Abstract:Patients with diabetes suffer from a variety of complications and easily develop diabetic chronic wounds. The microenvironment of diabetic wounds is characterized by an excessive amount of reactive oxygen species (ROS) and an imbalance of proinflammatory and anti-inflammatory cells/factors, which hinder the regeneration of chronic wounds. In the present study, a wound dressing with immunomodulation and electroconductivity properties was prepared and assayed in vitro and in vivo. [2-(acryloyloxy) ethyl] Trimethylammonium chloride (Bio-IL) and gelatin methacrylate (GelMA) were 3D printed onto a doxycycline hydrochloride (DOXH)-loaded and ROS-degradable polyurethane (PFKU) nanofibrous membrane, followed by UV irradiation to obtain conductive hydrogel strips. DOXH was released more rapidly under a high ROS environment. The dressing promoted migration of endothelial cells and polarization of macrophages to the anti-inflammatory phenotype (M2) in vitro. In a diabetic rat wound healing test, the combination of conductivity and DOXH was most effective in accelerating wound healing, collagen deposition, revascularization, and re-epithelialization by downregulating ROS and inflammatory factor levels as well as by upregulating the M2 macrophage ratio. STATEMENT OF SIGNIFICANCE: The microenvironment of diabetic wounds is characterized by an excessive amount of reactive oxygen species (ROS) and an imbalance of proinflammatory and anti-inflammatory cells/factors, which hinder the regeneration of chronic wounds. Herein, a wound dressing composed of a DOXH-loaded ROS-responsive polyurethane membrane and 3D-printed conductive hydrogel strips was prepared, which effectively accelerated skin regeneration in diabetic wounds in vivo with better epithelialization, angiogenesis, and collagen deposition. DOXH regulated the dysfunctional wound microenvironment by ROS scavenging and polarizing macrophages to M2 phenotype, thereby playing a dominant role in diabetic wound regeneration. This design may have great potential for preparing other similar materials for the therapy of other diseases with excessive inflammation or damage to electrophysiological organs, such as nerve defect and myocardial infarction.

Abstract:The negatively charged and ROS‐responsive hydrogel (NCRH)promotes chronic diabetic wound remodeling by leading to a decrease in M1 macrophages and an increase in M2 macrophages via capturing positively charged inflammatory factors and scavenging reactive oxygen species.In vivo evaluation of diabetic rat skin model confirms that the NCRH can effectively promote tissue regeneration by modulating the immune microenvironment during the wound healing. Diabetic wound exhibits the complex characteristics involving continuous oxidative stress and excessive expression of pro‐inflammatory cytokines to cause a long‐term inflammatory microenvironment. The repair healing of chronic diabetic wounding is tremendously hindered due to persistent inflammatory reaction. To address the aforementioned issues, here, a dual‐functional hydrogel is designed, consisting of N1‐(4‐boronobenzyl)‐N3‐(4‐boronophenyl)‐N1, N1, N3, N3‐tetramethylpropane‐1, 3‐diaminium (TSPBA) modified polyvinyl alcohol (PVA) and methacrylamide carboxymethyl chitosan (CMCSMA) can not only electrostatically adsorb proinflammatory cytokines of IL1‐β and TNF‐α, but can also chemically scavenge the excessive reactive oxygen species (ROS) in situ. Both in vitro and in vivo evaluations verify that the negatively charged and ROS‐responsive hydrogel (NCRH) can effectively modulate the chronic inflammatory microenvironment of diabetic wounds and significantly enhance wound remodeling. More importantly, the well‐designed NCRH shows a superior skin recovery in comparison with the commercial competitor product of wound dressing. Consequently, the current work highlights the need for new strategies to expedite the healing process of diabetic wounds and offers a wound dressing material with immunomodulation.

