Abstract:Background/Objective: Non-invasive neuromodulation techniques, such as repetitive Transcranial Magnetic Stimulation (rTMS) and transcranial Direct Current Stimulation (tDCS), have increasingly been investigated for their potential as treatments for neurological and psychiatric disorders. Despite widespread dissemination of these techniques, the underlying therapeutic mechanisms and the ideal stimulation site for a given disorder remain unknown. Increasing evidence support the possibility of non-invasive neuromodulation affecting a brain network rather than just the local stimulation target. In this article, we present evidence in a clinical setting to support the idea that non-invasive neuromodulation changes brain networks. Method: This article addresses the idea that non-invasive neuromodulation modulates brain networks, rather than just the local stimulation target, using neuromodulation studies in tinnitus and major depression as examples. We present studies that support this hypothesis from different perspectives. Main Results/Conclusion: Studies stimulating the same brain region, such as the dorsolateral prefrontal cortex (DLPFC), have shown to be effective for several disorders and studies using different stimulation sites for the same disorder have shown similar results. These findings, as well as results from studies investigating brain network connectivity on both macro and micro levels, suggest that non-invasive neuromodulation affects a brain network rather than just the local stimulation site targeted. We propose that non-invasive neuromodulation should be approached from a network perspective and emphasize the therapeutic potential of this approach through the modulation of targeted brain networks.

Abstract:During the past decade, it became clear that the electric field elicited by non-invasive brain stimulation (NIBS) techniques such as transcranial direct current stimulation (tDCS) and transcranial magnetic stimulation (TMS) are substantially influenced by variations in individual head and brain anatomy. In addition to structural variations in the healthy, several psychiatric disorders are characterized by anatomical alterations that are likely to further constrain the intracerebral effects of NIBS. Here, we present high-resolution realistic head models derived from structural magnetic resonance imaging data of 19 healthy adults and 19 patients diagnosed with major depressive disorder (MDD). By using a freely available software package for modelling the electric fields induced by different NIBS protocols, we show that our head models are well-suited for assessing inter-individual and between-group variability in the magnitude and focality of tDCS-induced electric fields for two protocols targeting the left dorsolateral prefrontal cortex.

Abstract:It has been recognized that the efficacy of TMS-based modulation may depend on the network profile of the stimulated regions throughout the brain. However, what profile of this stimulation network optimally benefits treatment outcomes is yet to be addressed. The answer to the question is crucial for informing network-based optimization of stimulation parameters, such as coil placement, in TMS treatments. In this study, we aimed to investigate the feasibility of taking a disease-specific network as the target of stimulation network for guiding individualized coil placement in TMS treatments. We present here a novel network-based model for TMS targeting of the pathological network. First, combining E-field modeling and resting-state functional connectivity, stimulation networks were modeled from locations and orientations of the TMS coil. Second, the spatial anti-correlation between the stimulation network and the pathological network of a given disease was hypothesized to predict the treatment outcome. The proposed model was validated to predict treatment efficacy from the position and orientation of TMS coils in two depression cohorts and one schizophrenia cohort with auditory verbal hallucinations. We further demonstrate the utility of the proposed model in guiding individualized TMS treatment for psychiatric disorders. In this proof-of-concept study, we demonstrated the feasibility of the novel network-based targeting strategy that uses the whole-brain, system-level abnormity of a specific psychiatric disease as a target. Results based on empirical data suggest that the strategy may potentially be utilized to identify individualized coil parameters for maximal therapeutic effects.

