Abstract:Continuous bleeding, bacterial infection, oxidative stress, and vascular damage within a diabetic wound microenvironment necessitate the development of multifunctional dressings that match the complex physio-logical process of skin healing. Herein, copper-tannic acid (CuTA) nanosheets were first synthesized by chelating copper ions with tannic acid (TA). These nanosheets were subsequently integrated into an injectable self-healing hydrogel comprising methacrylic anhydride-modified gelatin (GelMA), cationic guar gum (CG), and borax. Through free-radical polymerization as well as hydrogen and borate ester bonds formation, we fabricated an acidic and highly reactive oxygen species (ROS)-responsive composite hydrogel named GGB-CT, which exhibited remarkable hemostasis and adhesion. With responsive decomposition of GGB-CT, the released CuTA efficiently scavenged excess ROS as well as effectively killed bacteria by inhibiting arginine synthesis, blocking the tricarboxylic acid cycle and inducing cuproptosis-like death in the tested microbes. In the later stages of wound healing, the composite hydrogel substantially promoted macrophage polarization and angiogenesis, thus accelerating the re-epithelialization of the wound area. Overall, this study proposes an innovative hydrogel for treating infected diabetic wounds and may inspire new thinking of high-performance hydrogels for biomedical applications.

Abstract:Management of diabetic wounds presents a global health challenge due to elevated levels of ROS in the wound microenvironment, persistent dysregulation of inflammation modulation, and limitations in commercially available dressings. Addressing this issue, we have developed a pH-responsive and glucose-sensitive multifunctional hydrogel dressing that dynamically responds to the wound microenvironment and enables on-demand drug release. The dressing incorporates a matrix material based on aminophenylboronic acid-functionalized alginate and polyhydroxy polymer, alongside an enhancer phase consisting of a self-assembled metal-phenol coordination nanospheres formed by tannic acid and iron ions. Through the dynamic borate ester bonds and catechol-metal ion coordination bonds, the dressing exhibits remarkable shape adaptability, self-healing capability, tissue adhesiveness, antioxidant activity, and photothermal responsiveness, without additional curatives or crosslinking agents. As a wound dressing, it elicits macrophage polarization towards an anti-inflammatory phenotype while maintaining long-lasting antimicrobial effects. In a diabetic mouse model of full-thickness wound infections, it effectively mitigated inflammation and vascular damage, significantly expediting the wound healing process with a commendable 97.7% wound closure rate. This work provides a new direction for developing multifunctional smart hydrogel dressings that can accelerate diabetic wound healing for human health.

Abstract:Favorable adhesion and on-demand detachment are the key characteristics for evaluating the effectiveness of diabetic wound dressings in clinical. Herein, a novel adhesive hydrogel is constructed based on dual dynamic covalent (Schiff base and borate ester bond) cross-linking among carboxymethyl chitosan (CMCS), 2-formylphenylboronic acid (2-FPBA) and epigallocatechin-3-gallate (EGCG) as a multifunctional dressing material for diabetic wound healing. The CMCS/2-FPBA/EGCG (CFE) adhesive hydrogels not only display good mechanical strengths (adhesive, injectable, moldable and self-healing), considerable pH-responsive EGCG releasing and photothermal properties, but also excellent antimicrobial, anti-oxidant, anti-inflammatory and angiogenetic activity. Interestingly, CFE adhesive hydrogels display on-demand detachable property while spraying natural organic acid (citric acid, salicylic acid, and vitamin C), which could avoid secondary injury during changing the wound dressings. Therefore, this study provides a promising and comprehensive strategy with great potential for application in the diabetic wound treatment.

