Abstract:Objective To investigate the changing characteristics of muscle mass by dual energy X-ray absorptiometry (DEXA) in middle aged and elderly people with different ages.Methods This study included 343 healthy people aged 50-89 years (178 males and 165 females).Subjects were divided into 4 groups:aged 50-59,60-69,70-79,and 80-89 years group.Fat-free body mass (FFM),the appendicular lean body mass (aLM),trunk lean mass (tLM) were measured by DEXA.The aLM/body weight (Wt％) and aLM index (ASMI) were calculated by DEXA.Results The tLM in both gender was decreased in aged 80-89 years group than in aged 50-59 years group [male (10.6± 1.8) kg vs.(18.4±2.9) kg,female (7.1±1.2) kg vs.(14.8±2.1) kg,F=5.7 and 7.8,P＜ 0.01].The average ASMI in both gender was decreased in aged 70-79 and 80-89 years groups than in aged 50-59 years groups [male (5.5±0.4) kg/m2,(4.9±0.7) kg/m2 vs.(6.1±0.5) kg/m2,female (4.3±0.4) kg/m2,(3.9±0.5) kg/m2 vs.(4.8±0.6) kg/m2,F=24.9 and 9.6,both P＜0.01].The quality of muscle mass was decreased with aging in the elderly.Conclusions Ageing is an important risk factor for sarcopenia in the elderly.DEXA can be used to evaluate the muscle mass in middle aged and the elderly,and ASMI can be used as a reliable measurement index.
Age- and sex-related differences in muscle strength and physical performance in older Chinese

Abstract:Background Little is known about muscle strength and physical performance in Chinese. Aim This study aimed to assess the age- and sex-related differences in muscle strength and physical performance in older Chinese. Methods Three hundred and eight healthy participants (110 males and 198 females) age 68.3 ± 6.1 (mean ± SD) years were enrolled in this cross-sectional study. The handgrip muscle strength (HGS) of the dominant hand was measured using a Jamar dynamometer. Physical performance was assessed by the Timed Up and Go test (TUG). The EuroQol five-dimension questionnaire (EQ-5D) was used to evaluate participants’ health status. Results Men showed higher levels of HGS with a smaller percentage having low muscle strength compared with women. No differences were observed in TUG between sexes. No significant association of TUG and age was observed in males. However, older females had increased TUG and hence poorer performance. Good health status was associated with better physical performance but was not related to muscle strength in either sex. Discussion In men, there was no correlation between age and TUG, although a negative association with handgrip muscle strength was observed. For women, both muscle strength and physical performance declined with age. The sex-related differences in aging effects on physical performance in our study could partly explain why women have a higher incidence of hip fracture than men. Conclusion Chinese women may be more vulnerable to severe sarcopenia in old age than men.
Association Between Low Handgrip Strength And 90-Day Mortality Among Older Chinese Inpatients: A National Multicenter Prospective Cohort Study

