Abstract:IntroductionAlzheimer’s disease (AD) is a progressive and irreversible brain degenerative disorder early. Among all diagnostic strategies, hippocampal atrophy is considered a promising diagnostic method. In order to proactively detect patients with early Alzheimer’s disease, we built an Alzheimer’s segmentation and classification (AL-SCF) pipeline based on machine learning.MethodsIn our study, we collected coronal T1 weighted images that include 187 patients with AD and 230 normal controls (NCs). Our pipeline began with the segmentation of the hippocampus by using a modified U2-net. Subsequently, we extracted 851 radiomics features and selected 37 features most relevant to AD by the Hierarchical clustering method and Least Absolute Shrinkage and Selection Operator (LASSO) algorithm. At last, four classifiers were implemented to distinguish AD from NCs, and the performance of the models was evaluated by accuracy, specificity, sensitivity, and area under the curve.ResultsOur proposed pipeline showed excellent discriminative performance of classification with AD vs NC in the training set (AUC=0.97, 95% CI: (0.96-0.98)). The model was also verified in the validation set with Dice=0.93 for segmentation and accuracy=0.95 for classification.DiscussionThe AL-SCF pipeline can automate the process from segmentation to classification, which may assist doctors with AD diagnosis and develop individualized medical plans for AD in clinical practice.

Abstract:Background Alzheimer's disease (AD) and vascular dementia (VaD) share overlapping pathophysiological characteristics, yet comparative genetic studies are rare. Understanding these overlaps may aid in identifying common diagnostic markers and therapeutic targets. Objective This study identifies shared diagnostic genes and mechanisms linking AD and VaD. Methods Datasets GSE5281 and GSE122063 from the GEO database were used to identify differentially expressed genes (DEGs). Intersection DEGs were analyzed using KEGG and GO enrichment to explore signaling pathways. A PPI network was constructed, and LASSO and SVM-RFE were applied to identify core genes. CIBERSORT assessed immune cell composition and their relationship with core genes. Diagnostic efficacy was evaluated using ROC curves, nomogram, and Decision Curve Analysis (DCA). Core genes were used to identify characteristic genes in various brain regions of AD patients. Results The analysis identified 9021 DEGs for AD and 373 DEGs for VaD, with 74 co-expressed genes and 8 core genes. ROC curves, nomogram, and DCA indicated high diagnostic accuracy. Core gene analysis revealed differential expression of characteristic genes in various brain regions of AD patients. Conclusions This research identified 74 co-expressed genes and 8 pivotal diagnostic genes. These genes likely play roles in signal transduction, neuroinflammation, and autophagy in both AD and VaD. The findings offer potential targets for future research and clinical interventions. Further research should use larger, more diverse datasets and incorporate custom NGS panels to identify novel genetic variants, enhancing precise diagnostic and therapeutic strategies.
SCRN: Single-cell Gene Regulatory Network Identification in Alzheimer's Disease

Abstract:Alzheimer's disease (AD) is the most common neurodegenerative disease, and it consumes considerable medical resources with increasing number of patients every year. Mounting evidence show that the regulatory disruptions altering the intrinsic activity of genes in brain cells contribute to AD pathogenesis. To gain insights into the underlying gene regulation in AD, we proposed a graph learning method, Single-Cell based Regulatory Network (SCRN), to identify the regulatory mechanisms based on single-cell data. SCRN implements the γ-decaying heuristic link prediction based on graph neural networks and can identify reliable gene regulatory networks using locally closed subgraphs. In this work, we first performed UMAP dimension reduction analysis on single-cell RNA sequencing (scRNA-seq) data of AD and normal samples. Then we used SCRN to construct the gene regulatory network based on three well-recognized AD genes (APOE, CX3CR1, and P2RY12). Enrichment analysis of the regulatory network revealed significant pathways including NGF signaling, ERBB2 signaling, and hemostasis. These findings demonstrate the feasibility of using SCRN to uncover potential biomarkers and therapeutic targets related to AD.

