Abstract:BACKGROUND The aim of the present study was to investigate immunohistochemically the distribution of secretoneurin, a novel 33 amino acid peptide, in comparison to chromogranin A, chromogranin B, and secretogranin II in carcinoids of the appendix. MATERIALS AND METHODS Paraffin-embedded tissues from 47 carcinoids were incubated with antibodies specific for chromogranin A, chromogranin B, the secretogranin II derived peptide LF- 19, and secretoneurin. RESULTS 44 tumors (94%) were positive for secretoneurin, whereas only 39 tumors (83%) were immunoreactive for chromogranin A. There was no significant correlation between neuropeptide expression and type of carcinoid, tumor size, vascular infiltration, serosal involvement or mesoappendiceal infiltration. CONCLUSIONS Our investigations revealed that secretoneurin is detected more frequently than chromogranin A in carcinoids of the appendix. This supports the theory that tumor cells of appendiceal carcinoids are of a different origin than other midgut carcinoids. No special tumor entity with a characteristic secretoneurin-chromogranin pattern could be identified.
ECMOve: A Mobilization Device for Extracorporeal Membrane Oxygenation Patients

Abstract:Extracorporeal membrane oxygenation (ECMO) is a temporary lifesaving treatment for critically ill patients with severe respiratory or cardiac failure. Studies demonstrated the feasibility of in-hospital mobilizing during and after ECMO treatment preventing neuromuscular weakness and impaired physical functioning. Despite more compact mobile ECMO devices, implementation of ambulatory ECMO remains labor-intensive, complex, and challenging. It requires a large multidisciplinary team to carry equipment, monitor and physically support the patient, and to provide a back-up wheelchair in case of fatigue. Moreover, there is no adequate solution to ensure the stability of the patient’s cannula and circuit management during ambulation. We developed a system contributing to improvement and innovation of current ambulatory ECMO patient programs. Our modular cart-in-cart system carries necessary ECMO equipment, features an extendable walking frame, and contains a folding seat for patient transport. An adjustable shoulder brace with lockable tubing-connectors enables safe fixation of the blood tubing. ECMOve provides safety, support, and accessibility while performing ambulatory ECMO for both patient and caregiver. Prototype evaluation in a simulated intensive care unit showed feasibility of our design, but needs to be evaluated in clinical care.

Abstract:BACKGROUND/PURPOSE Teamwork involving multiple disciplines is increasingly emphasized in health research, services, education and policy. The terms multidisciplinary, interdisciplinary and transdisciplinary are increasingly used in the literature, but are ambiguously defined and interchangeably used. This paper is the first of two in a series. It discusses the definitions, objectives, and evidence of effectiveness of such teamwork. METHODS The paper is a literature review based on dictionaries, and Google and MEDLINE (1982-2006) searches. RESULTS Multidisciplinarity draws on knowledge from different disciplines but stays within their boundaries. Interdisciplinarity analyzes, synthesizes and harmonizes links between disciplines into a coordinated and coherent whole. Transdisciplinarity integrates the natural, social and health sciences in a humanities context, and transcends their traditional boundaries. The objectives of multiple disciplinary approaches are to resolve real world or complex problems, to provide different perspectives on problems, to create comprehensive research questions, to develop concensus clinical definitions and guidelines, and to provide comprehensive health services. Multiple disciplinary teamwork has both benefits and drawbacks. CONCLUSION The three terms refer to the involvement of multiple disciplines to varying degrees on the same continuum. The common words for multidisciplinary, interdisciplinary and transdisciplinary are additive, interactive, and holistic, respectively. With their own specific meanings, these terms should not be used interchangeably. The more general term "multiple disciplinary" is suggested for when the nature of involvement of multiple disciplines is unknown or unspecified. While multiple disciplinary teamwork is appropriate for complex problems, it is not always necessary in every single project.