Abstract:Higher-order cognitive and affective functions are supported by large-scale networks in the brain. Dysfunction in different networks is proposed to associate with distinct symptoms in neuropsychiatric disorders. However, the specific networks targeted by current clinical transcranial magnetic stimulation (TMS) approaches are unclear. While standard-of-care TMS relies on scalp-based landmarks, recent FDA-approved TMS protocols use individualized functional connectivity with the subgenual anterior cingulate cortex (sgACC) to optimize TMS targeting. Leveraging previous work on precision network estimation and recent advances in network-level TMS targeting, we demonstrate that clinical TMS approaches target different functional networks between individuals. Homotopic scalp positions (left F3 and right F4) target different networks within and across individuals, and right F4 generally favors a right-lateralized control network. We also modeled the impact of targeting the dorsolateral prefrontal cortex (dlPFC) zone anticorrelated with the sgACC and found that the individual-specific anticorrelated region variably targets a network coupled to reward circuitry. Combining individualized, precision network mapping and electric field (E-field) modeling, we further illustrate how modeling can be deployed to prospectively target distinct closely localized association networks in the dlPFC with meaningful spatial selectivity and E-field intensity and retrospectively assess network engagement. Critically, we demonstrate the feasibility and reliability of this approach in an independent cohort of participants (including those with Major Depressive Disorder) who underwent repeated sessions of TMS to distinct networks, with precise targeting derived from a low-burden single session of data. Lastly, our findings emphasize differences between selectivity and maximal intensity, highlighting the need to consider both metrics in precision TMS efforts.

Abstract:Neuromodulation is based on the principle that brain stimulation produces plastic changes in cerebral circuitry. Given the intersubject structural and functional variability, neuromodulation has a personalized effect in the brain. Moreover, because of cerebral dominance and interhemispheric functional and structural differences in the same individual, the characterization of specific brain circuitries involved is currently not feasible. This notion is extremely important for neuromodulation treatments applied in neuropsychiatry. Specifically, the efficacy of the neuromodulation treatments is critically dependent on the anatomical precision of the brain target and the circuitry which has been affected. However, a complete understanding of how the brain behaves under stimulation needs a combined characterization of its neurophysiological response. This can be achieved by TMS-EEG guided by current multimodal neuroimaging techniques in real time. Herein, we present novel data based on dMRI tractography-guided TMS-EEG on one healthy young adult volunteer.

Abstract:Repetitive transcranial magnetic stimulation (rTMS) holds promise for treating psychiatric disorders; however, the variability in treatment efficacy among individuals underscores the need for further improvement. Growing evidence has shown that TMS induces a broad network modulatory effect, and its effectiveness may rely on accurate modulation of the pathological network specific to each disorder. Therefore, determining the optimal TMS coil setting that will engage the functional pathway delivering the stimulation is crucial. Compared to group-averaged functional connectivity (FC), individual FC provides specific information about a person's brain functional architecture, offering the potential for more accurate network targeting for personalized TMS. However, the low signal-to-noise ratio (SNR) of FC poses a challenge when utilizing individual resting-state FC. To overcome this challenge, the proposed solutions include increasing the scan duration and employing the cluster method to enhance the stability of FC. This study aimed to evaluate the stability of a personalized FC-based network targeting model in individuals with major depressive disorder or schizophrenia with auditory verbal hallucinations. Using resting-state functional magnetic resonance imaging data from the Human Connectome Project, we assessed the model's stability. We employed longer scan durations and cluster methodologies to improve the precision in identifying optimal individual sites. Our findings demonstrate that a scan duration of 28 minutes and the utilization of the cluster method achieved stable identification of individual sites, as evidenced by the intraindividual distance falling below the ~1cm spatial resolution of TMS. The current model provides a feasible approach to obtaining stable personalized TMS targets from the scalp, offering a more accurate method of TMS targeting in clinical applications.