Abstract:Background: The knowledge of the association between low handgrip strength and mortality among older Chinese inpatients is limited. Given China's aging society, a great number of older adults require hospital admission. Objective: To explore the association between low handgrip strength and 90-day mortality, providing evidence for clinicians to predict the risk of mortality and improve clinical outcomes for older inpatients. Materials and Methods: We conducted a national multicenter cohort study with a baseline survey from October 2018 to February 2019 and followed up for 90 days to record mortality outcomes. The assessment of handgrip strength was conducted using a hand dynamometer with the cutoff (handgrip strength < 28 kg for men and < 18 kg for women) to define low handgrip strength. Multivariable logistic regression analysis was applied to explore the association between low handgrip strength and 90-day mortality. Results: A total of 8,910 older Chinese inpatients [mean (SD) age, 72.39 (5.68) years; 3,750 women (42.09%)], with a prevalence of low handgrip strength, at 49.57%, were included. Compared to inpatients with normal handgrip strength, inpatients with low handgrip strength were older, had less education, more were female, had lower activities of daily living (ADL) score, had lower BMI, higher frailty, higher rates of depression, and poorer cognitive function (all p < 0.05). At 90 days, after adjusting for gender, age, education, frailty, depression, ADL score, malnutrition, and diagnosis, low handgrip strength was independently associated with 90-day mortality, compared to normal handgrip strength (OR = 1.64, 95% CI:1.14-2.37; P = 0.008). Additionally, subgroup and interaction analysis showed a significant interaction effect (P = 0.031) between two age groups (65-74 years older vs. >= 75 years old), with the OR being 3.19 (95%CI:2.07-4.93) and 1.49 (95%CI:0.87-2.55), respectively. Conclusion: Older Chinese inpatients with low handgrip strength had a 1.64-fold risk of 90-day mortality, compared to those with normal handgrip strength, indicating that clinicians need to screen early for handgrip strength and recommend corresponding interventions, such as resistance training and nutrition, as a priority for older inpatients.
Age-Related Changes of Skeletal Muscle Mass and Muscle Strength in Chinese Urban Residents

Abstract:Purpose:To investigate age-related change of skeletal muscle mass and muscle strength in Chinese urban residents.Methods:A total of 1 013 Chinese urban residents(468 men,545women)aged 3~69 in Jinan,Guangzhou,Chengdu and Xi'an participated in this study.The body weight,height,hand grip and some other physical fitness variables were measured according to the handbook of national physical fitness surveillance in 2005.The skeletal muscle mass were measured by dual energy X-ray absorptiometry(DEXA),and leg strength were measured by Muscle Evaluation System(MES).One way ANOVA was used to compare selected variables among different age groups.Results and conclusions:1) skeletal muscle mass of Chinese urban residents increased from 3 to 19 years old.Both men and women got their peak muscle mass level at age of 20~24 years.Then,the muscle mass began to decreased slowly.2) Both hand grip strength and leg strength of Chinese urban residents kept increasing since their birth until 30~40 years old.After that,hand grip strength began to slowly decrease.Compared with hand grip strength,the leg strength of Chinese urban residents decreased more quickly.3) The decrease of muscle strength is caused by both decrease in muscle mass and decrease in muscle strength per unit.
Age-Related Decline in Skeletal Muscle Mass and Function among Elderly Men and Women in Shanghai, China: A Cross Sectional Study

Abstract:Objective: To investigate the relationship of muscle mass and muscle function with age. Methods and Study Design: The study including 415 participants (aged 60-99 years). Upper (UMM) and lower (LMM) limbs muscle mass and whole body fat free mass (FFM) were measured by bioelectrical impedance analysis. The appendicular skeletal muscle mass (ASM) index (ASM/height(2)) was calculated. Muscle function was assessed by measuring hand grip strength (HGS) and gait speed. Results: Using ASM index cutoff values we found that higher prevalence of sarcopenia in women than in men (33.5% vs 23.6%, p=0.025). In the upper limb, HGS (beta=-0.809) declined more rapidly with age than did UMM (beta=-0.592) in men, but not in women (beta=-0.389 and (beta=-0.486 respectively). In the lower limb, gait speed declined more rapidly than LMM in both men (beta=-0.683 vs beta=-0.442) and women (beta=-1.00 vs (beta=-0.461). The variance of UMM explained 28-29% of the variance of HGS, and LMM explained 7-8% of the variance of gait speed in women and men respectively. In addition to the common predictors (BMI and age), the specific predictors were smoking, exercise and education for FFM and ASM, and smoking, drinking and exercise for HGS (p<0.05). Conclusions: Loss of muscle function is greater than the decline of muscle mass particularly in the upper limbs in men. However, women are more prone to have low muscle mass than the men. Exercise programs need to be designed gender specifically.
Associations of lower-limb muscle strength performance with static and dynamic balance control among older adults in Taiwan

