Abstract:Recently, more and more attention has been paid to the development of a new generation of injectable bone cements that are bioactive, biodegradable and are able to have appropriate mechanical properties for treatment of vertebral compression fractures (VCFs). In this study, a novel PSC/CS composite cement with high content of PSC (a phytic acid-derived bioactive glass) was prepared and evaluated in both vitro and vivo. The PSC/CS cement showed excellent injectability, good resistance to disintegration, radiopacity and suitable mechanical properties. The in vitro test showed that the cement was bioactive, biocompatible and could maintain its shape sustainably, which made it possible to provide a long-term mechanical support for bone regeneration. Radiography, microcomputed tomography and histology of critical sized rabbit femoral condyle defects implanted with the cements proved the resorption and osteoinductivity of the cement. Compared with the PMMA and CSPC, there were more osteocyte and trabeculae at the Bone-Cement interface in the group PSC/CS cement. The volume of the residual bone cement suggested that PSC/CS had certain ability of degradation and the resorption rate was much lower than that of the CSPC cement. Together, the results indicated that the cement was a promising bone cement to treat the VCFs.

Abstract:Background: Calcium phosphate cement (CPC) can be molded or injected to form a scaffold in situ, which intimately conforms to complex bone defects. Bioactive glass (BG) is known for its unique ability to bond to living bone and promote bone growth. However, it was not until recently that literature was available regarding CPC-BG applied as an injectable graft. In this paper, we reported a novel injectable CPC-BG composite with improved properties caused by the incorporation of BG into CPC. Materials and methods: The novel injectable bioactive cement was evaluated to determine its composition, microstructure, setting time, injectability, compressive strength and behavior in a simulated body fluid (SBF). The in vitro cellular responses of osteoblasts and in vivo tissue responses after the implantation of CPC-BG in femoral condyle defects of rabbits were also investigated. Results: CPC-BG possessed a retarded setting time and markedly better injectability and mechanical properties than CPC. Moreover, a new Ca-deficient apatite layer was deposited on the composite surface after immersing immersion in SBF for 7 days. CPC-BG samples showed significantly improved degradability and bioactivity compared to CPC in simulated body fluid (SBF). In addition, the degrees of cell attachment, proliferation and differentiation on CPC-BG were higher than those on CPC. Macroscopic evaluation, histological evaluation, and micro-computed tomography (micro-CT) analysis showed that CPC-BG enhanced the efficiency of new bone formation in comparison with CPC. Conclusions: A novel CPC-BG composite has been synthesized with improved properties exhibiting promising prospects for bone regeneration.
A Bioactive Degradable Composite Bone Cement Based on Calcium Sulfate and Magnesium Polyphosphate

Abstract:Calcium sulfate bone cement (CSC) is extensively used as a bone repair material due to its ability to self-solidify, degradability, and osteogenic ability. However, the fast degradation, low mechanical strength, and insufficient biological activity limit its application. This study used magnesium polyphosphate (MPP) and constructed a composite bone cement composed of calcium sulfate (CS), MPP, tricalcium silicate (C3S), and plasticizer hydroxypropyl methylcellulose (HPMC). The optimized CS/MPP/C3S composite bone cement has a suitable setting time of approximately 15.0 min, a compressive strength of 26.6 MPa, and an injectability of about 93%. The CS/MPP/C3S composite bone cement has excellent biocompatibility and osteogenic capabilities; our results showed that cell proliferation is up to 114% compared with the control after 5 days. After 14 days, the expression levels of osteogenic-related genes, including Runx2, BMP2, OCN, OPN, and COL-1, are about 1.8, 2.8, 2.5, 2.2, and 2.2 times higher than those of the control, respectively, while the alkaline phosphatase activity is about 1.7 times higher. Therefore, the CS/MPP/C3S composite bone cement overcomes the limitations of CSC and has more effective potential in bone repair.

