Abstract:We present six cases of psoas abscess, three primary and three secondary. The etiologic, pathogenic and therapeutic considerations are outlined. Staphylococcal infections prevail in the primary abscesses. The diagnostic difficulties are considerable but nowadays are facilitated by echography and TAC. In the secondary ones, the psoas abscess is an epiphenomenon of a more easily recognised clinical syndrome. It is surprising how these septic processes are increasingly placed in the hands of urologists in recognition of their mastery of retroperitoneal surgery.
Advanced Radio Frequency Applicators for Thermal Magnetic Resonance Theranostics of Brain Tumors

Abstract:Thermal Magnetic Resonance (ThermalMR) is a theranostic concept that combines diagnostic magnetic resonance imaging (MRI) with targeted thermal therapy in the hyperthermia (HT) range using a radiofrequency (RF) applicator in an integrated system. ThermalMR adds a therapeutic dimension to a diagnostic MRI device. Focused, targeted RF heating of deep-seated brain tumors, accurate non-invasive temperature monitoring and high-resolution MRI are specific requirements of ThermalMR that can be addressed with novel concepts in RF applicator design. This work examines hybrid RF applicator arrays combining loop and self-grounded bow-tie (SGBT) dipole antennas for ThermalMR of brain tumors, at magnetic field strengths of 7.0 T, 9.4 T and 10.5 T. These high-density RF arrays improve the feasible transmission channel count, and provide additional degrees of freedom for RF shimming not afforded by using dipole antennas only, for superior thermal therapy and MRI diagnostics. These improvements are especially relevant for ThermalMR theranostics of deep-seated brain tumors because of the small surface area of the head. ThermalMR RF applicators with the hybrid loop+SGBT dipole design outperformed applicators using dipole-only and loop-only designs, with superior MRI performance and targeted RF heating. Array variants with a horse-shoe configuration covering an arc (270°) around the head avoiding the eyes performed better than designs with 360° coverage, with a 1.3 °C higher temperature rise inside the tumor while sparing healthy tissue. Our EMF and temperature simulations performed on a virtual patient with a clinically realistic intracranial tumor provide a technical foundation for implementation of advanced RF applicators tailored for ThermalMR theranostics of brain tumors.

Abstract:Thermal Magnetic Resonance (ThermalMR) leverages radio frequency (RF)-induced heating to examine the role of temperature in biological systems and disease. To advance RF heating with multi-channel RF antenna arrays and overcome the shortcomings of current RF signal sources, this work reports on a 32-channel modular signal generator (SGPLL). The SGPLL was designed around phase-locked loop (PLL) chips and a field-programmable gate array chip. To examine the system properties, switching/settling times, accuracy of RF power level and phase shifting were characterized. Electric field manipulation was successfully demonstrated in deionized water. RF heating was conducted in a phantom setup using self-grounded bow-tie RF antennae driven by the SGPLL. Commercial signal generators limited to a lower number of RF channels were used for comparison. RF heating was evaluated with numerical temperature simulations and experimentally validated with MR thermometry. Numerical temperature simulations and heating experiments controlled by the SGPLL revealed the same RF interference patterns. Upon RF heating similar temperature changes across the phantom were observed for the SGPLL and for the commercial devices. To conclude, this work presents the first 32-channel modular signal source for RF heating. The large number of coherent RF channels, wide frequency range and accurate phase shift provided by the SGPLL form a technological basis for ThermalMR controlled hyperthermia anti-cancer treatment.

