Abstract:Background and Objectives: Janumet® XR is the combination of sitagliptin and extended metformin hydrochloride produced by Merck Sharp & Dohme. It is specially designed for diabetes mellitus patients taking both drugs already. Janumet® XR exhibited clinically significant blood glucose lowering efficacy and long-term use safety. However, no generic form of Janumet® XR has been approved in western countries. The relatively high cost made the medication less prescribed. A more affordable form of this drug may benefit an immense diabetes mellitus population. The current study compared the bioequivalence (BE) of sitagliptin 100 mg and metformin 1000 mg produced by Nanjing Chia-Tai Tianqing Pharmaceutical Company to Janumet® XR in healthy Chinese subjects. Methods: Twenty-eight healthy Chinese subjects were enrolled in Study 1 and 2, respectively. Both studies were conducted with an open, randomized, two-period crossover design using the test (T) or the reference (R) drug. Study 1 is conducted under the fasting state, and Study 2 is under the fed state. Subjects received an oral dose of sitagliptin 100 mg and metformin 1000 mg, and plasma concentrations of sitagliptin and metformin were determined up to 72 h post-dose. Pharmacokinetic (PK) parameters, including maximum serum concentration (Cmax) and area under the concentration-time curve up to the last quantifiable concentration (AUC0-t) of both sitagliptin and metformin, were calculated and compared between the T and R treatments. Results: In the fasting study, the geometric mean ratios of Cmax, AUC0-t, and AUC0-∞ for sitagliptin were 109.42%, 101.93%, and 101.95%, respectively; the corresponding ratios for metformin were 98.69%, 94.12%, and 93.42%, respectively. In the fed study, the geometric mean ratios of Cmax, AUC0-t, and AUC0-∞ for sitagliptin were 98.41%, 100.30%, and 100.24%, respectively; the corresponding ratios for metformin were 97.79%, 99.28%, and 100.69%, respectively. The 90% CIs of Cmax, AUC0-t, and AUC0-∞ in both studies were all within acceptance limits (80.00%-125.00%). Conclusion: The results demonstrated for the first time that sitagliptin 100 mg and metformin 1000 mg produced by Nanjing Chia-Tai Tianqing Pharmaceutical Company was bioequivalent to the branded Janumet® XR, and both drugs were well tolerated.
Comparative effectiveness of metformin monotherapy in extended release and immediate release formulations for the treatment of type 2 diabetes in treatment-naïve Chinese patients: analysis of results from the CONSENT trial.

Abstract:Aims: Metformin treatment for type 2 diabetes mellitus (T2DM) can be limited by gastrointestinal (GI) adverse events (AEs), resulting in treatment discontinuation. We investigated whether once-daily metformin extended release (XR) is superior in terms of GI tolerability, with non-inferior efficacy, compared with thrice-daily metformin immediate release (IR) in treatment-naive Chinese patients with T2DM. Materials and Methods: This prospective, open-label, randomized, multicentre, phase IV interventional study enrolled Chinese T2DM patients to receive either metformin XR or metformin IR with a 2-week screening period, a 16-week treatment period and a 2-week follow-up period without treatment. Co-primary endpoints were a non-inferiority assessment of metformin XR vs metformin IR in glycated haemoglobin (HbA1c) least squares mean (LSM) change from baseline to week 16 and the superiority of GI tolerability for metformin XR vs metformin IR. Results: Overall, 532 patients were randomized to metformin IR (n=267) or metformin XR (n=265). The HbA1c LSM change was -1.61% and -1.58% in each group, respectively (LSM difference, 0.03; 95% confidence interval [CI], -0.10, 0.17). Incidences of drug-related AEs were 26.5% (n=66) in the metformin IR-only group and 32.2% (n=85) in the metformin XR-only group, and GI AEs were 23.8% and 22.3% in each group, respectively (difference, -1.52; 95% CI, -8.60, 5.56). The treatment difference met the predefined non-inferiority upper CI margin of 0.4% in HbA1c. Conclusions: Metformin XR was non-inferior to metformin IR for the LSM change in HbA1c from baseline to week 16 and not superior to metformin IR for overall GI AE incidence during treatment of Chinese T2DM patients.