Abstract:Autografts, allografts, and xenografts are the most widely used methods to enhance effective reconstruction of bone defects, but natural donor bone has some deficiencies, such as limited supply, excessive damage of transplant-supplied-parts, constrained growth and high complication rates.<a class="ejp-citation-link js-ejp-citation-link">[1]</a> Bioactive bone replacement materials provide an alternative for bone defect repair and regeneration. Augmented injectable bone scaffold material applied to bone replacement therapies in orthopedics and dentistry has generated much discussion in bone tissue engineering for bone defect repair. Injectable cement materials currently in clinical use and those under research can be classified into three main categories: polymethylmethacrylate (PMMA), calcium-phosphate-based bone cements (CPCs), and calcium-sulfate-based bone cements (CSCs). Of these, the chemical components of CPCs are most similar to the inorganic components of bones. CPCs mimic the mineral phase of bone to formulate a natural lattice for bone tissue to promote natural bone ingrowth and remodeling.<a class="ejp-citation-link js-ejp-citation-link">[2]</a> Furthermore, in comparison with the most widely used PMMA cements, CPCs have a lower exothermic reaction temperature and promote better osteointegration. For these reasons, they have become one of the most promising bone repair materials.<a class="ejp-citation-link js-ejp-citation-link">[3]</a> Nevertheless, CPCs have some limitations: (1) Their slow degradation rate and lack of macroporosity to promote the formation of new bone make their osteoinduction capacity insufficient for clinical needs.<a class="ejp-citation-link js-ejp-citation-link">[4]</a> Modification efforts include fabricating porous/nano structures and/or drug delivery CPC composites to promote bone growth; (2) Another drawback is that the mechanical strength of CPCs cannot match the strength of human cortical bone; thus, strength modification is another research direction.

Injectable materials show their special merits in regeneration of damaged/degenerated bones in minimally-invasive approach. Injectable calcium phosphate bone cement (CPC) has attracted broad attention for its bioactivity, as compared to non-degradable polymethyl methacrylate cement. However, its brittleness, poor anti-washout property and uncontrollable biodegradability are the main challenges to limit its further clinical application mainly because of its stone-like dense structure and fragile inorganic-salt weakness. Herein, we developed a kind of injectable CPC bone cement with porous structure and improved robustness by incorporating poly(lactide-co-glycolic acid) (PLGA) nanofiber into CPC, with carboxymethyl cellulose (CMC) to offer good injectability as well as anti-wash-out capacity. Furthermore, the introduction of PLGA and CMC also enabled a formation of initial porous structure in the cements, where PLGA nanofiber endowed the cement with a dynamically controllable biodegradability which provided room for cell movement and bone ingrowth. Interestingly, the reinforced biodegradable cement afforded a sustainable provision of Ca 2+ bioactive components, together with its porous structure, to improve synergistically new bone formation and osteo-integration in vivo by using a rat model of femur condyle defect. Further study on regenerative mechanisms indicated that the good minimally-invasive bone regeneration may come from the synergistic enhanced osteogenic effect of calcium ion enrichment and the improved revascularization capacity contributed from the porosity as well as the lactic acid released from PLGA nanofiber. These results indicate the injectable bone cement with high strength, anti-washout property and controllable biodegradability is a promising candidate for bone regeneration in a minimally-invasive approach. Abstract:By incorporating calcium/magnesium phosphate cement (CMPC) particles, the polymerization of monomers is controlled through hydration heat release, resulting in in situ hydrogel gelation. CMPC hydrates act as inorganic fillers, improve mechanical strength, reduce swelling, and accelerate biodegradation, thus meeting the requirements for bone regeneration. The incorporation of CMPC particles shows promise for improving clinical bone regeneration with hydrogel‐based materials. Composite hydrogels with excellent properties can open new opportunities to terminate the need for auto/allografts in bone augmentations. However, their clinical application has been limited by their insufficient mechanical strength and lack of osteoinductivity. Here we report a new strategy to design an injectable bioactive double network hydrogel reinforced by inorganic calcium/magnesium phosphate cement (CMPC) hydrates to meet the mechanical performance requirements for bone regeneration. The engineered CMPC hydration endows the composite hydrogel with an appropriate gelation time and temperature for injection, which shows no harm in the defect site. CMPC hydrates could also provide a lower swelling ratio and higher biodegradation rate fitting the in vivo bone regeneration needs. In vitro and in vivo experiments prove that the ions released from inorganic particles endow biocompatibility, cell migration, adhesion, differentiation, and significantly higher bone regeneration capacity. Taken together, the simple addition of CMPC particles imparts in‐demand features that bring us closer to the clinical utilization of hydrogel‐based materials for bone regeneration.

