Abstract:Purpose: To examine the socioeconomic and behavioural risk factors for periodontal disease in women of childbearing age and evaluate the extent of public awareness of the association between oral health and pregnancy in China.Materials and Methods: Cross-sectional data from 832 women (including 188 pregnant women) from Yuyao, Zhejiang Province were collected using a structured questionnaire. Demographic data were used to measure the participants' socioeconomic status. The questionnaire assessed knowledge and behaviours related to personal oral hygiene and utilisation of dental care services. Data were divided into pregnant and non-pregnant groups for multivariate logistic regression analysis.Results: In total, 88.3% pregnant women and 74.2% non-pregnant women reported periodontal symptoms. Abnormal body mass index (BMI <= 18.5, odds ratio, OR = 0.76, 95% CI 0.27-0.97, P = 0.024; BMI >= 23.9, OR = 1.83, 95% CI 1.12-3.35, P = 0.035) was significantly associated with self-reported periodontal disease. Minimal mental stress (OR = 0.56, 95% CI 0.43-0.94, P = 0.028), high annual household income (OR = 0.69, 95% CI 0.17-0.82, P = 0.008), advanced oral hygiene aids (OR = 0.33, 95% CI 0.18-0.49, P < 0.001) and knowledge of the link between pregnancy and periodontal disease (OR = 0.57, 95% CI 0.33-0.96, P = 0.016) were associated with decreased incidence of self-reported periodontal disease.Conclusions: A low socioeconomic background was correlated with the high incidence of self-reported periodontal disease among women of childbearing age in China. Education about primary oral health and equitable distribution of dental services might be expected to improve oral health in this specific population.

Abstract:Background The awareness of women towards oral health during pregnancy is an important aspect of her general health. It can compromise pregnancy outcomes, as well as it may affect the newborn’s overall health. Every pregnant woman plays a critical role in shaping the oral hygiene habits adopted by her if she is aware of pregnancy-related oral health and result in good perinatal outcomes. This study aimed to assess knowledge of oral health during pregnancy and associated factors among pregnant mothers who attend antenatal care at South Omo Zone public hospitals, Southern Ethiopia, 2021. Methods An institution-based cross-sectional study was conducted among pregnant women attending routine antenatal care from October 01 to November 30. A systematic random sampling method was conducted to select study participants. Data had been collected through face-to-face interviews using semi-structured questionnaires. Data were entered using epi data version 3.1 and exported to Statistical Package for the Social Sciences version 25 for data analysis. Descriptive statistics had calculated for each variable, and binary logistic regression analysis with 95% confidence intervals was carried out to assess the factors associated with the outcome variables. Variables with P-value less than 0.05 were used to declare statistical significance. Result Overall good knowledge of pregnant mothers regarding oral health was 34.1% with 95% CI, (32.76, 35.44). Having access to health facility (AOR = 2.60, 95% CI, 1.44, 4.70), having the educational status of secondary and above (AOR 1.37, 95% CI, 1.44, 4.31), having the educational status of primary education (AOR = 0.43, 95% CI, 0.20, 0.92), having a median income of > = 1500 Ethiopian birr (AOR = 0.41, 95% CI, 0.23, 0.72), being governmental employee (AOR = 0.11, 95% CI, 0.03, 0.41), received oral health hygiene counseling during pregnancy (AOR = 1.33, 95% CI, 1.62, 2.84) were significantly associated with good knowledge of oral health of pregnant mothers. Conclusion This study showed that the knowledge of pregnant mothers about oral health was 34.1%. Educational status, monthly income, occupation, access to health services, and receiving counseling on oral hygiene at antenatal care were some factors associated with good knowledge of oral health during pregnancy. Therefore strengthening counseling during antenatal care, improving access to a health facility, improving educational status, monthly income, and being government employee are crucial to enhance knowledge of women towards oral health during pregnancy.

