Abstract:Objective:To observe the effects of transurethral split of the prostate (TUSP) on the urodynamics, the sexual function and the quality of life of benign prostatic hyperplasia.Methods:Eighty patients with benign prostatic hyperplasia admitted to the Deqing People′s Hospital of Zhejiang Province from January 2017 to June 2019 were enrolled into the research objects prospectively. According to the random digits table method, they were divided into group A and group B, with 40 cases in each group. The group A was treated with TUSP, while the group B was treated with bipolar transurethral plasma kinetic prostatectomy (TUPKP). The operation time, intraoperative blood loss, bladder irrigation time, urinary catheter indwelling time, hospitalization time of the two groups were compared and surgical effects were evaluated. The changes of hemoglobin (Hb), serum sodium (Na), post-void residual volume (PVR), maximum urinary flow rate (Q max), international prostate symptom score (IPSS) and international index of erectile function score (IIEF-5), the quality of life index (QOL) of 2 groups before and after the operation were measured. Surgical complications were recorded. Results:The operation time, bladder irrigation time, urinary catheter indwelling time and hospital stay in group A were all shorter than that in group B: (15.63 ± 4.17) min vs. (58.79 ± 10.45) min, (6.26 ± 1.17) h vs. (45.51 ± 10.03) h, (3.07 ± 0.68) d vs. (5.67 ± 1.51) d, (3.63 ± 0.43) d vs. (6.08 ± 1.72) d, (18.32 ± 2.79) ml vs. (65.26 ± 20.64) ml, and there were statistical differences ( P<0.05); there were no statistical difference between 2 groups in surgical curative effect grade and Na 1 d after operation ( P>0.05); 1 day after operation, Hb in group A was higher than that in group B: (115.63 ± 9.78) g/L vs. (109.65 ± 8.36) g/L, and there was statistical difference ( P<0.05); PVR, Q max, IPSS, IIEF-5 and QOL 3 and 6 months after surgery were improved in 2 groups ( P<0.05), but the difference between 2 groups was not statistically significant ( P>0.05); the total complication rate in group A was lower than that in group B: 10.00% (4/40) vs. 27.50% (11/40), and there was statistical difference ( P<0.05). Conclusions:TUSP and TUPKP are equally effective in the treatment of benign prostatic hyperplasia, both of which can improve the symptoms of prostatic hyperplasia, improve the quality of life and improve sexual function. But TUSP has less intraoperative bleeding, shorter operation time, faster postoperative recovery, fewer complications, and higher safety.

Abstract:Objective To investigate the clinical efficacy and safety of the transurethral resection of the prostate(TURP) on patients with high-risk benign prostatic hyperplasia(BPH).Methods The clinical condition and surgical risks were preoperatively evaluated and properly adjusted during perioperative period.Then improved techniques for TURP timely.One hundred and eighty-seven patients with high-risk and large-volume BPH were treated with TURP.Results All patients safely got through perioperative period and received follow-up about 3~12 months.The voiding function recovered very well,the average IPSS score decreased 18.4 points and the average residual urinary volume(RU) was less than 28ml.Conclusion Only if the patients could be properly managed and the surgical techniques could be improved during perioperative period,the TURP should be a safe and effective therapy for BPH patients with high-risk and large-volume.
Six Novel Minimally Invasive Therapies for Benign Prostatic Hyperplasia

Abstract:Benign prostatic hyperplasia (BPH) is the most common cause of lower urinary tract symptoms in men. When drug treatment is ineffective or conventional surgery is not suitable, novel minimally invasive therapies can be considered. These include prostatic urethral lift, prostatic artery embolisation, water vapor thermal therapy, Aquablation-image guided robotic waterjet ablation, temporary implantable nitinol device and prostatic stents. These novel therapies can be performed in outpatient setting under local anesthesia, with shorter operative and recovery times, and better protection of ejaculatory function and erectile function. General conditions of the patient and advantages and disadvantages of the each of these therapies should be fully considered to make individualized plans.
An evaluation of transurethral plasma kinetic resection versus transurethral resection of the prostate for the treatment of symptomatic BPH