Abstract:Management of diabetic wounds presents a global health challenge due to elevated levels of ROS in the wound microenvironment, persistent dysregulation of inflammation modulation, and limitations in commercially available dressings. Addressing this issue, we have developed a pH-responsive and glucose-sensitive multifunctional hydrogel dressing that dynamically responds to the wound microenvironment and enables on-demand drug release. The dressing incorporates a matrix material based on aminophenylboronic acid-functionalized alginate and polyhydroxy polymer, alongside an enhancer phase consisting of a self-assembled metal-phenol coordination nanospheres formed by tannic acid and iron ions. Through the dynamic borate ester bonds and catechol-metal ion coordination bonds, the dressing exhibits remarkable shape adaptability, self-healing capability, tissue adhesiveness, antioxidant activity, and photothermal responsiveness, without additional curatives or crosslinking agents. As a wound dressing, it elicits macrophage polarization towards an anti-inflammatory phenotype while maintaining long-lasting antimicrobial effects. In a diabetic mouse model of full-thickness wound infections, it effectively mitigated inflammation and vascular damage, significantly expediting the wound healing process with a commendable 97.7% wound closure rate. This work provides a new direction for developing multifunctional smart hydrogel dressings that can accelerate diabetic wound healing for human health.

Abstract:Management of diabetic wound has long been a clinical challenge due to pathological microenvironment of excessive inflammation, persistent hyperglycemia, and biofilm infection caused by overdue reactive oxygen species (ROS) production and defective blood vessels. Herein, a multifunctional hydrogel with ROS scavenging and photothermal antibacterial activity based on oxidized dextran (Odex), gallic acid-grafted gelatin (GAG) and Ferric ion, named OGF, was developed for treatment of infected wound in a diabetic mouse. This hydrogel was double-crosslinked by the dynamically Schiff-base bonds formed between aldehyde groups in Odex and amino groups in GAG and the metal coordination bonds formed between Ferric ion and polyphenol groups or carboxyl groups in GAG, which endowed the resulted OGF hydrogel with well injectable, self-healing and adhesive properties. Due to the high-efficiency photothermal effect of Ferric ion/polyphenol chelate, this hydrogel killed Staphylococcus aureus and Escherichia coli rapidly and completely within 3.5 min under near-infrared light radiation. Furthermore, this composed hydrogel presented good antioxidation, hemostasis and biocompatibility. It also remarkably accelerated the complete re‐epithelialization of Staphylococcus aureus ‐infected wound in diabetic mice within 18 days by eliminating infection, mitigating oxidative stress and inflammation, and facilitating angiogenesis. Therefore, the proposed multifunctional hydrogel exerts a great potential for translation in the clinical management of diabetic wounds. Statement of Significance High reactive oxygen species (ROS) levels and vascular defects in diabetic wounds can lead to excessive inflammation, persistent hyperglycemia, biofilm infection and other pathological microenvironments, which can further develop to the chronic wounds. In this study, we designed a multifunctional hydrogel with ROS-scavenging ability and photothermal antibacterial activity for the treatment of infected diabetic wound. As expected, this multifunctional hydrogel dressing highly accelerated the complete re‐epithelialization of Staphylococcus aureus ‐infected wound in diabetic mouse by eliminating infection, mitigating oxidative stress and inflammation, as well as facilitating angiogenesis. This work provides a promising therapeutic strategy for infected diabetic wound by inhibition of oxidative stress and biofilm infection.

Abstract:The proficient handling of diabetic wounds, a rising issue coinciding with the global escalation of diabetes cases, poses significant clinical difficulties. A range of biofunctional dressings have been engineered and produced to expedite the healing process of diabetic wounds. This study proposes a multifunctional hydrogel dressing for diabetic wound healing, which is composed of Polyvinyl Alcohol (PVA) and N1-(4-boronobenzyl)-N3-(4-boronophenyl)-N1, N1, N3, N3-teramethylpropane-1, 3-diaminium (TSPBA), and a dual-drug loaded Gelatin methacryloyl (GM) microgel. The GM microgel is loaded with sodium fusidate (SF) and nanoliposomes (LP) that contain metformin hydrochloride (MH). Notably, adhesive and self-healing properties the hydrogel enhance their therapeutic potential and ease of application. In vitro assessments indicate that SF-infused hydrogel can eliminate more than 98% of bacteria within 24 h and maintain a sustained release over 15 days. Additionally, MH incorporated within the hydrogel has demonstrated effective glucose level regulation for a duration exceeding 15 days. The hydrogel demonstrates a sustained ability to neutralize ROS throughout the entire healing process, predominantly by electron donation and sequestration. This multifunctional hydrogel dressing, which integrated biological functions of efficient bactericidal activity against both MSSA and MRSA strains, blood glucose modulation, and control of active oxygen levels, has successfully promoted the healing of diabetic wounds in rats in 14 days. The hydrogel dressing exhibited significant effectiveness in facilitating the healing process of diabetic wounds, highlighting its considerable promise for clinical translation.

