Abstract:Abstract Background Augmented Reality (AR) is a rapidly emerging technology finding growing acceptance and application in different fields of surgery. Various studies have been performed evaluating the precision and accuracy of AR guided navigation. This study investigates the feasibility of a commercially available AR head mounted device during orthopedic surgery. Methods Thirteen orthopedic surgeons from a Swiss university clinic performed 25 orthopedic surgical procedures wearing a holographic AR headset (HoloLens, Microsoft, Redmond, WA, USA) providing complementary three-dimensional, patient specific anatomic information. The surgeon’s experience of using the device during surgery was recorded using a standardized 58-item questionnaire grading different aspects on a 100-point scale with anchor statements. Results Surgeons were generally satisfied with image quality (85 ± 17 points) and accuracy of the virtual objects (84 ± 19 point). Wearing the AR device was rated as fairly comfortable (79 ± 13 points). Functionality of voice commands (68 ± 20 points) and gestures (66 ± 20 points) provided less favorable results. The greatest potential in the use of the AR device was found for surgical correction of deformities (87 ± 15 points). Overall, surgeons were satisfied with the application of this novel technology (78 ± 20 points) and future access to it was demanded (75 ± 22 points). Conclusion AR is a rapidly evolving technology with large potential in different surgical settings, offering the opportunity to provide a compact, low cost alternative requiring a minimum of infrastructure compared to conventional navigation systems. While surgeons where generally satisfied with image quality of the here tested head mounted AR device, some technical and ergonomic shortcomings were pointed out. This study serves as a proof of concept for the use of an AR head mounted device in a real-world sterile setting in orthopedic surgery.

Abstract:Objective: To compare the accuracy of portable monitoring for obstructive sleep apnea (OSA) with polysomnography (PSG) in commercial drivers. Methods: A consecutive case series of drivers were screened for OSA using recent validated consensus conference criteria at a single occupational medicine clinic; those screening positive were tested with a portable OSA screening device while awaiting PSG. Results: Three hundred forty-six drivers were screened and 34 completed PSG and portable monitoring. Comparing the portable device to PSG at a definition of OSA as an apnea hypopnea index (AHI) ≥15, positive predictive value is 0.64 with a 0.87 negative predictive value. Positive likelihood ratio is 4.20. Linear regression model found statistically significant relationship (P = 0.0004) between the portable device and PSG AHI values. Conclusion: Although not perfect, portable monitoring may be useful in further stratifying risk in drivers who have a high probability of having OSA.

Abstract:Surgeons must accomplish complex technical and intellectual tasks that can generate unexpected and serious challenges with little or no room for error. In the last decade, computer simulations have played an increasing role in surgical training, pre-operative planning, and biomedical research. Specifically, visuo-haptic simulations have been the focus of research to develop advanced e-Learning systems facilitating surgical training. The cost of haptic hardware was reduced through mass scale production and as haptics gained popularity in the gaming industry. Visuo-haptic simulations combine the tactile sense with visual information and provide training scenarios with a high degree of reality. For surgical training, such scenarios can be used as ways to gain, improve, and assess resident and expert surgeons' skills and knowledge.

Abstract:A 22-year-old man with non-Hodgkin's lymphoma (B-cell lymphoblastic lymphoma, Stage IVA) received chemotherapy and radiation therapy and achieved complete remission. He was admitted for allogeneic bone marrow transplantation (BMT) using a graft from his completely HLA-matched mother. Although he had HBV infection, allogeneic BMT was performed because he still had normal liver function and strongly requested the procedure. He developed both acute and chronic GVHD after the procedure, but showed no liver damage related to HBV. Treatment with lamivudine (150 mg/day) was started because the HBV-DNA level increased gradually after allogeneic BMT. Although the HBV-DNA then decreased gradually and there was no evidence of severe liver damage, the patient died following relapse of NHL. It seems that in this case, treatment of HBV with lamivudine may have prevented serious liver damage after allogeneic BMT. Therefore, allogeneic BMT may be done safely in patients with HBV infection if lamivudine is administered.

