Abstract:Introduction Heart Failure (HF) affects more than 6.5 million Americans with an estimated half a million new cases diagnosed annually. Readmission continues to be one of the highest burdens on the US healthcare system, with an average cost of ∼$14,000 for each hospitalization, and $31 billion annually. HF poses the highest risk for rehospitalization among other medical conditions, with 2011-2014 data indicating a 30-day readmission rate of 22%. Factors that increase a patient's HF readmission risk are often linked to social vulnerability factors (health care illiteracy, lacking transportation, etc.). Aim To improve HF readmission rates in our hospital by 5% within three months. Methods The Institute of Healthcare Improvement model was used and the Plan, Do, Study, Act (PDSA) cycle was used to format this project and test change. IRB exemption was obtained. In partnership with the HF nurse practitioners, a survey was distributed to all cardiology attendings at our hospital to identify the most pressing health care disparities in HF patients specific to our patient population. This data, along with American Heart Association and American College of Cardiology 2022 HF guidelines, was used to design the PDSA 1 education based intervention. In January 2023, a peer-led interactive workshop was used to train residents on identifying and addressing health care disparities amongst heart failure patients. A dot phrase was created and shared with residents to document HF specific clinical status, factors that could contribute to increased risk of readmission, and patient-specific counseling that was conducted to mitigate HF exacerbation outpatient (changes in medications, appointments and follow ups, lifestyle changes, etc.). The primary outcome to measure change will be hospital-wide HF readmission rates. Post-intervention analysis will be done using simple descriptive statistics. Results The 2019 HF readmission rate (18.8%) at our hospital exceeded the target by 15.10%. Survey responses from cardiology attendings (n=5) showed that 60% felt patient education and understanding of the disease was the most important factor affecting HF readmission risk. Other causes reported were medication nonadherence and low income. When asked about which intervention would have the most impact on addressing health care disparities in HF patients, 60% of attending highlighted patient education as top priority. Post-PDSA 1 intervention data collection is still ongoing and projected to be completed mid-April. Conclusion Our project aimed to identify and address factors of social vulnerability contributing to high readmission risk in HF patients at our hospital. Future PDSA cycles of our initiative will focus on educating other health provider groups involved in patient discharge planning, e.g. floor nurses, and designing a more integrated EMR change such as mandatory HF documentation checklist linked to discharge diagnosis of HF exacerbation.

Reducing 30-day Acute Care Readmissions for Heart Failure Patients Through Implementation of a Discharge Bundle

Abstract:Purpose: Hospital readmissions have been a long-standing problem in the American health care system. Despite many efforts, programs, papers, and interventions identified and studied, 14% of all adult admissions result in a readmission. Readmissions are mostly considered preventable and are considered an indicator of care quality for a hospital. Due to unexpected readmissions, patients are at an increased risk for illness or injury, increased stress, financial strain, and diminished quality of life. Readmissions also negatively impact hospital systems related to decreased bed availability, stretched resources, and potential financial penalties and payment reductions. Patients with an admission related to heart failure are at an increased risk of readmission, with a national readmission rate of 23%. Primary practice setting: The quality improvement project was implemented on two telemetry units at an acute care hospital. Methodology and sample: A gap analysis identified procedural and organizational reasons for readmission in the heart failure population at an acute care hospital. Using evidence-based best practice guidelines established by the American Heart Association, American College of Cardiology, and the Heart Failure Society of America, a four-pronged proactive discharge bundle was implemented using the plan-do-study-act framework for continuous improvement. All patients admitted to the telemetry units with a primary or secondary diagnosis of heart failure received the discharge planning bundle: (1) an early assessment by the case management department, (2) patient-centered specialty heart failure education, (3) predischarge medication delivery, and (4) predischarge physician follow-up appointment scheduling within 7 days of discharge. A total of 133 patients were evaluated for inclusion in the heart failure cohort. Of those, 52 patients received the evidence-based intervention. Results: The evidence-based project was implemented over 7 weeks, September through October of 2023 on the medical telemetry units. Of the 52 patients receiving the evidence-based sample, two of the patients experienced a readmission due to heart failure (3.85%). Incidentally, it was found that patients without a readmission had an average of 2.3 completed interventions, while those with readmissions had an average of 1.5 interventions. Implications for case management practice: Case managers are an integral part of the care transition from the acute care setting back to the community. Often, it is the case manager leading this effort through various interventions. Findings from this quality improvement project suggest the use of the evidence-based, four-pronged approach to discharge planning for the heart failure patient population reduced the risk and rate of heart failure-related readmissions for the involved nursing units. These findings also surmise that there is a correlation between the number of interventions and the rate of readmission.

