Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>: This systematic review and meta-analysis aimed to summarize and synthesize the available evidence in adult Chinese cardiac patients to determine the effect of education interventions on health behaviours, disease-related knowledge, self-efficacy, depressive symptoms, anxiety symptoms, health-related quality of life, morbidity, and mortality.</p><p>Methods: Seven databases were searched from database inception until January 2020 for randomized controlled trials. Characteristics of education interventions were described and random-effects meta-analysis was performed where feasible.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>: Overall, 18 randomized controlled trials were included in this systematic review and suggested that education interventions are effective in improving patients' physical activity, dietary habits, medication behaviour, disease-related knowledge, and health-related quality of life. Meta-analysis of two studies demonstrated benefit on physical activity (standardized mean difference [SMD] 1.27, 95% confidence interval [CI] 1.06-1.48; participants = 422; I<sup>2</sup> = 0%), dietary habits (SMD 0.76, 95%CI 0.44-1.08; participants = 422; I<sup>2</sup> = 61%), and medication behaviour (mean difference [MD] 0.31, 95%CI 0.17-0.46; participants = 422; I<sup>2</sup> = 28%).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>: This study supports the benefits of education interventions for adult Chinese cardiac patients on health behaviours, disease-related knowledge, and health-related quality of life. Future studies should characterize their education interventions in detail to facilitate reproducibility and comparison.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Practice Implications</h3><p>: This study identified the need for studies on the outcome of alcohol consumption and in Chinese immigrant populations.</p>

Chronic disease patient education: lessons from meta-analyses

Abstract:Twelve meta-analyses were identified concerning education for people with chronic diseases where behaviour modification is a part of the treatment regime. By combining the results of these meta-analyses a second stage descriptive meta-analysis was conducted. The aim of the exercise was to explore the effects of patient education and implications for educational treatment. The results provided evidence of the gaps that exist in current research practice. These gaps include the need to utilise rigorous research designs to explore the quantitative effects of patient education, and the need to qualitatively explore the processes by which these effects have transpired. Where randomised controlled trials had been conducted the effects of patient education were usually small and were only known for 6 months of follow-up. In addition, the educational interventions tested were generally poorly described, and failed to adhere to theoretical models. The results of this review have highlighted the need for practitioners to use theoretically based teaching strategies which include behaviour change tactics that affect feelings and attitudes. In alignment with these conclusions, the review has provided guidelines for future research practice.

Evaluating the efficacy of an education and treatment program for patients with coronary heart disease

Abstract:Background: Until now, there has not been any evaluated, disease-specific education and treatment program for patients with coronary heart disease (CHD) in Germany. It is thought that education can improve these patients' physical activity and quality of life and teach them how to lessen their risk factors, what to do in case of an emergency, and how to assess their own medical treatment. Methods: A randomized controlled open intervention trial was carried out from February 2010 to September 2011. 196 patients were assigned to receive the intervention (patient education), while 199 were assigned to a control group. In an intention-to-treat analysis, baseline and follow-up data were compared after a mean interval of 220 days. The evaluative instruments included the Freiburg Questionnaire of Physical Activity, the MacNew Heart Disease Quality of Life Questionnaire, questionnaires regarding knowledge about CHD, ergometric performance ability, and the body-mass index. Results: The patients in the intervention group reported having increased their physical activity by a mean of 9.3 MET/week (MET=metabolic equivalent of task), compared to 2.5 MET/week in the control group; the difference of 6.8 MET/week was statistically significant (p = 0.015). The patients in the intervention group also rated their quality of life higher than those in the control group (0.2 ± 0.56 vs. 0.09 ± 0.53 [mean ± standard deviation], p = 0.056). They were significantly better informed than patients in the control group about risk factors and about what to do in an emergency. Conclusion: Persistently unhealthy lifestyle is a common problem of CHD patients; the education and treatment program presented here may be a suitable means of improving patients' lifestyle for secondary prevention. Further studies will be needed to document long-term efficacy and to determine whether occasional refresher courses will be needed as well.
Do health education initiatives assist socioeconomically disadvantaged populations? A systematic review and meta-analyses

