Abstract:Background Patients suffering from major depressive disorder (MDD) regularly experience non-response to treatment for their depressive episode. Personalized clinical decision making could shorten depressive episodes and reduce patient suffering. Although no clinical tools are currently available, machine learning analysis of electroencephalography (EEG) shows promise in treatment response prediction. Methods With a systematic review and meta-analysis, we evaluated the accuracy of EEG for individual patient response prediction. Importantly, we included only prediction studies that used cross-validation. We used a bivariate model to calculate prediction success, as expressed by area-under the curve, sensitivity and specificity. Furthermore, we analyzed prediction success for separate antidepressant interventions. Results 15 studies with 12 individual patient samples and a total of 479 patients were included. Research methods varied considerably between studies. Meta-analysis of results from this heterogeneous set of studies resulted in an area under the curve of 0.91, a sensitivity of 83 % (95 % CI 74–89 %), and a specificity of 86 % (95 % CI 81–90 %). Classification performance did not significantly differ between treatments. Although studies were all internally validated, no externally validated studies have been reported. We found substantial risk of bias caused by methodological shortcomings such as non-independent feature selection, though performance of non-biased studies was comparable. Limitations Sample sizes were relatively small and no study used external validation, increasing the risk of overestimation of accuracy. Conclusions Electroencephalography can predict the response to antidepressant treatment with high accuracy. However, future studies with more rigorous validation are needed to produce a clinical tool to guide interventions in MDD. PROSPERO registration number CRD42021268169.

Abstract:The Tolles and Lawson equations were programmed on the IBM 370-65 computer. These equations predict the magnetic signals which are generated by the permanent moments, the induced moments and the eddy-current terms of an aircraft as it maneuvers in the Earth's magnetic field. The least mean squared (LMS) solution of these equations, which was developed for the microprocessor compensation program, was also simulated on the IBM 370 computer. By measuring the simulated figure of merit (FOM) before and after applying compensation, several types of error were studied. It was found that the error introduced by truncation effects on a 16-bit microprocessor limits the FOM to 50 milligammas. Since this figure is an order of magnitude smaller than the design goal, it is concluded that the algorithm is not limited by the microprocessor capacity. Other studies show that although the baseline model is insensitive to bearing separation, the bearing separation should be 900° ± 7.50° if 3 iterations are used in the post processor and 900° ± 15° if 5 iterations are used. Although the algorithm is robust and relatively insensitive to various types of noise, experimental evidence suggests that hysteresis can cause temporal variations in the Tolles and Lawson model. It is suggested that whitening and/or adaptive techniques could be used to minimize such variations.
Predicting treatment response using EEG in major depressive disorder: A machine-learning meta-analysis

