Abstract:Pressure ulcers remain a major source of morbidity and mortality in veterans with neurologic impairment. Management of pressure ulcers typically involves pressure relief over skin regions containing wounds, but this can lead to loss of mobility and independence when the wounds are located in regions that receive pressure from sitting. An innovative, low-cost, multidisciplinary effort was undertaken to maximize quality of life in a veteran with a thoracic-4 level complete spinal cord injury and a stage 4 ischial wound. The person's power wheelchair was converted into a motorized prone cart, allowing navigation of the Department of Veterans Affairs spinal cord injury hospital ward and improved socialization while relieving pressure on the wound. Physical and occupational therapy assisted with the reconfiguration of the power chair and verified safe transfers into the chair and driving of the device. Psychology verified positive psychosocial benefit, while nursing and physician services verified an absence of unwanted pain or skin injury resulting from use of the device. Further investigation of ways to apply this technique is warranted to improve the quality of life of persons with pressure ulcers.
Clinical Outcome of CT-Guided Stereotactic Ablative Brachytherapy for Unresectable Early Non-Small Cell Lung Cancer: A Retrospective, Multicenter Study

Abstract:Objective To analyze the efficacy and safety of low dose rate stereotactic ablative brachytherapy (L-SABT) for treatment of unresectable early-stage non-small cell lung cancer (NSCLC). Methods Data of patients with early-stage NSCLC who received CT-guided L-SABT (radioactive I-125 seeds implantation) at eight different centers from December 2010 to August 2020 were retrospectively analyzed. Treatment efficacy and complications were evaluated. Results A total of 99 patients were included in this study. Median follow-up duration was 46.3 months (6.1-119.3 months). The 1-year, 3-year, and 5-year local control rates were 89.1%, 77.5%, and 75.7%, respectively. The 1-year, 3-year, and 5-year overall survival rates were 96.7%, 70.1%, and 54.4%, respectively. Treatment failure occurred in 38.4% of patients. Local/regional recurrence, distant metastasis, and recurrence combined with metastasis accounted for 15.1%, 12.1%, and 11.1%, respectively. Pneumothorax occurred in 47 patients (47.5%) with 19 cases (19.2%) needing closed drainage. The only radiation-related adverse reaction was two cases of grade 2 radiation pneumonia. KPS 80–100, T1, the lesion was located in the left lobe, GTV D90 ≥150 Gy and the distance between the lesion and chest wall was < 1 cm, were associated with better local control (all P < 0.05); on multivariate analysis KPS, GTV D90, and the distance between the lesion and chest wall were independent prognostic factors for local control (all P < 0.05). KPS 80–100, T1, GTV D90 ≥150 Gy, and the distance between the lesion and chest wall was < 1 cm were also associated with better survival (all P < 0.05); on multivariate analysis KPS, T stage, and GTV D90 were independent prognostic factors for survival (all P < 0.05). The incidence of pneumothorax in patients with lesions <1 cm and ≥1cm from the chest wall was 33.3% and 56.7%, respectively, and the differences were statistically significant ( P = 0.026). Conclusion L-SABT showed acceptable efficacy in the treatment of unresectable early-stage NSCLC. But the incidence of pneumothorax is high. For patients with T1 stage and lesions <1 cm from the chest wall, it may have better efficacy. Prescription dose greater than 150 Gy may bring better results.

Abstract:Abstract Background As the standard practice of our institution, 60Gy is prescribed to PGTV and 54Gy to PTV in the radical radiotherapy of non-small cell lung cancer (NSCLC). We estimate whether this practice could achieve similar tumor control, and protect lymphocyte at the same time. Methods Failure patterns of 46 stage III NSCLC patients received radical radio-chemotherapy were analyzed. Target delineation criterion were as follows: GTV include primary tumor and involved lymph nodes, PGTV expanded from GTV with 5-8mm to compensate for genomic uncertainty. CTV include high-risk area, PTV was extended from CTV with set-up error. 60Gy was prescribed for PGTV, 54Gy for PTV over 30 fractions. The relationship between lymphopenia during radiotherapy and dose-volume parameter was evaluated using Spearman’s correlation analysis. Results With median follow-up of 21.06 months, 22 local-regional recurrence were evidenced, 13 patients have in-field recurrence, one patient failure marginally, and the other 8 patients have out-of field recurrence. Lymphocyte is the most sensitive cell to radiation, lymphopenia during radiotherapy was associated with both PTV(r = 0.489, p = 0.003) and PGTV(r = 0.313, p = 0.076), with larger volume predicted severe lymphopenia. In addition, we observed Lung V(5), V(10) and most of heart or aorta DVH parameters (from V10-V50) are important predictors for lymphocyte nadir. Conclusions Delivered 54Gy to subclinical lesions does not compromise marginal recurrence risk, at the same time lower severe radiation-induced lymphopenia risk. This finding supports further exploration of dosage reduction to CTV in locally advanced NSCLC.

