Statistical methods for chemical mixtures: a roadmap for practitioners

Abstract:Quantitative characterization of the health impacts associated with exposure to chemical mixtures has received considerable attention in current environmental and epidemiological studies. With many existing statistical methods and emerging approaches, it is important for practitioners to understand when each method is best suited for their inferential goals. In this study, we conduct a review and comparison of 11 analytical methods available for use in mixtures research, through extensive simulation studies for continuous and binary outcomes. These methods fall in three different classes: identifying important components of a mixture, identifying interactions and creating a summary score for risk stratification and prediction. We carry out an illustrative data analysis in the PROTECT birth cohort from Puerto Rico. Most importantly we develop an integrated package “CompMix” that provides a platform for mixtures analysis where the practitioner can implement a pipeline for several types of mixtures analysis. Our simulation results suggest that the choice of methods depends on the goal of analysis and there is no clear winner across the board. For selection of important toxicants in the mixture and for identifying interactions, Elastic net by Zou et al. (Enet), Lasso for Hierarchical Interactions by Bien et al (HierNet), Selection of nonlinear interactions by a forward stepwise algorithm by Narisetty et al. (SNIF) have the most stable performance across simulation settings. Additionally, the predictive performance of the Super Learner ensembling method by Van de Laan et al. and HierNet are found to be superior to the rest of the methods. For overall summary or a cumulative measure, we find that using the Super Learner to combine multiple Environmental Risk Scores can lead to improved risk stratification properties. We have developed an R package “CompMix: A comprehensive toolkit for environmental mixtures analysis”, allowing users to implement a variety of tasks under different settings and compare the findings. In summary, our study offers guidelines for selecting appropriate statistical methods for addressing specific scientific questions related to mixtures research. We identify critical gaps where new and better methods are needed.

Abstract:The estimation of the effect of environmental exposures and overall mixtures on a survival time outcome is common in environmental epidemiological studies. While advanced statistical methods are increasingly being used for mixture analyses, their applicability and performance for survival outcomes has yet to be explored. We identified readily available methods for analyzing an environmental mixture's effect on a survival outcome and assessed their performance via simulations replicating various real-life scenarios. Using prespecified criteria, we selected Bayesian Additive Regression Trees (BART), Cox Elastic Net, Cox Proportional Hazards (PH) with and without penalized splines, Gaussian Process Regression (GPR) and Multivariate Adaptive Regression Splines (MARS) to compare the bias and efficiency produced when estimating individual exposure, overall mixture, and interaction effects on a survival outcome. We illustrate the selected methods in a real-world data application. We estimated the effects of arsenic, cadmium, molybdenum, selenium, tungsten, and zinc on incidence of cardiovascular disease in American Indians using data from the Strong Heart Study (SHS). In the simulation study, there was a consistent bias-variance trade off. The more flexible models (BART, GPR and MARS) were found to be most advantageous in the presence of nonproportional hazards, where the Cox models often did not capture the true effects due to their higher bias and lower variance. In the SHS, estimates of the effect of selenium and the overall mixture indicated negative effects, but the magnitudes of the estimated effects varied across methods. In practice, we recommend evaluating if findings are consistent across methods.
Considerations and targeted approaches to identifying bad actors in exposure mixtures

Abstract:Variable importance is a key statistical issue in exposure mixtures, as it allows a ranking of exposures as potential targets for intervention, and helps to identify bad actors within a mixture. In settings where mixtures have many constituents or high between-constituent correlations, estimators of importance can be subject to bias or high variance. Current approaches to assessing variable importance have major limitations, including reliance on overly strong or incorrect constraints or assumptions, excessive model extrapolation, or poor interpretability, especially regarding practical significance. We sought to overcome these limitations by applying an established doubly-robust, machine learning-based approach to estimating variable importance in a mixtures context. This method reduces model extrapolation, appropriately controls confounding, and provides both interpretability and model flexibility. We illustrate its use with an evaluation of the relationship between telomere length, a measure of biologic aging, and exposure to a mixture of polychlorinated biphenyls (PCBs), dioxins, and furans among 979 US adults from the National Health and Nutrition Examination Survey (NHANES). In contrast with standard approaches for mixtures, our approach selected PCB 180 and PCB 194 as important contributors to telomere length. We hypothesize that this difference could be due to residual confounding in standard methods that rely on variable selection. Further empirical evaluation of this method is needed, but it is a promising tool in the search for bad actors within a mixture.

