Practices and Knowledge of Female Gynecologists Regarding Contraceptive Use: a Real-World Chinese Survey.

Abstract:Background: There is no evidence of the practices of obstetricians and gynecologists (OB/GYNs) regarding contraceptive use and determinants influencing contraceptive choices, including emergency methods such as combined oral contraceptives (COCs) and levonorgestrel intrauterine system (LNGIUS). This survey determines the practices and knowledge among Chinese female OB/GYNs regarding modern contraceptive methods. Methods: A multicenter questionnaire was completed by 2000 female OB/GYNs participating in training courses organized by the gynecological endocrinology training committee of the Chinese Medical Doctor Association from February to May 2013. Results: This survey achieved a response rate of 51.4%. The rate of induced abortion among this group was 56.3%; this may be attributable to unreliable contraceptive methods (55.5%) and failure of reliable contraceptive methods (18.9%). Intrauterine devices (IUDs) were more commonly used by parous women than nulliparous women (42.6% vs 1.7%; p < 0.0001), followed by condom and rhythm method (24.2% vs 20.8%). However, nulliparous women commonly used rhythm method (48.3% vs 3.3%; p < 0.0001) and condoms (19.2% vs 10.5%; p = 0.008). OB/GYNs demonstrated misconceptions of OB/GYNs about COCs, such as the risk of breast cancer, amenorrhea and premature ovarian failure, and decreased fertility as reported by 37.10, 10.6, and 7.5% of the respondents, respectively. Conclusions: IUDs were commonly used by parous Chinese OB/GYNs. Unreliable contraceptive methods and misconceptions about the side effects of COCs may result in the high rate of unintended pregnancies. Hence, awareness of safe and effective contraceptive methods should be strengthened among OB/GYNs in China.

Abstract:Objective: To study the impact of informed choice on prevalence of induced abortion.Methods: A questionnaire survey was conducted in 10 clinics of family planning station.The respondents were married women of reproductive age.Each woman had an unexpected pregnancy due to the failure of intrauterine device(IUD) or condom or oral contraceptive(OC).Results: The main reasons for IUD failure were the IUD translocation and expulsion,accounting for 75.5%.The majority of IUD failure occurred in the health centers of township and street.Most OC and condom failures were attributed to "typical use".Conclusion: Further strengthening the propaganda and popularizing the knowledge of emergency contraception,taking the routine counseling and follow-up seriously,and improving the technical service capabilities of the grassroots are required to improve the efficiency of contraception,and protect against unexpected pregnancies,and promote reproductive health.

Abstract:Abstract Background For reasons not fully explained to date, contraception usage among women with HIV remains low. The aim of our study was to understand attitudes toward and lifetime use of contraception among women with HIV. Methods We administered an anonymous, community-informed, voluntary survey to cisgender, English-speaking women with HIV (≥18 years of age) at a Southern urban HIV clinic. It included multiple choice and Likert-scale questions on reproductive health. Participants reported contraception use, recollection of provider conversations about contraception, and perceived empowerment and knowledge regarding reproductive health. We used chi-square and Fisher exact tests to compare attitudes and prior conversations about contraception by age (< vs ≥45 years), race (Black vs non-Black), and lifetime contraception use. Results The median age of the 114 participants was 52 years, and 62% of the women identified as Black and 31% as White. Women reported a median of 2 unique family planning methods used throughout life, with oral contraceptive pills being most the common (59%). Only 20% of women reported having ever used long-acting reversible contraception (LARC). Only 56% of women recalled talking with a provider about contraception. Women of non-Black race and those who had used LARC were more likely to remember (72 vs 52%; P = .035; 87 vs 56%; P = .022; respectively). When asked about preferences, 82% of women age <45 years wanted a nondaily method, and 60% felt uncomfortable with device insertion. Conclusions Throughout life, participants reported using a diversity of contraceptives. Only half of women remembered a provider conversation about contraception. Understanding women's preferences regarding contraception should guide counseling.

Abstract:Objectives : The purpose of this study was to explore contraceptive awareness among college women in the Southeast United States. This study sheds light on the gaps in contraceptive use found in college women and can be used to improve educational interventions on college campuses. Participants : College-age women ages 18–25 completed in-depth qualitative interviews over Zoom. Methods : Interviews examined levels of awareness by asking participants to list the contraceptive methods they were aware of, followed by probes that assessed their understanding of different contraceptive methods. Results : Prominent themes ( n = 44) included high awareness of condoms, the pill and IUD, positive attitudes toward contraceptive options, fear and emotional anxiety from hormonal contraceptives and inability to explain how contraceptives work in the body. Conclusion : Results from this study will inform future contraceptive education campaigns for college women.

