Abstract:Sharp-object penetration into the chest is rare and may lead to life-threatening complications, hence the significance of early detection and removal. We present an uncommon case of the accidental penetration of a sewing needle into the chest of a 5-year-old girl, with the needle traveling from the entrance site to the right ventricle through the lung tissue. Due to the possibility of cardiovascular accidents, the patient was transferred to a special ward. For positioning and correct actions, TTE and CT scan of the chest with and without contrast were performed and the presence of a tangential needle with the right ventricular wall and inside the pericardium was reported. The patient was taken to the operating room, and after exploring, a 2 cm needle was inserted into the right ventricle and removed. The patient was discharged after 5 days.

Abstract:A 37-year-old woman with depression inserted two embroidery needles into her left anterior wall a year ago, experiencing stabbing pain and discomfort in the left shoulder joint during the second insertion. She has since intermittently experienced localized pain in her back and left upper limb but recently sought treatment at our hospital due to increased frequency and duration of pain episodes. Admission physical examination is as follows: body temperature was 36.2°C, heart rate at 71 b.p.m., respiratory rate at 20 b.p.m., and blood pressure was 110/78 mmHg. The cardiac dimensions were normal with a regular rhythm and no murmurs in any valve areas. Cardiac computed tomography angiography (CTA) revealed around 2.2 cm long patchy dense opacity within the left ventricle involving interventricular septum. Additionally, there was an approximately 0.8 cm long dense shadow within the interventricular septum, along with a short rod-shaped dense shadow of ∼0.5 cm in length located in the posterior wall of the left ventricle ( Panels B–D , green arrow; Supplementary data online , Video S1 ). Furthermore, a small tubular dense shadow around 4 cm in length was identified within the subcutaneous soft tissue near the midline of left anterior chest wall ( Panels A and B , blue arrow). The patient was diagnosed with metallic foreign bodies in the left ventricular septum, left ventricular posterior wall, and left anterior chest wall after clinical evaluation. Successful surgeries, including intracardiac foreign body removal, pericardial release, and chest wall foreign body removal, were performed. The patient had a satisfactory recovery and was discharged after 5 days.

The Role Of Endovascular Stent Graft In The Successful Management Of Esophageal Foreign Body With Dangerous Vascular Complications

Abstract:The esophageal foreign body with dangerous vascular complications is a rare but usually fatal disease, including aortoesophageal fistula (AEF), arterio-esophageal fistula, penetrating aortoesophageal injury, artery dissection etc. Although surgical intervention is mandatory to these entities, there is little consensus about the optimal management of these conditions due to rare cases reported. With advent of the endovascular surgical strategy, the notion of treatment has been changed, and the successful cases reported had increased. Herein, we report three cases with dangerous vascular complications. First case is a 31-year-old man accidentally swallowing a pill with an oblong plastic packaging one day before admission and clinical manifestation of retrosternal pain and worsening dysphagia, with the diagnose of potential AEF. The second case is a 58-year-old woman swallowing a duck bone accidentally five days before admission and clinical manifestation of mild odynophagia, with the diagnose of aortic intramural hematoma. The third case is a 46-year-old-man swallowing a fish bone accidentally one day before admission and clinical manifestation of mild odynophagia and abnormal foreign body sensation, with the diagnose of one penetrating aortoesophageal injury. They were all successfully managed by combination of an emergent endovascular stent graft and endoscopic technique, and uneventful with follow-up of 14 months, 19 months and 23 months respectively.

Abstract:To summarize the emergency nursing experience of a patient who underwent emergency thoracotomy for intracardiac mental foreign body removal due to accidental swallowing of the suture needle which penetrated into the adventitia of the pulmonary artery. Comprehensive assessment of the risk of major hemorrhage,pneumothorax,and cardiac tamponade should be carried out because the suture needle has broken into the pericardial cavity and was adjacent to the great blood vessels. According to the expert consensus on emergency treatment of esophageal foreign bodies in adults（2020 version）,the multidisciplinary diagnosis and treatment plan was quickly launched,the prevention and early identification of bleeding and pneumothorax and standardized transport and handover plans were adjusted in a targeted manner. Close team work allowed the patient valuable time for early surgery. Refinement nutrition management,complication prevention and nursing were the difficult points of postoperative care. After 14 days of treatment and nursing,the patient recovered and was discharged from the hospital. After 3 months of follow-up,the patient was in good condition.

