Abstract:Purpose: To reduce postoperative scar formation and to improve duction using amniotic membrane (AM) in strabismus reoperations. Methods: A prospective study of interventional case series comprised of 14 patients with restrictive strabismus. Objective clinical findings (visual acuity, angle of deviations and degree of duction deficits) were recorded in both the pre- and post-operative periods. Strabismus surgery included the excision of adhesions and scar tissue, repositioning of extraocular muscles (according to the degree of deviations) and placement of two sheets, one between muscle and tenon and the other between muscle and sclera. Conjunctival recession with covering of the bare sclera using AM was also performed. Results: The mean preoperative deviation of the patients was 34.3 prism diopters (PD) and mean post-operative deviation was 4.6 PD. The mean pre-operative duction deficit of the patients was 1.7; mean post-operative duction deficit was 0.2. Postoperatively all patients improved in relation to deviation and duction. Less than 8 PD deviations with no duction deficits were achieved in 12 (86 %) of the patients. Conclusions: Amniotic membrane placement around the extraocular muscle improves the duction and decreases the residual angle of deviations by inhibiting postoperative scar formation.

Abstract:ABSTRACT Postoperative adhesion formation following complex strabismus surgery limits surgical outcomes. The benefits of amniotic membrane transplant (AMT) are widely recognized in ophthalmology. There are only published isolated case reports regarding its use in strabismus surgery. The limitation of these reports is that they describe use of AMT in cases of restrictive strabismus resulting from periocular surgery and indirect involvement of the extraocular muscles. In our series, the amniotic membrane was used to manage complex strabismus surgery. In this retrospective case series we describe surgical management with AMT in 8 strabismus cases between 2007 and 2010 in a tertiary referral center. Of these, 6 cases resulted from complex and repeat strabismus procedures to the extraocular muscles and 2 cases were a result of periocular surgery. Descriptive methodology was used. Outcomes examined were postoperative objective improvement in angle of deviation, subjective improvement in patient symptoms, and need for further interventions. Mean follow-up period was 15 months. All patients had multiple strabismus surgery in the past with a mean number of 3.2 ± 1.5 procedures (range 1-5). The indication for surgery using amniotic membrane graft was in 7 cases a large hypotropia with inferior conjunctival fibrosis and in one case a large consecutive horizontal deviation. The mean angle of vertical deviation was reduced from 32.1 ± 13 prism diopters (PD) preoperatively to 19.75 ± 11.4 PD postoperatively, with better outcomes in cases where AMT was used early in the surgical plan. Six out of the eight cases had objective improvement also consistent with improvement in the binocular field of vision. This is the largest reported case series on amniotic membrane grafts for strabismus surgery complicated by fibrosis and especially cases of consecutive hypotropia. Our outcomes indicate that AMT is beneficial, but we suggest it be used in the early stages of the surgical management of these cases.

Abstract:Purpose: To evaluate the outcome of amniotic membrane transplantation (AMT) for the repair of severe conjunctival dehiscence that occurred during or after fornix-incision strabismus surgery with adjustable sutures. Design: Retrospective, interventional case series. Methods: Four patients in whom severe conjunctival dehiscence developed during or after strabismus surgery were treated with amniotic membrane grafts. The extent of conjunctival reepithelization over sclera, range of ocular motility, and patient comfort were evaluated immediately after the surgery and at 1, 2, and 4 weeks. Results: The mean follow-up period was 16 weeks (range 4 to 28 weeks). Conjunctival dehiscence was detected 0 to 14 days after surgery. AMT was performed 0 to 14 days later. The conjunctival defect was reepithelized in all patients by the fourth postoperative week without evidence of scarring or restriction of motility. Conclusions: In the treatment of large conjunctival defects that may follow strabismus surgery, AMT may be an alternative to conservative management or primary conjunctival closure.

Abstract:Vaccines comprising combinations of diphtheria, tetanus and pertussis (DTP) with Haemophilus influenzae type b polysaccharide-protein conjugate (Hib), inactivated poliomyelitis virus (IPV) and hepatitis B virus (HBV) are already available, and new combinations using acellular pertussis components in a triple vaccine (DTaP) are under development. Evidence to date has shown that control of the efficacy, safety and stability of combination vaccines cannot be based on information already available on the individual components or existing licensed formulations. Several examples of immunological interference between components of a combination vaccine have been observed both in clinical trials and in laboratory tests. Examples of these for D, T and Hib components in DTP and DTaP combinations have been investigated.

