Abstract:Objective The purpose of this study was to explore the long-term safety and clinical outcomes of permanent His-bundle pacing(HBP). Methods We consecutively recruited 105 patients scheduled to have permanent HBP implantation from January 2013 to December 2017 at Sir Run Run Shaw Hospital,Zhejiang University College of Medicine and explored the parameter changes and clinical outcomes. Results Eighty-eight patients received permanent HBP successfully(64 with selective HBP and 24 with non-selective HBP). In permanent HBP,the acute pacing threshold was(1.3±0.7) V/0.4 ms,and the sensed potentials were(4.1±3.5) mV,and the impedance was(582.7±157.6) Ω. During median follow-up of 15 months,the threshold and the sensed potentials had no significant difference compared with the measurements at implantation respectively. Median NYHA functional class was improved compared with pre-implantation. left ventricular ejection fraction(LVEF)increased from baseline in patients with reduced LVEF(n=33)while no significant change was found in patients with preserved LVEF. The rehospitalization rate was 15.9%(n=14), with 11 patitents due to heart failure. Conclusion Permanent HBP is feasible and safe for patients with a standard pacemaker indication,particularly for the patients with systolic heart failure.
Minimally Invasive Perventricular Device Closure of Perimembranous Ventricular Septal Defect Without Cardiopulmonary Bypass: Multicenter Experience and Mid-Term Follow-Up

Abstract:：ObjectiveTransesophageal echocardiography (TEE) guided, minimally invasive perventricular deviceocclusion of ventricular septal defects ( VSDs) without cardiopulmonary bypass ( CPB) hasbeen applied in multiple centers. We reported experiences and the mid-term results.Methods Four hundred and thirty-two cases from 4 cardiac centers were involved in thestudy. There were 235 males and 197 females, aged from 3 months to 15 years, with a bodyweight varying from 4.0 to 26.0 kg. Three hundred and fifty-one patients hadperimembranous VSDs, 57 had intracristal or supracristal VSDs and 24 had muscular VSDs (17had multiple muscular VSDs). The diameter of the VSD ranged from 3 to 12 (5.3 ±1.6 )mm.For those with perimembranous or muscular VSDs, a 3 to 5 cm inferior sternotomy wasmade, but for those with intracristal or supracristal VSDs, a 2 to 3 cm incision was madeparastemally through the left third intercostal space. Being monitored and guided withTEE, the device was deployed to occlude the VSD through the puncture at the free wall ofthe right ventricle. TEE was used for assessing the residual shunting, the left and rightventricular outlet tracts, valvular function and for detecting any arrhythmia, The deviceswould be released if the heart rhythm was normal, as well as the residual shunting andvalvular regurgilalion were not detected. Results The procedure was completed successfullyin 417 cases(96.5% ) and converted to traditional surgical closure with CPB in the other15 cases(3.5% ). Concentric devices were used in 238 cases(57.1% )and eccentric deviceswere used in 179 patients(42.9% ). Successful procedures finished in less than 90 minutes,and the deployment and evaluation of the devices were completed in 5 to 60 (18. 2 ± 8.6)minutes. No residual shunt and detectable aortic or tricuspid insufficiency and arrhythmiawas observed. Patients were extubated within 2 hours and discharged 3 to 5 days after theoperation. During fellow-up period from 3 months to 2 years, no clinically significantcomplications occurred. Conclusion The minimally invasive device closure of VSD under TEEguidance without CPB is proved to be a simple, safe and effective treatment for aconsiderable number of children with VSD. Its use in the clinical practice should beencouraged.
Minimally Invasive Experiences on Perventricular Device Closure of Ventricular Septal Defects in Children