Prevalence and Risk Factors of Periodontal Disease among Pregnant Women in a Rural Area of China

Abstract:Abstract BackgroundPeriodontal disease is one of the most important common oral diseases contributing to the global burden of chronic disease. Systematic reviews have demonstrated that poor periodontal condition during pregnancy was significantly associated with adverse pregnancy outcomes such as preterm birth, low birth weight, preeclampsia, etc. Oral health care is often a neglected area in low- and middle- income countries, particularly in rural areas due to the limited health human resources and poor infrastructures resources. Very few studies have explored the risk factors of periodontal disease among pregnant women in rural areas of China.MethodsA cross-sectional study was conducted among 648 women between 2 and 28 weeks of gestation at an antenatal clinic in Maternal and Child Health Hospital of Leping, Jiangxi Province, China. Periodontal Screening & Recording (PSR) was used to determine periodontal disease. Data on respondents’ demographic characteristics, gestational age, regular oral health habits, and mental health condition were collected using a self-administered questionnaire. ResultsThe prevalence of periodontal disease in pregnant women was 79.78% (95% CI: 76.7%~82.7%). Periodontal disease was significantly associated with increased maternal age (aOR =1.13, 95%CI: 1.06~1.21, P<0.001) and gum bleeding during tooth brushing (aOR =3.76, 95%CI: 2.05~7.00, P<0.001). The severity of periodontal disease advanced with the increased maternal age and gum bleeding during pregnancy. ConclusionsPregnant women with increased maternal age and gum bleeding during tooth brushing were at a higher risk of periodontal disease and needed to have the access to oral health care.

Abstract:Background Pregnancy-related periodontal health is vital for maternal and fetal well-being, with implications on birth outcomes. However, comprehensive data on periodontal health among pregnant women in Pakistan are lacking. This research aimed to assess the periodontal health status and treatment needs among pregnant women in Pakistan. Methodology This study conducted at Multan Medical and Dental College in Multan, Pakistan, utilized a cross-sectional design over one year from January 2023 to December 2023. It enrolled 230 pregnant women from the prenatal care clinic, employing strict inclusion and exclusion criteria to ensure data integrity. Data collection involved a systematic questionnaire and clinical examination by qualified dental practitioners, covering demographic details, obstetric history, dental hygiene habits, and periodontal health parameters. Statistical analysis included descriptive statistics, chi-square tests, and logistic regression to evaluate periodontal health status determinants. Results This study, comprising 230 participants, delineates key demographic and periodontal health indicators. Notably, the age group between 26 and 30 years made up the greatest percentage (n = 87, 37.83%), followed by the age group between 18 and 25 years (n = 58, 25.22%). The chi-square test showed a significant association between age and periodontal health (χ² = 8.23, df = 3, p = 0.041). University-educated participants showed decreased periodontal risks (odds ratio = 0.51, p = 0.037), with education level also emerging as a significant factor (χ² = 12.76, df = 2, p = 0.002). Regarding dental hygiene, 44.35% of people brushed twice a day, and 27.83% flossed every day. Periodontal data revealed that 53.04% of individuals had gingivitis and that the mean probing depth was 3.22 mm. Scaling and root planing were the most requested therapy (50.00%). Conclusions This study provides valuable insights into the periodontal health status and treatment needs of pregnant women in Pakistan. Investigative analyses including chi-square tests and logistic regression identified significant associations between demographic factors, oral hygiene practices, and periodontal health outcomes among pregnant women in Pakistan, emphasizing the need for tailored interventions to enhance maternal and child health.

Abstract:Background: It has been more than two decades since a link was found between maternal oral health and adverse pregnancy outcome. Obstetricians and gynecologists play a core role in the screening, prevention and referral of the pregnant women for periodontal diseases. Thus, a study was conducted to assess the knowledge of the association between periodontal status and pregnancy outcomes. Methods: An online survey was done by distributing self-administered questionnaire to the obstetricians and gynecologists. The survey consisted of three parts: demographic variables, consent and questionnaire. The qualitative data was recorded and analyzed. Results: Out of 237 participants, 95.8% of the participants agreed that there is a relation between periodontal disease and pregnancy; 73.8% with preterm birth, 60.3% with low birth weight infants and 38.4% with pre-eclampsia. Conclusions: The knowledge of the association between periodontal status and pregnancy outcomes was found to be insufficient. The gynecologists must be provided with periodic orientation to enhance and update their knowledge on maternal periodontal health.

Abstract:Following successful implementation of psychooncological support in a community hospital the focus has now been shifted to build up support for outpatients. This concentrates on cancer patients and their relatives during and following ambulatory cancer therapy. It is important to integrate these new structures into the preexisting support structures (self-help groups and counselling centres). Experiences of recent years have shown that information and psychosocial support should also be given to families and friends of cancer patients as well as to the involved clinical staff. Outpatient groups have been und will be offered in the psychooncological aftercare. Relaxation, imagination and creative therapy help to cope with the disease and enable a health-promoting life style.

