Abstract:Significant decrease of congestion in portal vein and shortened blocking time were obtained because of the application of veno-venous bypass in vitro during complete vascular clamping. This new procedure, with such advantages as simple vessel processing, quality anastomosis, less postoperative hemorrhage and higher success rate, etc, effectively prevents ischemical reperfusion injury of the host liver and deserves to be spread.
Observations on Hemodynamic Changes and Graft Function in Pig Model for Auxiliary Partial Orthotopic Liver Transplantation

Abstract:Objective: To observe the intraoperative hemodynamic changes and graft function in a new pig model of auxiliary partial orthotopic liver transplantation. Methods: Sixteen healthy male or female domestic pigs, weighing 22 to 27 kg, were randomly selected to be either a donor (n=8) or a recipient(n=8). After intubation, anesthesia was maintained by pentothal sodium. The left lobe of the donor liver was resected in vitro, the right lobe was used as a graft. Donor suprahepatic inferior vena cava was anastomosed end-to-side to host suprahepatic inferior vena cava, donor portal vein end-to-side to host portal vein, donor hepatic artery end-to-end to host spleen artery. The common bile duct of the donor was intubated and bile was collected with an extracorporeal bag. At the end of operation, the donor's infrahepatic inferior vena cava was ligated. Cold ischemia time averaged about (58±4.0) min. Immunosuppressants were not administered and antibiotics were administered for three days after operation. During transplantation, arterial blood pressure, central venous pressure were monitored. BB, SB and pH of arterial blood samples were determined and analysed in order to assess the impact of operation on the animals. Color ultrasound examinations were performed 5 days after operation in some recipients and autopsy was made on all the recipients. Wedge biopsy specimens of every pig was obtained and stained with hematoxylin-eosin. Results: Seven of the 8 recipients (87.5%) survived for five days, one recipient did not recover from anesthesia, probably due to hyperkalemia and severe acidosis. The hemodynamis recovered rapidly and stabilized after graft reperfusion. Color ultrasound examination showed an increase of blood flow of graft on the 5th day compared to the 1st day after operation. When the animals were sacrificed on the 5th day after operation, thrombosis of HV and PV were not found; histopathological examination of liver specimens revealed evidence of damage with mild steatosis and sporadic necrotic foci and focal hepatic lobules degeneration in the graft,especially around the pericentral venous area. Infiltration by inflammatory cells was mild in the portal or central veinous area. Conclusions: The new pig model was successfully tried in our laboratory. The new procedure showed the advantages of easy anastomoses, stable intraoperative hemodynamics, less complications and high survival rate.

Abstract:AIM: To establish a new pig model for auxiliary partial orthotopic liver transplantation (APOLT).METHODS: The liver of the donor was removed from its body. The left lobe of the liver was resected in vivo and the right lobe was used as a graft. After the left lateral lobe of the recipient was resected, end-to-side anastomoses of suprahepatic inferior vena cava and portal vein were performed between the donor and recipient livers, respectively. End-to-end anastomoses were made between hepatic artery of graft and splenic artery of the host. Outside drainage was placed in donor common bile duct.RESULTS: Models of APOLT were established in 5 pigs with a success rate of 80%. Color ultrasound examination showed an increase of blood flow of graft on 5(th) d compared to the first day after operation. When animals were killed on the 5th d after operation, thrombosis of hepatic vein (HV) and portal vein (PV) were not found. Histopathological examination of liver samples revealed evidence of damage with mild steatosis and sporadic necrotic hepatocytes and focal hepatic lobules structure disorganized in graft. Infiltration of inflammatory cells was mild in portal or central vein area. Hematologic laboratory values and blood chemical findings revealed that compared with group A (before transplantation), mean arterial pressure (MAP), central venous pressure (CVP), buffer base (BB), standard bicarbonate (SB) and K(+) in group B (after portal vein was clamped) decreased (P<0.01). After reperfusion of the graft, MAP, CVP and K(+) restored gradually.CONCLUSION: Significant decrease of congestion in portal vein and shortened blocking time were obtained because of the application of in vitro veno-venous bypass during complete vascular clamping. An Experimental Study on the Venous Drainage of Mini Tissue Flaps