Abstract:Humans are exposed to a large number of environmental chemicals: Some of these may be toxic, and many others have unknown or poorly characterized health effects. There is intense interest in determining the impact of exposure to environmental chemical mixtures on human health. As the study of mixtures continues to evolve in the field of environmental epidemiology, it is imperative that we understand the methodologic challenges of this research and the types of questions we can address using epidemiological data. In this article, we summarize some of the unique challenges in exposure assessment, statistical methods, and methodology that epidemiologists face in addressing chemical mixtures. We propose three broad questions that epidemiological studies can address: a) What are the potential health impacts of individual chemical agents? b) What is the interaction among agents? And c) what are the health effects of cumulative exposure to multiple agents? As the field of mixtures research grows, we can use these three questions as a basis for defining our research questions and for developing methods that will help us better understand the effect of chemical exposures on human disease and well-being.

Abstract:Purpose of This ReviewThis review provides a summary of statistical approaches that researchers can use to study environmental exposure mixtures. Two primary considerations are the form of the research question and the statistical tools best suited to address that question. Because the choice of statistical tools is not rigid, we make recommendations about when each tool may be most useful.Recent FindingsWhen dimensionality is relatively low, some statistical tools yield easily interpretable estimates of effect (e.g., risk ratio, odds ratio) or intervention impacts. When dimensionality increases, it is often necessary to compromise this interpretablity in favor of identifying interesting statistical signals from noise; this requires applying statistical tools that are oriented more heavily towards dimension reduction via shrinkage and/or variable selection.SummaryThe study of complex exposure mixtures has prompted development of novel statistical methods. We suggest that further validation work would aid practicing researchers in choosing among existing and emerging statistical tools for studying exposure mixtures.

Abstract:The ever-increasing number of chemicals and complex mixtures demands a time-saving and cost-effective platform for environmental risk assessment. However, there is limit promising tool for evaluating the contri-bution of each component to the total toxicity effects of the mixture. Here, four widely distributed environmental pollutants with different mode-of-actions, i.e., cadmium chloride (Cd), nitrofurazone (NFZ), triclosan (TCS), and tris(2-chloroethyl) phosphate (TCEP), were selected as components of artificial mixture. Integration of leave-one -out method and high-dimensional live cell array system was used to explore relative contribution of each component from the mixture. A quaternary mixture (All_4_chems) and four ternary mixtures (Leave_Cd, Leav-e_NFZ, Leave_TCS and Leave_TCEP) were investigated by Escherichia coli (E. coli) live cell array system with 90 environmental stress genes modified by green fluorescent protein (GFP) expressing reporter vectors. E. coli cytotoxicity tests demonstrated that TCS has antagonism effect with other three chemicals (Cd, NFZ and TCEP), while it was additive effect in other three binary combinations. A total of 26, 23, 13, 31 and 23 genes were significantly altered with fold-change greater than 2 over the 4 h exposure by All_4_chems, Leave_Cd, Leave_NFZ, Leave_TCS and Leave_TCEP, respectively. Clustering analysis based on time-series gene expression patterns and transcriptional effect level index (TELI) showed that Leave_TCEP has similar profiles with All_4_chems, demonstrating TCEP has the least contribution among four components to the quaternary mixture. Leave_NFZ has the least number of significantly altered genes, implying NFZ has the largest toxicity effect contribution to the quaternary mixture. The relative contribution in different pathways indicated that Cd has the most contri-bution to the mixture in redox stress, while TCS has the least contribution in DNA stress pathway. Collectively, our results demonstrated the utility of high-dimensional toxicogenomics data and leave-one-out method in prioritizing the relative contribution of each component in mixture.