Abstract:BACKGROUND: Nearly 25% of patients hospitalized with heart failure (HF) are readmitted within 30 days.PURPOSE: To assess the efficacy, comparative effectiveness, and harms of transitional care interventions to reduce readmission and mortality rates for adults hospitalized with HF.DATA SOURCES: MEDLINE, Cochrane Library, CINAHL, ClinicalTrials.gov, and World Health Organization International Clinical Trials Registry Platform (1 January 1990 to late October 2013).STUDY SELECTION: Two reviewers independently selected randomized, controlled trials published in English reporting a readmission or mortality rate within 6 months of an index hospitalization.DATA EXTRACTION: One reviewer extracted data, and another checked accuracy. Two reviewers assessed risk of bias and graded strength of evidence (SOE).DATA SYNTHESIS: Forty-seven trials were included. Most enrolled adults with moderate to severe HF and a mean age of 70 years. Few trials reported 30-day readmission rates. At 30 days, a high-intensity home-visiting program reduced all-cause readmission and the composite end point (all-cause readmission or death; low SOE). Over 3 to 6 months, home-visiting programs and multidisciplinary heart failure (MDS-HF) clinic interventions reduced all-cause readmission (high SOE). Home-visiting programs reduced HF-specific readmission and the composite end point (moderate SOE). Structured telephone support (STS) interventions reduced HF-specific readmission (high SOE) but not all-cause readmissions (moderate SOE). Home-visiting programs, MDS-HF clinics, and STS interventions produced a mortality benefit. Neither telemonitoring nor primarily educational interventions reduced readmission or mortality rates.LIMITATIONS: Few trials reported 30-day readmission rates. Abstract:Circulation: Heart Failure, Ahead of Print. BACKGROUND:Home-time is an emerging, patient-centered outcome that represents the amount of time a patient spends alive and outside of health care facility settings, comprising of hospitals, skilled nursing facilities, and acute rehabilitation centers. Studies evaluating home-time in the context of heart failure are limited, and the impact of quality improvement interventions on home-time has not been studied.METHODS:Medicare beneficiaries aged 65 years or older who were hospitalized for heart failure in the Get With the Guidelines-Heart Failure registry between 2019 and 2021 were included. Postdischarge home-time, mortality, and readmission rates at 30 days and 1 year were calculated with the goal of establishing baseline metrics before the initiation of IMPLEMENT-HF, a multicenter quality improvement program aimed at improving heart failure management.RESULTS:Overall, 66 019 patients were included across 437 sites. Median 30-day and 1-year home-time were 30 (18–30) and 333 (139–362) days, respectively. Only 22.1% of patients experienced 100% home-time in the year after discharge. Older patients spent significantly less time at home, with a median 1-year home-time of 302 (86–359) compared with 345 (211–365) days in patients over 85 and those between 65 and 74 years old, respectively (P<0.001). Black patients also experienced the least amount of home-time with only 328 (151–360) days at 1-year follow-up. Rates of heart failure readmission and all-cause mortality 1-year post-discharge were high at 29.8% and 37.0%, respectively.CONCLUSIONS:In this contemporary multicenter cohort, patients hospitalized with heart failure spent a median of 91.2% of their time in the year after discharge alive and at home, largely driven by high mortality rates. These findings serve as a preimplementation baseline for IMPLEMENT-HF, which will evaluate the impact of targeted heart failure initiatives on home-time and other clinical outcomes.

