Hysterectomy was employed for treatment. Various recent conservative and surgical treatment modalities employed for this condition are discussed.

Abstract:PURPOSE To investigate the importance and range of tooth wear in deciduous teeth from the 14th century based on the examination of child skeletons from the Cedynia cemetery in Poland. As mechanical wear was widespread in early human populations, this study recorded symptoms of attrition and abrasion (facets and scoops), relating them to possible causative factors. MATERIALS AND METHODS The study material comprised 84 child skulls (458 teeth) categorised into two age groups: 1. up to 7 years; 2. from 8 to 14 years. The degree of tooth wear was determined on the basis of a modified Smith and Knight's classification. Significant differences between examined features were determined by the chi-square and Fisher Freeman Halton non-parametric tests with the significance level set al p < 0.05. RESULTS Statistically significant differences between age groups in the canines and molars were found (p < 0.05). More advanced wear of deciduous teeth was noticed in the older group. There was no difference in distribution of tooth wear between the maxilla and the mandible. CONCLUSION Examination of children's dentition showed that wear of deciduous teeth rose with the age of individuals, which agrees with the referenced literature data. This study confirms the relationship between dietary patterns and tooth wear and corroborates a role of tooth wear in determining developmental age and growth of the masticatory system.

Abstract:Background: COVID-19 pandemic put an unprecedented burden on the hospital system in the USA, however, limited data is available from southern states.Objective: The current study was conducted to evaluate the health outcomes and inpatient costs of COVID-19 patients in a community hospital in central Alabama.Methods: The demographic and clinical data were obtained for patients admitted from March 2020 to March 2022, with a primary diagnosis of COVID-19.Mortality rate, Length of Stay (LOS) at the hospital, total hospital cost and discharge disposition were analyzed against demographic and clinical characteristics of patients.Results: A cohort of 2135 adult patients with a total of 2222 admissions were selected.Mean age of the cohort was 61.27 years, with 51.33% females.Average LOS was 10.06 days and mean hospital cost was $20226.African Americans had shorter LOS and lower total hospital cost.Patients with older age, obesity, more comorbidities, and End-Stage Renal Disease (ESRD) had significantly longer LOS and total higher hospital cost.White patients, older patients, and patients with comorbidities, as well as patients with obesity or ESRD, had higher mortality rate and greater likelihood to be discharged to rehab or skilled nursing facility, instead of home care.Conclusion: The study reveals how demographics and comorbidities are associated with longer LOS, higher hospital cost and higher mortality, among the patients admitted to a community hospital in central Alabama.The results would facilitate early assessment of disease prognosis upon admission and efficient resource allocation and discharge planning.
Risk Factors Associated with Mortality in COVID-19 Hospitalized Patients: Data from the Middle East

Abstract:This study aimed to assess the risk factors for COVID-19 mortality among hospitalized patients in Jordan. All COVID-19 patients admitted to a tertiary hospital in Jordan from September 20, 2020, to August 8, 2021, were included in this study. Demographics, clinical characteristics, comorbidities, and laboratory results were extracted from the patients' electronic records. Multivariable logistic and machine learning (ML) methods were used to study variable importance. Out of 1,613 COVID-19 patients, 1,004 (62.2%) were discharged from the hospital (survived), while 609 (37.8%) died. Patients who were of elderly age (>65 years) (OR, 2.01; 95% CI, 1.28-3.16), current smokers (OR, 1.61; 95%CI, 1.17-2.23), and had severe or critical illness at admission ((OR, 1.56; 95%CI, 1.05-2.32) (OR, 2.94; 95%CI, 2.02-4.27); respectively), were at higher risk of mortality. Comorbidities including chronic kidney disease (OR, 2.90; 95% CI, 1.90-4.43), deep venous thrombosis (OR, 2.62; 95% CI, 1.08-6.35), malignancy (OR, 2.22; 95% CI, 1.46-3.38), diabetes (OR, 1.31; 95% CI, 1.04-1.65), and heart failure (OR, 1.51; 95% CI, 1.02-2.23) were significantly associated with increased risk of mortality. Laboratory abnormalities associated with mortality included hypernatremia (OR, 11.37; 95% CI, 4.33-29.81), elevated aspartate aminotransferase (OR, 1.81; 95% CI, 1.42-2.31), hypoalbuminemia (OR, 1.75; 95% CI, 1.37-2.25), and low platelets level (OR, 1.43; 95% CI, 1.05-1.95). Several demographic, clinical, and laboratory risk factors for COVID-19 mortality were identified. This study is the first to examine the risk factors associated with mortality using ML methods in the Middle East. This will contribute to a better understanding of the impact of the disease and improve the outcome of the pandemic worldwide.