Abstract:We read with interest the case report of de Jong et al. 1 describing two cases of spontaneous intracerebellar migration of a pseudomeningocele. In this report, the authors indicate that the probable mechanism is a slow but progressive cerebrospinal fl uid (CSF) movement into the pseudomeningocele with secondary herniation or dissection through a weakened dura into the cerebellum. We had met some cases with suboccipital pseudomeningocele following posterior fossa surgery. Although there are no migrations into the cerebellum in our cases, we would like to add our view on migrating pseudomeningocele causing posterior fossa syndrome. We believe that neck movement may drive the migration of a suboccipital pseudomeningocele. Iatrogenic pseudomeningoceles may or may not be associated with an arachnoid tear, but a dural tear is necessary for one to form. 2 Th e intradural pressure of the CSF causes a constant outfl ow of CSF through the dural tear, which forms a sac of epidural CSF collection in the traumatized postoperative bed of the cervical musculature. Th erefore, a communication is formed between the subarachnoid space and the epidural sac of CSF collection. If the arachnoid is not violated, the arachnoid membrane may subsequently herniate through the dural defect. When intact arachnoid herniates into the sac, the communication is more likely to remain open and form a pseudomeningocele, whereas when an arachnoidal tear occurs, the likelihood of closure of the communication is greater. Sometimes the communication is a one-way direction between the subarachnoid space and the sac in patients with the pseudomeningocele. Due to the formation of a one-way ball-valve with a tissue fl ap, only allowing a one-way fl ow of CSF into the pseudomeningocele. Occasionally, there is a lack of communication between the subarachnoid space and the sac in few patients; in this respect it resembles the speculation by de Jong et al. Th is lack of communication or a one-way communication is probably secondary to the scarring of soft-tissue wound. Anyway, a pseudomeningocele is to be formed in postoperative bed of the cervical musculatures. Th e sac of pseudomeningocele is extruded by the muscles and other soft tissues in the occipitocervical region of neck during the fl exion of the neck (Fig. 1), and extruded by the occipital bone and the supercervical spine during the extension of the neck (Fig. 2). In addition, the direction of the force generated by neck movement is from the extracranial space to the intracranial cavity. On the contrary, the direction of the force generated by CSF pulsation is from the intracranial cavity to the extracranial space. Th e repeated extrusion of the sac results in the increase of the sac cavity pressure, forming a pressure gradient between the sac and the cerebellum followed by the migration of CSF in the pseudomeningocele into the cerebellar parenchyma through a weak area in the dura. An Operational Approach for Optimizing Transcranial Direct Current Stimulation

Abstract:Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique that has been utilized for treating brain disorders and improving cognitive function. In order to achieve targeted tDCS, many optimization methods of montages and electric currents have been proposed. However, these methods have some limitations. Most of them were proposed for single-objective optimization (focality or intensity) and have no constrain with the number of electrodes (Most devices only have less than 8 electrodes currently). In this study, we proposed an operational optimization approach for well-targeted tDCS, which aims to optimize for two objectives of electric field (EF) intensity and focality with constraints on the number of electrodes. Compared with traditional tDCS in our cohort (10 subjects), our method significantly improves the EF focality. When compared to commonly used 4×1 high-definition tDCS (HD-tDCS), our method can achieve higher EF intensity in the target region with less than 8 electrodes. Our method can balance the two objectives of EF and shorten optimization time, which is convenient for practical application.
Real-time estimation of the optimal coil placement in transcranial magnetic stimulation using multi-task deep learning

Abstract:Transcranial magnetic stimulation (TMS) has emerged as a promising neuromodulation technique with both therapeutic and diagnostic applications. As accurate coil placement is known to be essential for focal stimulation, computational models have been established to help find the optimal coil positioning by maximizing electric fields at the cortical target. While these numerical simulations provide realistic and subject-specific field distributions, they are computationally demanding, precluding their use in real-time applications. In this paper, we developed a novel multi-task deep neural network which simultaneously predicts the optimal coil placement for a given cortical target as well as the associated TMS-induced electric field. Trained on large amounts of preceding numerical optimizations, the Attention U-Net-based neural surrogate provided accurate coil optimizations in only 35 ms, a fraction of time compared to the state-of-the-art numerical framework. The mean errors on the position estimates were below 2 mm, i.e., smaller than previously reported manual coil positioning errors. The predicted electric fields were also highly correlated (r> 0.97) with their numerical references. In addition to healthy subjects, we validated our approach also in glioblastoma patients. We first statistically underlined the importance of using realistic heterogeneous tumor conductivities instead of simply adopting values from the surrounding healthy tissue. Second, applying the trained neural surrogate to tumor patients yielded similar accurate positioning and electric field estimates as in healthy subjects. Our findings provide a promising framework for future real-time electric field-optimized TMS applications.