Abstract:BACKGROUND:Periodontal disease is one of the most common chronic infectious diseases. It has been reported that periodontal disease is associated with various adverse pregnancy outcomes including preterm birth, low birth weight, and gestational diabetes mellitus. Given the fact that the treatment for periodontal disease during pregnancy was ineffective in improving pregnancy outcomes by most of studies, the pre-conception period has been put forward as a more optimal time. However, very few studies have reported the prevalence of periodontal disease among pre-conception women. This study aimed to examine the prevalence and risk factors of periodontal disease among Chinese pre-conception women.METHODS:A survey was conducted among pre-conception women at the Maternal and Child Health Hospital, Changzhou, China between January 2012 and December 2014. A total of 987 pre-conception women were recruited for a full-mouth dental examination after providing informed consent. A dental examination was carried out by probing six sites per tooth using a manual UNC-15 probe and a recording form.RESULTS:The overall rate of periodontal disease among participants was 73.9% (729/987) (95% confidence interval (CI): 71.0-76.6%). Among women with periodontal disease, 48.0% of cases were mild, 50.9% were moderate and 1.1% were severe. Self-reported bleeding during tooth brushing was the only significant predictive factor for overall periodontal disease (adjusted odds ratio (aOR): 3.71, 95% CI: 2.24, 6.15, P < 0.001) and moderate/severe periodontal disease (aOR: 5.17, 95% CI: 3.05, 8.79, P < 0.001).CONCLUSION:A high prevalence of periodontal disease was found in pre-conception Chinese women. Women who have bleeding during tooth brushing could be at increased risk of periodontal disease, and might require further oral health care.

Abstract:The purpose of this study was to systematically assess the epidemic trend of periodontal disease in pregnancy.Eligibility criteria comprised studies that reported periodontitis and the periodontal indicators of BOP (+) or CAL≥4 mm or PD≥4 mm among pregnant women. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were applied where applicable. Risk of bias was assessed using the Critical Appraisal Checklist for prevalence studies proposed by The Joanna Briggs Institute (JBI). Meta-analyses were conducted to estimate the pooled effect measures. Q-statistic, I2 statistic, subgroup and sensitivity analyses assessed study heterogeneity.Electronic search of articles was conducted using PubMed, Web of Science, EMBASE, Scopus, and Ovid from January 2000 to January 2022.A total of 20 studies were included in the meta-analysis. The prevalence of periodontitis among pregnancy was 40% (95% Confidence Interval (CI): [0.15, 1.00]). The prevalence rates were 67% (CI [0.56, 0.80]), 42% (CI [0.27, 0.57]) and 24% (CI [0.12, 0.37]) for BOP (+), PD≥4 mm and CAL≥4 mm respectively. Regarding subgroup meta-analyses, the prevalence rates of BOP (+) and PD≥4 mm presented a gradual increase throughout pregnancy, while the highest prevalence rate of CAL≥4 mm was in the 2nd trimester.It was observed a high prevalence of periodontal disease in pregnancy. However, heterogeneity was high among included studies. More high-quality epidemiologic investigations on periodontal disease in pregnancy are still needed.Periodontal disease in pregnancy is highly prevalent which results in a reduced quality of life, frequent systemic pathologies and adverse pregnancy outcomes. Given the unhealthy consequences, public health impact, and expansive disease burden, it is worthwhile to investigate more aspects of periodontal disease during pregnancy.
Oral Health Status and Dental Services Utilisation Among a Vulnerable Sample of Pregnant Women