Abstract:Objective To introduce the minimally invasive experiences on perventricular device closure of ventricular septal defects (VSDs) under transesophageal echocardiography (TEE) guidance in two children hospital heart centers.Methods 142 cases of VSDs undertook perventricular device closure under TEE guidance in two centers between July 2007 and July 2011,in which perimembranous VSDs was found in 95 cases,intracristal VSDs in 37 cases and muscular VSDs in 10 cases.Their clinical data was retrospectively analyzed.The diameter of the VSD ranged from 3.0 to 8.5 mm (mean4.7 mm) by TTE,and ranged from 2.5 to 8.0 mm (mean 4.3 mm) by TEE.Results The procedures was completed successfully in 140 cases (97.2％).1 patient (0.7％) was complicated with Ⅲ atrialventricular conduction block in post-operation 5th day.VSD was closed with CPB in the secondary operation,the patient gradually reached to sinus rhythm in post-operation.The device in 1 patient (0.7％) fell off and located in the left pulmonary artery after the operatior.The device was taken out and followed by VSD closure with CPB in the secondary operation.Concentric devices were used in 93cases and eccentric devices were used in 47 cases.During fellow-up,no clinically significant complications were found.Conclusions The minimally invasive penventricular device closuer of VSD without CPB is a safe and effective therapy in children.It should be encouraged in clinical practice.
Long-Term Outcomes of Permanent Pacemaker Implantation in Bicaval Heart Transplant Recipients

Abstract:Background: Permanent pacemakers (PPMs) may be necessary in up to 10% of patients after heart transplantation (HT). Objectives: The purpose of this study was to evaluate long-term outcomes and clinical courses of heart transplant recipients who received PPM. Methods: All patients who required PPM after bicaval HT at Columbia University between January 2005 and December 2021 were included. Cases were compared to matched heart transplant recipients by age, sex, and year of transplantation. Patient and device characteristics including complications and device interrogations were reviewed. Outcomes of re-transplantation or graft failure/death were compared between groups. Results: Of 1,082 heart transplant recipients, 41 (3.8%) received PPMs. The median time from transplantation to PPM was 118 days (IQR: 18-920 days). The most common indications were sinus node dysfunction (60%, n = 25) and atrioventricular (AV) nodal disease (41.5%, n = 17). Post-implantation complications included pocket hematoma (n = 3), lead under-sensing (n = 2), and pocket infection requiring explant (n = 1). Rates of death and re-transplantation at 10 years post-HT were similar between groups. In multivariable analysis, after adjustment for mechanical circulatory support, pretransplantation amiodarone use, donor ischemic time and age, only older donor age was associated with increased risk of PPM implantation (P = 0.03). There was a significant decrease in PPM placement after 2018 (1.2% vs 4.4%, P = 0.02), largely driven by a decline in early PPM placement. There were no differences in mortality or need for re-transplantation between groups. Conclusions: PPMs are implanted after HT for sinus and atrioventricular node dysfunctions with low incidence of device-related complications. Our study shows a decrease in PPM implantation after 2018, likely attributable to expectant management in the early postoperative period.

Abstract:Duchenne muscular dystrophy (DMD) is an X-linked muscular dystrophy in relation with dystrophin deficient. Heart and respiratory function are classically involved and affect long-term prognosis. Significant atrio-ventricular block may occur in some patients. Implantation of traditional pacemaker may be challenging in patients with tracheotomy and on permanent home mechanical ventilation. We report the first case of a successful leadless intra-cardiac trans-catheter pacing system implantation in an adult DMD on wheelchair and on permanent home mechanical ventilation.