Abstract:Surgical education has traditionally relied on cadaveric dissection and supervised training in the operating theatre. However, both these forms of training have become inefficient due to issues such as scarcity of cadavers and competing priorities taking up surgeons time. Within this context, computer-based simulations such as virtual reality have gained popularity as supplemental modes of training. Virtual reality simulation offers repeated practice in a riskfree environment where standardised surgical training modules can be developed, along with systems to provide automated guidance and assessment. In this paper, we discuss the design and evaluation of such a training module, specifically aimed at training an advanced temporal bone procedure, namelycochlear implant surgery.

Abstract:Purpose: The data which is available to surgeons before, during and after surgery is steadily increasing in quantity as well as diversity. When planning a patient's treatment, this large amount of information can be difficult to interpret. To aid in processing the information, new methods need to be found to present multi-modal patient data, ideally combining textual, imagery, temporal and 3D data in a holistic and context-aware system. Methods: We present an open-source framework which allows handling of patient data in a virtual reality (VR) environment. By using VR technology, the workspace available to the surgeon is maximized and 3D patient data is rendered in stereo, which increases depth perception. The framework organizes the data into workspaces and contains tools which allow users to control, manipulate and enhance the data. Due to the framework's modular design, it can easily be adapted and extended for various clinical applications. Results: The framework was evaluated by clinical personnel (77 participants). The majority of the group stated that a complex surgical situation is easier to comprehend by using the framework, and that it is very well suited for education. Furthermore, the application to various clinical scenarios - including the simulation of excitation-propagation in the human atrium - demonstrated the framework's adaptability. As a feasibility study, the framework was used during the planning phase of the surgical removal of a large central carcinoma from a patient's liver. Conclusion: The clinical evaluation showed a large potential and high acceptance for the VR environment in a medical context. The various applications confirmed that the framework is easily extended and can be used in real-time simulation as well as for the manipulation of complex anatomical structures.

Abstract:Abstract Virtual reality (VR) may be a good alternative for cadaveric temporal bone surgical dissection courses, which are an important part of otolaryngology resident’s training. The aim of the study was to assess the VR temporal bone surgery simulator in an antromastoidectomy simulation. The VR system was based on the Geomagic Touch Haptic Device from 3D System. The research was designed as a prospective study, with three sessions of VR simulation training. The group of four ENT specialists unexperienced in otosurgery and 11 otorhinolaryngology residents performed a series of virtual dissections of a VR temporal bone model. Two experts with a broad experience in ear surgery participated in the study as supervisors for all the participants. At the end of each session, the experts controlled the accuracy of the simulated surgery performance assigning positive points for each correctly performed step and negative points for each mistake. After each session, participants of the study were asked to fill in the questionnaire concerning their impression of a VR system simulation. The evaluation of every simulation (total score) was based on the duration of a VR session, the quality of performance (positive points) and the number of mistakes (negative points). During consecutive VR sessions, evident shortening of the length of performance, as well as an improvement in the quality of performance and reduction in mistakes, was observed. Sixty percent of study participants answered that signaling damage to the critical elements was good (40%—sufficient), and 67% assessed that they had made a progress in consecutive sessions. After three sessions, 100% of participants indicated higher self-confidence in relation to their own surgical skills. Also, all the participants indicated that VR training should be included in a routine educational program for medical students. VR training provides a structured, safe and supportive environment to familiarize oneself with complex anatomy and practical skills.