Abstract:Objective To evaluate the clinical effect in the near future of transurethral plasma kinetic resection versus transurethral resection of the prostate for the treatment of symptomatic benign prostatic hyperplasia(BPH).Methods 80 patients who were diagnosised BPH actually were divided into two groups and treated by either TUPKRP or TURP. The perioperative marker and theraputic results were recorded and analyzed.Results There was no significant difference of preoperative factors between the two groups.6 months postoperatively, IPSS,QOL and Q_(max) were significantly improved.Nosignicicant difference in the improvement of subjective sympotoms and objective results has been noted with the different procedures.The incidence of blood transfusion and TRUS occuring,the mean bladder irrigating time,catheration time,hospital stay time were significant shorter in TUPKRP than in TURP.Conclusions Although TUPKRP is as effective as TURP for the treatment of symptomatic BPH.TUPKRP has less adverse side effect and recovering time than TURP.Thus TUPKRP is a better treatment option for BPH.

Abstract:OBJECTIVE:To explore the safety and efficiency of transurethral plasmakinetic enucleation of prostate (TUPKEP) and suprapubic small cut in the treatment of high-risk and senior patients with benign prostatic hyperplasia and bladder stones.METHODS:A retrospective review was conducted for 68 high-risk and senior patients with benign prostatic hyperplasia and bladder stones. All of them were treated by TUPKEP and suprapubic small cut.RESULTS:Operation was successfully performed in all 68 cases. And there was no instance of transurethral resection syndrome, shock, myocardial infarct, cerebral infarction, cerebral hemorrhage, permanent urinary incontinence or surgical site infection. Seven patients with temporal urinary incontinence recovered at a mean time of (9.48 ± 1.52) days post-operation. The mean operative duration was (48.63 ± 4.14) min and the mean volume of blood loss (50.97 ± 5.33) ml. The changes of maximum flow rate (Qmax), international prostatic symptom score (I-PSS) and quality-of-life (QOL) were statistically significant before and after operation. Qmax increased from (4.56 ± 0.35) to (18.82 ± 1.65) ml/s (P < 0.001), I-PSS decreased form (21.96 ± 1.89) to (11.23 ± 0.86) (P = 0.018) and QOL decreased from (4.94 ± 0.35) to (1.95 ± 0.32) (P = 0.011).CONCLUSION:The approach of TUPKEP and suprapubic small cut is both safe and effective in the treatment of high-risk and senior patient with benign prostatic hyperplasia and bladder stones and should be widely applied.

Abstract:Objective. To observe the efficacy and safety of transurethral columnar balloon dilation of the prostate (TUCBDP) for benign prostatic hyperplasia (BPH) in a multicenter trial. Method. This multicenter study included 2050 patients with BPH who underwent TUCBDP from 11 cities of Zhejiang Province, from September 2015 to June 2021. Clinical assessment included recording and measurement of preoperative and postoperative data including prostate volume, serum prostate-specific antigen (PSA) levels, IPSS score, quality of life (QoL), maximum urinary flow rate (Qmax), postvoid residual urine (PVR), International Index of Erectile Function (IIEF-5), and Male Sexual Health Questionnaire-Ejaculatory Dysfunction Short Form (MSHQ-EjD-SF). Additionally, the correlation of the indicators was analyzed using linear regression and early postoperative complications were also recorded. Results. One month after surgery, the patients' IPSS score, QoL, and PVR were significantly decreased, while the Qmax, IIEF-5, and MSHQ-EjD-SF scores were increased considerably, compared with preoperative data. After surgery, the patient's IPSS score, QoL, and Qmax were improved year by year, while PVR gradually decreased. Three months after TUCBDP, IIEF-5 and MSHQ-EjD-SF levels reached the climax. Linear regression analysis showed that the serum PSA level was significantly positively correlated with Qmax at 3 months after TUCBDP, while at 6 months after surgery, it was negatively related to IPSS and QoL. Early postoperative complications appeared in 384 cases during follow-up. Conclusion. Collectively, TUCBDP may effectively improve the urinary and sexual function of BPH patients, with fewer postoperative complications, and its efficacy is not limited by age and prostate volume. It can be considered a better treatment option for BPH.

