Abstract:Objective To analyze the short-term effect and tolerance of different dose of amino acid in parenteral nutrition for preterm infants. Methods Eighty cases of preterm infants accepting parenteral nutrition in pediatric department of No. 215 Hospital of Shaanxi Nuclear Industry from November 2014 to November 2015 were selected as study objects, and they were randomly divided into observation group and control group with 40 cases in each group. Fat emulsion was given to patients in each group with the dosage of 1g/( kg. d) on first day after birth and increased by 0. 5g/(kg. d) but not beyond 3g/(kg. d). Specific amino acid was given within 12-24 hours after birth. Except for dosage difference in amino acid, other symptomatic treatments of patients in each group were identical, and simultaneously routine parenteral nutrition and enteral nutrition were implemented. Compound amino acid was given to 40 infants in the control group with the dosage of 1. 0g/(kg. d) on day 1, and increased by 0. 5g/(kg. d) until 3. 5g/(kg. d). Amino acid dosage given to 40 infants in the observation group was 2. 0g/(kg. d) at beginning, increased by 1. 0g/(kg. d) on day 2 and further increased by 0. 5g/(kg. d) on day 3 and continuously increased to 4g/(kg. d). Time for recovery to normal birth weight, body mass growth rate, head circumference growth rate, length of hospital stay and relative biochemical and blood gas analysis index was monitored for patients in two groups and statistical analysis was performed on data. Results Time for recovery to birth weight of infants in the observation group was significantly shorter than that in the control group (t=2. 839, P<0. 05). Difference in body mass growth rate within two weeks after birth between two groups was statistically significant (t=4. 781, P<0. 05). Body mass increased faster with increasing dose of amino acids. Differences in serum biochemical indexes of infants measured after admission and before parenteral nutrition were not statistically significant between two groups (all P >0. 05). At day 7 after birth, serum biochemical indexes were measured again. Comparison of serum prealbumin level between two groups showed that it was significantly higher in the observation group than in the control group ( t =-4.791, P<0. 05). Difference in complications between two groups was not statistically significant (P >0. 05). Conclusion Early infusion of high-dose amino acid has very positive influence in improving premature infant nutrition status, and can effectively promote growth and development of preterm infants and improve treatment effect. Application of high-dose amino acid can cause transient elevation of creatinine, but it is within tolerance. Clinical application of high-dose amino acid is safe and effective, and it is worthy of promotion.