Abstract:Alzheimer's disease (AD) is a pervasive neurodegenerative disorder that leads to memory and behavior impairment severe enough to interfere with daily life activities. Understanding this disease pathogenesis can drive the development of new targets and strategies to prevent and treat AD. Recent advances in high-throughput single-cell RNA sequencing technology (scRNA-seq) have enabled the generation of massive amounts of transcriptomic data at the single-cell level provided remarkable insights into understanding the molecular pathogenesis of Alzheimer's disease. In this study, we introduce ScAtt, an innovative Attention-based architecture, devised specifically for the concurrent identification of cell-type specific AD-related genes and their associated gene regulatory network. ScAtt incorporates a flexible model capable of capturing nonlinear effects, leading to the detection of AD-associated genes that might be overlooked by traditional differentially expressed gene (DEG) analyses. Moreover, ScAtt effectively infers a gene regulatory network depicting the combined influences of genes on the targeted disease, as opposed to examining correlations among genes in conventional gene co-expression networks. In an application to 95,186 single-nucleus transcriptomes from 17 hippocampus samples, ScAtt shows substantially better performance in modeling quantitative changes in expression levels between AD and healthy controls. Consequently, ScAtt performs better than existing methods in the identification of AD-related genes, with more unique discoveries and less overlap between cell types. Functional enrichments of the corresponding gene modules detected from gene regulatory network show significant enrichment of biologically meaningful AD-related pathways across different cell types.

Abstract:The pathogenesis of Alzheimer disease (AD) involves complex gene regulatory changes across different cell types. To help decipher this complexity, we introduce single-cell Bayesian biclustering (scBC), a framework for identifying cell-specific gene network biomarkers in scRNA and snRNA-seq data. Through biclustering, scBC enables the analysis of perturbations in functional gene modules at the single-cell level. Applying the scBC framework to AD snRNA-seq data reveals the perturbations within gene modules across distinct cell groups and sheds light on gene-cell correlations during AD progression. Notably, our method helps to overcome common challenges in single-cell data analysis, including batch effects and dropout events. Incorporating prior knowledge further enables the framework to yield more biologically interpretable results. Comparative analyses on simulated and real-world datasets demonstrate the precision and robustness of our approach compared to other state-of-the-art biclustering methods. scBC holds potential for unraveling the mechanisms underlying polygenic diseases characterized by intricate gene coexpression patterns.
Integration of Single-Cell and Spatial Transcriptomic Data Reveals Spatial Architecture and Potential Biomarkers in Alzheimer's Disease

Abstract:Alzheimer's disease (AD) is a complex neurodegenerative disorder characterized by the gradual loss of neurons and the accumulation of amyloid plaques and neurofibrillary tangles. Despite advancements in the understanding of AD's pathophysiology, the cellular organization and interactions in the prefrontal cortex (PFC) remain elusive. Eight single-cell RNA sequencing (scRNA-seq) datasets from both normal controls and individuals with AD were harmonized. Stringent preprocessing protocols were implemented to uphold dataset integrity. Unsupervised clustering and annotation revealed 22 distinct cell clusters corresponding to 19 unique cell types. The spatial architecture of the PFC region was constructed using the CARD tool. Further analyses encompassed trajectory examination of Oligodendrocyte subtypes, evaluation of regulon activity scores, and spot clustering within white matter regions (WM). Differential expression analysis and functional enrichment assays unveiled molecular signatures linked to AD progression and were validated using microarray data sourced from neurodegenerative disorder patients. Our investigation employs scRNA-seq and spatial transcriptomics to uncover the cellular atlas and spatial architecture of the human PFC in AD. Moreover, our results indicate that Oligodendrocytes are more prevalent in AD patients, showcasing diverse subtypes and spatial organization within WM regions. Each subtype appears to be associated with distinct biological processes and transcriptional regulators, shedding light on their involvement in AD pathology. Notably, the Oligodendrocyte_C6 subtype is linked to neurological damage in AD patients, characterized by heightened expression of genes involved in cell–cell connections, cell membrane stability, and myelination. Additionally, 12 target genes regulated by NFIA were identified, which are upregulated in AD patients and associated with disease progression. Elevated PLXDC2 expression in peripheral blood was also identified, suggesting its potential as a non-invasive biomarker for early AD detection. Our study provides novel insights into the role of Oligodendrocytes in AD and highlights the potential of PLXDC2 as a blood biomarker for non-invasive diagnosis and monitoring of AD patients.

