Abstract:Background/Objectives: The Ministry of Health, Labor and Welfare in Japan has been promoting physicians' working style reforms since 2019. This study aimed to update the relationship between working hours, depressive symptoms, and suicidal ideation among obstetricians and gynecologists, based on the physicians' working style reforms. Methods: A questionnaire-based survey was conducted among obstetricians and gynecologists, and valid responses were obtained from 1164 physicians. Multivariable logistic regression analysis was performed to identify significant associations. Results: Of the respondents, 49.8% were female, and most physicians were aged 30–39 (32.1%). Precisely, 57.4% worked in public hospitals, and 47.9% worked in urban areas. Physicians working 40 ≤ x 100 h per week, and working in rural areas. None of these variables was significantly associated with suicidal ideation. Conclusions: The physicians' working style reforms have reduced the number of working hours for obstetricians and gynecologists. However, rates of depressive symptoms and suicidal ideation have not improved.

Experiences of Negotiations for Improving Research Environment and Burnout among Young Physician Researchers in Japan

Abstract:Physician scientists in Japan are often too busy to be sufficiently involved in research work. This cross-sectional study aimed to investigate their experiences negotiating with their superiors to improve their research environment and determine its relationship with psychological burnout. Among 1790 physician awardees of Grants-in-Aid for Young Scientists in 2014-2015, 490 responded (response rate 27.4%) and 408 were eligible for analysis. Outcome measures included two negotiation experiences: for reduction of clinical duty hours/promotion opportunities and for increased space or equipment/increased research budget. The main explanatory variables were personal, patient-related, and work-related burnout measured by the Copenhagen Burnout Inventory. The percentages of the above-mentioned two types of negotiations were 20-24% in women and 17-20% in men. Multivariable stepwise logistic regression analyses demonstrated that (1) the negotiation for reduction of clinical duty hours/promotion opportunities was significantly associated with physician scientists who had a short amount of weekly research time and high patient-related burnout score, and (2) the negotiation for increased space or equipment/increased research budget was significantly associated with older age, single status, and high personal and patient-related burnout scores. High burnout is related to negotiation experiences among physician researchers in Japan.

Abstract:Background: Physicians report high prevalence of depression, work long hours and are exposed to many occupational stresses (OSs). Aims: To investigate the cross-sectional association between working hours, OS and depression among physicians. Methods: A self-administered questionnaire was mailed to 1902 alumni of a medical school. The questionnaire evaluated working hours in the previous week, OS assessed by the effort-reward imbalance model, social support and depression evaluated by the Center for Epidemiologic Studies Depression scale. The associations between these occupational factors and depression were analyzed using multiple logistic regression. Results: The questionnaire was returned by 795 alumni (response rate, 42%), and 706 respondents (534 men and 172 women) were suitable for analysis. The odds ratio (OR) of depression in the long working hours group (>70 h/week) was 1.8 (95% CI: 1.1-2.8) compared with the short working hours group (<54 h/week), adjusted for basic attributes. The adjusted ORs of depression in the upper effort-reward ratio (ERR) tertile versus the lower ERR tertile were 0.6 (0.2-1.8) in the short working hours group, 8.5 (3.0-24.0) in the middle working hours group and 9.9 (3.8-25.7) in the long working hours group. The adjusted ORs of depression stratified according to working hours and ERR tended to be higher in the groups with a higher ERR, but no association between working hours and depression was found. Conclusions: This study indicates that the management of OS is needed as a countermeasure against depression among physicians.
Objective alertness, rather than sleep duration, is associated with burnout and depression: A national survey of Japanese physicians