Abstract:Abstract The membrane oxygenator is an essential component in the Extracorporeal Membrane Oxygenation (ECMO) system to offer temporary support to the respiratory system. This study aims to optimize the hemodynamic performance and reduce the thrombosis risk of a membrane oxygenator prototype using computational fluid dynamics. Numerical simulations of steady laminar flow in a full-scale oxygenator prototype (model 1) and two optimized structure Models 2 and 3 at flow rates of 5∼7L/min are carried out using the porous media model. Flow-field-based hydraulic performance indicators and the thrombus risk indicator of the three models are compared extensively. Detailed internal flow analysis revealed that adverse flow states such as insufficient flow circulation in the inlet shunt cone zone, large flow separations at the top corners of heat exchangers, and intensive flow impingement at the exit elbow tube are notably improved in the optimized model 2 and 3. The improvement is more significant at high flow rates. Performance parameters quantitatively validate the effect of optimized configurations. Specifically, at a flow rate of 7L/min, the flow uniformity indexes for the original model at the shunt cone exit increase from 0.884 to 0.923 and 0.890 in the two modified models. The total pressure loss is reduced by over 14%, and maximum wall shear stress is notably reduced from 241.46Pa to 135.9Pa. Additionally, the optimized models exhibited lower thrombus risk. The optimized designs emphasize the importance of smooth transitions between cross-sections and minimizing abrupt changes in flow direction. The employment of flow-field-based parameters allows for the establishment of the relationship between geometric and performance parameters to guide effective design optimization and ensure the safe clinical operation of oxygenator prototypes.

Abstract:Microfluidic membrane oxygenators are designed to mimic branching vasculature of the native lung during extracorporeal lung support. To date, scaling of such devices to achieve clinically relevant blood flow and lung support has been a limitation. We evaluated a novel multilayer microfluidic blood oxygenator (BLOx) capable of supporting 750–800 ml/min blood flow versus a standard hollow fiber membrane oxygenator (HFMO) in vivo during veno-venous extracorporeal life support for 24 hours in anesthetized, mechanically ventilated uninjured swine (n = 3/group). The objective was to assess feasibility, safety, and biocompatibility. Circuits remained patent and operated with stable pressures throughout 24 hours. No group differences in vital signs or evidence of end-organ damage occurred. No change in plasma free hemoglobin and von Willebrand factor multimer size distribution were observed. Platelet count decreased in BLOx at 6 hours (37% dec, P = 0.03), but not in HFMO; however, thrombin generation potential was elevated in HFMO (596 ± 81 nM·min) versus BLOx (323 ± 39 nM·min) at 24 hours ( P = 0.04). Other coagulation and inflammatory mediator results were unremarkable. BLOx required higher mechanical ventilator settings and showed lower gas transfer efficiency versus HFMO, but the stable device performance indicates that this technology is ready for further performance scaling and testing in lung injury models and during longer use conditions.

Abstract:Intraosseous (IO) access and infusion is a safe and rapid alternative to intravenous access in obtaining vascular access for administering fluids and drugs. Healthcare professionals, such as primary and advanced care paramedics, use IO access and infusion in emergency circumstances where peripheral intravenous routes are inaccessible. IO access skills require hands-on training, which can be done remotely if the participants have access to simulation, instructions, guidance, and feedback. For the purpose of moving the training outside of the simulation laboratories, we have developed (1) an inexpensive and scalable three-dimensional (3D) printed and silicone-based advanced adult proximal tibial IO access and infusion simulator and (2) a unique learning management system (LMS) for remote simulation-based training. The LMS was built using the Django platform and supports experiential learning by providing access to educational and instructional content (including virtual simulation and serious games), allowing peers to communicate among themselves and with subject-matter experts, provide and receive feedback asynchronously, and engage in learning using gamification elements. The aim of this technical report is to describe the process of development and the final product of the LMS as a research and educational tool to scaffold remote learning of emergency IO skills by paramedics-in-training.