Abstract:The COVID-19 epidemic has strained healthcare systems globally. Effective patient care and public health policies need understanding COVID-19 mortality variables. This research examines how demographics, comorbidities, cytokine parameters, and hospitalization time affect COVID-19 mortality. The research retrospectively examined 100 Iraqi COVID-19 patients in 2021. Data from Medical City Hospital and Al-Kadhimiya Hospital were analyzed using SPSS and Excel. Study participants were of various ages and genders. The 2021 COVID-19 death rate was 22%, depending on age, gender, and comorbidities. Out of persons aged ≤30 (n=9), 22.2% died from the illness. In the 31-40 age range (n=24), 25.0% died. The 41-50 age group (n=15) had a 40.0% death rate. In the 51-60 age range (n=27), 25.9% died. The 61-70 age group (n=18) had a 5.6% death rate. No one over 70 (n=7) died. The age of patients, but not gender, are significantly associated with mortality. Among the parameters analyzed, ferritin, D-dimer, lymphocytes, and the neutrophil-to-lymphocyte ratio (NLR) levels showed significant differences between the deceased and alive groups (p-value <0.05). Comorbidity analysis demonstrated that cardiovascular diseases, malignancy, kidney diseases, and bacterial infections were significantly associated with higher mortality rates in COVID-19 patients, while liver disease, diabetes mellitus (DM), and hypertension (HT) did not show significant associations. IL6, IL10, and IL12 cytokines levels differed significantly between died and surviving patients. Elevated levels of these interleukins were associated with increased mortality risk, while TNF-a levels showed no significant difference.

Abstract:Background Understanding the factors associated with disease severity and mortality in Coronavirus disease (COVID-19) is imperative to effectively triage patients. We performed a systematic review to determine the demographic, clinical, laboratory and radiological factors associated with severity and mortality in COVID-19. Methods We searched PubMed, Embase and WHO database for English language articles from inception until May 8, 2020. We included Observational studies with direct comparison of clinical characteristics between a) patients who died and those who survived or b) patients with severe disease and those without severe disease. Data extraction and quality assessment were performed by two authors independently. Results Among 15680 articles from the literature search, 109 articles were included in the analysis. The risk of mortality was higher in patients with increasing age, male gender (RR 1.45, 95%CI 1.23–1.71), dyspnea (RR 2.55, 95%CI 1.88–2.46), diabetes (RR 1.59, 95%CI 1.41–1.78), hypertension (RR 1.90, 95%CI 1.69–2.15). Congestive heart failure (OR 4.76, 95%CI 1.34–16.97), hilar lymphadenopathy (OR 8.34, 95%CI 2.57–27.08), bilateral lung involvement (OR 4.86, 95%CI 3.19–7.39) and reticular pattern (OR 5.54, 95%CI 1.24–24.67) were associated with severe disease. Clinically relevant cut-offs for leukocytosis(>10.0 x10 9 /L), lymphopenia(< 1.1 x10 9 /L), elevated C-reactive protein(>100mg/L), LDH(>250U/L) and D-dimer(>1mg/L) had higher odds of severe disease and greater risk of mortality. Conclusion Knowledge of the factors associated of disease severity and mortality identified in our study may assist in clinical decision-making and critical-care resource allocation for patients with COVID-19.