The cases reported here described rupture following evacuation curettage which had caused an unrecognized uterine perforation. While such perforations often have no serious consequences, this explaining the usual abstention from treatment, it is nevertheless important that acute complications (intestinal or vascular trauma) or long term (uterine rupture of pregnant uterus) should not pass unrecognized.
Combining machine learning algorithms and single-cell data to study the pathogenesis of Alzheimer’s disease

Abstract:Extracting valuable insights from high-throughput biological data of Alzheimer’s disease to enhance understanding of its pathogenesis is becoming increasingly important. We engaged in a comprehensive collection and assessment of Alzheimer’s microarray datasets GSE5281 and GSE122063 and single-cell data from GSE157827 from the NCBI GEO database. The datasets were selected based on stringent screening criteria: a P-value of less than 0.05 and an absolute log fold change (|logFC|) greater than 1. Our methodology involved utilizing machine learning algorithms, efficiently identified characteristic genes. This was followed by an in-depth immune cell infiltration analysis of these genes, gene set enrichment analysis (GSEA) to elucidate differential pathways, and exploration of regulatory networks. Subsequently, we applied the Connectivity Map (cMap) approach for drug prediction and undertook single-cell expression analysis. The outcomes revealed that the top four characteristic genes, selected based on their accuracy, exhibited a profound correlation with the Alzheimer’s disease (AD) group in terms of immune infiltration levels and pathways. These genes also showed significant associations with multiple AD-related genes, enhancing the potential pathogenic mechanisms through regulatory network analysis and single-cell expression profiling. Identified three subpopulations of astrocytes in late-stage of AD Prefrontal cortex dataset. Discovering dysregulation of the expression of the AD disease-related pathway maf/nrf2 in these cell subpopulations Ultimately, we identified a potential therapeutic drug score, offering promising avenues for future Alzheimer’s disease treatment strategies.

Abstract:Alzheimer’s disease (AD) represents one of the most important healthcare challenges of the current century, characterized as an expanding, “silent pandemic”. Recent studies suggest that the peripheral immune system may participate in AD development; however, the molecular components of these cells in AD remain poorly understood. Although single-cell RNA sequencing (scRNA-seq) offers a sufficient exploration of various biological processes at the cellular level, the number of existing works is limited, and no comprehensive machine learning (ML) analysis has yet been conducted to identify effective biomarkers in AD. Herein, we introduced a computational workflow using both deep learning and ML processes examining scRNA-seq data obtained from the peripheral blood of both Alzheimer’s disease patients with an amyloid-positive status and healthy controls with an amyloid-negative status, totaling 36,849 cells. The output of our pipeline contained transcripts ranked by their level of significance, which could serve as reliable genetic signatures of AD pathophysiology. The comprehensive functional analysis of the most dominant genes in terms of biological relevance to AD demonstrates that the proposed methodology has great potential for discovering blood-based fingerprints of the disease. Furthermore, the present approach paves the way for the application of ML techniques to scRNA-seq data from complex disorders, providing new challenges to identify key biological processes from a molecular perspective.

Abstract:The distinguishable subregions that compose the hippocampus are differently involved in functions associated with Alzheimer’s disease (AD). Thus, the identification of hippocampal subregions and genes that classify AD and healthy control (HC) groups with high accuracy is meaningful. In this study, by jointly analyzing the multimodal data, we propose a novel method to construct fusion features and a classification method based on the random forest for identifying the important features. Specifically, we construct the fusion features using the gene sequence and subregions correlation to reduce the diversity in same group. Moreover, samples and features are selected randomly to construct a random forest, and genetic algorithm and clustering evolutionary are used to amplify the difference in initial decision trees and evolve the trees. The features in resulting decision trees that reach the peak classification are the important “subregion gene pairs”. The findings verify that our method outperforms well in classification performance and generalization. Particularly, we identified some significant subregions and genes, such as hippocampus amygdala transition area (HATA), fimbria, parasubiculum and genes included RYR3 and PRKCE. These discoveries provide some new candidate genes for AD and demonstrate the contribution of hippocampal subregions and genes to AD.