Abstract:Calcium-phosphate cement (CPC) is currently used to treat bone defects; however, it may cause inflammation due to pH changes by an acid-base reaction during setting. Therefore, we have developed bioresorbable chelate-setting β-tricalcium phosphate (β-TCP) cement based on surface modification of inositol phosphate (IP6). In order to improve the bioresorbability, we have previously succeeded in preparing IP6/β-TCP cement hybridized with poly(lactic-co-glycolic acid) (PLGA) particles as a pore-forming agent. However, the hybrid cement had a prolonged setting time. In this study, calcium sulfate hemihydrate (CSH) was added to shorten the setting time and further improve the cement bioabsorbability, and the material properties of the resulting samples were clarified, together with an in vivo reaction. The cement setting time with the simultaneous addition of 10 mass% PLGA particles and CSH was successfully reduced from 26.3 min, the cement setting time without CSH addition, to 15 min; and the compressive strength of the cement exhibited 30 MPa or higher. The hard tissue compatibility of the resulting cement was qualitatively evaluated using a rat calvaria defect model. Subsequently, the bioresorption and bone formation rates of the cement were quantitatively determined using large animal pigs, which are more similar to humans in terms of their in vivo response. The highest bioabsorption (39.6%) and bone formation rates (24.2%) were confirmed for cement with the simultaneous addition of 10 mass% PLGA particles and CSH. Organic/inorganic hybrid cement with the simultaneous addition of CSH and PLGA particles is expected to be clinically applicable as a bone repair cement with a short setting time, improved compressive strength, and bioresorbability compatible with the bone remodeling cycle.

Abstract:Background:The repair of bone defects has always been a significant challenge in clinical medicine. To address this challenge, doctors often utilize autologous bone grafts, allogeneic bone grafts and artificial bone substitutes. However, the former two methods may result in additional trauma and complications, while allogeneic bone grafts carry the risks of immune rejection and disease transmission. Magnesium phosphate cement (MPC), as a artificial bone substitutes, has been a potential biomaterial for repairing bone defects, but its clinical application is limited by insufficient mechanical strength and poor osteoinductive activity. Methods:In this study, the cement liquid phase base on rhBMP-2 and chitosan solution into MPC were obtained and investigated. After mixing with a cement liquid, the structural and phase composition, morphology, chemical structure, setting time, compressive strength, degradation behavior, solubility, and cellular responses and bone regeneration in response to CHI-rhBMP2 MPC were investigated in vitro and in vivo. Results:After the chemical component modification, CHI-rhBMP2 MPC possessed controllable degradation rate, moderate setting time, appropriate cuing temperature, good injectability, and improved initial strength. In vitro tests showed that the CHIrhBMP2 MPC could promote cell proliferation and adhesion, as well as that contribute to osteoblast differentiation and mineralization. In addition, cement materials were implanted into the rabbit femoral condyles for in vivo osseointegration evaluation. The results displayed that more new bone grew around CHI-rhBMP2 MPC, verifying improved osseointegration capacity. Transcriptome analysis revealed that focal adhesion, Forkhead box O（FoxO） signaling pathway and P13K/AKT signaling pathway were may involved in CHI-rhBMP2 MPC induced new bone formation. Conclusion:This work provides a new strategy for the rational design of potential bone repair candidate materials.

Abstract:Biomedical cements are considered promising injectable materials for bone repair and regeneration. Calcium phosphate composition sized with tens of micrometers is currently one of the major powder forms. Here we report a unique cement form made from mesoporous bioactive glass nanoparticles (BGn). The nanopowder could harden in reaction with aqueous solution at powder-to-liquid ratios as low as 0.4-0.5 (vs. 2.0-3.0 for conventional calcium phosphate cement CPC). The cementation mechanism investigated from TEM, XRD, FT-IR, XPS, and NMR analyses was demonstrated to be the ionic (Si and Ca) dissolution and then reprecipitation to form Si-Ca-(P) based amorphous nano-islands that could network the particles. The nanopowder-derived nanocement exhibited high surface area (78.7 m2/g); approximately 9 times higher than conventional CPC. The immersion of nanocement in simulated body fluid produced apatite nanocrystallites with ultrafine size of 10 nm (vs. 55 nm in CPC). The ultrafine nanocement adsorbed protein molecules (particularly positive charged proteins) at substantial levels; approximately 160 times higher than CPC. The nanocement released Si and Ca ions continuously over the test period of 2 weeks; the Si release was unique in nanocement whereas the Ca release was in a similar range to that observed in CPC. The release of ions significantly stimulated the responses of cells studied (rMSCs and HUVECs). The viability and osteogenesis of rMSCs were significantly enhanced by the nanocement ionic extracts. Furthermore, the in vitro tubular networking of HUVECs was improved by the nanocement ionic extracts. The in vivo neo-blood vessel formation in CAM model was significantly higher by the nanocement implant when compared with the CPC counterpart, implying the Si ion release might play a significant role in pro-angiogenesis. Furthermore, the early bone forming response of the nanocement, based on the implantation in a rat calvarial bone defect, demonstrated a sign of osteoinductivity along with excellent osteocondution and bone matrix formation. Although more studies remain to confirm the potential of nanocement, some of the intriguing physico-chemical properties and the biological responses reported herein support the promise of the new 'nanopowder-based nanocement' for hard tissue repair and regeneration.