Abstract:Abstract Background Health education interventions are considered critical for the prevention and management of conditions of public health concern. Although the burden of these conditions is often greatest in socio-economically disadvantaged populations, the effectiveness of interventions that target these groups is unknown. We aimed to identify and synthesize evidence of the effectiveness of health-related educational interventions in adult disadvantaged populations. Methods We pre-registered the study on Open Science Framework https://osf.io/ek5yg/ . We searched Medline, Embase, Emcare, and the Cochrane Register from inception to 5/04/2022 to identify studies evaluating the effectiveness of health-related educational interventions delivered to adults in socio-economically disadvantaged populations. Our primary outcome was health related behaviour and our secondary outcome was a relevant biomarker. Two reviewers screened studies, extracted data and evaluated risk of bias. Our synthesis strategy involved random-effects meta-analyses and vote-counting. Results We identified 8618 unique records, 96 met our criteria for inclusion – involving more than 57,000 participants from 22 countries. All studies had high or unclear risk of bias. For our primary outcome of behaviour, meta-analyses found a standardised mean effect of education on physical activity of 0.05 (95% confidence interval (CI) = -0.09–0.19), (5 studies, n = 1330) and on cancer screening of 0.29 (95% CI = 0.05–0.52), (5 studies, n = 2388). Considerable statistical heterogeneity was present. Sixty-seven of 81 studies with behavioural outcomes had point estimates favouring the intervention (83% (95% CI = 73%-90%), p < 0.001); 21 of 28 studies with biomarker outcomes showed benefit (75% (95%CI = 56%-88%), p = 0.002). When effectiveness was determined based on conclusions in the included studies, 47% of interventions were effective on behavioural outcomes, and 27% on biomarkers. Conclusions Evidence does not demonstrate consistent, positive impacts of educational interventions on health behaviours or biomarkers in socio-economically disadvantaged populations. Continued investment in targeted approaches, coinciding with development of greater understanding of factors determining successful implementation and evaluation, are important to reduce inequalities in health.

Effectiveness of Person-Centered Health Education in the General Practice of Geriatric Chronic Disease Care

Abstract:Objective: This study assesses the impact of personalized health education on elderly patients with chronic diseases in a general practice setting. The rationale behind the incorporation of personalized health education stems from the growing recognition of the need for patient-centered care approaches, particularly in geriatric populations, where such interventions could lead to improved health outcomes. Our study aims to evaluate the effects of personalized health education on elderly patients with chronic diseases in a general practice context. The initiation of this study is grounded in the increasing acknowledgment of patient-centered care's significance, especially in geriatric demographics. We hypothesize that personalized health education interventions could significantly enhance health outcomes in this patient group. Methods: We conducted a randomized controlled trial involving 126 elderly patients with chronic diseases, assigning them equally to receive either standard care or standard care supplemented with personalized health education. The effectiveness of this education was measured through outcomes related to cognition, self-care, health literacy, psychological and physical health, quality of life, and prognosis. In our study, we executed a randomized controlled trial encompassing 126 elderly patients diagnosed with a range of chronic diseases. These participants were evenly divided into two groups: one receiving standard care and the other receiving standard care enhanced with personalized health education. The study spanned over a specified period, during which the impact of the personalized health education was meticulously evaluated. To comprehensively measure the effectiveness of the personalized health education, we employed a variety of tools and scales. These instruments were specifically chosen to assess changes in cognition, self-care abilities, health literacy, and psychological and physical health. Additionally, we evaluated the quality of life and prognosis of these patients, aiming to capture the holistic impact of the intervention. This approach ensured a thorough and nuanced understanding of how personalized health education influences the health outcomes of elderly patients with chronic diseases. Results: The intervention group demonstrated significant improvements across all measured outcomes compared to the control group, highlighting the efficacy of personalized health education in enhancing comprehensive health parameters in geriatric patients with chronic conditions. In our study, the intervention group, which received personalized health education, exhibited notable improvements in several key areas compared to the control group. Specifically, there was a marked enhancement in cognition and health literacy, with patients showing improved understanding and management of their conditions. Additionally, significant gains were observed in the quality of life, indicating that the tailored health education effectively addressed the holistic needs of geriatric patients with chronic diseases. These specific findings underscore the substantial impact of personalized health education in improving critical health outcomes in this patient population. Conclusion: Personalized health education in geriatric chronic disease management significantly betters disease comprehension, health literacy, self-care, psychological well-being, and physical health while also lowering the risk of adverse events. This study underscores the value of patient-centered educational strategies in chronic disease care for the elderly.Our study conclusively demonstrates that personalized health education plays a pivotal role in managing chronic diseases among the elderly. It significantly improves disease comprehension, health literacy, self-care capabilities, psychological well-being, and physical health. Furthermore, it contributes to a reduced risk of adverse health events. These findings emphasize the critical importance of integrating patient-centered educational strategies into general practice care for the elderly. By doing so, we can enhance their overall well-being and quality of life, making personalized health education an essential component in the holistic care of elderly patients with chronic conditions. This approach not only aligns with the principles of modern geriatric care but also sets a benchmark for the future of chronic disease management in older populations.
Educational programmes for improving medication adherence among older adults with coronary artery disease: a systematic review and meta-analysis

