Abstract:We investigated the feasibility of the generation of synthetic CT (sCT) from CBCT images with deep learning and the dose evaluation for CBCT-guided breast cancer adaptive radiotherapy. A total of sixty-eight patients receiving radiotherapy after breast-conserving surgery were retrospectively included in this study. We compared the performance of three deep-learning methods in generating sCT from CBCT, including U-Net, Cycle generative adversarial network (CycleGAN) and pix2pix. The original treatment plan was transferred to sCT keeping the same parameters. The dosimetric evaluation was performed by a quick dose recalculation on sCT based on gamma analysis. The U-Net model obtained the lowest mean absolute error (MAE) within the body, clinical target volume (CTV) and organs at risk (OARs), with 62.53 +/- 9.14 HU within the body, 35.99 +/- 6.32 HU within tumor bed, and 30.15 +/- 6.36 HU within CTV. In terms of dose comparison, the gamma pass rates under 3%/3 mm and 2%/2 mm criteria were 91.40 +/- 3.52% and 85.95 +/- 4.75% for the U-Net model, whereas 89.50 +/- 3.46% and 83.65 +/- 4.00% for the pix2pix model, and 89.84 +/- 3.47% and 83.69 +/- 4.28% for the CycleGAN model, respectively. The sCT images generated by the U-Net model can provide higher image similarity and dosimetric accuracy than those generated by the pix2pix and CycleGAN models. The approach could be used to realize accurate dose calculation for breast cancer adaptive radiotherapy based on CBCT.

Abstract:Cone beam computed tomography (CBCT) images can be used for dose calculation in adaptive radiation therapy (ART). The main challenges are the large artefacts and inaccurate Hounsfield unit (HU) values. Currently, deformed planning CT images are often used for this purpose, although anatomical accuracy might be a concern. Ideally, we would like to convert CBCT images to CT images with artifacts removed or greatly reduced and HU values corrected while keeping the anatomical accuracy. Recently, deep learning has achieved great success in image-to-image translation tasks. It is very difficult to acquire paired CT and CBCT images with exactly matching anatomy for supervised training. To overcome this limitation, we developed and tested a cycle generative adversarial network (CycleGAN) which is an unsupervised learning method and does not require paired training datasets to synthesize CT images from CBCT images. The synthesized CT (sCT) images have been compared with the deformed planning CT (dpCT) showing visual and quantitative similarity with artifacts being removed and HU value errors being reduced from 71.78 HU to 27.98 HU. Dose calculation accuracy using sCT images has been improved over the original CBCT images, with the average Gamma Index passing rate increased from 95.4% to 97.4% for 1 mm/1% criteria. A deformable phantom study has been conducted and demonstrated better anatomical accuracy for sCT over dpCT.

Abstract:Objective: To generate synthetic CT (sCT) images with high quality from CBCT and planning CT (pCT) for dose calculation by using deep learning methods. Methods: 169 NPC patients with a total of 20926 slices of CBCT and pCT images were included. In this study the CycleGAN, Pix2pix and U-Net models were used to generate the sCT images. The Mean Absolute Error (MAE), Root Mean Squared Error (RMSE), Peak Signal to Noise Ratio (PSNR), and Structural Similarity Index (SSIM) were used to quantify the accuracy of the proposed models in a testing cohort of 34 patients. Radiation dose were calculated on pCT and sCT following the same protocol. Dose distributions were evaluated for 4 patients by comparing the dose-volume-histogram (DVH) and 2D gamma index analysis. Results: The average MAE and RMSE values between sCT by three models and pCT reduced by 15.4 HU and 26.8 HU at least, while the mean PSNR and SSIM metrics between sCT by different models and pCT added by 10.6 and 0.05 at most, respectively. There were only slight differences for DVH of selected contours between different plans. The passing rates of 2D gamma index analysis under 3 mm/3% 3 mm/2%, 2 mm/3%and 2 mm/2% criteria were all higher than 95%. Conclusions: All the sCT had achieved better evaluation metrics than those of original CBCT, while the performance of CycleGAN model was proved to be best among three methods. The dosimetric agreement confirmed the HU accuracy and consistent anatomical structures of sCT by deep learning methods.

