Abstract:Background: Invasive Aspergillosis is a fungal infection caused by Aspergillus species, typically posing life-threatening risks to immunocompromised individuals. While occurrences in immunocompetent hosts are rare, a recent case report documented fulminant pulmonary aspergillosis in an immunocompetent patient during autopsy. Here, we present a case of invasive aspergillosis in an immunocompetent woman, manifesting with disseminated lesions. Case presentation: A 29-year-old Asian woman presented to our hospital in March 2022, reporting chest pain and shortness of breath persisting for two months. Upon examination, she appeared thin and unwell, with no notable abnormalities otherwise. Radiographic imaging revealed an ill-defined lesion in her left lung. Subsequent bronchoscopy and lavage were performed, followed by initiation of empirical antibiotic therapy. Lavage results were negative for gram staining, culture, and ZN staining for AFB, but revealed numerous septate hyphae on fungal smear. Histopathological examination indicated chronic granulomatous inflammation with septal fungal hyphae, indicative of aspergillosis. Subsequent culture confirmed Aspergillus species, prompting initiation of voriconazole therapy. Remarkably, the patient exhibited significant improvement, with weight gain and restored appetite observed within a short period. Within 2 months of treatment, her symptoms resolved, and she resumed near-normal daily activities. Conclusion: This case highlights the diagnosis of aspergillosis in an immunocompetent individual presenting with disseminated nodular lesions across the lungs, mediastinum, and abdomen. Clinicians should maintain a high index of suspicion for aspergillosis in cases of non-resolving pneumonia and disseminated nodular lesions, even in patients lacking traditional predisposing factors.

Abstract:A 40-year-old male patient was referred to our radiology department complaining of chest pain, fever, and sputum. His medical history included acute lymphoblastic leukemia, bone marrow transplantation, and graft-versus-host disease. Laboratory studies showed a total leukocyte count of 14.2×10 3 uL (92.9% of neutrophils), with raised procalcitonin (0.44 ng/ml) and C-reactive protein (75.9 mg/L) levels. Thoracic CT revealed multiple nodules and masses, some in cavitary form, dispersed in both lungs (Figure 1). Sputum culture yielded Aspergillus fumigatus and flavus . At follow-up for invasive pulmonary aspergillosis, a newly developed hypodense lesion was detected in the liver parenchyma on control thoracic CT (Figure 2). MRI revealed a heterogeneous (due to hypointense areas) hyperintense lesion on T2-weighted image and a hypointense non-enhancing lesion on T1-weighted images (Figure 3). Aspergillosis was confirmed histopathologically. Abstract:Aspergillus is a genus of filamentous, ubiquitous, and opportunistic pathogenic fungi. Patients with underlying lung disease are susceptible to Aspergillus due to their immunodeficiency, but the symptoms become atypical as other lung diseases are contracted (1-3). Aspergillus fumigatus, A. flavus, and A. niger are the primary pathogenic species in most literatures (4, 5). The principal forms of pulmonary aspergillosis (PA) are allergic bronchopulmonary aspergillosis (ABPA), chronic (and saprophytic) pulmonary aspergillosis (CPA), and invasive pulmonary aspergillosis (IPA). In non-neutropenic patients, various forms are considered a semi-continuous spectrum of aspergillosis (4). The analysis of the clinical features can deepen our understanding of the spectrum of clinical manifestations of Aspergillus infection and provide references for its diagnosis.

Abstract:Mucormycosis is a rare fungal infection belonging to Mucorales order fungi. Rhino-cerebral form is more noted in patients with diabetes mellitus, but pulmonary mucormycosis is a rare manifestation with hematological malignancy, malignant tumors, and transplant recipient patients. Aspergillus species are also known to cause allergic bronchopulmonary aspergillosis, aspergilloma, chronic pulmonary aspergillosis, and invasive aspergillosis in immunocompromised individuals. We report a case of pulmonary mucormycosis superimposed with aspergillus infection presenting in an immunocompromised patient with metastatic prostate cancer on chemotherapy, initially misdiagnosed as aspergillus infection.
Adenocarcinoma of the Lung Masquerading As Invasive Pulmonary Aspergillosis in an Elderly Lady: A Diagnostic Challenge

