Correlations Between Acute Severe Radiation Pneumonitis Incidence and Pre-Treatment Pulmonary Function Parameters: Analysis among NSCLC Patients with Moderate Pulmonary Dysfunction Receiving Definitive Concurrent Chemo-Radiotherapy

Abstract:Radiation pneumonitis (RP) is a common complication of radiation therapy, and severe acute radiation pneumonitis (SARP) which can lead to a decline in quality of life and, in severe cases, death. Pre-treatment pulmonary function (PF) might have an effect on SARP incidence among lung cancer patients receiving radiotherapy. So far, there was no study reporting the incidence of the SARP among lung cancer patients with moderate pulmonary dysfunction who had received definitive concurrent chemo-radiotherapy (CCRT) and its correlations with pre-treatment pulmonary function parameters. We retrospectively studied patient with pathologically diagnosed non-small cell lung cancer (NSCLC) in our hospital from 2014 to 2018 who had completed definitive CCRT (range of irradiation dose: 60-66 Gy). A total of 632 patients was identified, and 122 patients were confirmed with moderate pulmonary dysfunction (FEV1% 60%-69%). ASRP was defined as the grade 3 or above RP within 3 months since the treatment completed. All parameters from pulmonary function test and dose-volume histogram were put into analysis, and statistical methods were based on statistical analysis software. Totally, 31 patients were recorded with ASRP in the studied population (25.4%, 31/122). None of the DVH parameters was found significantly associated with the ASRP incidence (all p>0.05). Univariate logistic regression analysis of PF parameters indicated that the DLCO ratio (DLCO%) (OR: 0.956, 95% CI 0.925-0.988, p=0.007), FEV1/FVC (OR: 1.043, 95% CI 1.006-1.082, p=0.022), PEF% (OR: 1.030, 95% CI 1.004-1.057, p=0.024), MEF75 (OR: 1.027, 95% CI 1.005-1.049, p=0.016) and MEF50 (OR: 1.027, 95% CI 1.001-1.055, p=0.045) were correlated significantly with the incidence of ASRP in this population, respectively. Multivariate analysis indicated that the DLCO% was the only predictor for ASRP (OR: 0.949, 95% CI 0.915-0.985, p=0.005). ROC curves revealed the AUC of the DLCO% was 0.656 (95% CI 0.556-0.756, p=0.010), with its sensitivity of 0.593 and specificity of 0.710. Among patients with the lower DLCO ratio, the relative risk (RR) of the ASRP incidence was 1.333 (95% CI 1.075-1.654, p=0.012), comparing with those patients whose DLCO% were on the higher level. The DLCO ratio might be a possible predictor for ASRP incidence in NSCLC patients with moderate pulmonary dysfunction received definitive CCRT. Prospective studies are warranted to verify these findings in future.