Abstract:Approximately 40% of Japanese physicians report working more than 960 hr of overtime annually, with 10% exceeding 1860 hr. To protect their health, annual overtime limits went into effect in 2024. The objective of this study was to investigate associations of self-reported sleep duration with psychological health and objective alertness. This was a cross-sectional National Survey for The Work Style Reform of Long Working Physicians. Physicians self-reported daily sleep duration, burnout (Abbreviated Maslach Burnout Inventory), depression (Center for Epidemiologic Studies Depression Scale) and traffic accidents. Alertness was then evaluated using the brief Psychomotor Vigilance Test. Of 20,382 physicians invited, 1226 completed the survey and brief Psychomotor Vigilance Test. Daily sleep duration was inversely associated with weekly work hours (β = -5.4; 95% confidence interval -6.8 to -4.0, p < 0.0001). Sleep duration < 6 hr and ≥ 8 hr per day was associated with slower responses on the brief Psychomotor Vigilance Test (adjusted p < 0.05). An additional 10 hr worked per week was associated with a 0.40 point (95% confidence interval 0.08-0.72) increase in burnout severity and a 1.7% (95% confidence interval 0.1-3.3%) increase in odds of reporting a traffic accident. Increased brief Psychomotor Vigilance Test lapses, indicating lower alertness, were associated with worse symptoms of depression (β = 0.23 points; 95% confidence interval 0.14-0.31, p < 0.0001) and burnout (β = 0.25 points; 95% confidence interval 0.13-0.36, p < 0.0001). This study emphasizes the importance of sufficient sleep to maintain alertness, and supports limiting work hours for Japanese physicians to protect psychological health. Performance on the brief Psychomotor Vigilance Test may be a useful indicator of psychological health.

Abstract:Workstyle reform for Japanese doctors is a priority of Japanese labor policy. Factors influencing this include the low birthrate and longevity of the population and the long working hours experienced by Japanese workers. Long working hours imperil the health of doctors and create discord between their work and other life roles. The Japanese government enacted measures to promote workstyle reform for doctors, including a provision that capped legal overtime work. The limit was set to sustain healthcare services in the community. However, the allowed level of overtime is substantially higher than the so-called “line of karoshi” at which point worker compensation for work-related medical disorders is approved. Despite limited research, several measures can be considered to reduce health risks of overworked doctors, such as recommending psychological detachment from clinical work and maintaining work-life balance. Doctors should realize that self-care is a professional imperative. Organizational efforts from public administration entities or hospitals are required to secure the health of doctors. The number of surgeons and obstetricians has not recently increased. Gender role stereotyping has encouraged male Japanese doctors to work long hours and makes it difficult for female doctors to continue their professional life after childbirth. Without reforming the workstyle and protecting the health of doctors, younger workers, particularly women, are liable to avoid entering medical professions. To ensure the provision of good healthcare, it is necessary to change traditional values and improve the work environment. Academics in occupational fields should produce evidence that will help improve Japanese labor policies.
Welfare, Wellness, And Job Satisfaction Of Chinese Physicians: A National Survey Of Public Tertiary Hospitals In China

Abstract:Little national data are available on Chinese physicians' welfare, wellness, and job satisfaction. We conducted a self-administered smartphone-based national survey in early 2016 of 17 945 physicians from 136 tertiary hospitals across 31 provinces in China. In addition to collecting the physicians' basic information, we also measured 5 domains (the ethical and working environments, welfare, wellness, and job satisfaction). Half of the physicians reported a hospital-based annual income of less than RMB 72 000 ($10 300), and 60.31% of them did not think that the current medical pricing system reflects physicians' value. More than half (58.64%) of them did not have or did not know about medical malpractice insurance. These physicians worked long hours (an average of 10 h) and slept short hours (average 6 h). Only 35.78% of them thought that they were in good health, and 51.03% were in good mental health. Approximately, a quarter of them had helped to pay medical bills for patients who could not afford care, and 1 in 7 has been penalised for seeing patients who generated bad debts. Only 33.42% of them thought that their occupation receives social recognition and respect, and 70.98% would not encourage their children to pursue a medical career. The top 3 factors that may influence physician job satisfaction as chosen by the physicians were as follows: (1) the income distribution policy (45.92%), (2) working environment safety (25.86%), and (3) public trust and respect for their job (16.10%). In conclusion, we found that Chinese physicians bear heavy physical, mental, and financial stress, and many of them lack confidence that they receive trust and respect from society.