Abstract:Objectives: To explore levels and patterns in contraceptive use since the introduction of informed choice in reproductive health services in China since 1994, and to assess the implications of reproductive health service needs among married women of reproductive age in China.Material and methods: Data from Chinese nationwide surveys of family planning and reproductive health undertaken in 1988, 1997, 2001, and 2006 were analyzed to assess levels and trends in patterns of contraceptive use among married women by age, residence, and number of children. Contraceptive methods were classified into two categories: provider-controlled and user-dependent methods.Results: The provider-controlled pattern for contraceptive use was predominant regardless of whether women were free to choose their own contraceptives. Older, rural women, and those with more than one child preferred provider-controlled contraceptive methods; this trend has changed little after 1997. In contrast, the user-controlled methods were preferred by young, urban women, strikingly with no or only one child, and geographically in more affluent areas in north or southwest China.Conclusion: A preference for user-dependent methods is noted in the urban areas but inclination towards provider-controlled contraceptive methods is still prevalent in rural areas in China. Use of Contraceptives and Uptake of Long-Acting Reversible Contraception among Postpartum Women in Rural China

Abstract:Background Pregnancies have been increasing since the universal two-child policy in China was announced and thus the prevention of short inter-pregnancy intervals has become more important. However, little is known about contraceptive use among postpartum women in rural areas of China. This study investigated the current situation and factors associated with contraceptive use, especially long-acting reversible contraception (LARC) practice among postpartum women. Study design A cross-sectional study was conducted with a probability proportionate to size (PPS) sampling method in Shimen County of Hunan province with 423 mothers whose youngest child was aged under 2 years. A questionnaire was used to collect information on participants’ demographics, reproductive history and contraceptive use. Results The rate of using all contraceptive methods was 66.4% in the study group and the rate of using LARC was 9.9%. Statistically significant predictors of contraceptive use were: no breastfeeding, no intention of having another child, and education beyond high school. Statistically significant predictors of LARC utilisation were: vaginal delivery, at least one child before the index delivery, and no breastfeeding. Conclusions The utilisation rate of postpartum contraception, especially LARC, was relatively low in rural China. The benefits of LARC have not been realised and breastfeeding is still incorrectly believed to be a definitely reliable method of contraception. Thus, health education and contraceptive knowledge and regulations need to be updated and enhanced based on research evidence, for increased adoption of postpartum contraception in rural China.
Cognition and attitude on contraception and hormone replacement therapy among 295 female obstetricians and gynecologists

Abstract:Objective To investigate the awareness about contraception and hormone replacement therapy( HRT) among female obstetricians and gynecologists. Methods A questionnaire survey was performed in female obstetricians and gynecologists who participated in gynecologic endocrine classes in Hebei province in March 2013 on her own choice of contraception and oral contraceptives,cognition about HRT. Results 1Totally 295 doctors( 98. 3%) fulfilled the survey. The average age was( 37. 6 ± 9. 6) years. Of them,75. 8%( 222) had given birth. 2Of 295 doctors,141( 47.8%) had an abortion due to unwanted pregnancy,of which,64.7% did not take any contraceptive measures. 3Currently,52. 6% chose intrauterine devices( IUD),20. 5% used condoms plus rhythm method,3. 8% took oral contraceptives,and 1. 3% used levonorgestrel releasing intrauterine system( LNG- IUS). 4About the cognition of compound oral contraceptives( COC),76. 8% doctors thought that it could treat dysmenorrhea and premenstrual tension syndrome,62. 9% agreed that it could improve the sebum secretion,polycystic ovary syndrome and reduce menstrual quantity and improve menorrhea,72. 8% worried about amenorrhea and ovarian failure after using a long time,and 52. 6% thought it should not be used 3- 6 months before pregnancy.5About 93.9% doctors were sure that HRT can relieve and treat menopausal syndrome,87.1% thought that it can prevent and treat osteoporosis,but up to 60. 9% worried about the risk of breast cancer. Conclusion 1The ratio of unwanted pregnancy and abortion is high among female obstetricians and gynecologists. 2The utilization rate of compound oral contraceptive among female obstetricians and gynecologist is low because of some misunderstanding on the side effects. 3The recognition of hormone replacement therapy is high but they fear about side effects meantime.
A revisit of knowledge, attitude and practice of emergency contraception in Hong Kong