Abstract:Perfusion, Ahead of Print. The aphorism "Primum non nocere" underscores the responsibility of healthcare professionals to prioritize patient safety. Perfusionists, experts in extracorporeal circulation, play a pivotal role in maintaining cardiac function during critical situations. However, the evolving landscape of medical technology has not been without challenges, particularly in ensuring equitable access to perfusion training. Easy Flow Cardiopulmonary Bypass Simulation Model stands out as a cost-effective alternative, utilizing routine CPB equipment found in operating theatres. The uniqueness lies in its incorporation of double reservoirs and two pumps, a novel approach not reported before in educational CPB simulation models. The benefits of this model extend beyond skill development to encompass team management, communication enhancement, and disaster management training. Multiple scenarios, from the initiation of CPB to addressing emergencies like massive air embolism, can be simulated. Although the model requires an instructor, this facilitates the integration of essential professional and communication skills into training. The adaptability of the Easy Flow model makes it a practical and sustainable solution. It provides hands-on experience for perfusion students, translating theoretical knowledge into practical competence. The model's simplicity, combined with its use of readily available materials, positions it to be an accessible tool for educational institutions and healthcare centers globally. In conclusion, the Easy Flow CPB Simulation Model not only fills a critical gap in perfusion education but also exemplifies how innovation can bridge disparities, ensuring that quality healthcare education is within reach for all. Abstract:The research on the miniaturization of oxygen generators is the key to ensure the oxygen supply in plateau field. Due to the current research on the application of membrane components in the relatively large field of oxygen production, in order to explore the matching relationship of small oxygen-rich membrane components, vacuum pump and fan, so as to facilitate the selection and design of small membrane oxygen generator parts, reduce the product development cycle. Based on Fick's law of permeability, the relationship between air intake and operating pressure of monolayer membrane was analyzed according to the cross-flow model, and the influence model of air intake and oxygen production performance of monolayer oxygen-rich membrane was obtained by using the fourth-order RK method. Then, the influence of the number of layers of oxygen-rich membrane on oxygen production performance was studied. The co-simulation model of membrane component, vacuum pump and fan was established for simulation analysis, and verified by experiments. The results show that the model is in good agreement with the experimental results, which provides a reference for the design and development of small membrane oxygen generator.

Abstract:Oxygen is vital to all animal life. Hypoxia is one of the most threatening factors to human health in emergency care clinics. However, the currently available methods to supply oxygen for patient with hypoxia are generally too slow to be effective. In this paper, we proposed a new conceptual method to directly deliver oxygen to the target tissues in the deep body, which is generally inaccessible by traditional way, through injection of solution with high oxygen content. Particularly, the cooling solution was administered to decrease the consumption rate of the oxygen and thus lengthen the endurable time for the patient, aiming to spare more valuable times for a clinical rescue. To evaluate the feasibility of this method, a mathematical model was established to characterize the multi-mode transport process, which is simultaneously coupled with fluid flow, heat transport and mass diffusion events. Preliminary simulation indicates that this method can quickly improve and maintain well the oxygen level in the target tissues inside the deep biological body. It may open a brand new technical strategy for tissue oxygen delivery, which would find significant applications in emergency rescue, especially in those clinical situations requesting immediate and volumetric oxygen supplying.

Abstract:Introduction: Medical and para-medical education is one of the key points of healthcare strategy. Training and education based on high-fidelity simulation is one of the gold standards in modern healthcare institutions. We describe a model of training dedicated to ICU nurses in charge of patients with ECMO. The aim of our educational tool was to teach ICU nurses ECMO basic knowledge and skills. Methods: An ECMO Specialist Course Committee implemented the training programme. It was on two consecutive days and consisted of theoretical, practical and high-fidelity, simulation-based teaching. A content expert implemented each scenario and learning objectives were defined. Participants were assessed pre- and post-test (Group 1 and Group 2). Results: In two years, seven sessions took place and 40 volunteers were enrolled. High-fidelity, simulation-based teaching consisted of seven scenarios. There was a significant improvement in mean score between pre- and post-test. Moreover, we noticed that the basic level (pre-test) of participants was improving over the time. The mean pre-test scores of Group 2 were significantly higher than Group1. Conclusion: The implementation of education and training course for ICU nurses in charge of patients on ECMO is feasible and reliable. It improves nurse personal levels, but also shares in improving the global level of the team to which they belong.

