Abstract:OBJECTIVE:To study the value of bone marrow biopsy imprint in evaluating cellularity. METHODS:The bone marrow tissues were obtained by trephine biopsy from 272 patients, and then put on the slides to make the imprints. The imprints was stained by Wright-Giemsa method, and the bone marrow smears and imprints were examined simultaneously according to the bone marrow cellularity criteria. RESULT:In bone marrow cellularity, four grades (distinct decrease, extreme decrease, distinct increase, and extreme increase) were significantly higher in bone marrow imprints than those in bone marrow smears (P <0.05), but there was no significantly differences between bone marrow imprints and sections (P >0.05). Using bone marrow sections as standard, in cellularily decreasing samples, the consistent rate of bone marrow imprints and smears were both high (84.4% and 97.9%), in the group of the normal and increased cellularity, the consistent rate of the bone marrow imprints (84.4% and 97.7%) was significantly higher than that in smears (60% and 64%, P<0.01). The sensitivity, specificity, Youden index, positive predictive value and positive likelihood rate of bone marrow imprints were all higher than those of the smears. Using the bone marrow sections as gold standard, in 124 cases with decreased cellularity in smears, the positive diagnosis rate for aplastic anemia and dyshaematopoiesis based on bone marrow imprints was 37.1% with a false positive rate of 7.3% which was lower than that of the bone marrow smears (false positive rate of 29.8%, P<0.01). CONCLUSION:To evaluate bone marrow cellularity, bone marrow imprint is better than bone marrow smear. The combination of the two examinations can make the diagnosis more convenient and quicker.
A Retrospective Comparative Study on the Diagnostic Efficacy and the Complications: Between CassiII Rotational Core Biopsy and Core Needle Biopsy

Abstract:Accurate pathologic diagnosis and molecular classification of breast mass biopsy tissue is important for determining individualized therapy for (neo)adjuvant systemic therapies for invasive breast cancer. The CassiII rotational core biopsy system is a novel biopsy technique with a guide needle and a “stick-freeze” technology. The comprehensive assessments including the concordance rates of diagnosis and biomarker status between CassiII and core needle biopsy were evaluated in this study. Estrogen receptor (ER), progesterone receptor (PgR), human epidermal growth factor receptor 2 (HER2), and Ki67 were analyzed through immunohistochemistry. In total, 655 patients with breast cancer who underwent surgery after biopsy at Sir Run Run Shaw Hospital between January 2019 to December 2021 were evaluated. The concordance rates (CRs) of malignant surgical specimens with CassiII needle biopsy was significantly high compared with core needle biopsy. Moreover, CassiII needle biopsy had about 20% improvement in sensitivity and about 5% improvement in positive predictive value compared to Core needle biopsy. The characteristics including age and tumor size were identified the risk factors for pathological inconsistencies with core needle biopsies. However, CassiII needle biopsy was associated with tumor diameter only. The CRs of ER, PgR, HER2, and Ki67 using Cassi needle were 98.08% (kappa, 0.941; p<.001), 90.77% (kappa, 0.812; p<.001), 69.62% (kappa, 0.482; p<.001), and 86.92% (kappa, 0.552; p<.001), respectively. Post-biopsy complications with CassiII needle biopsy were also collected. The complications of CassiII needle biopsy including chest stuffiness, pain and subcutaneous ecchymosis are not rare. The underlying mechanism of subcutaneous congestion or hematoma after CassiII needle biopsy might be the larger needle diameter and the effect of temperature on coagulation function. In summary, CassiII needle biopsy is age-independent and has a better accuracy than CNB for distinguishing carcinoma in situ and invasive carcinoma.

Abstract:Molecular analyses such as fluorescence in situ hybridization (FISH) and polymerase chain reaction (PCR) are demanded to improve diagnostic accuracy in addition to immunohistopathology of bone marrow (BM) trephine specimens. Conventional BM embedding method needs decalcification, and its procedure may impair tissue morphology and DNA quality. Here, we report an undecalcified method by which glycol methacrylate resin is polymerized at low temperature (4 degrees C). Using this method, BM enzyme activity and antigenic determinants are well preserved, and moreover, DNA extracted from plastic embedding sections is suitable for PCR amplification and sequencing, FISH analysis can be well done because of the DNA integrity of BM sections. If working with BM trephine specimen, our protocol offers the possibility to combine superior morphology with modern molecular analysis. Microsc. Res. Tech. 73: 1067-1071, 2010. The Clincal Application in Cytology Examination of Bone Marrow Biopsy Imprints

