Abstract:Introduction Cryptococcosis is a potentially fatal fungal infection in solid organ transplant recipients. Meningoencephalitis is the most common clinical manifestation with mortality rate of 25% in USA to 70% in developing countries. We present a case of cryptococcal meningitis in a heart transplant recipient who survived after an unusual initial presentation. Case description A 62-year-old man with a history of nonischemic cardiomyopathy status post heart transplantation presented to the ER 4 months after transplant with bifrontal and bimaxillary facial pain for 5 days. The pain was dull, progressively worsening, 8/10 in severity, non-positional, and slightly relieved with Tylenol. He also reported a month of fatigue for which endomyocardial biopsy was performed a week prior to hospitalization and negative for rejection. His immunosuppressive therapy included tacrolimus, mycophenolate, and prednisone. He reported no fevers or chills, and vital signs were normal on admission. Physical exam was without localizing infectious signs. He was fully oriented, had no nuchal rigidity, and his neurologic exam was non-focal with no ocular signs of papilledema, ischemia, or giant cell arteritis. Complete blood count and comprehensive metabolic panel labs were unremarkable. Pertinent labs included elevated LDH 417 U/L, ESR 49 mm/hr and CRP 34 mg/L. Imaging including maxillofacial CT, brain MRI and MRV were unremarkable. Infectious workup on admission was initially unremarkable, however on day 3 blood cultures returned with budding yeast and serum cryptococcal antigen test was positive. Blood cultures speciated to Cryptococcus neoformans. CSF obtained via LP was notable for elevated opening pressure of 38cm H2O, positive cryptococcal antigen and India ink stain, with cultures growing C. neoformans. TEE showed EF 60% with no intracardiac vegetation. He received a 14-day induction phase of flucytosine and 28 days of liposomal amphotericin B. He continued to have headaches requiring serial LPs with elevated opening pressure, thus requiring a lumbar drain placement. A week later, he was found to have Staphylococcus epidermidis meningitis due to lumbar drain, which was removed and treated with vancomycin. Immunosuppression was reduced in the setting of infection on admission. His 28-day antifungal induction phase was longer than the standard 14 days due to persistent growth of C. neoformans in CSF at 14 days. He was then transitioned to consolidation therapy with fluconazole for 8 weeks, followed by maintenance therapy for 1 year. He continues to follow in our clinic and is 3 years post transplant with no residual neurological symptoms. Conclusion The present case highlights an unusual presentation for a life-threatening fungal infection in a heart transplant patient. The absence of fever, leukocytosis and meningeal signs should not rule out infection in immunosuppressed heart transplant patients presenting with headache since these patients cannot mount appropriate immunological response.