Abstract:Background/Objectives: Over time, the scientific community has developed a growing interest in the effects of mixtures of different compounds, for which there is currently no established evidence or knowledge, in relation to certain categories of xenobiotics. It is well known that exposure to pollutants causes oxidative stress, resulting in the overproduction of reactive oxygen species (ROS), which can affect signaling pathways that regulate the cell cycle, apoptosis, energy balance, and cellular metabolism. The aim of this study was to investigate the effects of sub-lethal concentrations of mixtures of emerging pollutants and pharmaceuticals on the modulation of biomarkers related to toxicity, oxidative stress, and cancer. Methods: In this study, the hepatoma cell line HepG2 was exposed to increasing concentrations of polybrominated diphenyl ether 47 (BDE-47), cadmium chloride (CdCl2), and carbamazepine (CBZ), both individually and in mixtures, for 72 h to assess cytotoxicity using the MTT assay. The subsequent step, following the identification of the sub-lethal concentration, was to investigate the effects of exposure at the gene expression level, through the evaluation of molecular markers related to cell cycle and apoptosis (p53), oxidative stress (NRF2), conjugation and detoxification of xenobiotics (CYP2C9 and GST), DNA damage (RAD51 and γH2AFX), and SUMOylation processes (SUMO1 and UBC9) in order to identify any potential alterations in pathways that are normally activated at the cellular level. Results: The results showed that contaminants tend to affect the enzymatic detoxification and antioxidant system, influencing DNA repair defense mechanisms involved in resistance to oxidative stress. The combined effect of the compounds at sub-lethal doses results in a greater activation of these pathways compared to exposure to each compound alone, thereby exacerbating their cytotoxicity. Conclusions: The biomarkers analyzed could contribute to the definition of early warning markers useful for environmental monitoring, while simultaneously providing insight into the toxicity and hazard levels of these substances in the environment and associated health risks.

A Comparison of Statistical Methods for Studying Interactions of Chemical Mixtures

Abstract:Properly assessing the effects of environmental chemical exposures on disease risk remains a challenging problem in environmental epidemiology. Various analytic approaches have been proposed, but there are few papers that have compared the performance of different statistical methods on a single dataset. In this paper, we compare different regression-based approaches for estimating interactions between chemical mixture components using data from a case-control study on non-Hodgkin's lymphoma. An analytic challenge is the high percentage of exposures that are below the limit of detection (LOD). Using imputation for LOD, we compare different Bayesian shrinkage prior approaches including an approach that incorporates the hierarchical principle where interactions are only included when main effects exist. Further, we develop an approach where main and interactive effects are represented by a series of distinct latent functions. We also fit the Bayesian kernel machine regression to these data. All of these approaches show little evidence of an interaction among the chemical mixtures when measurements below the LOD were imputed. The imputation approach makes very strong assumptions about the relationship between exposure and disease risk for measurements below the LOD. As an alternative, we show the results of an analysis where we model the exposure relationship with two parameters per mixture component; one characterizing the effect of being below the LOD and the other being a linear effect above the LOD. In this later analysis, we identify numerous strong interactions that were not identified in the analyses with imputation. This case study demonstrated the importance of developing new approaches for mixtures when the proportions of exposure measurements below the LOD are high.
From Cohort to Cohort: A Similar Mixture Approach (SMACH) to Evaluate Exposures to a Mixture Leading to Thyroid-Mediated Neurodevelopmental Effects Using NHANES Data

Abstract:Prenatal exposure to a mixture (MIX N) of eight endocrine-disrupting chemicals has been associated with language delay in children in a Swedish pregnancy cohort. A novel approach was proposed linking this epidemiological association with experimental evidence, where the effect of MIX N on thyroid hormone signaling was assessed using the Xenopus eleuthero-embryonic thyroid assay (XETA OECD TG248). From this experimental data, a point of departure (PoD) was derived based on OECD guidance. Our aim in the current study was to use updated toxicokinetic models to compare exposures of women of reproductive age in the US population to MIX N using a Similar Mixture Approach (SMACH). Based on our findings, 66% of women of reproductive age in the US (roughly 38 million women) had exposures sufficiently similar to MIX N. For this subset, a Similar Mixture Risk Index (SMRIHI) was calculated comparing their exposures to the PoD. Women with SMRIHI > 1 represent 1.1 million women of reproductive age. Older women, Mexican American and other/multi race women were less likely to have high SMRIHI values compared to Non-Hispanic White women. These findings indicate that a reference mixture of chemicals identified in a Swedish cohort-and tested in an experimental model for establishment of (PoDs)-is also of health relevance in a US population.
Associations of prenatal exposure to a mixture of persistent organic pollutants with social traits and cognitive and adaptive function in early childhood: Findings from the EARLI study