Abstract:OBJECTIVE To investigate hemostatic analyte abnormalities and their association with mortality in dogs with naturally occurring heatstroke. DESIGN Prospective observational study. SETTING University teaching hospital. ANIMALS Thirty client-owned dogs with naturally occurring heatstroke. INTERVENTIONS None. MEASUREMENTS AND MAIN RESULTS Citrated and EDTA blood samples were collected at presentation and at 4, 12, 24, 36, and 48 hours postpresentation (PP). Hemostatic tests performed included platelet count, prothrombin and activated partial thromboplastin times (PT and aPTT, respectively), antithrombin activity (ATA), total protein C activity (tPCA), fibrinogen, and D-dimer concentrations. The overall survival rate was 60% (18/30 dogs). Older age, higher heart rate and rectal temperature at presentation, and time from onset of clinical signs to presentation were significantly associated with mortality. Hemostatic analytes at presentation were not associated with mortality. Prolonged PT and aPTT at 12-24 hours PP, lower tPCA at 12 hours PP, and hypofibrinogenemia at 24 hours PP were significantly (P < 0.05) associated with mortality. Increased D-dimer concentration and low ATA were common at all time points, but were not associated with mortality. The frequency of disseminated intravascular coagulation (DIC) increased in nonsurvivors throughout hospitalization, but the development of DIC was not associated with mortality. The number of abnormal coagulation disturbances during the first 24 hours was significantly higher in nonsurvivors (P = 0.04). CONCLUSIONS Hemostatic derangements are common in dogs with naturally occurring heatstroke. Alterations in PT, aPTT, tPCA, and fibrinogen concentrations appear to be associated with the outcome at 12-24 hours PP, exemplifying the need for serial measurement of multiple laboratory hemostatic tests during hospitalization, even when within reference interval on presentation. The development of DIC, as defined in this cohort, was not associated with mortality; however, nonsurvivors had significantly more coagulation abnormalities during the first 24 hours PP.

Abstract:Previous studies have confirmed the possibility of using an intravascular MR imaging guidewire (MRIG) as a heating source to enhance vascular gene transfection/expression. This motivated us to develop a new intravascular system that can perform MR imaging, radiofrequncy (RF) heating, and MR temperature monitoring simultaneously in an MR scanner. To validate this concept, a series of mathematical simulations of RF power loss along a 0.032-inch MRIG and RF energy spatial distribution were performed to determine the optimum RIF heating frequency. Then, an RF generator/amplifier and a filter box were built. The possibility for simultaneous RIF heating and MR thermal mapping of the system was confirmed in vitro using a phantom, and the obtained thermal mapping profile was compared with the simulated RF power distribution. Subsequently, the feasibility of simultaneous RF heating and temperature monitoring was successfully validated in vivo in the aorta of living rabbits. This MR imaging/RF heating system offers a potential tool for intravascular MR-mediated, RF-enhanced vascular gene therapy. (c) 2005 Wiley-Liss, Inc.

Abstract:Introduction Little is known about how the nutrition environment in after-school settings may affect children’s dietary intake. We measured the nutritional quality of after-school snacks provided by programs participating in the National School Lunch Program or the Child and Adult Care Food Program and compared them with snacks brought from home or purchased elsewhere (nonprogram snacks). We quantified the effect of nonprogram snacks on the dietary intake of children who also received program-provided snacks during after-school time. Our study objective was to determine how different sources of snacks affect children’s snack consumption in after-school settings. Methods We recorded snacks served to and brought in by 298 children in 18 after-school programs in Boston, Massachusetts, on 5 program days in April and May 2011. We measured children’s snack consumption on 2 program days using a validated observation protocol. We then calculated within-child change-in-change models to estimate the effect of nonprogram snacks on children’s dietary intake after school. Results Nonprogram snacks contained more sugary beverages and candy than program-provided snacks. Having a nonprogram snack was associated with significantly higher consumption of total calories (+114.7 kcal, P < .001), sugar-sweetened beverages (+0.5 oz, P = .01), desserts (+0.3 servings, P < .001), and foods with added sugars (+0.5 servings; P < .001) during the snack period. Conclusion On days when children brought their own after-school snack, they consumed more salty and sugary foods and nearly twice as many calories than on days when they consumed only program-provided snacks. Policy strategies limiting nonprogram snacks or setting nutritional standards for them in after-school settings should be explored further as a way to promote child health.