Abstract:With benefits to patient outcomes, provider burden, and health system costs, the goal of reducing hospital readmissions for patients with diabetes mellitus (DM) is multifaceted. Current studies have neither elucidated a clear approach to accurately identify at-risk patients nor implemented reliable interventions to significantly reduce readmissions. We trialed a Quality Improvement project utilizing a diabetes Nurse Practitioner (NP)/Certified Diabetes Care and Education Specialist (CDCES) to see at-risk patients during initial hospitalization and in close clinic follow-up after discharge with an endpoint of reducing future readmissions. Over 21 months, 79 hospitalized patients with DM were identified as high-risk for readmission. Identifying factors included previous admission, DKA/HHS, new DM diagnosis, new to insulin, high-dose steroid use, or provider discretion. The NP/CDCES provided DM management, education, and coordinated discharge. Patients were called shortly after discharge and had a 2-4 week clinic follow-up with the NP/CDCES. The primary endpoint was number of readmissions 180 days post-discharge after NP/CDCES intervention versus number of previous readmissions pre-intervention. Change in hemoglobin A1C, time to post-hospital phone call, and time to outpatient follow-up were also evaluated. Pre- and post-intervention readmission rates remained unchanged; average pre-intervention readmissions per patient was 1.7 versus 1.88 post-intervention readmissions (p=0.74). However, a secondary endpoint of reduction in A1C was statistically significant, with a mean decrease in A1C of 0.97% per patient (p=0.0025). Time to post-hospital phone call and early follow-up did not affect readmission rates. While targeted inpatient-to-outpatient intervention for high-risk patients with DM did not reduce readmission rates, our data suggests coordinated inpatient intervention with close hospital follow-up may improve A1C in high-risk patients with diabetes. Abstract:Background Heart Failure (HF) admissions among young patients in the United States are on the rise. With that said, there is limited data on readmission outcomes of such patients. Methods The National Readmission Database (NRD) was examined to identify all admissions among adults aged 18-45 year-old with a primary diagnosis of HF who had at least one unplanned readmission within 30-days between 2010 - 2019. Patients were excluded if <18 years old, transferred to another acute care hospital, died during hospitalization or had missing data for age, sex, or mortality, or had an index hospitalization on December 1 or beyond. Multivariable Poisson regression analysis was performed for yearly trends with year as the independent variable and readmission as the dependent variable. The study was divided into two phases based on hospital readmissions reduction program (HRRP), the pre-HRRP period was 2010-2012 and post-HRRP period was 2013-2019. Outcomes were adjusted for age, sex, hospital characteristics (volume, bedsize), income, insurance and Charlson comorbidity index. All analyses were performed using STATA 16 software. Results We included 440,901 hospitalizations (mean age: 38.06 ± 0.02 years, males: 62.5%) for 30-day readmission analysis. Linear trend of higher all-cause readmission rate was noted across the study period (absolute difference of 0.5%, trend-p<0.001). Similarly, linear trend of higher HF-specific readmission rate was noted across the study period (absolute difference of 0.9%, trend-p<0.001), Figure 1A. Mean all-cause readmission rate was higher in post-HRRP era ( 20.7% vs. 19.6%, p<0.001). Mean heart failure specific readmission rate was also higher in post-HRRP era (9.9% vs. 9.5%, p=0.037), Figure 1B. Conclusion 30-day readmissions among young patients with Heart Failure are on the rise. HRRP had deleterious effect on readmission outcomes of young patients with Heart Failure.

The Effect of Clinical Pharmacists on Readmission Rates of Heart Failure Patients in the Accountable Care Environment

Abstract:Background: Recent changes in the health care delivery landscape have expanded opportunities for clinical pharmacists in the ambulatory care setting. This article describes the successful integration of a clinical pharmacist-led chronic disease management service in a patient-centered medical home (PCMH) and accountable care organization (ACO) environment. Program description: In 2008, the year before PCMH implementation, 36% of patients who were hospitalized at Advocate Trinity Hospital for a heart failure exacerbation were readmitted within 30 days of their hospital stay for heart failure exacerbation. This high rate of heart failure hospital readmissions, compared with national standards, drove the implementation of the PCMH at Advocate Medical Group - Southeast Center (AMG-SE), the adjoining outpatient medical clinic. A clinical pharmacist was added to the health care team to help achieve the collective goal of improving patient outcomes and decreasing hospitalizations. Observations: From November 1, 2009, through August 30, 2010, the clinical pharmacist conducted visits and intervened in the care of 111 chronic heart failure patients. A pre/post analysis of those 111 patients during the 10 months before and after the integration of the clinical pharmacist showed that those patients were hospitalized 63 times in the 10 months before having regularly scheduled visits with the clinical pharmacist and 30 times in the 10 months after establishing care. This reduction from 63 to 30 visits translated to an approximate 50% decrease in heart failure hospitalizations in patients being followed by the clinical pharmacist within the first 10 months. Once the clinical pharmacist became better integrated into the workflow through development of rapport with the medical team, the outcomes improved further. In an 18-month analysis from May 1, 2010, through November 30, 2011, only 2% of patients (3 of 153) designated as high-risk patients managed by the clinical pharmacist had a 30-day readmission for heart failure exacerbation. Implications: Outcomes-based models have expanded opportunities for clinical pharmacist involvement and can provide unique reimbursement options. Demonstration of cost savings and an improvement in quality measures are paramount to establishing and justifying the clinical pharmacist's role in a team-based model of care. Socioeconomic and Demographic Determinants of Readmission Rates in Congestive Heart Failure Patients: Insights From the Nationwide Readmissions Database