Abstract:The incidence of type 2 diabetes is high, and the existing metformin hydrochloride (MH) tablets of 250 mg cannot meet the demands of the Chinese drug market. This study aimed to evaluate the bioequivalence and safety of generic formulations of MH tablets (test formulation [T], 250 mg/tablet) and innovative products (reference formulation [R], 250 mg/tablet) under fasting conditions. This was an open‐label, single‐dose, 2‐period, 2‐sequence crossover, single‐center, randomized phase I clinical trial. T and R were considered bioequivalent if the adjusted geometric mean ratios (GMRs) and 90% confidence intervals of the area under the curve (AUC) and maximum concentration (Cmax) were within the range of 0.8‐1.25. Thirty‐five participants completed the trial. The T/R adjusted GMRs (95.7% for Cmax, 98.7% for AUC0→t, 98.8% for AUC0→∞) were within the acceptable bioequivalence range of 80%‐125%. No serious adverse events or suspected or unexpected serious adverse reactions occurred during this trial. The study findings confirmed that generic MH is a well‐tolerated and bioequivalent alternative to innovative products under fasting conditions in healthy Chinese participants. Racial Comparison of the Pharmacokinetics and Safety of Fixed-Dose Combination of Dapagliflozin/Sitagliptin in Western and Korean Healthy Adults

Abstract:Purpose: We evaluated the pharmacokinetics, safety, and tolerability of a fixed-dose combination (FDC) of dapagliflozin/sitagliptin versus individual component (IC) tablets in healthy Western and Korean participants. The combination of these antihyperglycemic drugs provides efficient glucose control, and the use of FDC has generally been shown to improve medication adherence in individuals with type 2 diabetes mellitus (T2DM). Methods: Two randomized, open-label, two-period, two-treatment, single-dose, single-center, crossover bioequivalence studies conducted on healthy fasted German participants (aged 18-55 years; Western study) and South Korean participants (aged 19-55 years; Korean study) were included. In both studies, pharmacokinetic parameters (maximum [peak] plasma concentration [Cmax], area under the plasma concentration-time curve from zero to the last quantifiable concentration [AUClast], and area under the plasma concentration-time curve from zero to infinity [AUCinf]) were used to assess the bioequivalence of 10 mg dapagliflozin/100 mg sitagliptin FDC (Treatment A) with their ICs (Treatment B) under fasted conditions. Safety and tolerability were assessed throughout the study. Findings: Forty-six healthy participants (male, 60.9%; mean age, 39.5 years; mean body mass index [BMI], 23.9 kg/m2) were randomized in the Western study, and 51 healthy participants (male, 100.0%; mean age, 24.6 years; mean BMI, 23.9 kg/m2) were randomized in the Korean study. In both studies, the participants were randomized 1:1 into treatment sequence AB and treatment sequence BA. Dapagliflozin/sitagliptin FDC was bioequivalent to IC tablets in both Western and Korean studies, as the 90% confidence interval of the FDC to IC ratios of the geometric least-squares means of the pharmacokinetic parameters for both dapagliflozin and sitagliptin was within the 0.8000-1.2500 bioequivalence criterion limit. The observed differences in pharmacokinetic parameters, such as Cmax, AUClast, and AUCinf, between the Western and Korean studies were not clinically meaningful. Dapagliflozin/sitagliptin FDC and their ICs were well tolerated, with no serious adverse events reported in any of the study populations. Implications: The 10 mg dapagliflozin/100 mg sitagliptin FDC and IC formulations were bioequivalent in fasted healthy Western and Korean participants, with no new safety concerns identified, thus offering a useful alternative for patients currently receiving individual medications as part of their treatment regimen. Abstract:ObjectiveThe purpose of this single-center, randomized, open, two-period, two-sequence crossover, single-dose administration, bioequivalence research was to evaluate the bioequivalence and safety of the generic formulations of metformin hydrochloride sustained-release (MH-SR) 500 mg tablets (test preparation [T]: Yuantang® SR) and the original formulation (reference preparation [R]: Glucophage® XR) in 36 healthy Chinese volunteers under postprandial conditions.MethodsSubjects received 500 mg T/R in each period, with a 7-day washout period. Venous blood samples of 4 mL each were collected from each subject 19 times spanning predose (0 h) to 36 h postdose. The metformin concentration in deproteinized plasma was determined by high-performance liquid chromatography–tandem mass spectrometry. Bioequivalence (80.00–125.00%) was assessed by adjusted geometric mean ratios (GMRs) and two-sided 90% confidence intervals (CIs) of the area under the curve (AUC) and maximum concentration (Cmax) for each component. SAS 9.4 software was used for statistical analysis and Phoenix WinNonlin software v7 was used to analyze the pharmacokinetic parameters.ResultsThirty-four volunteers completed the clinical study. The 90% CIs (96.12–105.44% for AUC from time zero to the time of the last measurable concentration [AUCt], 96.22–105.54% for AUC extrapolated from time zero to infinity [AUC∞], and 98.42–105.00% for Cmax) of T/R adjusted GMRs were within the bioequivalence acceptance range of 80.00–125.00%, indicating that they are bioequivalent. No serious adverse events occurred in this study, indicating that the two formulations were effective and well tolerated.ConclusionsYuantang® SR was confirmed to be a well tolerated and bioequivalent alternative to Glucophage® XR when taken under postprandial conditions in healthy Chinese volunteers. Pharmacokinetic and Bioequivalence Studies of 2 Metformin Glibenclamide Tablets in Healthy Chinese Subjects Under Fasting and Fed Conditions