Abstract:Background: Aging is an inevitable process of life development. These physical changes can cause a decline in the functional adaptability and health status of older adult individuals. Aims: The purpose of this study was to investigate the association of lower-limb muscle strength performance with static and dynamic balance control among older adults in Taiwan. Methods: We conducted a cross-sectional study and reviewed data derived from the National Physical Fitness Survey in Taiwan 2015–2016. A total of 20,846 Taiwanese older adult individuals aged 65 years old or older were recruited as study participants. Demographic characteristics, anthropometric assessments, lifestyle habits, and health-related physical fitness measurements from this dataset were analyzed using the chi-square test, one-way analysis of variance, and linear regression analysis. Lower-limb muscle strength performance was classified into 4 groups based on quartile (Quartile 1 [Q1], Quartile 2 [Q2], Quartile 3 [Q3], and Quartile 4 [Q4]) values. Results: Increased lower-limb muscle strength levels were significantly associated with static balance in men (Q2: β = 2.539, p < 0.0001; Q3: β = 4.590, p < 0.0001; Q4: β = 7.700, p < 0.0001) and women (Q2: β = 2.022, p < 0.0001; Q3: β = 4.179, p < 0.0001; Q4: β = 6.920, p < 0.0001) after adjusting for potential confounders. In addition, we observed that increased lower-limb muscle strength levels were significantly associated with dynamic balance in men (Q2: β = −1.661, p < 0.0001; Q3: β = −2.434, p < 0.0001; Q4: β = −3.091, p < 0.0001) and women (Q2: β = −1.660, p < 0.0001; Q3: β = −2.548, p < 0.0001; Q4: β = −3.196, p < 0.0001) after adjusting for potential confounders. Conclusion: Lower-limb muscle strength was the most important factor, as it was an improved method for static and dynamic balance control in both genders.

Abstract:The associations of sarcopenia with adverse health status have highlighted the importance of sarcopenia research and intervention. This study was designed to analyze the characteristics of aging-related differences in appendicular skeletal muscle mass (ASM), handgrip strength (HS), gait speed (GS) and their associated factors in older Chinese, in order to generate guidance for sarcopenia intervention in this population.Population-based cross-sectional study. The criteria proposed by Asian Working Group for Sarcopenia were used to define low ASM, HS, and GS. The time required for five repeated chair stands (RCS) was also measured to evaluate physical performance. The differences of continuous variables were compared using one-way ANOVA tests and the Pearson correlation was used to analyze the relationship of each measurement adjusted by gender and age. Stepwise logistic regression was used to determine associated factors of low HS and low physical performance.The data were analyzed in a total of 218 younger adults (aged 20-59, 76 males, 142 females) and 461 older adults (≥60 year, 207 males and 254 females). There were significant differences among age groups for HS, GS, and RCS while females were found to have significantly lower HS and GS values. ASM was significantly correlated with HS but not with other measures. Correlations among HS and GS, RCS were influenced by age differences. In the older group, unstructured daily routine (OR = 2.77) was associated with the risk of low GS, while physical exercise (OR = 0.27), and engaging in hobbies (OR = 0.11) were associated with faster GS. Co-morbidity (OR = 1.99) was associated with the risk of reduced performance of RCS, while engaging in hobbies was associated with faster RCS performance (OR = 0.35).Muscle strength and physical performance varied with aging in older Chinese. Measures of GS, HS, and RCS provide a readily available and effective method for assessing the risk of functional mobility decline. Maintaining a healthy life style and physical activity throughout life is beneficial for older people to improve their physical performance, especially in the early stages of aging.
Correlation between hand grip strength and regional muscle mass in older Asian adults: an observational study