Abstract:Background Congestive heart failure (CHF) is a leading cause of hospitalizations and readmissions, placing a significant burden on the healthcare system. Identifying factors associated with readmission risk is crucial for developing targeted interventions and improving patient outcomes. This study aimed to investigate the impact of socioeconomic and demographic factors on 30-day and 90-day readmission rates in patients primarily admitted for CHF. Methods The study was carried out using a cross-sectional study design, and the data were obtained from the Nationwide Readmissions Database (NRD) from 2016 to 2020. Adult patients with a primary diagnosis of CHF were included. The primary outcomes were 30-day and 90-day all-cause readmission rates. Multivariable logistic regression was used to identify factors independently associated with readmissions, including race, ethnicity, insurance status, income level, and living arrangements. Results A total of 219,904 patients with a primary diagnosis of CHF were used in the study. The overall 30-day and 90-day readmission rates were 17.3% and 23.1%, respectively. In multivariable analysis, factors independently associated with higher 30-day readmission risk included Hispanic ethnicity (OR 1.18, 95% CI 1.03-1.35), African American race (OR 1.15, 95% CI 1.04-1.28), Medicare insurance (OR 1.24, 95% CI 1.12-1.38), and urban residence (OR 1.11, 95% CI 1.02-1.21). Higher income was associated with lower readmission risk (OR 0.87, 95% CI 0.79-0.96 for highest vs. lowest quartile). Similar patterns were observed for 90-day readmissions. Conclusion Socioeconomic and demographic factors, including race, ethnicity, insurance status, income level, and living arrangements, significantly impact 30-day and 90-day readmission rates in patients with CHF. These findings highlight the need for targeted interventions and policies that address social determinants of health and promote health equity in the management of CHF. Future research should focus on developing and evaluating culturally sensitive, community-based strategies to reduce readmissions and improve outcomes for high-risk CHF patients.
Improving Education On Discharge For Patients Admitted With Heart Failure

Abstract:Introduction Heart failure (HF) remains a leading cause of hospitalization among US adults. Studies have shown that education decreases readmission rates and improves outcomes. The American Heart Association (AHA) created an educational booklet describing symptoms, treatments, and eating plans for heart failure. The goal of this project was to increase utilization of the AHA workbook in the form of discharge instructions to provide HF patients with valuable information. Hypothesis The authors hypothesized that after the intervention, more patients would receive the AHA workbook upon discharge. The authors also hypothesized that there would be fewer 7-day readmissions among those who received the AHA workbook. Methods In August 2022, an email was sent to internal medicine residents and Division of Hospital Medicine providers with instructions for attaching the AHA workbook to HF discharges. In addition, physical reminders were attached to hospital computers. The following information was collected (from June 1, 2022 to November 10, 2022): number of admissions, number of discharges, number of patients who received the AHA workbook, and number of 7-day readmissions (in September and October). The primary endpoint was the rate at which providers discharged HF patients with the AHA workbook. The secondary endpoint was 7-day readmissions. Chi squared tests were used for analysis. Results From June 1, 2022 to August 21, 2022, there were 114 HF discharges, and the AHA workbook was included 4 times (3.5%). From August 22, 2022 to November 10, 2022, there were 95 HF discharges, and the AHA workbook was included 13 times (13.7%; P=0.007). In September and October 2022, 23 of the 360 discharged HF patients were readmitted within 7 days (6.39%). There were 0 readmissions for the 11 patients who received the AHA workbook (0%). There were 23 readmissions for the 349 patients who did not receive the AHA workbook (6.59%; P=1.0). Conclusions There was a statistically significant increase in utilization of the AHA workbook after the intervention. This indicates that email and physical reminders are effective tools for changing providers’ habits. However, there is still room for improvement, and future projects should focus on new ways to increase the use of the AHA workbook. There were no 7-day readmissions for patients who received the AHA workbook, but the difference was not statistically significant. This may be attributed to the small sample size, as relatively few patients received the workbook. When use of the AHA workbook further increases, 7-day readmissions should be reinvestigated. This project highlights the importance of simple educational interventions and may pave the way for future projects.