Abstract:Intrahepatic foreign bodies are extremely rare before 6 months of age. We reported a case of a 5-month-old boy with a needle-like foreign body in the liver. The foreign body was incidentally found in the right hepatic lobe on the x-ray image. He was asymptomatic, with neither a history of swallowing a needle nor an abdominal cutaneous scar. Three-dimensional reconstruction of spiral computed tomographic scan showed an intrahepatic needle, close to the base of the heart, with its proximal end close to the gallbladder fossae. Because of the localization of the needle and subsequent risks of complications, surgical removal was recommended. At laparotomy, a tiny scar was recognized in the upper surface of the right lobe of the liver, confirming the migration route. Postoperative course was uneventful, and the child was discharged on postoperative day 10 and is thriving perfectly 2 months after surgery. We reviewed the clinical issues of intrahepatic foreign bodies and briefly discussed its approach and implications.

Abstract:Abstract Background Penetrating cardiac injuries are rare but often fatal, with 16–55% mortality. We report a patient who suffered a non-fatal occupational cardiac injury. Case presentation A 47-year-old man was operating an ironworker machine. A thin 3-cm metal fragment catapulted from the machine piercing the chest wall and the right ventricular outflow tract (RVOT), burrowing into the interventricular septum (IVS). The patient remained hemodynamically stable and walked to the nearest hospital. ECG-gated computed tomography revealed the exact location of the fragment within the IVS, allowing for detailed preoperative planning. The fragment was removed through a sternotomy and an incision through the RVOT. The postoperative course was uneventful. Conclusions This case underscores the value of detailed preoperative imaging and the wide spectrum of clinical scenarios of penetrating cardiac injuries.

Abstract:Introduction: Penetrating thoracic trauma with retained foreign bodies, such as needles, is rare. These injuries present management challenges due to diverse aetiologies and potential complications, including tension pneumothorax, cardiac tamponade, and major haemorrhage. Cardiac penetration can result in arrhythmias, ischaemia, valvular and septal defects. Effective management depends on patient status, injury mechanism, and resources. Case presentation: A 2-year-old girl presented with left chest wall pain after falling. Examination revealed a small puncture wound and a rhythmic bulge at the left fifth intercostal space. She was stable, with normal vitals and no respiratory distress. Imaging, including chest X-ray and echocardiography, revealed a 40 mm needle in the left pleural space, contacting the pericardium near the left ventricular apex. She was transferred to a paediatric cardiothoracic centre, where a left anterolateral thoracotomy confirmed the needle's location and facilitated its removal. She recovered without complications. Discussion: Retained foreign bodies in the heart can arise from various injuries and pose complications from asymptomatic to life-threatening. This case underscores the importance of early assessment, particularly in children where symptoms may be subtle. Essential imaging aids in diagnosis and surgical planning. The surgical approach was context-specific, favouring minimally invasive options when feasible. Trauma networks play a crucial role in improving outcomes by ensuring timely access to specialized care. Conclusion: This case highlights the importance of early assessment and intervention in paediatric patients with penetrating thoracic trauma involving foreign bodies. The successful outcome was due to prompt recognition, accurate imaging, and coordinated efforts within a specialized trauma network.

Abstract:Abstract Background Cardiac foreign bodies are extremely rare in clinical patients, especially when foreign bodies damage the internal structure of the heart coincidentally after they penetrate the heart. Case presentation Here, we report the case of a two-year-old girl whose heart was penetrated by a needle, which triggered mitral valve regurgitation and endocarditis. After a comprehensive inspection, accurate judgment and surgical preparation, we removed the needle and repaired her mitral valve. Fortunately, she recovered postoperatively. Conclusion From this case, we can know that when cardiac foreign bodies are suspected, ultrasound is an important inspection method. Moreover, the approaches for handling each such case are different depending on the associated injuries.