Abstract:Objective To set up an animal model of mini tissue flaps and explore the mechanism of retrograde vein drainage.Methods A mini tissue flap based on the saphenous artery(flap size:1.0 cm×1. 0 cm)was created on both hind limbs of 30 Sprague Dawley rats.The left and right hind limbs of each rat were randomly assigned into group 1 and 2.In group 1,the flaps were elevated and the saphenous artery and its accompanying veins were kept in situ.In group 2,the flaps were elevated and only the saphenons artery were kept in situ,while its accompanying veins and other superficial veins were ligated.Survival of the flaps,their appearance and consistency were observed.Reconfiguration of venous drainage of the flaps was evaluated by angiography.Results Flaps of group 1 had normal skin color at postoperative day 1.Only 3 flaps in this group showed mild swelling which subsided at pestoperative day 2.All flaps had normal consistency.All the flaps of group 2 showed mild cyanosis and obvious swelling at postoperative day 1.There was tension of the flaps. Swelling of the flaps decreased and color of the flaps turned normal at postoperative day 2.Tension of the flaps also reduced.For swelling of the flaps,the difference between the two groups were statistically significant in the first and second days postoperatively(P0.01),but insignificant in the 3,7,9 and the 14 days postoperatively. For flap survival,in group 1 all flaps survived with normal skin color,good appearance and elasticity except one flap presented about 0.1 cm marginal skin necrosis.In group 2,there were 2 flaps with about 0.1 em marginal skin necrosis and 3 flaps with complete necrosis.The survived flaps had normal skin color and no swelling,but their appearance and elasticity were slightly worse than that of the flaps in group 1.Conclusion The survival mechanism of mini tissue flaps is different from that of conventional flaps.Restoration of venous drainage relies on collateral anastomosis between the flap and its tissue bed and surrounding margins in the first 3 postoperative days.When arterial flow is patent,mini tissue flaps still have a high survival rate even if the venous flow is obstructed due to a failed vein anastomosis.However a good venous anastomosis is required to ensure good flap appearance and consistency.
Orthotopic Abdominal Multivisceral Transplantation Without Venovenous Bypass in Pigs

Abstract:Background. Because venovenous bypass (VVB) can cause specific complications, a simplified orthotopic abdominal multivisceral transplantation (MVTX) technique without VVB in pigs has been evaluated in terms of morbidity and mortality. Materials and methods. Outbred large-white pigs weighing 25 to 40 kg of random sex underwent MVTX operation. After in situ cold perfusion through the aorta and superior mesenteric vein, the multivisceral allograft was procured from the donor and tailored at the back table. The multivisceral allograft, including liver, pancreas, stomach, duodenum, and proximal 2 m of jejunum, was en bloc transplanted into recipient after resection of entire foregut and midgut; VVB was not used. We analyzed the hemodynamic change, arterial blood gas data, and fluid requirements intraoperatively. Results. Among 25 MVTXs, 19 (76%) animals survived after the operation. Without using an immunosuppressant, postoperative survival time was 2 to 146 hours. Ten recipient pigs died within 24 hours. Seven animals were lost between postoperative days 2 and 5. Two pigs survived for more than 5 days. The recipient pigs were mostly in a state of hypovolemic shock and metabolic acidosis during the reperfusion phase. Conclusions. Despite a high morbidity and mortality, the simplified technique without VVB is feasible to successfully achieve MVTX in the pig.
[Preliminary Experimental Study on Treatment of Portal Hypertension with Auxiliary Partial Orthotopic Liver Transplantation].

