Abstract:Abstract Background Although age and regional climate are considered to have effects on the incidence ratio of heat-related illness, quantitative estimation of age or region on the effect of occurring temperature for heat stroke is limited. Methods By utilizing data on the number of daily heat-related ambulance transport (HAT) in each of three age groups (7–17, 18–64, 65 years old, or older) and 47 prefectures in Japan, and daily maximum temperature (DMT) or Wet Bulb Globe Temperature (DMW) of each prefecture for the summer season, the effects of age and region on heat-related illness were studied. Two-way ANOVA was used to analyze the significance of the effect of age and 10 regions in Japan on HAT. The population-weighted average of DMT or DMW measured at weather stations in each prefecture was used as DMT or DMW for each prefecture. DMT or DMW when HAT is one in 100,000 people (T 1 and W 1 , respectively) was calculated for each age category and prefecture as an indicator of heat acclimatization. The relation between T 1 or W 1 and average DMT or DMW of each age category and prefecture were also analyzed. Results HAT of each age category and prefecture was plotted nearly on the exponential function of corresponding DMT or DMW. Average R 2 of the regression function in 47 prefectures in terms of DMW was 0.86, 0.93, and 0.94 for juveniles, adults, and elderly, respectively. The largest regional difference of W 1 in 47 prefectures was 4.5 and 4.8 °C for juveniles and adults, respectively between Hokkaido and Tokyo, 3.9 °C for elderly between Hokkaido and Okinawa. Estimated W 1 and average DMT or DMW during the summer season for 47 prefectures was linearly related. Regarding age difference, the regression line showed that W 1 of the prefecture for DMW at 30 °C of WBGT was 31.1 °C, 32.4 °C, and 29.8 °C for juveniles, adults, and elderly, respectively. Conclusions Age and regional differences affected the incidence of HAT. Thus, it is recommended that public prevention measures for heat-related disorders take into consideration age and regional variability.

Abstract:CONTEXT Few data exist on the diagnostic criteria, and on the effects of puberty induction, in boys with constitutional delay in growth and puberty (CDGP). OBJECTIVE To develop puberty nomograms based on Danish boys with normal pubertal development. To evaluate the different diagnostic criteria and the effect of oral testosterone undecanoate (TU) in boys with CDGP. DESIGN A cross-sectional and longitudinal study of Danish boys with normal pubertal development (COPENHAGEN puberty study). A retrospective observational study of 451 boys evaluated for delayed puberty between 1990 and 2013. SETTING Tertiary referral center for pediatric endocrinology. PARTICIPANTS One hundred and sixty-four (36%) boys evaluated for CDGP were excluded due to missing data, reclassification, or associated comorbidities, yielding 287 (64%) eligible for analysis. MAIN OUTCOME MEASURES The number of patients with CDGP classified by the puberty nomogram (genital stage < -2 SD for age) versus the classical criteria (genital stage 1 at ≥ 14 years). The effect of one year of oral TU treatment on pubertal progression, circulating hormones, height, and predicted adult height (PAH). RESULTS Seventy-eight (27%) of the 287 boys had delayed pubertal onset according to the classical criteria, whereas 173 (60%) of the 287 boys had impaired pubertal progression according to the puberty nomogram. Ninety-six (56%) of these 173 boys were treated with oral TU for 0.8 years (0.5; 1.3) [median (25th; 75th percentiles)], which resulted in beneficial effects on pubertal progression. Height increased from -1.9 SD (-2.5; -1.2) to -1.5 SD (-2.1; -0.7) (P < .001), and PAH increased from 172.3 cm (170.3; 182.8) to 178.1 cm (171.4; 191.7) (P = .001) following one year of treatment. CONCLUSIONS The puberty nomogram evaluates both delayed pubertal onset as well as delayed pubertal progression and allows separation of normal versus abnormal pubertal development. Oral TU treatment was followed by pubertal induction and progression and short-term growth without compromising final height.
Random forest analysis of the relative importance of meteorological indicators for heatstroke cases in Japan based on the degree of severity and place of occurrence