Abstract:A wireless theranostic wound patch is designed to tackle the critical need by continuous monitoring of glucose and pH, on‐demand delivering of insulin via iontophoresis, and electrical stimulation. Moreover, integrating wearable bioelectronics with advanced multifunctional and electro‐responsive hydrogels provides a stretchable, antimicrobial, and moist environment for wounds, resulting in high efficacy in diabetic wound management. Chronic, non‐healing wounds pose significant challenges for public health, particularly in the context of diabetes, and carry significant economic consequences. This article introduces a new solution in the form of a wireless theranostic patch, developed to meet the critical need for real‐time monitoring and targeted treatments to facilitate optimal healing. The patch incorporates advanced materials that are both multifunctional and electro‐responsive, leveraging a sophisticated blend of smart hydrogels and wearable bioelectronics to support diabetic wound management with unparalleled efficacy. Abstract:Antibacterial, anti-inflammatory, and healing-promoting treatment of chronically infected wounds is not only clinical problems to be solved urgently but also important needs related to people’s livelihood. However, multifunctional hydrogel dressings with highly efficient antibacterial, anti-inflammatory, and wound microenvironment sensitive drug release performance still remain a challenge. In this work, a multifunctional hydrogel dressing was achieved by introducing mussel-like super adhesion catechol groups and glucose/ROS-sensitive dynamic phenylboronester bond cross-linking network, which endowed the hydrogel with excellent injectable, self-healing, tissue adhesion properties, and glucose/ROS-sensitive drug release performance. Additionally, high-efficiency photothermal synergistic antibacterial TP@Ag NPs were prepared and introduced into the hydrogel. The glucose/ROS responsive borate bond in the hydrogel network could realize the responsive release of TP@Ag NPs, thereby promoting M2 polarization to achieve an efficient anti-inflammatory effect. And the excess reactive oxygen (ROS) could also be scavenged by the synergistic effect of the catechol group and ROS-sensitive borate bond in the hydrogel. Moreover, excellent biocompatibility and antioxidant performance were shown in the hydrogels. Finally, the hydrogel could fully fill and adhere to irregular wound tissue through injection and significantly promote infected diabetic wound healing in vivo through efficient photothermal synergistic sterilization, ROS scavenging ability, and modulation of macrophage M2 polarization to reduce inflammatory response. Thus, it illustrated the superiority of the hydrogel as an advanced functional dressing for infected diabetic wound healing.

Abstract:Diabetic wounds are challenging to heal due to high glucose levels, and oxidative stress, impaired immune response and delayed angiogenesis. Hydrogel dressings capable of adapting and improving multiple microenvironments have broad prospects for application in clinical diabetes wound healing. In this study, we developed a photocrosslinked chitosan hydrogel using the antioxidant molecule "lipoic acid", eliminating the need for additional photoinitiators. The obtained hydrogel exhibited respectable adhesion (adhesion strengths to iron, wood and skin are 0.045 MPa, 0.049 MPa and 0.041 MPa, respectively), photo-induced self-healing, and responsiveness to pH, H 2 O 2 and glucose. Notably, as compared to conventional injectable methacrylated chitosan hydrogel, LAMC hydrogels displayed the abilities to regulate the wound environment, such as reducing oxidative stress, blood sugar levels, and pH, and creating a favorable microenvironment for wound healing. Exosome-loaded LAMC hydrogels enabled targeted exosome release based on glucose concentration, pH, and H 2 O 2 to promote angiogenesis and accelerate healing. Our study offers a novel method for preparing injectable chitosan hydrogels and introduces a new strategy for developing drug-delivery systems that effectively promote diabetic wound healing, potentially transforming current therapeutic approaches.