Abstract:Transcranial magnetic stimulation (TMS) is an effective treatment for depression but is limited in that the optimal therapeutic target remains unknown. Early TMS trials lacked a focal target, and thus positioned the TMS coil over the prefrontal cortex using scalp measurements. Over time, it became clear that this method leads to variation in the stimulation site and that this could contribute to heterogeneity in antidepressant response. Newer methods allow for precise positioning of the TMS coil over a specific brain location, but leveraging these precise methods requires a more precise therapeutic target. Here, we review how neuroimaging is being used to identify a more focal therapeutic target for depression. We highlight recent studies showing that more effective TMS targets in the frontal cortex are functionally connected to deep limbic regions such as the subgenual cingulate. We review how connectivity might be used to identify an optimal TMS target for use in all patients, and potentially even a personalized target for each individual patient. We address the clinical implications of this emerging field and highlight critical questions for future research.

Abstract:Objective Transcranial magnetic stimulation (TMS) can efficiently and robustly modulate synaptic plasticity, but little is known about how TMS affects functional connectivity (rs-fMRI). Accordingly, this project characterized TMS-induced rsFC changes in depressed patients who received 3 days of left prefrontal intermittent theta burst stimulation (iTBS). Methods rs-fMRI was collected from 16 subjects before and after iTBS. Correlation matrices were constructed from the cleaned rs-fMRI data. Electric-field models were conducted and used to predict pre-post changes in rs-fMRI. Site by orientation heatmaps were created for vectors centered on the stimulation site and a control site (contralateral motor cortex). Results For the stimulation site, there was a clear relationship between both site and coil orientation, and connectivity changes. As distance from the stimulation site increased, prediction accuracy decreased. Similarly, as eccentricity from the optimal orientation increased, prediction accuracy decreased. The systematic effects described above were not apparent in the heatmap centered on the control site. Conclusions These results suggest that rs-fMRI following iTBS changes systematically as a function of the distribution of electrical energy delivered from the TMS pulse, as represented by the e-field model. Significance This finding lays the groundwork for future studies to individualize TMS targeting based on how predicted rs-fMRI changes might impact psychiatric symptoms.

Abstract:Transcranial magnetic stimulation (TMS) has been gaining popularity in various neurological treatments and an automated platform with better precision is desired. This paper proposes a dexterous articulated robotic arm with TMS coils as the end-effectors. Most robotic arm based positioning systems utilize vision feedback from stationary cameras for dynamic positioning, but these are not suitable for clinics with limited space. In this paper, a compact approach to identify the size, position and orientation of the patient's head relative to a robotic arm using a non-vision, range data based system is proposed. An accurate distance measurement sensor is used in tandem with a robotic arm to scan the patient's head and produce a growing 3D point cloud. An efficient surface filling algorithm, taking into consideration the similarity of the human head to an ellipsoid, is presented to simultaneously extract position, orientation and geometrical information of the target head. Simulations and experiments are conducted and promising results are obtained to demonstrate its capability to identify the location and orientation of the patient's head.
Stimulation Effects Mapping for Optimizing Coil Placement for Transcranial Magnetic Stimulation