Abstract:Background Coronary artery disease (CAD) is the leading cause of death worldwide. Adhering to CAD medications is the priority of its treatment. However, medication adherence is suboptimal among older adults with CAD. Educational programmes are widely used and recommended in improving medication adherence among older adults with CAD. However, the evidence about the effects of educational programmes on medication adherence among older adults with CAD is limited. Aim To evaluate the best available research evidence on the effects of educational programmes designed for improving medication adherence among older adults with CAD. Methods 12 English databases and five Chinese databases were searched from inception to January 2024. Randomised controlled trials or quasi-experimental studies examining the effects of educational programmes for improving medication adherence among older adults with CAD were included. The educational programmes included a set of planned activities aiming at improving the knowledge, attitudes, and skills of medication adherence. The outcome of medication adherence was measured by either objective or subjective methods. The quality of the included studies was assessed by the Cochrane Risk of Bias Tool v2 or the Risk of Bias In Non-randomised Studies of Interventions tool. Meta-analysis was conducted using random-effect models as appropriate. Narrative synthesis was conducted if the results of the included studies were not appropriate or possible for meta-analysis. The Grading of Recommendations, Assessment, Development, and Evaluation Approach was used to assess the certainty of evidence. Results 5,607 records were retrieved and 5,599 records were excluded. Seven studies including six randomised controlled trials and one quasi-experimental study were included. Two studies were rated as low risk of bias. Five studies were rated as some concerns or moderate risk of bias due to inadequate information on blinding, allocation concealment, and the analysis of data according to the intention-to-treat principle. The results showed that the educational programmes were associated with significant improvements in medication adherence at immediately post-intervention (Standardised mean difference (SMD): 1.61, 95% CI: 0.03 to 3.19, P=0.05, Low quality of evidence), and at one to six months post-intervention (SMD: 1.13, 95% CI: 0.33 to 1.94, P=0.006, Moderate quality of evidence). Narrative synthesis found that the educational programmes potentially improved medication adherence over six months post-intervention. Conclusions The results show that the educational programmes could improve medication adherence among older adults with CAD. Designing the educational programmes with theoretical frameworks would be helpful to address the complex health needs of the older adults with CAD. More rigorous evaluation of the effectiveness of the programmes on long-term medication adherence of the older adults is warranted.Figure 1.Immediate post-interventionFigure 2.Medium-term post-intervention

Abstract:OBJECTIVE: The aim of this study was to evaluate the efficacy of a long-term secondary prevention programme following inpatient cardiovascular rehabilitation on cardiovascular risk and health-related quality of life in a cohort of middle-aged (≤58 years) coronary artery disease (CAD) patients of low educational level compared to usual care.DESIGN AND METHODS: The study included 600 patients with CAD, with 271 in the intervention group (IG) and 329 in the control group (CG). The average age was nearly 50 years in both groups, nearly 90% were male, and 77% had less than 10 years of school education. No significant differences existed between the groups at baseline. Both groups had a 3-week comprehensive cardiovascular inpatient rehabilitation programme at the beginning, the intervention consisted of one further rehabilitation session in hospital after 6 months and regular telephone reminders over a period of 36 months. Analyses were conducted on an intention-to-treat basis. To evaluate the individual risk level, we used the PROCAM score and intima-media thickness (IMT) was measured at the common carotid artery on both sides following international standards. Health-related quality of life was assessed with the EUROQOL and HADS.RESULTS: Patients in the IG showed better 3-year risk profile outcomes. The PROCAM score increased by 3.0 (IG) and by 3.7 (CG) from the beginning to after 3 years (p > 0.05 intention-to-treat). The average IMT increased by 0.04 mm in the CG and was reduced by 0.03 mm in the IG (p = 0.014 for the difference). The IG had a significant improvement in health-related quality of life. Mortality, myocardial infarction, and stroke were not different although 'other cardiac events' (cardiac surgery or intervention) were significantly lower in the IG than the CG patients (p < 0.05).CONCLUSION: This long-term secondary prevention programme with inpatient rehabilitation at the beginning and telephone reminder for a 3-year period was successful. There were significant differences in health-related quality of life between the IG and CG, despite the relatively positive outcomes in the CG. In this low-education (predominantly male), middle-aged cohort, the positive impact on cardiovascular risk was pronounced in the high-risk subgroup (PROCAM 10-year risk 10-40%).

Multiple health behaviour change interventions for primary prevention of cardiovascular disease in primary care: systematic review and meta-analysis

