Abstract:Properly assessing the effects of environmental chemical exposures on disease risk remains a challenging problem in environmental epidemiology. Various analytic approaches have been proposed, but there are few papers that have compared the performance of different statistical methods on a single dataset. In this paper, we compare different regression-based approaches for estimating interactions between chemical mixture components using data from a case-control study on non-Hodgkin's lymphoma. An analytic challenge is the high percentage of exposures that are below the limit of detection (LOD). Using imputation for LOD, we compare different Bayesian shrinkage prior approaches including an approach that incorporates the hierarchical principle where interactions are only included when main effects exist. Further, we develop an approach where main and interactive effects are represented by a series of distinct latent functions. We also fit the Bayesian kernel machine regression to these data. All of these approaches show little evidence of an interaction among the chemical mixtures when measurements below the LOD were imputed. The imputation approach makes very strong assumptions about the relationship between exposure and disease risk for measurements below the LOD. As an alternative, we show the results of an analysis where we model the exposure relationship with two parameters per mixture component; one characterizing the effect of being below the LOD and the other being a linear effect above the LOD. In this later analysis, we identify numerous strong interactions that were not identified in the analyses with imputation. This case study demonstrated the importance of developing new approaches for mixtures when the proportions of exposure measurements below the LOD are high.

Abstract:Quantitative characterization of the health impacts associated with exposure to chemical mixtures has received considerable attention in current environmental and epidemiological studies. With many existing statistical methods and emerging approaches, it is important for practitioners to understand when each method is best suited for their inferential goals. In this study, we conduct a review and comparison of 11 analytical methods available for use in mixtures research, through extensive simulation studies for continuous and binary outcomes. These methods fall in three different classes: identifying important components of a mixture, identifying interactions and creating a summary score for risk stratification and prediction. We carry out an illustrative data analysis in the PROTECT birth cohort from Puerto Rico. Most importantly we develop an integrated package “CompMix” that provides a platform for mixtures analysis where the practitioner can implement a pipeline for several types of mixtures analysis. Our simulation results suggest that the choice of methods depends on the goal of analysis and there is no clear winner across the board. For selection of important toxicants in the mixture and for identifying interactions, Elastic net by Zou et al. (Enet), Lasso for Hierarchical Interactions by Bien et al (HierNet), Selection of nonlinear interactions by a forward stepwise algorithm by Narisetty et al. (SNIF) have the most stable performance across simulation settings. Additionally, the predictive performance of the Super Learner ensembling method by Van de Laan et al. and HierNet are found to be superior to the rest of the methods. For overall summary or a cumulative measure, we find that using the Super Learner to combine multiple Environmental Risk Scores can lead to improved risk stratification properties. We have developed an R package “CompMix: A comprehensive toolkit for environmental mixtures analysis”, allowing users to implement a variety of tasks under different settings and compare the findings. In summary, our study offers guidelines for selecting appropriate statistical methods for addressing specific scientific questions related to mixtures research. We identify critical gaps where new and better methods are needed.

Abstract:Purpose of This ReviewThis review provides a summary of statistical approaches that researchers can use to study environmental exposure mixtures. Two primary considerations are the form of the research question and the statistical tools best suited to address that question. Because the choice of statistical tools is not rigid, we make recommendations about when each tool may be most useful.Recent FindingsWhen dimensionality is relatively low, some statistical tools yield easily interpretable estimates of effect (e.g., risk ratio, odds ratio) or intervention impacts. When dimensionality increases, it is often necessary to compromise this interpretablity in favor of identifying interesting statistical signals from noise; this requires applying statistical tools that are oriented more heavily towards dimension reduction via shrinkage and/or variable selection.SummaryThe study of complex exposure mixtures has prompted development of novel statistical methods. We suggest that further validation work would aid practicing researchers in choosing among existing and emerging statistical tools for studying exposure mixtures.