Abstract:Objective To study the application value in bone marrow biopsy imprint(called imprint for short).Methods After the 272 patients' marrow aspirates were obtained through the posterior superior iliac spine,the marrow tissue(length usually than 0.6 cm) which was obtained by trephine biopsy was put on the slides to make the imprints(the tissues were fixed in Bouin's liquid) and the imprints cytology examination was carried out compared with the marrow smears and sections.Results The cellularity in imprints was significantly higher than that in smears,but there was no significantly difference compared with sections.When the cellularity in imprints decreased,it should be decreased in smears,but when the cellularity in smears decreased,that in the imprints often increased.As the marrow section was used as the standard,the diagnosis function index and Youden index in imprints were both better than that in smears.Conclusion The imprints have obviously advantage in evaluating cellularity,and also can know the general situation of tissue morphology to remedy the deficiency of smears.To take examination combining with smears can improve the diagnosis level of smears,so application should be extended as a routine item.
Comparison of Different Fixation Methods for Combined Histological and Biomolecular Analysis of Fixed and Decalcified Bone Samples

Abstract:The combination of histological and biomolecular analyses provides deep understanding of different biological processes and is of high interest for basic and applied research. However, the available analytical methods are still limited, especially when considering bone samples. This study compared different fixation media to identify a sufficient analytical method for the combination of histological, immuno-histological and biomolecular analyses of the same fixed, processed and paraffin embedded bone sample. Bone core biopsies of rats’ femurs were fixed in different media (RNAlater + formaldehyde (R + FFPE), methacarn (MFPE) or formaldehyde (FFPE)) for 1 week prior to decalcification by EDTA and further histological processing and paraffin embedding. Snap freezing (unfixed frozen tissue, UFT) and incubation in RNAlater were used as additional controls. After gaining the paraffin sections for histological and immunohistological analysis, the samples were deparaffined and RNA was isolated by a modified TRIZOL protocol. Subsequently, gene expression was evaluated using RT-qPCR. Comparable histo-morphological and immuno-histological results were evident in all paraffin embedded samples of MFPE, FFPE and R + FFPE. The isolated RNA in the group of MFPE showed a high concentration and high purity, which was comparable to the UFT and RNAlater groups. However, in the groups of FFPE and R + FFPE, the RNA quality and quantity were statistically significantly lower when compared to MFPE, UFT and RNAlater. RT-qPCR results showed a comparable outcome in the group of MFPE and UFT, whereas the groups of FFPE and R + FFPE did not result in a correctly amplified gene product. Sample fixation by means of methacarn is of high interest for clinical samples to allow a combination of histological, immunohistological and biomolecular analysis. The implementation of such evaluation method in clinical research may allow a deeper understanding of the processes of bone formation and regeneration.

Abstract:To investigate the effect of pretreatment of tumor biopsy specimens using fixed, dehydrated and transparent three-in-one composite environmental protection reagent ultrasound tissue rapid processing technique on subsequent detection. From April 2020 to October 2020, a total of 100 cases including breast, stomach and lung tissues were submitted to our diagnosis, and 3 specimens were collected from each specimen and divided into the control group (traditional biopsy tissue processing method), experimental group 1 (3.7% neutral buffered formaldehyde fixation, compound environmental protection reagent rapid ultrasound tissue processing technique, processing temperature 48 °C, time 20 minutes/time, twice, wax immersion temperature 62 °C, time 25 minutes) and experimental group 2 (3.7% neutral buffered formaldehyde fixation, compound environmental protection reagent rapid ultrasound tissue processing technique, processing temperature 50 °C, time 15 min/time, twice, Wax dipping temperature 64 °C, time 20 minutes). The effects of different treatments on hematoxylin eosin section, immunohistochemistry (IHC) and molecular pathological examination were analyzed. The detection results of hematoxylin eosin, fluorescence in situ hybridization and IHC against human epidermal growth factor receptor 2 and epidermal growth factor receptor gene mutation in the experimental group were completely consistent with those in the control group. There was no significant difference in the results between experiment 1 and experiment 2 groups. The rapid processing technique of ultrasound tissue with compound environmental protection reagent can be applied to the rapid detection of tumor biopsy specimens, and different processing temperatures and durations have no significant effect on the accuracy of HE staining, IHC, fluorescence in situ hybridization and gene mutation detection.
Improvement and Application of Diagnostic Techniques of the Skeletal Muscle Biopsy