Abstract:Selecting a course of treatment in psychiatry remains a trial-and-error process, and this long-standing clinical challenge has prompted an increased focus on predictive models of treatment response using machine learning techniques. Electroencephalography (EEG) represents a cost-effective and scalable potential measure to predict treatment response to major depressive disorder. We performed separate meta-analyses to determine the ability of models to distinguish between responders and non-responders using EEG across treatments, as well as a performed subgroup analysis of response to transcranial magnetic stimulation (rTMS), and antidepressants (Registration Number: CRD42021257477) in Major Depressive Disorder by searching PubMed, Scopus, and Web of Science for articles published between January 1960 and February 2022. We included 15 studies that predicted treatment responses among patients with major depressive disorder using machine-learning techniques. Within a random-effects model with a restricted maximum likelihood estimator comprising 758 patients, the pooled accuracy across studies was 83.93% (95% CI: 78.90–89.29), with an Area-Under-the-Curve (AUC) of 0.850 (95% CI: 0.747–0.890), and partial AUC of 0.779. The average sensitivity and specificity across models were 77.96% (95% CI: 60.05–88.70), and 84.60% (95% CI: 67.89–92.39), respectively. In a subgroup analysis, greater performance was observed in predicting response to rTMS (Pooled accuracy: 85.70% (95% CI: 77.45–94.83), Area-Under-the-Curve (AUC): 0.928, partial AUC: 0.844), relative to antidepressants (Pooled accuracy: 81.41% (95% CI: 77.45–94.83, AUC: 0.895, pAUC: 0.821). Furthermore, across all meta-analyses, the specificity (true negatives) of EEG models was greater than the sensitivity (true positives), suggesting that EEG models thus far better identify non-responders than responders to treatment in MDD. Studies varied widely in important features across models, although relevant features included absolute and relative power in frontal and temporal electrodes, measures of connectivity, and asymmetry across hemispheres. Predictive models of treatment response using EEG hold promise in major depressive disorder, although there is a need for prospective model validation in independent datasets, and a greater emphasis on replicating physiological markers. Crucially, standardization in cut-off values and clinical scales for defining clinical response and non-response will aid in the reproducibility of findings and the clinical utility of predictive models. Furthermore, several models thus far have used data from open-label trials with small sample sizes and evaluated performance in the absence of training and testing sets, which increases the risk of statistical overfitting. Large consortium studies are required to establish predictive signatures of treatment response using EEG, and better elucidate the replicability of specific markers. Additionally, it is speculated that greater performance was observed in rTMS models, since EEG is assessing neural networks more likely to be directly targeted by rTMS, comprising electrical activity primarily near the surface of the cortex. Prospectively, there is a need for models that examine the comparative effectiveness of multiple treatments across the same patients. However, this will require a thoughtful consideration towards cumulative treatment effects, and whether washout periods between treatments should be utilised. Regardless, longitudinal cross-over trials comparing multiple treatments across the same group of patients will be an important prerequisite step to both facilitate precision psychiatry and identify generalizable physiological predictors of response between and across treatment options.

Abstract:ObjectivesPredictors for unfavorable treatment outcome in major depressive disorder (MDD) applicable fo treatment selection are still lacking. The database of a longitudinal multi‐center study on 1079 acutely depressed patients, performed by the German research network on depression (GRND), allows supervised and unsupervised learning to further elucidate the interplay of clinical and psycho‐sociodemographic variables and their predictive impact on treatment outcome phenotypes. Experimental ProceduresTreatment response was defined by a change of HAM‐D 17 item baseline score ≥ 50% and remission by the established threshold of ≤ 7, respectively after up to eight weeks of in‐patient treatment. After hierarchical symptom clustering and stratification by treatment subtypes (serotonin‐reuptake‐inhibitors, tricyclic antidepressants, antipsychotic and lithium augmentation) prediction models for different outcome phenotypes were computed with random forest in a cross‐center validation design. In total, 86 predictors were implemented. ResultsClustering revealed four distinct HAM‐D sub‐scores related to emotional, anxious, sleep and appetite symptoms, respectively. After feature selection classification models reached moderate to high accuracies up to 0.85. Highest accuracies were observed for the SSRI and TCA subgroups and for sleep and appetite symptoms while anxious symptoms showed poor predictability. ConclusionOur results support a decisive role for machine learning in the management of antidepressant treatment. Treatment‐ and symptom specific algorithms may increase accuracies by reducing heterogeneity. Especially predictors related to duration of illness, baseline depression severity, anxiety and somatic symptoms, and personality traits moderate treatment success. However, prospectives application of machine learning models will be necessary to prove their value for the clinic.

Predicting therapy outcome in a digital mental health intervention for depression and anxiety: A machine learning approach