Abstract:Heatstroke is a serious health concern in Japan. To reduce heatstroke risk, the government of Japan implemented the "Heatstroke Alert" nationwide in 2021, employing the wet bulb globe temperature (WBGT) as a criterion. Although the WBGT is a useful meteorological indicator for assessing the risk of heatstroke, other important meteorological indicators must also be investigated. Therefore, using a random forest approach, this study analyzed the relative importance of several meteorological indicators, including those representing heat acclimatization, for each of the 47 Japanese prefectures. Using the generalized linear model, important meteorological indicators were employed as explanatory variables in the heatstroke prediction model to determine the predictive meteorological indicator. Heatstroke cases were evaluated separately by the degree of severity and the place of occurrence. The results showed that the relative temperature (RelTemp), which represents heat acclimatization and was calculated considering past temperature history, was the most predictive (i.e., provided the best goodness of fit) concerning the degree of severity, place of occurrence, and prefectures. RelTemp can be a complementary indicator of WBGT in countries and regions such as Japan, where seasonal differences in heat acclimatization must be considered. In addition, the findings of this study contribute to the development of a more accurate assessment of heatstroke risk.

Abstract:Background: Climate change, as a defining issue of the current time, is causing severe heat-related illness in the context of extremely hot weather conditions. In Japan, the remarkable temperature increase in summer caused by an urban heat island and climate change has become a threat to public health in recent years. Methods: This study aimed to determine the potential risk factors for heatstroke by analysing data extracted from the records of emergency transport to the hospital due to heatstroke in Fukuoka City, Japan. In this regard, a negative binomial regression model was used to account for overdispersion in the data. Age-structure analyses of heatstroke patients were also embodied to identify the sub-population of Fukuoka City with the highest susceptibility. Results: The daily maximum temperature and wet-bulb globe temperature (WBGT), along with differences in both the mean temperature and time-weighted temperature from those of the consecutive past days were detected as significant risk factors for heatstroke. Results indicated that there was a positive association between the resulting risk factors and the probability of heatstroke occurrence. The elderly of Fukuoka City aged 70 years or older were found to be the most vulnerable to heatstroke. Most of the aforementioned risk factors also encountered significant and positive associations with the risk of heatstroke occurrence for the group with highest susceptibility. Conclusion: These results can provide insights for health professionals and stakeholders in designing their strategies to reduce heatstroke patients and to secure the emergency transport systems in summer.

Abstract:Background: The association between ambient temperature and mortality has been well documented worldwide. However, limited data are available on nonfatal health outcomes, such as emergency department visits (EDVs), particularly from China.Objectives: To examine the temperature-EDV association in 12 Chinese cities; and to assess the modification effects by region, gender and age.Methods: Daily meteorological data and non-accidental EDVs were collected during 2011-2014. Poisson regression with distributed lag non-linear model was applied to examine the temperature-lag-EDV association in each city. The effect estimates were pooled using multivariate meta-analysis at the national and regional level. Stratified analyses were performed by gender and age-groups. Sensitivity analyses adjusting for air pollution and relative humidity were conducted.Results: A total of 4,443,127 EDVs were collected from the 12 cities. Both cold and hot temperatures were associated with increased risk of EDVs, with minimum-mortality temperature located at 64th percentile of temperature. The effect of cold temperature appeared on day 2 and persisted until day 30, causing a cumulative relative risk (RR) of 1.80 (1.54, 2.11). The effect of hot temperature appeared immediately and lasted until day 3, with a cumulative RR of 1.15 (1.03,1.29). The effect of temperature on EDVs was similar in male and female but was attenuated with increasing age. The effect of cold temperature on EDVs was greater in southern areas of the country whereas the hot effect was greater in northern cities. The association was robust to a large range of sensitivity analyses.Conclusions: In China, there is a U-shaped association between temperature and risk of EDVs that is independent of air pollution and humidity. The temperature-EDV association varies with latitude and age-groups but is not affected by gender. Forecasting models for hospital emergency departments may be improved if temperature is included as an independent predictor. (C) 2017 Elsevier Ltd. All rights reserved.