Abstract:PURPOSE: This effort describes a third-party performance evaluation of a novel, commercial, dual-armed Archimedean spiral array hyperthermia applicator. The applicator is analysed for its ability to couple efficiently into muscle equivalent phantom loads, operate over a broad bandwidth to help accommodate variable tissue properties and generate predictable and repeatable SAR contours that are adaptable to clinically probable disease shapes.MATERIALS AND METHODS: Characterization of the applicator includes E-field and return-loss measurements in liquid muscle tissue-equivalent phantom, as well as comparison of 'treatment-planning' simulations of several possible array SAR patterns with measured SAR from non-coherently driven spiral array antennae.RESULTS: The applicator demonstrates a reasonably low return loss over a large bandwidth and the ability to generate a very uniform heating pattern. Ability to adjust SAR contours spatially to fit specific shapes is also demonstrated.CONCLUSIONS: This device should prove a welcome addition to a currently limited set of superficial heating applicators to provide controllable heating of superficial tissue disease.

Abstract:This research describes a metamaterial-based applicator with and without water bolus for cancer treatment. The metamaterial based applicator consists of a double spiral antenna and a slotted square shape artificial magnetic conductor (SSA) structure. The antenna is designed on a low-cost FR4 substrate with dimensions of 32 × 32 × 3.27 mm 3 , and the SSA unit cell which behaves as a metamaterial, is designed on an RT-duroid substrate with dimensions of 15 × 15 × 0.76 mm 3 (size of the unit cell). The 4 × 4 unit cells of the SSA structure are optimized to direct the maximum field toward the cancer tissue. The proposed applicator (antenna + SSA structure) is tested for hyperthermia treatment using a heterogeneous phantom (skin, fat, and muscle layers) and the human-mimicked model with an air medium and a water bolus layer. The applicator's performance is evaluated in terms of specific absorption rate (SAR), penetration depth (PD), and effective field size (EFS). Further, thermal analysis is performed, with 1.5 W of input power at the antenna port, and the maximum temperature rise of 44 0 C is obtained. The cancer tissue (tumor) temperature ranges between 41 °C to 45 °C, sufficient for effective hyperthermia therapy. Finally, the suggested metamaterial based applicator is built and tested in the presence of a phantom and head tissue simulating liquid.

[The possibilities of hyperthermia from an engineering standpoint]

Abstract:The possibilities of hyperthermia are discussed from the engineering technology standpoint. Microwave equipment yields powerful effects against a superficial tumor. RF capacitive method and annular phased array system (APAS) are typical heating methods against a deep-seated tumor. We have proposed that in the RF capacitive method the diameter of the applicator electrode should be more than 1.5 times the distance between two electrodes. In APAS another exciting method (TE mode) has been proposed. Electric current flows through the short axis of the body by the TE mode excitation. We are developing re-entrant resonant cavity applicators which have the characteristics to heat cylindrical regions of small diameter. We can install an x-ray tube inside the re-entrant electrode. This method is suitable for deep and concentrated heating. We conclude that the recent rapid development of various kinds of hyperthermia apparatus makes it possible to heat more safely and effectively the deep-seated regions of the human body.