Abstract:Introduction and aims: Oral health conditions during pregnancy can negatively impact both mother and fetus, highlighting the importance of maintaining dental care. In Canada, significant disparities exist between general and dental healthcare access, with limited evidence on oral health status and dental care utilisation among pregnant women. This study aimed to provide initial insights into self-perceived oral health status and dental utilisation patterns among a vulnerable sample of pregnant women in southwestern Ontario. Methods: This cross-sectional study recruited a convenient sample of pregnant women referred to the Family Medicine and Obstetrics Clinic, serving those without a family physician. Data were collected using a self-administered questionnaire adapted from the Canadian Health Measure Survey, the Canadian Community Health Survey, and the Survey of Maternal Health. Andersen and Newman's framework for health service utilisation was used as the theoretical framework. Descriptive and univariable analyses were conducted, followed by a forward stepwise selection of variables with p-values < 0.1 from the univariable analyses. Results: One-hundred-fifty patients were approached and 130 (86.7%) completed the questionnaire. Of these, 96 (73.9%) reported their oral health as good/excellent while 34 (26.2%) reported having poor/fair oral health. Education level and quality of life were the strongest predictors of oral health status. While 101 (77.7%) visited a dentist within the last 2 years, only 35 (26.9%) had a dental visit during pregnancy. Toothbrushing frequency was the main predictor of time since last dental visit, with no other predictive factors once toothbrushing was considered. Conclusion: The study sample showed relatively positive self-perceived oral health and dental visit patterns. However, the low rate of dental visits during pregnancy highlights the need for better integration of dental care into prenatal care. To ensure maternal and child health, pregnant women should be a high priority in policies aimed at improving access to dental care.
Prevalence and determinants of adverse perinatal outcomes of preeclampsia with severe features at two selected public hospitals in Addis Ababa, Ethiopia

Abstract:Background: Preeclampsia is a new onset of hypertension and either proteinuria or end-organ failure after 20 weeks of gestation. It is a prevalent cause of perinatal mortality, morbidity, and neonatal complications in developing nations including Ethiopia. Thus, the aimed to assess the prevalence and determinants of adverse perinatal outcomes among women with preeclampsia with severity features (PEWSF) at two selected public hospitals in Addis Ababa, Ethiopia, 2023. Method: A cross-sectional study was carried out among 348 mothers between January 1, 2023, and July 1, 2023. A structured, pre-tested questionnaire was used to gather data from in-person interviews and a review of the patient's medical record. The statistical program Epi-Data version 4.6 was used to enter the data, and SPSS version 26.0 was used for analysis. Binary logistic regression analysis was used to find factors that were associated with unfavorable perinatal outcomes. A p -value of less than 0.05 was used to declare the significance level. Result: The overall prevalence of unfavorable perinatal outcomes was 59.2% (95% CI: 54.0–63.8). Among the complications, low birth weight, prematurity, NICU admission, and a low fifth-minute APGAR score, encompass 48.9%, 39.4%, 20.4%, and 14.7%, respectively. No formal education [OR = 5.14, 95% CI: (1.93–13.63)], unemployment [OR = 0.42, 95% CI: (0.24–0.73)], referral cases [OR = 2.03, 95% CI: (1.08–4.06), inadequate antenatal care (ANC) contact [OR = 3.63, 95% CI: (1.22–10.71)], and family history of hypertension [OR = 1.99, 95% CI: (1.03–3.85)] have shown a statistically significant association with unfavorable perinatal outcomes. Conclusion: In this study, the prevalence of unfavorable perinatal outcomes was high compared to other studies in Ethiopia. Level of education, occupation, mode of admission, ANC contact, and family history of hypertension were significant predictors of unfavorable perinatal outcomes. Socio-economic development, improving referral systems, and adequate antenatal care contact are needed to improve unfavorable outcomes. Additionally, antenatal screening and specialized care for high-risk mothers, e.g., those with a family history of hypertension are recommended.

Association Between Periodontitis and Preterm Birth in a Cohort of Pregnant Women in Ivory Coast

Abstract:Purpose: The aim of this study was to investigate the possible association between periodontitis and preterm birth in Ivory Coast. Materials and methods: A cohort study including 446 volunteers (pregnant women) aged 15-50 years was performed in the Gynecology-Obstetrics Department of the University Hospital Center of Cocody-Abidjan in Ivory Coast. Socioeconomic and periodontal status was obtained during pregnancy. After delivery, obstetric data was collected. Periodontitis was diagnosed according to the new 2018 EFP/AAP classification of Periodontal and Peri-Implant Diseases and Conditions, as follows: a subject presenting with interdental CAL at two non-adjacent teeth or buccal/oral CAL ≥ 3 mm with pocketing > 3 mm was diagnosed with periodontitis. Any birth before the 37th week was considered a preterm birth (PTB). Results: The prevalence of periodontitis and preterm birth were 59.47% and 18.34%, respectively. Periodontitis was mainly stage 1. PTB was statistically significantly higher in pregnant women with periodontitis compared to women without periodontitis (p = 0.0002). Multivariate analysis showed that periodontitis was associated with PTB (p = 0.0002). Logistic regression showed that periodontitis is a risk factor for preterm birth (OR = 3.62; 95% CI: 1.80-7.31; p = 0.0003). Conclusion: The results of this study suggest that periodontitis is an additional risk factor for preterm birth in Ivory Coast.