Abstract:OBJECTIVE:To evaluate the therapeutic efficacy of auxiliary partial orthotopic liver transplantation (APOLT) on portal hypertension in liver cirrhosis with amelioration of portal vein congestion, changes in portal vein pressure and status of the graft.METHODS:The recipients were porcine model of biliary cirrhosis reproduced by ligation of the common bile duct. During transplantation, arterial blood pressure, central venous pressure were recorded. Buffer base, standard bicarbonate and pH of arterial blood samples were determined and analyzed in order to assess the impact of operation on the animals. The hemodynamics were also monitored. Color Doppler ultrasonographic examination was performed on recipients before operation, intra-operation and 7 days after operation, respectively. Portal vein pressure, blood bilirubin and variables of liver function were measured by using an autoanalyzer. Wedge biopsy specimens of each pig were obtained, stained with hematoxylin-eosin, and examined. Analysis of variance was performed. Otherwise non-parametric tests were used.RESULTS:Eight weeks after ligation of the common bile duct, biliary cirrhosis was reproduced in all the pigs, and histopathological examination of the liver specimen showed a large number of pseudo-lobules. In 6 pigs with hepatic cirrhosis liver transplantation was done. Five of the 6 (83.3%) animals survived for 7 days. One recipient died because of unsuccessful operation, the others showed stable hemodynamics during the operation. Seven days after transplantation, the blood flow of the two liver portal veins was observed by the use of color Doppler ultrasonography. It was found that the blood flow in the donor portal vein was much richer than that of native portal vein. The venous outflow of the graft was ample and smooth, and no thrombosis of the hepatic vein or portal vein was found. The variables, including alanine aminotransferase, aspartate aminotransferase, bilirubin and total bilirubin, were significantly improved 7 days after operation compared with pre-operative data. Portal pressure was found to be (20.76+/-2.42)cm H(2)O(1 cm H(2)O=0.098 kPa), (17.62+/-2.33)cm H(2)O and (14.72+/-2.25)cm H(2)O before the operation, during the operation, and 7 days after operation, respectively, and the difference was statistically significant(P<0.01). On the 7 days after transplantation, histopathological examination revealed evidence of damage with mild steatosis and sporadic necrotic hepatocytes and focal hepatic lobular degeneration in the graft, especially in the area around the central vein.CONCLUSION:APOLT is a hopeful option for the treatment of portal hypertension. This procedure can provide not only some improvement of the liver function but also decrease the pressure of portal vein.
Establishment of Splenic Fossa Auxiliary Partial Heterotopic Living Donor Liver Transplantation Models in Miniature Pigs

Abstract:Objective This study was performed to develop a clinically relevant porcine model of auxiliary partial heterotopic living donor liver transplantation in splenic fossa.Methods Twenty female Guangxi Bama miniature pigs with similar body weight were randomly divided into donor group (n=10) and recipient group (n=10).The left hemi hepatic graft without middle hepatic vein was procured from the living donor under the general anesthesia.The liver graft was tailored at the back table;an 8-mm diameter polytetrafluoroethylene graft about 2.5 cm long was connected to the left hepatic vein.The liver graft was implanted in the splenic fossa after splenectomy and left nephrectomy,with a hepatorenal venous anastomosis (via artificial graft),splenpotal venous anastomosis,and splenohepatic arterial anastomosis in end-to-end fashion,respectively.A polyethylene tube was inserted into the left bile duct branch of the graft and led through the body wall as an external biliary drain.No immunosuppressive drugs were given in this series.Survival rate and complications were observed with 2 weeks after transplantation.Results Ten pig models of auxiliary partial heterotopic living donor liver transplantation were successfully established.Two-week survival rates were 70% and 60% for donors and recipients,respectively.But only three recipients had well-functioning graft livers.Conclusion Despite a high morbidity rates including hepatic artery thrombosis,portal vein thrombosis and artificial graft mural thrombus formation,it is feasible to use left hemi hepatic graft without middle hepatic vein from living donor and splenic fossa auxiliary partial heterotopic liver transplantation in miniature pig.
A Comparison Between Splenic Fossa and Subhepatic Fossa Auxiliary Partial Heterotopic Liver Transplantation in a Porcine Model

Abstract:To test the alternative possible locations for the placement of a liver graft and the relevant surgical technique issues, we developed a porcine model of auxiliary partial heterotopic liver transplantation (APHLT) and evaluated the difference between 2 styles of liver transplantation, either subhepatic fossa or splenic fossa APHLT, by comparing survival and biochemical indexes. Thirty-eight miniature pigs were randomly divided into 2 groups. A left hemihepatic graft without the middle hepatic vein (HV) was procured from the living donor. In group A (n=9), an 8 mm diameter polytetrafluoroethylene (PTFE) graft approximately 2.5 cm long was connected to the left HV while another PTFE graft of the same size was connected to the left portal vein (PV). The liver graft was implanted in the right subhepatic fossa following splenectomy and right nephrectomy. In group B (n=10), a PTFE graft of the same size was connected to the left HV while the liver graft was implanted in the splenic fossa following splenectomy and left nephrectomy. Survival rate and complications were observed at 2 weeks after transplantation. Data were collected from 5 animals in group A and 6 animals in group B that survived longer than 2 weeks. The liver function and renal function of the recipients returned to normal at 1 week after surgery in both groups. Eighty-eight percent (14/16) of the PTFE grafts remained patent at 2 weeks after surgery, but 44% of the PTFE grafts (7/16) developed mural thrombus. No significant differences in the survival rate and biochemistry were found between the 2 groups. In conclusion, the splenic fossa APHLT can achieve beneficial outcomes similar to the subhepatic fossa APHLT in miniature pigs, although it also has a high morbidity rate due to hepatic artery thrombosis, PV thrombosis, and PTEF graft mural thrombus formation. Venovenous bypass in liver transplantation: Exploring the benefits, efficacy, and safety

