Abstract:Purpose: This study aimed at comparing the risk of breakage of lengthened sacroiliac screw and ordinary sacroiliac screw for the treatment of bilateral vertical sacral fractures to provide reference for clinical application. Methods: A finite element model of type C pelvic ring injury (bilateral type Denis II fracture of sacrum) was produced. The bilateral sacral fractures were fixed with lengthened sacroiliac screw and ordinary sacroiliac screw in seven types of models, respectively. The maximal Von Mises stresses and stress distribution of the two kinds of screws in the case of standing on both feet were measured and compared. Results: (1) Whether in lengthened sacroiliac screw or ordinary sacroiliac screw, the maximal Von Mises stress of screw fixation only in S1 segment is the largest, and the maximal Von Mises stress of screw fixation only in S2 segment is minor, and the maximal Von Mises stress of screw fixation in S1 and S2 segments, respectively, is the least. (2) When S1 and S2 were both fixed with ordinary screws, the maximal Von Mises stress of screw in S1 segment is larger than that of S2. When S1 and S2 were both fixed with lengthened screws, the maximal Von Mises stress of screw in S1 segment is similar to that of S2. (3) The maximal Von Mises stresses of bilateral symmetrical screws are similar. (4) When only S1 was fixed, the maximal Von Mises stress of lengthened screw is less than that of ordinary screw. When only S2 was fixed, the maximal Von Mises stress of lengthened screw is larger than that of ordinary screw. When S1 and S2 were both fixed, the maximal Von Mises stress of lengthened screw is slightly less than that of ordinary screw. (5) Whether in lengthened screw or ordinary screw, the stress concentrations all exhibited at the regions of screws corresponding to the sacral fracture regions and the part between sacral bilateral fracture lines. Compared with ordinary screw, the stress distribution in lengthened screw is more homogeneous. Whether in lengthened screw or ordinary screw, the stress distribution of only one sacral segment fixation is more concentrated than that of two sacral segments' fixation. When S1 and S2 were both fixed, the stress distribution of upper screw is more concentrated and that of lower screw is more homogeneous. Conclusion: In a finite elements simulated type C pelvic ring disruption (bilateral type Denis II sacral fracture), the breakage risk of screws fixed in double-segment bilaterally symmetrically is low, and the breakage risk of screws fixed in S2 segment is lower than that of S1 segment. The bilaterally symmetrical screw fixation in double-segment is strongly recommended to reduce the breakage risk of screws. In addition, the breakage risk of lengthened screws is lower than that of ordinary screws except when screws are fixed in only S2 segment, which merits attention.
Mechanical comparison between lengthened and short sacroiliac screws in sacral fracture fixation: a finite element analysis

Abstract:Objective: To compare the stability of lengthened sacroiliac screw and standard sacroiliac screw for the treatment of unilateral vertical sacral fractures; to provide reference for clinical applications. Methods: A finite element model of Tile type C pelvic ring injury (unilateral Denis type II fracture of the sacrum) was produced. The unilateral sacral fractures were fixed with lengthened sacroiliac screw and sacroiliac screw in six different types of models respectively. The translation and angle displacement of the superior surface of the sacrum (in standing position on both feet) were measured and compared. Results: The stability of one lengthened sacroiliac screw fixation in S1 or S2 segment is superior to that of one sacroiliac screw fixation in the same sacral segment. The stability of one lengthened sacroiliac screw fixation in S1 and S2 segments respectively is superior to that of one sacroiliac screw fixation in S1 and S2 segments respectively. The stability of one lengthened sacroiliac screw fixation in S1 and S2 segments respectively is superior to that of one lengthened sacroiliac screw fixation in S1 or S2 segment. The stability of one sacroiliac screw fixation in S1 and S2 segments respectively is markedly superior to that of one sacroiliac screw fixation in S1 or S2 segment. The vertical and rotational stability of lengthened sacroiliac screw fixation and sacroiliac screw fixation in S2 is superior to that of S1. Conclusion: In a finite element model of type C pelvic ring disruption, S1 and S2 lengthened sacroiliac screws should be utilized for the fixation as regularly as possible and the most stable fixation is the combination of the lengthened sacroiliac screws of S1 and S2 segments. Even if lengthened sacroiliac screws cannot be systematically used due to specific conditions, one sacroiliac screw fixation in S1 and S2 segments respectively is recommended. No matter which kind of sacroiliac screw is used, if only one screw can be implanted, the fixation in S2 segment is more recommended than that in S1. Level of evidence: Experimental study Level III.