Abstract:Objective Predicting the outcomes of individual participants for treatment interventions appears central to making mental healthcare more tailored and effective. However, little work has been done to investigate the performance of machine learning-based predictions within digital mental health interventions. Therefore, this study evaluates the performance of machine learning in predicting treatment response in a digital mental health intervention designed for treating depression and anxiety. Methods Several algorithms were trained based on the data of 970 participants to predict a significant reduction in depression and anxiety symptoms using clinical and sociodemographic variables. As a random forest classifier performed best over cross-validation, it was used to predict the outcomes of 279 new participants. Results The random forest achieved an accuracy of 0.71 for the test set (base rate: 0.67, area under curve (AUC): 0.60, p = 0.001, balanced accuracy: 0.60). Additionally, predicted non-responders showed less average reduction of their Patient Health Questionnaire-9 (PHQ-9) (−2.7, p = 0.004) and General Anxiety Disorder Screener-7 values (−3.7, p < 0.001) compared to responders. Besides pre-treatment Patient Health Questionnaire-9 and General Anxiety Disorder Screener-7 values, the self-reported motivation, type of referral into the programme (self vs. healthcare provider) as well as Work Productivity and Activity Impairment Questionnaire items contributed most to the predictions. Conclusions This study provides evidence that social-demographic and clinical variables can be used for machine learning to predict therapy outcomes within the context of a therapist-supported digital mental health intervention. Despite the overall moderate performance, this appears promising as these predictions can potentially improve the outcomes of non-responders by monitoring their progress or by offering alternative or additional treatment.
Can we predict who will benefit from cognitive-behavioural therapy? A systematic review and meta-analysis of machine learning studies

Abstract:Cognitive-behavioural therapy (CBT) is the first line of treatment for several mental health disorders. However, not all patients show clinical improvements after receiving CBT. Machine learning allows inferences at the individual level and therefore is a promising approach for predicting who will and will not benefit from CBT. A comprehensive literature search was conducted to identify all studies that used machine learning to predict clinical response to CBT. A random-effects meta-analysis of proportions was used to estimate an overall performance accuracy across all studies. Twenty-four studies (N = 7497) were identified, covering five diagnostic groups: Major Depressive Disorder (k = 4), Obsessive-Compulsive Disorder (OCD, k = 5), Post-Traumatic Stress Disorder (k = 2), Anxiety Disorders (AD, k = 7), Substance Use Disorders (k = 4) and two transdiagnostic models. Studies used clinical, neuroimaging, cognitive and genetic data, or a combination of these, as predictors. The overall performance accuracy across studies was 74.0% [70.0-77.8]. Accuracies differed significantly between diagnostic groups and was highest in PTSD (78.7%, 69.1-87.0), AD (77.6%, 67.5-86.4) and OCD (76.1%, 67.3-84.0). Some studies were at a high risk of bias due to how the outcome was operationalised and/or how the analyses were conducted/reported. There are many challenges to overcome before these promising results can be applied to real-world clinical practice.

Abstract:For many years, psychiatrists have tried to understand factors involved in response to medications or psychotherapies, in order to personalize their treatment choices. There is now a broad and growing interest in the idea that we can develop models to personalize treatment decisions using new statistical approaches from the field of machine learning and applying them to larger volumes of data. In this pursuit, there has been a paradigm shift away from experimental studies to confirm or refute specific hypotheses towards a focus on the overall explanatory power of a predictive model when tested on new, unseen datasets. In this paper, we review key studies using machine learning to predict treatment outcomes in psychiatry, ranging from medications and psychotherapies to digital interventions and neurobiological treatments. Next, we focus on some new sources of data that are being used for the development of predictive models based on machine learning, such as electronic health records, smartphone and social media data, and on the potential utility of data from genetics, electrophysiology, neuroimaging and cognitive testing. Finally, we discuss how far the field has come towards implementing prediction tools in real‐world clinical practice. Relatively few retrospective studies to‐date include appropriate external validation procedures, and there are even fewer prospective studies testing the clinical feasibility and effectiveness of predictive models. Applications of machine learning in psychiatry face some of the same ethical challenges posed by these techniques in other areas of medicine or computer science, which we discuss here. In short, machine learning is a nascent but important approach to improve the effectiveness of mental health care, and several prospective clinical studies suggest that it may be working already.