Abstract:Wound healing in diabetes is a complex physiological process with risks of ulceration and amputation. Real-time monitoring and treatment of diabetic wounds is an effective prevention of further deterioration. Herein, a multifunctional dressing is developed by encapsulating europium-containing bioactive glass (EuBG) and MoO3-x in biocompatible sodium alginate (SA) dressing (MoO3-x-EuBG-SA), aiming to simultaneously monitor the wound pH value and hydrogen peroxide (H2O2) concentration, establish anti-inflammatory microenvironment, and promote wound healing. pH-responsive fluorescence quenching effect of the EuBG contributes to visible pH values monitoring, and MoO3-x enables high responsiveness and accurate colorimetric indication of H2O2 variability to understand the dynamic wound status. Then, these fluorescent and colorimetric photographs can be captured by smartphones and transformed into signals for wound pH value and H2O2 concentration, enabling rapid and convenient real-time assessment of wound dynamic conditions. Furthermore, MoO3-x-EuBG-SA dressing can reduce inflammation and promote the proliferation, differentiation, and migration of fibroblasts and endothelial cells in vitro with the help of the released bioactive Si, Ca, and Mo ions. Notably, the MoO3-x-EuBG-SA dressing exhibits excellent abilities of monitoring wound microenvironment, immune regulation, and promoting diabetic wound healing in vivo. Overall, the multifunctional dressing has great potential for managing diabetic wounds and promoting wound healing.

Abstract:Diabetic wounds are serious clinical complications which manifest wet condition due to the mass exudate, along with disturbed regulation of inflammation, severe oxidative stress and repetitive bacterial infection. Existing treatments for diabetic wounds remain unsatisfactory due to the lack of ideal dressings that encompass mechanical performance, adherence to moist tissue surfaces, quick repair, and diverse therapeutic benefits. Herein, we fabricated a wet adhesive, self-healing, glucose-responsive drug releasing hydrogel with efficient antimicrobial and pro-healing properties for diabetic wound treatment. PAE hydrogel was constructed with poly(acrylic acid-co-acrylamide) (AA-Am) integrated with a dynamic E-F crosslinker, which consisted of epigallocatechin gallate (EGCG) and 4-(2-acrylamidoethylcarbamoyl)-3-fluorophenylboronic acid (AFPBA). Due to the dynamic crosslinking nature of boronate esters, abundant catechol groups and hydrogen bonding, PAE hydrogel demonstrated excellent mechanical properties with about 1000 % elongation, robust adhesion to moist tissues, fast self-healing, and absorption of biofluids of 10 times of its own weight. Importantly, PAE hydrogel exhibited sustained and glucose-responsive release of EGCG. Together, the bioactive PAE hydrogel had effective antibacterial, antioxidative, and anti-inflammatory properties in vitro, and accelerated diabetic wound healing in rats via reducing tissue-inflammatory response, enhancing angiogenesis, and reprogramming of macrophages. Overall, this versatile hydrogel provides a straightforward solution for the treatment of diabetic wound, and shows potential for other wound-related application scenarios.

Abstract:The clinical treatment of chronic diabetic wounds is a long-standing thorny issue. Strategies targeting the diabetic micro-environment have been developed to promote wound healing. However, it remains challenging to reverse the adverse conditions and re-activate tissue regeneration and angiogenesis. In this work, we develop injectable hydrogels that are responsive to acidic conditions, reactive oxygen species (ROS), and high glucose levels in a diabetic wound micro-environment to sustainably deliver tannic acid (TA) to augment antibacterial, anti-inflammatory, and anti-oxidative activities. This triple-responsive mechanism is designed by introducing dynamic acylhydrazone and phenylboronic ester bonds to crosslink modified hyaluronic acid (HA) chains. At a diabetic wound, the acylhydrazone bonds may be hydrolyzed at low pH. Meanwhile, glucose may compete with TA, and ROS may oxidize the C–B bond to release TA. Thus, sustained release of TA is triggered by the diabetic micro-environment. The released TA effectively scavenges ROS and kills bacteria. In vivo experiments on diabetic mice demonstrate that the hydrogel dressing highly promotes angiogenesis and extracellular matrix (ECM) deposition, leading to eventual full healing of diabetic skin wounds. This micro-environment-triggered triple-responsive drug release provides a promising method for chronic diabetic wound healing.