Abstract:Objective: Adaptive radiotherapy workflows require images with the quality of computed tomography (CT) for re-calculation and re-optimisation of radiation doses. In this work we aim to improve quality of cone beam CT (CBCT) images for dose calculation using deep learning. Approach: We propose a novel framework for CBCT-to-CT synthesis using cycle-consistent Generative Adversarial Networks (cycleGANs). The framework was tailored for paediatric abdominal patients, a challenging application due to the inter-fractional variability in bowel filling and smaller patient numbers. We introduced the concept of global residuals only learning to the networks and modified the cycleGAN loss function to explicitly promote structural consistency between source and synthetic images. Finally, to compensate for the anatomical variability and address the difficulties in collecting large datasets in the paediatric population, we applied a smart 2D slice selection based on the common field-of-view across the dataset (abdomen). This acted as a weakly paired data approach that allowed us to take advantage of scans from patients treated for a variety of malignancies (thoracic-abdominal-pelvic) for training purposes. We first optimised the proposed framework and benchmarked its performance on a development dataset. Later, a comprehensive quantitative evaluation was performed on an unseen dataset, which included calculating global image similarity metrics, segmentation-based measures and proton therapy-specific metrics. Main results: We found improved performance, compared to a baseline implementation, on imagesimilarity metrics such as Mean Absolute Error calculated for a matched virtual CT (55.0±16.6 proposed, 58.9±16.8 baseline). There was also a higher level of structural agreement for gastrointestinal gas between source and synthetic images measured through dice similarity overlap (0.872±0.053 proposed, 0.846±0.052 baseline). Differences found in water-equivalent thickness metrics were also smaller for our method (3.3±2.4% proposed, 3.7±2.8% baseline). Significance: Our findings indicate that our innovations to the cycleGAN framework improved the quality and structure consistency of the synthetic CTs generated.

Abstract:Objective: To develop high-quality synthetic CT (sCT) generation method from low-dose cone-beam CT (CBCT) images by using attention-guided generative adversarial networks (AGGAN) and apply these images to dose calculations in radiotherapy. Methods: The CBCT/planning CT images of 170 patients undergoing thoracic radiotherapy were used for training and testing. The CBCT images were scanned under a fast protocol with 50% less clinical projection frames compared with standard chest M20 protocol. Training with aligned paired images was performed using conditional adversarial networks (so-called pix2pix), and training with unpaired images was carried out with cycle-consistent adversarial networks (cycleGAN) and AGGAN, through which sCT images were generated. The image quality and Hounsfield unit (HU) value of the sCT images generated by the three neural networks were compared. The treatment plan was designed on CT and copied to sCT images to calculated dose distribution. Results: The image quality of sCT images by all the three methods are significantly improved compared with original CBCT images. The AGGAN achieves the best image quality in the testing patients with the smallest mean absolute error (MAE, 43.5 ± 6.69), largest structural similarity (SSIM, 93.7 ± 3.88) and peak signal-to-noise ratio (PSNR, 29.5 ± 2.36). The sCT images generated by all the three methods showed superior dose calculation accuracy with higher gamma passing rates compared with original CBCT image. The AGGAN offered the highest gamma passing rates (91.4 ± 3.26) under the strictest criteria of 1 mm/1% compared with other methods. In the phantom study, the sCT images generated by AGGAN demonstrated the best image quality and the highest dose calculation accuracy. Conclusions: High-quality sCT images were generated from low-dose thoracic CBCT images by using the proposed AGGAN through unpaired CBCT and CT images. The dose distribution could be calculated accurately based on sCT images in radiotherapy.
Deep Learning-Based Synthetic Computed Tomography for Low-Field Brain Magnetic Resonance-Guided Radiation Therapy