Abstract:Gypsum is a promising material for bone defect repair due to its osteoconductivity, whereas it is still limited in orthopedic and dental surgeries due to its low bioactivity and too rapid resorption so that one major concern is the significant loss in microstructural stability in vivo. In the present strategy some key features were significantly improved by introducing rapidly biodegradable but highly bioactive calcium silicate (CS) for regulating the physicochemical properties and biological performances of the gypsum-based cements at the same liquid/solid ratio. We demonstrated that introduction of 23% CS into beta-calcium sulfate hemihydrate (CSH) could improve the physicochemical properties but would not compromise the mechanical strength of the composite. The surface bioactivity was significantly enhanced by introducing 23% CS, and these biphasic composites were favorable for decelerating the biodegradation rate by nearly 18.5% in 28 days in vitro. A mild bioresorption rate, with 39.6% of composite residual 4 weeks after operation, was determined when implanted in subcutaneous tissue of rats. 8 weeks after implantation, the composite cement containing 23% CS significantly enhanced new bone tissue regeneration with a much higher relative bone content (similar to 68.6%) than pure gypsum in critical size femoral defects in rabbits. The novel CSH-CS biocements represent promising candidates for rapid bone resconstruction and repair in trauma and pathological conditions.

Calcium phosphate cements (CPC) are currently widely used bone replacement materials with excellent bioactivity, but have considerable disadvantages like slow degradation. For critical-sized defects, however, an improved degradation is essential to match the tissue regeneration, especially in younger patients who are still growing. We demonstrate that a combination of CPC with mesoporous bioactive glass (MBG) particles led to an enhanced degradation in vitro and in a critical alveolar cleft defect in rats. Additionally, to support new bone formation the MBG was functionalized with hypoxia conditioned medium (HCM) derived from rat bone marrow stromal cells. HCM-functionalized scaffolds showed an improved cell proliferation and the highest formation of new bone volume. This highly flexible material system together with the drug delivery capacity is adaptable to patient specific needs and has great potential for clinical translation. Abstract:The 17p13.1 deletion that causes loss of the p53‐encoding TP53 gene is the most powerful predictor of a poor response to conventional therapy and shortened survival in patients with chronic lymphocytic leukemia (CLL). The results of this study have demonstrated that the cladribine and cyclophosphamide regimen may improve treatment results in this poor‐risk patient population.

Abstract:In this study, calcium phosphate (CP)/calcium sulfate biphasic bone repair materials were modified with bioactive-glass (BG) to construct a self-curing bone repair material. Tetracalcium phosphate, calcium hydrogen phosphate dihydrate, and calcium sulfate hemihydrate (CSH) with different BG ratios and phosphate solution were reacted to prepare a porous self-curing bone repair material (CP/CSH/BG). The solidification time was about 12 min, and the material was morphologically stable in 24 h. The porosity was about 50%, with a pore size around 200 mu m. The strength of CP/CSH/BG was approaching trabecular bone, and could be gradually degraded in Tris-HCl solution. MC3T3-E1 cells were cultured in the leaching solution of the materials. Cytotoxicity was detected using Cell Counting Kit 8 assays, and the expression of osteogenesis-related biomarkers was detected using quantitative real-time reverse transcription PCR (qRT-PCR). The results showed that all BG groups had increased ALP and ARS staining, implying that the BG groups could promote osteoblast mineralization in vitro. qRT-PCR showed significant upregulation of bone-related gene expression (Osx, Ocn, Runx2, and Col1) in the 20% BG group (p < 0.05). Therefore, the CP/CSH/BG self-curing bone repair materials can promote osteogenesis, and might be applied for bone regeneration, especially for polymorphic bone defect repair.

However, the low porosity of MPC prevents growth factors and osteoblasts from fully growing into the material, thereby limiting its clinical use. In this study, different concentrations (0-5%) of calcium carbonate and citric acid (CA) were used as foaming agents to prepare porous MPC. The MPC containing 3% CaCO3/CA exhibited the best physicochemical properties, including greater porosity, improved injectability, extended setting time, and decreased hydration temperature. The proliferation and adhesion of cells on 3%CaCO3/CA-MPC were higher than those on MPC alone. To explore its osteogenesis in vivo, 3% CaCO3/CA-MPC and Bio-Oss® bone powder were implanted into periodontal bone defects in rats for 4 weeks and 12 weeks, respectively. Micro-CT and histological analysis demonstrated the improved bone regeneration of 3%CaCO3/CA-MPC compared to the blank group (P < 0.05); it had slightly lower bone regeneration than the Bio-Oss® group but no statistical difference. The results indicated that porous MPC foamed with calcium carbonate and CA improved its physicochemical properties and enhanced its biocompatibility, making it a promising material for bone regeneration.