Abstract:Abstract Background Previous research has demonstrated a correlation between hand grip strength (HGS) and muscle strength. This study aims to determine the relationship between HGS and muscle mass in older Asian adults. Methods We retrospectively reviewed the dual-energy X-ray absorptiometry (DXA) records of 907 older adults (239 (26.4%) men and 668 (73.6%) women) at one medical institution in Taipei, Taiwan, from January 2019, to December 2020. Average age was 74.80 ± 9.43 and 72.93 ± 9.09 for the males and females respectively. The inclusion criteria were: 1) aged 60 and older, 2) underwent a full-body DXA scan, and 3) performed hand grip measurements. Patients with duplicate results, incomplete records, stroke history, and other neurological diseases were excluded. Regional skeletal muscle mass was measured using DXA. HGS was measured using a Jamar handheld dynamometer. Results Total lean muscle mass (kg) averaged 43.63 ± 5.81 and 33.16 ± 4.32 for the males and females respectively. Average HGS (kg) was 28.81 ± 9.87 and 19.19 ± 6.17 for the males and females respectively. In both sexes, HGS and regional muscle mass consistently declined after 60 years of age. The rates of decline per decade in upper and lower extremity muscle mass and HGS were 7.06, 4.95, and 12.30%, respectively, for the males, and 3.36, 4.44, and 12.48%, respectively, for the females. In men, HGS significantly correlated with upper ( r = 0.576, p < 0.001) and lower extremity muscle mass ( r = 0.532, p < 0.001). In women, the correlations between HGS and upper extremity muscle mass ( r = 0.262, p < 0.001) and lower extremity muscle mass ( r = 0.364, p < 0.001) were less strong, though also statistically significant. Conclusion Muscle mass and HGS decline with advancing age in both sexes, though the correlation is stronger in men. HGS measurements are an accurate proxy for muscle mass in older Asian adults, particularly in males.

Abstract:Objective: To describe the regional and population-related differences in skeletal muscle mass and handgrip strength across the 10 regions of China. Methods: 24 533 participants aged 38-88 years from the second resurvey of China Kadoorie Biobank were included in our analyses. Appendicular and trunk skeletal muscle mass were assessed using the bioelectrical impedance analysis (TANITA). Handgrip strength was measured using Jamar hand-held dynamometer. Low muscle mass and low muscle strength were defined as the lowest quintile of height-adjusted appendicular muscle mass or handgrip strength according to the Consensus Report of the Asian Working Group for Sarcopenia. We analyzed the mean value of absolute muscle mass, height-adjusted muscle mass, weight-adjusted muscle mass and handgrip strength. We also reported the prevalence of low muscle mass and low muscle strength. Results: The average appendicular and total skeletal muscle mass were (22.0±0.02) kg and (49.7±0.05) kg in men, which were higher than in women [(15.9±0.02) kg and (37.2±0.04) kg, respectively]. The handgrip strength was (32.6±0.06) kg in men, which was higher than (19.9±0.05) kg in women. The absolute muscle mass was higher in north area and urban region (P<0.001). The weight-adjusted muscle mass showed reverse patterns of regional difference compared with height-adjusted muscle mass. Both muscle mass and handgrip strength decreased by age (trend P<0.001), with a larger decline observed in handgrip strength. According to AWGS criteria, the proportions of low muscle mass and strength increased by age. Among participants over 80 years old, the prevalence of low muscle mass and strength were 56.2% and 74.5% in men, and 35.7% and 66.0% in women. Conclusions: Levels of skeletal muscle mass and strength varied greatly among people from 10 regions and among participants with different demographic characteristics. The prevalence of low muscle mass and strength was extremely high in elderly.