Abstract:The rational combination of oral antidiabetic agents is more likely to provide better glycemic control than monotherapy. Metformin glibenclamide tablets can be used as second-line therapy for patients with type 2 diabetes mellitus who cannot successfully control their blood glucose levels by diet and exercise plus metformin or sulfonylureas. The aim of this study was to evaluate the bioequivalence and safety of metformin hydrochloride and glibenclamide tablets (500 mg/5 mg) prepared by 2 different vendors in healthy Chinese subjects under fasting and fed conditions. This is an open-label, single-center, randomized, 2-formulation, 2-period crossover study. After screening, 40 subjects were enrolled in the fasting trial, while 40 subjects were enrolled in the fed trial. Qualified subjects were randomly assigned to receive a monotherapy dose of 500 mg/5 mg of the test or reference formulation, and after a 1-week washout period, they took the alternative formulation. Blood samples were collected from 24 blood collection sites per cycle for pharmacokinetic analysis until 36 hours after oral administration. In total, 78 subjects completed the study. Under fasting and fed conditions, the geometric mean ratios of maximum plasma concentration, area under the plasma concentration-time curve (AUC) from time 0 to time of last quantifiable drug level , and AUC from time 0 to infinity between the 2 products, as well as the corresponding 90%CIs, were all within the range of 80%-125%. It was found that exposure (AUC from time 0 to infinity) to metformin is decreased by about 25% in the fed state compared to fasting, whereas glibenclamide exposure is increased by about 30% in the fed state. No severe adverse events were observed in the study.

Abstract:Objective: To assess the average bioequivalence of two formulations of metformin after single-dose administration of each treatment to healthy subjects under fasting conditions by assessing the pharmacokinetic measures of systemic exposure and evaluating the confidence intervals (CIs) for each pharmacokinetic parameter.Design: Randomised, comparative, single-dose, open-label, balanced, two-period, two-treatment, crossover study under fasting conditions. Participants: 20 healthy volunteers (ten men and ten women) took part in the study.Methods: Subjects were investigated after a single dose of 850mg after a washout period of 7 days. Plasma samples were taken at regular time intervals according to the study protocol for measuring plasma metformin concentrations. Systemic exposure was estimated with the use of pharmacokinetic parameters (area under the curve of the plasma drug concentrations from time zero to the last sampling time [AUC0-36], area under the curve of the plasma drug concentrations from time zero to infinity [AUC0-∞], time to peak drug concentration [tmax], partial area under the concentration-time curve with a cut-off point at the tmax of the reference product [AUCp], peak plasma drug concentration [Cmax], the ratio Cmax/AUC0-∞, and mean residence time [MRT]). The point estimates of pharmacokinetic parameters (geometric means of the ratios test [T]/reference [R] and the 90% CIs for the ratios of geometric means [T]/[R]), estimated by parametric and nonparametric analysis, were used in the statistical analysis.Results: The point estimates and the 90% CIs after parametric analysis of AUC0-∞ were 0.98 and (0.96–1.21), and after nonparametric analysis were 1.06 and (0.95–1.207), respectively. The two drug products were considered to be bioequivalent and with significant variability across subjects for the pharmacokinetic parameters AUC0–36, AUC0-∞, Cmax and MRT according to analysis of variance of log-transformed data.Conclusions: The two studied formulations of metformin were found to be bioequivalent. They showed similar extents and rates of absorption and similar exposure. However, analysis of variance of logarithmically transformed data revealed significant variability among individuals in AUC0–36, AUC0-∞ and Cmax, making careful individualisation of the metformin dosage important.