Abstract:OBJECTIVE:To summarize the improvement of various muscle staining techniques and discuss their application in diagnosis of neuromuscular diseases.METHODS:Three hundred cases of skeletal muscle biopsy samples were examined by histopathological methods. The flash-freezing techniques were used for the preparation of frozen section, which were stained with HE, Gomori trichromic (GMR), glycogen (PAS), and fat acid (oil red O); the enzyme histochemical staining with myosin adenosine triphosphatase (ATPase), and NADH-TR. Those stained methods had been improved. The immunohistochemical staining with dystrophin; and transmission electronic microscopy were used.RESULTS:Deepfreeze with heteropentane-liquid nitrogen and flash-freezing techniques could avoid artifacts of ice crystal vacuolation. GMR staining mainly showed the degenerative and necrotic lesions of mitochondria and muscle fibers. Oil red O staining showed the increase of lipid in muscle fibers. PAS staining mainly showed glycogen and glycoprotein. Application of frozen sections in muscular tissue was better than that of paraffin sections. NADH-TR staining and ATPase staining could distinguish the two types of muscle fibers, and show the changes of inner structure of muscle fibers and mitochondria enzymes.CONCLUSION:The distribution and characteristic pathological changes of the two types of muscle fibers can be showed clearly by enzyme and non-enzyme histochemical staining techniques of the skeletal muscle. These methods can compliment with each other. Only after understanding the technical principles can we master and apply these methods.
Qualitative Histopathological Classification of Primary Bone Tumors Using Deep Learning: A Pilot Study

Abstract:Background Histopathological diagnosis of bone tumors is challenging for pathologists. We aim to classify bone tumors histopathologically in terms of aggressiveness using deep learning (DL) and compare performance with pathologists. Methods A total of 427 pathological slides of bone tumors were produced and scanned as whole slide imaging (WSI). Tumor area of WSI was annotated by pathologists and cropped into 716,838 image patches of 256 × 256 pixels for training. After six DL models were trained and validated in patch level, performance was evaluated on testing dataset for binary classification (benign vs . non-benign) and ternary classification (benign vs . intermediate vs . malignant) in patch-level and slide-level prediction. The performance of four pathologists with different experiences was compared to the best-performing models. The gradient-weighted class activation mapping was used to visualize patch’s important area. Results VGG-16 and Inception V3 performed better than other models in patch-level binary and ternary classification. For slide-level prediction, VGG-16 and Inception V3 had area under curve of 0.962 and 0.971 for binary classification and Cohen’s kappa score (CKS) of 0.731 and 0.802 for ternary classification. The senior pathologist had CKS of 0.685 comparable to both models ( p = 0.688 and p = 0.287) while attending and junior pathologists showed lower CKS than the best model (each p < 0.05). Visualization showed that the DL model depended on pathological features to make predictions. Conclusion DL can effectively classify bone tumors histopathologically in terms of aggressiveness with performance similar to senior pathologists. Our results are promising and would help expedite the future application of DL-assisted histopathological diagnosis for bone tumors.

In this paper we show how recently developed robust identification and control synthesis techniques can be brought to bear on the problem. These results are experimentally validated using a Bisight robotic head.

Abstract:Bone marrow biopsy is one of the important means of hematopathological diagnosis, which has decisive diagnostic significance for various benign and malignant lymphohematopoietic system diseases. Its diagnostic value includes morphological observation, immunohistochemistry, genetics, and molecular biology testing. Owing to the unique nature of bone marrow biopsy, decalcification is an essential step in the pre-treatment process. Its purpose is to remove calcium from bone tissue, preserve intact collagen fiber components, facilitate tissue sectioning, and prevent tissue detachment during staining. If bone marrow biopsy lacks sufficient decalcification, preparing a section is difficult. Conversely, if decalcification is excessive, it can seriously disrupt tissue antigen activity. Therefore, a decalcification method with high decalcification efficiency and mild antigen damage is essential for bone marrow biopsy. This article introduces a bone marrow biopsy tissue decalcification method with high efficiency and less antigen loss: decalcification is performed at room temperature with 12% formic acid and 8% hydrochloric acid decalcification solution on a shaker.
Recent progress of molecular pathology for bone tumors

Abstract:Although classic morphology is the cornerstone for the diagnosis of bone tumors, the rapidly developed molecular pathology based on Next Generation Sequencing (NGS) has evolved current diagnostic techniques. At the same time, new disease entities based on molecular abnormalities were constantly reported, which converted the morphological pathological classifications into molecular categories. At present, bone tumors could be roughly classified as tumors with simple karyotypes and those with complex karyotypes by molecular alterations. The previous classification can be subclassified as tumors that carry specific translocations, somatic gene mutations, or those with specific amplifications. However, the later groups usually lack specific alterations. The present review discusses various updates on molecular pathology in detail based on new categories of bone tumors proposed by World Health Organization (WHO) in 2019 aiming to further provide guidance for evidenced-based treatment. Some examples are included, such as giant cell tumor of bone (H3F3A p.G34W), chondroblastoma (H3F3B p.K36M), chondrosarcoma (IDH1/2 mutation), aneurysmal bone cyst (USP6 rearrangements) and so on. All mutual alterations play an increasingly important role in reaching a diagnosis and in patients management.