Abstract:Heart failure (HF), a worldwide epidemic with significant morbidity and mortality risks, is frequently secondary to cardiovascular disorders and probably is the common final way to survive patients. Almost 25% of hospitalized patients with acute HF are expected to be readmitted within 30 days post-discharge, and the rates of rehospitalization increase to almost one-third at 60 days and 60 percent within one year of discharge. Although care planning for patients with heart failure is complex, multidisciplinary, and resource-dependent, optimal self-care management along with appropriate educational intervention and follow-up strategy could be able to reduce readmissions, decline the duration of hospitalization, increase life expectancy, decrease the rates of mortality, and reduce costs of healthcare services for patients with HF. However, there are contradictions in previous reports about the efficacy of self-care, mainly due to patients' non-adherence to self-care behaviors. Therefore, the current study aimed to review the investigations on the effectiveness of self-care of HF patients in reducing hospital readmissions and increasing quality of life, and discuss novel approaches for predischarge educational interventions and postdischarge follow-up strategies.
Developing and Implementing an mHealth Heart Failure Self-care Program to Reduce Readmissions: Randomized Controlled Trial

Abstract:Background Patients admitted with decompensated heart failure (HF) are at risk for hospital readmission and poor quality of life during the discharge period. Lifestyle behavior modifications that promote the self-management of chronic cardiac diseases have been associated with an improved quality of life. However, whether a mobile health (mHealth) program can assist patients in the self-management of HF during the acute posthospital discharge period is unknown. Objective We aimed to develop an mHealth program designed to enhance patients’ self-management of HF by increasing knowledge, self-efficacy, and symptom detection. We hypothesized that patients hospitalized with HF would be willing to use a feasibly deployed mHealth program after their hospital discharge. Methods We employed a patient-centered outcomes research methodology to design a stakeholder-informed mHealth program. Adult patients with HF admitted to a large academic hospital were enrolled and randomized to receive the mHealth intervention versus usual care. Our feasibility outcomes included ease of program deployment, use of the clinical escalation process, duration of participant recruitment, and participant attrition. Surveys assessing the demographics and clinical characteristics of HF were measured at baseline and at 30 and 90 days after discharge. Results The study period was between July 1, 2019, and April 7, 2020. The mean cohort (N=31) age was 60.4 (range 22-85) years. Over half of the participants were men (n=18, 58%) and 77% (n=24) were White. There were no significant differences in baseline measures. We determined that an educational mHealth program tailored for patients with HF is feasibly deployed and acceptable by patients. Though not significant, we found notable trends including a higher mean quality of life at 30 days posthospitalization among program users and a longer duration before rehospitalization, which are suggestive of better HF prognosis. Conclusions Our mHealth tool should be further assessed in a larger comparative effectiveness trial. Our pilot intervention offers promise as an innovative means to help HF patients lead healthy, independent lives. Abstract:Introduction Patients hospitalized with heart failure (HF) or requiring outpatient intravenous diuretics are at high risk of admission, with associated morbidity and mortality. National average for 30-day readmission in the Medicare population is 21.3% despite contemporary medical therapy. Objective To assess if patients identified to be a high risk for HF (re)admission followed in a Nurse Practitioner (NP) driven high risk heart failure (HRHF) clinic have lower than expected rates of (re)admission. Methods A HRHF clinic protocol was piloted at our institution in 2021 enrolling HFrEF patients with > 1 HF admission in the preceding 3 months and HF patients who required IV diuretics in the office. Patients are seen by HF NP weekly until stable, typically for a minimum of 1 month, with associated device downloads as appropriate, then at frequent intervals as clinically indicated. Hospitalizations for HF in enrolled patients were reviewed quarterly. Mechanisms to ensure accuracy of enrollment and tracking were verified by a lead NP. Results A total of 278 patients were enrolled, ranging 48 to 63 per quarter (see Figure 1) with 30-day (re)admission ranging between 4-9 patients per quarter (8-16%) The mean age of enrolled patients was 75 with 45% women, 39% with reduced EF. Evidence-based BB were prescribed in 79%, ACEI/ARB/ARBNI in 51%, MRA in 51%, SGLT2i in 34%. Conclusion Patients enrolled in an NP based HRHF clinic maintained lower than expected 30-day (re)admission rates despite high baseline risk. Based on these initial results, further evaluation of clinical outcomes and investigation into clinical and demographic variables associated with (re)admission are planned to inform refinement of enrollment criteria and clinic protocols.