Abstract:BackgroundIn Japan, the field of Basic Sciences encompasses clinical, academic, and translational research, as well as the teaching of medical sciences, with both an MD and PhD typically required. In this study, it was hypothesized that the characteristics of a Basic Sciences career path could offer the professional advancement and personal fulfillment that many female medical doctors would find advantageous. Moreover, encouraging interest in Basic Sciences could help stem shortages that Japan is experiencing in medical fields, as noted in the three principal contributing factors: premature resignation of female clinicians, an imbalance of female physicians engaged in research, and a shortage of medical doctors in the Basic Sciences. This study examines the professional and personal fulfillment expressed by Japanese female medical doctors who hold positions in Basic Sciences. Topics include career advancement, interest in medical research, and greater flexibility for parenting.MethodsA cross-sectional questionnaire survey was distributed at all 80 medical schools in Japan, directed to 228 female medical doctors whose academic rank was assistant professor or higher in departments of Basic Sciences in 2012. Chi-square tests and the binary logistic regression model were used to investigate the impact of parenthood on career satisfaction, academic rank, salary, etc.ResultsThe survey response rate of female physicians in Basic Sciences was 54.0%. Regardless of parental status, one in three respondents cited research interest as their rationale for entering Basic Sciences, well over twice other motivations. A majority had clinical experience, with clinical duties maintained part-time by about half of respondents and particularly parents. Only one third expressed afterthoughts about relinquishing full-time clinical practice, with physicians who were parents expressing stronger regrets. Parental status had little effect on academic rank and income within the Basic Sciences,ConclusionScientific curiosity and a desire to improve community health are hallmarks of those choosing a challenging career in medicine. Therefore, it is unsurprising that interest in research is the primary motivation for a female medical doctor to choose a career in Basic Sciences. Additionally, as with many young professionals with families, female doctors seek balance in professional and private lives. Although many expressed afterthoughts relinquishing a full-time clinical practice, mothers generally benefited from greater job flexibility, with little significant effect on career development and income as Basic Scientists.

Abstract:Background Previous studies have not elucidated the factors affecting marriage and pregnancy/childbirth. This study identifies whether work characteristics and conditions affect marriage and pregnancy/childbirth among women physicians. Methods This nationwide survey was conducted on women members of three major Japanese medical societies: internal medicine, obstetrics and gynecology, and surgery. Results A total of 7747 women participated in this study. Although the mean age at marriage was the same, the age at the first childbirth in all specialties, especially in surgery, was significantly higher than that for the general Japanese population. In addition, being in a junior position, being board‐certified, and working in a workplace other than a university hospital were positively associated with the experience of pregnancy/childbirth, whereas working hours showed no impact. Conclusions We should change the mindset regarding pregnancy/childbirth and build effective strategies in academic institutions to improve the environment for safer pregnancy/childbirth while advancing careers.

Abstract:A questionnaire survey was conducted on 8,402 members of the Japanese Neurological Society to examine the current status and countermeasures for physician burnout, and 1,261 respondents (15.0%) responded. In this paper, we report the results of a comparison between male and female physicians. There was a significant difference in working and living conditions only for married people. It was confirmed that men work under stricter conditions in terms of working hours, and that the burden on women is heavier in the division of housework. Analysis using the Japanese Burnout Scale revealed no gender differences in overall scores, but as for factors related to burnout, in addition to factors common to both men and women, factors specific to men or women were clarified.