Abstract:Venovenous bypass (VVB) is a technique used in liver transplantation (LT) to maintain hemodynamic stability and abdominal organ perfusion and thereby improve patient outcomes. Despite its perceived benefits, VVB utilization has declined globally due to concerns related to heparinization, major bleeding and the need for expertise. Recent advancements, such as percutaneous cannulation techniques and improved extracorporeal technology have improved the safety of VVB in LT. This paper presents a modified VVB circuit with enhanced safety features. Cannulation plays a pivotal role in VVB establishment, with percutaneous methods increasingly favored. Studies demonstrate VVB's efficacy in improving patient outcomes with lower incidence of acute kidney injury and reduced operative time and blood loss, with no added morbidity or mortality. However, its routine use faces challenges, with alternative techniques gaining traction. Our experience highlights VVB's role in various clinical scenarios, including patients with high Model for End-Stage Liver Disease (MELD) scores, challenging surgical anatomy, portal vein thrombosis and pre-existing cardiovascular disease, emphasizing its safety and efficacy. Continued research is needed to optimize VVB techniques and ensure better outcomes for liver transplant recipients.
Magnetic-assisted laparoscopic liver transplantation in swine

Abstract:BACKGROUND: Although laparoscopic technology has achieved rapid development in the surgical field, it has not been applied to liver transplantation, primarily because of difficulties associated with laparoscopic vascular anastomosis. In this study, we introduced a new magnetic-assisted vascular anastomosis technique and explored its application in laparoscopic liver transplantation in pigs.METHODS: Two sets of magnetic vascular anastomosis rings (MVARs) with different diameters were developed. One set was used for anastomosis of the suprahepatic vena cava (SHVC) and the other set was used for anastomosis of the infrahepatic vena cava (IHVC) and portal vein (PV). Six laparoscopic orthotopic liver transplantations were performed in pigs. Donor liver was obtained via open surgery. Hepatectomy was performed in the recipients through laparoscopic surgery. Anastomosis of the SHVC was performed using hand-assisted magnetic anastomosis, and the anastomosis of the IHVC and PV was performed by magnetic anastomosis with or without hand assistance.RESULTS: Liver transplants were successfully performed in five of the six cases. Postoperative ultrasonographic examination showed that the portal inflow was smooth. However, PV bending and blood flow obstruction occurred in one case because the MVARs were attached to each other. The durations of loading of MVAR in the laparoscope group and manual assistance group for IHVC and PV were 13 ± 5 vs. 5 ± 1 min (P < 0.01) and 10 ± 2 vs. 4 ± 1 min (P < 0.05), respectively. The durations of MVAR anastomosis in the laparoscope group and manual assistance group for IHVC and PV were 5 ± 1 vs. 1 ± 1 min (P < 0.01), and 5 ± 1 vs. 1 ± 1 min (P < 0.01), respectively. The anhepatic phase was 43 ± 4 min in the laparoscope group and 23 ± 2 min in the manual assistance group (P < 0.01).CONCLUSIONS: Our study showed that magnetic-assisted laparoscopic liver transplantation can be successfully carried out in pigs.