Abstract:Abstract Background: The association between regional skeletal muscle mass and metabolic syndrome (MetS) remains unclear. Therefore, the present study aimed to investigate these associations with MetS risk among Chinese elders.Methods: On the basis of health check-up program, a case-control study was performed among participants over 65 years of age, 250 MetS participants were identified and 750 healthy subjects were randomly selected as controls. Skeletal muscle mass was estimated via bioelectric impedance analysis. Muscle mass of each region was standardized by weight to obtain its muscle mass index. Conditional logistic regression models and restricted cubic spline models were implemented to evaluate the associations between MetS and the muscle mass in different regions.Results: As compared the highest category with the lowest category, leg muscle mass index (LMI) (OR = 0.12; 95%CI: 0.05, 0.32; P for trend < 0.001), trunk muscle mass index (TMI) (OR = 0.11; 95%CI: 0.04, 0.29; P for trend < 0.001) and skeletal muscle mass index (SMI) (OR = 0.11; 95%CI: 0.04, 0.29; P for trend < 0.001) were inversely associated with MetS risk after adjusting for age, gender, duration of education, exercise, smoking status, alcohol drinking status, total energy intake, proteins, fats, carbohydrates, serum fasting glucose and lipid profiles. Dose-response analysis showed that per standard deviation increment of LMI (OR = 0.45; 95%CI: 0.34, 0.58; P for trend < 0.001), TMI (OR = 0.39; 95%CI: 0.28, 0.53; P for trend < 0.001) and SMI (OR = 0.29; 95%CI: 0.19, 0.43; P for trend < 0.001) were inversely associated with MetS risk, respectively.Conclusion: The present results showed that higher LMI, TMI and SMI were associated with the low risk of MetS in Chinese elders.

Abstract:In this cross-sectional study, we assessed the physical fitness levels of active community-dwelling older adults. Moreover, we investigated the correlation of their (stratified by age and sex) fitness levels with handgrip strength (HGS). Comprehensive physical fitness tests, including sarcopenia screening, were conducted with a total of 2,130 older adults residing in a rural area of Taiwan. The 5th, 10th, 25th, 50th, 75th, 90th, and 95th percentiles of age- and sex-specific physical fitness levels were determined. Furthermore, we identified the key parameters for assessing the physical fitness of older adults and performed stepwise multiple linear regression analysis. Both men and women exhibited age-related decreases in all aspects of functional fitness, a trend indicating that older adults in Taiwan may lose their independence in the future. Furthermore, the regression analysis revealed that HGS was positively correlated with sex, body mass index, and the results of 30-s arm curl and back scratch tests but negatively correlated with age and the result of 8-foot up-and-go test. Our reference values for physical fitness may help assess the fitness levels of active community-dwelling older adults and design community-based health programs to prevent the early loss of independence in community-dwelling older adults in Taiwan.

Physical activity, physical frailty and depressive symptoms among Chinese male and female older adults: do different indicators make a difference?

Abstract:Objectives: Older adults become more inactive and frailer with aging. Physical status is closely linked to mental health, but it is unclear which physical indicator is more strongly associated with depressive symptoms in older adults. The present study aimed to compare relationships between self-reported physical activity, physical frailty (muscle mass, muscle strength, and gait ability) and depressive symptoms in community male and female older adults. Methods: A total of 1,180 adults aged 60 years and older were recruited to participate in this study from a Chinese community receiving annual check-up service from September 2018 to May 2019. Physical activity was assessed by the International Physical Activity Questionnaire (IPAQ). The Bio-electrical Impedance Analyzer was used to determine the muscle mass. As the indicators of muscle function, grip strength and gait ability were assessed by the dynamometer and Timed Up and Go Test (TUGT), respectively. The 15-item version of Geriatric Depression Scale (GDS-15) was used to examine depressive symptoms. Demographic variables, health status and sleep quality were collected using questionnaire. Results: 11.8% men and 11.9% women reported depressive symptoms. Logistic regression showed that depressive symptoms was associated with low grip strength ( OR = 2.42, 95% CI : 1.04–5.63), slow gait ability ( OR = 3.60, 95% CI : 1.28–10.13) in older males, and associated with low level of self-reported physical activity ( OR = 3.85, 95% CI : 2.00–7.42) in older females. No significant association was found between muscle mass and depressive symptoms. Conclusion: There were gender differences in the relationship between physical activity, physical frailty, and depressive symptoms. Grip strength and gait ability may be a better indicator of frailty for predicting depressive symptoms in older men while physical activity may be useful in predicting depressive symptoms in older women.