Abstract:The estimation of the effect of environmental exposures and overall mixtures on a survival time outcome is common in environmental epidemiological studies. While advanced statistical methods are increasingly being used for mixture analyses, their applicability and performance for survival outcomes has yet to be explored. We identified readily available methods for analyzing an environmental mixture's effect on a survival outcome and assessed their performance via simulations replicating various real-life scenarios. Using prespecified criteria, we selected Bayesian Additive Regression Trees (BART), Cox Elastic Net, Cox Proportional Hazards (PH) with and without penalized splines, Gaussian Process Regression (GPR) and Multivariate Adaptive Regression Splines (MARS) to compare the bias and efficiency produced when estimating individual exposure, overall mixture, and interaction effects on a survival outcome. We illustrate the selected methods in a real-world data application. We estimated the effects of arsenic, cadmium, molybdenum, selenium, tungsten, and zinc on incidence of cardiovascular disease in American Indians using data from the Strong Heart Study (SHS). In the simulation study, there was a consistent bias-variance trade off. The more flexible models (BART, GPR and MARS) were found to be most advantageous in the presence of nonproportional hazards, where the Cox models often did not capture the true effects due to their higher bias and lower variance. In the SHS, estimates of the effect of selenium and the overall mixture indicated negative effects, but the magnitudes of the estimated effects varied across methods. In practice, we recommend evaluating if findings are consistent across methods.

Abstract:The assessment of personal exposure to air pollution is a critical component of epidemiological studies associating air pollution and health effects. This paper critically reviewed 157 studies over 29 years that utilized one of five categories of exposure methods (proximity, air dispersion, hybrid, human inhalation, and biomarkers). Proximity models were found to be a questionable technique as they assume that closer proximity equates to greater exposure. Inhalation models and biomarker estimates were the most effective in assessing personal exposure, but are often cost prohibitive for large study populations. This review suggests that: (i) factors such as uncertainty, validity, data availability, and transferability related to exposure assessment methods should be considered when selecting a model; and (ii) although an entirely discreet new class of approach is not necessary, significant progress could be made through the development of a 'hybrid' model utilizing the strengths of several existing methods. Future work should systematically evaluate the performance of hybrid models compared to other individual exposure assessment methods utilizing geospatial information technologies (e.g. geographic information systems (GIS) and remote sensing (RS)) to more robustly refine estimates of ambient exposure and quantify the linkages and differences between outdoor, indoor and personal exposure estimates.

Abstract:Humans are exposed to a large number of environmental chemicals: Some of these may be toxic, and many others have unknown or poorly characterized health effects. There is intense interest in determining the impact of exposure to environmental chemical mixtures on human health. As the study of mixtures continues to evolve in the field of environmental epidemiology, it is imperative that we understand the methodologic challenges of this research and the types of questions we can address using epidemiological data. In this article, we summarize some of the unique challenges in exposure assessment, statistical methods, and methodology that epidemiologists face in addressing chemical mixtures. We propose three broad questions that epidemiological studies can address: a) What are the potential health impacts of individual chemical agents? b) What is the interaction among agents? And c) what are the health effects of cumulative exposure to multiple agents? As the field of mixtures research grows, we can use these three questions as a basis for defining our research questions and for developing methods that will help us better understand the effect of chemical exposures on human disease and well-being.

Abstract:Because humans are invariably exposed to complex chemical mixtures, estimating the health effects of multi-pollutant exposures is of critical concern in environmental epidemiology, and to regulatory agencies such as the U.S. Environmental Protection Agency. However, most health effects studies focus on single agents or consider simple two-way interaction models, in part because we lack the statistical methodology to more realistically capture the complexity of mixed exposures. We introduce Bayesian kernel machine regression (BKMR) as a new approach to study mixtures, in which the health outcome is regressed on a flexible function of the mixture (e.g. air pollution or toxic waste) components that is specified using a kernel function. In high-dimensional settings, a novel hierarchical variable selection approach is incorporated to identify important mixture components and account for the correlated structure of the mixture. Simulation studies demonstrate the success of BKMR in estimating the exposure-response function and in identifying the individual components of the mixture responsible for health effects. We demonstrate the features of the method through epidemiology and toxicology applications.