Abstract:The percutaneous sacroiliac (SI) screw is a common fixation option for posterior ring disruption in pelvic fractures. However, SI screw placement is difficult and can injure adjacent neurovascular structures. The sacral-alar-iliac screw (SAI) is a safe, reliable free-hand sacral pelvic fixation technique. To investigate the biomechanical stability of SAI for SI joint dislocation, finite element analysis was performed in unstable Tile-Type B and C pelvic ring injuries. The displacement in S1 (fixation of a unilateral S1 segment with one SI screw), TS1 (fixation of the S1 segment with a transsacra 1 screw), TS2 (fixation of the S2 segment with a transsacra 2 screw), S1AI, and S2AI exceeded the normal SI joint mobility. Sufficient stability after SI joint dislocation was obtained with (TS1 + TS2), (TS2 + S1), (S1AI + S2AI + rod), (S1AI + S2AI), and (S1 + S2AI + S1 pedicle) fixation. The TS1 + TS2 group had the smallest displacement and lowest peak screw stress, followed by (S1 + S2AI + S1 pedicle) placement. Our findings suggest that SAI screws are a valuable option for SI joint dislocation.

Abstract:Objective: To determine the best method for anterior internal fixation by comparing the biomechanical characteristics of various anterior fixation methods for vertically unstable sacroiliac dislocations.Methods: Eight pelves with Tile C fracture with vertical loading were measured separately. Three‐ and two‐hole anterior double plates were fixed at different angles and compared with sacroiliac screws.Results: For two‐hole anterior double plates, with increasing angle, axial stability increases while lateral stability decreases. For three‐hole plates with an angle of 60°, axial stability is clearly better when placed horizontally, while lateral stability shows no obvious differences.Conclusion: For fixing sacroiliac dislocations, three‐hole anterior double plates placed at about 60° produce excellent stability, which would be greatly increased if screws were added on the iliac side.

Abstract:To compare the stability and biomechanical characteristics of four commonly used triangular osteosynthesis techniques to treat unilateral vertical sacral fractures and provide a clinical application reference. Finite element models of Tile C-type pelvic ring injury (unilateral Denis II sacral fracture) were produced. In four models, sacral fractures were fixed with a combination of unilateral L5, unilateral L4, and L5 iliac lumbar fixation with lengthened or normal sacroiliac screws. The biomechanical properties of the four fixation models were measured and compared under bipedal stance and lumbar rotation. The fixation stability of the model with the lengthened sacroiliac screw was excellent, and the fracture end was stable. The stability of fixation using unilateral L4 and L5 segments was close to that of unilateral L5 segment fixation. Triangular osteosynthesis transverse stabilization devices using lengthened sacroiliac screws can increase the vertical stability of the sacrum after internal fixation and increase the stability of the fracture. When triangular osteosynthesis lumbar fixation segments were selected, simultaneous fixation of L4 and L5 segments versus only L5 segments did not significantly enhance the vertical stability of the sacrum or the stability of the fracture end.

Abstract:Background Femoral neck fractures are a common traumatic injury. The removal of the internal fixation remains controversial, especially in terms of mechanical stability. Moreover, collapsed necrosis of the femoral head continues to occur after fracture healing. We believe that sclerotic cancellous bone (SCB) formation around the screw is associated with femoral head necrosis. We aimed to compare mechanical features before and after implant removal and determine the effect of SCB formation on stress distribution. Methods Cylindrical cancellous bone sections were collected from a relatively normal region and an SCB region of a necrotic femoral head, and their elastic moduli were measured. Four femoral finite element models were developed: a) femoral neck fracture healing with implants, b) fracture healing without implants, c) sclerosis around the screw with implants, and d) sclerosis around the screw without implants. Results The maximum von Mises peak stresses of models a and b were 66.643 MPa and 63.76 MPa, respectively, and were concentrated in the upper lateral femur. The main stress was scattered at the lowest screw tail, femoral calcar region, and lateral femur shaft. Moreover, coronal plane strain throughout the screw paths near the femoral head in models a and b was mostly in the range of 1000–3000 με. The maximum stress concentrations in models c and d were located at the lower femoral head and reached 91.199 MPa and 78.019 MPa, respectively. Conclusions The stresses in the sclerotic model around the cannulated screws are more concentrated on the femoral head than in the healing model without sclerotic bone. The overall stresses in the healing femoral neck fracture model were essentially unchanged before and after removal of the internal fixation.