Abstract:Abstract Background The pathogenesis of Alzheimer’s disease (AD) varies greatly depending on environmental and heritable factors. Neuropathological scores aid in the understanding of disease severity in postmortem tissue, but fail to provide insights into disease onset and progression. Conversely, mouse models of AD like 5XFAD offer predictable timelines of amyloid accumulation due to specific human AD risk signals, but have limitations due to species differences. Studying AD in Down Syndrome (AD in DS) patients provides an opportunity to study the transcriptomics of AD progression due to overexpression of APP , which results in a more predictable disease time course compared to the more stochastic disease progression in the general population. Method We present a rigorous transcriptomic analysis of AD using single‐nucleus RNA‐seq (snRNA‐seq) and spatial transcriptomics (ST) in cortical samples from donors with early‐stage AD, late‐stage AD, AD in DS, and cognitively normal controls. We performed ST (10X Genomics Visium) in these groups, and additionally performed snRNA‐seq (Parse Biosciences) in tissue from AD in DS donors and controls. Finally, we profiled 5XFAD and wild type mice at four time points (4, 6, 8, and 12 months) using ST to facilitate cross‐species comparisons. We applied state‐of‐the‐art bioinformatic and statistical approaches to integrate these datasets with published AD snRNA‐seq data and to perform systematic analyses of gene expression and gene networks in these groups. Result We found shared and distinct differentially expressed genes (DEGs) between AD and AD in DS in the ST and snRNA‐seq datasets, with greater concordance in neuronal cell types compared to glia. Cell‐cell communication network analysis of integrated snRNA‐seq and ST data revealed vast network remodeling in disease, highlighting an increase in WNT and ANGPTL signaling and a decrease in CD99 signaling among other disrupted pathways. Using geospatial statistical analysis, we identified amyloid plaque proximal DEGs in human and mouse ST data, which were largely species‐specific with little overlap. Conclusion The primary data generated here will be a valuable resource for the AD community, and our study provides new insights into the transcriptomics of AD and amyloid pathology by performing a holistic analysis considering sporadic and genetic forms of AD.

Large Diurnal Fluctuations in Intraocular Pressure Are an Independent Risk Factor in Patients With Glaucoma

Abstract:Purpose: To study the risk associated with diurnal intraocular pressure (IOP) variations in patients with open‐angle glaucoma. Patients and Methods: Sixty‐four patients (105 eyes) from the practices of two glaucoma specialists successfully performed home tonometry with a self‐tonometer five times a day for 5 days. All patients had open‐angle glaucoma and documented IOP below 25 mm Hg over a mean follow‐up period of 5 years. Baseline status and time to progression of visual field loss were identified from the clinical charts. The level and variability of diurnal IOP obtained using home tonometry were characterized. Risk of progression was analyzed using a nonparametric time‐to‐event model, incorporating methods for correlated outcomes. Results: Although mean home IOP and baseline office IOP were similar (16.4 ± 3.6 mm Hg and 17.6 ± 3.2 mm Hg, respectively), the average IOP range over the 5 days of home tonometry was 10.0 ± 2.9 mm Hg. Baseline office IOP had no predictive value (relative hazard, 0.98). The diurnal IOP range and the IOP range over multiple days were significant risk factors for progression, even after adjusting for office IOP, age, race, gender, and visual field damage at baseline (relative hazards [95% confidence intervals], 5.69 [1.86, 17.35] and 5.76 [2.21, 14.98]). Eighty‐eight percent of patients in the upper twenty‐fifth percentile of IOP and 57% of patients in the lower twentyfifth percentile progressed within 8 years. Conclusions: In patients with glaucoma with office IOP in the normal range, large fluctuations in diurnal IOP are a significant risk factor, independent of parameters obtained in the office. Fluctuations in IOP may be important in managing patients with glaucoma. Development of methods to control fluctuations in IOP may be warranted.

Abstract:Alzheimer's disease (AD) is the most widespread diagnosed cause of dementia in the elderly. It is a progressive neurodegenerative disease that causes memory loss as well as other detrimental symptoms that are ultimately fatal. Due to the urgent nature of this disease, and the current lack of success in treatment and prevention, it is vital that different methods and approaches are applied to its study in order to better understand its underlying mechanisms. To this end, we have conducted network-based gene co-expression analysis on data from the Alzheimer's Disease Neuroimaging Initiative (ADNI) database. By processing and filtering gene expression data taken from the blood samples of subjects with varying disease states and constructing networks based on that data to evaluate gene relationships, we have been able to learn about gene expression correlated with the disease, and we have identified several areas of potential research interest.