Abstract:Patients with heart failure exhibit a neurohumoral excitatory state and abnormal baroreflex control of the cardiovascular system. We determined whether arterial baroreflexes are impaired during left ventricular dysfunction (LVD) caused by chronic myocardial infarction in the absence of congestive heart failure and whether abnormal central mechanisms contribute to this impairment. Baroreceptors were stimulated in anesthetized rats with and without LVD by increasing arterial pressure with phenylephrine. Lumbar sympathetic nerve and phrenic nerve activity as well as heart rate were recorded. Rats were divided into different groups based on infarct size. Rats with moderate LVD showed impaired baroreflex control of sympathetic, ventilatory, and heart rate responses. Baroreflex gains were inversely related to the size of the infarct. The central gain for sympathetic nerve activity, obtained by using electrical stimulation of the aortic depressor nerve, also was impaired. Baroreflex control of the cardiorespiratory system is thus impaired in rats with moderate LVD in the absence of congestive heart failure. The attenuated baroreflexes are likely due to abnormal afferent mechanisms, although central mechanisms contribute to the impaired barosympathetic reflex.
Exploring factors associated with healthcare professionals subjective perceptions of complex issues in primary care in Japan: a self-administered survey study on confidence, satisfaction and burden levels

Abstract:Objective The aim of this study was to explore factors associated with healthcare professionals' subjective perceptions of complex issues in primary care settings in Japan. Design Cross-sectional survey conducted through a self-administered web-based questionnaire. Setting Japan, from June to October 2020. Participants Healthcare professionals recruited via an email list from the Japan Primary Care Association. Measures The questionnaire assessed subjective perception of satisfaction, confidence and burden regarding complex issues using a 100 mm Visual Analogue Scale (VAS). Explanatory variables included the Japanese version of the Self-assessment Scale of Interprofessional Competency (JASSIC), basic demographic information, administrative experience and an organisational climate scale. This scale comprised the 'Plan, Do, See' (PDS) factor for management and the 'Do' factor in a leader-centred direction for those working under compulsion. Factors associated with subjective perceptions were analysed using binomial logistic regression analysis and Bonferroni analysis (p<0.017). Results Data from 593 participants (average age of 41.2 years, including 133 nurses, 128 physicians and 120 social workers) were analysed. Median (quartile) VAS scores for satisfaction, confidence and burden were 50 (36–70), 52 (40–70) and 50 (30–66), respectively. Higher satisfaction group was significantly associated with PDS factor, Do factor and JASSIC Score. Greater confidence group associated with older age, male, Do factor, administrative experience and JASSIC Score. No factors were significantly associated with the higher perceived burden. Conclusion These findings reveal that interprofessional competency self-assessment influence perceptions of complex issues among healthcare professionals. Moreover, satisfaction with complex issues might be enhanced by a manageable organisational climate, while confidence might be influenced by personal attributes.

Abstract:This letter reports on a novel enhancement-mode (E-mode) AlGaN/GaN high electron mobility transistor (HEMT) that combines nanowire channel and fluorine plasma treatment using inductively coupled plasma (ICP). Compared with the conventional HEMTs, the threshold voltage of E-mode HEMT shifts from −2.8 to +0.7 V, and the Schottky reverse leakage current is reduced by one order of magnitude. The device exhibits a superior performance with a drain current of 460 mA/mm at <inline-formula> <tex-math notation="LaTeX">${V} _{{{\text {GS}}}}$ </tex-math></inline-formula>-<inline-formula> <tex-math notation="LaTeX">${V} _{{{\text {TH}}}} = 2$ </tex-math></inline-formula> V. High <inline-formula> <tex-math notation="LaTeX">$\text{I}_{{ \mathrm{\scriptscriptstyle ON}}}/\text{I}_{{ \mathrm{OFF}}}$ </tex-math></inline-formula> ratio of approximately <inline-formula> <tex-math notation="LaTeX">$10^{{\mathbf {9}}}$ </tex-math></inline-formula> and high breakdown voltage of 138 V with <inline-formula> <tex-math notation="LaTeX">$\text{L}_{{{\text {DS}}}}=4~\mu \text{m}$ </tex-math></inline-formula> are obtained, i.e., the breakdown voltage of E-mode HEMT is improved by 25% than that in conventional HEMTs. The device exhibits an intrinsic current gain cutoff frequency <inline-formula> <tex-math notation="LaTeX">$f_{{\text T}}$ </tex-math></inline-formula> of 22 GHz and a maximum oscillation frequency <inline-formula> <tex-math notation="LaTeX">${f} _{{\text {max}}}$ </tex-math></inline-formula> of 60 GHz.
Morphology and Cytochemistry of Blood Cells from Asian Wild Dog (Cuon alpinus)