Abstract:BACKGROUND: It is uncertain whether multiple health behaviour change (MHBC) interventions are effective for the primary prevention of cardiovascular disease (CVD) in primary care. A systematic review and a meta-analysis were performed to evaluate the effectiveness of MHBC interventions on CVD risk and CVD risk factors; the study also evaluated associations of theoretical frameworks and intervention components with intervention effectiveness.METHODS: The search included randomised controlled trials of MHBC interventions aimed at reducing CVD risk in primary prevention population up to 2017. Theoretical frameworks and intervention components were evaluated using standardised methods. Meta-analysis with stratification and meta-regression were used to evaluate intervention effects.RESULTS: We identified 31 trials (36 484 participants) with a minimum duration of 12 months follow-up. Pooled net change in systolic blood pressure (16 trials) was -1.86 (95% CI -3.17 to -0.55; p=0.01) mm Hg; diastolic blood pressure (15 trials), -1.53 (-2.43 to -0.62; p=0.001) mm Hg; body mass index (14 trials), -0.13 (-0.26 to -0.01; p=0.04) kg/m2; serum total cholesterol (14 trials), -0.13 (-0.19 to -0.07; p<0.001) mmol/L. There was no significant association between interventions with a reported theoretical basis and improved intervention outcomes. No association was observed between intervention intensity (number of sessions and intervention duration) and intervention outcomes. There was significant heterogeneity for some risk factor analyses, leading to uncertain validity of some pooled net changes.CONCLUSIONS: MHBC interventions delivered to CVD-free participants in primary care did not appear to have quantitatively important effects on CVD risk factors. Better reporting of interventions' rationale, content and delivery is essential to understanding their effectiveness.

Abstract:Abstract Aims To evaluate a self-administered digital education resource for patients after a heart attack (adapted simplified Chinese version of Cardiac CollegeTM) on secondary prevention knowledge and health behaviour change outcomes. Methods and results Chinese immigrants recovering from a heart attack were recruited from cardiac rehabilitation programmes at four metropolitan tertiary hospitals. Participants provided access to Cardiac CollegeTM (adapted simplified Chinese version), a self-learning secondary prevention virtual education resource over 4 weeks. The web-based resources include 9 booklets and 10 pre-recorded video education sessions. Assessments included health literacy, secondary prevention knowledge, self-management behaviours, self-reported physical activity, and a heart-healthy diet. Satisfaction, acceptability, and engagement were also assessed. From 81 patients screened, 67 were recruited, and 64 (95.5%) completed the study. The participants’ mean age was 67.2 ± 8.1 years old, 81.2% were males, and the majority had no English proficiency (65.6%). Following the intervention, significant improvements were observed for secondary prevention knowledge overall and in all subdomains, with the most improvement occurring in medical, exercise, and psychological domains (P < 0.001). Dietary and self-management behaviours also improved significantly (P < 0.05). According to participants, the educational materials were engaging (100%), and the content was adequate (68.8%); however, 26.6% found the information overwhelming. Overall, 46.9% were highly satisfied with the resources. Conclusion A self-learning virtual patient-education package improved secondary prevention knowledge and self-care behaviour in Chinese immigrants after a heart attack. The culturally adapted version of Cardiac CollegeTM offers an alternative education model where bilingual staff or translated resources are limited.

[The value of the C-reactive protein assay for the early diagnosis of neonatal infection at the maternity ward and pediatric service of a general hospital center].

Abstract:Five hundred and ninety-four children hospitalized in the Maternity Ward and Pediatric Department of a General Hospital were enrolled in this retrospective study. Two CRP assays were performed with 24 hours of birth in 105 neonates at high risk for materno-fetal infection. Results suggested infection in 12 cases (2%). CRP kinetics were studied comparatively in these 105 patients and in 55 control neonates of identical gestational age. Differences between the two groups were evidenced and were already significant for the six to 12-hour age groups. Results indicated that the upper limit of normal is 6 mg at age 12 hours and 8 mg at age 24 hours, whereas levels above 12 mg at age 12 hours and 20 mg at age 24 hours are abnormal. This early marker for neonatal infection is especially valuable in general hospitals lacking sophisticated equipment for the management of neonates. The number of neonates referred from the maternity ward and unnecessarily treated by antimicrobial agents fell by 20% approximately, with no increase in risk. CRP assays proved sensitive in streptococcus B infections and were valuable for evaluating the effectiveness of antimicrobial therapy.
The effectiveness of different approaches to pre-intervention education for adult cardiac patients: A Systematic Review