Abstract:Abstract The molecular characterization of soft tissue and bone tumors is a rapidly evolving field that has changed the perspective of how these tumors are diagnosed today. Morphology and clinico-radiological context still represent the cornerstone of diagnostic considerations but are increasingly complemented by molecular data that aid in objectifying and confirming the classification. The spectrum of analyses comprises mutation or gene fusion specific immunohistochemical antibodies, fluorescence in situ hybridization, DNA and RNA sequencing as well as CpG methylation profiling. This article provides an overview of which tools are presently available to characterize bone and soft tissue neoplasms molecularly, what limitations should be considered, and what conclusions can be drawn from the individual findings.

Molecular Genetic Analysis in Surgical Pathology

Abstract:Objective:To highlight the molecul ar biologic markers and techniques of four diseases studied currently in our lab oratory. Methods: Molecular genetic techniques such as PCR, RT -PCR, PCR-RFLP, TG GE, direct DNA sequencing and immunostaining are applicable to diagnostic surgi cal pathology. Results:(1)Synovial sarcoma: A characteristic S YT-SSX fusion gene resulting from chromosomal translocation t(X;18)(p11;q11) was detectable in sy n ovial sarcoma. SYT-SSX fusion transcripts were detected by the reverse-transcr ip tase polymerase chain reaction in paraffin-embedded synovial sarcoma tissues. I t was considered as a defining diagnostic marker of synovial sarcomas and the su bty pes of SYT-SSX fusion transcripts(SYT-SSX1 and SYT-SSX2)might yield prognosti c inf ormation.(2)Anaplastic large cell lymphoma(ALCL): Some ALCLs showed a unique chr omo somal abnormality t(2;5)(p23;q35) which caused ALCLs expressing the anaplastic l ymphoma kinase(ALK) protein fused with NPM. Immunostaining for ALK-NPM protein s howed both nuclear and cytoplasmic staining in these tumor cells carrying t(2;5). It is important to recognize the ALK + group among the ALCLs because of the e xc ellent prognosis.(3)Familial hypercholesterolemia(FH): Molecular genetic analysis of gene for the LDL receptor in affected families could contribute towards earl y assessment for the diagnosis of FH and thus to prevention of life threatening ca rdiovascular episodes in asymptomatic subjects. Promoter region and all of the 1 8 exons of LDLR gene including the intron-exon boundaries were amplified by pol y merase chain reaction(PCR)and were analyzed by temperature gradient gel electrop horesis(TGGE). TGGE combined with DNA sequencing can be used in the genetic diag nosis of FH.(4)Rapid detection of YMDD variants of HBV polymerase gene with long - term lamivudine therapy: long-term lamivudine therapy is associated with mutati o ns in the YMDD locus of the HBV polymerase gene, leading to lamivudine resistanc e. Detection of emerging HBV variants by direct sequencing of the HBV genome is excessibly time-consuming and high-cost for studying a large number of clinica l samples. It is essential to look for a rapid low-cost technique. HBV DNA of a c o nserved polymerase gene segment containing the YMDD locus, was amplified from se rum and liver biopsies by polymerase chain reaction(PCR) and sequenced. HBV geno type was analysed by means of restriction fragment length polymorphism(RFLP) of the PCR product. The PCR-RFLP assay used in this study, in perfect concordance with direct sequencing, is an accurate and simple method for genotyping lamivudi ne-resistant HBV variants. Conclusion: The practice of surgical pat hology tradit ionally relies on the evaluation of the gross and microscopic features of tissue specimens, integrated with the clinical history and other laboratory data. Hist ologic examination of biopsy specimens has for many decades played the central role in the determination and classification of diseases.Yet there are definite limitations to histologic diagnosis. For instance, histologic examination of ti ssues is only partly successful to predict prognosis. It is in the desire to ove rcome the limitations of histologic examination that molecular genetic analysis of tissue specimens has been introduced with the overall goal of classifying dis eases into biologically homogenous groups.
Update on immunohistochemistry in bone and soft tissue tumors: Cost-effectively replacing molecular testing with immunohistochemistry