Abstract:Humans are routinely exposed to mixtures of chemical and other environmental factors, making the quantification of health effects associated with environmental mixtures a critical goal for establishing environmental policy sufficiently protective of human health. The quantification of the effects of exposure to an environmental mixture poses several statistical challenges. It is often the case that exposure to multiple pollutants interact with each other to affect an outcome. Further, the exposure-response relationship between an outcome and some exposures, such as some metals, can exhibit complex, nonlinear forms, since some exposures can be beneficial and detrimental at different ranges of exposure. To estimate the health effects of complex mixtures we propose a flexible Bayesian approach that allows exposures to interact with each other and have nonlinear relationships with the outcome. We induce sparsity using multivariate spike and slab priors to determine which exposures are associated with the outcome, and which exposures interact with each other. The proposed approach is interpretable, as we can use the posterior probabilities of inclusion into the model to identify pollutants that interact with each other. We illustrate our approach's ability to estimate complex functions using simulated data, and apply our method to two studies to determine which environmental pollutants adversely affect health.

Abstract:In recent years, the precision of exposure assessment methods has been rapidly improved and more widely adopted in epidemiological studies. However, such methodological advancement has introduced additional heterogeneity among studies. The precision of exposure assessment has become a potential confounding factors in meta-analyses, whose impacts on effect calculation remain unclear. To explore, we conducted a meta-analysis to integrate the long- and short-term exposure effects of PM2.5, NO2, and O3 on all-cause, cardiovascular, and respiratory mortality in the Chinese population. Literature was identified through Web of Science, PubMed, Scopus, and China National Knowledge Infrastructure before August 28, 2023. Sub-group analyses were performed to quantify the impact of exposure assessment precisions and pollution levels on the estimated risk. Studies achieving merely city-level resolution and population exposure are classified as using traditional assessment methods, while those achieving sub-kilometer simulations and individual exposure are considered finer assessment methods. Using finer assessment methods, the RR (under 10 μg/m3 increment, with 95% confidence intervals) for long-term NO2 exposure to all-cause mortality was 1.13 (1.05-1.23), significantly higher (p-value = 0.01) than the traditional assessment result of 1.02 (1.00-1.03). Similar trends were observed for long-term PM2.5 and short-term NO2 exposure. A decrease in short-term PM2.5 levels led to an increase in the RR for all-cause and cardiovascular mortality, from 1.0035 (1.0016-1.0053) and 1.0051 (1.0021-1.0081) to 1.0055 (1.0035-1.0075) and 1.0086 (1.0061-1.0111), with weak between-group significance (p-value = 0.13 and 0.09), respectively. Based on the quantitative analysis and literature information, we summarized four key factors influencing exposure assessment precision under a conceptualized framework: pollution simulation resolution, subject granularity, micro-environment classification, and pollution levels. Our meta-analysis highlighted the urgency to improve pollution simulation resolution, and we provide insights for researchers, policy-makers and the public. By integrating the most up-to-date epidemiological research, our study has the potential to provide systematic evidence and motivation for environmental management.

Abstract:Considering the chemical complexity and toxicity data gaps of environmental mixtures, most studies evaluate the chemical risk individually. However, humans are usually exposed to a cocktail of chemicals in real life. Mixture health assessment remains to be a research area having significant knowledge gaps. Characterization of chemical composition and bioactivity/toxicity are the two critical aspects of mixture health assessments. This review seeks to introduce the recent progress and tools for the chemical and biological characterization of environmental mixtures. The state-of-the-art techniques include the sampling, extraction, rapid detection methods, and the in vitro, in vivo, and in silico approaches to generate the toxicity data of an environmental mixture. Application of these novel methods, or new approach methodologies (NAMs), has increased the throughput of generating chemical and toxicity data for mixtures and thus refined the mixture health assessment. Combined with computational methods, the chemical and biological information would shed light on identifying the bioactive/toxic components in an environmental mixture.