Abstract:The proficient handling of diabetic wounds, a rising issue coinciding with the global escalation of diabetes cases, poses significant clinical difficulties. A range of biofunctional dressings have been engineered and produced to expedite the healing process of diabetic wounds. This study proposes a multifunctional hydrogel dressing for diabetic wound healing, which is composed of Polyvinyl Alcohol (PVA) and N1-(4-boronobenzyl)-N3-(4-boronophenyl)-N1, N1, N3, N3-teramethylpropane-1, 3-diaminium (TSPBA), and a dual-drug loaded Gelatin methacryloyl (GM) microgel. The GM microgel is loaded with sodium fusidate (SF) and nanoliposomes (LP) that contain metformin hydrochloride (MH). Notably, adhesive and self-healing properties the hydrogel enhance their therapeutic potential and ease of application. In vitro assessments indicate that SF-infused hydrogel can eliminate more than 98% of bacteria within 24 h and maintain a sustained release over 15 days. Additionally, MH incorporated within the hydrogel has demonstrated effective glucose level regulation for a duration exceeding 15 days. The hydrogel demonstrates a sustained ability to neutralize ROS throughout the entire healing process, predominantly by electron donation and sequestration. This multifunctional hydrogel dressing, which integrated biological functions of efficient bactericidal activity against both MSSA and MRSA strains, blood glucose modulation, and control of active oxygen levels, has successfully promoted the healing of diabetic wounds in rats in 14 days. The hydrogel dressing exhibited significant effectiveness in facilitating the healing process of diabetic wounds, highlighting its considerable promise for clinical translation.

Abstract:Antibacterial, anti-inflammatory, and healing-promoting treatment of chronically infected wounds is not only clinical problems to be solved urgently but also important needs related to people’s livelihood. However, multifunctional hydrogel dressings with highly efficient antibacterial, anti-inflammatory, and wound microenvironment sensitive drug release performance still remain a challenge. In this work, a multifunctional hydrogel dressing was achieved by introducing mussel-like super adhesion catechol groups and glucose/ROS-sensitive dynamic phenylboronester bond cross-linking network, which endowed the hydrogel with excellent injectable, self-healing, tissue adhesion properties, and glucose/ROS-sensitive drug release performance. Additionally, high-efficiency photothermal synergistic antibacterial TP@Ag NPs were prepared and introduced into the hydrogel. The glucose/ROS responsive borate bond in the hydrogel network could realize the responsive release of TP@Ag NPs, thereby promoting M2 polarization to achieve an efficient anti-inflammatory effect. And the excess reactive oxygen (ROS) could also be scavenged by the synergistic effect of the catechol group and ROS-sensitive borate bond in the hydrogel. Moreover, excellent biocompatibility and antioxidant performance were shown in the hydrogels. Finally, the hydrogel could fully fill and adhere to irregular wound tissue through injection and significantly promote infected diabetic wound healing in vivo through efficient photothermal synergistic sterilization, ROS scavenging ability, and modulation of macrophage M2 polarization to reduce inflammatory response. Thus, it illustrated the superiority of the hydrogel as an advanced functional dressing for infected diabetic wound healing.

Abstract:Diabetic severe wounds are difficult to heal and regenerate, and are extremely challenging to manage in clinical practice with common wound dressings. There is a need to develop multifunctional dressings that meet the requirements for efficient treatment of diabetic wounds. Therefore, the nanocomposite hydrogel with oxidized chondroitin sulfate (OCS), carboxymethyl chitosan (CMC), and phenol red-modified ε-poly- l -lysine (EPL-PR) as the backbones, encapsulated with chondroitin sulfate-modified selenium nanoparticles () and infinite coordination polymer nanomedicine (ICPs) composed of procyanidins, berberine hydrochloride and Fe(III), was designed and fabricated. The prepared nanocomposite hydrogel system allows for rapid in situ gelation and complete coverage of irregular wounds. In vivo and in vitro investigations demonstrate that the nanocomposite hydrogel shows excellent tissue adhesion, antibacterial activities, biocompatibility and free radical scavenging properties. The rapid hemostasis of the nanocomposite hydrogel is confirmed through hemorrhage models of liver and caudal amputation. All the advantages enable the nanocomposite hydrogel to effectively induce skin tissue remodeling, promote vascular and hair follicle regeneration, remove free radicals from wounds, and accelerate wound healing. The nanocomposite hydrogel can also successfully monitor the pH range of 5–10, thus enabling real-time visual monitoring of the wound healing phase, facilitating wound management in clinical settings. This multifunctional wound dressing opens up ideas for the treatment and management of diabetic full-thickness wounds.