Abstract:Objective:To explore the main influencing factors for physical performance assessed by the short physical performance battery(SPPB)in the elderly in communities in Shanghai.Methods:A total of 500 elderly subjects from four communities in Shanghai who met the inclusion criteria with 165 males and 335 females aged 60-95 years(70.5±7.8)years were enrolled.A questionnaire was used to assess the general situation.International Physical Activity Questionnaire-Short Form(IPAQ-SF)was used to assess physical activity(PA). The appendicular skeletal muscle mass(ASM)was measured by bioelectrical impedance analysis, and the relative skeletal muscle mass index(RSMI). The lower extremity muscle strength(quadriceps strength, QS)was measured by a hand-held dynamometer.Lower body muscle quality(LB-MQ) was ascertained by taking the ratio of strength to muscle mass for lower extremity.SPPB score was used to assess physical activity, including full tandem test, 4-meter walking speed test and five timed chair stands test.Results:The age, RSMI, ASM and quadriceps strength were higher, and the body fat content was lower in males than in females(all P<0.001). There were no significant differences in BMI, SPPB scores, LBMQ, and PA between males and females( P>0.05). Pearson correlation analysis showed that SPPB score was negatively correlated with age( r=-0.615, P<0.001)and body fat content( r=-0.103, P=0.022), but SPPB score was positively correlated with quadriceps strength( r=0.351, P<0.001), RSMI( r=0.134, P=0.003), LBMQ( r=0.268, P<0.001)and amount of PA( r=0.204, P<0.001), however, SPPB score had no relationship with gender( r=0.068, P=0.132). Conclusions:Aging and frailty are the main factors for the decline of physical performance in the elderly.Participating physical activity, reducing body fat, maintaining muscle mass, and especially increasing muscle quality, may be the main interventions to prevent the further deterioration of physical performance in the elderly.
Exercise, disability and physical functioning in older Chinese adults during their last six years of life: a cohort study

Abstract:Abstract Background Little is known about the potential effects of exercise behaviour on disability and physical functioning (PF) in late-life. This study aimed to investigate whether exercise behaviour with four different long term exercise status influences disability and PF in older Chinese adults during their last six years of life. Methods Secondary data analyses were performed using participants (≥ 65 years) from the Chinese Longitudinal Healthy Longevity Study. Exercise behaviour was collected every three years during their last six years of life. 4265 participants were enrolled and divided into four groups, including long-term non-exercising (NN), non-exercising to exercise (NY), exercise to non-exercising (YN), and long-term exercising (YY). Disability was measured based on the Katz Index of Activities of Daily Living scale and divided into three disability levels. PF was included as chair-rise, pick-up book and turn-around. Risks burden in disability and PF among four groups were compared using logistic regression analyses. Results Compared with NN, the risk of developing severe disability was attenuated in YY (OR 0.11, 95%CI 0.06 to 0.21) and attenuated in NY (OR 0.27, 95%CI 0.17 to 0.44) and increased in YN (OR 1.36, 95%CI 1.08 to 1.72). Moderate disability risk was increased in YN (OR 1.85, 95%CI 1.26 to 2.71) only. The risk of developing physical dysfunction ( chair-rise, pick-up book, and turning around 360°) was decreased in YY and NY, and did not have significant difference in YN. Conclusions EP were associated with disability and PF in older Chinese adults during their late-life. Late-life exercise behaviour is still beneficial. Our findings suggest that it is meaning to encourage older people doing EP at late-life in China.
Longitudinal study on the progression of muscle status among community-dwelling ambulatory older multiethnic Asians with type 2 diabetes mellitus