Abstract:Background: SARS-CoV-2 has been a causative agent of severe acute respiratory syndrome since last 2019. Early diagnosis of severe cases is crucial to decrease a patient's hospital stay and death risk. severity and prognosis Patients and Methods: This retrospective study included COVID-19 patient underwent chest computed tomography scan and a battery of laboratory tests, including measurements of leukocytes, neutrophils, lymphocytes, lactic dehydrogenase, creatinine level, ferritin, D-dimer, albumin, and C-reactive protein. In addition, the CRP to lymphocyte ratio (CLR), CRP to albumin ratio (CAR), CRP to platelet ratio (CPR) and the ANDC score. Patients' clinical outcomes including length of hospital stays (LOS) and mortality were recorded. Results: Out of 98 patients, 51 patients had passed away. There was a statistically significant difference between survivors and non-survivors regarding age, TLC, ANC, NLR, D-Dimer, and albumin. Moreover, a highly statistically significant difference regarding CRP levels, CAR, CPR, CLR, and ANDC was noted. Serum CRP level > 123 ng/ml, CAR > 36.77, CPR level > 462, and CLR > 84 had sensitivity; (64.71 %, 66.6 %, 72.5 %, and 76.4 %, respectively) and specificity; (85.1 %, 78.7 %, 72.3 %, and 72.3 % respectively) in mortality prediction. Meanwhile, the ANDC score was the most sensitive indicator (88.2 %) for mortality outcome. Multivariable regression analysis revealed that aging, CPR, and ANDC level were independently associated with mortality with H.R. [1.025 (1.002–1.050); 2.338 (1.189–4.599) and 2.896 (1.191–7.044)] Conclusion: The value of the ANDC score and CRP-derived inflammatory indicators correlate with the likelihood of mortality, so the efficacy of these metrics might assist in urgent early dialogues about treatment escalation.

Abstract:Background. It is well known that laboratory markers could help in identifying risk factors of severe illness and predicting outcomes of diseases. Here, we performed a retrospective modeling study of severity and mortality predictors of hematological and biochemical laboratory parameters in Iranian COVID-19 patients. Methods. Data were obtained retrospectively from medical records of 564 confirmed Iranian COVID-19 cases. According to the disease severity, the patients were categorized into two groups (severe or nonsevere), and based on the outcome of the disease, patients were divided into two groups (recovered or deceased). Demographic and laboratory data were compared between groups, and statistical analyses were performed to define predictors of disease severity and mortality in the patients. Results. The study identified a panel of hematological and biochemical markers associated with the severe outcome of COVID-19 and constructed different predictive models for severity and mortality. The disease severity and mortality rate were significantly higher in elderly inpatients, whereas gender was not a determining factor of the clinical outcome. Age-adjusted white blood cells (WBC), platelet cells (PLT), neutrophil-to-lymphocyte ratio (NLR), red blood cells (RBC), hemoglobin (HGB), hematocrit (HCT), erythrocyte sedimentation rate (ESR), mean corpuscular hemoglobin (MCHC), blood urea nitrogen (BUN), and creatinine (Cr) also showed high accuracy in predicting severe cases at the time of hospitalization, and logistic regression analysis suggested grouped hematological parameters (age, WBC, NLR, PLT, HGB, and international normalized ratio (INR)) and biochemical markers (age, BUN, and lactate dehydrogenase (LDH)) as the best models of combined laboratory predictors for severity and mortality. Conclusion. The findings suggest that a panel of several routine laboratory parameters recorded on admission could be helpful for clinicians to predict and evaluate the risk of disease severity and mortality in COVID-19 patients.