Abstract:An increase in the global surface temperature and changes in urban morphologies are associated with increased heat stress especially in urban areas. This can be one of the contributing factors underlying an increase in heat strokes. We examined the impact of summer minimum air temperatures, which often represent nighttime temperatures, as well as a maximum temperature on a heat stroke. We collected data from the records of daily ambulance transports for heat strokes and meteorological data for July and August of 2017–2019 in the Tottori Prefecture, Japan. A time-stratified case-crossover design was used to determine the association of maximum/minimum air temperatures and the incidence of heat strokes. We used a logistic regression to identify factors associated with the severity of heat strokes. A total of 1108 cases were identified with 373 (33.7%) calls originating in the home (of these, 59.8% were the age of ≥ 75). A total of 65.8% of cases under the age of 18 were related to exercise. Days with a minimum temperature ≥ 25 °C had an odds ratio (95% confidence interval) of 3.77 (2.19, 6.51) for the incidence of an exercise-related heat stroke (reference: days with a minimum temperature < 23 °C). The odds ratio for a heat stroke occurring at home or for calls for an ambulance to the home was 6.75 (4.47, 10.20). The severity of the heat stroke was associated with older age but not with air temperature. Minimum and maximum air temperatures may be associated with the incidence of heat strokes and in particular the former with non-exertional heat strokes.
Prediction of the number of heatstroke patients transported by ambulance in Japan’s 47 prefectures: proposal of heat acclimatization consideration

Abstract:Abstract The incidence of heatstroke is affected by various meteorological variables. However, previous studies in Japan have mainly investigated and adopted a single temperature metric or composite index for their analyses. Herein, we conducted a time series study through multivariate analysis of different weather conditions simultaneously, in order to analyze the relative importance of meteorological variables to determine the number of heatstroke patients transported by ambulance in all of Japan’s 47 prefectures. We proposed a method that considers heat acclimatization, which has been found to impact the heatstroke, by manipulating certain meteorological variables. For the heatstroke data, we utilized the secondary data provided by the Fire and Disaster Management Agency, Japan. The time period considered was from May 2015 to September 2019. All calculations were performed using R 3.5.1. For the analysis, the machine learning method of random forest (RF) was applied. The results showed that the relative temperature (RelTemp), which represents heat acclimatization, had the highest ranking among all the meteorological variables studied. Then, we developed the exponential model and the RF model to predict the number of heatstroke patients transported by ambulance by adopting the highly ranked meteorological variables including RelTemp as explanatory variables. To confirm the effectiveness of heat acclimatization, we also developed the exponential model and the RF model both without RelTemp (instead, with maximum temperature). According to the results, the R 2 values of the exponential and the RF models, including RelTemp, were 0.76 and 0.74, respectively, and those of the exponential and the RF models, excluding RelTemp, were 0.68 and 0.67, respectively. We confirmed the effectiveness of considering heat acclimatization via RelTemp and found that the exponential model with RelTemp provided the higher accuracy. Better predictions by the exponential model with RelTemp would contribute to better preemptive allocation of ambulances and medical staff in medical facilities.

Association between ambient temperature and cause-specific cardiovascular disease admissions in Japan: A nationwide study

Abstract:Background: Substantial evidence suggests that non-optimal temperatures can increase the risk of cardiovascular disease (CVD) mortality and morbidity; however, limited studies have reported inconsistent results for hospital admissions depending on study locations, which also lack national-level investigations on cause-specific CVDs. Methods: We performed a two-stage meta-regression analysis to examine the short-term associations between temperature and acute CVD hospital admissions by specific categories [i.e., ischemic heart disease (IHD), heart failure (HF), and stroke] in 47 prefectures of Japan from 2011 to 2018. First, we estimated the prefecture-specific associations using a time-stratified case-crossover design with a distributed lag nonlinear model. We then used a multivariate meta-regression model to obtain national average associations. Results: During the study period, a total of 4,611,984 CVD admissions were reported. We found cold temperatures significantly increased the risk of total CVD admissions and cause-specific categories. Compared with the minimum hospitalization temperature (MHT) at the 98th percentile of temperature (29.9 °C), the cumulative relative risks (RRs) for cold (5th percentile, 1.7 °C) and heat (99th percentile, 30.5 °C) on total CVD were 1.226 [95% confidence interval (CI): 1.195, 1.258] and 1.000 (95% CI: 0.998, 1.002), respectively. The RR for cold on HF [RR = 1.571 (95% CI: 1.487, 1.660)] was higher than those of IHD [RR = 1.119 (95% CI: 1.040, 1.204)] and stroke [RR = 1.107 (95% CI: 1.062, 1.155)], comparing to their cause-specific MHTs. We also observed that extreme heat increased the risk of HF with RR of 1.030 (95% CI: 1.007, 1.054). Subgroup analysis showed that the age group ≥85 years was more vulnerable to these non-optimal temperature risks. Conclusions: This study indicated that cold and heat exposure could increase the risk of hospital admissions for CVD, varying depending on the cause-specific categories, which may provide new evidence to reduce the burden of CVD.
Spatiotemporal variation in heat-related out-of-hospital cardiac arrest during the summer in Japan