Abstract:Purpose: Adhesions between the extraocular muscles and surrounding tissues pose a main cause of failure of strabismus reoperations. Amniotic membrane (AM) transplantation during extraocular muscle surgery, to prevent the formation of adhesions, has been a subject of research during the past decade. This review aims to determine the value, indications, and tips on usage of AM transplantation during strabismus surgery. Materials and methods: All references cited in PubMed in English were searched using the key words: amniotic membrane strabismus or amniotic membrane extraocular muscles, and a brief summary of these was described. In addition, certain articles were chosen to provide introductory information on wound healing and fibrosis, AM properties and how it works after transplantation, and AM processing and preservation. Results: AM used for transplantation during extraocular muscle surgery may be cryopreserved, dried, or fresh. It may be oriented with its stroma or epithelium towards the muscle. It may or may not be fixed with sutures. What were the best choices? Various studies attempted to answer these questions. Many of the studies reviewed, however, were inconclusive or contradictory. Fresh AM seemed effective, but carried a risk of transmission of communicable diseases. Dried membrane was not of value in preventing adhesions. Histopathologically, cryopreserved membrane prevented the development of adhesions in the region of its presence, regardless of its orientation, and without the need for suture fixation. To accentuate this histopathological effect during clinical practice, it was recommended to utilize the largest segment possible of cryopreserved membrane and limit its usage to cases where adhesions are expected to be the main cause of failure of strabismus surgery. Conclusion: Cryopreserved AM transplantation was safe and histopathologically effective in preventing adhesions. This effect was, however, less pronounced clinically. Its use during strabismus reoperations is merited if previous recommendations and precautions are considered.

Abstract:Purpose: To evaluate the effects of using cryopreserved human amniotic membrane (AM) transplant during strabismus reoperations. Methods: A total of 30 patients with persistent strabismus were included in this prospective study. Patients were divided into two groups of 15 patients each. The AM group underwent strabismus reoperation with wrapping of the muscles with cryopreserved amniotic membrane. Controls underwent strabismus reoperation without an AM wrap. Final follow-up visit was scheduled between 3 and 12 months postoperatively. Surgical success was defined as 0Δ-10Δ of horizontal tropia and 0Δ-4Δ of vertical tropia, with no limitation of ductions exceeding -1. A cosmetically acceptable outcome was defined as a tropia of 0Δ-15Δ. Results: Three patients were excluded in the AM group due to incomplete follow-up. A successful outcome was achieved in 7 patients in each group (58% and 47% in groups AM and C, resp.; P = 0.63). A cosmetically acceptable outcome was achieved in 10 patients in the AM group (83.3%) and 12 (80%) in the control group (P = 0.48). The mean ocular deviation angles improved to 8.7Δ ± 12Δ in the AM group and 12.3Δ ± 17.4Δ in the control group (P = 0.63). Ductions improved in 66.7% and 36.4% of the muscles with limited motility in groups AM and C, respectively (P = 0.019). Conclusions: Wrapping the extraocular muscles with cryopreserved AM during strabismus reoperations was of limited clinical benefit. We attributed this to the small segment of AM used and to the presence of other causes of failure rather than adhesions.

Abstract:Introduction: In ophthalmology amniotic membrane is usually used for the treatment of corneal ulcerations and chemical burns of the ocular surface. Also, it is useful in the treatment of conjunctival defects. There are few publications on the long-term outcomes with the use of amniotic membrane to replace the conjunctiva. In our cases, we used it in patients with restrictive strabismus or conjunctival problems during or following complex eye muscle surgery. Materials and methods: In this retrospective case study, we reviewed the charts of the patients who had been treated with Ambio5® (IOP Inc. USA) for the replacement of conjunctiva in the treatment of conjunctival problems during or following complex eye muscle surgeries in our ophthalmology department between 2009 and 2015. Ambio5® is a processed dry, multi-layered amniotic membrane of 110-micron thickness. Inclusion criteria were the use of it in the treatment of conjunctival problems during or following complex eye muscle surgery. Results: Thirty-two patients were identified. Among the 32 identified patients, 7 patients (6 female, 1 male) had restrictive conjunctiva following complex strabismus surgery with functional problems or aesthetic conjunctival scarring requiring transplantation. The mean age of these patients was 47 years (range: 22-60 years). Mean follow-up time was 32 months (range 18-55 months). Only one of these seven patients suffered a discomfort related to conjunctival scarring. The other six patients had restrictive strabismus, four of them suffered from Graves' ophthalmopathy. All patients improved although three patients developed symblepharon. One of these patients needed revision because of a symblepharon. Discussion: The result of our study indicates that commercially prepared amniotic membrane is a good alternative graft to replace conjunctiva. It helps to prevent or cure restrictive strabismus, scarring of the conjunctiva with discomfort, and lack of conjunctiva. It is possible that commercially storable products may increase the risk of symblepharon, the reason is unknown.
New strabismus surgical techniques