Abstract:Purpose: Magnetic resonance (MR)-guided radiation therapy enables online adaptation to address intra- and interfractional changes. To address the need of high-fidelity synthetic computed tomography (synCT) required for dose calculation, we developed a conditional generative adversarial network for synCT generation from low-field MR imaging in the brain. Methods and materials: Simulation MR-CT pairs from 12 patients with glioma imaged with a head and neck surface coil and treated on a 0.35T MR-linac were prospectively included to train the model consisting of a 9-block residual network generator and a PatchGAN discriminator. Four-fold cross-validation was implemented. SynCT was quantitatively evaluated against real CT using mean absolute error (MAE), peak signal-to-noise ratio (PSNR), and structural similarity index measure (SSIM). Dose was calculated on synCT applying original treatment plan. Dosimetric performance was evaluated by dose-volume histogram metric comparison and local 3-dimensional gamma analysis. To demonstrate utilization in treatment adaptation, longitudinal synCTs were generated for qualitative evaluation, and 1 offline adaptation case underwent 2 comparative plan evaluations. Secondary validation was conducted with 9 patients on a different MR-linac using a high-resolution brain coil. Results: Our model generated high-quality synCTs with MAE, PSNR, and SSIM of 70.9 ± 10.4 HU, 28.4 ± 1.5 dB, and 0.87 ± 0.02 within the field of view, respectively. Underrepresented postsurgical anomalies challenged model performance. Nevertheless, excellent dosimetric agreement was observed with the mean difference between real and synCT dose-volume histogram metrics of -0.07 ± 0.29 Gy for target D95 and within [-0.14, 0.02] Gy for organs at risk. Significant differences were only observed in the right lens D0.01cc with negligible overall difference (<0.13 Gy). Mean gamma analysis pass rates were 92.2% ± 3.0%, 99.2% ± 0.7%, and 99.9% ± 0.1% at 1%/1 mm, 2%/2 mm, and 3%/3 mm, respectively. Secondary validation yielded no significant differences in synCT performance for whole-brain MAE, PSNR, and SSIM with comparable dosimetric results. Conclusions: Our conditional generative adversarial network model generated high-fidelity brain synCTs from low-field MR imaging with excellent dosimetric performance. Secondary validation suggests great promise of implementing synCTs to facilitate robust dose calculation for online adaptive brain MR-guided radiation therapy.
Geometric and Dosimetric Evaluation of Deep Learning-Based Automatic Delineation on CBCT-Synthesized CT and Planning CT for Breast Cancer Adaptive Radiotherapy: A Multi-Institutional Study

Abstract:Purpose We developed a deep learning model to achieve automatic multitarget delineation on planning CT (pCT) and synthetic CT (sCT) images generated from cone-beam CT (CBCT) images. The geometric and dosimetric impact of the model was evaluated for breast cancer adaptive radiation therapy. Methods We retrospectively analyzed 1,127 patients treated with radiotherapy after breast-conserving surgery from two medical institutions. The CBCT images for patient setup acquired utilizing breath-hold guided by optical surface monitoring system were used to generate sCT with a generative adversarial network. Organs at risk (OARs), clinical target volume (CTV), and tumor bed (TB) were delineated automatically with a 3D U-Net model on pCT and sCT images. The geometric accuracy of the model was evaluated with metrics, including Dice similarity coefficient (DSC) and 95% Hausdorff distance (HD95). Dosimetric evaluation was performed by quick dose recalculation on sCT images relying on gamma analysis and dose-volume histogram (DVH) parameters. The relationship between ΔD95, ΔV95 and DSC-CTV was assessed to quantify the clinical impact of the geometric changes of CTV. Results The ranges of DSC and HD95 were 0.73–0.97 and 2.22–9.36 mm for pCT, 0.63–0.95 and 2.30–19.57 mm for sCT from institution A, 0.70–0.97 and 2.10–11.43 mm for pCT from institution B, respectively. The quality of sCT was excellent with an average mean absolute error (MAE) of 71.58 ± 8.78 HU. The mean gamma pass rate (3%/3 mm criterion) was 91.46 ± 4.63%. DSC-CTV down to 0.65 accounted for a variation of more than 6% of V95 and 3 Gy of D95. DSC-CTV up to 0.80 accounted for a variation of less than 4% of V95 and 2 Gy of D95. The mean ΔD90/ΔD95 of CTV and TB were less than 2Gy/4Gy, 4Gy/5Gy for all the patients. The cardiac dose difference in left breast cancer cases was larger than that in right breast cancer cases. Conclusions The accurate multitarget delineation is achievable on pCT and sCT via deep learning. The results show that dose distribution needs to be considered to evaluate the clinical impact of geometric variations during breast cancer radiotherapy.