Abstract:The impact of hospital readmission reduction program (HRRP) on heart failure (HF) outcomes has been debated. Limited data exist regarding trends of HF readmission rates beyond 30 days from all-payer sources. The aim of this study was to investigate temporal trends of 30- and 90-day HF readmissions rates from 2010 to 2017 in patients from all-payer sources. The National Readmission Database was utilized to identify HF hospitalizations between 2010 and 2017. In the primary analysis, a linear trend in 30-day and 90-day readmissions from 2010 to 2017 was assessed. While in the secondary analysis, a change in aggregated 30- and 90-day all-cause and HF-specific readmissions pre-HRRP penalty phase (2010–2012) and post-HRRP penalties (2013–2017) was compared. Subgroup analyses were performed based on (1) Medicare versus non-Medicare insurance, (2) low versus high HF volume, and (3) HF with reduced versus preserved ejection fraction (heart failure with reduced ejection fraction and heart failure with preserved ejection fraction). Multiple logistic and adjusted linear regression analyses were performed for annual trends. A total of 6 669 313 index HF hospitalizations for 30-day, and 5 077 949 index HF hospitalizations for 90-day readmission, were included. Of these, 1 213 402 (18.2%) encounters had a readmission within 30 days, and 1 585 445 (31.2%) encounters had a readmission within 90 days. Between 2010 and 2017, both 30 and 90 days adjusted HF-specific and all-cause readmissions increased (8.1% to 8.7%, P trend 0.04, and 18.3% to 19.9%, P trend <0.001 for 30-day and 14.8% to 16.0% and 30.9% to 34.6% for 90-day, P trend <0.001 for both, respectively). Readmission rates were higher during the post-HRRP penalty period compared with pre-HRRP penalty phase (all-cause readmission 30 days: 18.6% versus 17.5%, P <0.001, all-cause readmission 90 days: 32.0% versus 29.9%, P <0.001) across all subgroups except among the low-volume hospitals. The rates of adjusted HF-specific and all-cause 30- and 90-day readmissions have increased from 2010 to 2017. Readmissions rates were higher during the HRRP phase across all subgroups except the low-volume hospitals.

Abstract:Background-It is unknown whether causes and temporal patterns of 30-day readmission vary between heart failure (HF) with preserved ejection fraction (HFpEF) and HF with reduced ejection fraction (HFrEF). We sought to address this question by examining a 5% national sample of Medicare beneficiaries. Methods and Results-We included individuals who experienced a hospitalization for HFpEF or HFrEF between 2007 and 2013. We identified causes of 30-day readmission based on primary discharge diagnosis and further classified causes of readmission as HF-related, non-HF cardiovascular-related, and non-cardiovascular-related. We calculated the cumulative incidence of these classifications for HFpEF and HFrEF in a competing risks model and calculated subdistribution hazard ratios of these classifications by comparing those with HFpEF and those with HFrEF. Among 60 640 Medicare beneficiaries, we identified 13 785 unique older adults hospitalized with HFpEF and 15 205 who were hospitalized with HFrEF. Noncardiovascular diagnoses represented the most common causes of 30-day readmission (HFpEF: 59%; HFrEF: 47%), a pattern that was observed for each week of the 30-day study period for both HFpEF and HFrEF participants. In comparing readmission diagnoses in an adjusted model, non-cardiovascular-related diagnoses were more common and HF-related diagnoses were less common in HFpEF participants. Conclusions-Non-cardiovascular-related diagnoses represented the most common causes of 30-day readmission following HF hospitalization for each week of the 30-day postdischarge period. HF diagnoses were less common among those with HFpEF compared with HFrEF. Future interventions aimed at reducing 30-day readmissions following an HF hospitalization would benefit from an increased focus on noncardiovascular comorbidity and interventions that target HFpEF and HFrEF separately.