Abstract:Background: Literature suggests that maternal exposure to persistent organic pollutants (POPs) may influence child neurodevelopment. Evidence linking prenatal POPs and autism spectrum disorder has been inconclusive and few studies have examined the mixture effect of the POPs on autism-related traits. Objective: To evaluate the associations between prenatal exposure to a mixture of POPs and autism-related traits in children from the Early Autism Risk Longitudinal Investigation study. Methods: Maternal serum concentrations of 17 POPs (11 polychlorinated biphenyls [PCBs], 4 polybrominated diphenyls [PBDEs], and 2 persistent pesticides) in 154 samples collected during pregnancy were included in this analysis. We examined the independent associations of the natural log-transformed POPs with social, cognitive, and behavioral traits at 36 months of age, including Social Responsiveness Scale (SRS), Mullen Scales of Early Learning-Early Learning Composite (MSEL-ELC), and Vineland Adaptive Behavior Scales (VABS) scores, using linear regression models. We applied Bayesian kernel machine regression and quantile g-computation to examine the joint effect and interactions of the POPs. Results: Higher ln-PBDE47 was associated with greater deficits in social reciprocity (higher SRS score) (β = 6.39, 95% CI: 1.12, 11.65) whereas higher ln-p,p'-DDE was associated with lower social deficits (β = -8.34, 95% CI: -15.32, -1.37). Positive associations were observed between PCB180 and PCB187 and cognitive (MSEL-ELC) scores (β = 5.68, 95% CI: 0.18, 11.17; β = 4.65, 95% CI: 0.14, 9.17, respectively). Adaptive functioning (VABS) scores were positively associated with PCB170, PCB180, PCB187, PCB196/203, and p,p'-DDE. In the mixture analyses, we did not observe an overall mixture effect of POPs on the quantitative traits. Potential interactions between PBDE99 and other PBDEs were identified in association with MSEL-ELC scores. Conclusions: We observed independent effects of PCB180, PCB187, PBDE47, and p,p' DDE with ASD-related quantitative traits and potential interactions between PBDEs. Our findings highlight the importance of assessing the effect of POPs as a mixture.

Abstract:Abstract There is growing interest in discovering interactions between multiple environmental chemicals associated with increased adverse health effects. However, most existing approaches (1) either use a projection or product of multiple chemical exposures, which are difficult to interpret and (2) cannot simultaneously handle multi-ordered interactions. Therefore, we develop and validate a method to discover shape-based interactions that mimic usual toxicological interactions. We developed the Multi-ordered explanatory interaction (Moxie) algorithm by merging the efficacy of Extreme Gradient Boosting with the inferential power of Weighted Quantile Sum regression to extract synergistic interactions associated with the outcome/odds of disease in an adverse direction. We evaluated the algorithm’s performance through simulations and compared it with the currently available gold standard, the signed-iterative random forest algorithm. We used the 2017–18 US-NHANES dataset ( n = 447 adults) to evaluate interactions among nine per- and poly-fluoroalkyl substances and five metals measured in whole blood in association with serum low-density lipoprotein cholesterol. In simulations, the Moxie algorithm was highly specific and sensitive and had very low false discovery rates in detecting true synergistic interactions of 2nd, 3rd, and 4th order through moderate ( n = 250) to large ( n = 1000) sample sizes. In NHANES data, we found a two-order synergistic interaction between cadmium and lead detected in people with whole-blood cadmium concentrations and lead above 0.605 ug/dL and 1.485 ug/dL, respectively. Our findings demonstrate a novel validated approach in environmental epidemiology for detecting shape-based toxicologically mimicking interactions by integrating exposure-mixture regression and machine learning methods.
Personalized mixture toxicity testing: A proof-of-principle in vitro study to evaluate the steroidogenic effects of reconstructed contaminant mixtures measured in blood of individual adults