Abstract:Due to image quality limitations, online Megavoltage cone beam CT (MV CBCT), which represents real online patient anatomy, cannot be used to perform adaptive radiotherapy (ART). In this study, we used a deep learning method, the cycle-consistent adversarial network (CycleGAN), to improve the MV CBCT image quality and Hounsfield-unit (HU) accuracy for rectal cancer patients to make the generated synthetic CT (sCT) eligible for ART. Forty rectal cancer patients treated with the intensity modulated radiotherapy (IMRT) were involved in this study. The CT and MV CBCT images of 30 patients were used for model training, and the images of the remaining 10 patients were used for evaluation. Image quality, autosegmentation capability and dose calculation capability using the autoplanning technique of the generated sCT were evaluated. The mean absolute error (MAE) was reduced from 135.84 ± 41.59 HU for the CT and CBCT comparison to 52.99 ± 12.09 HU for the CT and sCT comparison. The structural similarity (SSIM) index for the CT and sCT comparison was 0.81 ± 0.03, which is a great improvement over the 0.44 ± 0.07 for the CT and CBCT comparison. The autosegmentation model performance on sCT for femoral heads was accurate and required almost no manual modification. For the CTV and bladder, although modification was needed for autocontouring, the Dice similarity coefficient (DSC) indices were high, at 0.93 and 0.94 for the CTV and bladder, respectively. For dose evaluation, the sCT-based plan has a much smaller dose deviation from the CT-based plan than that of the CBCT-based plan. The proposed method solved a key problem for rectal cancer ART realization based on MV CBCT. The generated sCT enables ART based on the actual patient anatomy at the treatment position.

Abstract:Purpose: CBCT-based adaptive radiotherapy requires daily images for accurate dose calculations. This study investigates the feasibility of applying a single convolutional network to facilitate CBCT-to-CT synthesis for head-and-neck, lung, and breast cancer patients. Methods: Ninety-nine patients diagnosed with head-and-neck, lung or breast cancer undergoing radiotherapy with CBCT-based position verification were included in this study. CBCTs were registered to planning CTs according to clinical procedures. Three cycle-consistent generative adversarial networks (cycle-GANs) were trained in an unpaired manner on 15 patients per anatomical site generating synthetic-CTs (sCTs). Another network was trained with all the anatomical sites together. Performances of all four networks were compared and evaluated for image similarity against rescan CT (rCT). Clinical plans were recalculated on CT and sCT and analysed through voxel-based dose differences and {\gamma}-analysis. Results: A sCT was generated in 10 seconds. Image similarity was comparable between models trained on different anatomical sites and a single model for all sites. Mean dose differences < 0.5% were obtained in high-dose regions. Mean gamma (2%,2mm) pass-rates > 95% were achieved for all sites. Conclusions: Cycle-GAN reduced CBCT artefacts and increased HU similarity to CT, enabling sCT-based dose calculations. The speed of the network can facilitate on-line adaptive radiotherapy using a single network for head-and-neck, lung and breast cancer patients.

Cycle-consistent Generative Adversarial Network Synthetic CT for MR-only Adaptive Radiation Therapy on MR-Linac

Abstract:Purpose: This study assesses the effectiveness of Deep Learning (DL) for creating synthetic CT (sCT) images in MR-guided adaptive radiation therapy (MRgART). Methods: A Cycle-GAN model was trained with MRI and CT scan slices from MR-LINAC treatments, generating sCT volumes. The analysis involved retrospective treatment plan data from patients with various tumors. sCT images were compared with standard CT scans using mean absolute error in Hounsfield Units (HU) and image similarity metrics (SSIM, PSNR, NCC). sCT volumes were integrated into a clinical treatment system for dosimetric re-evaluation. Results: The model, trained on 8405 frames from 57 patients and tested on 357 sCT frames from 17 patients, showed sCTs comparable to dCTs in electron density and structural similarity with MRI scans. The MAE between sCT and dCT was 49.2 +/- 13.2 HU, with sCT NCC exceeding dCT by 0.06, and SSIM and PSNR at 0.97 +/- 0.01 and 19.9 +/- 1.6 respectively. Dosimetric evaluations indicated minimal differences between sCTs and dCTs, with sCTs showing better air-bubble reconstruction. Conclusions: DL-based sCT generation on MR-Linacs is accurate for dose calculation and optimization in MRgART. This could facilitate MR-only treatment planning, enhancing simulation and adaptive planning efficiency on MR-Linacs.