Abstract:Nowadays, robot assisted surgery training relies more and more on computer-based simulation. However, the application of such training technologies is still limited to the early stages of practical training. To broaden the usefulness of simulators, multi-sensory feedback augmentation has been recently investigated. This study aims at combining initial predictive (guidance) and subsequent error-based (feedback) training augmentation in the visual and haptic domain. 32 participants performed 30 repetitions of a virtual reality task resembling needle-driving by using the surgeon console of the da Vinci Research Kit. These trainees were randomly and equally divided into four groups: one group had no training augmentation, while the other groups underwent visual, haptic and visuo-haptic augmentation, respectively. Results showed a significant improvement, initially introduced by guidance, in the task completion capabilities of all the experimental groups against control. In terms of accuracy, the experimental groups outperformed the control group at the end of training. Specifically, visual guidance and haptic feedback played a significant role in error reduction. Further investigations on long term learning could better delineate the optimal combination of guidance and feedback in these sensory domains.

Abstract:Recent advances in haptic hardware and software technology have generated interest in novel, multimodal interfaces based on the sense of touch. Such interfaces have the potential to revolutionize the way we think about human computer interaction and open new possibilities for simulation and training in a variety of fields. In this paper we review several frameworks, APIs and toolkits for haptic user interface development. We explore these software components focusing on minimally invasive surgical simulation systems. In the area of medical diagnosis, there is a strong need to determine mechanical properties of biological tissue for both histological and pathological considerations. Therefore we focus on the development of affordable visuo-haptic simulators to improve practice-based education in this area. We envision such systems, designed for the next generations of learners that enhance their knowledge in connection with real-life situations while they train in mandatory safety conditions.

Abstract:Objective In contrast to the rising amount of financial investments for research and development in medical technology worldwide is the lack of usability and clinical readiness of the produced systems. We evaluated an augmented reality (AR) setup under development for preoperative perforator vessel mapping for elective autologous breast reconstruction. Methods In this grant-supported research pilot, we used magnetic resonance angiography data (MR-A) of the trunk to superimpose the scans on the corresponding patients with hands-free AR goggles to identify regions-of-interest for surgical planning. Perforator location was assessed using MR-A imaging (MR-A projection) and Doppler ultrasound data (3D distance) and confirmed intraoperatively in all cases. We evaluated usability (System Usability Scale, SUS), data transfer load and documented personnel hours for software development, correlation of image data, as well as processing duration to clinical readiness (time from MR-A to AR projections per scan). Results All perforator locations were confirmed intraoperatively, and we found a strong correlation between MR-A projection and 3D distance measurements (Spearman r = 0.894). The overall usability (SUS) was 67 ± 10 (=moderate to good). The presented setup for AR projections took 173 min to clinical readiness (=availability on AR device per patient). Conclusion In this pilot, we calculated development investments based on project-approved grant-funded personnel hours with a moderate to good usability outcome resulting from some limitations: assessment was based on one-time testing with no previous training, a time lag of AR visualizations on the body and difficulties in spatial AR orientation. The use of AR systems can provide new opportunities for future surgical planning, but has more potential for educational (e.g., patient information) or training purposes of medical under- and postgraduates (spatial recognition of imaging data associated with anatomical structures and operative planning). We expect future usability improvements with refined user interfaces, faster AR hardware and artificial intelligence-enhanced visualization techniques.

Abstract:Virtual reality (VR) is being increasingly used in medical education. However, investigations into its effectiveness in this field have yielded mixed results, indicating that no general conclusions can be drawn in this regard. Therefore, the suitability of a system to teach a given task has to be investigated on a case-by-case basis. In this paper, we focus on teaching clinically oriented surgical ear anatomy, as it is important to provide a well-rounded view of ear anatomy as a foundation for understanding clinical surgery. Although there are some VR systems that teach basic ear anatomy, for example, identification of structures and spatial relationships, to our knowledge, those that deliver a complete clinical understanding of ear anatomy do not yet exist. As such, we discuss the design and development of a 3D interactive VR tutor with integrated haptic capability to teach clinically oriented surgical anatomy of the ear and establish its effectiveness through a user study.
Modifications of ambulation-increasing effect of morphine and buprenorphine following repeated administration in mice.