Abstract:The health risks associated with individual air pollutant exposures have been studied and documented, but in real-life, the population is exposed to a multitude of different substances, designated as mixtures. A body of literature on air pollutants indicated that the next step in air pollution research is investigating pollutant mixtures and their potential impacts on health, as a risk assessment of individual air pollutants may actually underestimate the overall risks. This review aims to synthesize the health effects related to air pollutant mixtures containing selected pollutants such as: volatile organic compounds, particulate matter, sulfur and nitrogen oxides. For this review, the PubMed database was used to search for articles published within the last decade, and we included studies assessing the associations between air pollutant mixtures and health effects. The literature search was conducted according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A number of 110 studies were included in the review from which data on pollutant mixtures, health effects, methods used, and primary results were extracted. Our review emphasized that there are a relatively small number of studies addressing the health effects of air pollutants as mixtures and there is a gap in knowledge regarding the health effects associated with these mixtures. Studying the health effects of air pollutant mixtures is challenging due to the complexity of components that mixtures may contain, and the possible interactions these different components may have.

Abstract:Pollutant discharge causing the deterioration of the watershed environment has seriously threatened human health and ecosystem function. The importance of improving the risk warning system is becoming more and more prominent. Traditional chemical risk assessment methods focused on toxicity and the exposure of pollutants without considering the impact of persistent pollutants in different environmental media. In this study, a new approach was proposed to reflect multi-dimensional evaluation with a synthetic risk factor (SRF) of pollutants. The integrating parameters of SRF include toxicity endpoint values, environmental exposure level, persistent properties, and compartment features. Selected pesticides, perfluorinated compounds, organophosphate esters and endocrine disruptors were analyzed by the proposed and traditional methods. The results showed a higher risk outcome using SRF analysis for PFOS, imazalil, testosterone, androstenedione and bisphenol A, which were different from those obtained by the traditional method, which were consistent with existing risk management. The study demonstrated that the SRF method improved the risk assessment of various pollutants in different environmental media in a more robust fashion, and also provided a more accurate decision basis for ecological environment protection.

Abstract:Previous research on respiratory system mortality primarily focused on understanding their combined effects and have neglected the fact that air pollution mixtures are interrelated. This study used Bayesian kernel machine regression (BKMR) to analyze the relationship between air pollutant mixtures and respiratory mortality in Hangzhou, China from 2014 to 2018. The results showed a significant association between pollutant mixtures and respiratory system mortality primarily driven by PM 2.5 and SO 2 . The joint exposure of air pollutants was positively correlated with respiratory system mortality at lag 01 and lag 02 days. The estimated joint effects of log-transformed mixture air pollution exposure on log-transformed respiratory system mortality increased from −0.02 (95% CI: −0.08–0.02) and −0.01 (95% CI: −0.05–0.04) at the 25th percentile to 0.06 (95% CI: 0.01–0.12) and 0.04 (95% CI: −0.001, 0.09) at the 75th percentile. Additionally, there was evidence of an interaction between O 3 and PM 10 . This study confirms that exposure to multiple pollutants is a significant public health problem facing the Hangzhou population given the compounded effect proven with regression analysis, while furthermore, the control of PM 2.5 and SO 2 also represents a serious concern. Implications: Evidence indicates interactions between O 3 and PM 10 . This study demonstrates that exposure to multiple pollutants exerts combined effects on the public health of the Hangzhou population, highlighting the importance of controlling PM 2.5 and SO 2 .

Abstract:Studies suggested that exposure to air pollutants, with endocrine disrupting (ED) properties, have a key role in breast cancer (BC) development. Although the population is exposed simultaneously to a mixture of multiple pollutants and ED pollutants may act via common biological mechanisms leading to synergic effects, epidemiological studies generally evaluate the effect of each pollutant separately. We aimed to assess the complex effect of exposure to a mixture of four xenoestrogen air pollutants (benzo-[a]-pyrene (BaP), cadmium, dioxin (2,3,7,8-Tétrachlorodibenzo-p-dioxin TCDD)), and polychlorinated biphenyl 153 (PCB153)) on the risk of BC, using three recent statistical methods, namely weighted quantile sum (WQS), quantile g-computation (QGC) and Bayesian kernel machine regression (BKMR). The study was conducted on 5,222 cases and 5,222 matched controls nested within the French prospective E3N cohort initiated in 1990. Annual average exposure estimates to the pollutants were assessed using a chemistry transport model, at the participants' residence address between 1990 and 2011. We found a positive association between the WQS index of the joint effect and the risk of overall BC (adjusted odds ratio (OR) = 1.10, 95% confidence intervals (CI): 1.03-1.19). Similar results were found for QGC (OR = 1.11, 95%CI: 1.03-1.19). Despite the association did not reach statistical significance in the BKMR model, we observed an increasing trend between the joint effect of the four pollutants and the risk of BC, when fixing other chemicals at their median concentrations. BaP, cadmium and PCB153 also showed positive trends in the multi-pollutant mixture, while dioxin showed a modest inverse trend. Despite we found a clear evidence of a positive association between the joint exposure to pollutants and BC risk only from WQS and QGC regression, we observed a similar suggestive trend using BKMR. This study makes a major contribution to the understanding of the joint effects of air pollution.