Abstract:Chemical risk assessments generally focus on single substances, overlooking real-world co-exposures to chemical mixtures. Mixture toxicology studies with representative mixtures can reveal potential chemical interactions, but does not account for the unique chemical profiles that occur in the blood of diverse individuals. Here we used the H295R steroidogenesis assay to screen personalized mixtures of 24 persistent organic pollutants (POPs) for cytotoxicity and endocrine disruption. Each mixture was reconstructed at a human exposure relevant concentration (1 × ), as well as at 10- and 100-fold higher concentration (10×, 100 × ) by acoustic liquid handling based on measured blood concentrations in a Swedish cohort. Among the twelve mixtures tested, nine mixtures decreased the cell viability by 4–18 %, primarily at the highest concentration. While the median and maximum mixtures based on the whole study population induced no measurable effects on steroidogenesis at any concentration, the personalized mixture from an individual with the lowest total POPs concentrations was the only mixture that affected estradiol synthesis (35 % increase at the 100 × concentration). Mixtures reconstructed from blood levels of three different individuals stimulated testosterone synthesis at the 1× (11–15 %) and 10 × concentrations (12–16 %), but not at the 100 × concentration. This proof-of-principle personalized toxicity study illustrates that population-based representative chemical mixtures may not adequately account for the toxicological risks posed to individuals. It highlights the importance of testing a range of real-world mixtures at relevant concentrations to explore potential interactions and non-monotonic effects. Further toxicological studies of personalized contaminant mixtures could improve chemical risk assessment and advance the understanding of human health as chemical exposome data become increasingly available.

Abstract:Exposure to polychlorinated biphenyls (PCBs) remains a potential human health risk due to their persistence in the environment, despite a global ban on their production. Understanding the composition of PCB mixtures is essential for the application of a mixtures-based approach to assessing health risks of PCB exposure. This work represents the most extensive effort to date to compile and make publicly available the PCB congener profiles for mixtures with toxicological data, providing a foundation for understanding toxicological potency of PCB mixtures in the environment. We searched for published congener profiles across 29 commercial and simulated environmental PCB mixtures, including various Aroclors, Phenoclors, Clophens, and Kanechlors, among others. A total of 117 references containing 401 distinct complete or partial tabularized profiles were found. Aroclor 1254 had the most published profiles, with 79 unique datasets characterizing multiple mixture lots. In contrast, no congener-specific composition data were identified for Fenclors, Clophen C, or Pyralenes. Eighty-seven of the most complete and clearly reported profiles underwent a detailed extraction of the congener data, PCB mixture source, and analytical methods. Challenges encountered during data extraction included congener coelutions, incomplete methods reporting, and inconsistencies in PCB nomenclature. These factors complicate data visualization, comparisons across datasets, and use of the data in subsequent analyses. Where possible, we have converted profiles to the same units and congener numbering convention to allow for easier comparison. The extracted data are publicly available online as interactive visuals and as a downloadable Microsoft Excel® workbook. This dataset provides researchers with an overview of the current PCB mixture profile landscape that can serve as a tool to support efforts to minimize the health impacts of environmental PCB exposure, including the exploration of links between mixture composition and toxicity and the identification of the most efficient and effective remediation strategies at contaminated sites.

Assessing Metabolic Differences Associated with Exposure to Polybrominated Biphenyl and Polychlorinated Biphenyls in the Michigan PBB Registry