Abstract:Interaction of behavioral effects of morphine (10 and 20 mg/kg s. c.) and buprenorphine (0.3 and 1 mg/kg s. c.) were investigated by means of ambulatory activity following repeated administration of either drug in mice. The repeated administration of buprenorphine (1 mg/kg, but not 0.3 mg/kg) total of 5 times at intervals of 3-4 days induced a gradual decrease in the sensitivity to its ambulation-increasing effect. The buprenorphine-experienced mice showed an increase in the sensitivity to the ambulation-increasing effect of morphine (20 mg/kg) in parallel with the doses of buprenorphine. On the other hand, the ambulation-increasing effect of morphine (10 and 20 mg/kg) progressively enhanced during the repeated administration schedule. The morphine-experienced mice demonstrated a decrease in the sensitivity to buprenorphine (1 mg/kg). The present results may reflect differential characteristics between morphine and buprenorphine in their ambulation-increasing effects in mice.
Immersive reality for robotic surgical training: a pilot study using 3D visors for immersive view of the operating field

Abstract:Immersive intracorporeal vision is a key feature of robotic surgery, limited today to only one trainee per operation when the dual console is available. We developed a tool that provides a virtually unlimited number of surgeons with the operator's view, with the possibility to also watch the surgeon's hand movements and the operating table. In this study, we aim to assess trainees' reaction to this innovative training method. Medical students and surgery residents were offered an immersive experience with head-mounted devices, showing a didactic video in a 360° virtual space with 3D intracorporeal robotic vision, the surgeon's hand movements and the surrounding operating room during a robotic rectal resection with total mesorectal excision. Subsequently, participants were asked to fill a questionnaire evaluating the user's reaction to the new training tool including the validated System Usability Scale (SUS) and Simulator Sickness Questionnaire (SSQ), and non-validated questions. 102 participants took part in the training and the assessment questionnaires, 94 (92%) medical students and 8 (8%) surgery residents. Users' feedback was overall positive. In the engagement and intention to use items, almost 90% of the respondents voted for a complete or near complete agreement. The median SUS score was 80 [IQR 70-90]. The median SSQ score was 44.88 [IQR 22.44-82.28]. Exposing trainees to immersive robotic vision of the surgical field had a positive reaction from our audience. Our initial results encourage further implementing this technology in surgical training of medical students and residents to prove its efficacy.

Abstract:Tactile perception plays an important role in medical simulation and training, specifically in surgery. The surgeon must feel organic tissue hardness, evaluate anatomical structures, measure tissue properties, and apply appropriate force control actions for safe tissue manipulation. Development of novel cost effective haptic-based simulators and their introduction in the minimally invasive surgery learning cycle can absorb the learning curve for residents. Receiving pre-training in a core set of surgical skills can reduce skill acquisition time and risks. We present the development of a cost-effective visuo-haptic simulator for the liver tissue, designed to improve practice-based education in minimally invasive surgery. Such systems can positively affect the next generations of learners by enhancing their knowledge in connection with real-life situations while they train in mandatory safe conditions.

Abstract:Abstract We present a summary of the development and clinical use of two custom designed high-fidelity virtual-reality simulator training platforms. This simulator development program began in 2016 to support the phase III clinical trial Archway (ClinicalTrials.gov identifier, NCT03677934) intended to evaluate the Port Delivery System (PDS) developed by Genentech Inc. and has also been used to support additional clinical trials. The two simulators address two specific ophthalmic surgical procedures required for the successful use of PDS and provide state-of-the-art physical simulation models and graphics. The simulators incorporate customized active haptic feedback input devices that approximate different hand pieces including a custom hand piece specifically designed for PDS implantation. We further describe the specific challenges of the procedure and the development of corresponding training strategies realized within the simulation platform.