Abstract:Objective: The objective of this review was to summarise the best available research evidence related to the effectiveness of pre-intervention educational methods for adult patients undergoing a cardiac intervention. The review will focus on educational methods aimed at reducing pain, anxiety, length of stay and improving knowledge retention. Methods: Adult hospital patients who were scheduled to undergo a cardiac intervention.Educational methods provided to patients prior to the cardiac intervention. Education methods included:Anxiety, pain, length of stay and/or knowledge retention.Randomised controlled trials (RCT).Where RCTs could not be identified the results of uncontrolled clinical trials and descriptive studies were incorporated into the results in a narrative form. Studies were excluded from the review if they were of poor methodological quality or the description of the participants, intervention, outcome measure or the research methods were inadequately reported. Search strategy: The search sought to identify published and unpublished studies. The databases searched included MEDLINE, CINAHL, The York Database of Abstracts of Reviews of Effectiveness (DARE), The Cochrane Library, Current Contents, Centre for Reviews and Dissemination (CRD), Health Star, Expanded Academic Index and EMBASE. Databases searched for unpublished studies included Dissertation Abstracts International Database and Proceedings First Database. Key journals and cardiac conference proceedings were also searched by hand. The reference lists and/or bibliographies of all retrieved articles were checked for additional studies. Assessment of quality: All identified studies that met the inclusion criteria were evaluated for methodological quality prior to inclusion in the review using a 'data collection tool' developed by the author based on tools of The Joanna Briggs Institute. Data extraction: Data was extracted from study reports using a 'data collection tool' developed by the author based on tools of The Joanna Briggs Institute. Data synthesis: Studies were summarised using both tabular and narrative summaries. Due to the lack of homogeneity between the study outcomes no meta-analysis could be completed. When appropriate, the findings from individual studies were detailed in graph format. For continuous data that used the same scale, weighted mean difference (WMD) and 95% confidence interval (CI) was used as a summary measure. Results: The literature search identified 29 articles that appeared to meet the initial inclusion criteria. However, only 5 studies met the review inclusion criteria. The included studies comprised of two RCTs and three randomised quasi-experimental studies. The included studies addressed comparisons in relation to different educational delivery methods and/or the timing of the educational delivery (pre-admission teaching times versus post-admission teaching times). Studies were then examined according to specific outcome measures: post intervention pain, anxiety, length of stay and/or knowledge retention. Summary: Two RCTs and three randomised quasi-experimental studies were identified that evaluated the impact of education methods for patients prior to cardiac interventions. The study by Mott (1999) compared the use of verbal education, video and a combination of both. The second study by Lamarche et al compared the provision of a pre-admission teaching session with an additional telephone call prior to the cardiac intervention. Neither study demonstrated a statistically significant impact on anxiety levels. In the Lamarche et al study however, a statistically significant difference in perceived knowledge was demonstrated between groups. The group who did not receive a telephone call prior to admission had higher perceived knowledge scores. The participants who received the telephone intervention prior to admission did not demonstrate any statistically significant reduction in anxiety levels when tested on admission and again at discharge.When the timing of the educational delivery was compared (pre-admission versus post-admission) the research by Cupples (1991) identified a statistically significant difference in state anxiety scores (p=0.02), however it must be noted that proximity to the time of the intervention may have had an impact. This study also showed a statistically significant difference in knowledge scores for a group that received pre-admission education compared to those receiving education on admission. No included studies had demonstrated improved outcomes for the patient post cardiac intervention when post-intervention pain or length of stay was considered.The main outcomes of this review are the recognition that there are very few studies specific to this area of practice. The only studies identified must be viewed with caution, as the participant numbers are small.Although many education methods are being used only a small number are being studied.

Abstract:Background: Coronary heart disease (CHD) is a major cause of death worldwide. Cardiac rehabilitation, an evidence-based CHD secondary prevention programme, remains underutilized. Telehealth may offer an innovative solution to overcome barriers to cardiac rehabilitation attendance. We aimed to determine whether contemporary telehealth interventions can provide effective secondary prevention as an alternative or adjunct care compared with cardiac rehabilitation and/or usual care for patients with CHD. Methods: Relevant randomized controlled trials evaluating telehealth interventions in CHD patients with at least three months’ follow-up compared with cardiac rehabilitation and/or usual care were identified by searching electronic databases. We checked reference lists, relevant conference lists, grey literature and keyword searching of the Internet. Main outcomes included all-cause mortality, rehospitalization/cardiac events and modifiable risk factors. (PROSPERO registration number 77507.) Results: In total, 32 papers reporting 30 unique trials were identified. Telehealth was not significant associated with a lower all-cause mortality than cardiac rehabilitation and/or usual care (risk ratio (RR)=0.60, 95% confidence interval (CI)=0.86 to 1.24, p=0.42). Telehealth was significantly associated with lower rehospitalization or cardiac events (RR=0.56, 95% CI=0.39 to 0.81, p<0.0001) compared with non-intervention groups. There was a significantly lower weighted mean difference (WMD) at medium to long-term follow-up than comparison groups for total cholesterol (WMD= −0.26 mmol/l, 95% CI= −0.4 to −0.11, p <0.001), low-density lipoprotein (WMD= −0.28, 95% CI = −0.50 to −0.05, p=0.02) and smoking status (RR=0.77, 95% CI =0.59 to 0.99, p=0.04]. Conclusions: Telehealth interventions with a range of delivery modes could be offered to patients who cannot attend cardiac rehabilitation, or as an adjunct to cardiac rehabilitation for effective secondary prevention.