Abstract:A randomized, open‐label, 2‐period, 2‐sequence crossover study was conducted to evaluate the pharmacokinetics and bioequivalence of 2 oral formulations of vildagliptin tablets under both fasting and fed conditions in healthy Chinese subjects. A total of 56 healthy subjects were randomized to receive a single 50‐mg dose of either a generic vildagliptin tablet (T) or the reference formulation (R). The washout period was 3 days. Blood samples were collected up to 24 hours postdosing during each period and analyzed for vildagliptin using liquid chromatography‐tandem mass spectrometry (LC‐MS/MS). The 90% confidence intervals for the geometric mean ratios (T:R) of maximum serum concentration, area under the serum concentration‐time curve from time 0 to the last measurable concentration, and area under the serum concentration‐time curve from time 0 to infinity were all within the predefined bioequivalence range of 80%‐125%. This indicates that the generic and reference formulations are bioequivalent under both fasting and fed states. All adverse events reported were mild and transient. High‐fat meals delayed absorption and reduced the maximum peak concentration of both formulations; however, they did not affect the overall exposure. Therefore, vildagliptin can be taken without regard to meals.

Abstract:OBJECTIVE To reevaluate the bioequivalence of post-marketing metformin hydrochloride tablets.METHODS With the original metformin hydrochloride tablets(Glucophage) as the reference preparation,the in vitro dissolution of Glucophage and generic metformin hydrochloride tablets from eight pharmaceutical factories(A-H) was investigated using method 1 described in the appendix X C in Chinese Pharmacopoeia(edition 2010),among which two generic products were chosen for the bioequivalence study as test 1 and test 2 preparations.In a randomized,three-way crossover study,24 healthy male volunteers were given a single oral dose of test 1,test 2 and reference preparations containing 500 mg of metformin hydrochloride.Plasma concentrations of metformin were determined by LC-MS/MS.The pharmacokinetic parameters and relative bioavailability were calculated.The bioequivalence between test 1 and reference preparation,test 2 and reference preparation,and the bioequivalence between the two test preparations were evaluated.RESULTS The in vitro dissolution of the metformin hydrochloride tablets from all the nine factories met the standard of Chinese Pharmacopoeia(edition 2010).Metformin hydrochloride tablets from factory B and H,of which the cumulative dissolution rate-time profiles were significantly different from that of Glucophage,were chosen to be test 1 and test 2 preparations,respectively.The F0-t and F0-∞ were(102.0±13.3)% and(101.9±13.3)% for test 1 preparation,and(94.6±14.7)% and(94.4±14.5)% for test 2 preparation to Glucophage.Both test 1 and test 2 preparations were bioequivalent with reference preparation,and the two test preparations were also bioequivalent.CONCLUSION The bioinequivalence risk of metformin hydrochloride tablets is low.Biowaivers can be granted for generic metformin hydrochloride tablets on the basis of in vitro dissolution evaluation,to save cost,improve efficiency in drug development and supervision,and reduce unnecessary drug exposure in human body.

Abstract:Introduction: Nateglinide or N -(trans-4-isopropylcyclohexyl-1-carbonyl)-D-phenylalanine is a drug with a rapid hypoglycemic effect that is mainly used in the treatment of type 2 diabetes. Very few studies have assessed bioequivalence based on feeding status. This study aimed to assess the pharmacokinetic bioequivalence and safety of nateglinide-containing tablets (0.12 g) in healthy Chinese volunteers under fasting and fed conditions. Methods: The studies were performed in 2017–2018 in the Phase I Clinical Trial Ward of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine, China. Eligible Chinese volunteers received a single 0.12-g dose of the test or reference formulation, followed by a 7-day washout period and administration of the alternate formulation. Blood samples were collected at various time intervals, and plasma nateglinide concentrations were analyzed by liquid chromatography-tandem mass spectrometry. Then, the adverse events, laboratory test results, vital signs, and physical exam findings were compared between the 2 groups. Results: The ratios of the geometric means of C max , AUC 0-t , and AUC 0-inf of the tested to reference preparations under fasting conditions were 105.03% (90% confidence interval [CI]: 99.53–110.83%), 104.02% (90% CI: 101.37–106.74%), and 104.04% (90% CI: 101.38–106.77%), respectively. The same ratios under fed conditions were 96.55% (90% CI: 85.80–108.65%), 103.08% (90% CI: 100.07–106.18%), and 103.07% (90% CI: 100.21–106.01%), respectively. The 90% CI values for C max , AUC 0-t , and AUC 0-inf fell within the accepted range of bioequivalence (80.00–125.0%). Common adverse events included hypoglycemia, heart rate increase, palpitation, sweating, dizziness, and diarrhea. Conclusions: The test formulation (0.12 g) met the CFDA’s regulatory definition for bioequivalence to the reference formulation. Both formulations were well tolerated by healthy Chinese subjects. Trial Registration: This trial has been registered in the Chinese Clinical trial registry (ChiCTR2000030694), March 10, 2020.