Abstract:Background: Muscle health decline with age, but its deterioration in older persons with type-2 diabetes mellitus (T2DM) over time is not well-established. This study aimed to determine the change in muscle mass, handgrip strength and gait speed over time among community-dwelling ambulatory older multi-ethnic Asian patients with T2DM and their associated factors. Methods: Among 387 eligible patients aged 60-89 years who were recruited at baseline, 348 (89.9%) were reviewed at a public primary care clinic in Singapore in the subsequent 9 to 34 months. The change in their clinical and functional status, levels of physical activity and muscle status (mass, grip strength and gait speed based on the Asian Working Group for Sarcopenia criteria) were recorded and compared. Their physical activity levels were assessed using the Physical Activity Scale for the Elderly (PASE) and International Physical Activity Questionnaire (IPAQ). Their quality of life was evaluated based on the World Health Organization Quality of Life (WHOQOL) domains. Linear regression analysis was used to identify the factors associated with muscle health change. Results: The study population comprised men (52.9%), Chinese (69.3%), mean age of 68.4 ± SD5.6 years and had at least secondary education (76.4%). Their mean muscle mass significantly decreased by 0.03 ± SD0.06 kg/m2/month, mean handgrip strength by 0.06 ± SD0.26 kg/month and negligible change in gait speed of 0.002 ± SD0.01 m/sec/month. Their mean weight significantly decreased by 0.5 ± SD3.9 kg, waist and hip circumferences by 2.5 ± SD6cm and 3.2 ± SD5.1 cm respectively, with no change in BMI. Linear regression shows significant associations between muscle mass change and education level (β = 0.36,p = 0.012, 95% CI = 0.08-0.64), BMI (β = 0.11,p = < 0.001, 95% CI = 0.05-0.17), change in medication class (β = 0.39,p = < 0.001, 95% CI = 0.06-0.71) and review interval (β = - 0.003, p < 0.001, 95% CI = -0.004--0.002). Gait speed change was associated with singlehood (β = - 0.13,p = 0.029, 95% CI = -0.25--0.01) and WHOQOL physical health (β = 0.01,p = 0.024, 95% CI = 0.00-0.02) domain. No factor was associated with handgrip strength change. Conclusions: The study population with T2DM showed significant decline in their mean weight, waist and hip circumferences, mean muscle mass and mean grip strength but gait speed was unaffected. Muscle mass change was associated with education level, BMI and length of review interval. Handgrip strength change was not significantly correlated with any factor. Gait speed change was associated with singlehood and physical health.

Abstract:BackgroundFall episodes are not unusual among community residents, especially the elderly, and lower muscle strength is an important issue to address in order to prevent falls.MethodsA community health survey was conducted in a suburban area of Taiwan, and 1067 older adults were selected for enrollment in the present study. All the enrolled subjects had been visited at their homes; the subjects’ strength of both hands and muscle mass of both legs were measured and well-established questionnaires were finished by certificated paramedic staffs.ResultsThe incidence of fall episodes in the previous 1 year in the Yilan elderly population was 15.1%, and the female predominance was significant. A significantly higher prevalence of cataracts was found in group who experienced a fall in the past year (64% vs. 54.9% in the non-fall group). Mild or more severe dementia was much more prevalent in the group who experienced a recent fall (33.8% vs. 25.7% in the non-fall group). The strength of both hands tested as the physical function was 17.6 ± 8.0 kg in the recent fall group, significantly weaker than that in the non-fall group (20.7 ± 8.7 kg). Multivariate regression analysis revealed a greater weekly exercise duration and greater strength of both hands reduced the occurrence of falls among the whole and the female population. The standardized effect sizes of hand grip strength between both groups, not trivial, were 0.29 and 0.37 for the total population and the female subpopulation respectively.ConclusionsLess weekly exercise duration and weaker muscle strength were f ound to be independent risk factors of fall episode(s) in an elderly Taiwanese population, especially in the female sub-population. Muscle strength, measured by average of both hands grip strength, was the most significantly factor of one-year fall episode(s) accessed retrospectively.