Abstract:Abstract Background and Purpose Pacemaker (PM) implantation after surgical or transcatheter tricuspid valve (TV) repair or replacement may be challenging. Transcatheter leadless pacemaker (LPM) implantation has become a routine procedure and is frequently adopted in patients without the need for an atrial lead, in cases with cardiac implantable device infection or in patients with marked comorbidities. The use of LPM in patients with previous TV repair or replacement has only been described in singular cases in literature. We present our single center experience of LPM implantation after TV repair. Methods Patients with a history of TV repair or replacement who underwent LPM in our institution between 01/2020 and 11/2023 were retrospectively analyzed. All patients underwent routine clinical follow-up investigations including LPM interrogation and transthoracic echocardiography to assess for TV status. For LPM implantation right anterior oblique (RAO) and left anterior oblique (LAO) caudal fluoroscopy projections were utilized to enable LPM device passage across the TV. Results The study population consisted of 10 patients (9 females, mean age 74.0±10.0 years). Two patients had a history of surgical TV annuloplasty repair and two patients a history of surgical valve replacement, four patients of transcatheter TV repair (one through the mitral clip), one patient of transcatheter valve-in-ring bio prosthesis implantation prosthesis after previous surgical ring deployment and one patient transcatheter TV replacement (Figure1). All patients suffered from permanent atrial fibrillation previous to implantation procedures. Three patients were on chronic hemodialysis therapy before LPM implantation. LPM systems were successfully implanted in all cases with a mean procedure time of 43.5±26.5 minutes and a mean fluoroscopy time of 5.9±5.5 minutes and a radiation dose of 1148,7±1424,9 cGy·cm2. For fluoroscopically guided TV crossing a mean LAO angle of 43.6±14.1° (range 22 to 53°) and caudal angle of 8.5±6.9° (range 0 to 20°) was utilized (Figure 2). The LPM device was positioned in the first positioning attempt in a right ventricular high septal localization in 8 cases. Mean pacing threshold was 0.36±0.07 V at 0.25 ms, with a mean sensing amplitude of 10.5±2.9 mV and a mean of 853±324 Ohms pacing impedance. No periprocedural complications occurred. Mean duration of follow-up was 8.1±9.2 months. There were no newly developed TV regurgitations noted. Pacing thresholds (0.37±0.1 V/0.25 ms), sensing amplitude (11.8±4.5 mV) as well as pacing impedances (865±365 Ohms) remained stable until the last follow-up investigation. Conclusion LPM implantation was feasible in patients with a history of TV repair or replacement. LPM implantation was safe and effective in various modalities of TV repair. LPM may be an alternative to conventional pacemaker implantations as well as surgical lead placement in selected patients.Figure 1.Figure 2.

Long-term outcome of permanent epicardial pacemaker implantation in neonates: Experience from an Indian center

Abstract:Introduction: Permanent pacemaker implantation (PPI) in neonates is challenging with respect to indications, device selection, implantation technique, and long-term outcomes. Complex anatomy, the need for long-term pacing with high rates, and a problematic postoperative period are the major problems. Methods: We prospectively followed up 22 newborns who underwent PPI below 28 days of life at our institute. Results: The median age at implantation was 2 days (interquartile range 1-9 days), and 9% were born preterm. The average heart rate before implantation was 46.4 ± 7.2 bpm. Maternal lupus antibodies were positive in 8 (36.4%) neonates, whereas 11 (50.0%) had associated congenital heart disease. Nineteen neonates underwent single chamber (VVI) and three underwent dual chamber (DDD) pacemaker implantation. Over a median follow-up of 46 months (range 2-123 months), the average ventricular pacing percentage was 87.5 ± 24.9%, with a stable pacing threshold. Seven children underwent pulse generator replacement due to battery depletion at a median age of 47 months. Pacing-induced ventricular dysfunction was seen in five children at a median age of 23.6 months, and two underwent upgradation to cardiac resynchronization therapy. Overall mortality was 13.6%, all due to tissue hypoperfusion and lactic acidosis in the postimplantation period. Conclusions: PPI in neonates has a favorable outcome with excellent lead survival. Overall mortality is 13.6%, which is predominantly in the postimplantation period and related to myocardial dysfunction.

Abstract:Objective To evaluate the feasibility and safety of permanent epicardial pacing in infants and children.Methods From July 2005 to June 2014,the clinical data were collected and analyzed for 18 children undergoing permanent epicardial pacemaker implantation at our cardiovascular center.There were 11 boys and 7 girls with an age range of 4 months to 7 years and a mean weight of 13.3 ± 5.8 kg.And 11 cases were diagnosed as atrium-ventricle block (AVB) after operations for congenital heart disease (CHD) while another 7 cases had no heart surgery.And their operative records and parametric changes during implantation were retrospectively analyzed.Results Among them,VVI (single chamber,n =15) and DDD (dual chamber,n =3) permanent epicardial pacing leads were implanted.Implantation was completed via midline thoracotomy and all surgical epicardial electrode leads were successfully implanted on right atrium and right ventricle.Pulse generators were placed in abdominal wall in a subcutaneous or submuscular pocket and pacing leads prepared for individual growth.No mortality occurred during an average follow-up period of 32.2 months.There were only lead fracture (n =1) and wound infection (n =1).Impedance,sensing and stimulation thresholds were all in normal ranges.Conclusions As a first-choice procedure for infants and children with severe AV conduction block,epicardial pacemaker is both safe and efficacious with fewer complications.