Abstract:Objectives: To revisit women's knowledge, attitudes and practices (KAP) regarding emergency contraception (EC) in Hong Kong. The research sought to provide insights for improving the accessibility and quality of EC services. Study design: A cross-sectional study, where self-administered questionnaires were distributed at a major community sexual and reproductive health service in Hong Kong over a 3-month period between July 2023 and October 2023. Descriptive and regression analyses were used. Results: Of 1127 respondents, the majority (n=1057, 93.8%) reported using contraception and 513 (45.6%) had used EC. The main contraceptive methods used were male condoms and oral contraceptive pills. The majority (n=1035, 91.8%) of respondents demonstrated awareness about EC and 938 (83.2%) participants correctly reported the timeframe for oral EC. Around two-thirds reported the internet as being their leading source of EC knowledge. Over 93% of respondents advocated for enhancing public awareness. Acceptance of non-traditional means of obtaining EC, such as over-the-counter provision (51.3%), pharmacy provision (49.8%) and telemedicine consultation (43.1%), were higher than face-to-face EC consultations (32.9%). Conclusions: Family planning service users' characteristics and KAP regarding EC have significantly changed over the last 20 years. Women now demonstrate greater awareness, knowledge and openness regarding EC, indicating improved readiness for more liberal delivery of EC nowadays. This study highlights the need for restructuring EC service provision in Hong Kong to address women's changing preferences and contraceptive needs, and to minimise barriers to EC access. We recommend reclassifying emergency contraceptive pills as non-prescription drugs in Hong Kong to align with international practice.

Abstract:The use of contraceptive methods is still a controversial subject in the societies of developing countries. This study aims to assess the knowledge, attitudes, and practices of contraceptive methods among students, aged 15 to 25 years, in public and private schools in the district of Bonaberi, Douala, Cameroon. This research was modeled on the pattern of descriptive study. The population in this study was composed of students ( N = 1649) from the five public and private high schools of Bonaberi, Cameroon. The sample size of this study was 379 students. These data were collected in person by the researcher between 1 and 30, June 2022. The data collection tools are the student information form, the Knowledge and Practices of Contraceptive Methods Questionnaire, and the Contraceptive Attitude Scale (CAS). The data of this study were analyzed using descriptive statistics tests, Kruskal–Wallis Test, Pearson Chi-Square, Kolmogorov-Smirnoff, Student's T-Test, Mann–Whitney Test, and the One-way ANOVA. It was found that almost half (45.91 percent) of students were 19–22 years old, 59.1 percent of them were male, and almost half (49.3 percent) of them were sexually active. Currently, the male condom is the most commonly used and known method of contraception among students. It was determined that students take an average of 3.406 points from the total score of CAS. This result indicated a positive attitude toward contraceptive methods. There were no statistically significant differences in contraceptive attitudes according to gender, religion, or sexual activity. There was a statistically significant difference in contraceptive attitudes according to marital status and age. It is recommended to provide family planning counseling services in these schools in order to increase the use of effective methods and provide students with a reliable source of information on contraceptive methods.

Gap in knowledge of health benefits and risks of combined oral contraceptives among Lebanese women

Abstract:Abstract Background Oral Contraceptive Pills (OCPs) are among the most commonly used forms of contraception, but they are associated with several health benefits and risks. This study aims to determine the gap in knowledge of the underlying health benefits and risks of OCPs among Lebanese women and to identify the factors that might influence their beliefs. Methods A questionnaire was completed by 817 Lebanese women aged 18–64 years old and assessed sociodemographic details, medical information, contraceptive practices, knowledge of underlying health benefits and risks, and information needs related to OCPs. Results Among the total participants, 41.5% of women reported using OCPs at some point in their lives yet 46.6% denied receiving information about their benefits and 48% denied receiving information about their risks. The mean total OCP knowledge score was 5.70 out of 25, the mean OCP risk knowledge score was 4.09 out of 15, and the mean OCP benefit knowledge score was 0.77 out of 6. Sociodemographic factors associated with greater total knowledge, risk knowledge and benefit knowledge included OCP usage, being a student, confidence in one’s knowledge and satisfaction with one’s information. Both the total and risk knowledge scores were found to be higher in women who found that receiving information related to OCPs was important. Finally, participants who lived in central governates had greater total knowledge scores, whereas those with higher levels of education and a family history of endometrial cancer demonstrated better benefit knowledge. Conclusions This study highlighted the poor knowledge of health benefits and risks associated with OCP use among Lebanese women and the associated sociodemographic factors that might influence their beliefs.