Abstract:Importance Patients with interstitial lung disease (ILD) and early-stage non–small cell lung cancer (NSCLC) have been reported to be at high risk of toxic effects after stereotactic ablative radiotherapy (SABR), but for many patients, there are limited alternative treatment options. Objective To prospectively assess the benefits and toxic effects of SABR in this patient population. Design, Setting, and Participants This prospective cohort study was conducted at 6 academic radiation oncology institutions, 5 in Canada and 1 in Scotland, with accrual between March 7, 2019, and January 12, 2022. Patients aged 18 years or older with fibrotic ILD and a diagnosis of T1-2N0 NSCLC who were not candidates for surgical resection were enrolled. Intervention Patients were treated with SABR to a dose of 50 Gy in 5 fractions every other day. Main Outcomes and Measures The study prespecified that SABR would be considered worthwhile if median overall survival—the primary end point—was longer than 1 year, with a grade 3 to 4 risk of toxic effects less than 35% and a grade 5 risk of toxic effects less than 15%. Secondary end points included toxic effects, progression-free survival (PFS), local control (LC), quality-of-life outcomes, and changes in pulmonary function. Intention-to-treat analysis was conducted. Results Thirty-nine patients enrolled and received SABR. Median age was 78 (IQR, 67-83) years and 59% (n = 23) were male. At baseline, 70% (26 of 37) of patients reported dyspnea, median forced expiratory volume in first second of expiration was 80% (IQR, 66%-90%) predicted, median forced vital capacity was 84% (IQR, 69%-94%) predicted, and median diffusion capacity of the lung for carbon monoxide was 49% (IQR, 38%-61%) predicted. Median follow-up was 19 (IQR, 14-25) months. Overall survival at 1 year was 79% (95%, CI 62%-89%; P &lt; .001 vs the unacceptable rate), and median overall survival was 25 months (95% CI, 14 months to not reached). Median PFS was 19 months (95% CI, 13-28 months), and 2-year LC was 92% (95% CI, 69%-98%). Adverse event rates (highest grade per patient) were grade 1 to 2: n = 12 (31%), grade 3: n = 4 (10%), grade 4: n = 0, and grade 5: n = 3 (7.7%, all due to respiratory deterioration). Conclusions and Relevance In this trial, use of SABR in patients with fibrotic ILD met the prespecified acceptability thresholds for both toxicity and efficacy, supporting the use of SABR for curative-intent treatment after a careful discussion of risks and benefits. Abstract:This article aimed to analyze the perception of the nursing team about the care of institutionalized elderly. It consists of a qualitative and exploratory study developed in the six long-term care nursing homes for elderly in the city of João Pessoa/PB, Brazil, from January to June 2013. The sample was comprised 50% of the nurses from all the institutions, totaling 13 participants, whose data were collected through semi-structured questionnaires. Content analysis was used in the study, which enabled the construction of two thematic categories: I) Perceptions of the delivery of care by the nursing staff of the institutions; II) Perceptions of the care activity to be performed by the nursing staff in the institutions. We conclude that the participants are aware of the need for an individualized and systematized care by the nursing staff, aimed to the prevention of diseases, health promotion and active aging.