Abstract:Severe acute respiratory syndrome (SARS-CoV-2) that causes novel coronavirus pneumonia (COVID-19) interacts with the receptor (angiotensin-converting enzyme) on the lung epithelial cells and leads to rapid deterioration of human health if no care is provided, hence risk identification of the case severity is important. The objective of the present study is to analyze the relationship between blood parameters and the severity, survival, and mortality of COVID-19 patients based on the hematological and biochemical abnormalities associated with Iraqi COVID-19 patients. A prospective multicenter observational study has been done on a total of 103 patients admitted to different hospitals between November 1, 2021, and April 1, 2022. Among these 30 (29.1%) patients died, 81% died within two weeks of admission to the hospital, and 73 patients (71.9%) survived. An independent sample t-test revealed a significant difference in the tests including AST, ALT, D-dimer, urea, creatinine, and chloride ion between the dead and survived patients, while there was a non-significant difference when these parameters were compared (age, BMI, CPR, PT, INR, APTT, WBC, LYM, GRA, HGB, RBC, ESR, Creatinine, Na and K ions). Crosstab’s study showed that most death (80%) were at ages >60 years, 53.3% of the deaths were having normal BMI and only 10% were obese. Three models for logistic regression were performed to test if the available data set is more effective than the null hypothesis, all three tests yield a similar conclusion that the available data can be used to check the risk factors in COVID-19 patients.

Abstract:Background and objectives There are multiple factors and comorbid conditions that can impact the outcomes of COVID-19. This study aimed to assess how patients with certain comorbidities and risk factors were affected by COVID-19. Methodology This retrospective study involved 578 inpatients who presented to the emergency room (ER) due to COVID-19 infection, diagnosed with COVID-19 between 2020 and 2021. This research takes note of COVID-19 severity, particularly in individuals with comorbidities such as chronic obstructive pulmonary disease (COPD), diabetes mellitus (DM), chronic kidney disease (CKD), coronary artery disease (CAD), and hypertension. Results A two-sample t-test found that age was a significant factor affecting hospital length of stay (LOS) and mortality. An ANOVA analysis of race, DM, and CAD showed a significant effect on LOS (p-values = 0.005, 0.01, and 0.01, respectively) but not on mortality and intubation. White patients had an increased LOS compared to other ethnicities. CKD and hypertension significantly affect mortality and LOS. However, COPD did significantly influence all three variables: mortality, intubation, and LOS (p-values = 0.005, 0.013, and 0.01, respectively). A multiple ANOVA test showed that COPD, hypertension, and CKD had a significant effect on mortality, intubation, and LOS (p-values = 0.014, 0.004, and 0.01, respectively). DM showed weaker significance on mortality, intubation, and LOS (p-value = 0.108). Conclusions Patients with all three comorbid conditions (COPD, hypertension, and CKD) had increased length of stay, intubation, and mortality; thus, appropriate measures including close observation and early intervention may be necessary to prevent mortality in these patients.
Patient Characteristics and Clinical Outcomes During the 2020-2021 COVID-19 Wave: An Observational Study at a Tertiary Hospital in Saudi Arabia

Abstract:Background and objective The continued prevalence and threat of coronavirus disease 2019 (COVID-19) have been reported, and evidence suggests that several people still get infected with the virus. Gaining a thorough understanding of the patient demographic factors and laboratory findings could contribute to assessing the severity, mortality, and progression of COVID-19. In light of this, the current study aimed to evaluate the demographic characteristics, laboratory findings, and outcomes of confirmed COVID-19 patients at a tertiary hospital in the Kingdom of Saudi Arabia (KSA). Methodology We collected data spanning the period 2020-2021 from the electronic health records of Al-Noor Specialized Hospital, Ma including demographics (age, gender, and nationality), severity (i.e., ICU admission), length of hospital stay, mortality, and laboratory parameters. Results We observed an overall mortality rate of 10.2% (338 of 3,307 patients). The mortality rate was significantly higher in males (n=210; 62.1%) and patients aged more than 70 years (n=91; 26.9%). Patients with blood group O comprised 131 (29%) of the 338 non-survivors, followed by those with A (n=85; 25.1%) and B groups (n=79; 23.4%). The mortality rate among ICU patients was 63.3% (n=214). Furthermore, the following laboratory findings showed abnormal mean values in terms of severity and mortality in COVID-19 patients: hemoglobin (HB) concentration, white blood cell (WBC) count, lymphocyte count (LC), C-reactive protein (CRP), creatinine (CREA), and uric acid (UA) levels. Conclusions Old age, male gender, and certain laboratory findings have a critical role in the severity and mortality risk in COVID-19 patients. There was no significant association between blood type and the severity and mortality of COVID-19. Continuous monitoring based on these findings may be essential to managing COVID-19 patients.
Mortality Risk Factors among Hospitalized COVID-19 Patients in a Major Referral Center in Iran