Abstract:Magnetic anastomosis substantially shortens the duration of vascular anastomosis. We aimed to apply magnetic anastomosis technology (MAT) to donor liver implantations in pig orthotopic liver transplantation (OLT). Twenty healthy adult pigs were randomly divided into donors and recipients, and major vascular anastomosis was performed using MAT during OLT. Recipient liver and kidney function was measured pre-surgery and 12, 24 and 72 h post-surgery. Vascular anastomoses examinations were performed using ultrasound or angiography weekly post-surgery, and pathological examinations of vascular anastomoses were performed during autopsy after animal euthanasia. All recipients survived 24 h after surgery, which is considered as successful transplantation. Anhepatic duration was only 13 min, and no anastomotic obstruction or stenosis, magnetic displacement and anastomotic angulation, or distortion was found upon postoperative examinations of major liver vasculature. Aspartate aminotransferase, alanine aminotransferase, and total bilirubin serum levels increased considerably postoperatively. The follow-up period for this study was 1 year, and the median survival time of all recipients was 115 d (interquartile range = 11-180 d). The main causes of death were liver failure, immune rejection, infection, and arterial anastomotic bleeding. Moreover, vascular anastomoses healed well with a survival time of more than two weeks. We developed a novel magnetic device to create a fast and safe technique to perform major vascular anastomoses in pig liver transplantations. Additionally, the liver graft implantation using MAT considerably shortened the recipient warm ischemia time, which will reduce the extent of ischemia-reperfusion injury. We conclude that MAT is an effective method for donor liver fast implantation in OLT in pigs.
A hemodynamic and hemolysis study of the axial pump with hydrodynamic-magnetically levitated impeller in swine

Abstract:Objective To explore the in-vivo hemodynamic and hemolysis effect of a newly designed axial continuousflow ventricular assist device(VAD) in swine.Methods Under general anesthesia,each of 5 swine [weight (40.0 ± 5.2)kg] was implanted with the axial continuous-flow VAD into the apex of left heart ventricle,and the outflow graft was anastomosised to descending aorta.Results All of the axial continuous-flow VAD were implanted successfully with post-operative survival rate 100％.All 5 animals survived over one week.There was a positive correlation between pump speed and assistance effect.The mean left ventricular systolic pressure was (131.6 ± 28.0) mmHg(1 mmHg =0.133 kPa).While the axial continuous-flow VAD was working,left ventricular end diastolic pressure decreased,along with mean intraventricular pressure declined.Peripheral hemodynamics was stable and peripheral blood pressure was not remarkably different from the pressure preoperation.Daily urine volume was in normal range within 1 week post operation.Free hemoglobin in plasma was slightly elevated on the surgery day,and gradually dropped to normal level within 1 week.International Normalized Ratio(INR) was maintained between 2.0-2.5 with oral adminiatration of warfarin of 3 mg/day.There was no thrombosis existing in VAD at autopsy.Conclusion The application of the axial pump with hydrodynamic-magnetically levitated impeller in animal experiment can provide stable hemodynamics,advanced heart unloaded effect,favorable peripheral perfusion,and blood compatibility is satisfactory.
Comparison of pig small-for-size liver transplantation models established by in vivo and in vitro methods

Abstract:BACKGROUND: The liver structure of pig was simple and similar to that of human,which is one of the best models for liver transplantation. However,the smaller liver volume is an obstacle for the clinical application of in vivo liver transplantation,accordingly,it has significance to create an animal model with stability and high efficiency. OBJECTIVE: To explored a simple and effective method in establishment of pig small-for-size liver transplantation models by comparative analysis of in vivo and in vitro methods in obtaining grafts. DESIGN,TIME AND SETTING: The randomized control experiment on animals was performed in the Shanghai Tyco's Medical Training Center from January to March in 2007. MATERIALS: Forty-two adult,healthy,Guangxi Bama miniature pigs were prepared for 21 pig small-for-size liver transplantation models. METHODS: The Guangxi Bama miniature pigs were selected as the donors and recipients,and divided into the in vivo (n=20) and in vitro (n=22) groups by random number table. The small-for-size grafts were harvested by resection of some liver lobes in vivo or in vitro by using cavitron ultrasound surgical aspirator (CUSA) plus bipolar coagulation forceps. MAIN OUTCOME MEASURES: The technical complications included bleeding volume during the operation,postoperative bile leakage and others technical data such as anhepatic phase,cold ischemia time and total operation time were recorded. RESULTS: The incidence of postoperative bile leakage in the in vitro group was higher than that of the in vivo group (50%,20%,P﹤ 0.05).The bleeding volume in the in vivo group was obvious greater than the in vivo group (P < 005).No obvious difference was found including anhepatic phase,cold ischemia and total operation time between the two groups (P > 0.05). CONCLUSION: Removing some lobes of the grafts with CUSA in vivo to obtain a small-for-size graft is an effective and practical method for establishment of small-for-size liver transplantation model in pigs.
The effects of venovenous bypass use in liver transplantation with piggyback technique: a propensity score-weighted analysis