Abstract:Background: Although several studies have reported the impacts of extremely high temperature on cardiovascular diseases, few studies have investigated the spatiotemporal variation in the incidence of out-of-hospital cardiac arrest (OHCA) due to extremely high temperature in Japan. Methods: Daily OHCA data from 2005 to 2014 were acquired from all 47 prefectures of Japan. We used time-series Poisson regression analysis combined with a distributed lag non-linear model to assess the temporal variability in the effects of extremely high temperature on OHCA incidence in each prefecture, adjusted for time trends. Spatial variability in the relationships between extremely high temperature and OHCA between prefectures was estimated using a multivariate random-effects meta-analysis. Results: We analyzed 166,496 OHCA cases of presumed cardiac origin occurring during the summer (June to September) that met the inclusion criteria. The minimum morbidity percentile (MMP) was the 51st percentile of temperature during the summer in Japan. The overall cumulative relative risk at the 99th percentile vs. the MMP over lags 0-10days was 1.21 (95% CI: 1.12-1.31). There was also a strong low temperature effect during the summer periods. No substantial difference in spatial or temporal variability was observed over the study period. Conclusions: Our study demonstrated spatiotemporal homogeneity in the risk of OHCA during periods of extremely high temperature between 2005 and 2014 in Japan. Our findings suggest that public health strategies for OHCA due to extremely high temperatures should be finely adjusted and should particularly account for the unchanging risk during the summer.
Heat stroke admissions during heat waves in 1,916 US counties for the period from 1999 to 2010 and their effect modifiers

Abstract:BackgroundHeat stroke is a serious heat-related illness, especially among older adults. However, little is known regarding the spatiotemporal variation of heat stroke admissions during heat waves and what factors modify the adverse effects.MethodsWe conducted a large-scale national study among 23.5 million Medicare fee-for-service beneficiaries per year residing in 1,916 US counties during 1999–2010. Heat wave days, defined as a period of at least two consecutive days with temperatures exceeding the 97th percentile of that county’s temperatures, were matched to non-heat wave days by county and week. We fitted random-effects Poisson regression models to estimate the relative risk (RR) of heat stroke admissions on a heat wave day as compared to a matched non-heat wave day. A variety of effect modifiers were tested including individual-level covariates, community-level covariates, meteorological conditions, and the intensity and duration of the heat wave event.ResultsThe RR declined substantially from 71.0 (21.3–236.2) in 1999 to 3.5 (1.9–6.5) in 2010, and was highest in the northeast and lowest in the west north central regions of the US. We found a lower RR among counties with higher central air conditioning (AC) prevalence. More severe and longer-lasting heat waves had higher RRs.ConclusionsHeat stroke hospitalizations associated with heat waves declined dramatically over time, indicating increased resilience to extreme heat among older adults. Considerable risks, however, still remain through 2010, which could be addressed through public health interventions at a regional scale to further increase central AC and monitoring heat waves.