Abstract:Objective: To assess the status of handgrip strength of elderly population from longevity areas in China, and to analyze the correlative factors of handgrip strength of elderly people. Methods: Data from Chinese Longitudinal Healthy Longevity Survey (CLHLS) in 2012 was used, from which1 967 participants aged ≥60 years old with valid data of grip strength value from 8 Chinese longevity areas were included. Information on demographics characteristic, life style and health status was collected using questionnaires. The handgrip strength of both left and right hands were measured by grip dynamometer. The different characteristics of group of participants with different grip strength were compared and then analyzed by adopting the Cumulative odds Logistic regression model to identify main factors associated with hand grip strength. Results: The P(50) (P(25)-P(50)) of hand grip strength of elderly people from the eight longevity areas was 20 (11-28) kg; The hand grip strength of males was 26 (18-34) kg, which was higher than that of females(14 (9-20) kg) (P<0.001). Cumulative odds Logistic regression model showed that the hand grip strength of females was lower than males, whose β value (95%CI) was-1.22 (-1.43--1.00). The elderly who was at a higher age, smoking, drinking or with anemia, had a comparatively lower handgrip strength, whose β (95%CI) value were separately-0.08(-0.09~-0.07),-0.29(-0.56~-0.02),-0.54(-0.80~-0.28), and-0.41(-0.62~-0.20). And the elderly who had a higher boby mass index, drinking tea and outdoor activities, had a comparatively higher handgrip strength, whose β(95%CI) value were separately 0.28 (0.15-0.40), 0.25(0.03-0.47) and 0.51(0.30-0.71). Conclusion: Age and gender were the main correlative factors, lifestyles and physical conditions might also be correlative factors of hand grip strength of the elderly from longevity areas in China.

Abstract:Background: The combination of multiple disease statuses, muscle weakness, and sarcopenia among older adults is an important public health concern, and a health burden worldwide. This study evaluates the association between chronic disease statuses, obesity, and grip strength (GS) among older adults in Taiwan. Methods: A community-based survey was conducted every 3 years among older adults over age 65, living in Chiayi County, Taiwan. Demographic data and several diseases statuses, such as diabetes mellitus, hypertension, cerebrovascular disease, cardiovascular disease, and certain cancers, were collected using a questionnaire. Anthropometric characteristics were measured using standard methods. Grip strength was measured using a digital dynamometer (TKK5101) method. Results: A total of 3739 older individuals were recruited (1600 males and 2139 females) with the mean age of 72.9 years. The mean GS was 32.8 ± 7.1 kg for males and 21.6 ± 4.8 kg for females. GS significantly decreased most in males with cerebrovascular disease (from 33.0—29.5 kg, p < 0.001) and in females with diabetes mellitus (from 21.8—21.0 kg, p < 0.01). GS was highest in older adults with obesity (body mass index ≥ 27 kg/m2); however, there was no significant change of GS as the disease number increased. Conclusion: Older adults who have two, rather than one or greater than three chronic diseases, have significantly lower GSs than those who are healthy. Stroke and CKD for males, and hypertension and diabetes for females, are important chronic diseases that are significantly associated with GS. Furthermore, being overweight may be a protective factor for GS in older adults of both sexes.

Abstract:CDISC standard has become a set of global data standards that can be used in clinical study, covering the full life cycle of clinical researches. After nearly 20 years of development and continuous version upgrades, CDISC standard can improve the quality and efficiency of clinical research and drug review, and to facilitate all stakeholders involved in researches to exchange the study data and communicate the outcomes. CDISC standard has been or is to be adopted as standard format in data submission by multiple regulatory authorities, and more widely implemented by the global pharmaceutical community. CDISC standard is gradually adopted in China. The feasibility and roadmap of CDISC standard as the Chinese data submission format requirements are undergoing exploration and piloting further.