Meta-Analysis: Secondary Prevention Programs for Patients with Coronary Artery Disease

Abstract:BACKGROUND: Although supervised exercise programs reduce mortality in survivors of myocardial infarction, the effects of other types of cardiac secondary prevention programs are unknown.PURPOSE: To determine the effectiveness of secondary cardiac prevention programs with and without exercise components.DATA SOURCES: The authors searched MEDLINE (1966-2004), the Cochrane Central Register of Controlled Trials, EMBASE, CINAHL, SIGLE, and the Cochrane Effective Practice and Organization of Care Study Registry. They also contacted primary study authors and hand-searched bibliographies provided by the Centers for Medicare & Medicaid Services.STUDY SELECTION: Randomized clinical trials.DATA EXTRACTION: Two reviewers chose studies and extracted data independently; random-effects summary risk ratios were calculated.DATA SYNTHESIS: The authors identified 63 randomized trials (21 295 patients with coronary disease). The summary risk ratio was 0.85 (95% CI, 0.77 to 0.94) for all-cause mortality, but this result differed over time with a risk ratio of 0.97 (CI, 0.82 to 1.14) at 12 months and 0.53 (CI, 0.35 to 0.81) at 24 months. The summary risk ratio was 0.83 (CI, 0.74 to 0.94) for recurrent myocardial infarction over a median follow-up of 12 months. Effects were similar for programs that included risk factor education or counseling with a structured exercise component (risk ratio, 0.88 [CI, 0.74 to 1.04] for mortality and 0.62 [CI, 0.44 to 0.87] for myocardial infarction), for programs that included risk factor education or counseling without an exercise component (risk ratio, 0.87 [CI, 0.76 to 0.99] for mortality and 0.86 [CI, 0.72 to 1.03] for myocardial infarction), and for programs that were solely exercise-based (risk ratio, 0.72 [CI, 0.54 to 0.95] for mortality and 0.76 [CI, 0.57 to 1.01] for myocardial infarction). Most of these programs improved quality of life or functional status, but effect sizes were small.LIMITATIONS: Although these programs may reduce total health care costs, published data on the costs of the programs are inadequate to conclusively comment on their cost-effectiveness.CONCLUSIONS: A wide variety of secondary prevention programs improve health outcomes in patients with coronary disease.

Abstract:Background Coronary heart disease (CHD) is the leading cause of death worldwide. Adequate physical activity has been firmly established as a protective factor against recurrent CHD events. However, emerging evidence highlights sedentary behaviour as an independent contributor to cardiovascular health. It is thereby imperative to identify effective strategies that promote adequate physical activity while currently reducing sedentary behaviour among patients with CHD. Purpose This systematic review aims to identify the best available evidence on the effectiveness of non-pharmacological interventions (NPIs) on promoting physical activity and/or sedentary behaviours among patients with CHD. Methods This review included randomized controlled trials that examine the effects of NPIs on physical activities or sedentary behaviours or both among patients with CHD. Systematic search was conducted in eight electronic databases (Scopus, MEDLINE, Embase, PsycINFO, Web of Science, CENTRAL, CNKI and WanFang) from inception to December 2023. The version 2 of the Cochrane risk-of-bias tool for randomized trials was used to assess the risk of bias of included studies. Meta-analysis was performed by using the Review Manager 5.4, when deem appropriate. A narrative summary was conducted to address studies that could not be included in the meta-analysis. Results A total of 24 articles of 22 studies, involving 5454 patients with CHD were included. The interventions were categorised as motivational interventions (k=6) and behavioural change technique-based interventions (k=18). The findings of this review involved pooling the results from 18 studies through meta-analysis, while a narrative summary was conducted for the remaining 4 studies. Results showed that both motivational interventions (SMD 0.34, 95% CI: 0.24, 0.45, P= 0.02, I2= 48%) and behavioural change techniques-based interventions (SMD 0.23, 95% CI: 0.10, 0.37, P= 0.0008, I2= 0%) increased the total physical activity time. Additionally, motivational interventions improve the moderate-to-vigorous physical activity time (SMD 0.42, 95% CI: 0.19, 0.65, P= 0.0004, I2= 41%). However, no significant improvement on sedentary behaviours was found from the meta-analysis. Conclusions The findings of this review suggests that motivational interventions have the potential effects to enhance total physical activity time and increase the duration of moderate-to-vigorous physical activity. Further studies on effective intervention that improve the physical activity and concurrently reduce sedentary behaviour are warranted for CHD population.Table 1.Charateristics of studiesFigure 1.Forest plot of outcomes

Impact of qualified nursing education on self-care and disease knowledge in heart failure patients