Abstract:A 59-year-old hypertensive woman presented with a year-long history of cough, expectoration, and progressive breathlessness, recently complicated by hemoptysis and significant weight loss. Initial investigations, including a chest x-ray and contrast-enhanced computed tomography (CECT) of the thorax, suggested an infective pathology. Despite negative bacterial, fungal, and tuberculosis cultures, elevated bronchoalveolar lavage (BAL) galactomannan and serum Aspergillus-specific IgG levels led to a diagnosis of invasive pulmonary aspergillosis (IPA), and antifungal treatment commenced. The patient's initial response was positive; however, symptoms recurred three months later. Further investigations revealed adenocarcinoma, confirmed by cytology from a thoracentesis. The patient, a non-smoker, began targeted therapy with tyrosine kinase inhibitors but declined further diagnostic evaluation. Despite the poor prognosis and palliative care options, the patient opted for discharge to home care. This case underscores the complexity of diagnosing lung pathologies and the importance of considering alternative diagnoses in persistent respiratory symptoms.
Concurrent aspergillosis and cystic pulmonary metastases in a patient with tongue squamous cell carcinoma

Abstract:Abstract Pulmonary Aspergillus infection may have a variety of manifestations depending on the patients’ immunity status and pre-existing lung conditions. Radiographically, aspergilloma, which is usually associated with noninvasive Aspergillus fumigatus conidia, may feature a characteristic mass in a cavity commonly located in the upper lobes of the lung. It is typically encountered upon pre-existing lung damage. Here we report Aspergillus growing in a pulmonary metastatic cavity in a 47-year-old male worker with a history of tongue cancer after a radical operation with neck dissection and concurrent chemotherapy in 2014. Chest radiography and computed tomography showed a cavitary lesion with a ball-in-hole lesion in the right upper lobe (RUL) and two cystic lesions within the bilateral upper lung field. Endobronchial ultrasound-guided transbronchial biopsy (EBUS-TBB) from the RUL anterior segmental bronchus (RB3) revealed the presence of Aspergillus conidia and squamous cell carcinoma. Wedge resection of the cystic lesion within the left upper lobe confirmed the diagnosis of metastatic squamous cell carcinoma. This is a rare case of aspergillosis within cavities of pulmonary metastases in a patient who was diagnosed with tongue squamous cell carcinoma. The conclusive distinction between neoplasm and fungal infection is difficult to achieve by radiography, and a pathological biopsy by EBUS-TBB is necessary to aid diagnosis. Clinicians should be aware of such an atypical presentation of metastases coexisting with Aspergillus infection.

A Case of Pulmonary MALToma Camouflaged Within Invasive Pulmonary Aspergillosis

Abstract:A 59-year-old non-smoking male, with a known case of COPD (chronic obstructive pulmonary disease), treated pulmonary tuberculosis with Category 1 antitubercular drugs (six-month regimen) and was admitted with repeated bouts of moderate haemoptysis (~60 mL/day) for three days. The patient had a history of self-limiting occasional mild haemoptysis (~20 mL) over three years. An HRCT chest revealed a left upper lobe fibro-cavitary lesion with an intracavitary mass (air crescent sign), adjacent pleural thickening and fibrosis. Bronchoalveolar lavage (BAL) was positive for galactomannan and negative for Mycobacterium tuberculosis GeneXpert®. With the above clinical factors, host factors, and microbiological factors, the case was diagnosed as 'probable' invasive pulmonary aspergillosis and was treated with voriconazole. However, given relapsing haemoptysis despite adequate antifungal treatment, a left upper lobectomy was done. The resected left upper lobe specimen culture demonstrated Aspergillus fumigatus with histopathology confirming hyphae invading lung tissues confirming 'proven' invasive aspergillosis. Resected tissue also showed florid lymphoid tissue hyperplasia with Immunohistochemistry confirming the presence of a peculiar malignancy; MALT lymphoma/MALToma in the resected lobe. The association of a rare malignancy such as MALToma with invasive pulmonary aspergilloma (IPA) has been identified and reported for the first time. This could be because of a chronic inflammatory reaction elicited by the Aspergillus antigen. Long-standing fibro-cavitary disease and aspergillosis are partners in crime, augmenting the damages inflicted by one another. In such a scenario, early surgical intervention may be warranted if haemoptysis is moderate to severe or relapsing, following conservative medical management. Surgical resection may lead to the identification of unexpected diseases as in our case.