Abstract:Medical surgery involves a high degree of skill and experience, making the learning curve for medical trainees quite long. For instance, in eye cataract surgery, despite it only taking around seven minutes for a well-trained surgeon to perform and having a success rate of 99 percent, medical residents need months to become proficient in this procedure to avoid its typical complications. Medical trainees traditionally have acquired surgical skills through apprenticeships in which trainees observe senior surgeons, then perform under guidance until they achieve mastery. Training often makes use of cadavers or laboratory animals, but this type of training is becoming increasingly difficult to do in many countries due to ethical reasons. An effective alternative is medical simulation, which can enhance understanding, improve performance, and assess competence; in preoperative settings, it assists surgeons in remaining at a high technical skill level. Surgical simulation can provide high-fidelity training that increases the diffusion of innovative and less- invasive procedures while decreasing the surgeon's learning curve.

Abstract:This study aims to develop a robotic system human-machine interaction (HMI) to facilitate surgical training through visual and kinaesthetic feedbacks. This is motivated by the pressing need for effective surgical training and the unaddressed gaps in existing surgical training simulator for minimally invasive procedures. This study establishes the design concept and scope for development to facilitate the required HMI training model. Subsequently, implementation and demonstration of the model is carried out with analytical experiments to assess the feasibility of the proposed concept. The design concept of the robotic system for training is demonstrated through an user experiment. Results suggest viability and observable benefits in the authors' proposed kinaesthetic HMI guidance for the trainee. Potential impact of this study includes the development of a novel training paradigm that engages trainees through collaborative training facilitated by human trainers and active kinaesthetic simulation. Although motivated by surgical training applications, the concept developed in this study can potentially be extended for general motor skill learning.

Abstract:Augmented reality technologies provide transformative solutions in various surgical fields. Our research focuses on the use of an advanced augmented reality system that projects 3D holographic images directly into surgical footage, potentially improving the surgeon's orientation to the surgical field and lowering the cognitive load. We created a novel system that combines exoscopic surgical footage from the "ORBEYE" and displays both the surgical field and 3D holograms on a single screen. This setup enables surgeons to use the system without using head-mounted displays, instead viewing the integrated images on a 3D monitor. Thirteen surgeons and surgical assistants completed tasks with 2D and 3D graphical surgical guides. The NASA Task Load Index was used to assess mental, physical, and temporal demands. The use of 3D graphical surgical guides significantly improved performance metrics in cochlear implant surgeries by lowering mental, physical, temporal, and frustration levels. However, for Bonebridge implantation, the 2D graphical surgical guide performed better overall (p = 0.045). Participants found the augmented reality system's video latency to be imperceptible, measuring 0.13 ± 0.01 s. This advanced augmented reality system significantly improves the efficiency and precision of cochlear implant surgeries by lowering cognitive load and improving spatial orientation.

Abstract:Learning surgical skills require critical visual-spatial motor skills. Current learning methods employ costly and limited in-person teaching in addition to supplementation by videos, textbooks, and cadaveric labs. Increasingly limited healthcare resources and in-person training has led to growing concerns for skills acquisition of trainees. Recent Mixed Reality (MR) devices offer an attractive solution to these resource barriers by providing three-dimensional holographic representations of reality that mimic in-person experiences in a portable, individualized, and cost-effective form. We developed and evaluated two holographic MR models to explore the feasibility of visual-spatial motor skill acquisition from a technical development, learning, and usability perspective. In our first, a pair of holographic hands were created and projected in front of the trainee, and participants were evaluated on their ability to learn complex hand motions in comparison to traditional methods of video and apprenticeship-based learning. The second model displayed a 3D holographic model of the middle and inner ear with labeled anatomical structures which users could explore and user experience feedback was obtained. Our studies demonstrated that scores between MR and apprenticeship learning were comparable. All felt MR was an effective learning tool and most noted that the MR models were better than existing didactic methods of learning. Identified advantages of MR included the ability to provide true 3D spatial representation, improved visualization of smaller structures in detail by upscaling the models, and improved interactivity. Our results demonstrate that holographic learning is able to mimic in-person learning for visual-spatial motor skills and could be a new effective form of self-directed apprenticeship learning.