Abstract:Importance Intrathoracic progression remains the predominant pattern of failure in patients treated with concurrent chemoradiation followed by a consolidation immune checkpoint inhibitor for locally advanced, unresectable non–small cell lung cancer (NSCLC). Objective To determine the maximum tolerated dose (MTD) and use of hypofractionated concurrent chemoradiation with an adaptive stereotactic ablative radiotherapy (SABR) boost. Design, Setting, and Participants This was an early-phase, single-institution, radiation dose-escalation nonrandomized controlled trial with concurrent chemotherapy among patients with clinical stage II (inoperable/patient refusal of surgery) or III NSCLC ( American Joint Committee on Cancer Staging Manual, seventh edition ). Patients were enrolled and treated from May 2011 to May 2018, with a median patient follow-up of 18.2 months. Patients advanced to a higher SABR boost dose if dose-limiting toxic effects (any grade 3 or higher pulmonary, gastrointestinal, or cardiac toxic effects, or any nonhematologic grade 4 or higher toxic effects) occurred in fewer than 33% of the boost cohort within 90 days of follow-up. The current analyses were conducted from January to September 2023. Intervention All patients first received 4 Gy × 10 fractions followed by an adaptive SABR boost to residual metabolically active disease, consisting of an additional 25 Gy (low, 5 Gy × 5 fractions), 30 Gy (intermediate, 6 Gy × 5 fractions), or 35 Gy (high, 7 Gy × 5 fractions) with concurrent weekly carboplatin/paclitaxel. Main Outcome and Measure The primary outcome was to determine the MTD. Results Data from 28 patients (median [range] age, 70 [51-88] years; 16 [57%] male; 24 [86%] with stage III disease) enrolled across the low- (n = 10), intermediate- (n = 9), and high- (n = 9) dose cohorts were evaluated. The protocol-specified MTD was not exceeded. The incidences of nonhematologic acute and late (&gt;90 days) grade 3 or higher toxic effects were 11% and 7%, respectively. No grade 3 toxic effects were observed in the intermediate-dose boost cohort. Two deaths occurred in the high-dose cohort. Two-year local control was 74.1%, 85.7%, and 100.0% for the low-, intermediate-, and high-dose cohorts, respectively. Two-year overall survival was 30.0%, 76.2%, and 55.6% for the low-, intermediate-, and high-dose cohorts, respectively. Conclusions and Relevance This early-phase, dose-escalation nonrandomized controlled trial showed that concurrent chemoradiation with an adaptive SABR boost to 70 Gy in 15 fractions with concurrent chemotherapy is a safe and effective regimen for patients with locally advanced, unresectable NSCLC. Efficacy of Template-Assisted Neoadjuvant 192ir Hypofractionated Stereotactic Ablative Brachytherapy in Resectable Non-Small Cell Lung Cancer

Abstract:Recently a phase I trial (Chinese Clinical Trials Register No. ChiCTR1800014541) was launched to assess the efficacy, technical safety and adverse events of stereotactic ablative brachytherapy (SABT) using template-assisted neoadjuvant 192Ir hypofractionated SABT before surgery in patients with early-stage non-small cell lung cancer (NSCLC). We report the assessment of the first 10 patients who completed SABT in this trial. In this officially approved study, we enrolled patients with histologically confirmed resectable early-stage NSCLC (T1–T2b). While the template-assisted SABT was delivered in a risk-adapted fractionation (30Gy/1F), surgical resection was scheduled to remove tumors 4 weeks after SABT. Patients underwent 18F-fludeoxyglucose-positron emission tomography (18F-FDG-PET) and dynamic contrast-enhanced computed tomography (DCECT) before SABT and again before surgery. Furthermore, we assessed the efficacy of the neoadjuvant by comparing tumor volume, PET maximum Standardized uptake value (SUVmax), tumor blood volume (TBV) and tumor blood flow (TBF) and recorded adverse events to assess technical safety. After 4 weeks of SABT, the evaluative indicators of neoadjuvant SABT differed significantly in all the patients as follows: tumor volume (P = 0.004), SUVmax (P < 0.001), TBV (P = 0.002) and TBF (P = 0.023). In fact, 3 out of 10 patients exhibited >50% decrease in tumor volume. However, the correlations among these indicators were not statistically significant. While 2 patients experienced minor pneumothorax during SABT, 2 had slight fibrosis around their tumors after SABT. The present study established the efficacy of template-assisted neoadjuvant SABT in resectable early-stage NSCLC, with no reported adverse events. Nevertheless, further studies are warranted to correlate the SUVmax response with decrease in tumor volume, TBV and TBF after SABT. Furthermore, the curative effect in combination with surgical interventions and final pathology results require further investigation in follow-up studies.