Abstract:This paper presented a special computer aided procedure to design a new sacroliliac anatomic bar-plate internal fixation system, and evaluated its biomechanical properties on an accurate patient-specific finite element model of entire pelvis, compared with two conventional internal fixation methods. Based on virtual anatomical measure of 30 digital pelvic models reconstructed from CT, an anatomic plate was designed according to the complicated structure of the outer table of the posterior ilium, and was integrated into the complete fixation system. Then, an ad hoc semi-automatic mesh generator was employed to construct a patient-specific finite element model of whole pelvis, including elaborate sacroiliac joints, important pelvic ligaments, and interpubic disc, as well as position-dependent cortical thickness and trabecular bone elastic modulus. Following, one side of sacroiliac joint related ligaments were deleted to simulate a complete unilateral sacroiliac joint disruption. Then the new anatomic fixation system was integrated to fix the fracture, and two comparing models including iliosacral screw fixation and front reconstruction plate fixation were also generated. Finally, all models were simulated under same loading conditions. The results demonstrated that the mechanical stability of the new anatomic fixation system was superior, with obviously improved stress distribution and little displacement, which implied an effective internal fixation method for potential clinical application.
Applied anatomic study of the parameters of screw fixation for the sacroiliac joint

Abstract:Objective: To provide anatomic basis for iliosacral screw fixation at the level of the pedicle of the first sacral vertebra (S_1), the present study was carried out. Methods: 22 pelvis specimens of native adults were studied, a total of 44 sacroiliac joint anthropometric measurement were performed on 22 cadaveric pelvises to determine the shape of S_1 pedicle transverse section, the optimal entry point for iliosacral screw on the outer table of the posterolateral ilium, the direction of screw, and the distance from entry point to contrary anterior cortex of S_1 vertebral body. After confirming the optimal entry point, verify the different methods for inserting iliosacral screw on 5 fresh pelvises specimens. Results: The shape of S_2 pedicle transverse section was almost ellipse. The broad and height of the pedicle of S_1 were 27.7±1.9 mm and 20.2±2.3 mm, respectively. The optimal entry point on the posterolateral ilium for screw fixation was found to exist 24.6±2.9 mm anterior to posterior inferior iliac spine and 41.8±3.4 mm superior to the greater sciatic notch. The distance from entry point to contrary anterior cortex of S_1 vertebral body was 86.5±3.8 mm.The direction of screw was perpendicular to the outer table of the posterolateral ilium, then 7.7°±3.8° in posterior oblique and 7.1°±2.3° in cauda oblique. The result of iliosacral screw fixation according to the entry point measured in this study was that no screw penetrate pedicle of S_1 or sacral vertebral body. The result of iliosacral screw fixation according to posterior gluteal line was that two of seven lag screws penetrate pedicle of S_1. The result of iliosacral screw fixation according to posterior inferior iliac spine was that four of ten screws penetrate pedicle of S_1 or sacral vertebral body. Conclusion: The posterior inferior iliac spine and the sciatic notch constant exist, they are easily exposed and identified in operation. The entry point was located 25mm anterior to posterior inferior iliac spine and 40mm superior to the greater sciatic notch. The length of iliosacral screw was 75 mm. The direction of screw was perpendicular to the outer table of the posterolateral ilium, then 5°～10° in posterior oblique and 5°～10° in cauda oblique.