Abstract:The Coronavirus Disease 2019 (COVID-19) pandemic has killed many people worldwide since December 2019, and Iran has been among the most affected countries. In this retrospective study, we aimed to determine the prognostic factors associated with mortality in COVID-19 patients by analyzing 396 survived and 63 non-survived patients in Shahid Modarres Hospital, Tehran, Iran, from January 30th until April 5th, 2020. As the results, the BMI > 35 (p = 0.0003), lung cancer (p = 0.007), chronic kidney disease (p = 0.002), Immunocompromised condition (p = 0.003), and diabetes (p = 0.018) were more frequently observed in the expired group. The history of statins use was more common in the discharged group (p = 0.002), while there was no significant difference in the drug history of angiotensin-converting enzyme inhibitors, angiotensin II receptor blockers, nonsteroidal anti-inflammatory drugs, aspirin, and/or steroids, and in the past-year influenza vaccination. Multivariable regression demonstrated rising odds of in-hospital death related with age (odds ratio (OR) = 1.055, p = 0.002), levels of C-reactive protein (CRP) (OR = 2.915, p < 0.001), creatinine (OR = 1.740, p = 0.023), lymphocyte count (OR = 0.999, p = 0.008), and magnesium level (OR = 0.032, p < 0.001) on admission. In conclusion, the patients with older age and higher BMI with lymphopenia, hypomagnesemia, elevated CRP and/or raised creatinine on admission are at higher risk of mortality due to the COVID-19 infection, which requires the physicians to use timely and strong therapeutic measures for such patients.
SIMulation of Medication Error induced by Clinical Trial drug labeling: the SIMME-CT study.

Abstract:OBJECTIVE To assess the impact of investigational drug labels on the risk of medication error in drug dispensing. DESIGN A simulation-based learning program focusing on investigational drug dispensing was conducted. SETTING The study was undertaken in an Investigational Drugs Dispensing Unit of a University Hospital of Lyon, France. PARTICIPANTS Sixty-three pharmacy workers (pharmacists, residents, technicians or students) were enrolled. INTERVENTION Ten risk factors were selected concerning label information or the risk of confusion with another clinical trial. Each risk factor was scored independently out of 5: the higher the score, the greater the risk of error. From 400 labels analyzed, two groups were selected for the dispensing simulation: 27 labels with high risk (score ≥3) and 27 with low risk (score ≤2). Each question in the learning program was displayed as a simulated clinical trial prescription. MAIN OUTCOME MEASURE Medication error was defined as at least one erroneous answer (i.e. error in drug dispensing). For each question, response times were collected. RESULTS High-risk investigational drug labels correlated with medication error and slower response time. Error rates were significantly 5.5-fold higher for high-risk series. Error frequency was not significantly affected by occupational category or experience in clinical trials. CONCLUSIONS SIMME-CT is the first simulation-based learning tool to focus on investigational drug labels as a risk factor for medication error. SIMME-CT was also used as a training tool for staff involved in clinical research, to develop medication error risk awareness and to validate competence in continuing medical education.