Abstract:Background: This study aimed to investigate the detection of periodontal indicators in pregnant women with periodontal disease and its impact on pregnancy outcomes. Our goal is to provide guidance for the clinical diagnosis and outcome prognosis evaluation for pregnant patients. Methods: From August 2020 to February 2021, we conducted a study to investigate the detection of periodontal indicators in pregnant women with periodontal disease and its impact on pregnancy outcomes. A total of 84 pregnant women with periodontal disease were selected as the experimental group, while 86 pregnant women with normal periodontal health during pregnancy were selected as the control group. The general data of the women in both groups postpartum were similar. We measured the volume of gingival crevicular fluid (GCF) obtained and determined the concentration of interleukin-1 beta (IL-1β) in the GCF. This research aimed to provide guidance for clinical diagnosis and pregnancy outcome prognosis. Results: The experimental group exhibited significantly higher levels of periodontal indicators such as the Simplified Oral Hygiene Index (sulcus bleeding index, SBI), probing depth (PD), and Oral Hygiene Index-Simplified (OHI-S) compared to the control group. Conversely, the clinical attachment levels (CAL) in the experimental group were significantly lower than those in the control group. Additionally, the IL-1β levels and GCF levels in the experimental group were significantly higher than those in the control group. Furthermore, the incidence of adverse pregnancy outcomes in the experimental group was 2 patients (2.38%), which was significantly lower compared to the control group where it was 13 (15.11%). The adverse pregnancy outcomes included amniotic fluid contamination, premature delivery, and low body weight. Conclusions: We observed that periodontal indicators, including OHI-S, CAL, SBI, and PD, were significantly elevated in pregnant women with periodontal disease. Additionally, we found that IL-1β levels had a substantial influence on pregnancy outcomes, such as amniotic fluid contamination, premature delivery, and low birth weight.

Abstract:1053 Background: Cabazitaxel (X), a new taxoid showed activity in taxane resistant MBC. Capecitabine (C) is approved in MBC pts pretreated with anthracycline and taxane. METHODS A standard 3+3 escalation scheme explored doses of combined intravenous X (Day (D)1) with oral C twice daily (D1to14), every 3 weeks (q3w). The study objectives were the identification of dose limiting toxicities (DLTs), recommended dose (RD) of the combination, assessment of safety, pharmacokinetics (PK) and activity at the RD in an expanded cohort. RESULTS 32 MBC pts pretreated with taxane and anthracycline were enrolled and treated (15 in the dose escalation part and 17 at the RD). Data for the first 25 pts, are available so far: median age 52 [34-74], ECOG-PS 0/1: 15/10, in first or second line chemotherapy, median of 3 (1-7) organs involved (mainly: bone, liver, lymph nodes). In the escalation part, X+C were administered at 3 dose levels (DL) (Table). DL2 was defined as the RD and the expansion cohort was initiated. PK analysis did not show any drug-drug interaction with this schedule of administration. Overall, out of the 25 pts (125cy), the main Gr3-4 toxicities (N pts) were asthenia (4), hand-foot syndrome (4), neutropenia (15), febrile neutropenia (1), neutropenic infection (1), no toxic death. Efficacy was observed at each DL with a total of 1 complete response, 4 partial responses (PR) and 16 stabilizations (including 3 unconfirmed PR). CONCLUSIONS X was safely combined to C. Abstract:Background: This study aimed to subjectively assess the changes in the oral health status of pregnant women, to find out their attitudes and knowledge about possible changes in the oral cavity that occurred during pregnancy, and their influence on the outcomes of pregnancy. Methods: 112 pregnant women who visited the Republican Siauliai Hospital during their pregnancy participated in the study. An anonymous self-administered questionnaire was used to conduct the research, surveying their demographic characteristics, oral health changes and habits during pregnancy, and awareness of possible negative pregnancy outcomes. Results: The study involved 112 pregnant women, with 35 in the first trimester, 28 in the second, and 48 in the third trimester. The findings revealed that pregnant women do not take sufficient care of their oral health: more than half of the respondents did not visit an oral care specialist during pregnancy; 22.3% of women reported brushing their teeth only once a day or less; and 35.7% did not clean interdental surfaces at all. Statistically, significantly more urban women believed there is a relationship between maternal oral health and adverse pregnancy outcomes than women living in rural areas (p = 0.013). While significant oral health changes were not observed in the first trimester, more women in the second and third trimesters reported issues such as bleeding gums, swelling, plaque accumulation, tartar, and caries (p < 0.001). Conclusions: The attitude of women towards oral health during pregnancy and their understanding of the possible risks for unfavorable pregnancy outcomes are insufficient. Notably, oral health issues become more prominent in the second and third trimesters, necessitating appropriate oral care to reduce the incidence of oral and dental diseases during pregnancy. This underscores the importance of educational and preventive public health policies focused on oral care for pregnant women, aimed at increasing awareness and promoting practices that safeguard both maternal and fetal health.