Abstract:Venovenous bypass (VVB) use during liver transplantation (LT) is notably variable among the centres and it is actually restricted to surgically complex cases, severely unstable recipients or grafts from high-risk donors. Historically, VVB was associated with the classical LT with caval cross clamping, while not much is known about the safety of this technique applied to piggyback LT. This retrospective observational study evaluated the effects of VVB applied to piggyback LT on mortality, hospital outcomes, postoperative graft and other organ dysfunction. We retrospectively collected data about recipient status, surgical complexity and graft quality of all the piggyback LTs performed at the Transplant Unit of IRCCS Azienda Ospedaliero-Universitaria di Bologna, Italy, from January 2012 to December 2022. A propensity score (PS) was built taking into account the variables possibly associated with either VVB choice and the investigated outcomes with the average treatment overlap method. PS-weighted general linear models (GLMs) were developed to investigate the adjusted effect of VVB use on the selected outcomes. The final analysis included 874 LT cases, of whom 74 (8.5%) underwent VVB. The effective sample sizes after PS-weighting were 280.2 and 64.3 patients in the no-VVB and VVB groups, respectively. PS-weighted GLMs did not show any differences regarding hospital and graft-related outcomes. However, significantly higher odds ratios for serum creatinine > 2 mg/dL and AKIN stage 2 or 3 during the first 24 h after ICU admission together with a higher renal replacement therapy need during ICU stay were reported for VVB exposure in the weighted analyses. This study suggests similar mortality and length of stay but a higher risk for postoperative acute kidney injury in patients undergoing piggyback LT with VVB.

Abstract:Background: Very little information is currently available on the use and outcomes of venovenous bypass (VVB) in liver transplantation (LT) in adults in Australia. In this study, we explored the indications, intraoperative course, and postoperative outcomes of patients who underwent VVB in a high-volume LT unit. Methods: The study was a single-center, retrospective observational case series of adult patients who underwent VVB during LT at Austin Health in Melbourne, Australia between March 2008 and March 2022. Information on baseline preoperative status and intraoperative variables, including specific VVB characteristics as well as postoperative and VVB-related complications was collected. The lengths of intensive care unit and hospital stays as well as intraoperative and in-hospital mortality were recorded. Results: Of the 900 LTs performed at this center during the aforementioned 14-year period, 27 (3%) included a VVB procedure. VVB was performed electively in 16 of these 27 patients (59.3%) and as a rescue technique to control massive bleeding in the other 11 (40.1%). The median (interquartile range [IQR]) age of those who underwent VVB procedures was 48 (39-55) years; the median age was 56 (47-62) years in the non-VVB group (p<0.0001). The median model for end-stage liver disease (MELD) scores were similar between the two patient groups. Complete blood data was available for 622 non-VVB patients. Twenty-six VVB (96.3%) and 603 non-VVB (96.9%) patients required intraoperative blood transfusions. The median (IQR) number of units of packed red blood cells transfused was 7 (4.8-12.5) units in the VVB group compared to 3.0 units (1.0-6.0) in the non-VVB group (p<0.0001). Inpatient mortality was 18.5% and 1.1% for the VVB and non-VVB groups, respectively (p<0.0001). There were no significant differences in length of hospital stay or incidence of acute kidney injury, primary graft dysfunction, or long-term graft failure between the two groups. Patients in the VVB group experienced a higher rate of postoperative non-anastomotic biliary stricture compared to patients in the non-VVB group (33% and 7.9%, respectively; p = 0.0003). Conclusions: VVB continues to play a vital role in LT. This case series highlights the heightened risk of major complications linked to VVB. However, the global transition to selective use of VVB underscores the urgent need for collaborative multi-center studies designed to address outstanding questions and parameters related to the safe implementation of this procedure.
Novel Technique for Suprahepatic Vena Cava Reconstruction in Rat Orthotopic Liver Transplantation