Abstract:Wound healing in diabetic patients presents significant challenges in clinical wound care due to high oxidative stress, excessive inflammation, and a microenvironment prone to infection. In this study, we successfully developed a multifunctional tandem dynamic covalently cross-linked hydrogel dressing aimed at diabetic wound healing. This hydrogel was constructed using cyanoacetic acid functionalized dextran (Dex-CA), 2-formylbenzoylboric acid (2-FPBA) and natural oligomeric proanthocyanidins (OPC), catalyzed by histidine. The resulting Dex-CA/OPC/2-FPBA (DPOPC) hydrogel can be dissolved triggered by cysteine, thereby achieving "controllable and non-irritating" dressing change. Furthermore, the incorporation of OPC as a hydrogel building block endowed the hydrogel with antioxidant and anti-inflammatory properties. The cross-linked network of the DPOPC hydrogel circumvents the burst release of OPC, enhancing its biosafety. In vivo studies demonstrated that the DPOPC hydrogel significantly accelerated the wound healing process in diabetic mice compared to a commercial hydrogel, achieving an impressive wound closure rate of 98 % by day 14. The DPOPC hydrogel effectively balanced the disrupted inflammatory state during the healing process. This dynamic hydrogel based on natural polyphenols is expected to be an ideal candidate for dressings intended for chronic wounds.

Abstract:The intricate microenvironment of diabetic wounds characterized by hyperglycemia, intense oxidative stress, persistent bacterial infection and complex pH fluctuations hinders the healing process. Herein, an injectable multifunctional hydrogel (QPTx) was developed, which exhibited excellent mechanical performance and triple responsiveness to pH, temperature, and glucose due to dynamic covalent cross-linking involving dynamic Schiff base bonds and phenylboronate esters with phenylboronic-modified quaternized chitosan (QCS-PBA), polydopamine coated tunicate cellulose crystals (PDAn@TCNCs) and polyvinyl alcohol (PVA). Furthermore, the hydrogels can incorporate insulin (INS) drugs to adapt to the complex and variable wound environment in diabetic patients for on-demand drug release that promote diabetic wound healing. Based on various excellent properties of the colloidal materials, the hydrogels were evaluated for self-healing, rheological and mechanical properties, in vitro insulin response to pH/temperature/glucose release, antibacterial, antioxidant, tissue adhesion, coagulation, hemostasis in vivo and in vitro, and biocompatibility and biodegradability. By introducing PDAn@TCNCs particles, the hydrogel has photothermal antibacterial activity, enhanced adhesion and oxidation resistance. We further demonstrated that these hydrogel dressings significantly improved the healing process compared to commercial dressings (Tegaderm™) in full-layer skin defect models. All indicated that the glucose-responsive QPTx hydrogel platform has great potential for treating diabetic wounds.

Abstract:The revolutionary role of tissue adhesives in wound closure, tissue sealing, and bleeding control necessitates the development of multifunctional materials capable of effective and scarless healing. In contrast to the use of traditionally utilized toxic oxidative crosslinking initiators (exemplified by sodium periodate and silver nitrate), herein, the natural polyphenolic compound tannic acid (TA) was used to achieve near instantaneous (<25s), hydrogen bond mediated gelation of citrate-based mussel-inspired bioadhesives combining anti-oxidant, anti-inflammatory, and antimicrobial activities (3A-TCMBAs). The resulting materials were self-healing and possessed low swelling ratios ( 90% bacterial death upon near-infrared (NIR) irradiation). In vivo evaluation in both an infected full-thickness skin wound model and a rat skin incision model demonstrated that 3A-TCMBAs + NIR treatment could promote wound closure and collagen deposition and improve the collagen I/III ratio on wound sites while simultaneously inhibiting the expression of pro-inflammatory cytokines. Further, phased angiogenesis was observed via promotion in the early wound closure phases followed by inhibition and triggering of degradation & remodeling of the extracellular matrix (ECM) in the late stage (supported by phased CD31 (platelet endothelial cell adhesion molecule-1) PDGF (platelet-derived growth factor) and VEGF (vascular endothelial growth factor) expression as well as elevated matrix metalloprotein-9 (MMP-9) expression on day 21), resulting in scarless wound healing. The significant convergence of material and bioactive properties elucidated above warrant further exploration of 3A-TCMBAs as a significant, new class of bioadhesive.