Abstract:Hyperkeratosis lenticularis perstans also known as Flegel's disease is a keratinisation disorder characterized by small keratotic papules with horny scales. Most cases have been reported in Europe with age of presentation between 35 and 60 years. We report a case of a 25-year-old man, who presented with 1-5 mm multiple asymptomatic hyperkeratotic papules of 15 years duration on both legs and hand along with lichenified plaques with Koebner phenomenon in the axillary folds, anticubital and popliteal fossae. Similar lesions were present in the eyelids also. There was no involvement elsewhere. Similar illness was found in the younger brother aged 13 years with the duration of illness for the past 5 years. Histopathology confirmed the clinical diagnosis. Patient was treated with oral isotretinoin in a daily dose of 20 mg per day to which he responded immediately with clearing of the lesions in two weeks.
SCING: Single Cell INtegrative Gene regulatory network inference elucidates robust, interpretable gene regulatory networks

Abstract:Gene regulatory network (GRN) inference is an integral part of understanding physiology and disease. Single cell/nuclei RNAseq (scRNAseq/snRNAseq) data has been used to elucidate cell-type GRNs; however, the accuracy and speed of current scRNAseq-based GRN approaches are suboptimal. Here, we present Single Cell INtegrative Gene regulatory network inference (SCING), a gradient boosting and mutual information based approach for identifying robust GRNs from scRNAseq, snRNAseq, and spatial transcriptomics data. Performance evaluation using Perturb-seq datasets, held-out data, and the mouse cell atlas combined with the DisGeNET database demonstrates the improved accuracy and biological interpretability of SCING compared to existing methods. We applied SCING to the entire mouse single cell atlas, human Alzheimer's disease (AD), and mouse AD spatial transcriptomics. SCING GRNs reveal unique disease subnetwork modeling capabilities, have intrinsic capacity to correct for batch effects, retrieve disease relevant genes and pathways, and are informative on spatial specificity of disease pathogenesis.

Abstract:Introduction: Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by gradual loss of cognitive function. Understanding the molecular mechanisms is crucial for developing effective therapies. Methods: Data from single-cell RNA sequencing (scRNA-seq) in the GSE181279 dataset and gene chips in the GSE63060 and GSE63061 datasets were collected and analyzed to identify immune cell types and differentially expressed genes. Cell communication, pseudotime trajectory, enrichment analysis, co- expression network, and short time-series expression miner were analyzed to identify disease-specific molecular and cellular mechanisms. Results: We identified eight cell types (B cells, monocytes, natural killer cells, gamma-delta T cells, CD8+ T cells, Tem/Temra cytotoxic T cells, Tem/Trm cytotoxic T cells, and mucosal-associated invariant T cells) using scRNA-seq. AD samples were enriched in monocytes, CD8+ T cells, Tem/Temra cytotoxic T cells, and Tem/Trm cytotoxic T cells, whereas samples from healthy controls were enriched in natural killer and mucosal-associated invariant T cells. Five co-expression modules that were identified through weighted gene correlation network analysis were enriched in immune- inflammatory pathways. Candidate genes with higher area under the receiver operating characteristic curve values were screened, and the expression trend of Ubiquitin-Fold Modifier Conjugating Enzyme 1 (UFC1) gradually decreased from healthy controls to mild cognitive impairment and then to AD. Conclusion: Our study suggests that peripheral immune cells may be a potential therapeutic target for AD. Candidate genes, particularly UFC1, may serve as potential biomarkers for progression of AD.

Abstract:The emergence of technologies that can support high-throughput profiling of single cell transcriptomes offers to revolutionize the study of brain tissue from persons with and without Alzheimer’s disease (AD). Integration of these data with additional complementary multiomics data such as genetics, proteomics and clinical data provides powerful opportunities to link observed cell subpopulations and molecular network features within a broader disease-relevant context. We report here single nucleus RNA sequencing (snRNA-seq) profiles generated from superior frontal gyrus cortical tissue samples from 101 exceptionally well characterized, aged subjects from the Banner Brain and Body Donation Program in combination with whole genome sequences. We report findings that link common AD risk variants with CR1 expression in oligodendrocytes as well as alterations in peripheral hematological lab parameters, with these observations replicated in an independent, prospective cohort study of ageing and dementia. We also observed an AD-associated CD83(+) microglial subtype with unique molecular networks that encompass many known regulators of AD-relevant microglial biology, and which are associated with immunoglobulin IgG4 production in the transverse colon. These findings illustrate the power of multi-tissue molecular profiling to contextualize snRNA-seq brain transcriptomics and reveal novel disease biology. The transcriptomic, genetic, phenotypic, and network data resources described within this study are available for access and utilization by the scientific community.