Abstract:Existing studies on the most impactful component remain controversial, hindering the optimization of future air quality standards that concerns particle composition. We aimed to summarize the health risk associated with PM2.5 components and identify those components with the greatest health risk. We performed a meta-analysis to quantify the combined health effects of PM2.5 components, and used the meta-smoothing to produce the pooled concentration-response (C-R) curves. Out of 8,954 initial articles, 80 cohort studies met the inclusion criteria, including a total of 198.08 million population. The pooled C-R curves demonstrated approximately J-shaped association between total mortality and exposure to BC, and NO3−, but U-shaped and inverted U-shaped relationship withSO42− and OC, respectively. In addition, this study found that exposure to various elements, including BC,SO42− NO3−, NH4+, Zn, Ni, and Si, were significantly associated with an increased risk of total mortality, with Ni presenting the largest estimate. And exposure to NO3−, Zn, and Si was positively associated with an increased risk of respiratory mortality, while exposure to BC, SO42−, and NO3− showed a positive association with risk of cardiovascular mortality. For health outcome of morbidity, BC was notably associated with a higher incidence of asthma, type 2 diabetes and stroke. Subgroup analysis revealed a higher susceptibility to PM2.5 components in Asia compared to Europe and North America, and females showed a higher vulnerability. Given the significant health effects of PM2.5 components, governments are advised to introduce them in regional monitoring and air quality control guidelines.Environmental ImplicationPM2.5 is a complex mixture of chemical components from various sources, and each component has unique physicochemical properties and uncertain toxicity, posing significant threat to public health. This study systematically reviewed cohort studies on the association between long-term exposure to 13 PM2.5 components and the risk of morbidity and mortality. And we applied the meta-smoothing approach to establish the pooled concentration-response associations between PM2.5 components and mortality globally. Our findings will provide strong support for PM2.5 components monitoring and the improvement of air quality-related regulations. This will aid in helping to enhance health intervention strategies and mitigating public exposure to detrimental particulate matter.

Abstract:An important goal of environmental health research is to assess the risk posed by mixtures of environmental exposures. Two popular classes of models for mixtures analyses are response-surface methods and exposure-index methods. Response-surface methods estimate high-dimensional surfaces and are thus highly flexible but difficult to interpret. In contrast, exposure-index methods decompose coefficients from a linear model into an overall mixture effect and individual index weights; these models yield easily interpretable effect estimates and efficient inferences when model assumptions hold, but, like most parsimonious models, incur bias when these assumptions do not hold. In this paper we propose a Bayesian multiple index model framework that combines the strengths of each, allowing for non-linear and non-additive relationships between exposure indices and a health outcome, while reducing the dimensionality of the exposure vector and estimating index weights with variable selection. This framework contains response-surface and exposure-index models as special cases, thereby unifying the two analysis strategies. This unification increases the range of models possible for analyzing environmental mixtures and health, allowing one to select an appropriate analysis from a spectrum of models varying in flexibility and interpretability. In an analysis of the association between telomere length and 18 organic pollutants in the National Health and Nutrition Examination Survey (NHANES), the proposed approach fits the data as well as more complex response-surface methods and yields more interpretable results.This article is protected by copyright.