Simple Clinical Factors Associated With Radiation-Induced Lung Toxicity After Stereotactic Body Radiation Therapy In Lung Cancer: A Pooled Analysis Of 70 Studies

Abstract:radiation therapy (SABR) from a toxicity standpoint, with RTOG 0813 designed to help address this issue. Here we report our institution’s experience in treating central lung tumor patients with SABR, compared to similarly treated peripheral lung tumors. Materials/Methods: We retrospectively reviewed outcomes in 82 patients with 96 primary or metastatic lung tumors treated by SABR. The cohort included 47 central and 49 peripheral tumors; all were treated with 50 Gy in 4 to 5 fractions. Outcomes, toxicities, and dosimetric parameters were compared between the 2 groups, not including patients in whom central and peripheral tumors were concurrently treated (8 such patients). Results: Median follow-up time was 15 months (range 1 to 59 months). Median overall survival was 34 and 40 months (PZ.75) for patients with central and peripheral tumors, respectively. We observed no hemoptysis, and no grade 5 toxicities. Rates of symptomatic (grade 2+) toxicity were higher in patients with central tumors by both univariate analysis (38% vs 11%, PZ.0056, HRZ3.7) and multivariate analysis (PZ.02, HRZ3.9), but were equivalent for severe (grade 3+) toxicity (5.4% vs 5.4%, PZ.93, HRZ1.1). The vast majority of observed toxicities were due to symptomatic pneumonitis, incidence of which was significantly greater in patients with central tumors by both univariate analysis (30% vs 2.7%, PZ.0021, HRZ6.5) and multivariate analysis (PZ.02, HRZ11.1). These patients also had higher rates of severe pneumonitis that trended towards significance (5.4% vs 0, PZ.15, HRZ7.6). Dosimetric analysis revealed that the central tumor patients in our study experienced significantly greater ipsilateral mean lung doses than their peripheral tumor counterparts (7.33 vs 5.15 Gy, PZ.0058). Total mean lung dose and total lung V20 were not significantly different between the 2 populations. Further analysis of the 35 available central tumor treatment plans revealed that 23 (66%) exceeded at least 1 of the dose constraints used in RTOG 0813, most commonly for central airway. Rates of symptomatic pneumonitis were not significantly different between patients exceeding RTOG 0813 central airway dosing constraints versus those meeting them (25% vs 20%, PZ.64), but trended towards significance for severe pneumonitis, with patients exceeding RTOG 0813 central airway dose constraints experiencing higher rates (0 vs 13%, PZ.10). Conclusions: Central lung tumor patients treated with SABR appear to experience higher rates of symptomatic pneumonitis compared to patients with peripherally located lung tumors, which grossly correlates with increased ipsilateral mean lung dose. Clinical and Radiobiologic Effect of Partial Stereotactic Ablative Radiation Therapy in NSCLC

Abstract:Retrospectively evaluated the effect of Partial- Stereotactic ablative radiotherapy (P-SABR) in nonsmall cell lung carcinoma (NSCLC) and managed to evaluate the appropriate BED. Between December 2012 and September 2016 conditions for patient eligibility: (1)not suitable for surgery, because of being technically or medically inoperable or because of patient refusal; (2) histologic confirmation of NSCLC ;(3)no prior radiation therapy, interventional chemotherapy or radiofrequency ablation to the targeted area; (4) no prior chemotherapy or targeted therapy;(5) concurrent chemotherapy with DDP 50mg per week or no concurrent chemotherapy because not suitable; (6)complete follow-up image data in our hospital;(7)Karnofsky Performance Status >70; (8)minimum age 18 years; A total of 18 patients were included. The median period of follow-up was 12 months (range, 3–32 months). The median age was 64 years old(range, 40–85 years old).Squamous cell carcinoma was 66.7% (n=12), adeno carcinoma was 5.6% (n=1), other NSCLC was 27.8%(n=5). Among the patients, Stage II NSCLC was 16.7% (n =3), Stage III NSCLC was 38.9% (n =7), Stage IV NSCLC was 38.9% (n =7), local recurrence after operation was 5.6% (n =1). The median tumor diameter was 8.46 (5.04- 22.74cm). 