Pattern of contraceptives choice among the married women attending the family planning clinic of a tertiary health institution

Abstract:Background: Rampart population growth is the most fundamental problem of our times. It affects adversely the advancement of nations and the wellbeing of all peoples. The fertility rate in Nigeria remains high at a national average of about 5.2 children per woman. When a woman effectively uses a modern method of contraceptive she is less likely to be exposed to the hazards of grand multiparity and also unlikely to resort to dangerous illegal abortion. Method: This is a retrospective study, a review of the records of the family planning clinic of Ladoke Akintola University of Technology Teaching Hospital, Osogbo was undertaken. These were new clients coming for the first time to the family planning clinic of the institution from January, 2001 to December 2006. Result: A total of 1355 married women were coming for the first time to the family planning clinic within the study period, and their age range between 18 and 51 years with a mean of 33.5+/-6.1 years. 170 (12.5%) were Para 1, Para 2-4 were 855 (63.1%), while Para 5 and above were 330 (24.4%). The clients source of information about family planning was: family planning clinic personnel 1039 (76.7%), media (print and electronic) 152 (11.2%), friends and relatives 127 (9.4%), and community health workers 37 (2.7%). Intrauterine contraceptive device was the most chosen method by the clients 1011 (74.6%), while condom was the least chosen 3 (0.2%); 264 (19.5%), and 77 (5.7%) clients chose injectables and pills, respectively. Conclusion: An increase in the level of knowledge on modern contraceptive methods through mass and interpersonal communications could be one of the key strategies to increase the utilization rate of modern contraceptive methods.
The current status of gender equity in medicine in Korea: an online survey about perceived gender discrimination

Abstract:Abstract Background Although the number of women doctors has increased in South Korea, and efforts to improve gender awareness have gained importance in recent years, the issue of gender equity in the medical field has not been fully evaluated. The aim of this study was to determine the current status of gender equity in the medical profession in Korea. Methods An online survey on perceived gender discrimination was conducted for 2 months, with both men and women doctors participating. The results were analyzed using descriptive statistics. Results A total of 1170 doctors responded to the survey (9.2% response rate). The survey found that 47.3% of the women respondents and 18.2% of the men had experienced gender discrimination in the resident selection process ( P < 0.05), 17.2% of the women and 8.7% of the men had experienced discrimination during the fellowship application process ( P < 0.05), and 36.2% of the women and 8.0% of the men had experienced discrimination during the professorship application process ( P < 0.05). Both men and women cited the issue of childbirth and parenting as the number one cause of gender discrimination against women doctors. Conclusions This study revealed the presence of perceived gender discrimination in the Korean medical society. To address discrimination, a basic approach is necessary to change the working environment so that it is flexible for women doctors, and to change the current culture where the burden of family care, including pregnancy, childbirth, and childcare, is the primary responsibility of women.

Abstract:Objectives: To examine the contraceptive preferences of women based on their age and to track changes in these preferences over time. Methods: This was an observational, retrospective cross-sectional study. Data from 2742 patients were evaluated for this study. The patient group of the first 4 years was classified as Group I (n=1371) and the last 4 years was classified as Group II (n=1371). The preferred contraception methods between the groups and their changes across the years were examined. Results: Coitus interruptus was found to be the prevailing contraceptive method across all age groups, with 304 (70.9%) in 18-24 age group; 1314 (65.4%) in 25-40 age group; and 148 (48.8%) in 41-53 age group using this method. Breastfeeding rates were also analyzed across age groups, revealing that 11% of patients aged 18-24 years, 5% of patients aged 25-40 years, and 1.7% of patients aged 41-53 years were breastfeeding. Among these patients, 114 (74.5%) were not using any contraception method, while coitus interruptus remained the most popular choice. Conclusion: It was determined that there are many couples who do not have sufficient knowledge regarding family planning and birth control in our country. At the same time, the use of birth control methods has increased due to the increase in the education level of women and easier access to sexual health services.

Abstract:Objective(s) While previous literature has shown clinician bias in adult contraceptive counseling, less is known on the biases clinicians may exhibit when counseling adolescents about contraception. Our study aimed to describe long-acting reversible contraception (LARC) counseling and prescribing practices of adolescent-serving clinicians. Study Design This study used a cross-sectional discrete choice experiment mixed methods design. We sent a survey containing vignettes and items pertaining to demographics and beliefs to a convenience sample of adolescent-serving clinicians across the United States. Results Of 296 clinicians, 80% were in pediatrics, and had geographic, practice setting, gender, and racial diversity. Most clinicians reported being up-to-date with current literature regarding contraception. Sixty-eight percent of respondent's practices administer contraceptive injections, but only 17% place IUDs. Of those who do insert IUDs, nearly half inserted five or fewer within the last year. Patients' younger age and Hispanic ethnicity were associated with lower odds, and history of pregnancy was associated with higher odds, of clinicians' recommending LARC. Across all vignettes, the top five reasons clinicians chose their first-choice method for the patient were adherence or compliance, efficacy, side effects, patient age, and reversibility. Conclusion(s) Clinicians often recommend contraceptives based on adherence, efficacy, and age, and we found younger age, minoritized race or ethnicity, and history of pregnancy were all associated with LARC recommendations, indicating potential biases against teen parents and assumptions about adolescents' priorities. This may impede the provision of patient-centered contraceptive counseling for adolescents, and highlights the need for improved education and practice changes post-Dobbs.