Abstract:The treatment of diabetic wounds remains a formidable clinical challenge worldwide. Because of persistent inflammatory reaction, excessive oxidative stress, cell dysfunction, poor blood microcirculation and other microvascular complications, diabetic wounds often fall into inflammatory circulation and are difficult to heal, making patients confront the risk of amputation. In this study, silver complex nanoparticles with Resina Draconis extract and Rhodiola rosea L. extract were loaded in situ onto thermoplastic polyurethane nanofibers to develop a multifunctional electrospun nanofiber wound dressing with excellent mechanical properties, superior water absorption and breathability, good coagulation promoting activity, strong antibacterial performance and antioxidant properties. This wound dressing could effectively enhance the migration and proliferation of fibroblasts, reduce the increased thickness of regenerated epidermis caused by diabetes, and the high expression and high lipid peroxidation levels of IL-1 β, IL-6, TNF α, iNOS and MMP-9, and raise the low expression of VEGF, which shows great potential to accelerate the wound healing of diabetic mouse models. The wound healing rate reached about 87.92%, close to the non-diabetic group. Our findings suggest a breakthrough in diabetic wound care, offering a viable solution to a long-standing medical shackle.

Abstract:Diabetic wounds management remains a significant global challenge due to impaired wounds healing, bleeding, and infection. Oxidative stress (OS), resulting from dysregulated mitochondrial reactive oxygen species (mtROS), plays a crucial role in the diabetic wounds pathogenesis. Furthermore, diabetic infected wounds can induce excessive ROS production and an acidic microenvironment due to the accumulation of bacterial metabolic products. Thus, a pH-sensitive intelligent dressing could serve as an ideal carrier for precise regulation of mtROS and promote faster diabetic wounds healing. In this study, we fabricated a pH-sensitive and mitochondria-modulating intelligent multilayered dressing system. We synthesized mesoporous silica-coated selenium particles (SS) for mtROS regulation and infection resistance. These SS were then loaded into a polyvinyl alcohol (PVA)/(N-[(2-hydroxy-3-trimethylamine) propyl] chitosan chloride, HTCC) hydrogel for pH-sensitive controlled release, blood coagulation, and antibacterial treatment. Finally, the resulting system was cross-linked with carboxymethyl cotton gauze (CG) to enhance hemostatic ability and reduce adhesiveness. Experimental results from cellular and animal models of diabetic wounds demonstrated that HTCC/PVA/SS-CG dressing promotes faster hemostasis, protects against microbial infection, and accelerates diabetic wounds healing through its mitochondria-modulating action, which offers a comprehensive treatment strategy for diabetic wounds and shows promising prospects for clinical transformation.
A 4d Printed Adhesive, Thermo‐Contractile, and Degradable Hydrogel for Diabetic Wound Healing