66.7% (n=12) received RT alone.Dose prescription : conventional fractioned: A total dose of 44-60Gy in 16–33 fractions, 1.8-3Gy per fraction was administered with 6-MV X-rays in the PTV dose. SABR: A total dose of 18-32Gy in3-4fractions, 6-9Gy per fraction was administered in the GTV-boost dose. The median Tumor volume was 123.89cm3 (35.22- 495.50 cm3); The median GTV-boost volume was 24.90cm3 (8.13-115.90 cm3); Grade 3 or 4 radiaton pneumonitis(RTOG criteria) was observed in only two patients. The median overall survival was 16.3 month. The overall 1-year survival rates was 49%. The in-field 3-month and 1-year local contral(CR+PR)rates were 81.4% and 71.2%, respectively. The in-field 1-year local contral(CR+PR+SD)rates was 100%. We defined V100 as the ration of volume of BED>100Gy to the in-field tumor. The 1-year local control(CR+PR) rate was 90.9% for V100>30%, compared with 31.3% for V100<30% (p<0.05). We defined P75 as the ration of volume of physical dose >75Gy to the in-field tumor. The 1-year local control (CR+PR) rate was 90.9%for P75>40%, compared with 31.3% for P75<40% (p<0.05). Partial-Stereotactic ablative radiotherapy (P-SABR) in nonsmall cell lung carcinoma (NSCLC) was feasible and beneficial. 30% was a recommended ratio of the volume of BED>100Gy to the in-field tumor. 40% was a recommended ratio of the volume of physical dose >75Gy to the in-field tumor.

Abstract:Background: With the aging of the population and the increased importance of lung cancer screening, the number of early-stage lung cancer patients has been on the rise in recent years, which can be classified into operable early-stage lung cancer and inoperable early-stage lung cancer. The most common pathological type is non-small cell lung cancer (NSCLC). Stereotactic body radiation therapy (SBRT) is the optimal treatment for inoperable early-stage NSCLC. The aim of this study was to investigate the prognosis of early-stage NSCLC patients treated with SBRT and its influencing factors in order to reduce the side effects of radiotherapy and improve the survival and quality of life. Methods: Clinical data and follow-up outcomes of early-stage NSCLC patients treated with SBRT in our hospital from August 2010 to August 2020 were collected. Kaplan-Meier method was used to assess the prognosis, and the Cox proportional risk model was used for multivariate prognostic analysis. Results: A total of 165 patients were included with a median follow-up time of 43.2 (range: 4.8-132.1) mon. The local control (LC) rates at 1-yr, 2-yr and 5-yr were 98.1%, 94.8% and 86.5% respectively. Karnofsky performance status (KPS) score greater than 80 was an independent prognostic factor for LC (P=0.02). The overall survival (OS) rates at 1-yr, 2-yr and 5-yr were 97.6%, 93.0% and 68.9% respectively. A biological equivalent dose when α/β=10 (BED10) greater than 132 Gy was an independent prognostic factor for OS (P=0.04). Progression-free survival (PFS) rates at 1-yr, 2-yr and 5-yr were 93.3%, 79.5% and 55.3% respectively. The distance metastasis free survival (DMFS) rates at 1-yr, 2-yr and 5-yr were 94.5%, 83.2% and 58.4% respectively. BED10 greater than 150 Gy was an independent prognostic factor for DMFS (P=0.02). The regional control (RC) rates at 1-yr, 2-yr and 5-yr were 98.8%, 95.4% and 87.9% respectively. Conclusions: SBRT is effective in treating early-stage NSCLC. KPS greater than 80 is an independent prognostic factor for LC; BED10 greater than 132 Gy is an independent prognostic factor for OS; BED10 greater than 150 Gy is an independent prognostic factor for DMFS.