Abstract:Background To improve transitional support after a Heart Failure (HF) hospitalization, Intermountain Healthcare (IH) partnered with CipherHealth (CH) to create an automated disease-specific follow up call program including IH nursing escalation algorithms (HFCalls). This partnership has previously reported a significant reduction in readmissions and thus we've sought to provide a contemporary analysis of the impact on readmissions and cost, including themes requiring nurse interventions that may be used to further improve care. Methods The time period for this analysis included January 2018 through December 2021. HF patients were identified using the daily, real-time IH HF Identification and Risk report and received 3 post-discharge calls (at 48 hours, 7- and 21-days). Volumes of automated calls and patient “reach rate” for calls answered were obtained from CH for the HFCalls program and patient issues with following HF specific self-care were identified. The IH “Health Answers” nursing team used internally developed and vetted escalation algorithms for prompt issue resolution based on issues identified. Themes were summarized regarding the most common issues across the calls. Potential savings from reducing readmissions was calculated using the Agency for Healthcare Research and Quality average cost of a HF readmission ($15,732). Results 4 years of data was collected from the study period from IH and CH, including a total of 16,679 automated calls. The average ‘reach rate’ of patients connecting with calls was 61%, and average time from alert generation from the automated CH call to IH nurses issue resolution was 2 hours. 72% of patients found the calls to be helpful. The average all-cause 30-day readmission rate for all patients was 14.2% (2,370/16,679), readmissions for patients reached were 12.0% (1,280/10,652) vs.18.1% (1,090/6,027) for those not reached (p-value < 0.0001). See Table for trends over the years. Most common themes for escalated calls include no follow up appointment, lack of weight scale, gaining weight and questions about discharge instructions. An estimated $10,210,068 potential savings was calculated through reducing readmissions in 649 of 16,679 patients. Conclusion Optimizing HF transitions requires an innovative, multifaceted and collaborative approach. This analysis shows that integrating automated HFCalls with nursing escalation algorithms is well received by patients after HF hospitalizations with enduring lower hospital readmission rates, and potential cost savings. These results show the importance of access to nursing support after discharge and reveal how patient themes may be used to guide HF patient education and quality improvement across transitions.

Abstract:Background Heart failure with preserved EF ( HFpEF) is a condition common in the older population related to multiple co morbidities. According to “The Get With The Guidelines-Heart Failure “registry which was linked to Medicare administrative data (2006 - 2014) data that hospitalized patients with HFpEFa are ≥65 years of age. The final cohort included 53,065 patients with HFpEF and the overall 30-day mortality was 5.87%; at 1 year, it was 33.1%. The 30-day all-cause readmission rate was 22.2%, and it was 67.0% at 1 year. In a systematic review conducted by Kalogirou et al in 2020 on the effectiveness of Heart Failure Disease Management among HFpEF, they concluded that since patients with HFpEF are older and multi morbid, Disease Management Programs (DMP) should not rely on single disease focus but addresses predisposing presentations and a comprehensive geriatric assessment. Other components should include lifestyle modification. Based on evidence, DMP may improve mortality, hospitalization rates, self-care and quality of life. Our institution's, Heart Failure Disease Management (HFDM) team readmission data had been below the hospital and national average. We are not certain regarding the percentage of HFpEF population seen. The management of HFpEF is different and dependent on patient's co-morbidities. Aim The primary purpose of this project is to determine if the Heart Failure Disease Management (HFDM) team interventions made an impact among patients with HF Preserved Ejection Fraction in a single institution PICO question: Among patients hospitalized with HFpEF, did HFDM interventions reduce 30-day readmissions? Methods This is a quality improvement project conducted in a single tertiary 1000 bed hospital in the Texas Medical Center in 2022. Heart failure patients are referred to the HFDM primarily composed of a Nurse Practitioner in the acute setting. HFDM interventions include an individualized education with Mini Cog assessment for 65 years and over and low literacy evaluation. For patients with positive Mini Cog, family or caregiver was involved in the education. HF education was focused on self-adherence strategies, diet and fluid restriction, with provision of scale and BP if lacking. Barriers to adherence were evaluated and collaborated with Social Work, Case Management and Pharmacy if needed. Seven days post discharge follow up was scheduled with HFDM clinic if needed. For patients with HFpEF as well, the NP education focused on the precipitating co morbidity and risk factor modification. Results Total HF cases were 1758 in 2022 with the following ICD codes: 113.0,111.0, 113.2, 150.33,150.3 and 150. Due to the complexity of HFpEF and multi co morbidities, these ICD10 codes were included. We excluded combined systolic and diastolic HF. The readmission rate for the hospital was (n= 325) 19.14%. The HFDM saw 394 cases with a readmission rate of (n=57) 14.44%. Conclusion These data showed that HFDM specific interventions in the acute setting made an impact on 30-day readmission for HFpEF patients. Further data abstraction will be done on the specifics for quantification as well as their self-care and quality of life.