Multivariate Analysis On Improvement Of Nocturia In Patients With Benign Prostatic Hyperplasia After Transurethral Plasmakinetic Prostatectomy

Abstract:Objective: To investigate the effect of transurethral plasmakinetic prostatectomy (TUPKP) on nocturia in patients with benign prostatic hyperplasia (BPH) and the factors affecting the postoperative efficacy. Methods: From May 2016 to June 2019, 158 patients with BPH treated in our hospital were selected and randomly separated into two groups. In the control group (CG), 71 patients received transurethral resection of prostate. In the observation group (OG), 87 patients received TUPKP. The postoperative indexes (PSA, maximum flow rate) of all patients were compared. The international prostate symptom score (IPSS), overactive bladder symptom score (OABSS) and quality of life score (QOL) were compared before and after operation. Logistic analysis was used to analyze the related factors affecting the improvement of nocturia. Results: After operation, the expression of PSA in OG was obviously lower than that in CG, while the expression of Qmax was significantly higher than that in CG. The postoperative IPSS (urinary frequency, urgency of urination, number of nocturia, endless urination, urination segmentation, thinning of urinary stream and waiting for urination) scores of patients in OG were obviously lower than those in CG. After operation, the OABSS scores in OG were obviously lower than those in CG, while the QOL scores of patients in OG were obviously higher than those in CG. Patients with age > 60, high PSA, low Qmax, high IPSS score, high OABSS score, low QOL score and transurethral resection of prostate had an increased risk of nocturia symptoms. Conclusion: The increase of nocturia is the result of multiple risk factors, and TUPKP intervention can reduce the number of nocturia, ameliorate micturition symptoms and improve the quality of life for patients with BPH.

Abstract:Background: Transurethral split of the prostate (TUSP) is effective in treating benign prostatic hyperplasia (BPH). However, there is still a lack of research focusing on the optimal target population for TUSP. This study aimed to compare the efficacy of TUSP in patients with different prostate volumes or ages. Methods: The study was a multicenter retrospective study. The outcomes of TUSP in BPH patients with different prostate volumes or different ages were compared. A total of 439 patients were included in the study. Patients were divided into two groups according to prostate volume, with a cut-off value of 50 mL. Similarly, the cut-off value for the age groups was 70 years. Baseline patient characteristics and perioperative outcomes were recorded. Follow-up was performed at 1, 6, and 12 months after surgery. Results: The mean age of the patients was 73.4 years, and the mean prostate volume was 51.2 mL. At 12-month follow-up after TUSP treatment, the patients' International Prostate Symptom Scores (IPSS), quality of life (QoL) scores, and postvoid residual (PVR) volumes decreased significantly, while peak urinary flow rate (Qmax) increased significantly. Intraoperative hemoglobin (Hb) reduction was significantly lower in the small volume group than in the large volume group. The incidence of postoperative urinary urgency and transient incontinence was lower in the small volume group. IPSS score, PVR, and Qmax in the small volume group showed more remarkable changes at several time points compared to the preoperative period. Postoperative pain scores were higher in the small volume group than in the large volume group. There were no differences between the two groups in terms of long-term complications. The younger group showed greater variation in PVR and Qmax at some time points but less variation in QoL than the older group. Conclusions: TUSP is overall safe and effective in treating BPH. This study showed differences in the outcomes of TUSP in treating different prostate volumes or ages of BPH patients. The optimal surgical approach for BPH patients might be selected clinically based on a combination of prostate volume or patient age.
[Transurethral columnar balloon dilation of the prostate for benign prostatic hyperplasia]

Abstract:Objective: To study the clinical effect of transurethral columnar balloon dilation of the prostate (TUCBDP) in the treatment of BPH and introduce the experience with the surgical procedure. Methods: We retrospectively analyzed the clinical data on 265 cases of BPH treated by TUCBDP from August 2016 to August 2019. Results: Operations were successfully completed in all the cases, with the mean operation time of (24.67 ± 7.6) min and the average intraoperative blood loss of (26.5 ± 21.4) ml, and all the patients had urinary patency after removal of the catheter. Follow-up examinations every 3 months after surgery showed significant improvement over the baseline in IPSS, quality of life score (QOL), maximum urinary flow rate (Qmax), postvoid residual urine (PVR) and other indicators (P < 0.05). Forty-five of the patients with sexual life exhibited no significant difference from the baseline in IIEF-5 and erectile hardness scale (EHS) scores (P > 0.05). Postoperative complications were observed in 53 cases (20%), including 28 cases of transient urinary incontinence (10.56%), 3 cases of hemorrhage (1.13%), 11 cases of urinary tract infection (4.15%), 1 case of urethral stricture (0.37%), and 8 cases of acute urinary retention (3.01%), which were all improved after regular treatment, with no occurrence of true urinary incontinence. Retrograde ejaculation occurred in 2 (4.45%) of the 45 patients with sexual life. Conclusions: Transurethral columnar balloon dilation of the prostate, with the advantages of short operation time and less intraoperative bleeding, has a significant short-term clinical effect in the treatment of BPH, particularly suitable for the elderly and those who want to retain the sexual function. Intraoperative localization of the protrusion may significantly influence the outcome of surgery, which deserves strengthened studies. Special attention should be paid to the incidence of postoperative transient urinary incontinence.