Abstract:Five new strabismus surgical procedures were developed by the authors and were presented as a workshop at the AAPOS annual meeting of the American Association for Pediatric Ophthalmology and Strabismus. They are summarized here to introduce these methods to the reader and provide surgeons with more options in approaching difficult strabismus problems. Diagnosis and correction of consecutive strabismus attributable to stretched scar is discussed. The pulley posterior fixation procedure is a unique approach to correct high accommodative convergence or to augment a maximal medial rectus recession. Diagnosis and repair of the partial avulsion (flap tear) of a rectus muscle allows correction of some restrictive strabismus following blunt trauma. The nasal myectomy of the inferior oblique muscle corrects persistent inferior oblique overaction without sacrificing the tether effect gained by a previous anterior transposition of the inferior oblique. The full advancement of the superior oblique tendon offers another option to strengthen a weak superior oblique muscle.
Long-term follow-up of cryopreserved amniotic membrane transplant during strabismus reoperations: Up 85 months' follow-up

Abstract:Purpose: To evaluate the long-term effect of cryopreserved human amniotic membrane transplant during strabismus reoperations. Methods: A total of 15 patients with persistent strabismus were included in a prospective study to evaluate the effect of wrapping the extra-ocular muscles with cryopreserved amniotic membrane during strabismus reoperations. The study end-point was the last follow-up visit. A successful outcome was defined as 0-10∆ of horizontal tropia and 0-4∆ of vertical tropia, with no limitation of ductions exceeding -1. A cosmetically acceptable outcome was defined as a tropia of 0-15∆. Results: Maximum follow-up was 85 months (mean: 25.4 ± 25.5 months). At the last follow-up visit, a successful outcome was achieved in 46.7%, a cosmetically acceptable outcome was achieved in 66.7%, and the mean ocular deviation angles improved from 38.60 ± 14.63∆, preoperatively, to 10.6 ± 11.08∆. Motility limitation on the final follow-up visit exceeded -1 in only 4/180 muscles (2.2%). Conclusion: The effect of cryopreserved amniotic membrane transplantation on the success of strabismus reoperations was moderate in terms of ocular alignment. Its effect was more pronounced in terms of ocular motility. The latter better reflects the level of adhesions. No long-term complications were documented, denoting safety of cryopreserved amniotic membrane usage during strabismus reoperations.
Identification and Correction of Restrictive Strabismus After Pterygium Excision Surgery

Abstract:Purpose: To report the characteristics of patients with restrictive diplopia following pterygium excision and a successful treatment approach for the strabismus. Design: Retrospective interventional case series. Methods: This study was set in a single academic institution and included 15 patients with restrictive diplopia after pterygium excision. Patients with any other reason for strabismus were excluded. Patients were evaluated for deficits with special attention to diplopic measures. The intervention was a combined procedure by a strabismologist and oculoplastic surgeon to correct the diplopia. The primary outcome measurements were subjective and objective improvement of diplopia. Results: Fifteen patients (mean age = 49 years) who developed diplopia after pterygium excision were included. Mean time to diplopia was 6 months. All patients had limited abduction in the previously operated eye causing esotropia in the abductive field (mean deviation = 18 prism diopters). After intervention, all patients were no longer diplopic in primary gaze. In the abductive field, 11 (73%) patients had residual small angle esotropia (mean = 7 prism diopters) in ipsilateral extreme end-gaze only. Only 2 patients required additional surgical intervention for scar tissue removal. No patients underwent medial rectus recession. Conclusions: Restrictive diplopia is a potential complication after pterygium excision, particularly for patients with a history of recurrent pterygia requiring multiple excisions and previous amniotic membrane graft placement with fibrin glue. However, diplopia after pterygium excision in primary position is surgically correctable with scar tissue removal and ocular surface reconstruction, without needing medial rectus recession. Given the high volume of pterygium excision, awareness of postoperative restrictive strabismus and the potential for correction is critical.
Sandwich (Amnion/Conjunctival-Limbal Autograft/Amnion) Transplantation for Recurrent Pterygium with Restrictive Strabismus