Abstract:Intraosseous (IO) infusion is an alternative way to access the vascular system to administer drugs and fluids, which is particularly helpful when the commonly used peripheral intravenous route is inaccessible. The IO procedure can be done using a drill that involves disinfecting the area, landmarking the insertion point, seating the needle in a firm and stable position in the bone, and then delivering a smooth fluid flush. However, in the current medical training landscape, access to commercially available IO drills such as the Arrow® EZ-IO® Power Driver (EZ-IO; Teleflex, Morrisville, North Carolina, United States) is difficult, especially for rural and remote areas, due to the high costs. Furthermore, the EZ-IO is not rechargeable and does not clearly indicate the remaining battery life, which could potentially put patients at risk during the IO procedure. This technical report aims to address these concerns by describing the development of an alternative, affordable, and reliable IO drilling system for training use: the maxSIMIO Drilling System. This system consists of a cordless and rechargeable IKEA screwdriver which connects to a conventional, hexagon-shaped 3D-printed drill bit needle adapter. Two needle adapters were created: Version A was designed to use a friction-based mechanism to couple the screwdriver with the EZ-IO training needle, while Version B relies on a magnetic mechanism. The major differences between the EZ-IO and the screwdriver are that a) the EZ-IO has only one rotation to advance the cannula while the screwdriver features both directions, b) the EZ-IO is not rechargeable while the screwdriver is, and c) the EZ-IO has a custom needle holder that can fit any EZ-IO training needle size while the screwdriver needs to have a custom needle adapter made to connect to the EZ-IO training needle. Overall, through this exploration, the features of the maxSIMIO Drilling System in comparison to the EZ-IO appear more accessible for IO training. Future considerations for this development include gathering clinical expertise through rigorous testing of this novel system.
Numerical simulation of the flow and concentration fields for oxygen delivery systems

Abstract:The flow and concentration fields for various medical oxygen delivery devices are numerically investigated. Simulations are performed for a classical Venturi mask and two new OxyArm portable devices. The velocity and oxygen concentration fields are investigated for: (i) a constant (steady-state) inhalation and (ii) a complete respiratory cycle (unsteady). The numerical results are qualitatively compared with clinical trials. It is found that the optimal functioning of these medical devices implies a balance between oxygen delivery by advection and the mixing process that allows for reliable CO2 monitoring (capnographic capability). Also, at the typical scales associated with these devices the flow is found to be Reynolds number dependent.

Abstract:Extracorporeal membrane oxygenator (ECMO) has been in development for nearly 70 years, and the oxygenator has gone through several generations of optimizations, with advances from bubble oxygenators to membrane oxygenators leading to more and more widespread use of ECMO. Membrane is the core of a ECMO system and the working mechanism of membrane oxygenator depends on the membrane material, from PDMS flat membrane to PMP hollow fiber membrane, which have experienced three generations. Blood compatibility on the surface of the membrane material is very vital, which directly determines the use duration of the oxygenator and can reduce the occurrence of complications. The mechanism of mass transfer is the basis of oxygenator operation and optimization. This review summarizes the membrane development history and preparation technology, modification approaches and mass transfer theory in the process of oxygen and blood exchange. We hoped that this review will provide more ideas for the study of gas blood exchange membrane.

Abstract:Veno-venous extracorporeal membrane oxygenation (VV-ECMO) is an important tool in the management of most severe forms of acute respiratory failure. The determinants and management of oxygen delivery in patients treated with VV-ECMO is a complex topic. The physiological principles of oxygenation on VV-ECMO are reviewed in many textbooks. However a numerical model is an additional instrument to be used in understanding and exploring this intricate subject matter. We present a numerical model of blood oxygenation during VV-ECMO. Using this model we examined the role and impact of each determinant on blood oxygenation. The numerical analysis of variation and interplay between each oxygenation determinants during VV-ECMO is presented in graphical form. These results corroborate all the findings of previous studies. The proposed numerical model facilitates understanding of oxygenation physiology during VV-ECMO; it can be used for a medical simulation system and for teaching the principles of oxygenation during VV-ECMO.