Feasibility of CycleGAN enhanced low dose CBCT imaging for prostate radiotherapy dose calculation

Abstract:Objective: Daily CBCT imaging during the course of fractionated radiotherapy treatment can enable online adaptive radiotherapy but also expose patients to a non-negligible amount of radiation dose. This work investigates the feasibility of low dose CBCT imaging capable of enabling accurate prostate radiotherapy dose calculation with only 25% projections by overcoming under-sampling artifacts and correcting CT numbers by employing cycle-consistent generative adversarial networks (cycleGAN). Approach: Uncorrected CBCTs of 41 prostate cancer patients, acquired with ∼350 projections (CBCT org ), were retrospectively under-sampled to 25% dose images (CBCT LD ) with only ∼90 projections and reconstructed using FDK. We adapted a cycleGAN including shape loss to translate CBCT LD into planning CT (pCT) equivalent images (CBCT LD_GAN ). An alternative cycleGAN with a generator residual connection was implemented to improve anatomical fidelity (CBCT LD_ResGAN ). Unpaired 4-fold cross-validation (33 patients) was performed to allow using the median of 4 models as output. Deformable image registration was used to generate virtual CTs (vCT) for Hounsfield units (HU) accuracy evaluation on 8 additional test patients. Volumetric modulated arc therapy (VMAT) plans were optimized on vCT, and recalculated on CBCT LD_GAN and CBCT LD_ResGAN to determine dose calculation accuracy. CBCT LD_GAN , CBCT LD_ResGAN and CBCT org were registered to pCT and residual shifts were analyzed. Bladder and rectum were manually contoured on CBCT LD_GAN , CBCT LD_ResGAN and CBCT org and compared in terms of dice similarity coefficient (DSC), average and 95th percentile Hausdorff distance (HD avg , HD 95 ). Main results: The mean absolute error decreased from 126 HU for CBCT LD to 55 HU for CBCT LD_GAN and 44 HU for CBCT LD_ResGAN . For PTV, the median differences of D 98% , D 50% and D 2% comparing CBCT LD_GAN to vCT were 0.3%, 0.3% and 0.3%, and comparing CBCT LD_ResGAN to vCT were 0.4%, 0.3% and 0.4%. Dose accuracy was high with both 2% dose difference pass rates of 99% (10% dose threshold). Compared to CBCT org -to-pCT registration, the majority of mean absolute differences of rigid transformation parameters were less than 0.20 mm/ 0.20°. For bladder and rectum, the DSC were 0.88 and 0.77 for CBCT LD_GAN and 0.92 and 0.87 for CBCT LD_ResGAN compared to CBCT org , and HD avg were 1.34 mm and 1.93 mm for CBCT LD_GAN , and 0.90 mm and 1.05 mm for CBCT LD_ResGAN . The computational time was ∼2 s per patient. Significance: This study investigated the feasibility of adapting two cycleGAN models to simultaneously remove under-sampling artifacts and correct image intensities of 25% dose CBCT images. High accuracy on dose calculation, HU and patient alignment were achieved. CBCT LD_ResGAN achieved better anatomical fidelity.

Abstract:Cone-beam computed tomography (CBCT) is the most commonly used 3D imaging modality in image-guided radiotherapy. However, severe artifacts and inaccurate Hounsfield units render CBCT images directly unusable for dose calculations in radiotherapy planning. The deformed pCT (dpCT) image produced by aligning the planning CT (pCT) image with the CBCT image can be viewed as the corrected CBCT image. However, when the interval between pCT and CBCT scans is long, the alignment error increases, which reduces the accuracy of dose calculations based on dpCT images. This study introduces a channel-spatial attention-guided cycle-consistent generative adversarial network (cycleGAN) called TranSE-cycleGAN, which learns mapping from CBCT to dpCT images and generates synthetic CT (sCT) images similar to dpCT images to achieve CBCT image correction. To enhance the network's ability to extract global features that reflect the overall noise and artifact distribution of the image, a TranSE branch, which is composed of a SELayer and an improved window-based transformer, was added parallel to the original residual convolution branch to the cycleGAN generator. To evaluate the proposed network, we collected data from 51 patients with head-and-neck cancer who underwent both pCT and CBCT scans. Among these, 45 were used for network training, and 6 were used for network testing. The results of the comparison experiments with cycleGAN and respath-cycleGAN demonstrate that the proposed TranSE-cycleGAN excels not only in image quality evaluation metrics, including mean absolute error, root mean square error, peak signal-to-noise ratio, and structural similarity but also in the Gamma index pass rate, a metric for dose accuracy evaluation. The superiority of the proposed method indicates its potential value in adaptive radiotherapy.

