staff report higher, needs related to integration on the interdisciplinary team), interprofessional (eg. lack of clarity and appreciation of other’s roles), organizational (eg. no one taking the lead), interorganizational (eg. team fluidity/temporary teams) and system factors (eg. no unified electronic system). We formulated potential learning objectives that were triangulated with and complementary to existing general frameworks in interprofessional education (ie. IPEC, 5 bouwstenen IPSIG, Canmeds, EIPEN). We have visualized the patient journey of three case studies (in a hospital, nursing home and home setting) to graphically demonstrate how integrated care transitions between professionals, organizations and care levels can be improved and how (comprising tips and trigger questions that might stimulate professionals to share and receive the relevant information with and from others). Next steps: Learning objectives can be easily integrated into existing education for health and social care professionals and is informative for subsequent interprofessional training development. Tools are currently being developed to support educators in doing this.

Efficacy and Safety of Allopurinol in Patients with the Lesch-Nyhan Syndrome and Partial Hypoxanthine- Phosphoribosyltransferase Deficiency: a Follow-up Study of 18 Spanish Patients

Abstract:Allopurinol is used widely for the treatment of purine disorders such as gout, but efficacy and safety of allopurinol has not been analyzed systematically in an extensive series of patients with HPRT deficiency. From 1984 to 2004 we have diagnosed 30 patients with HPRT deficiency. Eighteen patients (12 with Lesch-Nyhan syndrome or complete HPRT deficiency, and 6 with partial HPRT deficiency) were treated with allopurinol (mean dose, 6.44 mg/Kg of weight per day) and followed-up for at least 12 months (mean follow-up 7,6 years per patient). Mean age at diagnosis was 7 years (range, 5 months to 35 years). Treatment with allopurinol was associated to a mean reduction of serum urate concentration of 50%, and was normalized in all patients. Mean urinary uric acid excretion was reduced by 75% from baseline values, and uric acid to creatinine ratio was close or under 1.0 in all patients. In contrast, hypoxanthine and xanthine urinary excretion rates increased by a mean of 6 and 10 times, respectively, compared to baseline levels. These modifications were similar in patients with complete or partial HPRT deficiency. In 2 patients xanthine stones were documented despite allopurinol dose adjustments to prevent markedly increased oxypurine excretion rates. Neurological manifestations did not appear to be influenced by allopurinol therapy. Allopurinol is a very efficacy and fairly safety drug for the treatment of uric acid overproduction in patients with complete and partial HPRT deficiency. Allopurinol was associated with xanthine lithiasis.
Body composition in Iranian boys with autism spectrum disorders

Abstract:The aim of this study was to assess body composition of boys with autism and also to investigate the association of demographic factors, autism severity and drug therapy with their body composition. We recruited 85 boys aged 7-14 years by stratifi ed random sampling in autism-specifi c schools in Tehran. Body composition was measured using bioelectrical impedance analysis. The study revealed that 9.4% of participants were underweight and 47.3% were suff ering from overweight and obesity. The mean total body fat percentage and fat mass index were 23.46% and 4.8, respectively. A high rate of obesity was found in Iranian children and adolescents with autism. Furthermore, boys whose mothers had higher educational levels may have less chance of being within the normal weight ranges in comparison with sons of lower-educated mothers (p=0.002), while father’s educational level had no eff ect on the child’s body mass index. In conclusion, a high prevalence of overweight and obesity was observed in Iranian male children and adolescents suff ering from autism according to their body mass index and body fat percentage.
Collaborative governance in integrated healthcare networks: insights from the cancer program in Québec (Canada)

Abstract:Context: The challenge of modernizing health services delivery for cancer patients and their loved ones involves moving towards integrated network-based practices. While initiatives at different levels of healthcare systems contribute to this movement, further efforts are needed to maintain and extend gains to adapt to the complex and fast-changing cancer environment. A collaborative governance regime, understood as a form of capacity building for joint action across boundaries, represents a promising way to improve and sustain integrated care. Objective: This presentation reports insights from providers and users of health services gained from the experience in operationalizing an integrated network-of-networks in the context of a national cancer program in Quebec (Canada). Methods: Methods: Insights are drawn from a larger multiple case study that took place from 2018 to 2021. Participants (n=39) are knowledgeable informants on how “mandated” integration structures and processes support collaboration among the various components of the network. Documents (n=45) related to network governance and integrated practices are also included. The data collection guide and analysis based on qualitative interpretive description are framed on the Collaborative Governance framework to capture critical dimensions in the integration process. These include clear mission, common and shared definition of complex problems and potential solutions, and identification of common values to support motivation, engagement and capacity for joint action. Results: We report on three main tensions related to integration in the cancer network that suggest strategies for enhancing collaborative governance: The first tension emerges as some participants perceive “mandated integration” within a “prescribed collaborative work” as a constraint that undermines previous collaborative work at local level. This tension leads to unresolved controversies within and between actor groups, weakening the mechanisms of collaborative governance. The second tension is a principle-to-practice divide around engaging people affected by cancer in network governance. Despite widespread motivation, participation at the local level varies according to managers' willingness and skill in creating open and inclusive processes where evidence-based medical practice and lived patient experience can be assembled. This tension is seen to contribute to a poor consideration of patient perspectives during the highly volatile period of decision-making during the COVID-19 pandemic. The third tension involves the hierarchy of cancer services promoted in the national cancer program and the standardization of practices. Organizing the network-of- networks by cancer specialization involves creating trajectories by type of cancer and their navigation according to established standards of practice and quality considerations. The consequent redistribution of human and financial resources, and revised ownership of care process creates perceived asymmetries between cancer network professionals and centers and controversies that hinder integrated practices. Conclusion: Despite favorable starting conditions from the national cancer program and its central leadership promoting collaborative governance, tensions that emerge through the pursuit of network integration limit the transition to a more collaborative regime. Taking the time to work out these tensions as integration proceeds appears essential to arrive at a governance model that is appropriate and acceptable for all network members.

Abstract:In recent years, Dutch authorities have attempted to shift from formal to informal care responsibilities to meet an increasing demand for care. There is a growing expectation that social and healthcare professionals will actively involve, support, and collaborate with partners, family members, and others who provide informal care. However, the practical implementation of such collaborations is not yet evident. In this qualitative study, 37 individual interviews and eight focus groups were conducted to explore the preferences of informal caregivers in the Netherlands regarding their collaboration with social and healthcare professionals. In recognition of the growing consensus that professionals should tailor their approach to caregivers' individual circumstances, we adopted an intersectional approach to examine the relationship between caregivers' social positions and preferences as well as the impact of systemic and power dynamics on these collaborations. The findings indicate that most of caregivers' worries concerning collaborations are rooted in the organisational context of their interactions with professionals rather than the interactions themselves. Caregivers' social positions do not necessarily lead to varying opinions regarding collaboration with professionals; however, caregivers do expect professionals to be aware of their social positions, as these positions shape the context within which informal care is provided and determine how easily caregivers can integrate care responsibilities into their lives. To enhance collaboration between caregivers and professionals in the short term, care organisations and social and healthcare educational programmes should encourage a professional mindset that recognises caregivers as equal partners in care. It is essential for professionals to acknowledge the caregiver's role and devise strategies for arranging care together that build on the existing relationship between the caregiver and care recipient.