Abstract:(1) Background: This study aimed to evaluate the clinical outcome of Sandwich (Amnion/Conjunctival-Limbal Autograft/Amnion) transplantation for recurrent pterygium with restrictive strabismus. (2) Methods: This retrospective study included 11 eyes in 11 patients diagnosed with recurrent pterygium with restrictive strabismus who received sandwich transplantation. The outcomes were measured by pterygium recurrence, best-corrected visual acuity, esotropia (prism diopters), and treatment complications. (3) Results: Eleven patients (six males, five females) had a mean age of 60.5 (range 36-80) years. The previously received pterygium excision surgery number was 1.8 ± 1.02 (range 1-4). The mean follow-up period was 19.9 ± 8.41 (range 12-36) months. All patients had a restriction of abduction in the previously operated eye, causing esotropia in the primary position. Pre-operative esotropia was 17.2 (range 10-30) prims diopter (PD). Five eyes (45.5%) had symblepharon before surgery. All patients were orthotropic until the last follow-up. Symblepharon was released in all eyes. Free ocular motility was present in all eyes. No donor site scar formation, scleral melt, or corneal ulcer was noted. (4) Conclusions: Sandwich transplantation for recurrent pterygium with restrictive strabismus is safe and effective.
The surgical treatment of consecutive strabismus following concomitant horizontal strabismus surgery

Abstract:Objective To investigate the results and techniques of surgical treatment of consecutive strabismus following concomitant horizontal strabismus surgery. Methods Fifty-five patients operated on for consecutive strabismus following their first surgical treatment of concomitant horizontal deviation were analyzed retrospectively. Preoperative visual acuity, deviation, mobility evaluation, stereoacuity with Titmus test and traction test were performed as well as post-operative residual deviation and binocular function. The mean postoperative follow-up time was 2years. Results Thirteen patients with consecutive exotropia due to overcorrection for primary concomitant esotropia were managed with exploratory operation (3 patients) or treated with unilateral lateral rectus muscle recession combined with medial rectus muscle advancement (10 patients),in which 10 patients (76.9%) achieved satisfactory alignment at final follow-up. Eleven patients with consecutive esotropia due to overcorrection for primary concomitant exotropia underwent medial rectus muscle resection and lateral rectus muscle complete of partial advancement to the original insertion and 9 patients in which achieved long-term satisfactory alignment. Fifteen patients with undercorrection for concomitant esotropia were treated with medial rectus marginal myotomy combined with lateral rectus muscle recession (8 patients), unilateral lateral rectus muscle recession (2 patients) or medial rectus recession and lateral rectus resection combined with inferior oblique muscle recession in fellow eye (5 patients with Oblique muscle abnormal), and postoperative orthophoria rate was 86.7%. Sixteen patients with undercorrection for concomitant exotropia were treated with lateral rectus muscle recession and medial rectus muscle resection (6 patients) or combined with inferior oblique muscle recession (2 patients with V-exotropia), cicatricotomy for lateral rectus muscle (4 patients) or adjusted of releasable sutures (4 patients), in which postoperative orthophoria rate was 87.5%. Conclusions The reversal of unilateral medial rectus and lateral rectus recession or resection is an effective alternative for treating postoperative consecutive exotropia due to overcorrection of concomitant horizontal strabismus. Adjusting of releasable sutures provide more stable expections of the results achieved by the re-operation.

Abstract:The steric factors that allow trivalent [(C(5)Me(5))(3)U] (1) to function as a three-electron reductant with C(8)H(8) to form tetravalent [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-C(8)H(8))] (2) have been explored by examining the synthesis and reactivity of the intermediate, "[(C(5)Me(5))(2)(C(8)H(8))U]" (3), and the slightly less crowded analogues, [(C(5)Me(5))(C(5)Me(4)H)(C(8)H(8))U] and [(C(5)Me(4)H)(2)(C(8)H(8))U], that have, successively one less methyl group. The reaction of [{(C(5)Me(5))(C(8)H(8))U(μ-OTf)}(2)] (4; OTf=OSO(2) CF(3)) with two equivalents of KC(5)Me(5) in THF gave ring-opening to "[(C(5)Me(5))(C(8)H(8))U{O(CH(2))(4)(C(5) Me(5))}]" consistent with in situ formation of 3. Reaction of 4 with two and four equivalents of KC(5)Me(4)H generates two equivalents of [(C(5)Me(5))(C(5)Me(4)H)(C(8)H(8))U] (5) and [(C(5)Me(4)H)(2)(C(8)H(8))U] (6), respectively, which in contrast to 3 were isolable. Tetravalent 5 reduces phenazine and PhEEPh (E=S, Se, and Te) to form the tetravalent uranium reduction products, [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-C(12)H(8)N(2))] (7), [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-SPh)(2)] (8), [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-SePh)(2)] (9), and [{(C(5)Me(5))(C(8)H(8))U}(2)(μ-TePh)(2)] (10), consistent with sterically induced reduction. In contrast, the less sterically crowded 6 does not react with these substrates.
Advancement of the muscle insertion toward the limbus as an alternative or adjunct to resection