A Multi-Responsive Hydrogel Combined With Mild Heat Stimulation Promotes Diabetic Wound Healing by Regulating Inflammatory and Enhancing Angiogenesis

Abstract:The repair of diabetic wound still encounters huge challenges, such as disordered inflammatory regulation and impaired neovascularization. Here, a pH/ROS/glucose responsive and photothermal hydrogel is developed for diabetic wound healing. The hydrogel is formed through cross-linkage between phenylboronic acid-modified carboxymethyl chitosan (CMCS-PBA) and oxide dextran (OXD), utilizing Schiff base and phenylboronate ester bonds. Additionally, insulin-like growth factor 1 C domain (IGF-1C) and deferoxamine-loaded polydopamine nanoparticles (D@P) are incorporated into the hydrogel. The hydrogel demonstrates sustained drug release, excellent photo thermal effect, prominent antioxidant, antibacterial and anti-inflammatory activities, desirable mechanical and tissue adhesive properties, enhanced tube formation, and cell migration. Furthermore, the hydrogel combined with mild heat treatment can regulate chronic inflammation by promoting the transformation of macrophages from M1 phenotype to M2 phenotype and enhance angiogenesis by up-regulating the expression levels of angiogenesis-related factors such as hypoxia-inducible factor-1 alpha (HIF-1α), vascular endothelial growth factor (VEGF), CD31, and α-SMA, thus greatly accelerates the wound healing in streptozotocin (STZ)-induced diabetic mice. Therefore, this multi-responsive and multifunctional hydrogel holds potential as a therapeutic strategy for diabetic wounds.

Abstract:Bacterial infections and long-term inflammation cause serious secondary damage to chronic diabetic wounds and hinder the wound healing processes. Currently, multifunctional hydrogels have shown promising effects in chronic wound repair. However, traditional hydrogels only keep the wound moist and protect it from bacterial infection, and cannot provide mechanical force to contract the wound edges to achieve facilitated wound closure. Here, an asymmetric composite dressing was created by combining biaxially oriented nanofibers and hydrogel, inspired by the double-layer structure of the traditional Chinese medicinal plaster patch, for managing chronic wounds. Specifically, electrospun Poly-(lactic acid-co-trimethylene carbonate) (PLATMC) nanofibers and methacrylate gelatin (GelMa) hydrogel loaded with Epinecidin-1@chitosan (Epi-1@CS) nanoparticles are assembled as the temperature-responsive self-contracting nanofiber/hydrogel (TSNH) composite dressing. The substrate layer of PLATMC nanofibers combines topological morphology with material properties to drive wound closure through temperature-triggered contraction force. The functional layer of GelMa hydrogel is loaded with Epi-1@CS nanoparticles that combine satisfactory cytocompatibility, and antioxidant, anti-inflammatory, and antibacterial properties. Strikingly, in vivo , the TSNH dressing could regulate the diabetic wound microenvironment, thereby promoting collagen deposition, facilitating angiogenesis, and reducing the inflammatory response, which promotes the rapid healing of chronic wounds. This study highlights the potential of synergizing mechanical and biochemical signals in enhancing chronic wound treatment. Overall, this TSNH composite dressing is provided as a reliable approach to solving the long-standing problem of chronically infected wound healing.

Abstract:Diabetic wound is one of the chronic wounds that is difficult to heal, and effective treatment of it still confronts a great challenge. Monitoring the variation of diabetic wound microenvironment (such as hydrogen peroxide (H2O2)) can understand the wound state and guide the wound management. Herein, we developed a multifunctional hydrogel with the abilities of monitoring the H2O2 concentration, alleviating oxidative stress and promoting wound healing, which was prepared by encapsulating manganese‐containing bioactive glass (MnBG) and CePO4:Tb in biocompatible Gelatin methacryloyl (GelMA) hydrogel (CPT‐MnBG‐Gel). On the one hand, the H2O2‐dependent fluorescence quenching effect of the CePO4:Tb contributed to visible monitoring of the H2O2 concentration of wounds via smartphone imaging, and the CPT‐MnBG‐Gel hydrogel could effectively monitor the H2O2 level of 10.35‐200 μmol L−1. On the other hand, MnBG could alleviate oxidative stress and promote the proliferation, migration and differentiation of fibroblasts and endothelial cells in vitro owing to the bioactive Mn and Si ions, and in vivo evaluation also demonstrated that the CPT‐MnBG‐Gel hydrogels could effectively accelerate wound healing. Abstract:A sandwich‐structured photovoltaic microcurrent hydrogel dressing (PMH dressing) for chronic bacteria‐infected diabetic wounds is developed. PMH dressing generates a biomimetic current under indoor light, which is delivered to the wound through the hydrogel interface. PMH dressing significantly enhances the healing of infected diabetic wounds through improved extracellular matrix regeneration, bacterial clearance, and regulation of inflammatory responses and vascular functions. Electrical stimulation (ES) is proposed as a therapeutic solution for managing chronic wounds. However, its widespread clinical adoption is limited by the requirement of additional extracorporeal devices to power ES‐based wound dressings. In this study, a novel sandwich‐structured photovoltaic microcurrent hydrogel dressing (PMH dressing) is designed for treating diabetic wounds. This innovative dressing comprises flexible organic photovoltaic (OPV) cells, a flexible micro–electro–mechanical systems (MEMS) electrode, and a multifunctional hydrogel serving as an electrode–tissue interface. The PMH dressing is engineered to administer ES, mimicking the physiological injury current occurring naturally in wounds when exposed to light; thus, facilitating wound healing. In vitro experiments are performed to validate the PMH dressing's exceptional biocompatibility and robust antibacterial properties. In vivo experiments and proteomic analysis reveal that the proposed PMH dressing significantly accelerates the healing of infected diabetic wounds by enhancing extracellular matrix regeneration, eliminating bacteria, regulating inflammatory responses, and modulating vascular functions. Therefore, the PMH dressing is a potent, versatile, and effective solution for diabetic wound care, paving the way for advancements in wireless ES wound dressings.