Abstract:Soft tissue tumors form part of a challenging domain in diagnostic pathology owing to their comparative rarity, astonishing histologic diversity, and overlap between entities. Many of these tumors are now known to be defined by highly recurrent, or, in some instances, unique molecular alterations. Insights from gene profiling continue to elucidate the wider molecular landscape of soft tissue tumors; many of these advances have been co-opted by immunohistochemistry (IHC) for diagnostic applications. There now exists a multitude of antibodies serving as surrogate markers of recurrent gene fusions, amplifications, and point mutations, which, in certain settings, can replace the need for more resource and time-intensive cytogenetic and molecular genetic analyses. IHC presents many advantages including rapid turnaround time, cost-effectiveness, and interpretative reproducibility. A sensible application of these immunohistochemical markers complemented by a working knowledge of the molecular pathogenesis of bone and soft tissue tumors permits accurate diagnosis in the majority of cases. In this review, we will outline some of these biomarkers while emphasizing molecular correlates and highlighting interpretative challenges and pitfalls.
Comparison of two pathological processing methods for large endoscopic submucosal dissection (ESD) specimens

Abstract:Aims Accurate histopathological evaluation of the endoscopic submucosal dissection (ESD) specimens is essential for clinicians to guide further triage and management. This study aimed to report a novel processing technique for large ESD (≥4 cm) specimens. Methods 92 patients with colorectal neoplasms who had undergone ESD were included. 46 ESD specimens were treated with conventional handling process, while the rest 46 cases were given the optimised method. Macrobiocassettes and L-shaped embedding moulds were applied in the optimised method. We evaluated the efficacy of this improved procedure in terms of the number of paraffin blocks, storage space and time consumption of pathological assessment. Results The average diameter of ESD specimens was 4.5±0.4 cm and 4.7±0.5 cm in the control and test group (p=0.023), respectively. In control group, 398 paraffin blocks of 46 cases were obtained. With the same cases number and larger lesion size, only 276 blocks were achieved in test group (p<0.001). As for the storage space, the total volume of paraffin blocks and slides (4554.0 cm 3 and 1207.5 cm 3 ) of optimised method was significantly reduced compared with the control group (6208.8 cm 3 and 1741.3 cm 3 ) (p=0.001, p<0.001). In addition, the optimised method was superior to the conventional one in shortening time consumption of pathological assessment (164.5 min and 269.0 min, p<0.001). Conclusions The optimised technique not only reduced the workload and storage space, but also facilitated accurate pathological assessment.

Abstract:Techniques were developed for the preparation of frozen sections from undecalcified bone marrow biopsy cores and bone marrow aspirates. Selected indirect antibody and biotin-avidin detection systems, employing immunoperoxidase or immunofluorescent labels, were studied to determine which were best suited for bone marrow frozen section immunohistopathology. Methods employing murine hybridoma monoclonal antibodies illustrate the immunophenotyping of representative lymphoid neoplasms, involving bone marrow. The results of immunohistologic staining were comparable to those of fluorescence-activated cell sorter (FACS) analysis of cell suspensions. The advantages and limitations of immunohistologic techniques are discussed as they relate to immunophenotypic studies previously feasible only with bone marrow cell suspensions. Frozen section immunohistologic techniques serve as useful adjuncts to the conventional evaluation of bone marrow aspirates and biopsies.

Abstract:Cell block preparations of body cavity effusions and washes are more reliable and reproducible than smears, cytospins, and ThinPrep. However, the routine preparation of cell blocks requires a large number of cells within a sample in order to obtain optimal paraffin blocks that contain a sufficient number of embedded cells. The achievement ratio of paraffin-embedded cell block sections from effusion samples has been shown to be 47·7%, which is not optimal for analysis. Therefore, we have developed an improved method called the twice paraffin embedding technique. We modified the current cell block preparation method for effusion samples by adding a ‘cylinder’ to obtain more cell pellets. One hundred and seven effusion and wash fluids samples, which were prospectively obtained from patients, were embedded using this new method. In our study, 82·2% (88/107) of the specimens were successfully embedded into paraffin cell block sections using the twice paraffin embedding technique. The twice paraffin embedding technique is a new, simple, and reliable cell block preparation technique that is suitable for different types of cytology specimens and requires less sample material. We hypothesize that this technique will be widely applicable in the field of cytopathology.