Abstract:In previous studies, the suture technique and the cuff method were applied to anastomoses of the suprahepatic vena cava (SHVC) in rat orthotopic liver transplantation. However, the anastomosis of SHVC is difficult during transplantation because of the short length of the SHVC. Here, we developed a novel method for anastomoses of SHVC, using a veno-lined stent technique. The veno-lined stent for SHVC anastomosis was prepared after the donor operation. The special veno-lined stent was a 4.0-mm-long polythene tube in which a venous segment from the donor was lined. During the recipient operation, the donor SHVC was anastomosed end-to-end to the recipient SHVC, using the veno-lined stent. Anastomoses of the portal vein and infrahepatic vena cava were performed using a cuff technique. Continuity of the bile duct was established using a stent. The hepatic artery was ligated, and the graft was not arterialized. As controls, the unlined stent, the suture technique, and the cuff method were also used for SHVC anastomoses, respectively, as three control groups with the identical procedures above. In total, 30 orthotopic liver isografts were performed using the veno-lined stent technique. The survival rate was 90% (27/30) after 1 week and 70% (21/30) after 2 months, with normal hepatocellular function. The SHVC anastomosis using a veno-lined stent took 10 +/- 2 (mean +/- SD) min. The anhepatic phase, recipient operative time, and complete operation time were about 14 +/- 2 min, 40 +/- 5 min, and 120 +/- 10 min, respectively. However, in the nonlined stent control group, a total of 20 orthotopic liver isografts used the nonlined stent for SHVC anastomoses, and all failed because of venous thrombosis in SHVC; none survived over 1 week. In the suture technique control group, 40 orthotopic liver isografts were performed using a suture technique for SHVC anastomoses. The results showed no significant difference with those of the veno-lined stent method. But in the cuff method control group, of all 20 orthotopic liver isografts performed using the cuff method for SHVC anastomoses, 10 failed because of failed ligation on the anastomostic site. The survival rates at 1 week and 2 months postoperatively were significantly different from those of the veno-lined stent method. The veno-lined stent technique provides a novel, simple, and reliable method for SHVC anastomoses. It avoids bleeding during suture and the ligation difficulties found with the cuff method. The operation's success rate is satisfactory. This model is successful, and could be applied in various experimental studies.
Genetic characterization of low pathogenic H5N1 and co-circulating avian influenza viruses in wild mallards (Anas platyrhynchos) in Belgium, 2008

Abstract:As part of a long-term wild bird monitoring programme, five different low pathogenic (LP) avian influenza viruses (AIVs) were isolated from wild mallards (subtypes H1N1, H4N6, H5N1, H5N3, and H10N7). A LP H5N1 and two co-circulating (same location, same time period) viruses were selected for full genome sequencing. An H1N1 (A/Anas platyrhynchos/Belgium/09-762/2008) and an H5N1 virus (A/Anas platyrhynchos/Belgium/09-762-P1/2008) were isolated on the same day in November 2008, then an H5N3 virus (A/Anas platyrhynchos/09-884/2008) 5 days later in December 2008. All genes of these co-circulating viruses shared common ancestors with recent (2001 to 2007) European wild waterfowl influenza viruses. The H5N1 virus shares genome segments with both the H1N1 (PB1, NA, M) and the H5N3 (PB2, HA) viruses, and all three viruses share the same NS sequence. A double infection with two different PA segments from H5N1 and from H5N3 could be observed for the H1N1 sample. The observed gene constellations resulted from multiple reassortment events between viruses circulating in wild birds in Eurasia. Several internal gene segments from these 2008 viruses and the N3 sequence from the H5N3 show homology with sequences from 2003 H7 outbreaks in Italy (LP) and the Netherlands (highly pathogenic). These data contribute to the growing sequence evidence of the dynamic nature of the avian influenza natural reservoir in Eurasia, and underline the importance of monitoring AIV in wild birds. Genetic information of potential hazard to commercial poultry continues to circulate in this reservoir, including H5 and H7 subtype viruses and genes related to previous AIV outbreaks.