Abstract:As the incidence of diabetes increases, its complication, diabetic foot ulcers, has become the main type of clinically chronic refractory wounds. Due to the hyperglycemic microenvironment of the diabetic wound, which leads to vascular defects and bacterial growth, the therapeutic effect of wound dressings lacking strategic design is relatively limited. In this study, we designed an injectable, "self-healing", and glucose-responsive multifunctional metal-organic drug-loaded hydrogel (DG@Gel) for diabetic wound healing. The functionalized hydrogel was prepared by phase-transfer-mediated metallo-nanodrugs, which were made by co-assembling zinc ions, organic ligands, and a small-molecule drug, deferoxamine mesylate (DFO), and the programmed loading of glucose oxidase (GOX). When injected into a diabetic wound, the GOX in DG@Gel changed the hyperglycemic wound microenvironment by decomposing excess glucose into hydrogen peroxide and glucuronic acid, which decreased the pH of the wound site. The low pH promoted the release of zinc ions and DFO, which exhibited synergistic antibacterial and angiogenesis activity for diabetic wound repair. In vitro experiments revealed the antibacterial activity and the cell proliferation, migration, and tube formation ability of DG@Gel. Moreover, in vivo experiments showed that DG@Gel can induce re-epithelialization, collagen deposition, and angiogenesis during wound healing in diabetic mice with good biocompatibility and biodegradability. The results suggest that this hydrogel is a promising innovative dressing for the treatment of diabetic wounds. STATEMENT OF SIGNIFICANCE: Diabetic ulcers, as one of the main types of chronic refractory wounds, are not treated effectively in the clinic due to a lack of strategic approach. Abstract:Smart hydrogel dressings capable of simultaneously highly effective antimicrobial, anti-inflammatory, and promoting re-epithelialization and angiogenesis are urgently needed for the management of diabetic wounds. Herein, a H2S-releasing multifunctional hydrogel was developed by utilizing the dynamic Schiff base reaction between carboxymethyl chitosan (CMC) and polyhexamethylene guanidine (PHMG)-modified aldehyde F108 (PFC). Diallyl trisulfide (DATS) was encapsulated into the PFC nanoparticles. Apart from possessing the essential properties necessary for an idealized hydrogel dressing, such as excellent injectability, tissue adhesion, self-healing, and stimulus–response degradation, the DATS@PFC&CMC also utilized the synergistic effect of the PHMG and DATS to provide an efficient antimicrobial effect; the H2S was slowly released from the DATS under the action of GSH and exerted excellent anti-inflammatory effects, by inhibiting the expression of p-ERK and p-STAT3 in activated macrophages, and promoting macrophage polarization to the M2 phenotype. Strikingly, following the completion of the efficient antimicrobial and anti-inflammatory effects, the continuously generated H2S further significantly accelerated the proliferation, migration, and vascularization of endothelial cells by extending the activation of the p-p38 and p-ERK1/2. Owing to superior performances, DATS@PFC&CMC significantly promoted the healing of diabetic wounds induced by streptozotocin with good biocompatibility. This study demonstrates that DATS@PFC&CMC is a versatile hydrogel dressing with great potential for the management of diabetic wounds.