Abstract:Purpose: A fixed-dose combination (FDC) product combining dapagliflozin and metformin may increase medication adherence in patients with type 2 diabetes mellitus (T2DM) by minimizing pill burden associated with co-administration of individual component (IC) formulations and, consequently, improve cost-efficiency and compliance. This study evaluated the bioequivalence of the dapagliflozin/metformin FDC product versus IC administration in healthy volunteers from a Chinese population and assessed the safety profile of the FDC product. In addition, pharmacokinetic (PK) and safety comparisons of dapagliflozin and metformin across different regions were conducted to evaluate regional differences. Methods: This single-center, open-label, parallel-cohort, randomized, 2-period, crossover study enrolled Chinese adults (aged 18-55 years). Volunteers in cohort 1 received either a single FDC tablet of dapagliflozin/metformin extended release (XR) (5/500 mg) or IC tablets (dapagliflozin [5 mg] and metformin XR [500 mg]). Volunteers in cohort 2 received a higher dosage in a similar manner (dapagliflozin [10 mg] and metformin XR [1000 mg]). Volunteers in each cohort were subsequently crossed over to receive the alternate cohort treatment. Plasma concentrations of dapagliflozin and metformin were determined, and bioequivalence analyses were performed under standard fed conditions. Findings: Eighty healthy Chinese volunteers (89.9% male; mean age, 28.7 years) were randomized into cohort 1 (n = 40) and cohort 2 (n = 39; 1 volunteer withdrew before receiving study treatment). The mean plasma concentration-time profiles of the dapagliflozin and metformin FDC and IC formulations for both doses were found to be nearly superimposable. Dapagliflozin and metformin XR FDC were bioequivalent to the IC tablets, with 90% CIs for each pairwise comparison contained within the 80% to 125% bioequivalence limits. Both the FDC and IC formulations were well tolerated, with no serious adverse events/death. PK parameters for dapagliflozin in the Chinese volunteers were slightly to moderately higher than those from studies conducted in Brazil, Russia, and the United States, and the safety profile of the dapagliflozin/metformin FDC product was consistent with that of other studies. The difference in PK parameters among the 4 regions was not clinically meaningful. Implications: The bioequivalence of the dapagliflozin/metformin FDC and IC formulations in healthy Chinese adults was established without any new safety concerns. Notably, the observed bioequivalence may be extrapolated to patients with T2DM as the PK parameters of dapagliflozin and metformin in healthy adults are similar to those reported in patients with T2DM. Efficacy and Safety of Combination Therapy with Vildagliptin and Metformin Versus Metformin Uptitration in Chinese Patients with Type 2 Diabetes Inadequately Controlled with Metformin Monotherapy: A Randomized, Open-Label, Prospective Study (Vision)