Abstract:Background. Tissue damage caused by COVID-19 could be detected by several clinical indicators including hematological, immunological, biochemical, and inflammatory markers. This study was to detect these clinical parameters to reveal the correlation between the factors and their roles in the development of COVID-19, to explore the hazard factors in severe cases. Materials and Methods. A total of 200 participants of both sexes were included in the study, with an age range of (25–72) years, categorized into three main groups: 50 healthy individuals, 62 mild infected patients, and 88 severe infected patients with pneumonia. Different hematological and clinical parameters were included in the analysis (Basrah city, Iraq). Serum levels of interleukin-6 (IL-6), ferritin, and high-sensitivity C-reactive protein (hs-CRP) were assessed for all participants using an enzyme-linked immunosorbent assay (ELISA). The liver, renal, and cardiac functions were assessed by clinical chemistry testing. Results. COVID-19 patients had leukocytosis, with an increased number of neutrophils and a decreased lymphocyte count, according to our findings. In regard to inflammatory parameters, both ESR and hs-CRP showed significant differences between the two groups, whereas IL-6 was significantly higher in the total severe group compared to the other two groups. Biochemical results revealed that each lactate dehydrogenase (LDH), ferritin, alanine aminotransferase (ALT), and aspartate aminotransferase (AST) had significant changes in the total severe group. Among pneumonic with an O2 requirement and pneumonic without an O2 requirement, there were significant differences in immunological and inflammatory markers ( p > 0.05 ). The neutrophils-lymphocytes ratio (NLR) was highly elevated in severe who required O2. Moreover, IL-6, lymphocytes, and neutrophils were possible risk factors for COVID-19 infection, with the strongest influence of IL-6 with a high odds ratio (OR: 24.138, 95% CI: 8.437–30.65, p < 0.01 ). Furthermore, there were significant correlations among the indicators. Conclusion. Each of IL-6, lymphocytes, and neutrophils might represent major factors in the severity of COVID-19 and IL-6 plays the main role in inducing the inflammatory and pathophysiology process that is known as the cytokine storm.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Coronavirus disease‐2019 (COVID‐19) has a deleterious effect on several systems, including the cardiovascular system. We aim to systematically explore the association of COVID‐19 severity and mortality rate with the history of cardiovascular diseases and/or other comorbidities and cardiac injury laboratory markers.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>The standardized mean difference (SMD) or odds ratio (OR) and 95% confidence intervals (CI) were applied to estimate pooled results from the 56 studies. The prognostic performance of cardiac markers for predicting adverse outcomes and to select the best cutoff threshold was estimated by ROC curve analysis. Decision tree analysis by combining cardiac markers with demographic and clinical features was applied to predict mortality and severity in COVID‐19 patients.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>A meta‐analysis of 17,794 patients showed patients with high cardiac troponin I (OR=5.22, 95%CI=3.73‐7.31, <i>p </i><0.001) and AST levels (OR=3.64, 95%CI=2.84‐4.66, <i>p </i><0.001) were more likely to develop adverse outcomes. High troponin I >13.75 ng/L combined with either advanced age >60 years or elevated AST level >27.72 U/L was the best model to predict poor outcomes. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>COVID‐19 severity and mortality are complicated by myocardial injury. Assessment of cardiac injury biomarkers may improve the identification of those patients at the highest risk and potentially lead to improved therapeutic approaches.</p><p>This article is protected by copyright. Abstract:Background: The COVID-19 pandemic has had a significant impact globally, and understanding the relationship between inflammatory markers and disease progression is crucial for effective management. This retrospective study aimed to examine the association between various inflammatory markers, such as C-reactive protein (CRP), the erythrocyte sedimentation rate (ESR), lactate dehydrogenase (LDH), D-dimer, ferritin, and procalcitonin (PCT), and the characteristics of disease progression and outcomes in individuals affected by COVID-19. Methods: This study collected raw data from 470 patients who tested positive for SARS-CoV-2 using RT-PCR. Results: The logistic regression analysis revealed that elevated LDH levels were associated with male gender, ICU admission, low oxygen saturation (O2 < 93%), the need for mechanical ventilation, death, and the presence of lung infiltrates. Higher D-dimer levels were associated with older age, diabetes mellitus, cardiac disease, and low oxygen saturation. Ferritin levels were significantly associated with older age, ICU admission, low oxygen saturation, mechanical ventilation, and lung infiltrates. In contrast, CRP was only significant regarding lung infiltrates and procalcitonin levels were not significantly associated with any of the examined factors. Conclusion: This study highlights the importance of monitoring key inflammatory markers, such as LDH, D-dimer, and ferritin, as they are significantly associated with the severity of COVID-19 illness. These findings can inform clinical decision-making and guide the development of targeted interventions to improve patient outcomes.