Abstract:The shape and position of abdominal and pelvic organs change greatly during radiotherapy, so image-guided radiation therapy (IGRT) is urgently needed. The world’s first integrated CT-linac platform, equipped with fan beam CT (FBCT), can provide a diagnostic-quality FBCT for achieve adaptive radiotherapy (ART). However, CT scans will bring the risk of excessive scanning radiation dose. Reducing the tube current of the FBCT system can reduce the scanning dose, but it will lead to serious noise and artifacts in the reconstructed images. In this study, we proposed a deep learning method, Content-Noise Cycle-Consistent Generative Adversarial Network (CNCycle-GAN), to improve the image quality and CT value accuracy of low-dose FBCT images to meet the requirements of adaptive radiotherapy. We selected 76 patients with abdominal and pelvic tumors who received radiation therapy. The patients received one low-dose CT scan and one normal-dose CT scan in IGRT mode during different fractions of radiotherapy. The normal dose CT images (NDCT) and low dose CT images (LDCT) of 70 patients were used for network training, and the remaining 6 patients were used to validate the performance of the network. The quality of low-dose CT images after network restoration (RCT) were evaluated in three aspects: image quality, automatic delineation performance and dose calculation accuracy. Taking NDCT images as a reference, RCT images reduced MAE from 34.34 ± 5.91 to 20.25 ± 4.27, PSNR increased from 34.08 ± 1.49 to 37.23 ± 2.63, and SSIM increased from 0.92 ± 0.08 to 0.94 ± 0.07. The P value is less than 0.01 of the above performance indicators indicated that the difference were statistically significant. The Dice similarity coefficients (DCS) between the automatic delineation results of organs at risk such as bladder, femoral heads, and rectum on RCT and the results of manual delineation by doctors both reached 0.98. In terms of dose calculation accuracy, compared with the automatic planning based on LDCT, the difference in dose distribution between the automatic planning based on RCT and the automatic planning based on NDCT were smaller. Therefore, based on the integrated CT-linac platform, combined with deep learning technology, it provides clinical feasibility for the realization of low-dose FBCT adaptive radiotherapy for abdominal and pelvic tumors.

Abstract:This study’s purpose is to develop a deep-learning-based approach to generate synthetic MRI (sMRI) for accurate online target delineation and localization and synthetic CT (sCT) for accurate dose calculation during CBCT-guided adaptive prostate cancer radiotherapy (RT). We propose to integrate dense blocks into a 3D cycle-generative adversarial network (cycle-GAN) framework to generate sMRI from daily CBCT, and incorporate residual block minimization into a 3D cycle-GAN framework to generate sCT from daily CBCT. Our networks can automatically learn the transformations from the daily CBCT to the sMRI/sCT through the training CBCT-MRI and CBCT-CT pairs. In addition, the cycle-GANs improve the learning outcomes by incorporating inverse transformations such that the cycle-CBCTs are produced from the sMRI and sCT. To generate synthetic/cycle images similar to the real training datasets, both transformations are implemented by a generator network and their outputs are judged by a discriminator. Training the generator takes into account of the discriminator feedback as well as the estimation errors between the synthetic and real/cycle and real ones. A supervised U-Net has been proposed to segment the prostate from the sMRI generated from daily CBCT. The original MRI-defined prostate was deformed to sMRI assess the prostate segmentation accuracy. Our sCT and CBCT were registered to planning CT (pCT) for generating sCT-based and CBCT-based treatment plans. Mean absolute error (MAE) and normalized cross-correlation (NCC) were used to quantify the differences between the sCT and pCT. Clinically-relevant dose volume histogram (DVH) metrics were extracted from sCT-based and CT-based treatment plans for quantitative dosimetric evaluation. The proposed adaptive RT procedure was tested using 20 prostate cancer patients with 36 treatment plans (mean prescription dose: 45±14 Gy), each of whom has CBCT, MRI and pCT. The mean volume Dice Overlap Coefficient was 89.8±5.5% between our sMRI-based U-net segmented and MRI-defined prostate. The mean MAE and NCC between sCT and pCT were 16.9±5.6 HU and 0.98±0.02. The mean prostate dose differences between sCT- and pCT-based plans were 0.11±0.68%, 0.09±0.41%, 0.04±0.38%, 0.04±0.34%, 0.05±0.38%, and 0.46±1.03% for Dmin, D10, D50, D95, Dmean, and Dmax, respectively. The mean bladder, rectum and femur head dose differences between sCT- and CT-based plans ranged from -0.22% to 0.85% for all DVH metrics. We have developed deep-learning-based methods to generate sMRI and sCT from daily CBCT. This study demonstrated that the proposed methods are capable of reliably improving target delineation and providing comparable dose accuracy to the pCT for prostate cancer treatment planning. The accurate sMRI-based prostate delineation and localization as well as precise sCT-based dose calculation warrants further development of a CBCT-guided adaptive RT workflow for prostate cancer.
Generation of abdominal synthetic CTs from 0.35T MR images using generative adversarial networks for MR-only liver radiotherapy