Abstract:Artificial bone substitutes for bone repair and reconstruction still face enormous challenges. Previous studies have shown that calcium magnesium phosphate cements (CMPCs) possess an excellent bioactive surface, but its clinical application is restricted due to short setting time. This study aimed to develop new CMPC/carboxymethyl chitosan (CMCS) comg of mixed powders of active MgO, calcined MgO and calcium dihydrogen phosphate monohydrate. With this novel strategy, it can adjust the setting time and improve the compressive strength. The results confirmed that CMPC/CMCS composite bone cements were successfully developed with a controllable setting time (18-70 min) and high compressive strength (87 MPa). In addition, the composite bone cements could gradually degrade in PBS with weight loss up to 32% at 28 d. They also promoted the proliferation of pre-osteoblasts, and induced osteogenic differentiation. The findings indicate that CMPC/CMCS composite bone cements hold great promise as a new type of bone repair material in further and in-depth studies.

Abstract:Osteoporotic vertebral compression fractures are a global issue affecting the elderly population. To explore a new calcium silicate bone cement, polylactic acid (PLGA)–polyethylene glycol (PEG)–PLGA hydrogel was compounded with tricalcium silicate (C3S)/dicalcium silicate (C2S)/plaster of Paris (POP) to observe the hydration products and test physical and chemical properties. The cell compatibility and osteogenic capability were tested in vitro. The rabbit femoral condylar bone defect model was used to test its safety and effectiveness in vivo. The addition of hydrogel did not result in the formation of a new hydration product and significantly improved the injectability, anti-washout properties, and in vitro degradability of the bone cement. The cholecystokinin octapeptide-8 method showed significant proliferation of osteoblasts in bone cement. The Alizarin red staining and alkaline phosphatase activity test showed that the bone cement had a superior osteogenic property in vitro. The computed tomography scan and gross anatomy at 12 weeks after surgery in the rabbit revealed that PLGA-PEG-PLGA/C3S/C2S/POP was mostly degraded, with the formation of new bone trabeculae and calli at the external orifice of the defect. Thus, PLGA-PEG-PLGA/C3S/C2S/POP composite bone cement has a positive effect on bone repair and provides a new strategy for the clinical application of bone tissue engineering materials.

Abstract:Calcium phosphate cement (CPC) is an excellent bone graft material due to its self-setting, easiness to shape, good biocompatibility and osteoconductivity. However, low osteogenic activity and slow degradation rate of CPC severely influence its bone regeneration efficiency. Herein, zinc silicate (ZS) and poly (lactic-co-glycolic acid) (PLGA) microspheres were incorporated into CPC to overcome these issues. The addition of ZS obviously downregulated the expression of inflammatory-related genes (interleukin-1β, interleukin-6, and tumor necrosis factor α), while markedly upregulated the anti-inflammatory gene expression (interleukin-10). The incorporation of ZS significantly promoted the adhesion and proliferation of mouse bone marrow stem cells (mBMSCs). In addition, macrophage-conditioned ZS/PLGA/CPC extracts profoundly promoted osteogenic differentiation of mBMSCs, and evidently suppressed osteoclastogenesis of RAW264.7 cells. The optimum added amount of ZS was 10 wt.%. In vivo results also displayed that the addition of 10 wt.% ZS significantly improved bone repair effect of the composite. Therefore, the addition of ZS together with PLGA microspheres is a promising way for modifying the osteogenic activity and degradability of CPC via in-situ pore generation, promoting osteogenic differentiation and macrophage immunomodulation due to the simultaneous release of active silicon and zinc ions.

Abstract:Network Traffic Classification using classical techniques such as port number based and payload based is becoming very difficult because many applications use dynamic port number and encryption technique to avoid detection. To overcome the drawbacks of classical techniques various machine learning techniques were proposed. Machine learning technique faces the problem of labeled instances (in supervised learning) and time consuming manual work (in unsupervised learning). To address the above problems we proposed a semi supervised machine learning technique. The key idea of proposed technique is to build a classifier from training dataset consisting of both labeled and unlabeled instances. For experimental purpose KDD CUP 99 intrusion detection dataset and MATLAB tool is used. We evaluate and compare the performance of the classifier build with 10%, 20% and 30% labeled instances in training dataset. The result of experiments show that classifier build with 30% labeled instances in training dataset has better performance at number of clusters equals to 50.