Abstract:Background: Polybrominated biphenyls (PBB) and polychlorinated biphenyls (PCB) are persistent organic pollutants with potential endocrine-disrupting effects linked to adverse health outcomes. Objectives: In this study, we utilize high-resolution metabolomics (HRM) to identify internal exposure and biological responses underlying PCB and multigenerational PBB exposure for participants enrolled in the Michigan PBB Registry. Methods: HRM profiling was conducted on plasma samples collected from 2013 to 2014 from a subset of participants enrolled in the Michigan PBB Registry, including 369 directly exposed individuals (F0) who were alive when PBB mixtures were accidentally introduced into the food chain and 129 participants exposed to PBB in utero or through breastfeeding, if applicable (F1). Metabolome-wide association studies were performed for PBB-153 separately for each generation and ΣPCB (PCB-118, PCB-138, PCB-153, and PCB-180) in the two generations combined, as both had direct PCB exposure. Metabolite and metabolic pathway alterations were evaluated following a well-established untargeted HRM workflow. Results: Mean levels were 1.75 ng/mL [standard deviation (SD): 13.9] for PBB-153 and 1.04 ng/mL (SD: 0.788) for ΣPCB. Sixty-two and 26 metabolic features were significantly associated with PBB-153 in F0 and F1 [false discovery rate (FDR) p<0.2], respectively. There were 2,861 features associated with ΣPCB (FDR p<0.2). Metabolic pathway enrichment analysis using a bioinformatics tool revealed perturbations associated with ΣPCB in numerous oxidative stress and inflammation pathways (e.g., carnitine shuttle, glycosphingolipid, and vitamin B9 metabolism). Metabolic perturbations associated with PBB-153 in F0 were related to oxidative stress (e.g., pentose phosphate and vitamin C metabolism) and in F1 were related to energy production (e.g., pyrimidine, amino sugars, and lysine metabolism). Using authentic chemical standards, we confirmed the chemical identity of 29 metabolites associated with ΣPCB levels (level 1 evidence). Conclusions: Our results demonstrate that serum PBB-153 is associated with alterations in inflammation and oxidative stress-related pathways, which differed when stratified by generation. We also found that ΣPCB was associated with the downregulation of important neurotransmitters, serotonin, and 4-aminobutanoate. Abstract:Identifying the impact of environmental mixtures on human health is an important topic. However, such studies face challenges when exposure measurements lie below limit of detection (LOD). While various approaches for accommodating a single exposure subject to LOD have been used, their impact on mixture analysis has not been thoroughly investigated. Our study aims to understand the impact of five popular LOD accommodation approaches on mixture analysis results with multiple exposures subject to LOD, including omitting subjects with any exposures below LOD (complete case analysis); single imputations by LOD/ , and by estimates from a censored accelerated failure time (AFT) model; and multiple imputation (MI) with or without truncation based on LOD.

Associations of serum persistent organic pollutant concentrations with incident diabetes in midlife women: The Study of Women's Health Across the Nation Multi-Pollutant Study

Abstract:Background: Organochlorine pesticides (OCPs), polybrominated diphenyl ethers (PBDEs), and polychlorinated biphenyls (PCBs) are persistent organic pollutants (POPs) that can negatively impact metabolic health through pathways including endocrine disruption. Few studies have evaluated diabetes risk associated with PBDEs. Little is known about the joint effect of exposure to POP mixtures on diabetes risk. Objectives: We investigated the relationship between POPs, individually and as mixtures, and diabetes development over 18 years (1999-2016) in midlife women. Methods: We measured lipid-standardized serum concentrations of 34 PCBs, 19 OCPs, and 14 PBDEs in 1040 midlife women aged 45-56 years from the Study of Women's Health Across the Nation. We tested the association between POPs measured in 1999/2000 and incident diabetes using Cox proportional hazards models. We evaluated diabetes risk associated with the overall POP mixture using Quantile-Based G-Computation (QBGC). Results: For most mixture components, single pollutant and mixtures analyses indicated null associations with diabetes risk, however results were inconsistent. After adjustment, hazard ratios (HRs) of developing diabetes (95% CI) associated with upper exposure tertiles (T2/T3) compared with the first tertile (T1), were 1.7 (1.0, 2.8) at T2 and 1.5 (0.84, 2.7) at T3 for hexachlorobenzene and 1.9 (1.1, 3.3) at T2 and 1.6 (0.88, 2.9) at T3 for PCB 123. A doubling of PBDE 47 was associated with 1.11 (1.00, 1.24) times the risk of T2D. QBGC identified no association for the overall joint effect of the POP mixture on diabetes (HR = 1.04 [0.53, 2.07]). Conclusion: Exposure to a mixture of PCBs, OCPs, and PBDEs was not associated with incident diabetes in midlife U.S. women, although some individual POPs demonstrated significant yet inconsistent associations with diabetes. Non-linear and non-monotonic dose-response dynamics deserve further exploration. More research is needed on the diabetogenic effects of PBDEs.