Abstract:Background: Contraceptive methods play a vital role in family planning and has been established a cornerstone in reducing maternal and infant mortality rate. Family planning is encouraged as a tool to address the reproductive health needs of men and women, as well as the pivotal problem in regard to rapid population increase. The total fertility rate in Uttarakhand is 2.1 children per woman with couple protection rate of 53%, according to National Family Health Services. Aims and Objectives: We aimed to assess the knowledge regarding contraceptives and its usage among married women of reproductive age group in urban area. Materials and Methods: The study was a cross-sectional study using a quantitative survey questionnaire of duration 5 months. Women in reproductive age-group (15–45 years) were included in the study. A total of 102 study participants were enrolled. Results: Mean ± standard deviation of study participants was 29.09 ± 5.04. "Knowledge" questions had over 90% of correct responses. The attitude of study participants toward contraceptive usage was mostly favorable. Contraception usage practice was fairly good. Conclusion: There is a need to promote positive attitude among the general public so that they can create an environment to raise awareness among the general public regarding contraception.
Contraceptive Practices of Women Visiting a Gynecology Clinic in Beijing, China

Survey on Knowledge and Attitude to Birth Control Options in Rural-to-urban Migrant Women in Shanghai

Abstract:Objective To understand the knowledge and attitude to birth control options in rural-to-urban migrant women(non-residents) in Shanghai for improving efficiency of health education on birth control to the target population.Methods 430 rural-to-urban migrant women at reproductive age visiting the gynecology department of the Bansongyuan Center of Community Health Services,Huangpu District,Shanghai,were recruited during the period from July 2007 to December 2008.A structured questionnaire was used to collect information on knowledge and attitude on birth control options which were assessed by a scoring system.Dichotomous Logistic regression was applied in data analysis.Results The average score of knowledge was 7.17±2.65(range 0～12) in the 430 participants.Knowledge of IUD and condoms was at high level,while knowledge of other nonuse contraceptive methods was at lower awareness rate.84.7% were willing to have further knowledge,those who want to know how do I choose the contraceptive method was the most,accounted for 74.5%.In the results of multivariate analysis,the scores of knowledge were higher in women aged 25～ and having at least junior high school education than others.The scores were also higher in women emigrated from urban areas,living without their husbands,and those with more than two abortions than others(P0.05).Conclusions The knowledge on birth control options was not comprehensive in rural-to-urban migrant women in Shanghai.The results suggested that health education services should be tailored for them with different demographics and socioeconomic status.

Abstract:Objectives The study aimed to investigate the knowledge, attitude and practice (KAP) of contraceptive methods among women with an unplanned pregnancy, aiming to improve their reproductive health and increase their understanding of contraceptive methods. Design This is a cross-sectional study. Setting The study was conducted at the Maternity and Child Healthcare Hospital of Hubei between 20 November 2022 and 20 January 2023. Participants Women with an unplanned pregnancy were included. Primary and secondary outcome measures The questionnaire was in the Chinese language and included demographic data, KAP assessments. Multivariate linear regression was performed to explore the factors associated with knowledge or practice scores. Results During the study period, 510 participants with valid questionnaires were included. The KAP scores were 7.30±2.91, 32.61±3.13 and 28.58±3.59, respectively. Place of residence (urban vs non-urban; B=0.66, 95% CI 0.02 to 1.29, p=0.043) and educational level (master's degree or above vs post secondary or below; B=1.07, 95% CI 0.17 to 1.96, p=0.020) were positively associated with knowledge. Knowledge (B=0.25, 95% CI 0.17 to 0.32, p<0.001) and attitude (B=0.26, 95% CI 0.19 to 0.32, p<0.001) were positively associated with practice. Conclusions This study indicates a low level of KAP regarding contraceptive methods among women facing unplanned pregnancies. Place of residence and educational level were positively associated with knowledge scores. These findings may help improve future sex education policies and programmes.