Abstract:AimsTo compare the efficacy and safety of combination of vildagliptin and metformin therapy with metformin uptitration in Chinese patients with type 2 diabetes (T2DM) inadequately controlled with low‐dose metformin.MethodsIn this 24‐week prospective, randomized, multicentre, open‐label study, patients with T2DM inadequately controlled with metformin ≤1000 mg daily were divided 1 : 1 : 1 : 1 into four prespecified subgroups based on age and body mass index (BMI). Patients in each subgroup were randomized 5 : 1 to receive either vildagliptin (50 mg twice daily) plus metformin [500 mg twice daily; vildagliptin and low‐dose metformin (VLDM) group] or metformin uptitration [1000 mg twice daily; high‐dose metformin (HDM) group]. The primary endpoint was change in glycated haemoglobin (HbA1c) from baseline at week 24. The key secondary endpoints included percentage of patients achieving target HbA1c without adverse gastrointestinal (GI) events and mean change in fasting plasma glucose (FPG) from baseline to week 24.ResultsA total of 3084 patients were randomized. HbA1c reduction of 0.54% at week 24 in the VLDM group was non‐inferior and statistically superior compared with 0.40% in the HDM group (P < 0.0001). VLDM's non‐inferiority to HDM was confirmed in the four subgroups and its superiority was shown for all subgroups (p < 0.05) except for the subgroup of patients aged <60 years with a BMI of ≥24 kg/m2. Compared with HDM, VLDM significantly increased the percentage of patients achieving HbA1c ≤6.5% and HbA1c ≤6.5% without GI events. FPG levels in the VLDM group were lower at week 24 numerically than in the HDM group. The two treatment arms had similar safety profiles.ConclusionsVLDM was non‐inferior and statistically superior to HDM in glycaemic control in Chinese patients with T2DM inadequately controlled with low‐dose metformin.

Abstract:OBJECTIVE: To reevaluate the bioequivalence of marked metformin hydrochloride enteric-coated tablets. METHODS: Using method 2 described in the appendix X D and apparatus 1 described in the appendix X C in Chinese Pharmacopoeia(edition 2010), the in vitro release of metformin hydrochloride enteric-coated tablets from different pharmaceutical factories (A, B, C, and D) was investigated. The contents were measured by utlraviolet (UV) spectroscopy. Generic tablets from factory A and D were chosen to be test 1 and test 2 preparations, with the innovative metformin hydrochloride tablets (Glucophage) as the reference preparation. In a randomized, three-way crossover study, 21 healthy male volunteers were given a single oral dose of test 1, test 2 and reference preparations containing 500 mg of metformin hydrochloride. Plasma concentrations of metformin were determined by LC-MS/MS. The pharmacokinetic parameters and relative bioavailability were calculated. The bioequivalence between test 1 and reference preparation, test 2 and reference preparation, and the bioequivalence between the two test preparations were evaluated. RESULTS: The in vitro release of metformin hydrochloride enteric-coated tablets from factory B, C and D met the standard of Chinese Pharmacopoeia(the supplement edition of 2010) while that of the tablets from factory A did not. Metformin hydrochloride enteric-coated tablets from factory A and D were chosen to be test 1 and test 2 preparations, respectively. The F 0-t and F 0-∞ were (72.8 ± 9.7)% and (73.2 ± 10.0)% for test 1 preparation, and (45.5 ± 16.2)% and (46.2 ± 16.0)% for test 2 preparation, respectively. It was failed to conclude that test 1 and reference preparation were bioinequivalent. Test 2 and reference preparation were bioinequivalent, and the two test preparations were also bioinequivalent. CONCLUSION: The bioinequivalence risk of metformin hydrochloride enteric-coated tablets is high. For metformin hydrochloride enteric-coated tablets, not only conducting pre-marketing bioequivalence study with proper reference preparation, but also enhancing post-marketing surveillance and reevaluation of bioequivalence are very important for maintaining the consistency of drugs quality. Abstract:Purpose Glucophage R (Merck Healthcare KGaA, Darmstadt, Germany) is the originator brand of metformin hydrochloride, an oral antidiabetic drug. Metformin is recommended in guidelines as first-line treatment of type 2 diabetes mellitus and increasingly in related insulin-resistant conditions, such as prediabetes and polycystic ovary syndrome. The GelShield R sustained-release formulation tablet of Glucophage R has been improved from the historic version marketed in 2000. Bioequivalence has been demonstrated stepwise along this evolution; however, a head-to-head evaluation between the initial and the current version is missing. This analysis aims to close this gap and to determine bioequivalence between related originator GelShield R sustained-release formulations of metformin, Glucophage R (GXR 500 mg), from Europe and the United States. Methods Data from seven randomized crossover bioequivalence studies in 361 healthy participants of Asian and non-Asian ethnicity from Europe, the United States, and Asia were considered. All evaluated a single oral dose of 500 mg of the test and reference formulation in healthy male and female participants in fed and fasted state. Bioequivalence was evaluated by means of a combined bridging analysis of available data on the current round tablet from Europe (rGXR EU) and the historic oblong tablet from the United States (oGXR US) in healthy Asian and non-Asian participants under fed and fasting conditions. Bioequivalence between the two formulations was assessed statistically with a mixed effects model for AUC 0-t , C max , and AUC 0-inf . Findings In all studies, bioequivalence between the respective test and reference formulations of GXR was shown. Statistical analysis of pooled pharmacokinetic data of 2 (primary pooling set) or 3 studies (secondary pooling set) demonstrated bioequivalence between rGXR EU and oGXR US via bridging with oGXR EU. The 90% CI for the geometric mean ratio of all pharmacokinetic parameters was within the bioequivalence range of 0.80 to 1.25. In the primary pooling set, geometric least squares mean ratios in the fed group ranged from 0.9931 (90% CI, 0.9151–1.0778) for AUC 0-inf to 1.1344 (90% CI, 1.0711–1.2014) for C max ; results in the fasted group were similar. The secondary pooling set, which added a study in Asians, confirmed these findings. Implications Bioequivalence was determined between sustained-release formulations of Glucophage R from Europe and the United States under fasted and fed conditions in healthy men and women, including different ethnicities. The efficacy and safety of Glucophage R XR can be claimed along the evolution from oGXR US, via oGXR EU to rGXR EU, and in several ethnicities and production sites.