Abstract:A simple formula to determine the transition point was found. The hypofractionated regimen is superior below this point but inferior above it compared with the hyperfractionated regimen using SABR techniques. In addition, the hyperfractionated regimens appear to be superior to the hypofractionated regimen when comparing LKB model‐estimated NTCP with mean lung BED. Conversely, hypofractionated regimens seem to be better when focusing on critical volume or V20. Background Limited literature exists on the feasibility and effectiveness of integrating stereotactic ablative radiotherapy (SABR) techniques with hyperfractionated regimens for patients with lung cancer. This study aims to assess whether the SABR technique with hyperfractionation can potentially reduce lung toxicity. Methods We utilized the linear‐quadratic model to find the optimal fraction to maximize the tumor biological equivalent dose (BED) to normal‐tissue BED ratio. Validation was performed by comparing the SABR plans with 50 Gy/5 fractions and hyperfractionationed plans with 88.8 Gy/74 fractions with the same tumor BED and planning criteria for 10 patients with early‐stage lung cancer. Mean lung BED, Lyman–Kutcher–Burman (LKB) normal tissue complication probability (NTCP), critical volume (CV) criteria (volume below BED of 22.92 and 25.65 Gy, and mean BED for lowest 1000 and 1500 cc) and the percentage of the lung receiving 20Gy or more (V20) were compared using the Wilcoxon signed‐rank test. Results The transition point occurs when the tumor‐to‐normal tissue ratio (TNR) of the physical dose equals the TNR of α/β in the BED dose‐volume histogram of the lung. Compared with the hypofractionated regimen, the hyperfractionated regimen is superior in the dose range above but inferior below the transition point. Abstract:Purpose: Bulky non-small cell lung cancer (NSCLC) is difficult to achieve effective local control by conventionally fractionated radiotherapy (CRT). The present work aims to evaluate the safety and efficacy of partial stereotactic ablative boost radiotherapy (P-SABR) in bulky NSCLC. Patients and methods: From December 2012 through August 2017, 30 patients with bulky NSCLC treated with P-SABR technique were analyzed. The P-SABR plan consisted of one partial SABR plan (5-9 Gy/f, 3-6 fractions) to gross tumor boost (GTVb), followed by one CRT plan to the planning target volume (PTV). GTVb was the max volume receiving SABR to guarantee the dose of organs-at-risks (OARs) falloff to about 3 Gy/f. The total dose of PTV margin was planned to above 60 Gy. The simply CRT plans were created using the same planning parameters as the original plan, with the goal to achieve comparable OARs doses and PTV margin dose to the P-SABR plan. Dosimetric variables were acquired in both P-SABR and compared CRT plans. Toxicity, local control, and survival were also evaluated. Results: Median follow-up in survivors was 10.3 months (range=2.3-39.4 months). Eleven patients (36.7%) had partial response (PR) and ten patients (33.3%) had stable disease (SD). Two-year overall survival was 55.6%. Two-year local control rate was 85.7%. No severe acute side effects > CTCAE Grade III were observed. Compared to the simply CRT plan, P-SABR plans achieved similar doses to the OARs and Dmin, but increased dose at the isocenter, Dmean, Dmax, and biological equivalent dose (BED) significantly (P < 0.05). BED in the tumor center could reach 107.3 Gy (93.2-132 Gy). Patients with B90 >= 65% achieved a higher local control rate than those with B90 < 65% (P=0.010). Conclusion: This retrospective study suggests that P-SABR is feasible and well tolerated in bulky NSCLC. Local control rate is encouraging, especially for the B90 >= 65% group, which may due to the ability of P-SABR to optimize BED with equivalent toxicity.

Abstract:BACKGROUND Primary tumor failure is common in patients treated with chemoradiation (CRT) for locally-advanced NSCLC (LA-NSCLC). Stereotactic body radiation therapy (SBRT) yields high rates of primary tumor control (PTC) in early-stage NSCLC. This trial tested an SBRT boost to the primary tumor prior to the start of CRT to improve PTC. MATERIALS/METHODS Patients with LA-NSCLC received an SBRT boost in 2 fractions (central location 12 Gy, peripheral location 16 Gy) to the primary tumor, followed by standard CRT (60 Gy in 30 fractions). The primary objective was PTC rate at 1-year with a hypothesis that the 1-year PTC rate is ≥90%. Secondary objectives included objective response rate (ORR), regional & distant control, disease-free survival (DFS), and overall survival (OS). Correlative studies included functional MRI (fMRI) and blood-based miRNA analysis. RESULTS The study enrolled 21 patients (10 male, 11 female), with median age 62 years (range 52-78). Median pre-treatment primary tumor size was 5.0 cm (range 1.0-8.3). The most common non-hematologic toxicities were pneumonitis, fatigue, esophagitis/dysphagia, dyspnea, and cough. Only 1 treatment-related grade 4 non-hematologic toxicity occurred (respiratory failure/radiation pneumonitis), and there were no grade 5 toxicities. ORR at 3 and 6 months was 72.7% and 80.0%, and PTC at 1 and 2 years was 100% and 92.3%, respectively. The 2-year regional and distant control were 81.6% and 70.3%, respectively. DFS and OS at 2 yrs were 46.1% and 50.3%, respectively. Median survival was 37.8 months. fMRI detected a mean relative decrease in BOLD signal of -87.1% (p=0.05), and miR.142.3p correlated with increased risk of grade≥3 pulmonary toxicity (p=0.01). CONCLUSIONS Dose escalation to the primary tumor utilizing upfront SBRT appears feasible and safe. PTC was high and other oncologic endpoints compared favorably to standard treatment. fMRI suggested changes in oxygenation with the first SBRT boost dose, and miR.142.3p correlated with pulmonary toxicity.