Abstract:Objective To observe the biomechanical effect of sacroiliac screw fixation with dif-ferent needling points in treatment of extorsion pelvic instabihty so as to provide experimental basis for clinical application. Methods Ten fresh adult pelvis specimens were fixated by sacroiliac screw with needling points B (2.5 cm anterior to posteroinferior iliac spine and 40. cm posterior to greater sciatic notch), A (0.1 cm anterior to the same horizontal plane of point B) and C ( 1.0 cm posterior to the same horinzontal plane of point B). The shifting of the pubic symphysis diastasis and the sacroiliac joint crack under the same extorsion force was measured in integrity ligament group, point A group, point B group and point C group to compare the stability of different needling points. Results In confronting extor-sion force, integrity ligament group and point C group were obviously inferior to point A group and point B group (P<0.05 ) but there was no statistical difference between point B group and point A group (P > 0.05). As for the stability of the sacroiliac joint, there was statistical difference among all groups ( P < 0.05 ), while there was no statistical difference between integrity ligament group and point B group ( P > 0.05). Conclusion Point B is the most stable, safe and effective needling point.
The comparisons between the unilateral and bilateral pedicle screws fixation based on the finite element analysis

Abstract:Objective: To construct an intact L3-L5 finite element model, then to evaluate the stress characteristics following unilateral and bilateral pedicle screw fixation based on the intact model. Methods: A intact L3-L5 three-dimensional finite element model was developed by using the computed tomography scans of a healthy male. Based on the intact one, two new models were created by simulating the unilateral pedicle screws (UPS) fixation and the bilateral pedicle screws (BPS) fixation. Meanwhile, 500 N preload was loaded on the upper surface of L3, then 8 N·m moments was added to simulate lumbar axial compression, flexion, extension, lateral bending, and axial rotation. The pedicle screw characteristics was recorded under all the different motions. Results: Most of the stress concentrated on the tail part of the pedicle screws and the junction of the screw-rod. The middle pedicle screw was almost endured no stress compared with the upper and the lower one, while the lower one got the highest stress. The maximum stress in UPS was higher than that in BPS under all the motions. Conclusions: In simulating the normal lumbar movements after the fixation for two levels, the tail part of lower pedicle screw and the junction of screw-rod got higher stress and had a high risk for fracture, as a higher risk does in the UPS model in contrast to in the BPS model.
Treatment of Thoracolumbar Fractures Through Different Short Segment Pedicle Screw Fixation Techniques: A Finite Element Analysis

Abstract:Objective: To compare the von Mises stresses of the pedicle screw system and the displacement of injured vertebrae using 3-D finite element analysis, and to evaluate the curative effect of the pedicle screw system. Methods: Finite element methods were used for biomechanical comparison of four posterior short segment pedicle screw fixation techniques. The different pedicle screw models are traditional trajectory (TT), Universal Spine System (USS), cortical bone trajectory (CBT), and CBT at the cranial level and pedicle screw (PS) at the caudal level (UP-CBT). The stress distribution of the screws and connecting rods under different working conditions and the displacement of the injured vertebrae were compared and analyzed. Results: After the pedicle screw system was fixed, the stress under vertical compression was mainly concentrated at the proximal end of the screw, while the stress was mainly concentrated on the connecting rod during flexion, extension, lateral flexion, and rotation. The TT group had the greatest stress during the flexion, extension, and left and right rotation. The UP-CBT group was most stressed when the left and right sides were flexed; the stress of the USS screw system was less than that of the other three models during flexion, lateral flexion, and rotation. The maximum von Mises stress values of pedicle screws in all exercise states were 556.2, 340.7, 458.1, and 533.4 MPa, respectively. In the USS group, the displacement of the injured vertebra was small in the flexion, and the left and right lateral flexion and the right rotation were higher than in the TT group and the CBT group. The maximum displacements of the injured vertebrae in all motion states were 1.679, 1.604, 1.752, and 1.777 mm, respectively. Conclusion: Universal Spine System pedicle screws are relatively less stressed under different working conditions, the risk of breakage is small, and the model is relatively stable; CBT screws do not exhibit better mechanical properties than conventional pedicle screws and USS pedicle screws.
[Study on biomechanical stability in simulated femoral neck fracture fixation by two cannulated screws].