Abstract:Purpose: To describe the surgical outcomes of patients that underwent strabismus surgery involving advancement of the muscular insertion line toward the limbus. Methods: The medical records of patients who underwent rectus muscle advancement surgery between January 2009 and June 2019 and had at least 1 year of follow-up were reviewed retrospectively. Patients were classified into one of four types of deviation: (1) convergence insufficiency, without distance deviation; (2) small-angle distance esotropia, with diplopia; (3) residual strabismus after previous surgery; and (4) large-angle exotropia. The first group underwent 2 mm advancement of the medial rectus insertion bilaterally; the second, 2-3 mm advancement of the lateral rectus muscle bilaterally; the third, 2-3 mm advancement of the lateral or medial rectus muscle, with or without simultaneous resection; and the fourth, 3 mm advancement of the medial rectus with simultaneous resection, with or after maximum recession of the lateral rectus, bilaterally or unilaterally. Results: A total of 25 patients were included (mean age, 28.76 ± 22.75 years): 4 with convergence insufficiency, 3 with small-angle distance esotropia and diplopia, 16 with residual strabismus, and 2 with large-angle exotropia. Mean postoperative follow-up was 4.76 ± 2.03 years. Surgery was successful in all patients at 1 year (residual deviation ≤10Δ), and all but 1 at 5 years; 1 patient required reoperation. Conclusions: Advancement of the muscle insertion proved successful as an alternative or adjunctive procedure to other forms of muscle tightening in the subtypes of patients evaluated in this study.

Abstract:Abstract Background: This study aimed to report the clinical characteristics and surgical methods of restrictive strabismus secondary to ophthalmic surgery. Methods: This retrospective case series covered 14 restrictive strabismus cases secondary to ophthalmic surgery. After evaluation of the clinical history and the basic ophthalmological findings, the following parameters were examined: squint angles (prism with alternative cover test, Krimsky’s test or Maddox cross), ocular motility, duction test and the forced duction test. All paitients underwent surgery, the strabismus surgery included the excision of adhesions and scar tissue, adhesiolysis, medial and lateral ligaments separated, repositioning of extraocular muscles (according to the degree of deviations). We described the clinical characteristics and evaluated the surgical results in strabismus eyes. Results: All patients were satisfied with the results of surgery, obtained anatomical reduction and partial functional recovery. In nine cases with preoperative diplopia, eight cases had no diplopia after surgery and one patient’s diplopia disappeared after the secondary surgery. The other five cases without preoperative diplopia, but had restricted ocular motility, which improved significantly than before surgery. Conclusions: Orbital surgery, trauma, conjunctival surgery, strabismus surgery and so on, can lead to secondary restrictive strabismus surgery. Strabismus surgical treatments including the full removal of the muscles around the scar, adhesiolysis, medial and lateral ligaments separated, eye muscle surgery, can provide excellent results and patient’s satisfaction.

Abstract:We intend to investigate the indications, complications, and final results of amniotic membrane (AM) transplantation in ocular motility restrictions. Surgeons have tried to prevent restrictive adhesions between the extraocular muscles and surrounding tissues because they cause unpredictable results. AM transplantation wrapping extraocular musculature has been proposed as a technique with good results for this purpose. A search was carried out in Medline, Embase, Cochrane Library and Clinicaltrial, Lilac and Ibecs databases, using the indexed terms" amnion", "strabismus," "strabismus-subheading-surgery" and "ocular motility disorders." The only exclusion criteria were studies conducted in non-humans or studies with insufficient data on eye motility. No study was discarded for analysis because of language, age or methodology. This review includes 165 patients (223 eyes), with a mean follow-up of 11.49 months. The mean age was 21 years old, 47% were males and 57% were children. 14 studies conducted in humans were eligible: 4 single case, 8 case series, 1 cohort study and 1 randomized clinical trial. In the vast majority of these papers, use of AM transplantation to treat (12 out of 14 papers) or either to prevent (2 out of 14 papers) motility restrictions or restrictive strabismus secondary to muscular adhesions. All the studies except one presents very favorable results improving postsurgical eye motility. The cryopreservation method was more widely used, presenting a good safety profile with few adverse effects in the short and medium term. Significant improvement was reported in most patients after the use of amniotic membrane transplantation to treat or prevent ocular motility limitations. Very few complications or adverse effects were documented.