Abstract:Blood cells from direct blood smears of four adult Asian wild dogs (Cuon alpinus) were examined and measured after stainning with modified Wrights Giemsa stain and cytochemical stains, including Sudan black B (SBB), ∝ -naphthyl acetate esterase (ANAE), and β-glucuronidase (β-glu). Red blood cells show uniform in sized, biconcave and prominent central pallor with 6.0 µm mean diameter. Reticulocytes and nucleated red blood cells stained positive for ANAE and β-glu. Neutrophils have tight constricted and multilobulated nucleus which mostly occupied the cells. Neutrophils stain strongly positive with SBB, negative for ANAE and weak positive for β-glu. Eosinophils contain numerous round red refractile granules with some vacuoles. Eosinophils stain moderately positive for SBB and β-glu but negative for ANAE. Basophils have variable numbers of intensely basophilic granules which do not obscure the very long lobulated nucleus. Basophils stain moderately positive for SBB but strongly positive for ANAE and β-glu. Most lymphocytes are small and medium size (65:68). Lymphocytes are negative for SBB but have 3 patterns of reactivity for ANAE and β-glu, including negative, focal dot staining and fine granular staining. Monocytes stain moderately positive for SBB and moderately to strongly positive for ANAE and β-glu. Platelets are negative for SBB and stain moderately to strongly positive for ANAE and β-glu.
Objective and subjective working hours and their roles on workers' health among Japanese employees

Abstract:This study investigated the correlation between objective and subjective working hours (OWH and SWH, respectively) and their relation to the workers' health. The study included 6,806 workers of a Japanese company (response rate=86.6%). OWH were collected as the monthly data during fiscal year 2017 from the company record. SWH were self-reported as the weekly data during the past month in November 2017. Both OWH and SWH corresponded to the same period of one month (October 2017). Additionally, the data for the annual health checkup in fiscal year 2017 and self-reported mental health in November 2017 were collected. The results indicated that the longer OWH was related to more underestimation of SWH. The analyses of covariance adjusted for the selected variables showed that irrespective of OWH or SWH, significant relationships were found for stress responses but not for body mass index, aspartate and alanine aminotransferase, fasting blood glucose, hemoglobin A1c, high-density lipoprotein cholesterol, or triglyceride. However, significant relationships with only OWH were noted for systolic and diastolic blood pressure, low-density lipoprotein cholesterol, gamma-glutamyl transpeptidase, and positive work-related state of mind. The present findings show that SWH should be used carefully when assessing the health effects of long working hours.
Work engagement influencing factors of Japanese midwives working in prenatal medical centers

Abstract:Introduction: There have been significant changes in the work environment of midwives, such as the establishment of prenatal medical centers and shared wards for obstetrics and other departments. These changes in the work environment pose a psychological burden on midwives. Under these circumstances, the concept of work engagement is essential for overcoming the ensuing difficulties. However, no studies have yet investigated midwives working in prenatal medical centers in Japan. This study examined the factors affecting work engagement among midwives working in prenatal medical centers in Japan. Methods: A cross-sectional survey of 498 midwives working at prenatal medical centers nationwide was conducted. Data were analyzed using the Mann-Whitney U test or Kruskal-Wallis test, correlation analysis using Spearman's rank correlation coefficient, and multiple regression analysis as linear regression. Results: The median of work engagement score was 3.00 (IQR: 2.40-3.60). The multiple linear regression analysis showed that professional identity (β=0.494, p<0.001), resilience (β=0.243, p<0.001), presence of in-hospital midwifery in the facility (β=0.138, p<0.011), and age (β=0.085, p=0.021) were significant. The adjusted R2 value was 0.490 (p<0.001). Conclusions: These results offer management insights into improving work engagement among midwives working in prenatal medical centers in Japan. The proposed measures include hospital/ward administrators establishing the professional identity of midwives and providing them with opportunities to demonstrate their expertise, for instance, within in-hospital midwifery systems at perinatal medical centers.