Abstract:Introduction: Endobronchial ultrasound-guided transbronchial needle aspiration is increasingly used as a minimally invasive procedure in clinical settings. It is generally regarded as a safe procedure with high diagnostic accuracy. However, a complication involving a needle fracture that penetrated a nearby artery has not been reported during this procedure. Case Presentation: A male patient, 58 years of age, underwent endobronchial ultrasound-guided transbronchial needle aspiration for a mediastinal lymph node biopsy at a local hospital. The aspiration needle fractured and penetrated from the right middle segmental bronchus into the right pulmonary artery. The patient was then transferred to our hospital. After conducting repeated chest imaging examinations to confirm the presence of the foreign body and holding multidisciplinary team consultations, we first inserted a deflated balloon catheter near the puncture site in the right middle segmental bronchus. Following the needle retrieval through a flexible bronchoscope, the balloon catheter was inflated to ensure local hemostasis. Follow-up evaluations revealed no further complications for this patient. Conclusion: Intragenic vascular injury can occur during endobronchial ultrasound-guided transbronchial needle aspiration. Careful pre-procedure preparations should be planned to minimize complications. In patients experiencing complications due to needle penetration, consultation and coordination with a multidisciplinary team are essential to ensure the safe retrieval of the broken needle.

Abstract:The first commercial use of 1-methylcyclopropene (1-MCP) was with cut flowers and potted plants, and treatment withthiscompound is still the preferred strategy forprotection ofmost ethylene-sensitive crops.Research iscontinuing into optimum treatment conditions and strategies for efficient application in commerce. In studies using carnation (Dianthus caryophyllusL. 'White Sim') petals to determine the optimal conditions for commercial treatment, we notedsome aspects of the inhibition response that were not consistent with the current competitive inhibition model of 1-MCP action. Our data are better explained by an alternative model in which 1-MCP binds to a site that is exposed during the allosteric changes that accompany the enzymatic activities of the binding site in the absence of ethylene.

Abstract:Intracardiac foreign bodies (IFB) are rare clinical conditions. There are now several reports on the percutaneous retrieval of IFB under fluoroscopy. However, some IFB are not radiopaque, and retrieval requires combined fluoroscopic and ultrasound guidance. We report the case of a bedridden 23-year-old male patient with T-lymphoblastic lymphoma treated with long-term chemotherapy. Ultrasound examination diagnosed a huge thrombus in the right atrium near the opening of the inferior vena cava which affected the patency of his PICC line. Ten days of anticoagulant therapy did not modify the thrombus size. Open heart surgery was not feasible because of the patient clinical condition. Snare-capture of the non-opaque thrombus was done from the femoral vein under fluoroscopic and ultrasound guidance with excellent outcomes. We also present a systematic review of IFB. We found out that percutaneous removal of IFBs is a safe and effective procedure. The youngest patient who received percutaneous IFB retrieval was 10 days old and weighed only 800 g, while the oldest patient was 70 years old. Port catheters (43.5%) and PICC lines (42.3%) were the most commonly found IFBs. Snare catheters and forceps were the most commonly used instruments.

Abstract:Background: Foreign body aspiration is a life threatening condition that must be treated immediately, such as nuts, plastic, hijab needles. Incidents commonly occur in children, especially under 3 years old. Clinical manifestations may vary from mild to severe. Treatment includes bronchoscopy, VATS and thoracotomy. Video Assisted Thoracoscopy Surgery can reduce postoperative morbidity, pain scale and length of stay. Conversion from VATS to open thoracic surgery is 2-23% Case Presentation: A 12 year old girl was admitted with chief complaints inhaling a hijab needle one day before admission. The patient tried to remove the needle by pulling out the needle from the base of the tongue. Saliva was not mixed with blood. There was no shortness of breath, cough, nor pain when swallowing, neither nausea nor vomiting. Physical examination was within normal limit. A chest x-ray showed a needle-like object projecting from the right bronchus at Th6-Th7. Bronchoscopy was carried out as first attempt, followed by VATS and eventually converted to thoracotomy. Patient remained stable after procedure and discharged without complications. Conclusion: Needle hijab aspiration often occurs in adolescent girls which can cause morbidity. Abstract:Ear, Nose &Throat Journal, Ahead of Print. Retained needles are frequently observed in intravenous drug users. However, an incidental discovery of multiple needles at the neck seldom occurs. These foreign bodies remain either asymptomatic or cause local and central complications that can be fatal. We report the case of a 43-year-old male having a medical record of human immunodeficiency virus and hepatitis C who presented to our department with a 3-day history of a febrile lateral neck swelling. He reported a history of repetitive intravenous drug use and previous abscesses that were surgically drained. We noted a left and tender 8 cm neck swelling with torticollis. Computed tomography scan showed a collected myositis of the sternocleidomastoid muscle in association to multiple metallic foreign bodies at the neck that were compatible with the aspect of broken needles. Imagery also showed a thrombosis of the ipsilateral jugular vein. No central embolism of a foreign body was noted. We performed a surgical drainage of the muscular abscess and managed to retrieve a 1 cm broken needle that was retained at the level of the collection. We did not retrieve the other needles since they were asymptomatic. The patient was placed under intravenous antibiotics for 1 week followed by oral amoxicillin-clavulanate with a total regression of the cellulitis. At the further follow-ups, the rest of the foreign bodies remained asymptomatic. Our study emphasizes on the importance of suspecting a retained foreign body in intravenous drug users to ensure an adequate management of the possible complications and to protect the medical team from incidental needle injuries.