Abstract:Abstract Purpose Guidelines on management of heart failure patients support intensive patient education on self-care. To evaluate the short and long-term impact of a structured education by a qualified heart failure nurse on patient ́s self-care and disease knowledge. Methods 131 patients (66±12 years) hospitalized for heart failure participated in a structured one hour education program by a heart failure nurse. Patients completed a questionnaire consisting of 15 questions, nine of the European Heart Failure Self-Care Behaviour Scale (EHFScB-9) and additionally six questions on the patient ́s disease knowledge 1 day before, 6 months and 12 months after the education course. Possible responses ranged from 1 (complete agreement) to 5 (complete disagreement). Clinical events during the 12 months follow-up period were determined. Factors predicting with potential impact on the teaching effect were evaluated. Seven patients refused further participation during follow-up, 13 patients were lost during follow-up, and one patient was excluded from the study due to progressive dementia. Results Nine patients died during the study and 33 patients had recurrent hospitalization for heart failure. EHFScB-9 scale improved from 24.0±7.2 at baseline to 17.0±7.4 (p<0.001) at 6 months, and 17.2±6.1 (p<0.001 vs. baseline) after 12 months. Concerning the disease knowledge score, it improved from 17.9±5.3 at baseline to 12.1±4.3 (p<0.05) at 6 months follow-up, and to 12.0±5.5 (p<0.05) at 12 months after education. Scores to individual questions prior to education ranged from 1.2±0.8 on adherence to the medication protocol to 2.7±1.4 on everyday weighting habits. Greatest improvement after education were obtained on reaction to weight gains (-1.9±1.3), daily weight control (-1.6±1.4) and knowledge on the reason of patient’s heart failure (-1.6±1.2). The NYHA-classification improved from 2.8±0.8 before teaching to 2.2±0.7 (p<0.001) at 12-months follow-up. No predictor of an insufficient teaching effect could be defined. Patients with a clinical event (death, recurrent hospitalization) had a combined EHFScB-9 and disease knowledge score of 32.6±8.1vs. 28.9±9.9 in those patients without a clinical event. Patients with more than one hospitalization due to acute decompensated heart failure during the follow-up period express insignificant improvement of the combined EHFScB-9 and disease knowledge score immediately after the education session. Conclusions A qualified nurse education program improves self-care behaviour and patient ́s disease knowledge with a persisting effect at 6 and 12 months follow-up. There are no patient characteristics which preclude the implementation of an educational session.

The effect of education and counselling on knowledge, attitudes and beliefs about responses to acute myocardial infarction symptoms

Abstract:The time that elapses from the onset of symptoms of acute myocardial infarction (AMI) to treatment has a significant effect on mortality and morbidity. This study reports the effectiveness of an education and counselling intervention on knowledge, attitudes and beliefs about AMI symptoms and the appropriate response to symptoms. The intervention was tested in a randomised controlled trial of 200 people with a history of coronary heart disease (CHD). The groups were equivalent at baseline on study outcomes, clinical history and sociodemographic characteristics with the exception of more women in the intervention group (38% vs. 24%). The results of repeated measures ANOVA showed that the intervention resulted in improved knowledge of CHD, AMI symptoms and the appropriate response to symptoms that was sustained to 12 months (p=0.02). There were no differences between groups' attitudes and beliefs over time. It is concluded that a short individual teaching and counselling intervention resulted in improved knowledge of CHD, AMI symptoms and the appropriate response to symptoms in people at risk of AMI sustained to 12 months.

Abstract:Abstract Background/introduction Patient education is a crucial component in the care of patients with coronary syndrome. Nursing has an important role in disease prevention and health promotion. More than half of patients with coronary artery disease have limited knowledge which is associated with low commitment to lifestyle modification, increased hospitalizations and higher risk of mortality. The core competencies of the nurses is to promote, maintain, and restore the health of individuals in preventive care. Knowledge assessment can support nurses to plan educational strategies to address the information needs of their patients, providing effective support in secondary prevention. Purpose (1) To assess secondary prevention knowledge in patients with coronary syndrome and (2) to compare knowledge levels among groups of patients aged <60 years and aged >60 years. Methods This was a cross-sectional study of hospitalised patients with coronary syndrome. The Coronary Artery Diseases Education Questionnaire (CADE-Q II) was used to evaluate knowledge. CADE-Q II has 31 items with 4 possible answers. Questions are related to health status, risk factors, exercise, nutrition, and psychosocial risk. The maximum score is 93, with higher scores indicating greater knowledge. Descriptive analysis of the CADE-Q II was performed. SPSS Version 22 was used for analysis, and the level of significance was set at 0.05 for all tests. No items were missing. Results We included 155 patients; mean age 58,26±8,37; 22 % female; 78 % male; 56 % < 60 years old; 44 % > 60 years old; 71 % hypertension; 22 % diabetes; 32 % smokers. Mean knowledge score of participants was 61,4/93. Patients obtained values of completely correct answers in the following areas: health status 55 %; risk factors 47 %; exercise 64 %; nutrition 50 %; psychosocial risk 64 %. The level of knowledge based on the scores obtained: great 0 %; good 37 %; acceptable 55 %; poor 7 %; inadequate 1 %. Within the age groups, we obtained statistically significant results, patients aged <60 years got score 63,2/93; patients aged >60 years got score 59,0/93 (p=0.012); in the exercise domain, patients aged <60 years got score 16,5/21; patients aged >60 years got score 13,9/21 (p=0.001); in the psychosocial domain, patients aged <60 years got score 10,9/15; patients aged >60 years got score 9,7/15 (p=0.034). Conclusion The results of study point to a lower of knowledge of patients with coronary syndrome regarding their disease and factors affecting cardiovascular risk in all studied domains. The use of an assessment tool in patients with coronary syndrome allows nurses to plan educational process, providing effective support in prevention. Adequate information, promotion of health literacy of patients are an essential part of prevention and minimization of risk factors. Nurses during the nursing process by using CADE-Q II ought to promote knowledge and attitudes towards cardioprotective lifestyle.