Abstract:Objectives: The objective of this study was to evaluate an interhospital collaborative approach to improve 7-day post-discharge follow-up (7dFU) rates and reduce 30-day readmissions in heart failure (HF) patients. Background: Early post-discharge follow-up after HF hospitalization is associated with lower 30-day readmission rates. Methods: Observational analyses of Medicare HF patients discharged from 10 collaborating hospitals (CH) participating in the Southeast Michigan See You in 7 Collaborative were carried out. We compared pre-intervention (May 1, 2011 to April 30, 2012) and intervention (May 1, 2012 to April 30, 2013) 7dFU rates, unadjusted 30-day readmissions, risk-standardized 30-day readmissions (RSRR), and Medicare payments in CH and Michigan nonparticipating hospitals (NPH). Results: 7dFU rates increased but remained low in both groups (CH: 31.1% to 34.4%; p < 0.001; NPH: 30.2% to 32.6%; p <0.001). During the intervention period, unadjusted readmissions decreased significantly in both groups (CH: 29.0% to 27.3%; p <0.001; NPH: 26.4% to 25.8%, p = 0.004); mean RSRR decreased more in CH than in NPH (CH: 31.1% to 28.5%; p < 0.001; NPH: 26.7% to 26.1%, p = 0.02; p = 0.015 for intergroup comparisons). Findings were similar when CH outcomes were matched 1:1 with similar NPH outcomes. Combined Medicare payments for inpatient and 30 days of post-discharge care decreased by $182 in CH and by $63 in NPH (per eligible HF discharge). Conclusions: See You in 7 Collaborative participation was associated with significantly lower 30-day readmissions and Medicare payments in HF patients. Increases in 7dFU were modest, but associated processes aimed at this goal may have improved the transition from inpatient to outpatient care. Regional hospital collaboration to share best practices could potentially reduce HF readmissions and associated costs.

Abstract:Introduction Nearly 1 in 4 heart failure (HF) patients are readmitted within 30 days of discharge and approximately half are readmitted within 6 months. The annual expenditure on patients with heart failure is approximately $30.7 billion, which is expected to rise to $53 billion by 2030. Multiple techniques are implemented at various levels with the aim to decrease the readmission rate. Heart Failure Management System (HFMS) is a non-invasive device, that uses radiofrequency technology for early detection of changes in pulmonary fluid levels, an increase of which is an early indicator of HF decompensation. Methods This was a single center pilot experience of patients diagnosed with heart failure, who were given HFMS prescription with the aim to reduce the 30-day HF readmission rate. Within 24-48 hours of discharge from their HF admission, the patch was delivered to the home with instructions for use. The national Independent Diagnostic Testing Facility (IDTF) helped in facilitating proper placement, connection and baselining. “Alerts” were defined as abnormal thoracic fluid index (TFI) (3 standard deviation above baseline for 3 consecutive days), ventricular tachycardia (VT) >150 for 30 seconds or asystole for 8 seconds. Based upon a predetermined alert protocol, specifically the hypervolemia protocol, the clinical team received an alert via a phone call and the report was also electronically delivered to the office. A weekly report was also received with and without alerts from the IDTF. Per hypervolemia protocol, adjustments were made to the diuretic and other medications as indicated. If an outpatient or emergency visit was deemed appropriate by the physician, the same was advised. Results Between February 2022 and July 2022, a total of 32 patients received the prescription for HFMS during their heart failure admission. All patients were NYHA Class III or IV and HF Stage C or D. Amongst 32 patients, 23 (72%) performed the initial baseline and were compliant with wearing the device, and 9 (28%) did not perform initial baseline. Amongst the 23 patients, one was electively readmitted with organ offer for heart transplantation. One patient out of 23 (4.3%) had 2 alerts for which telephonic adjustment of diuretic was made followed by outpatient appointment. Amongst the 9 patients who did not perform the initial baseline assessment, 5 (55%) were readmitted to the hospital within 30 days. The average 30-day readmission rate for HF at our center without HFMS was 22%, similar to national average. Conclusion About a quarter of patients who were prescribed the HFMS did not comply and did not perform baseline metrics. Greater than 50% of the patients who never baselined were readmitted within 30 days. In our small study, one readmission was prevented by using the HFMS device. Large scale data and follow up is required around this novel technology, however compliance seems to be the primary hurdle.