Abstract:Conductive hydrogels with high antimicrobial activity, tissue adhesion, and excellent mechanical properties are designed and developed with bimetallic phenolic networks. The electrical stimulation and antimicrobial activator jointly shorten the wound healing process and can be monitored by electrical signals. This hydrogel realizes the integration of antimicrobial‐electrical stimulation‐wound monitoring, which can be beneficial for smart healthcare. The design and fabrication of novel soft bioelectronic materials for rapid wound healing and real‐time monitoring are critical for smart healthcare. However, developing such integrated multifunctional materials devices remains challenging due to fabrication dynamics and sensing interface issues. Herein, a novel strategy is presented for accelerating the kinetics of hydrogels integrating antimicrobial, electrotherapeutic, and wound monitoring functions via bimetallic phenolic networks. The Al3+ catalyzes the radical copolymerization reaction of acrylic acid, resulting in the gelation of the system within 10 s, and also catalyzes the redox reaction between silver and lignin, inducing the sustained release of catechol, which significantly enhances the hydrogel's antimicrobial activity and shortened the wound healing process. Meanwhile, the abundant non‐covalent interactions enhance the hydrogel's tissue adhesion, and mechanical properties (tensile strength 1.558 MPa and elongation 1563%). In addition, the bimetallic ions endow the hydrogels with excellent sensing properties. Under the synergy of electrical stimulation, the wound healing rate is accelerated. Notably, wound assessment can be performed by monitoring changes in electrical signals over the wound, which can assist physicians and patients in achieving intelligent wound management. This work provides new insights into the design and application of multifunctional smart bioelectronic materials.

Abstract:This study reports an electronic‐embedded, drug‐loading hydrogel bioelectronics for accelerating diabetic wound healing. The wearable device combining programmable drug release and electrostimulative are shown to effectively accelerate the healing of diabetic wounds in rat models. Its refillable and remotely programmable features support versatile modalities for diabetic wound management, promoting the development of chronic, customizable diabetic wound treatment. Diabetic wounds, characterized by complex pathogenesis and high infection rates, pose significant challenges in treatment due to prolonged recovery times and high recurrence rates, often leading to severe complications such as amputation and death. Traditional dry dressing treatments fail to address the unique microenvironment of diabetic wounds and tend to cause secondary damage due to frequent replacement. In this study, an electronic‐embedding, drug‐loading hydrogel bioelectronics is reported for accelerating diabetic wound healing using a combination of programmable pharmaceutical and electrostimulative approaches. Encapsulated in stretchable and biocompatible materials, this device is capable of multiple drug refilling and accelerated drug release modulated by on‐board electronics. In vivo experiments on diabetic model rats confirm the device's effectiveness in promoting wound healing. This innovative approach implies the potential for improving diabetic wound management using a combination of physical, material, and pharmaceutical interventions.

Abstract:The clinical treatment of chronic diabetic wounds is a long-standing thorny issue. Strategies targeting the diabetic micro-environment have been developed to promote wound healing. However, it remains challenging to reverse the adverse conditions and re-activate tissue regeneration and angiogenesis. In this work, we develop injectable hydrogels that are responsive to acidic conditions, reactive oxygen species (ROS), and high glucose levels in a diabetic wound micro-environment to sustainably deliver tannic acid (TA) to augment antibacterial, anti-inflammatory, and anti-oxidative activities. This triple-responsive mechanism is designed by introducing dynamic acylhydrazone and phenylboronic ester bonds to crosslink modified hyaluronic acid (HA) chains. At a diabetic wound, the acylhydrazone bonds may be hydrolyzed at low pH. Meanwhile, glucose may compete with TA, and ROS may oxidize the C–B bond to release TA. Thus, sustained release of TA is triggered by the diabetic micro-environment. The released TA effectively scavenges ROS and kills bacteria. In vivo experiments on diabetic mice demonstrate that the hydrogel dressing highly promotes angiogenesis and extracellular matrix (ECM) deposition, leading to eventual full healing of diabetic skin wounds. This micro-environment-triggered triple-responsive drug release provides a promising method for chronic diabetic wound healing.