Abstract:Numerous designs and methods have been examined to improve penetration depth (PD), but there is a need for research to explore the potential increase in PD through uniform heating, a compact applicator, and low input power. This paper presents metasurface based hyperthermia lens applicator with water bolus for uniform heating of cancerous tissues. The proposed applicator consists of a stacked spiral antenna and a spiral-shaped frequency selective surface as a superstrate. The spiral antenna and superstrate are optimized on a low cost FR4 substrate having a size of 32 × 32 × 3.27mm3 and 10 × 10 × 1.6mm3 (size of the unit cell), respectively. The proposed applicator is simulated with heterogeneous phantom (skin, fat, and muscle layers) and with the Gustav voxel model with and without a water bolus layer. The number of unit cells in the superstrate is optimized to direct the maximum energy toward the tumor location. The performance study of the applicator is carried out in terms of specific absorption rate, PD, and effective field size. Further, thermal analysis is carried out with 1.9 W of input power at the antenna port, and the highest 44.7 °C temperature rise is obtained. The cancerous tissue's (tumor) surrounding temperature is between 41 and 45 °C, which is adequate for efficient hyperthermia treatment. Finally, the proposed metasurface hyperthermia lens applicator is fabricated and experimentally validated in a mimicked phantom's presence. Abstract:Mild whole-body hyperthermia has been shown to have anti-tumor effects through an immune-modulating mechanism. Before it is widely applied in the clinic, tremendous mechanistic research in animals is necessary to adhere to evidence-based principles. The radio frequency electromagnetic field (RF-EMF) based heating facility could be a good choice for hyperthermia treatment, but the heating characteristics of a facility, including structure design, electromagnetic and thermal dosimetry, and the biologic effects of hyperthermia, need to be well elucidated. Here, we reported the heating characteristic study on a resonant chamber (RC) excited by a 1800 MHz solid source. The EMF in the RC was stirred by 24 static reflectors, which resulted in the standard deviation of electric field intensity being below 3 dB in the EM homogeneity evaluation. For the exposure scenario, six free-moving mice were loaded into separate cases and exposed simultaneously in the RC. The EMF energy absorption and distribution in exposed mice were calculated with the 12-plane-waves method of numerical simulation. Different levels of core body temperature increment in exposed mice were achieved through regulation of the source output power. Overexpression of heat shock proteins (HSPs) was detected in the liver, lung and muscle, but not in the brain of the exposed mice. The levels of representative inflammatory cytokines in the serum, TNF-α and IL-10 increased post RC exposure. Based on the heating characteristic study and validation, the applied RC would be a qualified heating system for mild whole-body hyperthermia effect research in mice. Mild whole-body hyperthermia has potential anti-tumor effects by modulating the immune system. A radio frequency electromagnetic field (RF-EMF)-based heating facility emerges as a suitable option for hyperthermia treatment. However, a qualified heating facility for scientific research must elucidate its heating characteristics and validate the biological effects associated with hyperthermia. In this study, we report the characteristics of a rodent heating chamber using EMF energy. The special structure of the chamber not only achieved efficient EMF usage but also ensured the homogeneity in EMF spatial distribution, animal EM absorption, and EMF-caused biological effects. Our work may offer insights for designing a low-cost yet reliable heating facility for scientific research.

Abstract:To investigate the potentialities of hyperthermia as a cancer therapy, computer simulations have been performed. This simulation consists of two tuccessive steps. First, the heat generated in a distribution of biological tissue when irradiated by a source of electromagnetic radiation is computed. The mathematical tool for determining the disbution of generated heat is the domain-integral-equation technique. This technique enables us to determine in a body with arbitrary distribution of permittivity and conductivity the electromagnetic field due to prescribed sources. The integral equation is solved numerically by an iterative minimization of the integrated square error. From the computed distribution of generated heat, the temperature distribution follows by solving numerically the pertaining heat transfer problem. The relevant differential equation together with initial and boundary conditions is solved numerically using a finite-element technique in space and a finite-difference technique in time. Numerical results pertaining to the temperature distribution in a model of the human pelvis are presented.

Abstract:A magnetic resonance (MR)-compatible three-dimensional (3-D) hyperthermia applicator was developed and evaluated in the magnetic resonance (MR) tomograph Siemens MAGNETOM Symphony 1.5 T. Radiating elements of this applicator are 12 so-called water coated antenna (WACOA) modules, which are designed as specially shaped and adjustable dipole structures in hermetically closed cassettes that are filled by deionized water. The WACOA modules are arranged in the applicator frame in two transversal antenna subarrays, six antennas per subarray. As a standard load for the applicator an inhomogeneous phantom was fabricated. Details of applicator's realization are presented and a 3-D comparison of calculated and measured temperature data sets is made. A fair agreement is achieved that demonstrates the numerically supported applicator's ability of phase-defined 3-D pattern steering. Further refinement of numerical models and measuring methods is necessary. The applicator's design and the E-field calculations were performed using the finite-difference time-domain (FDTD) method. The calculation and optimization of temperature patterns was obtained using the finite element method (FEM). For MR temperature measurements the proton resonance frequency (PRF) method was used.
IMMUNE RESPONSE IN MALIGNANT GLIOMA