Abstract:Surgical therapies for symptomatic bladder outlet obstruction (BOO) due to benign prostatic hyperplasia (BPH) are many, and vary from minimally invasive office based to high-cost operative approaches. This Guideline presents effective evidence-based surgical management of male lower urinary tract symptoms secondary/attributed to BPH (LUTS/BPH). See accompanying algorithm for a detailed summary of procedures (figure). The Minnesota Evidence Review Team searched Ovid MEDLINE, Embase, Cochrane Library, and AHRQ databases to identify eligible studies published between January 2007 and September 2020, which includes the initial publication (2018) and amendments (2019, 2020). The Team also reviewed articles identified by Guideline Panel Members. When sufficient evidence existed, the body of evidence was assigned a strength rating of A (high), B (moderate), or C (low) for support of Strong, Moderate, or Conditional Recommendations. In the absence of sufficient evidence, information is provided as Clinical Principles and Expert Opinions (table). Twenty-four guideline statements pertinent to pre-operative and surgical management were developed. Appropriate levels of evidence and supporting text were created to direct urologic providers towards suitable and safe operative interventions for individual patient characteristics. A re-treatment section was created to direct attention to longevity and outcomes with individual approaches to help guide patient counselling and therapeutic decisions. Pre-operative and surgical management of BPH requires attention to individual patient characteristics and procedural risk. Clinicians should adhere to recommendations and familiarize themselves with criteria that yields the highest likelihood of surgical success when choosing a particular approach for a particular patient.

An analog closed-loop SC accelerometer interface for a capacitive high-Q sensor element

Abstract:This paper presents an accelerometer interface circuit using a vacuum packaged MEMS sensor element. In order to damp the sensor element, increase the linearity and improve the input range of the sensor, a closed-loop analog SC interface with a PD-controller is used. The theoretical input range of sensor is ±1.4 g while measured range of the sensor is from −0.7 g to +1.4 g. The interface achieves maximum non-linearity of 0.67% over temperature range of −40 to +85 °C. The sensor has bias stability of 11 µg and the noise density of 2 µg√Hz. The interface has been implemented in a 0.35 µm CMOS process with a total active area of 1.3 mm2, and it consumes 2.4 mA from a 3.6 V supply.

Abstract:Background Although there are different treatments for benign prostate hyperplasia, their efficacy and safety differ. We are currently exploring a new minimally invasive interventional therapy for benign prostatic hyperplasia (BPH). Purpose To determine the feasibility, effectiveness, and safety of ultrasound-guided transperineal laser ablation (US-TPLA) for the treatment of BPH. Material and Methods Twenty patients with BPH (mean age = 73.9 ± 9.2 years) who underwent US-TPLA from June 2018 to January 2020 with a subsequent six-month follow-up were retrospectively reviewed. After local anesthesia, a 21-G trocar was inserted into the prostate tissue under ultrasound monitoring, followed by 1064 nm diode laser irradiation. Changes in international prostate symptom score (IPSS), quality of life (QoL), maximum urinary flow rate (Qmax), postvoid residual (PVR), prostate volume, and complications were evaluated six months after surgery. Results All patients underwent the operation successfully without serious complications. After six months, the average IPSS improved from 22.7 ± 5.3 to 9.1 ± 3.2 ( P < 0.001), the QoL improved from 4.9 ± 1.7 to 2.3 ± 1.3 ( P < 0.001), the Qmax improved from 8.5 ± 3.0 to 15.2 ± 4.8 mL/s ( P < 0.001), the PVR increased from 78.7 ± 58.8 to 30.3 ± 34.2 ( P < 0.05), and the mean prostate volume ranged from 70.8 ± 23.8 to 54.7 ± 20.9 mL ( P < 0.05). Conclusion US-TPLA is safe and feasible for the treatment of BPH. An evaluation at the six-month follow-up is effective.