Abstract:An aspergilloma is a conglomeration of Aspergillus hyphae, fibrin, mucus, and cellular debris, typically found within a pulmonary cavity or ectatic bronchus. Computerized tomography (CT) scans often depict a cavity containing a solid mass and a notable crescent sign. Though these signs are indicative of aspergilloma, the European Respiratory Society emphasizes the need for a more detailed diagnostic criteria. A patient with a history of hemoptysis was initially diagnosed with an aspergilloma based on CT chest findings, showing a cavitary lesion in the left upper lobe with an intracavitary lobular opacity. Post resection, histological examination contradicted the initial diagnosis, revealing lung cancer instead. This case underscores the importance of tissue diagnosis from the outset or employing a multifaceted diagnostic criteria encompassing radiological findings, serology and microbiology tests. In the absence of an initial tissue diagnosis, rigorous follow-up, including early interval scanning, is crucial.

Abstract:The association of noncavitary untreated lung cancer with coexisting pulmonary Aspergillosis in immunocompetent patients is an extremely rare occurrence. The present paper aims to summarize the clinical data, and gives an overview of the current knowledge on the etiology, diagnosis and treatment of this condition.We report four cases and review seven cases in the literature.The four cases describe pulmonary solid tumor with coexisting pulmonary Aspergillosis condition. All cases underwent complicated procession for the terminal diagnosis.Clinicians should be alert to underlying malignant disease if airway Aspergillus infection is suspicious in a patient without strong risk factors for invasive fungal disease. On the other hand, when lung cancer is coexisting with airway necrotizing Aspergillosis, clinicians should properly manage these two diseases simultaneously.

Abstract:Rationale: Cerebral aspergillosis (CA) is a rare manifestation of invasive aspergillosis. It usually affects seriously immunocompromised hosts. Pancreatic bacterial or/and fungal infection is common in patients with severe acute pancreatitis. Patient concerns: We report the first case of an immunocompetent woman with infected necrotizing pancreatitis due to multidrug resistant Acinetobacter baumannii who, in the course of treatment, developed isolated CA. Diagnoses: Magnetic resonance imaging, rather than computed tomography, revealed latent homolateral sinus disease-the possible source of the Aspergillus infection. Interventions: The pancreatic infection was controlled by open necrosectomy, and the CA was disappeared after neuronavigation-guided drainage and voriconazole antifungal therapy. Outcome: The patient was discharged without complications. Our report revealed that persistent hyperglycemia, sepsisassociated immunoparalysis, and prolonged antibiotic use could impair severe patient's immunocompetence, making them more susceptible to opportunistic cerebral Aspergillus infection; the risk may be especially high in patients with paranasal sinus diseases. Lessons: Timely neurosurgical intervention combined with voriconazole antifungal therapy can provide a favorable outcome

Abstract:Pulmonary Aspergillus infections are caused by the ubiquitous fungus mold when spores found in compost, dust, and plant material are inhaled. Manifestation of the disease is represented by a delicate balance between host and pathogen interactions, making Aspergillus infection more commonly observed in immunocompromised individuals. Chronic cavitary pulmonary aspergillosis (CCPA) is a subtype of pulmonary Aspergillosis associated with history of chronic or prior lung disease. Here we present the case of a non-immunocompromised 24-year-old male with a history of intermittent asthma with chronic pulmonary symptoms and finding of a lung mass on imaging. We describe the diagnostic challenge and surgical treatment of CCPA in medically refractory disease.

Abstract:Trichosporon asahii is an emerging opportunistic pathogen that is life-threatening particularly for immunosuppressed patients. Only a few studies have described Trichosporon infection in kidney transplant recipients. This study reports a 67-year-old male kidney transplant recipient who developed fatal fungemia and pneumonia caused by T. asahii during caspofungin treatment. Although funguria is benign, kidney transplant recipients are still at risk of T. asahii fungemia and invasive T. asahii infection even if they are under antifungal therapy, particularly echinocandins.