Abstract:Abstract Background Extracorporeal membrane oxygenation (ECMO) research using large animals requires a significant amount of resources, slowing down the development of new means of ECMO anticoagulation. Therefore, this study developed and evaluated a new rat ECMO model using a 3D-printed mock-oxygenator. Methods The circuit consisted of tubing, a 3D-printed mock-oxygenator, and a roller pump. The mock-oxygenator was designed to simulate the geometry and blood flow patterns of the fiber bundle in full-scale oxygenators but with a low (2.5 mL) priming volume. Rats were placed on arteriovenous ECMO at a 1.9 mL/min flow rate at two different heparin doses (n = 3 each): low (15 IU/kg/h for eight hours) versus high (50 IU/kg/h for one hour followed by 25 IU/kg/h for seven hours). The experiment continued for eight hours or until the mock-oxygenator failed. The mock-oxygenator was considered to have failed when its blood flow resistance reached three times its baseline resistance. Results During ECMO, rats maintained near-normal mean arterial pressure and arterial blood gases with minimal hemodilution. The mock-oxygenator thrombus weight was significantly different (p < 0.05) between the low (0.02 ± 0.006 g) and high (0.003 ± 0.001 g) heparin delivery groups, and blood flow resistance was also larger in the low anticoagulation group. Conclusions This model is a simple, inexpensive system for investigating new anticoagulation agents for ECMO and provides low and high levels of anticoagulation that can serve as control groups for future studies.

Abstract:Perfusion, Ahead of Print. Prognostic modelling techniques have rapidly evolved over the past decade and may greatly benefit patients supported with ExtraCorporeal Membrane Oxygenation (ECMO). Epidemiological and computational physiological approaches aim to provide more accurate predictive assessments of ECMO-related risks and benefits. Implementation of these approaches may produce predictive tools that can improve complex clinical decisions surrounding ECMO allocation and management. This Review describes current applications of prognostic models and elaborates on upcoming directions for their clinical applicability in decision support tools directed at improved allocation and management of ECMO patients. The discussion of these new developments in the field will culminate in a futuristic perspective leaving ourselves and the readers wondering whether we may "fly ECMO by wire" someday.

Abstract:Chronic respiratory diseases are the third most frequent cause of death worldwide. However, the limited availability of transplantable organs led to an increased research effort in artificial lung systems. Current artificial lung systems use hollow fibre membrane-based oxygenators. Historic oxygenators using a direct liquid-gas or liquid-liquid interface outperformed todays hollow-fibre technology in terms of gas-transfer. However, these approaches were no longer pursued due to risk of embolization by gas or liquid droplet formation in the blood stream. The herein presented approach of a membrane-supported liquid-liquid oxygenator combines todays hollow-fibre technology with historic liquid-liquid approaches to achieve a stable interface without the risk of embolization but with the advantages of a higher gas-transfer and a decreasing foreign membrane surface of the oxygenator. Perfluoro-n-hexane was chosen as a hemocompatible carrier fluid for the presented liquid-liquid approach. In the proof-of-principle oxygenator, O2 and CO2 gas-transfers were measured to be 1.83 mmol O2 and 1.34 mmol CO2 , respectively. No Perfluoro-n-hexane droplets were detected in the blood after the experiment. This study demonstrates the proof of principle of a new membrane-supported liquid-liquid oxygenator.

Abstract:Objective To test the oxygenation property of membraneous oxygenator by means of the model established simulating invo and ex vivo gas exchange in circulation.Methods The model established respiratory circulation gas exchange was connected to the testing membraneous oxygenator with polyvinyl chloride tube.Thus the close loop of circulation was completed.The fresh bovine blood added anticoagulant was adopted as test medium.The blood samples were taken from the inlet and outlet of tested oxygenator in 3,5,60,120,240 and 360 minutes for gas analysis and blood routine examination.Then compared the oxygenation property of testing Xi'an Xijian Ⅰ oxygenator and Affinity(USA) oxygenator with the model.Results It demonstrated by the model there was good oxygenation property of the testing home made oxygenator,the SaO2,PaO2 and PaCO2 were 95 %,13.3 kPa(100 mmHg) and 5.33 kPa(40 mmHg) respectively after circulating 360 minutes.Conclusion It is demonstrated that the construction of the model is satisfying the fundamental principle of extracorporal circulation and it is reliable and applicable for the study on properties of membraneous oxygenator and fundamental principle of extracorporal circulation.