Abstract:BACKGROUND:The impact of temperature on morbidity remains largely unknown. Moreover, extensive evidence indicates contrasting patterns between temperature-mortality and temperature-morbidity associations. A nationwide comparison of the impact of temperature on mortality and morbidity in more specific subgroups is necessary to strengthen understanding and help explore underlying mechanisms by identifying susceptible populations.OBJECTIVE:We performed this study to quantify and compare the impact of temperature on mortality and morbidity in 47 prefectures in Japan.METHODS:We applied a two-stage time-series design with distributed lag nonlinear models and mixed-effect multivariate meta-analysis to assess the association of temperature with mortality and morbidity by causes (all-cause, circulatory, and respiratory) at prefecture and country levels between 2015 and 2019. Subgroup analysis was conducted by sex, age, and regions.RESULTS:The patterns and magnitudes of temperature impacts on morbidity and mortality differed. For all-cause outcomes, cold exhibited larger effects on mortality, and heat showed larger effects on morbidity. At specific temperature percentiles, cold (first percentile) was associated with a higher relative risk (RR) of mortality [1.45; 95% confidence interval (CI): 1.39, 1.52] than morbidity (1.33; 95% CI: 1.26, 1.40), as compared to the minimum mortality/morbidity temperature. Heat (99th percentile) was associated with a higher risk of morbidity (1.30; 95% CI: 1.28, 1.33) than mortality (1.04; 95% CI: 1.02, 1.06). For cause-specific diseases, mortality due to circulatory diseases was more susceptible to heat and cold than morbidity. However, for respiratory diseases, both cold and heat showed higher risks for morbidity than mortality. Subgroup analyses suggested varied associations depending on specific outcomes.DISCUSSION:Distinct patterns were observed for the association of temperature with mortality and morbidity, underlying different mechanisms of temperature on different end points, and the differences in population susceptibility are possible explanations. Future mitigation policies and preventive measures against nonoptimal temperatures should be specific to disease outcomes and targeted at susceptible populations. https://doi.org/10.1289/EHP12854.
Projection of future heat-related morbidity in three metropolitan prefectures of Japan based on large ensemble simulations of climate change under 2 °C global warming scenarios

Abstract:Recently, global warming has become a prominent topic, including its impacts on human health. The number of heat illness cases requiring ambulance transport has been strongly linked to increasing temperature and the frequency of heat waves. Thus, a potential increase in the number of cases in the future is a concern for medical resource management. In this study, we estimated the number of heat illness cases in three prefectures of Japan under 2 °C global warming scenarios, approximately corresponding to the 2040s. Based on the population composition, a regression model was used to estimate the number of heat illness cases with an input parameter of time-dependent meteorological ambient temperature or computed thermophysiological response of test subjects in large-scale computation. We generated 504 weather patterns using 2 °C global warming scenarios. The large-scale computational results show that daily amount of sweating increased twice and the core temperature increased by maximum 0.168 °C, suggesting significant heat strain. According to the regression model, the estimated number of heat illness cases in the 2040s of the three prefectures was 1.90 (95%CI: 1.35-2.38) times higher than that in the 2010s. These computational results suggest the need to manage ambulance services and medical resource allocation, including intervention for public awareness of heat illnesses. This issue will be important in other aging societies in near future.

COVID-19 Pandemic Modifies Temperature and Heat-Related Illness Ambulance Transport Association in Japan: a Nationwide Observational Study.

Abstract:Background During the COVID-19 pandemic, several illnesses were reduced. In Japan, heat-related illnesses were reduced by 22% compared to pre-pandemic period. However, it is uncertain as to what has led to this reduction. Here, we model the association of maximum temperature and heat-related illnesses in the 47 Japanese prefectures. We specifically examined how the exposure and lag associations varied before and during the pandemic. Methods We obtained the summer-specific, daily heat-related illness ambulance transport (HIAT), exposure variable (maximum temperature) and covariate data from relevant data sources. We utilized a stratified (pre-pandemic and pandemic), two-stage approach. In each stratified group, we estimated the 1) prefecture-level association using a quasi-Poisson regression coupled with a distributed lag non-linear model, which was 2) pooled using a random-effects meta-analysis. The difference between pooled pre-pandemic and pandemic associations was examined across the exposure and the lag dimensions. Results A total of 321,655 HIAT cases was recorded in Japan from 2016 to 2020. We found an overall reduction of heat-related risks for HIAT during the pandemic, with a wide range of reduction (10.85 to 57.47%) in the HIAT risk, across exposure levels ranging from 21.69 degrees C to 36.31 degrees C. On the contrary, we found an increment in the delayed heat-related risks during the pandemic at Lag 2 (16.33%; 95% CI: 1.00, 33.98%). Conclusion This study provides evidence of the impact of COVID-19, particularly on the possible roles of physical interventions and behavioral changes, in modifying the temperature-health association. These findings would have implications on subsequent policies or heat-related warning strategies in light of ongoing or future pandemics.
Effects of high ambient temperature on ambulance dispatches in different age groups in Fukuoka, Japan