Abstract:SUMMARY Objective: Malignant gliomas are primary brain tumors with excessive mortality and high resistance to chemotherapy and radiotherapy. The survival time for glioblastoma multiforme is about 6-12 months. As key pathogenetic mechanisms are recognized the massive necrosis, angiogenesis and hypoxia within the tumor, as well as the resistance to apoptosis. It is also suspected that altered immune response might contribute to the fatal clinical outcome. The aim of the present study was to determine the immune status of patients with malignant gliomas. Material and methods: Peripheral blood lymphocytes were collected preoperatively from 9 patients (aged 57-76) diagnosed as anaplastic astrocytoma grade III (n=4) and glioblastoma multiforme (n=5). The following lymphocyte populations were analyzed by flow cytometry: CD19+, CD3+, CD3+CD4+, CD3+CD8+, CD3-CD56+, CD3+CD56+, CD3+CD25+, CD8-CD11b+, CD8+CD11b+, CD8+CD11b-. The results obtained were compared to reference values for each cell population. Results: No significant alterations were detected in CD19+, CD3+, CD3+CD4+, CD3+CD8+ cells, but the CD4/CD8 ratio was below the reference range in some cases. No obvious decrease in (CD3-CD56+) NK cells and (CD3-CD56+) NKT cells was observed in most patients. A reproducible phenomenon of increased CD8+CD11b+ and decreased CD8+CD11b- cells was noticed. These preliminary results suggest that the immune response in patients with malignant glioma is seriously disregulated. The rapid clinical deterioration, relapses and high mortality could be at least partially explained with the suppressed activity of NK-cells which are the major cytotoxic antitumoral cells. The increase in the population of activated suppressor-effector cells also contributes to the unfavourable outcome in malignant brain tumors. Conclusion: This pilot study reveals the presence of altered immune response in malignant gliomas and opens possibilities for prospective investigations concerning immune status and clinical outcome.

Abstract:Advancement of the recent micro/nanotechnology stimulates the renaissance of using magnetic micro/nanoparticles embedded in tissues for the target tumor hyperthermia. However, there is a strong lack of quantitative understanding of the temperature profiles thus induced by the applied external electromagnetic (EM) field, which may impede the successful operation of this therapy. In the current study, the three-dimensional quasi-steady-state EM field and transient tissue temperature behavior induced by two planar electrodes were numerically investigated. Detailed computations indicated that nanoparticles exhibit an extraordinary highly focused heating on target tumor tissue, which is much stronger than that in the surrounding areas. This heating effect depends heavily on the properties of the magnetic nanoparticles, which may vary appreciably for different samples depending on their particle sizes and microstructures. The effect of micro/nanoparticle concentration, heating area, and the frequency and strength of the external alternating EM field were also tested. Moreover, a criterion to determine the appropriate particle concentration thermally important for medical treatment was established. Given accurate thermal and EM parameters for cancerous tissue embedded with nanoparticles, the current model could possibly be applied in the hyperthermia treatment planning and help optimize the surgical procedures.

Abstract:In the 21st century, new and alternative non-radioactive methods for cancer treatment have been developed. One of this is magnetic hyperthermia (MH), which is based on the ability of magnetic nanoparticles (MNP) to transduce electromagnetic energy into heat. If MNP are inside cancer cells, heat induces cells death. The success of HM as cancer therapy depends on two factors, first, the NPM properties and second, the frequency and amplitude of the radio frequency (RF) applied field. Therefore, in this work a design of a portable radio frequency source for magnetic hyperthermia experiments is presented. The design consists of two parts, the first part corresponds to the RF source composed by a RLC parallel circuit, the capacity and inductance values have been chosen in order to achieve the RF range of frequency, and the second part corresponds to the design of a magnetization sensor circuit. While the first circuit produces the RF field, the second circuit serves as a detector of the NPM induced magnetization. The development of this radiofrequency source makes it possible to determine the applicability of an NPM system in the treatment of cancer by magnetic hyperthermia, opening doors for future research focused on the application of nanotechnology in biomedicine.