Abstract:OBJECTIVE:To investigate the stability of internal fixation for femoral neck fracture(FNF) using two or three cannulated screws directed to different fracture lines, and to provide a theoretical basis for the clinical decision making. METHODS:Fifteen fresh frozen human femur samples were selected and the FNF model was established with Pauwels angle of 30°, 50° and 70° respectively. The models were fixed with three or two cannulated screws, and their differences at tensile strength, subsidence, shear strength and composite displacement, gap distance, torsional angle and stiffness values of four types of femur were evaluated to identify their biomechanical stability. RESULTS:With Pauwels angle of 30° and 50°, no statistical differences at the referenced outcomes were observed between two and three cannulated screw fixation for FNF(P>0.05). With the Pauwels angle of 70 °, three cannulated screws fixation were significantly superior to two cannulated screws fixation for FNF(P<0.05). Nevertheless, when Pauwels angle was 70°, three cannulated screws fixation turned out to be relative instable compared with Pauwels angle of 30° and 50°. CONCLUSIONS:With Pauwels angle of 30° and 50°, two cannulated screws fixation was similarly stable as using three cannulated screws for FNF. Three cannulated screws fixation were obviously superior to two cannulated screws fixation when Pauwels angle was 70°, but the application of three cannulated screws could still be insufficiently stable when only such routine fixations were utilized. Therefore, Angle Stabilizing System or anti-sliding screws were needed to enhance the stability of FNF fixation.
Pelvic Ring Fractures: Does Spinopelvic Alignment Affect Sacral and Lumbopelvic Fixation Stability?

Abstract:Background Context: The pelvic ring is a complex bony and ligamentous complex, and its structural integrity is paramount to withstand high forces. Pelvic ring fractures, which can result from low-energy impacts or high-velocity injuries and falls from great heights, can severely impair the structural integrity of the pelvic ring. These injuries can be fatal, especially in geriatric patients, with mortality rates ranging between 10% and 16%. Pelvic ring injuries require immediate stabilization, typically performed through internal fixation, to maintain circulatory balance and achieve anatomical restoration of the pelvis. Methods: A previously validated FE model of the spine, pelvis, and femur model was used to evaluate how different fixation techniques for pelvic ring injury affect spinopelvic biomechanics. The SS, PI, PT, and LL of the normal, intact model were modified to generate two more SS models for this study. One modified model with an SS of 20 degrees was considered the low SS model (SSD), while the second modified model with an SS of 32 degrees was termed the high SS model (SSI). A unilateral pelvic ring fracture was simulated by resecting the left side of the sacrum and pelvis. The fractures were stabilized using two different posterior stabilization techniques 1) The fractures were stabilized using two different posterior stabilization techniques, and 2) L5-Ilium posterior screw fixation without cross connector (L5_PF_WO_CC). Results: SSI demonstrated the higher range of motion, followed by the Normal model and least by the SSD model at the right Sacroiliac Joint. No differences were seen between the with and without cross-connector models for a range of motion at the right SIJ. For L5-S1, motion models treated with cross connectors demonstrated the least motion compared to those treated without cross connectors for all configurations and loading conditions. For the sacrum fracture, models with high sacral slopes recorded the least horizontal displacement, followed by decreased sacral slope models for both configurations and loading conditions.