Abstract:Electron density maps must be accurately estimated to achieve valid dose calculation in MR-only radiotherapy. The goal of this study is to assess whether two deep learning models, the conditional generative adversarial network (cGAN) and the cycle-consistent generative adversarial network (cycleGAN), can generate accurate abdominal synthetic CT (sCT) images from 0.35T MR images for MR-only liver radiotherapy. A retrospective study was performed using CT images and 0.35T MR images of 12 patients with liver (n=8) and non-liver abdominal (n=4) cancer. CT images were deformably registered to the corresponding MR images to generate deformed CT (dCT) images for treatment planning. Both cGAN and cycleGAN were trained using MR and dCT transverse slices. Four-fold cross-validation testing was conducted to generate sCT images for all patients. The HU prediction accuracy was evaluated by voxel-wise similarity metric between each dCT and sCT image for all 12 patients. dCT-based and sCT-based dose distributions were compared using gamma and dose-volume histogram (DVH) metric analysis for 8 liver patients. sCTcycleGAN achieved the average mean absolute error (MAE) of 94.1 HU, while sCTcGAN achieved 89.8 HU. In both models, the average gamma passing rates within all volumes of interest were higher than 95% using a 2%, 2 mm criterion, and 99% using a 3%, 3 mm criterion. The average differences in the mean dose and DVH metrics were within +/-0.6% for the planning target volume and within +/-0.15% for evaluated organs in both models. Results demonstrated that abdominal sCT images generated by both cGAN and cycleGAN achieved accurate dose calculation for 8 liver radiotherapy plans. sCTcGAN images had smaller average MAE and achieved better dose calculation accuracy than sCTcyleGAN images. More abdominal patients will be enrolled in the future to further evaluate two models.

Abstract:This study aims to utilize a hybrid approach of phantom correction and deep learning for synthesized CT (sCT) images generation based on cone-beam CT (CBCT) images for nasopharyngeal carcinoma (NPC). 52 CBCT/CT paired images of NPC patients were used for model training (41), validation (11). Hounsfield Units (HU) of the CBCT images was calibrated by a commercially available CIRS phantom. Then the original CBCT and the corrected CBCT (CBCT_cor) were trained separately with the same cycle generative adversarial network (CycleGAN) to generate SCT1 and SCT2. The mean error and mean absolute error (MAE) were used to quantify the image quality. For validations, the contours and treatment plans in CT images were transferred to original CBCT, CBCT_cor, SCT1 and SCT2 for dosimetric comparison. Dose distribution, dosimetric parameters and 3D gamma passing rate were analyzed. Compared with rigidly registered CT (RCT), the MAE of CBCT, CBCT_cor, SCT1 and SCT2 were 346.11 ± 13.58 HU, 145.95 ± 17.64 HU, 105.62 ± 16.08 HU and 83.51 ± 7.71 HU, respectively. Moreover, the average dosimetric parameter differences for the CBCT_cor, SCT1 and SCT2 were 2.7% ± 1.4%, 1.2% ± 1.0% and 0.6% ± 0.6%, respectively. Using the dose distribution of RCT images as reference, the 3D gamma passing rate of the hybrid method was significantly better than the other methods. The effectiveness of CBCT-based sCT generated using CycleGAN with HU correction for adaptive radiotherapy of nasopharyngeal carcinoma was confirmed. The image quality and dose accuracy of SCT2 were outperform the simple CycleGAN method. This finding has great significance for the clinical application of adaptive radiotherapy for NPC.