Abstract:Chronic wound healing remains a substantial clinical challenge. Current treatments are often either prohibitively expensive or insufficient in meeting the various requirements needed for effective diabetic wound healing. A 4D printing multifunctional hydrogel dressing is reported here, which aligns perfectly with wounds owning various complex shapes and depths, promoting both wound closure and tissue regeneration. The hydrogel is prepared via digital light process (DLP) 3D printing of the mixture containing N‐isopropylacrylamide (NIPAm), curcumin‐loaded Pluronic F127 micelles (Cur‐PF127), and poly(ethylene glycol) diacrylate‐dopamine (PEGDA575‐Do), a degradable crosslinker. The use of PEGDA575‐Do ensures tissue adhesion and degradability, and cur‐PF127 serves as an antibacterial agent. Moreover, the thermo‐responsive mainchains (i.e., polymerized NIPAm) enables the activation of wound contraction by body temperature. The features of the prepared hydrogel, including robust tissue adhesion, temperature‐responsive contraction, effective hemostasis, spectral antibacterial, biocompatibility, biodegradability, and inflammation regulation, contribute to accelerating diabetic wound healing in MRSA‐infected full‐thickness skin defect diabetic rat models and liver injury mouse models, highlighting the potential of this customizable, mechanobiological, and inflammation‐regulatory dressing to expedite wound healing in various clinical settings. Abstract:Diabetic wound healing and angiogenesis remain a worldwide challenge for both clinic and research. The use of adipose stromal cells (ASCs) derived exosomes delivered by bioactive dressing provides a potential strategy for repair diabetic wounds with less scar formation and fast healing. In this study, we fabricated an injectable adhesive thermosensitive multifunctional polysaccharide-based dressing (FEP) with sustained pH-responsive exosomes release for promoting angiogenesis and diabetic wound healing. The FEP dressing possessed multifunctional properties including efficient antibacterial activity/multidrug-resistant bacteria (MDRB), fast hemostatic ability, self-healing behavior, tissue-adhesive and good UV-shielding performance. FEP@exosomes (FEP@exo) can significantly enhance the proliferation, migration and tube formation of endothelial cells in vitro. In vivo results from a diabetic full-thickness cutaneous wound model showed that FEP@exo dressing accelerated the wound healing by stimulating the angiogenesis process of the wound tissue. The enhanced cell proliferation, granulation tissue formation, collagen deposition and remodeling and re-epithelialization probably lead to the fast healing with less of scar tissue formation and skin appendages regeneration. This study showed that combining bioactive molecules into multifunctional dressing should have great potential in achieving satisfactory healing in diabetic and other vascular-impaired related wounds.

Abstract:Currently, adhesive hydrogels have shown promising effect in chronic diabetic wound repair. However, there are issues and challenges in treating diabetic wounds due to inadequate wet adhesion, unable to fill irregular and deep wounds, and oxidative stress. Herein, a mussel-inspired naturally hydrogel dressing with rapid shape adaptability, wet adhesion and antioxidant abilities for irregular, deep and frequently movement diabetic wounds repair was constructed by comprising catechol modified carboxymethyl cellulose (CMC-DA) and tannic acid. Benefiting from the reversible hydrogen bonding, the resulting hydrogels exhibited injectability, remarkable self-healing ability, rapid shape adaptability and strong tissue adhesion (45.9 kPa), thereby contributing to self-adaptive irregular-shaped wounds or moving joint parts. Especially, the adhesion strength of the hydrogel on wet tissue still remained at 14.9 kPa. Besides, the hydrogels could be easily detached from the skin by ice-cooling that avoided secondary damage caused by dressing change. Remarkably, the hydrogels possessed excellent antioxidant, satisfactory biocompatibility, efficient hemostasis and antibacterial properties. The in vivo evaluation further demonstrated that the hydrogel possessed considerable wound-healing promotion effect by regulating diabetic microenvironment, attributed to that the hydrogel could significantly reduce inflammatory response, alleviate oxidative stress and regulate neovascularization. Overall, this biosafe adhesive hydrogel had great potentials for diabetic wound management.
Pomegranate-inspired multifunctional nanocomposite wound dressing for intelligent self-monitoring and promoting diabetic wound healing

Abstract:Diabetic wounds are chronically hard-healing wounds. Bacterial infection, persistent inflammation and impaired angiogenesis are key factors affecting diabetic wound healing. Herein, inspired by pomegranate, Au/Ag nanodots (Au/AgNDs) with fluorescent and photothermal properties were adopted as the pomegranate-like core, and the polyvinyl alcohol hydrogel as the pomegranate-like shell to obtain the multifunctional nanocomposite wound dressing for promoting diabetic wounds healing and real-time self-monitoring the dressing status. On the one hand, the antibacterial and photothermal therapy synergistic strategy based on the nanocomposite has an excellent treatment effect on diabetic wounds by highly antibacterial, anti-inflammation, accelerating collagen deposition and angiogenesis. On the other hand, the nanocomposite can be used as "smart messenger" to determine the appropriate time for dressing replacement. With the release of Au/AgNDs from the nanocomposite, the photothermal performance and antibacterial activity of the wound dressing were reduced, and the fluorescence intensity decreases. The change of fluorescence intensity can be visualized by the naked eye, which guides the appropriate time for dressing replacement, and avoids secondary wound damage caused by frequent and blind dressing replacement. This work provides an effective strategy for the treatment of diabetic wounds and intelligent self-monitoring of the state of dressings in clinical practice.