Abstract:Introduction Pacemaker implantation can be challenging in patients with congenital heart disease. Methods and Results In a patient with Ebstein disease and symptomatic sinus node dysfunction, despite multiple attempts, the Micra® pacemaker could not be implanted in the severely dilated right ventricle. In that context, and after iodine injection to confirm the appropriate location, the Micra® pacemaker was successfully implanted in the right appendage at the first attempt. Conclusion Despite the recent development of dedicated dual‐chamber leadless pacemaker, to the best of our knowledge, this case is the first off‐label Micra® implantation in a right appendage.

Abstract:Despite the important role of cardiac pacing in preventing syncope and sudden cardiac death in familial amyloid polyneuropathy (FAP), we lack clear guidelines as to the ideal timing and indications for permanent pacemaker implantation. Purpose: The purpose of this study was to evaluate the ideal timing for pacemaker implantation in FAP patients submitted to liver transplantation.Retrospective study of 258 FAP patients submitted to liver transplantation between 1992 and 2012. Comparison of three groups: (A) patients without pacemaker (N = 122); (B) patients submitted to pacemaker implantation after liver transplantation, with documented conduction disorders (N = 73); and (C) patients submitted to "prophylactic" pacemaker implantation before transplantation, (N = 73). Patients were followed up for 12.2 ± 6.7 years.The majority of patients (57%) were referred for pacemaker implantation, which occurred before liver transplantation in 50% of cases. Patients who required pacemaker after transplantation presented significantly higher Machado-Joseph Score during pre-transplant evaluation than those who did not require pacemaker (24 ± 10 vs 20 ± 10, p = .025), and also exhibited higher levels of hepatic cytolysis enzymes and hyperbilirubinemia. The most common indication for permanent pacemaker was first degree atrioventricular block, with a mean time between transplantation and pacemaker implantation of 8.7 ± 4.2 years. During long-term follow-up, all-cause mortality was 27% and was lowest in the group submitted to pacemaker implantation only after liver transplantation (p = 0.002).The majority of FAP patients submitted to liver transplantation will need a pacemaker at some time of follow-up. However, it seems that there is no benefit in "prophylactic" cardiac pacing before liver transplantation.

Single-Chamber and Dual-Chamber Pacemaker Devices in Adults with Moderate and Complex Congenital Heart Disease: A Single Tertiary Referral Center Experience

Abstract:The number of device implantation procedures has increased in adult patients with congenital heart disease (ACHD). Despite significant improvements in materials and implantation techniques, these patients are exposed to higher risk of device related complications than general population. Herein, we describe our single tertiary referral center experience on transvenous pacemaker (PM) implantation and follow-up in adult patients with moderate and complex congenital heart disease (CHD) as limited data are available on long-term outcome. We considered all adults with moderate and complex CHD aged more than 16 years who underwent transvenous single-chamber and dual-chamber PM implant for sinus node dysfunction or atrioventricular block between January 2013 to December 2022 at our Unit. Seventy-one ACHD patients were included in the study (mean age 38.6 ± 15.2 years, 64% with moderate CHD, 36% with complex CHD). Among 32 patients implanted with a dual chamber PM (DDD PM), 4 devices were reprogrammed in VDD mode, 3 in VVI and 2 in AAI mode during follow-up because of lead dysfunction or permanent atrial arrhythmia. In addition, 26 patients had a single chamber PM (AAI or VVI PM) and 13 patients had single-lead pacing system with a free-floating atrial electrode pair (VDD PM). Just one of 13 single-lead VDD PM was reprogrammed in VVI mode due to a low atrial sensing. In DDD PM group, 10 re-interventions were needed due to lead dysfunction (8 cases) and lead-related infective endocarditis (2 cases). Only 3 patients in the single-lead PM group developed lead dysfunction with 2 re-interventions needed, but no infective endocarditis was reported. The rate of long-term complications is high in moderate and complex ACHD with transvenous PM devices, and it is mainly lead-related. In our experience, the less leads implanted, the less complications will occur. Considering the heterogeneity of the ACHD population, transvenous single-chamber or dual-chamber PM device implantation should always be tailored on the single patient, balancing risks and benefits in this complex population.