Comparative Efficacy and Safety of Glucose-Lowering Drugs as Monotherapy for Type 2 Diabetes: A Systematic Review and Indirect Treatment Comparison

Abstract:Background: There are multiple alternative glucose-lowering drugs available as initial monotherapy for patients with type 2 diabetes (T2DM) with contraindications or intolerance of metformin. However, little systematic and comparative data is available for them as initial monotherapy. We compared the efficacy and safety of them as monotherapy for T2DM. Methods: In this systematic review and indirect treatment comparison, PubMed, Embase, Web of Science, CNKI, WanFang Data, and Chongqing VIP were searched for randomized controlled trials (1990-2020) comparing a glucose-lowering drug monotherapy with placebo (or lifestyle-intervention) in adult T2DM patients. The drug classes included metformin, SUs, TZDs, glinides, AGIs, DPP-4is, SGLT2is, GLP-1RAs, and insulins. The drugs were firstly compared with placebo/lifestyle-intervention using meta-analysis, then compared against each other using indirect treatment comparison. Weighted mean difference with 95% CIs were calculated for HbA1c, FPG, BMI, TC, HDL-C, and SBP. Relative risk with 95% CIs were calculated for hypoglycemia. This study is registered with PROSPERO (CRD42020170769). Findings: 185 studies assessing 23 drugs from eight drug classes were included (no eligible study was found for insulin). SUs were most efficacious in reducing HbA1c and FPG but increased hypoglycemic risks. Metformin was most efficacious in lowering BMI. GLP-1RAs were most efficacious in reducing TC. TZDs were most efficacious in increasing HDL-C. SGLT2is were most efficacious in lowering SBP. Overall, GLP-1RAs comprehensively controlled HbA1c, FPG, BMI, TC, and SBP, whilst metformin controlled HbA1c, FPG, BMI, TC, and HDL-C, and both of them did not increase hypoglycemic risks. Our observations also showed that although drug class is important, drugs within the same class may display important differences in treatment effects. Interpretation: Our results, along with individualized patient characteristics, preferences, and clinical needs, should be considered when selecting a drug as monotherapy for T2DM in clinical practice. Abstract:Background Type 2 diabetes mellitus (T2DM) arises due to a range of pathological abnormalities, necessitating a combination therapy to achieve optimal glycemic control. Vildagliptin, an effective and selective DPP-4 inhibitor, and pioglitazone, an insulin sensitizer, offer distinct mechanisms of action. Hence, the integration of these medications represents a logical and justified therapeutic strategy Objective To compare the efficacy, safety, and tolerability of vildagliptin and pioglitazone 50 mg/15 mg fixed-dose combination (FDC) tablets with individual monotherapy vildagliptin 50 mg and pioglitazone 15 mg tablets in Indian T2DM patients who were inadequately controlled on metformin monotherapy. Methods This was a randomized, open-label, comparative, multicenter, phase III study involving 195 T2DM patients with inadequate glycemic control on metformin ≥ 1000 mg/day. Patients were randomly assigned in a 1:1:1 ratio to the test product group (n=65) (vildagliptin 50 mg + pioglitazone 15 mg FDC tablets), reference product group 1 (n=65) (vildagliptin 50 mg tablet), or reference product group 2 (n=65) (pioglitazone 15 mg tablet reference product). The primary endpoint was the mean change in HbA1c levels from baseline to end of the study visit (12 weeks (84 days ±2)). The secondary endpoints were the mean change in fasting plasma glucose (FPG) and 2-hr postprandial plasma glucose (2-hr PPG) levels. Safety parameters were assessed till the end of the study. Results A total of 178 patients completed the study. At 12 weeks, the mean HbA1c level in the test group reduced to 6.85 ± 1.27%, in the reference product 1 group to 7.56 ± 1.72%, and in the reference product 2 groups to 7.37 ± 1.59%. The mean change in Hb1Ac from baseline in the test group was statistically significant compared to the reference groups (p=0.037). Similarly, the mean changes in the FPG and 2hr-PPG with the test product were statistically significant compared to reference products (p=0.041). The adverse events were comparable across all the treatment groups. Conclusion In Indian T2DM patients inadequately controlled on a daily maximum dose of metformin, treatment with vildagliptin and pioglitazone FDC showed better glycemic control than either vildagliptin or pioglitazone along with a good tolerability profile.