Abstract:Background: The position and orientation of transcranial magnetic stimulation (TMS) coil, which we collectively refer to as coil placement, significantly affect both the assessment and modulation of cortical excitability. TMS electric field (E-field) simulation can be used to identify optimal coil placement. However, the present E-field simulation required a laborious segmentation and meshing procedure to determine optimal coil placement. Objective: We intended to create a framework that would enable us to offer optimal coil placement without requiring the segmentation and meshing procedure. Methods: We constructed the stimulation effects map (SEM) framework using the CASIA dataset for optimal coil placement. We used leave-one-subject-out cross-validation to evaluate the consistency of the optimal coil placement and the target regions determined by SEM for the 74 target ROIs in MRI data from the CASIA, HCP15 and HCP100 datasets. Additionally, we contrasted the E-norms determined by optimal coil placements using SEM and auxiliary dipole method (ADM) based on the DP and CASIA II datasets. Results: We provided optimal coil placement in 'head-anatomy-based' (HAC) polar coordinates and MNI coordinates for the target region. The results also demonstrated the consistency of the SEM framework for the 74 target ROIs. The normal E-field determined by SEM was more significant than the value received by ADM. Conclusion: We created the SEM framework using the CASIA database to determine optimal coil placement without segmentation or meshing. We provided optimal coil placement in HAC and MNI coordinates for the target region. The validation of several target ROIs from various datasets demonstrated the consistency of the SEM approach. By streamlining the process of finding optimal coil placement, we intended to make TMS assessment and therapy more convenient. Abstract:Personalized functional imaging identifies brain stimulation target for a patient with trauma-induced functional disruption Dear editorNon-invasive neuromodulatory techniques such as transcranial magnetic stimulation (TMS) have been increasingly used to treat a wide range of psychiatric and neurological disorders [1].Recent studies have shown that personalized functional MRI (fMRI)guided neuromodulation may outperform conventional anatomyguided stimulation for patients with treatment-resistant major depressive disorder (MDD) [2,3].In these studies, subject-specific targets identified in the dorsolateral prefrontal region by their robust functional anti-correlations with the subgenual anterior cingulate cortex (sgACC) achieved better treatment outcomes than those target regions showing scant sgACC anti-correlations.This empirically-driven approach to TMS target identification is advantageous in MDD, but for a variety of brain disorders without an empirically defined circuit, identification of personalized TMS targets is challenging.We designed a strategy for personalized brain stimulation based on abnormal RSFC assessed by resting-state fMRI (rsfMRI) and applied it to a 21-year-old patient suffering from severe motor, cognitive and mood impairments due to brain trauma.We stimulated the personalized target using TMS and evaluated the RSFC changes along with symptom improvement following treatment.Here, we leveraged fMRI and TMS to provide proof of concept for personalized fMRI-guided closed-loop stimulation that includes functional brain assessment for target identification, neuromodulation, and reassessment of functional connectivity.

Abstract:Transcranial magnetic stimulation (TMS) protocols often include a manual search of an optimal location and orientation of the coil or peak stimulating electric field to elicit motor responses in a target muscle. This target search is laborious, and the result is user-dependent. Here, we present a closed-loop search method that utilizes automatic electronic adjustment of the stimulation based on the previous responses. The electronic adjustment is achieved by multi-locus TMS, and the adaptive guiding of the stimulation is based on the principles of Bayesian optimization to minimize the number of stimuli (and time) needed in the search. We compared our target-search method with other methods, such as systematic sampling in a predefined cortical grid. Validation experiments on five healthy volunteers and further offline simulations showed that our adaptively guided search method needs only a relatively small number of stimuli to provide outcomes with good accuracy and precision. The automated method enables fast and user-independent optimization of stimulation parameters in research and clinical applications of TMS.

Abstract:Transcranial magnetic stimulation (TMS) is a non-invasive method for neuromodulation which involves the application of brief magnetic pulses to the cortex to induce intracortical currents below the area of stimulation. The manipulation of stimulation parameters can alter the spatial and temporal patterns of cortical activation. Combining TMS with neurophysiological and neuroimaging modalities, such as electroencephalography (EEG) and stereotactic neuronavigation, respectively, can allow for a cause-and-effect approach to study cortical processes in vivo with very good spatial and excellent temporal precision. Investigation of cortical network properties may allow for the development of predictors and biomarkers of the pathophysiology of brain disorders, which can help bridge the gap between basic research and clinical applications. Already, therapeutic TMS paradigms are being used to treat medication-resistant depression and can potentially be used to ameliorate symptoms in other psychiatric or neurological disorders. Furthermore, navigated TMS procedures are increasingly being used for preoperative functional mapping of motor and language regions and can improve postoperative outcomes. Thus, TMS carries tremendous promise to assess brain properties in healthy and diseased states and can be used to optimize the efficacy of brain stimulation treatments in clinical populations.