Abstract:Background: Recent evidence suggests that integration of multi-modal data improves performance in machine learning prediction of depression treatment outcomes. Here, we compared the predictive performance of three machine learning classifiers using differing combinations of sociodemographic characteristics, baseline clinical self-reports, cognitive tests, and structural magnetic resonance imaging (MRI) features to predict treatment outcomes in late-life depression (LLD). Methods: Data were combined from two clinical trials conducted with depressed adults aged 60 and older, including response to escitalopram ( N = 32, NCT01902004) and Tai Chi ( N = 35, NCT02460666). Remission was defined as a score of 6 or less on the 24-item Hamilton Rating Scale for Depression (HAMD) at the end of 24 weeks of treatment. Features subsets were constructed from baseline sociodemographic and clinical features, gray matter volumes (GMVs), or both. Three classification algorithms were compared: (1) Support Vector Machine-Radial Bias Function (SVMRBF), (2) Random Forest (RF), and (3) Logistic Regression (LR). A repeated 5-fold cross-validation approach with a wrapper-based feature selection method was used for model fitting. Model performance metrics included Area under the ROC Curve (AUC) and Matthews correlation coefficient (MCC). Cross-validated performance significance was tested by permutation analysis. Classifiers were compared by Cochran's Q and post-hoc pairwise comparisons using McNemar's Chi-Square test with Bonferroni correction. Results: For the RF and SVMRBF algorithms, the combined feature set outperformed the clinical and GMV feature sets with a final cross-validated AUC of 0.83 ± 0.11 and 0.80 ± 0.11, respectively. Both classifiers passed permutation analysis. The LR algorithm performed best using GMV features alone (AUC 0.79 ± 0.14) but failed to pass permutation analysis using any feature set. Performance of the three classifiers differed significantly for all three features sets. Important predictive features of treatment response included anterior and posterior cingulate volumes, depression characteristics, and self-reported health-related quality scores. Conclusion: This preliminary exploration into the use of ML and multi-modal data to identify predictors of general treatment response in LLD indicates that integration of clinical and structural MRI features significantly increases predictive capability. Identified features are among those previously implicated in geriatric depression, encouraging future work in this arena.

Abstract:Improving response and remission rates in major depressive disorder (MDD) remains an important challenge. Matching patients to the treatment they will most likely respond to should be the ultimate goal. Even though numerous studies have investigated patient-specific indicators of treatment efficacy, no (bio)markers or empirical tests for use in clinical practice have resulted as of now. Therefore, clinical decisions regarding the treatment of MDD still have to be made on the basis of questionnaire- or interview-based assessments and general guidelines without the support of a (laboratory) test. We conducted a narrative review of current approaches to characterize and predict outcome to pharmacological treatments in MDD. We particularly focused on findings from newer computational studies using machine learning and on the resulting implementation into clinical decision support systems. The main issues seem to rest upon the unavailability of robust predictive variables and the lacking application of empirical findings and predictive models in clinical practice. We outline several challenges that need to be tackled on different stages of the translational process, from current concepts and definitions to generalizable prediction models and their successful implementation into digital support systems. By bridging the addressed gaps in translational psychiatric research, advances in data quantity and new technologies may enable the next steps toward precision psychiatry.

Abstract:Single-incision laparoscopic surgery (SILS) has recently increased in number. A systematic review of English-language literature from 2009 to 2011 was performed to evaluated the feasibility and outcomes of SILS for various indications in children. Our study reports seven previously published studies of 218 cases. There were insignificant differences in operative time and length of hospital stay of SILS when compared to multiport laparoscopic cholecystectomy. Despite SILS is a more challenging technique to perform, it is a safe and feasible alternative for cholecystectomy in children.