Abstract:Genome instability is a hallmark of most human cancers. Although a mutator phenotype is not required for tumorigenesis, it can foster mutations that promote tumor progression. Indeed, several inherited cancer-prone syndromes are due to mutations in DNA repair pathways. However, sporadic tumors are usually proficient in DNA repair, making it unlikely that unrepaired lesions are a major source of genome instability in sporadic cancers. A decade ago, I argued in another CSHL Press publication that a "collapse in telomere function can explain a significant portion of the genetic instability in tumors" (de Lange 1995). Since that time, the structure of mammalian telomeres has been analyzed, the consequences of telomere dysfunction have been determined, a mouse model for cancer-relevant aspects of telomere biology has been developed, and the nature and magnitude of cancer genome rearrangements have been revealed. In light of these developments, this is an opportune time to revisit the conjecture that telomere dysfunction contributes to genome instability in human cancer.
Prognostic Factors for Mortality in Adults Hospitalized With COVID-19 Infection in the Americas

Abstract:Many reports on COVID-19 have highlighted an association between patient characteristics upon admission for COVID-19 and how they affect variable progressions of the disease severity and, ultimately mortality. In this cohort, we analyzed data from patients in the Americas who tested positive for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the virus that causes COVID-19, and were hospitalized. We retrieved data from identified studies selected from full-text screening to identify relevant patient demographics: age, gender, race, duration of hospital stay, and comorbidities. Data were assessed for consistency, missing values, presence of outliers, and implausible values were documented. Our analysis was performed by calculating the case fatality rates for data reported by the original authors within the studies. The primary outcome was mortality. Participants in the review were COVID-19 inpatients diagnosed through reverse-transcriptase polymerase chain reaction tests (RT-PCR) and above the age of 45 years in the North and South American continents. The study included 12,895 patients who were hospitalized with the COVID-19 infection, and an overall 14.4% in-hospital case-fatality rate was demonstrated. The prognostic factor with the most significant association for increased risk of in-hospital mortality in the elderly was having a high viral load of the COVID-19 virus with a case-fatality rate of 41.4%. In addition, being of Caucasian ethnicity and having the comorbidity of chronic obstructive pulmonary disease (COPD) were also associated with higher rates of mortality among elderly hospitalized patients with case-fatality rates of 40.8% and 35.7% respectively. Prognostic factors for COVID-19 mortality were determined by having a high viral load of the infection, being of Caucasian ethnicity, and having underlying pulmonary comorbidities such as COPD. In the future research should be undertaken to determine the underlying mechanisms behind these effects.

Abstract:Background: The global pandemic, known as the coronavirus disease 2019 (COVID-19) and caused by the severe acute respiratory syndrome, coronavirus 2 (SARS-CoV-2), poses a significant threat, particularly to individuals with comorbidities such as hypertension, chronic obstructive pulmonary disease (COPD), diabetes, HIV, cardiovascular disease (CVD), and cancer. Methods: This descriptive retrospective study investigates the impact of comorbidities on COVID-19-positive patients. The study includes individuals that were tested positive for SARS-CoV-2 via polymerase chain reaction at the Security Forces Hospital, Makkah, KSA, between February, 2022, and June, 2022. A total of 208 patients (107 males, 101 females) were examined, and the laboratory results revealed normal parameters. Results: An analysis indicates that 86.5% of the patients were discharged, 2.9% remained hospitalized, and 10.6% succumbed to the disease, indicating a 10.6% mortality rate among comorbid COVID-19-positive patients. Notably, the study identifies specific comorbidities (chronic kidney disease, diabetes mellitus, hypertension) and changes in laboratory parameters (red blood cells, hemoglobin, C-reactive protein, white blood cells, ferritin, D-dimer, ALT, troponin, LDH, neutrophils) associated with ICU admission during hospitalization. Conclusions: This study underscores the critical impact of comorbidities, such as chronic kidney disease, diabetes, and hypertension, on the clinical outcomes of COVID-19-positive patients. The identification of specific laboratory parameters linked with ICU admission provides valuable insights for risk stratification and tailored management strategies.