Abstract:Background There is no consensus on the definition or recommended radiotherapy treatment of ultracentral non-small cell lung cancer (NSCLC). Here, we report our institution's experience in treating ultracentral lung cancer patients with stereotactic ablative radiotherapy (SABR) of 60 Gy in eight fractions. Methods We retrospectively reviewed the outcomes of 21 ultracentral NSCLC patients treated with 60 Gy SABR in eight fractions. We defined ultracentral lung cancer as the planning target volume (PTV) directly abutting or overlapping central structures, including the proximal bronchial tree, heart, and great vessels but not the esophagus. The Kaplan-Meier method was used to estimate overall survival (OS), progression-free survival (PFS) and local control (LC). Toxicity was scored per the CTCAE v4.03. Results The median follow-up time was 15 months, and the median OS was 15 months. The one- and two-year OS rates were 87.5% and 76.6%, respectively. The one- and two-year PFS rates were 71.1% and 64.0%, respectively. The one- and two-year LC rates were 92.9% and 92.9%, respectively. The rate of grade 2 treatment-related toxicities was 19.1%. There was no grade >= 3 treatment-related toxicity. Conclusion SABR of 60 Gy in eight fractions is feasible for ultracentral NSCLC.

Abstract:Purpose/Objective(s): Treatments for superior sulcus non-small cell lung cancer (SS-NSCLC) have evolved, but adequate treatments of T4 disease have not been found. The aim of our study was to evaluate the prognostic factors and optimal treatment strategy for patients with T4 SS-NSCLC. Materials/Methods: We utilized the Surveillance, Epidemiology, and End Results (SEER) database (1973-2015) to identify patients diagnosed with T4 stage SS-NSCLC (according to the 7th edition American Joint Committee on Cancer (AJCC) staging system) from 2004 to 2015; those with M1 disease were excluded. Propensity score matching (PSM) with Kaplan-Meier and Cox proportional hazards' models was performed to estimate prognosis. Results: A total of 384 patients were included. The majority was male (59.4%) at stage IIIB (56.6%), with N2 accounting for 45.3%. A total of 47 patients underwent cancer-directed surgery, while radiotherapy alone was received by 60.2% of patients. Median overall survival (OS) and lung cancer-specific survival (LCSS) were 12 and 17 months, respectively, and the 5-year OS and LCSS rates were 15.8 and 25.4%, respectively. In the matched population, the median survival outcomes were better following surgery (OS: 25 compared with 9.0 months, P<0.001; LCSS: not available (NA) compared with 11.0 months, P<0.001). Multivariate Cox analysis showed that ages ≥ 66 years (hazard ratio (HR) = 1.639, P=0.001), unmarried status (HR = 1.356, P=0.034), and tumor size ≥ 6.0 cm (HR = 1.694, P<0.001) were associated with inferior OS. Cancer-directed surgery (HR = 0.537, P=0.009) and radiotherapy (HR = 0.644, P=0.006) were independent prognostic factors for patients with T4 SS-NSCLC. Conversely, in the subgroup analysis, favorable impacts of radiotherapy were observed for nonsurgical patients (OS: HR = 0.58, P<0.001; LCSS: HR = 0.55, P<0.001). Conclusion: Our study showed that T4 stage SS-NSCLC patients had a poor prognosis. Surgical resection remains the best option for those with resectable disease. For nonsurgical T4 SS-NSCLC patients, radiotherapy should be actively considered.