Abstract:Objective: Machine learning (ML) has been widely used to detect and evaluate major depressive disorder (MDD) using neuroimaging data, i.e., resting-state functional magnetic resonance imaging (rs-fMRI). However, the diagnostic efficiency is unknown. The aim of the study is to conduct an updated meta-analysis to evaluate the diagnostic performance of ML based on rs-fMRI data for MDD. Methods: English databases were searched for relevant studies. The Quality Assessment of Diagnostic Accuracy Studies (QUADAS-2) was used to assess the methodological quality of the included studies. A random-effects meta-analytic model was implemented to investigate the diagnostic efficiency, including sensitivity, specificity, diagnostic odds ratio (DOR), and area under the curve (AUC). Regression meta-analysis and subgroup analysis were performed to investigate the cause of heterogeneity. Results: Thirty-one studies were included in this meta-analysis. The pooled sensitivity, specificity, DOR, and AUC with 95% confidence intervals were 0.80 (0.75, 0.83), 0.83 (0.74, 0.82), 14.00 (9, 22.00), and 0.86 (0.83, 0.89), respectively. Substantial heterogeneity was observed among the studies included. The meta-regression showed that the leave-one-out cross-validation (loocv) (sensitivity: p < 0.01, specificity: p < 0.001), graph theory (sensitivity: p < 0.05, specificity: p < 0.01), n > 100 (sensitivity: p < 0.001, specificity: p < 0.001), simens equipment (sensitivity: p < 0.01, specificity: p < 0.001), 3.0T field strength (Sensitivity: p < 0.001, specificity: p = 0.04), and Beck Depression Inventory (BDI) (sensitivity: p = 0.04, specificity: p = 0.06) might be the sources of heterogeneity. Furthermore, the subgroup analysis showed that the sample size ( n > 100: sensitivity: 0.71, specificity: 0.72, n < 100: sensitivity: 0.81, specificity: 0.79), the different levels of disease evaluated by the Hamilton Depression Rating Scale (HDRS/HAMD) (mild vs. moderate vs. severe: sensitivity: 0.52 vs. 0.86 vs. 0.89, specificity: 0.62 vs. 0.78 vs. 0.82, respectively), the depression scales in patients with comparable levels of severity. (BDI vs. HDRS/HAMD: sensitivity: 0.86 vs. 0.87, specificity: 0.78 vs. 0.80, respectively), and the features (graph vs. functional connectivity: sensitivity: 0.84 vs. 0.86, specificity: 0.76 vs. 0.78, respectively) selected might be the causes of heterogeneity. Conclusion: ML showed high accuracy for the automatic diagnosis of MDD. Future studies are warranted to promote the potential use of these classification algorithms in clinical settings.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>Selective serotonin reuptake inhibitors (SSRIs) are suggested as the first-line treatment for patients with major depressive disorder (MDD), but the remission rate is unsatisfactory. We aimed to establish machine learning models and explore variables available at baseline to predict the 8-week outcome among patients taking SSRIs.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Data from 400 patients were used to build machine learnings. The last observation carried forward approach was used to determine the remitter/non-remitter status of the patients at week 8. Using least absolute shrinkage and selection operator (LASSO) to select features, we built 4 different machine learning algorithms including gradient boosting decision tree, support vector machine (SVM), random forests, and logistic regression with five-fold cross-validation. Then, we adopted Shapley additive explanations (SHAP) values to interpret the model output.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The remission rate is 67.8%. We obtained 78 features from the baseline characteristics, including 25 sociodemographic characteristics, 31 clinical features, 15 psychological traits and 7 neurocognitive functions, and 13 of these features were selected to establish SVM. The accuracy of the SVM prediction is 74.49%, reaching an average area under the curve of 0.734±0.043. The sensitivity is 0.899±0.038 with a positive predictive value of 0.776±0.028. The specificity is 0.422±0.091 with a negative predictive value of 0.674±0.086. According to the SHAP values, neurocognitive functions and anxiety and hypochondriasis symptoms were important predictors.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Our study supports the utilization of machine learning approaches with inexpensive and highly accessible variables to accurately predict the 8-week treatment outcome of SSRIs in patients with MDD.</p>

Abstract:To review the recent progress of machine learning for the early diagnosis of depression (major depressive disorder). The source of data was 32 original studies in the Web of Science. The search terms were “depression” (title) and “random forest” (abstract). The eligibility criteria were the dependent variable of depression, the interventions of machine learning (the decision tree, the naïve Bayesian, the random forest, the support vector machine and/or the artificial neural network), the outcomes of accuracy and/or the area under the receiver operating characteristic curve (AUC) for the early diagnosis of depression, the publication year of 2000 or later, the publication language of English and the publication journal of SCIE/SSCI. Different machine learning methods would be appropriate for different types of data for the early diagnosis of depression, e.g., logistic regression, the random forest, the support vector machine and/or the artificial neural network in the case of numeric data, the random forest in the case of genomic data. Their performance measures reported varied within 60.1–100.0 for accuracy and 64.0–96.0 for the AUC. Machine learning provides an effective, non-invasive decision support system for early diagnosis of depression.