Abstract:Non-invasive brain stimulation (NIBS) technologies, such as repetitive transcranial magnetic stimulation (rTMS), offer significant therapeutic potential for a growing number of neuropsychiatric conditions. Concurrent with the expansion of this field is the swift evolution of rTMS methodologies, including approaches to optimize stimulation site planning. Traditional targeting methods, foundational to early successes in the field and still widely employed today, include using scalp-based heuristics or integrating structural MRI co-registration to align the transcranial magnetic stimulation (TMS) coil with anatomical landmarks. Recent evidence, however, supports refining and personalizing stimulation sites based on the target's structural and/or functional connectivity profile. These connectomic approaches harness the network-wide neuromodulatory effects of rTMS to reach deeper brain structures while also enabling a greater degree of personalization by accounting for heterogenous network topology. In this study, we acquired baseline multimodal magnetic resonance (MRI) at two time points to evaluate the reliability and reproducibility of distinct connectome-based strategies for stimulation site planning. Specifically, we compared the intra-individual difference between the optimal stimulation sites generated at each time point for (1) functional connectivity (FC) guided targets derived from resting-state functional MRI and (2) structural connectivity (SC) guided targets derived from diffusion tensor imaging. Our findings suggest superior reproducibility of SC-guided targets. We emphasize the necessity for further research to validate these findings across diverse patient populations, thereby advancing the personalization of rTMS treatments.

Optimization of multifocal transcranial current stimulation for weighted cortical pattern targeting from realistic modeling of electric fields

Abstract:Recently, multifocal transcranial current stimulation (tCS) devices using several relatively small electrodes have been used to achieve more focal stimulation of specific cortical targets. However, it is becoming increasingly recognized that many behavioral manifestations of neurological and psychiatric disease are not solely the result of abnormality in one isolated brain region but represent alterations in brain networks. In this paper we describe a method for optimizing the configuration of multifocal tCS for stimulation of brain networks, represented by spatially extended cortical targets. We show how, based on fMRI, PET, EEG or other data specifying a target map on the cortical surface for excitatory, inhibitory or neutral stimulation and a constraint on the maximal number of electrodes, a solution can be produced with the optimal currents and locations of the electrodes. The method described here relies on a fast calculation of multifocal tCS electric fields (including components normal and tangential to the cortical boundaries) using a five layer finite element model of a realistic head. Based on the hypothesis that the effects of current stimulation are to first order due to the interaction of electric fields with populations of elongated cortical neurons, it is argued that the optimization problem for tCS stimulation can be defined in terms of the component of the electric field normal to the cortical surface. Solutions are found using constrained least squares to optimize current intensities, while electrode number and their locations are selected using a genetic algorithm. For direct current tCS (tDCS) applications, we provide some examples of this technique using an available tCS system providing 8 small Ag/AgCl stimulation electrodes. We demonstrate the approach both for localized and spatially extended targets defined using rs-fcMRI and PET data, with clinical applications in stroke and depression. Finally, we extend these ideas to more general stimulation protocols, such as alternating current tCS (tACS).
An Image-Guided Robotic System for Transcranial Magnetic Stimulation: System Development and Experimental Evaluation

Abstract:Transcranial magnetic stimulation (TMS) is a noninvasive medical procedure that can modulate brain activity, and it is widely used in neuroscience and neurology research. Compared to manual operators, robots may improve the outcome of TMS due to their superior accuracy and repeatability. However, there has not been a widely accepted standard protocol for performing robotic TMS using fine-segmented brain images, resulting in arbitrary planned angles with respect to the true boundaries of the modulated cortex. Given that the recent study in TMS simulation suggests a noticeable difference in outcomes when using different anatomical details, cortical shape should play a more significant role in deciding the optimal TMS coil pose. In this work, we introduce an image-guided robotic system for TMS that focuses on (1) establishing standardized planning methods and heuristics to define a reference (true zero) for the coil poses and (2) solving the issue that the manual coil placement requires expert hand-eye coordination which often leading to low repeatability of the experiments. To validate the design of our robotic system, a phantom study and a preliminary human subject study were performed. Our results show that the robotic method can half the positional error and improve the rotational accuracy by up to two orders of magnitude. The accuracy is proven to be repeatable because the standard deviation of multiple trials is lowered by an order of magnitude. The improved actuation accuracy successfully translates to the TMS application, with a higher and more stable induced voltage in magnetic field sensors.