Abstract:Radiotherapy commonly utilizes cone beam computed tomography (CBCT) for patient positioning and treatment monitoring. CBCT is deemed to be secure for patients, making it suitable for the delivery of fractional doses. However, limitations such as a narrow field of view, beam hardening, scattered radiation artifacts, and variability in pixel intensity hinder the direct use of raw CBCT for dose recalculation during treatment. To address this issue, reliable correction techniques are necessary to remove artifacts and remap pixel intensity into Hounsfield Units (HU) values. This study proposes a deep-learning framework for calibrating CBCT images acquired with narrow field of view (FOV) systems and demonstrates its potential use in proton treatment planning updates. Cycle-consistent generative adversarial networks (cGAN) processes raw CBCT to reduce scatter and remap HU. Monte Carlo simulation is used to generate CBCT scans, enabling the possibility to focus solely on the algorithm's ability to reduce artifacts and cupping effects without considering intra-patient longitudinal variability and producing a fair comparison between planning CT (pCT) and calibrated CBCT dosimetry. To showcase the viability of the approach using real-world data, experiments were also conducted using real CBCT. Tests were performed on a publicly available dataset of 40 patients who received ablative radiation therapy for pancreatic cancer. The simulated CBCT calibration led to a difference in proton dosimetry of less than 2%, compared to the planning CT. The potential toxicity effect on the organs at risk decreased from about 50% (uncalibrated) up the 2% (calibrated). The gamma pass rate at 3%/2 mm produced an improvement of about 37% in replicating the prescribed dose before and after calibration (53.78% vs 90.26%). Real data also confirmed this with slightly inferior performances for the same criteria (65.36% vs 87.20%). These results may confirm that generative artificial intelligence brings the use of narrow FOV CBCT scans incrementally closer to clinical translation in proton therapy planning updates.

Abstract:Purpose: To overcome the imaging artifacts and Hounsfield unit inaccuracy limitations of cone-beam computed tomography, a conditional generative adversarial network is proposed to synthesize high-quality computed tomography-like images from cone-beam computed tomography images. Methods: A total of 120 paired cone-beam computed tomography and computed tomography scans of patients with head and neck cancer who were treated during January 2019 and December 2020 retrospectively collected; the scans of 90 patients were assembled into training and validation datasets, and the scans of 30 patients were used in testing datasets. The proposed method integrates a U-Net backbone architecture with residual blocks into a conditional generative adversarial network framework to learn a mapping from cone-beam computed tomography images to pair planning computed tomography images. The mean absolute error, root-mean-square error, structural similarity index, and peak signal-to-noise ratio were used to assess the performance of this method compared with U-Net and CycleGAN. Results: The synthesized computed tomography images produced by the conditional generative adversarial network were visually similar to planning computed tomography images. The mean absolute error, root-mean-square error, structural similarity index, and peak signal-to-noise ratio calculated from test images generated by conditional generative adversarial network were all significantly different than CycleGAN and U-Net. The mean absolute error, root-mean-square error, structural similarity index, and peak signal-to-noise ratio values between the synthesized computed tomography and the reference computed tomography were 16.75 ± 11.07 Hounsfield unit, 58.15 ± 28.64 Hounsfield unit, 0.92 ± 0.04, and 30.58 ± 3.86 dB in conditional generative adversarial network, 20.66 ± 12.15 Hounsfield unit, 66.53 ± 29.73 Hounsfield unit, 0.90 ± 0.05, and 29.29 ± 3.49 dB in CycleGAN, and 16.82 ± 10.99 Hounsfield unit, 58.68 ± 28.34 Hounsfield unit, 0.92 ± 0.04, and 30.48 ± 3.83 dB in U-Net, respectively. Conclusions: The synthesized computed tomography generated from the cone-beam computed tomography-based conditional generative adversarial network method has accurate computed tomography numbers while keeping the same anatomical structure as cone-beam computed tomography. It can be used effectively for quantitative applications in radiotherapy.

Abstract:Purpose: To develop a deep learning framework based on a hybrid dataset to enhance the quality of CBCT images and obtain accurate HU values. Materials and Methods: A total of 228 cervical cancer patients treated in different LINACs were enrolled. We developed an encoder–decoder architecture with residual learning and skip connections. The model was hierarchically trained and validated on 5279 paired CBCT/planning CT images and tested on 1302 paired images. The mean absolute error (MAE), peak signal to noise ratio (PSNR), and structural similarity index (SSIM) were utilized to access the quality of the synthetic CT images generated by our model. Results: The MAE between synthetic CT images generated by our model and planning CT was 10.93 HU, compared to 50.02 HU for the CBCT images. The PSNR increased from 27.79 dB to 33.91 dB, and the SSIM increased from 0.76 to 0.90. Compared with synthetic CT images generated by the convolution neural networks with residual blocks, our model had superior performance both in qualitative and quantitative aspects. Conclusions: Our model could synthesize CT images with enhanced image quality and accurate HU values. The synthetic CT images preserved the edges of tissues well, which is important for downstream tasks in adaptive radiotherapy.