Abstract:Immunodeficiency is a state of compromised immune mechanisms that leads to increased vulnerability to opportunistic infections. [ 1 ] Opportunistic infections in immunocompromised patients can present a diagnostic challenge. Nocardia and Aspergillus fumigatus are opportunistic pathogens and easily overlooked owing to their rarity, leading to delayed diagnosis and treatment. In patients with connective tissue diseases, steroids use may lead to an increased risk of opportunistic infections with a poor prognosis. Pneumonia in immunocompromised individuals often has high incidence and mortality rates. However, co-infection with Nocardia and A fumigatus has rarely been reported. We report a case of pneumonia with Nocardia and A fumigatus successfully treated.

Abstract:We present a case of invasive pulmonary aspergillosis in an immunocompetent young female. An 18-year-old female presented with symptoms of a left-sided middle cerebral artery (MCA) stroke with right arm weakness and aphasia. Computed tomography (CT) brain confirmed the diagnosis of stroke. Further history revealed that the patient had been experiencing low-grade fevers with occasional shortness of breath for the past year. The blood work had eosinophilia at that time for which she was given mebendazole but saw little improvement. Chest X-rays showed upper lobe consolidation for which a tuberculosis (TB) workup was also done, which also came out negative. At the current presentation, she underwent further workup with echocardiography and eventual ultrasound-guided mediastinal biopsy that ultimately led to the correct diagnosis of aspergillosis. However, sadly, it was already too late for the patient who passed away one day after the commencement of the amphotericin B therapy. This paper hopes to decrease the threshold of clinical suspicion for invasive aspergillosis (IA) regardless of the immunity status of the patient, especially if they are presenting with an unrelenting mediastinal or pulmonary symptom complex in the setting of eosinophilia.
An unusual Case of Pulmonary Sarcomatoid Carcinoma mimicking Aspergillus

Abstract:Abstract Sarcomatoid carcinoma is a rare malignancy with strong invasion and high malignancy, and it contains epithelial cells and sarcoma or sarcoma-like components. Here we reported one case of pulmonary sarcomatoid carcinoma in a 55-year-old COPD male who presented with wet coughing and hemoptysis, chest CT showed a mass in the left lung. According to tracheoscopy, the patient was initially diagnosed as Aspergillus, but was eventually corrected by histology and Immunohistochemistry of lung puncture specimens. In this study, we describe the diagnostic procedure, as well as chemotherapy and targeted therapy response in this pulmonary sarcomatoid carcinoma patient with EGFR mutation, and review relevant literature.

She denied any history of fever, cough, or COVID infection. She has hypothyroidism controlled on thyroxine 25mcg. During the evaluation, she was found to have an enhancing solitary pulmonary nodule (11 x 10 x 9mm) in the anterior segment of the left upper lobe (Figure 1). The patient was given a course of oral antibiotics (amoxicillin /clavulanic acid) and supportive treatment for hemoptysis. Sputum for Ziehl–Neelsen stain and cartridge based nucleic acid amplification test (CBNAAT) was negative. CT- guided biopsy of the nodule was performed. Abstract:A 76?year?old female patient complained of right chest pain for three months. CT scan showed a clump?like high?density shadow measuring 4.8 cm × 3.0 cm in size in the dorsal portion of the right lower lobe of the lung. Aspiration biopsy was performed, and biopsy samples were subjected to fungal culture and histopathological examination. Histopathological examination showed chronic granulomatous inflammation with hyaline septate hyphae. After 4?day culture, white villous dense colonies were formed on the Sabouraud′s agar medium. The center of the colonies was slightly elevated with wrinkles or radiating striae on the surface, and the bottom of the colonies was faint yellow in color. Microculture yielded abundant septate branched hyphae, and very few colorless hyaline quasi?circular spores. DNA sequencing of rDNA internal transcribed spacer (ITS) regions and β?tubulin genes was performed to identify the isolate, and antifungal susceptibility testing was carried out in vitro. The MEGA7.0 software was used to build phylogenetic trees of Aspergillus fumigatus complex and its closely related species. The isolate was identified as Aspergillus fumigatus by molecular biologic sequencing. The patient was diagnosed with pulmonary aspergilloma. After administration of itraconazole oral solution and vorionazole tablets, the condition got better obviously.