Abstract:OBJECTIVE To perform a systematic comparison of transurethral plasmakinetic resection of the prostate (PKRP) to conventional transurethral resection of the prostate for treating benign prostate hyperplasia (BPH) in aged high-risk patients. METHODS Three hundred twenty-nine symptomatic patients diagnosed with BPH underwent endourological treatment by transurethral resection of the prostate (n = 136) or PKRP (n = 193). Preoperative and postoperative assessments were conducted for the International Prostate Symptom Scores, quality of life (QoL), postvoid residual urine (PVRU) volumes, maximal urine flow rates (Q(max.)), and prostate-specific antigen. Perioperative data were collected for operative time, weight of resected tissue, blood loss, cases of open surgery, duration of bladder irrigation, and duration of catheter use. Patients were re-evaluated at postoperative months 3, 6, 12, 18, and 24. Postoperative complications were recorded. RESULTS In the perioperative period, no significant differences were found between the 2 surgery groups for weight of resected tissue or cases of open surgery. However, PKRP was associated with significantly shorter operative time, duration of bladder irrigation, and duration of catheter use, as well as less blood loss. At the 2-year follow-up, both procedures were found to have significantly improved International Prostate Symptom Scores, QoL, Q(max.), PVRU, and prostate-specific antigen. In addition, each procedure was associated with some postoperative complications, some of which were significantly reduced in one over the other, such as secondary hemorrhage in PKRP. CONCLUSION The currently available endourological treatments, transurethral resection of the prostate, and PKRP, are safe and effective therapies for treating aged high-risk patients with benign prostatic hyperplasia (BPH), although PKRP is superior in many aspects, including perioperative outcomes. Abstract:Objective: To compare the effects of transurethral resection of the prostate (TURP) and transurethral columnar balloon dilatation of the prostate (TUCBDP) in the treatment of BPH. Methods: This study included 218 BPH patients treated in Qinhuangdao Workers' Hospital from July 2021 to November 2022, 109 by TURP and the other 109 by TUCBDP. We followed up the patients for 12 months, observed their postoperative recovery, complications, serum pain, inflammatory index, cytokine level, urodynamic index, symptom improvement and quality of life (QOL) and compared the data obtained between the two groups of patients. Results: At 12 months after surgery, the total effectiveness rate was significantly higher in the TUCBDP than in the TURP group (93.58% vs 84.40%, P< 0.05), and the postoperative recovery was better in the former than in the latter (P< 0.05). Compared with the baseline, the levels of serum prostaglandin E2 (PGE2), substance P, tumor necrosis factor-alpha (TNF-α) and high sensitive C-reactive protein (hs-CRP) were remarkably increased in both of the groups on the first day after surgery (P< 0.05), more significantly in the TURP than in the TUCBDP group (P< 0.05), while the levels of serum PSA and E2 decreased and the T level elevated in all the patients at 3 months postoperatively (P< 0.05), more significantly in the TUCBDP than in the TURP group (P< 0.05). Before and at 3 and 12 months after operation, the postvoid residual urine volume (PVR) and NIH-CPSI, IPSS and QOL scores showed a decreasing trend, while the maximum urinary flow rate (Qmax), maximum cystometric capacity (MCC) and maximum urethral closure pressure (MUCP) exhibited an increasing trend in both of the two groups, even more significantly in the TUCBDP than in the TURP group (P< 0.05). Conclusion: TUCBDP is advantageous over TURP in promoting postoperative recovery, improving QOL, reducing postoperative pain, inflammation and complications, regulating the levels of serum cytokines, and improving urodynamics and clinical symptoms in BPH patients. However, with the extension of postoperative time, the two strategies are basically comparable in improving the urodynamics, symptoms and QOL of the patients.