Abstract:Acarbose and metformin have been recommended both as monotherapy and add-on therapy in type 2 diabetes mellitus. A novel fixed-dose combination (FDC) of acarbose and metformin has been developed to improve compliance and patient adherence to therapy. The current study investigated the bioequivalence (BE) between acarbose/metformin FDC (50 mg/500 mg) with corresponding loose combination of individual components under fasting conditions in healthy Chinese male and female subjects, using a randomized, 2-period, 2-way crossover study design. Pharmacodynamic parameters of serum glucose ratio between treatment day and baseline (ratio of maximum concentration [Cmax ], day 1/Cmax , day -1 and ratio of area under the concentration-time curve [AUC] from time 0 to 4 hours, day 1/AUC from time 0 to 4 hours, day -1) were used as the primary variables to evaluate BE of acarbose. Pharmacokinetic parameters Cmax , AUC from time 0 to the last data point greater than the lower limit of quantification, and AUC were used to evaluate BE of metformin. The results showed that the 90% confidence intervals of the ratios of all primary target variables including ratio of Cmax , day 1/Cmax , day -1 and ratio of AUC from time 0 to 4 hours, day 1/AUC from time 0 to 4 hours, day -1 for acarbose, and Cmax , AUC from time 0 to the last data point greater than the lower limit of quantification, and AUC for metformin all fell within the acceptance limits of 0.8 to 1.25. Thus, BE between 50-mg acarbose and 500-mg metformin as an FDC and loose combination was established. Furthermore, different kinds of exploratory pharmacodynamic parameters (based on either serum glucose or insulin) including several newly proposed parameters were also investigated for acarbose BE evaluation in this study, and inconsistent results were observed.

Abstract:This article aims to assess the bioequivalence of the test and the reference metformin hydrochloride tablets in healthy Chinese volunteers under fasting and fed conditions and to explore the effect of food on the pharmacokinetic (PK) profiles of both formulations. In total, 56 healthy Chinese subjects (28 in each group) were enrolled in this randomized, open, single‐center, single‐dose, 2‐treatment, 2‐sequence, 2‐cycle cross clinical trial. The subjects were administrated a single dose of the test and the reference tablets at 0.25 g with a 7‐day washout. Venous blood samples of all subjects were taken from predose 0 hour to postdose 24 hours according to the planned times. PK parameters for metformin were analyzed and calculated with noncompartmental methods. There were no significant differences in the PK parameters between the 2 formulations under both the fasting and the fed states. The 90% confidence intervals of 2 formulations were within 80.00%‐125.00% based on Cmax, AUC0‐t, and AUC0‐∞ under both conditions. High‐fat and high‐calorie diets delayed the Tmax and reduced the AUC0‐t and AUC0‐∞. No severe adverse events occurred in this study. Two metformin hydrochloride tablets were bioequivalent under both fasting and fed states; the high‐fat and high‐calorie diet could lower the rate and extent of absorption of metformin in healthy Chinese volunteers.