Abstract:Cancer affects the lives of many people across the world. Indeed, it is the second leading cause of death globally. There are a number of existing cancer treatments but they have limitations. For example, many treatments destroy diseased tissue but also damage healthy tissue in the process. Professor Kazuo Kato, Department of Mechanical Engineering Informatics, School of Science and Technology, Meiji University, Japan, is working to discover how cancer cells can be killed without harming the patient. This work centres on the principles that underlie hyperthermia, which is a cancer treatment that can treat cancer without damaging the human body. It does this by exposing body tissue to high temperatures and cancer cells are more sensitive to heat than normal cells. Kato and the team are developing a hyperthermia system that is able to heat cells in various tissues while using ultrasound images to non-invasively measure the 3D temperature distribution inside the human body. Using this technique, cancerous tissues can be heated and treated non invasively. An additional benefit of the novel ultrasound imaging system and cavity resonator that the researchers are developing is that it is smaller and less expensive than existing treatments.
A Feasibility Study of Temperature Rise Measurement in A Tissue Phantom As an Alternative Way for Characterization of the Therapeutic High Intensity Focused Ultrasonic Field

Abstract:The feasibility that temperature field measurements in vitro as an alternative way to characterize the high intensity focused ultrasound (HIFU) field used in therapeutic applications has been explored in a phantom study. Thermocouples (copper-constantan, diameter 0.125mm) are embedded in a phantom filled with tissue mimicking material that simulates the thermal and acoustic properties of soft-tissue. The temperature rises as a function of ultrasound exposure time near the focus of a HIFU transducer (1.1MHz, active radius a=32mm, geometric focal length=62mm) of various acoustic powers up to 30W are measured and compared with predicted values using a simple nonlinear Gaussian model. The experimental results can be explained well by the model if no acoustic cavitation takes place. When the acoustic power become higher (>5W) and the local temperature elevation >15°C and the local temperature is >40°C at the focal point, cavitation vapor bubbles appear. The presence of the cavitation bubbles may increase the temperature rise rate initially. The bubble aggregates may form along the beam axis under sonication and then eventually makes the temperature elevation reach a saturated value. When acoustic cavitation occurs, the bubble-assisted enhancement of the initial temperature rise (exposure time t<2s) can still be predicted by the theory.
Fast Optimization of Temperature Focusing in Hyperthermia Treatment of Sub-Superficial Tumors

Abstract:Microwave hyperthermia aims at selectively heating cancer cells to a supra-physiological temperature. For non-superficial tumors, this can be achieved by means of an antenna array equipped with a proper cooling system (the water bolus) to avoid overheating of the skin. In patient-specific treatment planning, antenna feedings are optimized to maximize the specific absorption rate (SAR) inside the tumor, or to directly maximize the temperature there, involving a higher numerical cost. We present here a method to effect a low-complexity temperature-based planning. It arises from recognizing that SAR and temperature have shifted peaks due to thermal boundary conditions at the water bolus and for physiological effects like air flow in respiratory ducts. In our method, temperature focusing on the tumor is achieved via a SAR-based optimization of the antenna excitations, but optimizing its target to account for the cooling effects. The temperature optimization process is turned into finding a SAR peak position that maximizes the chosen temperature objective function. Application of this method to the 3D head and neck region provides a temperature coverage that is consistently better than that obtained with SAR-optimization alone, also considering uncertainties in thermal parameters. This improvement is obtained by solving the bioheat equation a reduced number of times, avoiding its inclusion in a global optimization process.

Abstract:In order to grow organisms in such a manner as to mimic their physiological growth state in vivo, it is often desirable to grow them as biofilms in the laboratory. There are numerous systems available to accomplish this; however, some are more suited to the growth of oral biofilms (dental plaque) than others. The operating parameters of one such model, the constant depth film fermentor (CDFF), are given in this unit. This model is particularly suited to studying the varied biofilms which exist in the oral cavity because environmental factors such as the substratum, nutrient source, and gas flow can be altered.