Abstract:OBJECTIVE:To elucidate the mechanisms of graft injury in small-for-size liver transplantation.METHODS:Animal models were established with skeletonized and denervated anatomic parahepatic dissection, hepatectomy and perfusion in situ. Chinese Bama miniature pigs were divided into three groups (n = 5): Group A, liver transplantation; Group B, partial liver transplantation with right hemi-liver graft and Group C, liver transplantation with right median and caudate lobe graft. Animals were followed for 7 days with regards to survival, dynamical portal venous pressure (PVP), portal blood flow (PBF) and graft histopathological examination.RESULTS:Animal survivals were as follows: Group A, 5/5, Group B, 5/5 and Group C, 1/5. PVP rose immediately after reperfusion. PVP in Group C peaked to 28.6 +/- 2.07 mm Hg. Portal blood flow (PBF) measured by CDFI showed that the index of PBF per gram liver tissue reached 3.56 +/- 0.11 ml x min(-1) x g(-1) at the first hour post-reperfusion in Group C. Hepatic morphological examination showed that severe pathological changes occurred in small-for-size grafts, including sinusoidal congestion, hemorrhage, hepatocytic ballooning change or necrosis, endothelial cell detachment, Disse's space widening or vanishing and significant apoptosis.CONCLUSION:Portal over perfusion and acute portal hypertension are the primary etiological mechanisms of graft injury in small-for-size liver transplantation.

Abstract:OBJECTIVES:To summarize hemodynamic and metabolic changes during bypass, and to evaluate the bypass in liver transplantation.METHODS:Fifty-four patients underwent orthotopic liver transplantation with venovenous bypass from May 2000 to May 2002. Their clinical features were analysed.RESULTS:SHR, MAP, CVP, CO, PaO(2), PaCO(2), serum K(+), Na(+), Ca(2+), BUN values were not significantly changed during bypass. Compared to the pre-bypass stage, pH was decreased in the post-bypass stage (P < 0.05), serum lactic acid value was increased in the bypass and post-bypass stage (P < 0.05), active clotting time was increased in the bypass stage (P < 0.05), serum creatinine value was increased on first postoperative day (P < 0.05).CONCLUSIONS:Venovenous bypass could improve hemodynamic and metabolic stability in the anhepatic phase, but it also could increase operation duration, liver ischemic time and cost.
Effects of Different Ways of Orthotopic Liver Transplantation without Venovenous Bypass on Anesthetic Management

Abstract:Objective To compare the effects of piggyback and classic orthotopic liver transplantation (OLT) without venovenous bypass on anesthetic management. Methods A retrospective review was conducted on the clinical data of 50 cases of OLT without venovenous bypass performed from November 2003 to December 2006, including 25 piggyback (group A) and 25 classic (group B) OLT. The perioperative hemodynamics, hemostatic conditions, hepatic and renal functions, changes of internal environment, and fluid therapy were compared between the two groups. Results At the beginning of anhepatic stage, the mean arterial pressure (MAP), central venous pressure (CVP), and cardiac index (CI) in the group A were significantly higher than those in the group B [MAP: (68±6) mm Hg vs (64±5) mm Hg; t=2.561, P=0.014. CVP: (5.4±3.3) mm Hg vs (3.5±2.3) mm Hg; t=2.362, P=0.022. CI: (3.7±0.8) L· min-1· m-2 vs (3.2±0.6) L· min-1· m-2;t=2.500, P= 0.016]. At the beginning of neohepatic phase, the MAP in the group A was significantly higher than that in the group B [(66±6) mm Hg vs (62±5) mm Hg; t=2.561,P=0.014], while the CVP was similar between the two groups [(8.4±4.0) mm Hg vs (10.6±4.2) mm Hg; t=-1.897,P=0.064]. The morbidity rate of reperfusion syndrome in the group A was significantly lower than that in the group B (8.0% vs 32.0%, χ2=4.500, P=0.034).At the early neohepatic stage, the blood pH and base excess (BE) values in the group A were significantly higher than those in the group B [pH: 7.34±0.05 vs 7.31±0.04; t=2.343, P=0.023. BE: (-4.9±2.3) mmol /L vs (-6.3±2.3) mmol /L; t=2.152, P=0.036].No significant differences were detected in the hemostatic states, hepatic and renal functions, intraoperative blood loss, urine output, fluid replacement, and component transfusion during OLT between the two groups (P>0.05). The requirement of vasoactive drugs in the group A was significantly smaller than that in the group B (P<0.05). Conclusions The hemodynamics and acid-base balance are relatively stable in the piggyback OLT compared to the classic OLT without venovenous bypass. Monitoring and regulation of hemodynamics, internal environment, coagulation state, hepatic and renal functions, and fluid administration should be emphasized during the OLT by using both the techniques.