Abstract:A 66-year-old woman came to the emergency room with symptoms of foreign-body sensation and pharyngeal pain after the accidental ingestion of a foreign body during breakfast, but she had no idea of what she had swallowed. Computed tomography of the neck, performed to confirm the presence of a foreign body and clarify its relationship with surrounding structures, showed a metallic foreign body in the right pharyngeal cavity ([Fig. 1 a]), and synthesized three-dimensional images of the structures of the neck showed a foreign body located at the level of the fifth cervical vertebra ([Fig. 1 b]). Esophagogastroscopy along the right piriform sinus revealed a needle-like foreign body with one end embedded in the wall ([Fig. 2]).

Abstract:<h3 class="Heading">Background</h3><p class="Para">Acupuncture is a famous traditional medicine in China, but the complications caused by broken acupuncture needles have been rarely reported. It seems easy to remove the foreign matters usually, but things become difficulty in special issues. Here, we reported a recently encountered case to provide an important teaching point of treating a chronically retained broken needle in retroperitoneum.</p><h3 class="Heading">Case presentation</h3><p class="Para">A 42-year-old man presented with a chronically retained broken needle in his body after acupuncture therapy two years ago. However, due to the discomfort at the left back recently and ordinary inconvenience such as security check, he came to our hospital for minimally invasive surgery. He was introduced to our department because the broken needle had migrated from subcutaneous to adipose tissue in retroperitoneum during the two years. Considering the position of the broken needle, the patient was performed by laparoscopy in general anesthesia. The operation time was about 31 min and there were only three 7 mm incisions in the left lateral abdominal wall. The X-ray exam was performed to confirm that the broken needle was removed integrally. The patients begun normal activity at 6 h after surgery and was discharged on the second day after surgery.</p><h3 class="Heading">Conclusions</h3><p class="Para">Acupuncture is widely used for pain treatment in China, but how to handle the complication of acupuncture needle broken in body are rarely reported. Laparoscopy will be the reasonable choice for treating needles broken in retroperitoneum.</p>

Abstract:Video-assisted thoracoscopic surgery (VATS) lobectomy, especially uniportal VATS, is increasingly used for pulmonary sequestration (PS). However, there are few descriptions of safe handling of the aberrant artery with atherosclerosis, especially the diameter of arteries exceeds than 2.0 cm, under uniportal VATS approach. Here we report a 56-year-old man who was diagnosed with pulmonary sequestration following trauma. The patient had a long history of cough with purulent sputum. One month before the trauma, he had copious expectoration with foul smell again. A contrast CT scan revealed a 7.5 cm mass in his right lower lobe. The mass was supplied by a thick aberrant atherosclerotic artery (over than 2 cm in diameter), which stemmed from the thoracic aorta with multiple calcifications on both. After adequate preoperative evaluation, we performed a right lower lobectomy under uniportal VATS approach. No surgical-associated complications occurred, and the patient was discharged on the 5th days after the operation. We organized an iMDT (international multidisciplinary team) to discuss the reasonability and optimal treatment pattern for this patient. We found that fully assess the quality of the aberrant arteries of PS following blocking and cutting off in an appropriate way are crucial to avoid the happening of fatal bleeding during the operation.