Monocentric experience of leadless pacing with focus on challenging cases for conventional pacemaker

Abstract:Aim: Leadless cardiac pacemaker has been developed to reduce complications related to cardiac pacing and is considered as an alternative to conventional pacemaker although safety and efficacy data in clinical practice are limited. The purpose of this study was to investigate the safety and efficacy profile of Micra Transcatheter Pacing System (TPS) used in daily clinical activity with a focus on challenging cases for conventional pacing. Methods: A total of 66 patients (46 men, 79.1 ± 9.7 years) having a Class I or II indication for ventricular pacing underwent a Micra TPS implant procedure. All patients were enrolled in a prospective registry. Follow-up visits were scheduled at discharge and after 1, 3, 6 and 12 months. Results: Primary indication for pacing was third degree atrioventricular block (30.3%), sinus node dysfunction (21.2%) or permanent atrial fibrillation with bradycardia (45.5%). The device was successfully implanted in 65 patients (98.5%). During follow-up of 10.4 ± 6.1 months (range 1-23 months), electrical measurements remained stable. Mean pacing capture threshold, pacing impedance and R-wave sensing were respectively 0.57 ± 0.32 V, 580 ± 103 Ohms, 10.62 ± 4.36 mV at the last follow-up. One major (loss of function) and three minor adverse events occurred. Pericardial effusion, dislodgement, device related infection or pacemaker syndrome were not observed. Micra TPS implantation was straightforward for patients with congenital or acquired cardiac and/or vascular abnormalities, previous tricuspid surgery and after heart transplantation. Conclusion: Our experience confirms that implantation of Micra is safe and efficient in a real world population including patients who present a challenging condition for conventional pacing.

Abstract:Background: The atrial sites suitable for lead placement are limited after complex surgical atrial procedures, and lead placement can be challenging in patients with congenitally corrected transposition of the great arteries (ccTGA) after intracardiac repair. Case summary: A 34-year-old man with ccTGA, who had undergone a double-switch operation with combined Senning and Jatene operations at the age of 14 was transferred to us. He experienced faintness and suffered cardiopulmonary arrest, and electrocardiography revealed ventricular fibrillation. After conversion to sinus rhythm by urgent external defibrillation, sinus bradycardia was revealed. Electrophysiological study was done using a three-dimensional (3D) mapping system (Ensite®) to evaluate the electrical condition of atria and to decide whether atrial lead can be transvenously placed. The electrical potential of the functional right atrium was good in the lateral or posterior wall, but the threshold was high. By contrast, the roof of the functional right atrium beyond cavoatrial junction was characterized by low voltage, but in a limited region of the roof of right atrium, the threshold was satisfactory and the electrical potential was normal. Thus, 3 weeks later, we implanted a transvenous implantable cardioverter-defibrillator (ICD). We used a 3D mapping system to place the atrial lead in the limited region of the roof of the right atrium mentioned above, the threshold was 0.7 V. Discussion: Electrophysiological examination using a 3D mapping system before implantation of a dual-chamber ICD is useful because atrial sites suitable for lead placement are limited in patients.

Abstract:Limited venous access and lateral left ventricular scar are impediments to traditional cardiac resynchronization therapy. We present a case where placement of an implantable cardioverter-defibrillator from a femoral approach while using left bundle branch area pacing led to clinical improvement. Abstract:An 80-years-old patient with permanent atrial fibrillation and symptomatic, paroxysmal atrioventricular blocks (AVBs) underwent leadless pacemaker (L-PM) implantation. Seven years after implantation, as a consequence of a progression of the AVB towards a persistent form, resulting in an increased need for pacing, he developed a pacing-induced cardiomyopathy. He then underwent a successful upgrade from L-PM to a transvenous pacemaker (T-PM) with left bundle branch area pacing (LBBAP). The L-PM did not interfere with the T-PM and was turned off and abandoned. One month after the upgrading the patient showed a significant improvement in cardiac function and functional capacity.