Development and Validation of a Deep-Learning Model for Differential Treatment Benefit Prediction for Adults with Major Depressive Disorder Deployed in the Artificial Intelligence in Depression Medication Enhancement (AIDME) Study

Abstract:INTRODUCTION: The pharmacological treatment of Major Depressive Disorder (MDD) relies on a trial-and-error approach. We introduce an artificial intelligence (AI) model aiming to personalize treatment and improve outcomes, which was deployed in the Artificial Intelligence in Depression Medication Enhancement (AIDME) Study. OBJECTIVES: 1) Develop a model capable of predicting probabilities of remission across multiple pharmacological treatments for adults with at least moderate major depression. 2) Validate model predictions and examine them for amplification of harmful biases. METHODS: Data from previous clinical trials of antidepressant medications were standardized into a common framework and included 9,042 adults with moderate to severe major depression. Feature selection retained 25 clinical and demographic variables. Using Bayesian optimization, a deep learning model was trained on the training set, refined using the validation set, and tested once on the held-out test set. RESULTS: In the evaluation on the held-out test set, the model demonstrated achieved an AUC of 0.65. The model outperformed a null model on the test set (p = 0.01). The model demonstrated clinical utility, achieving an absolute improvement in population remission rate in hypothetical and actual improvement testing. While the model did identify one drug (escitalopram) as generally outperforming the other drugs (consistent with the input data), there was otherwise significant variation in drug rankings. On bias testing, the model did not amplify potentially harmful biases. CONCLUSIONS: We demonstrate the first model capable of predicting outcomes for 10 different treatment options for patients with MDD, intended to be used at or near the start of treatment to personalize treatment. The model was put into clinical practice during the AIDME randomized controlled trial whose results are reported separately.

Abstract:Major depressive disorder is a heterogeneous diagnostic category with multiple available treatments. With the goal of optimizing treatment selection, researchers are developing computational models that attempt to predict treatment response based on various pretreatment measures. In this paper, we review studies that use brain activity data to predict treatment response. Our aim is to highlight and clarify important methodological differences between various studies that relate to the incorporation of domain knowledge, specifically within two approaches delineated as data-driven and theory-driven. We argue that theory-driven generative modeling, which explicitly models information processing in the brain and thus can capture disease mechanisms, is a promising emerging approach that is only beginning to be utilized in treatment response prediction. The predictors extracted via such models could improve interpretability, which is critical for clinical decision-making. We also identify several methodological limitations across the reviewed studies and provide suggestions for addressing them. Namely, we consider problems with dichotomizing treatment outcomes, the importance of investigating more than one treatment in a given study for differential treatment response predictions, the need for a patient-centered approach for defining treatment outcomes, and finally, the use of internal and external validation methods for improving model generalizability.
Examining factors related to low performance of predicting remission in participants with major depressive disorder using neuroimaging data and other clinical features