Abstract:BACKGROUND: The elderly population has been the primary target of intervention to prevent heat-related illnesses. According to the literature, the highest risks have been observed among the elderly in the temperature-mortality relationship. However, findings regarding the temperature-morbidity relationship are inconsistent.OBJECTIVES: This study aimed to examine the association of temperature with ambulance dispatches due to acute illnesses, stratified by age group. Specifically, we explored the optimum temperature, at which the relative health risks were found to be the lowest, and quantified the health risk associated with higher temperatures among different age groups.METHODS: We used the data for ambulance dispatches in Fukuoka, Japan, during May and September from 2005 to 2012. The data were grouped according to age in 20-year increments. We explored the pattern of the association of ambulance dispatches with temperature using a smoothing spline curve to identify the optimum temperature for each age group. Then, we applied a distributed lag nonlinear model to estimate the risks of the 85th-95th percentile temperature relative to the overall optimum temperature, for each age group.RESULTS: The relative risk of ambulance dispatches at the 85th and 95th percentile temperature for all ages was 1.08 [95% confidence interval (CI): 1.05, 1.12] and 1.12 (95% CI: 1.08, 1.16), respectively. In comparison, among age groups, the optimum temperature was observed as 25.0°C, 23.2°C, and 25.3°C for those aged 0-19, 60-79, and ≥80, respectively. The optimum temperature could not be determined for those aged 20-39 and 40-59. The relative risks of high temperature tended to be higher for those aged 20-39 and 40-59 than those for other age groups.CONCLUSIONS: We did not find any definite difference in the effect of high temperature on ambulance dispatches for different age groups. However, more measures should be taken for younger and middle-aged people to avoid heat-related illnesses.

Abstract:Climate change, especially through heatwaves, significantly affects human health and is a growing global concern. Concurrently, population aging is occurring worldwide, with many countries experiencing an increase in their elderly populations. As the elderly are particularly susceptible to extreme temperatures and unevenly distributed due to internal migration of younger populations, a spatio-temporal analysis integrating temperature changes and demographic data is essential. This study focuses on Japan, a super-aged society where over 25% of the total population is 65 years or older. We examined the effects of climate and the proportion of the elderly population on heatstroke deaths through spatio-temporal analysis within a Bayesian framework. We estimated the annual relative risk of heatstroke-related deaths at the prefecture level from 2008 to 2019. The results indicate a strong spatial autocorrelation in heatstroke deaths across Japan. The spatio-temporal interaction model was the best-performing, showing that regional and temporal variations significantly impact heatstroke mortality. In this model, a one-degree increase in temperature anomaly was linked to a 0.35 (95% CI 0.25 to 0.46) times higher odds of heatstroke deaths, while a 1% increase in the population aged 65 years or older was associated with 4.85 (95% CI 0.92 to 8.65) times higher odds. We found that not only metropolitan areas but also rural areas, such as the Tohoku and Shikoku regions, face a high risk of heatstroke, emphasizing the need to address the challenges in rural communities. Our study highlights the necessity of integrating temperature changes and demographic data in a spatio-temporal context for heatstroke risk assessment. It demonstrates the profound effects of temperature anomalies and the proportion of the elderly population on heatstroke mortality. This research framework could be applicable to other countries experiencing aging and heatwave issues, aiding in the development of targeted public health interventions.

