Abstract:BACKGROUND: Theimpactoftemperatureonmorbidityremainslargelyunknown. Moreover, extensiveevidenceindicatescontrastingpatternsbetween temperature-mortalityandtemperature-morbidityassociations. Anationwidecomparisonoftheimpactoftemperatureonmortalityandmorbidityin morespecific subgroupsisnecessarytostrengthenunderstandingandhelpexploreunderlyingmechanismsbyidentifyingsusceptiblepopulations. OBJECTIVE: We performedthisstudytoquantifyandcomparetheimpactoftemperatureonmortalityandmorbidityin47prefecturesinJapan. METHODS: We appliedatwo-stagetime-seriesdesignwithdistributedlagnonlinearmodelsandmixed-effect multivariatemeta-analysistoassessthe association oftemperaturewithmortalityandmorbiditybycauses(all-cause, circulatory, andrespiratory) atprefectureandcountrylevelsbetween 2015 and2019. Subgroupanalysiswasconductedbysex, age, andregions. RESULTS: The patternsandmagnitudesoftemperatureimpactsonmorbidityandmortalitydiffered. Forall-causeoutcomes, coldexhibitedlarger effects onmortality, andheatshowedlargereffects onmorbidity. Atspecific temperaturepercentiles, cold(first percentile) wasassociatedwitha higher relativerisk(RR) ofmortality[1.45; 95% confidence interval(CI): 1.39,1.52] thanmorbidity(1.33; 95% CI: 1.26,1.40), ascomparedtothe minimum mortality/morbiditytemperature. Heat(99thpercentile) wasassociatedwithahigherriskofmorbidity(1.30; 95% CI: 1.28,1.33) thanmortality (1.04; 95% CI: 1.02,1.06). Forcause-specific diseases, mortalityduetocirculatorydiseaseswasmoresusceptibletoheatandcoldthanmorbidity. However, forrespiratorydiseases, bothcoldandheatshowedhigherrisksformorbiditythanmortality. Subgroupanalysessuggestedvaried associations dependingonspecific outcomes. DISCUSSION: Distinct patternswereobservedfortheassociationoftemperaturewithmortalityandmorbidity, underlyingdifferent mechanismsoftemperature ondifferent endpoints, andthedifferences inpopulationsusceptibilityarepossibleexplanations. Futuremitigationpoliciesandpreventive measures againstnonoptimaltemperaturesshouldbespecific todiseaseoutcomesandtargetedatsusceptiblepopulations. https://doi.org/10.1289/ EHP12854