Abstract:Objective:To explore the efficacy and safety of modified plasma kinetic resection of the prostate (PKRP) and transurethral resection of the prostate(TURP)in the treatment of benign prostate hyperplasia (BPH).Methods:Retrospective analysis of clinical data of a total of 104 BPH patients who were treated in Peking university first hospital from August 2018 to August 2020 were performed and divided into TURP group and modified PKRP group, with 52 cases in each group. BPH patients in TURP group were treated by TURP, and BPH patients in modified PKRP group were treated by modified PKRP. The operation time, weight of excised tissue, bleeding volume and extubation time of the two groups were compared; the erectile confidence, insertion ability and erectile maintenance ability of the two groups were compared with the international erectile function index score(IIEF-5); the international score of prostate symptoms (IPSS) and quality of life (QOL) scores of the two groups were compared; the erectile dysfunction, semen volume reduction, retrograde ejaculation and complications of the two groups were observed.Results:Compared with TURP group, the operation time, intraoperative hemorrhage and extubation time of modified PKRP group were lower, the weight of excised tissue was higher than that of TURP group (all P<0.05); the erectile confidence, insertion ability and erectile maintenance ability of modified PKRP group were higher than that of TURP group (all P<0.05); the IPSS and QOL scores of modified PKRP group were lower than that of TURP group (all P<0.05); the erectile function of modified PKRP group was better than that of TURP group( P=0.006, 0.046, 0.025); the total incidence of complications in the modified PKRP group was lower than that in the TURP group ( P<0.05). Conclusions:Compared with TURP, modified PKRP has good therapeutic effect, high safety, lower IPSS and QOL scores, less impact on patients' injury symptoms, and fewer complications, which is worthy of clinical promotion.
Transurethral bipolar plasmakinetic prostatectomy for benign prostatic hyperplasia in high-risk and senior patients in China: a systematic review and meta-analysis

Abstract:Objective To evaluate the effectiveness and safety of transurethral bipolar plasmakinetic prostatectomy in the treatment of benign prostatic hyperplasia in high-risk and senior patients in China.Methods The PubMed,Cochrane Library,CBM,CNKI and WanFang databases were searched with computer for collecting relevant interventional case series from establishment dates to September 14,2018.After quality evaluation and data extraction independently conducted by two authors,the Meta-analysis was performed using the Comprehensive Meta-analysis V2 software.Results Eighteen studies involving 1 899 patients are included.Maximum flow rate increased to 12.28 ml/s (95％CI:8.42-16.14),12.88 ml/s (95％CI:9.85-15.92),14.32 ml/s (95％CI:10.47-18.18),14.93 ml/s(95％CI:10.19-19.67) and 20.00 ml/s (95％CI:19.08-20.92) in 1,3,6,12 and 24 months after surgery,respectively.International prostate symptom score decreased to-18.60 (95％CI:-23.20～-14.00),-17.62 (95％CI:-20.21～-15.03),-19.14 (95％CI:-20.70～-17.59),-19.06 (95％CI:-21.53～-16.60) and-22.90 (95％CI:-24.26～-21.54),respectively.Quality of life decreased to-2.38 (95％CI:-4.26～-0.50),-3.39 (95％CI:-4.57～-2.21),-3.75 (95％CI:-4.14～-3.36),-3.36(95％CI:-4.56～-2.16),and-4.58(95％CI:-4.75～-4.41).Post void residual decreased to-231.16 ml (95％CI:-288.30～-174.01),-76.10 ml (95％CI:-116.71～-35.50),-159.90 rnl(95％CI:-207.21～-112.59) and-87.70 ml (95％CI:-91.91～-83.48).The event rate of postoperative adverse reactions all were not high.Conclusion Transurethral bipolar plasmakinetic prostatectomy has better clinical efficacy and no obvious side effects in the treatment of benign prostatic hyperplasia in high-risk and senior patients in China.
Transurethral enucleation of the prostate versus transvesical open prostatectomy for large benign prostatic hyperplasia: a systematic review and meta-analysis of randomized controlled trials