Abstract:Abstract The heat-related health impacts of extremely high temperatures have become increasingly severe worldwide. Quantitative information on these impacts is necessary to plan and implement adaptation measures. The present study aimed to predict the number of heatstroke ambulance calls (HAC) and ambulance occupancy rate of patients with heatstroke (AORPH) under extremely high temperatures in Tokyo, Japan. Extremely high temperatures were defined as the annual highest value of daily maximum temperature that occurs once every 30 y (30y temp) or 50 y (50y temp). Combinations of three greenhouse gas (GHG) emission scenarios and five climate models were adopted to predict the impacts during the base years (1985–2014), mid-21st century (2021–2050), and late 21st century (2071–2100). Furthermore, two types of populations were adopted: fixed and changing. In the case of 50y temp, the HAC during the peak hour in the base years ranged from 52 to 130 persons across the climate models. In the case of a fixed population, in the late 21st century it ranged from 123 to 2618 persons across the climate models and GHG scenarios. In the case of a changing population, the corresponding values in late 21st century ranged from 128 to 3078 persons. The AORPH during the peak hour ranged from 32 % to 80 % during the base years for 50y temp, assuming that all ambulances were dispatched for heatstroke only. In the late 21st century, it ranged from 75 % to 1605 % in the fixed population case. With population change, the corresponding values in late 21st century ranged from 83 % to 1781 %. In many scenarios, not all patients with heatstroke could be transported (AORPH ≥ 100 %) even if all ambulances have been dispatched. Therefore, adaptation measures that mitigate such an ambulance transport system collapse must be planned and implemented.
Possible adaptation measures for climate change in preventing heatstroke among older adults in Japan

Abstract:Introduction: Heatstroke mortality is highest among older adults aged 65 years and older, and the risk is even doubled among those aged 75 years and older. The incidence of heatstroke is expected to increase in the future with elevated temperatures owing to climate change. In the context of a super-aged society, we examined possible adaptation measures in Japan that could prevent heatstroke among older people using an epidemiological survey combined with mathematical modeling. Methods: To identify possible interventions, we conducted a cross-sectional survey, collecting information on heatstroke episodes from 2018 to 2019 among people aged 75 years and older. Responses were analyzed from 576 participants, and propensity score matching was used to adjust for measurable confounders and used to estimate the effect sizes associated with variables that constitute possible interventions. Subsequently, a weather-driven statistical model was used to predict heatstroke-related ambulance transports. We projected the incidence of heatstroke-related transports until the year 2100, with and without adaptation measures. Results: The risk factor with the greatest odds ratio (OR) of heatstroke among older adults was living alone (OR 2.5, 95% confidence interval: 1.2-5.4). Other possible risk factors included an inability to drink water independently and the absence of air conditioning. Using three climate change scenarios, a more than 30% increase in the incidence of heatstroke-related ambulance transports was anticipated for representative concentration pathways (RCP) 4.5 and 8.5, as compared with a carbon-neutral scenario. Given 30% reduction in single living, a 15% reduction in the incidence of heatstroke is expected. Given 70% improvement in all three risk factors, a 40% reduction in the incidence can be expected. Conclusion: Possible adaptation measures include providing support for older adults living alone, for those who have an inability to drink water and for those without air conditioning. To be comparable to carbon neutrality, future climate change under RCP 2.6 requires achieving a 30% relative reduction in all three identified risks at least from 2060; under RCP 4.5, a 70% reduction from 2050 at the latest is needed. In the case of RCP 8.5, the goal of heatstroke-related transports approaching RCP 1.9 cannot be achieved.
Quantitative Assessment of the Contribution of Meteorological Variables to the Prediction of the Number of Heat Stroke Patients for Tokyo

Abstract:This study reveals the best combination of meteorological variables for the prediction of the number of emergency transport due to heat stroke over 64 years old in Tokyo metropolis based on a generalized linear model using 2008-2016 data. Temperature, relative humidity, wind speed, and solar radiation were used as candidates of the explanatory variables. The variable selection with Akaike's information criterion (AIC) showed that all the four meteorological elements were selected for the prediction model. Additional analysis showed that the combination of daily mean temperature, maximum relative humidity, maximum wind speed, and total solar radiation as explanatory variables gives the best prediction, with approximately 19% less error than the conventional single-variable model which only uses the daily mean temperature. Finally, we quantitatively estimated the relative contribution of each variable to the prediction of the daily number of heat stroke patients using standardized partial regression coefficients. The result reveals that temperature is the largest contributor. Solar radiation is second, with approximately 20% of the temperature effect. Relative humidity and wind speed make relatively small contributions, each contributing approximately 10% and 9% of the temperature, respectively. This result provides helpful information to propose more sophisticated thermal indices to predict heat stroke risk.