Effectiveness of a virtual vs. in-person group-based education curriculum to increase disease-related knowledge and change health behaviour among cardiac rehabilitation participants

Abstract:Objective: To investigate the effectiveness of a virtual 12-week group-based education curriculum and to compare results with a retrospective cohort that received the same education in-person for 24 weeks. Methods: Participants completed online surveys (pre- and post-intervention) assessing disease-related knowledge, adherence to the Mediterranean diet, exercise self-efficacy, and satisfaction. The number of steps taken per day was recorded using a wearable device. Paired t tests and repeated measures ANOVA were used. A Bonferroni correction was applied(p < 0.01). Results: 80 CR participants receiving virtual education completed both assessments. Following virtual education, participants significantly increased knowledge(p < 0.001), adherence to the Mediterranean diet(p < 0.001) and number of daily steps(p = 0.01). These results were similarly observed in the in-person education group(n = 80), with no significant differences between groups. Virtual education participants decreased their self-efficacy post-intervention(p < 0.001); in contrast, participants of the in-person education increased their exercise self-efficacy(p < 0.001). Overall,31% of virtual and 71% of in-person education participants reported being satisfied with the education delivery format. Conclusions: A virtual group-based education curriculum was effective at improving knowledge and changing behaviour. Similar results were observed in those that received in-person education. Tailoring virtual education interventions to support exercise self-efficacy is warranted. Practice implication: This study strengthens the evidence supporting virtual education in CR.
Impact of health literacy on outcomes and effectiveness of an educational intervention in patients with chronic diseases

Abstract:OBJECTIVE: Study impact of health literacy on educational intervention for patients "Living with Coronary Artery Disease."METHODS: 187 patients were randomized to: VHS/DVD plus printed booklet; or booklet alone prior to scheduled visit. Main outcome measures included CAD knowledge assessment, clinical outcomes (weigh and blood pressure) and health behaviors (diet, exercise, and smoking); while functional health literacy was assessed as a possible predictor variable.RESULTS: Knowledge scores and health behaviors improved following both interventions. Those receiving the booklet and video also had a significant improvement in exercise, and weight loss. There was a trend (p=0.07) towards greater improvement in test scores among those receiving the booklet plus video. Patients with lower health literacy benefited as much as higher literacy patients.CONCLUSIONS: Incorporation of an educational program into clinical visits for patients with chronic disease improved disease-specific knowledge and prompted patients to become activated and involved in their care, improving health behaviors and outcomes. Lower health literacy was not a barrier to this beneficial effect.PRACTICE IMPLICATIONS: Patients with lower health literacy may also benefit from educational, shared decision-making interventions.

Abstract:Introduction Addressing the ongoing needs of individuals with diabetes, particularly in low- and middle-income countries like the Philippines, requires a focus on regular follow-ups with healthcare teams, adherence to healthy behaviors, and effective patient education to prevent long-term complications. The aim of this study was to ascertain the impact of a comprehensive educational program for those living with diabetes in the Philippines. Methods In a prospective study, a convenience sample of patients living with diabetes attending a cardiac rehabilitation or an outpatient diabetes clinic in the Philippines received a 12-week education intervention. Participants completed surveys at pre- and post-intervention assessing disease-related knowledge, health literacy, dietary habits, and tobacco use. Physical activity was measured by steps taken per day using wearable devices and by self-report of minutes of moderate or vigorous-intensity exercise per week. Satisfaction with the educational materials was also evaluated by a survey composed of Likert-type scale and open-ended questions. Descriptive statistics, paired t -tests or chi-square were used for data analysis. Results Overall, 184 individuals living with diabetes type 2 (mean age = 54.4 ± 12.4, 32% female) completed both assessments. There was significant improvement in disease-related knowledge ( p < 0.001), daily steps measured by a wearable device and self-reported minutes of moderate/vigorous-intensity exercise ( p < 0.001), and the number of fruit and vegetable servings consumed per day ( p = 0.001). No significant changes were observed in health literacy levels. One participant stopped using tobacco at post-education. Educational materials were highly satisfactory to participants. Lack of time, family responsibilities, and poor internet access were the main barriers to learning reported by participants. Suggestions to improve the education provided included assessment of information needs at the start of the education, having short summaries about the topics, follow-ups post-intervention, and inviting family members to sessions. Discussion Results of this study demonstrated the positive effects a comprehensive structured patient education intervention on disease-related knowledge and behaviour changes among people living with type 2 diabetes in the Philippines.