Abstract:Introduction: Conducting dental consultations during pregnancy is considered an important challenge in the context of Maternal and Child Health Policies, as it is surrounded by myths rooted among users and health professionals. In this sense, it is important to identify barriers and facilitators to the search for dental assistance in this period, in order to support strategies that make this practice feasible. Objective: To analyze the search for dental care during high-risk pregnancies, according to sociodemographic, gestational and health characteristics. Methods: Observational study with a cross-sectional design, carried out with high-risk pregnant women referred to a teaching hospital in southern Brazil, from January to May 2018. Data collection was performed using an unprecedented structured form and considered as a dependent variable the search for dental care during pregnancy and as independent variables sociodemographic, gestational and dental characteristics. Pearson's chi-square association test and Fisher's exact test were used. Results: To reach the sample of 190 pregnant women at high gestational risk, a total of 230 women considered valid were approached, counting on the following losses: refusal to participate (n=23), no answer to any question (n=10), duplicity in participant approach (n=7). Advanced maternal age (p=0.000) and history of premature birth in previous pregnancies (p=0.047) were factors associated with a lower frequency of seeking dental care in the current pregnancy. On the other hand, the habit of dental consultation prior to the gestational period (p=0.001), the knowledge about the importance of this monitoring (p=0.050), as well as the safety (p=0.000) in performing dental prenatal care, were related positively to the search during pregnancy. Conclusion: Specific incentive strategies and access to dental prenatal care are necessary to neutralize barriers that may compromise the search for oral health services during pregnancy. For this reason, identifying the facilitators and hinders to the dental service is essential for planning effective actions related to prenatal care.
Periodontal condition is associated with adverse perinatal outcomes and premature rupture of membranes in low-income pregnant women in Bogota, Colombia: a case-control study

Abstract:Objectives: To assess the periodontal condition as a factor associated with adverse perinatal outcomes, premature rupture of membranes (PRM), and preeclampsia in low-income pregnant women treated at public hospitals in Bogotá, Colombia.Methods: Pregnant women with preterm birth (PTB) and low birth weight (LBW) or both conditions (n = 107/428), or only PTB (n = 73/292) or LBW (n = 74/296) or with PRM (n = 98/392) or preeclampsia (n = 76/304) in a ratio of four controls for each case, coming from three hospitals of the public Northern Network of Bogotá, Colombia were studied. Sociodemographic, perinatal adverse outcome history, antenatal care, chronic infections, periodontal condition, threatened abortion, bleeding in the second half of pregnancy, oligohydramnios, diabetes, gestational diabetes, alcohol consumption, hypertension, smoking, alcohol during pregnancy were determined. Logistic regression was conducted to establish factors associated to perinatal adverse outcomes. Multiple correspondence analysis was conducted as secondary analysis.Results: Threatened abortion, absence of antenatal care, hypertension, chronic infections, and periodontal condition were the most important factors associated with perinatal adverse outcomes. The presence of periodontal pockets was associated with LBW OR 2.52 (IC95% 1.36-4.70), PTB OR 2.04 (IC95% 1.10-3.64), PTB-LBW or both OR 2.08 (IC95% 1.18-3.31), PRM OR 2.04 (IC95% 1.17-3.56). Periodontal pockets presence was not associated with preeclampsia. Multiple correspondence analyses showed high correlation between PRM with chronic infection and presence of periodontal pockets.Conclusions: Periodontal condition is a factor independent of other important risk factors for a perinatal adverse outcome and PRM. Prevention of periodontal disease should be included in preconception and prenatal care programs.
Predictors of adverse perinatal outcome among women who gave birth at Medical Center of Southwest Ethiopia: a retrospective cohort study