Abstract:Purpose: To evaluate the efficacy and safety of transurethral enucleation of the prostate (TUEP) versus transvesical open prostatectomy (OP) for the management of large benign prostatic hyperplasia (BPH). Methods: Randomized controlled trials (RCTs) comparing TUEP and OP were identified from PubMed, Embase and Web of Science up to February 28, 2015. A meta-analysis was conducted with the STATA 12.0 software. Results: Nine RCTs including 758 patients were enrolled in our meta-analysis. There were no significant differences between the two groups in the maximum urinary flow rate at 1, 3, 6 months, 1 and 2 years: postvoiding residual urinary volume, prostate-specific antigen, international prostate symptom score and quality of life score at 1, 3, 6 months and 1 year; or international index of erectile function at 3, 6 months and 1 year. Perioperative outcomes including hemoglobin level drop, catheter period, irrigation length and hospital stay favored TUEP, while operative time and resected prostate weight favored OP. There was significantly less blood transfusion with TUEP, but no significant differences were found in other complications such as recatheterization, urinary tract infection, reintervention for clots and bleeding control, incidence of pneumonia and infarction, transient incontinence, bladder neck contracture, urethral stricture and recurrent adenoma. Conclusions: TUEP can be performed effectively and safely with functional outcomes and complications similar to OP for large BPH, whereas it has the advantages of a shorter catheter period, shorter hospital stays and less blood transfusion. These findings seem to support TUEP as the next-generation "gold standard" of surgery for large BPH.

Abstract:Objectives: To evaluate the feasibility and safety of bipolar-plasmakinetic transurethral enucleation and resection of the prostate (B-TUERP) in day surgery. Methods: From January 2021 to August 2022, 34 patients with benign prostatic hyperplasia (BPH) underwent B-TUERP in day surgery in the First Affiliated Hospital of Anhui Medical University. Patients completed the screening and anesthesia evaluation before admission and received the standard surgery which implements "anatomical enucleation of the prostate" and "absolute bleeding control" on the same day of admission, and by the same doctor. Bladder irrigation was stopped, catheter was removed and the discharge evaluation was performed on the first day after operation. The baseline data, perioperative conditions, time of recovery, treatment outcomes, hospitalization costs, and postoperative complications were analyzed. Results: All operations were successfully conducted. The average age of the patients was (62.2±7.8) years, average prostate volume was (50.2±29.3) mL. The average operation time was (36.5±19.1) min, the average hemoglobin and blood sodium were decreased by (16.2±7.1) g/L and (2.2±2.0) mmol/L, respectively. The average postoperative length of hospital stay, and total length of hospital stay were (17.7±2.2) and (20.8±2.1) h, respectively, and the average hospitalization cost was (13 558±2320) CNY. All patients were discharged on the day after surgery except for one patient who was transferred to a general ward. Three patients received indwelling catheterization after catheter removal. The 3-month follow-up results showed a substantial improvement in the International Prostate Symptom Score, quality of life score and maximum urinary flow rate (all P<0.01). Three patients experienced temporary urinary incontinence, 1 patient experienced urinary tract infection, 4 patients were diagnosed with urethral stricture and 2 patients experienced bladder neck contracture. No complications above Clavien grade Ⅱ occurred. Conclusions: The preliminary results showed that B-TUERP ambulatory surgery is a safe, feasible, economical and effective treatment for appropriately selected patients with BPH.

Abstract:Objective: To systematically evaluate the quality of clinical practice guidelines for the diagnosis and treatment of benign prostatic hyperplasia (BPH), and to compare the context of recommendations in order to provide references for clinical application. Methods: We searched databases such as the National Guideline Clearinghouse (NGC), Guidelines International Network (GIN), National Institute for Health and Clinical Excellence (NICE), Scottish Intercollegiate Guidelines Network (SIGN) and World Health Organization (WHO), PubMed, Embase, CNKI, VIP, WanFang Data, CBM, and Medlive from their establishment until August 13, 2016, to collect evidence-based guidelines and/or consensus on BPH. Method: Methodological quality of included guidelines was assessed according to the AGREE Ⅱ instrument, and differences and similarities among recommendations were compared. Results: A total of 15 guidelines were included. According to the AGREE Ⅱ instrument, the score of scope and purpose, stakeholder involvement, rigour of formulate, clarity of presentation, applicability, and editorial independence was 72%, 38%, 30%, 58%, 16%, and 40%, respectively. The recommendations of different guidelines were basically similar, only with conflicts in some areas. Conclusions: The quality of included guidelines remains to be unified, the context of them can provide valuable implications for development or improvement.