Abstract:Introduction Recent attention has been brought to physician fertility in the last decade, most notably amongst female physicians who suffer an increased risk of infertility compared to the general population. Various factors related to a career in medicine play a role, such as a delay in pursuit of childbearing, a demanding work schedule, shift work, and stress associated with the profession. However, risk of infertility in the male physician population has yet to be studied. Objective Therefore, we sought to evaluate the experiences and childbearing patterns of a sample of American male physicians with special attention to timing of conception and childbearing, reflections upon professional and personal decision-making, and general knowledge regarding fertility. The primary objective of this study was to determine the incidence of infertility among American male physicians. Methods Utilizing a third party to access the American Medical Association (AMA) database, licensed US physicians registered as male and between the ages of 18-65 were identified. Within this cohort, 690 men were randomly selected and contacted via paper mail (340) or email (350) and asked to participate in the survey study. Surveys were administered electronically via REDCap (obtained through a QR code) and included questions regarding the physicians' demographics, medical specialty, and whether they have attempted to conceive. Those who have attempted to conceive were also surveyed on their work experience at the time of their first attempt at conception and their experience and partner's experience with fertility and childbearing. Internal Review Board approval was obtained for this study (STUDY 20211036). Results Fifty-three men responded to the survey (7.7%). All respondents were cisgender males with cisgender female partners. The majority (94.3%) had attempted to conceive and occurred most during residency (46.0%). The average age when first attempting conception was 31.6 years, and this was most commonly during residency (46.0%) or after training (30.0%). During this time, many (72.0%) were responsible for night shifts, overnight call duties, or both. In addition, as many as 34 men (68.0%) had no access to paid parental leave during this time. Of 50 men who attempted conception, 14 (28.0%) of their partners experienced a miscarriage. Eight men (17.0%) underwent a fertility workup, and 6 underwent infertility treatment(s). Only 4 respondents had insurance partially covering their infertility treatments, while the other two had no insurance coverage. Conclusions Male physicians are often exposed to long work hours, nighttime duties, and sparse paid parental leave during the time of their first attempt at conception, most commonly in residency. Our cohort shows a high rate of miscarriage (28.0%) and infertility assessment (16.0%) among this population. Future studies are needed to continue characterizing fertility amongst physicians. Abstract:Aim: The chronic lack of obstetrics-gynecology (Ob/Gyn) doctors is a serious problem to be tackled in Japan. To address this issue, it is necessary for medical students to be interested in working as an Ob/Gyn doctor and more junior residents need to specialize in Ob/Gyn. This study aims to identify the preferences concerning working conditions among students and discusses what information should be provided and what conditions should be changed. Methods: A questionnaire survey was conducted among participants in the Summer School 2023 of the Japanese Society of Obstetrics and Gynecology. The respondents were medical students and junior residents interested in Ob/Gyn. Ob/Gyn specialists contributing to the summer school were also asked the same questions for comparison. The resulting data focused on important working conditions, concerns about future work, and expected future working conditions. Results: Responses from 132 medical students (97.8%, N = 135), 122 residents (99.2%, N = 123), and 76 specialists (95.0%, N = 80) were obtained. There were three main findings. First, large gaps between students/residents and specialists were observed in terms of preferences. Second, differences between male and female students/residents were not negligible, although the difference was relatively small in specialists. Third, students/residents were more concerned about risks related to medical malpractice than specialists. Conclusions: Gaps in preferences and perceptions regarding working conditions between students/residents and specialists should be narrowed through sufficient information provision. Working conditions should become gender-inclusive rather than focus on female persons.