Abstract:Objectives The aim of this study was to determine predictors of adverse perinatal outcome among women who gave birth at Medical Center of Southwest Ethiopia. Setting Institutional based retrospective cohort study was conducted among women who gave birth at Medical Center of Southwest Ethiopia. Participants Medical record of 777 women was included in the study by using maternity HMIS logbook as entry point. Simple random sampling technique without replacement was employed to select individual medical record using computer generated random numbers. Primary outcome measured Predictors of adverse perinatal outcome were examined using modified Poisson regression with a robust SE. Results Majority, 74.1% of the participants were in the age group of 21–34 years and the median age was 26 (IQR=7) years. More than one-third, 35.9% of the mothers were primigravida and only 21.2% of them had above four antenatal cares (ANC) visit. The overall incidence of adverse perinatal outcome was 31.5% (95% CI: 28.3 to 34.9). Maternal age less than 20 years (adjusted risk ratio, aRR=1.3; 95% CI: 1.01 to 1.5), rural residence (aRR=1.27; 95% CI: 1.04 to 1.59), presence of antepartum haemorrhage in current pregnancy (aRR=1.7; 95% CI: 1.38 to 2.07), maternal anaemia (aRR=1.25; 95% CI: 1.03 to 1.53), lack of ANC visit (aRR=2.29; 95% CI: 1.35 to 3.90), induced labour (aRR=1.77; 95% CI: 1.43 to 2.19) and being positive for venereal disease research laboratory (VDRL) test in current pregnancy (aRR=2.0; 95% CI: 1.16 to 3.38) were found to be significantly associated with adverse perinatal outcome. Conclusion The incidence of adverse perinatal outcome in the study area is high and maternal age less than 20, rural residency, maternal anaemia, antepartum haemorrhage in the current pregnancy, inadequate ANC visit, induction of labour and being positive for VDRL test were found to predict occurrence of adverse perinatal outcome. Majority of these problems can be managed by providing quality antenatal, intrapartum and post-natal care.

Abstract:Background: Periodontal disease (PD) is a chronic inflammatory disease of multifactorial origin that affects the supporting tissues of the tooth. According to WHO in 2022, it affects around 19% of the adult population worldwide. Its risk factors include the presence of periodontopathogenic bacteria in biofilm and the susceptibility of the host's immune system, among others. Preterm birth is defined as birth occurring before 37 weeks of gestational age. It also has a multifactorial origin and it's associated with risk factors such as intrauterine and extrauterine infections. There is a possibility that periodontal disease in pregnant women increases the risk of preterm birth through hematogenous pathways or the presence and intervention of inflammatory mediators. Material and methods: Through a systematic review of existing scientific articles from 2014 to 2024, five randomized clinical trials were selected, including a total of 1984 pregnant patients diagnosed with periodontal disease. Half of these patients received non-surgical treatment, while the other half did not, aiming to evaluate a possible association between periodontal disease and/or its treatment and the occurrence of preterm birth. The risk of bias was assessed using the Cochrane "RoB 2" tool, and finally, a meta-analysis was conducted to compare the results obtained in the selected studies. Results: Four articles showed a trend favoring non-surgical periodontal treatment as a preventive measure for preterm birth. These articles accounted for 92.2% of the total weight, while the remaining 7.85% corresponded to the single article that did not favor the treatment. However, none of the articles showed statistically significant results. Conclusions: There is no demonstrated association between periodontal disease in pregnant women and the incidence of preterm birth. On the other hand, there is a slightly favorable trend towards non-surgical periodontal treatment during pregnancy as a measure to reduce the incidence of preterm birth, but it's not statistically significant. To obtain a definitive answer, more randomized clinical trials following similar study and design parameters are needed.