Abstract:Major depressive disorder (MDD), a prevalent mental health issue, affects more than 8% of the US population, and almost 17% in the young group of 18–25 years old. Since Covid-19, its prevalence has become even more significant. However, the remission (being free of depression) rates of first-line antidepressant treatments on MDD are only about 30%. To improve treatment outcomes, researchers have built various predictive models for treatment responses and yet none of them have been adopted in clinical use. One reason is that most predictive models are based on data from subjective questionnaires, which are less reliable. Neuroimaging data are promising objective prognostic factors, but they are expensive to obtain and hence predictive models using neuroimaging data are limited and such studies were usually in small scale (N<100). In this paper, we proposed an advanced machine learning (ML) pipeline for small training dataset with large number of features. We implemented multiple imputation for missing data and repeated K-fold cross validation (CV) to robustly estimate predictive performances. Different feature selection methods and stacking methods using 6 general ML models including random forest, gradient boosting decision tree, XGBoost, penalized logistic regression, support vector machine (SVM), and neural network were examined to evaluate the model performances. All predictive models were compared using model performance metrics such as accuracy, balanced accuracy, area under ROC curve (AUC), sensitivity and specificity. Our proposed ML pipeline was applied to a training dataset and obtained an accuracy and AUC above 0.80. But such high performance failed while applying our ML pipeline using an external validation dataset from the EMBARC study which is a multi-center study. We further examined the possible reasons especially the site heterogeneity issue.

Abstract:Major depressive disorder (MDD) is the leading cause of disability worldwide, yet treatment selection still proceeds via "trial and error". Given the varied presentation of MDD and heterogeneity of treatment response, the use of machine learning to understand complex, non-linear relationships in data may be key for treatment personalization. Well-organized, structured data from clinical trials with standardized outcome measures is useful for training machine learning models; however, combining data across trials poses numerous challenges. There is also persistent concern that machine learning models can propagate harmful biases. We have created a methodology for organizing and preprocessing depression clinical trial data such that transformed variables harmonized across disparate datasets can be used as input for feature selection. Using Bayesian optimization, we identified an optimal multi-layer dense neural network that used data from 21 clinical and sociodemographic features as input in order to perform differential treatment benefit prediction. With this combined dataset of 5032 individuals and 6 drugs, we created a differential treatment benefit prediction model. Our model generalized well to the held-out test set and produced similar accuracy metrics in the test and validation set with an AUC of 0.7 when predicting binary remission. To address the potential for bias propagation, we used a bias testing performance metric to evaluate the model for harmful biases related to ethnicity, age, or sex. We present a full pipeline from data preprocessing to model validation that was employed to create the first differential treatment benefit prediction model for MDD containing 6 treatment options.

Abstract:BACKGROUND: In this modern era, depression is one of the most prevalent mental disorders from which millions of individuals are affected today. The symptoms of depression are heterogeneous and often coincide with other disorders such as bipolar disorder, Parkinson's, schizophrenia, etc. It is a serious mental illness that may lead to other health problems if left untreated. Currently, identifying individuals with depression is totally based on the expertise of the clinician's experience. In order to assist clinicians in identifying the characteristics and classifying depressed people, different types of data modalities and machine learning techniques have been incorporated by researchers in this field. This study aims to find the answers to some important questions related to the trend of publications, data modality, machine learning models, dataset usage, pre-processing techniques and feature extraction and selection techniques that are prevalent and guide the direction of future research on depression diagnosis.METHODS: This systematic review was conducted using a broad range of articles from two major databases: IEEE Xplore and PubMed. Studies ranging from the years 2011 to April 2021 were retrieved from the databases resulting in a total of 590 articles (53 articles from the IEEE Xplore database and 537 articles from the PubMed database). Out of those, the articles which satisfied the defined inclusion criteria were investigated for further analysis.RESULTS: A total of 135 articles were identified and analysed for this review. High growth in the number of publications has been observed in recent years. Furthermore, significant diversity in the use of data modalities and machine learning classifiers has also been noted in this study. fMRI data with an SVM classifier was found to be the most popular choice among researchers. In most of the studies, data scarcity and small sample size, particularly for neuroimaging data are major concerns. The use of identical data pre-processing tools for similar data modalities can be seen. This study also provides statistical analysis of the current framework with respect to the modality, machine learning classifier, sample size and accuracy by applying one-way ANOVA and the Tukey - Kramer test.CONCLUSION: The results indicate that an effective fusion of machine learning techniques with a potential data modality has a promising future for assisting clinicians in automatic depression diagnosis.