Abstract:To compare the biomechanical properties of several anterior pelvic ring external fixators with two new configurations in the treatment of Tile C pelvic fractures, in order to evaluate the effectiveness of the new configurations and provide a reference for their clinical application. A finite element model of a Tile C pelvic ring injury (unilateral longitudinal sacral fracture and ipsilateral pubic fracture) was constructed. The pelvis was fixed with iliac crest external fixator (IC), anterior inferior iliac spine external fixator (AIIS), combination of IC and AIIS, combination of anterior superior iliac spine external fixator (ASIS) and AIIS, and S1 sacroiliac screw in 5 types of models. The stability indices of the anterior and posterior pelvic rings under vertical longitudinal load, left–right compression load and anterior–posterior shear load were quantified and compared. In the simulated bipedal standing position, the results of the vertical displacement of the midpoint on the upper surface of the sacrum are consistent with the displacement of the posterior rotation angle, and the order from largest to smallest is IC, AIIS, ASIS + AIIS, IC + AIIS and S1 screw. The longitudinal displacement of IC is greater than that of the other models. The displacements of ASIS + AIIS and IC + AIIS are similar and the latter is smaller. In the simulated semi-recumbent position, the vertical displacement and posterior rotation angle displacement of the midpoint on the upper surface of the sacrum are also consistent, ranking from large to small: IC, AIIS, ASIS + AIIS, IC + AIIS and S1 screw. Under the simulated left–right compression load state, the lateral displacements of the highest point of the lateral sacral fracture end are consistent with the highest point of the lateral pubic fracture end, and the order from large to small is S1 screw, IC, AIIS, ASIS + AIIS and IC + AIIS, among which the displacements of S1 screw and IC are larger, and the displacements of ASIS + AIIS and IC + AIIS are similar and smaller than those of other models. The displacements of IC + AIIS are smaller than those of ASIS + AIIS. Under the simulated anterior–posterior shear load condition, the posterior displacements of the highest point of the lateral sacral fracture end and the highest point of the lateral pubic fracture end are also consistent, ranking from large to small: IC, AIIS, ASIS + AIIS, IC + AIIS and S1 screw. Among them, the displacements of IC and AIIS are larger. The displacements of ASIS + AIIS and IC + AIIS are similar and the latter are smaller. For the unstable pelvic injury represented by Tile C pelvic fracture, the biomechanical various stabilities of the combination of IC and AIIS are superior to those of the external fixators of conventional configurations. The biomechanical stabilities of the combination of ASIS and AIIS are also better than those of the external fixators of conventional configurations, and slightly worse than those of the combination of IC and AIIS. Compared with sacroiliac screw and conventional external fixators, the lateral stabilities of IC + AIIS and ASIS + AIIS are particularly prominent.

Abstract:Fur-DNA interactions were analyzed within the regulatory regions of aerobactin and hemolysin operons by a combination of biochemical and ultrastructural methods. Cartography of the Fur binding sites, carried out from electron micrographs, agreed with the data obtained by DNase I footprinting. Visualization of the complexes confirmed the specificity and metal-dependence of Fur binding and demonstrated that the protein polymerizes on its binding sites. Such a polymerization could be involved in the repression process of the bacterial regulator.

S2-Alar-Iliac Screw Fixation Versus Iliac Screw Fixation in the Treatment of Sacral Fractures.

Abstract:Purpose To compare the short-term therapeutic effects of S2-alar-iliac (S2AI) screw fixation and iliac screw fixation techniques in managing sacral fractures. Methods From September 2015 to May 2020, 42 patients with sacral fractures who underwent lumbopelvic fixation by a single surgeon were analyzed. The patients were divided into the S2AI screw group (19 patients) and the iliac screw group (23 patients). Operative data, reduction quality, postoperative complications, and functional outcomes were evaluated. Results The incidence of unplanned reoperation was significantly different between patients treated with S2AI screws than in those treated with iliac screws (0 versus 6, p = 0.02). The mean intraoperative estimated blood loss was 405.26 +/- 229.67 mL in the S2AI screw group and 539.13 +/- 246.32 mL in the iliac screw group (P = 0.08). No significant difference was observed in either group regarding the quality of reduction, functional outcome, or low back pain. The reduction quality based on the Matta criteria and excellent/good outcomes were 21/2 in the iliac screw group and 17/2 in the S2AI screw group. The functional outcomes based on the Majeed score and excellent/good/fair outcomes were 17/3/3 in the iliac screw group and 17/1/1 in the S2AI screw group. No complications, including implant breakage, loosening of the implant, or loss of reduction were found in either group during follow-up. Conclusion Both S2AI screws and iliac screws were effective in the treatment of sacral fractures. The use of S2AI screws, however, was independently associated with fewer unplanned reoperations for surgical site infection, wound dehiscence, and symptoms of screw protrusion than the use of iliac screws.
