Abstract:Purpose: This study aimed to assess the stress distribution in pulpotomized primary molars with different types of restorative materials using 3D-finite element analysis (FEA), and provide valuable insights into the selection and application of restorative materials, with the ultimate goal of reducing the risk of pulpotomy failure and protecting residual dental tissue. Methods: Four 3D models of pulpotomized primary molars with different restorative materials according to the material and its elastic modulus were analysed: resin composite, stainless steel crowns (SSCs), prefabricated zirconia crowns and endocrowns. The food layer was also designed before vertical and bucco-lingual forces were applied to simulate physiological masticatory conditions. The results were obtained by colorimetric graphs of the von Mises stresses (VMS) in the restoration and tooth remnant. The maximum shear stress on the bonding interfaces and pressure stress on the Mineral trioxide aggregate (MTA)-pulp interfaces were recorded. Results: The results of the 3D-FEA showed that all restorative materials generated stresses and strains on the tooth structure after pulpotomy. In the resin composite group, the marginal enamel exhibited the highest stress peaks. In the zirconia crown and SSC groups, there was a concentration of stress at the dentin-restoration margin. The shear stress concentrations were mainly at the adhesive margins, with lower levels around endocrowns compared to other groups. MTA in the resin composite group experienced more VMS than in the other group. The resin composite group also generated relatively higher pressure stress values at the MTA-pulp interface compared to the other groups. Significance: In the model of primary teeth following pulpotomy, the three types of restorations covering the occlusal surface can effectively reduce the stress on pulp capping materials under occlusal loads, thereby potentially decreasing the risk of pulpotomy failure. In addition, the group of endocrowns demonstrated reduced stress at the bonding interface and in the stress concentration zone near the dentist-restoration edge, making them more effective at protecting residual dental tissue.

Abstract:This study compares the maternal sociodemographic and prenatal care characteristics and birth outcomes of US resident white and Japanese-American mothers, using data from the state of Hawaii. The specific focus is to determine to what extent these factors can explain variations in newborn maturity and mortality indicators. Single livebirths to resident, non-military dependent white and Japanese-American mothers were selected for analysis from the 1979-1990 linked livebirth-infant death files from Hawaii. Compared with white mothers, Japanese-American mothers were significantly more likely to be married, age 18 years and older, have higher educational attainment, and have adequate prenatal care utilisation. The majority of Japanese-American mothers were born in Hawaii, while the majority of white mothers were born on the US mainland. The mean birthweight of Japanese-American infants was 200 g lighter than that of white infants. Infant mortality rates (IMRs) for both groups were below the US Year 2000 Health Objective. After controlling for maternal sociodemographic and prenatal care factors with logistic regression, Japanese-American infants had significantly higher risks of low birthweight, preterm and very preterm birth and of being small-for-gestational age. These findings indicate that populations with preferential maternal sociodemographic and prenatal care risk indicators may still exhibit higher low birthweight percentages, but achieve comparatively low IMRs.

Abstract:OBJECTIVE:To compare the fracture resistance of endodontically-treated teeth restored with prefabricated zirconia posts or one-piece computer aided design and computer aided manufacture (CAD/CAM) zirconia posts and cores, and unrestored endodontically-treated teeth.METHODS:Recently extracted human maxillary central incisors (n = 36) were endodontically treated and divided into three groups (n = 12 each): group 1 was restored with prefabricated zirconia posts (Comospost) and hot pressed ceramic cores; group 2 restored with one-piece CAD/CAM zirconia posts and cores; group 3 not restored. The teeth were fixed in a universal load-testing machine; a compressive load was applied at 135 degrees to the long axis of each tooth at a crosshead speed of 1 mm/min until fracture. One-way analysis of variance (Student-Newman-Keuls) was used to determine the significance of the differences in failure load between the groups.RESULTS:The mean fracture loads were (311.75 ± 70.12) N, (423.83 ± 54.58) N and (736.33 ± 82.91) N, respectively. The unrestored teeth exhibited significantly higher fracture resistance than the two groups of restored teeth. The teeth restored with one-piece CAD/CAM zirconia posts and cores had higher fracture resistance than the teeth restored with prefabricated zirconia posts and hot pressed ceramic cores (P < 0.001).CONCLUSION:Within the limitations of this study, the root canal-treated teeth restored with one-piece CAD/CAM zirconia posts and cores had a significantly higher failure resistance than the teeth restored with prefabricated zirconia posts. one-piece CAD/CAM zirconia posts and cores can offer some advantages for esthetic prosthodontics.
Efficacy evaluation of zirconia crown on primary molars with caries defects

Abstract:Objectives: This study aimed to estimate the therapeutic effects of preformed metal crown (PMC) and prefabricated zirconia crowns (ZC) on decayed primary morals in children, as well as to analyze the possible influencing factors. Methods: A retrospective cohort study was performed on the data of 192 patients (aged 3 to 8) in the Stomatological Department of Shenzhen Children's Hospital from October 2021 to December 2021. The decayed mandibular first molars were selected and restored by vital-pulp therapy followed by PMC and ZC, including 96 cases (96 caries) in the PMC group and 96 cases (96 caries) in the ZC group. Oral clinical examination was performed at 3 months, 1 year, and 2 years after treatment, overwiewing the clinical therapeutic effects and periodontal status of PMC and ZC groups, as well as recording the crown integrity, gingival index (GI), probing bleeding index (BI), plaque index (PLI) and various prosthetic indices. Results: No significance differences existed in the periodontal status of PMC and ZC groups at 3 months, 1 year, and 2 years after treatment (P>0.05). However, the GI, BI, and PLI in the PMC group were higher than those in the ZC group at 3 months, 1 year, and 2 years after treatment, and the difference was dramatically significant (P<0.05). No significances difference existed in various prosthetic indices (P>0.05), as well as in the GI, BI, and PLI, between the two groups (P>0.05). No significant differences existed in various prosthetic indices between genders after PMC restoration (P>0.05). The scores of girls in various prosthetic indices after ZC restoration were higher than those of boys (P<0.05). Pearson correlation analysis indicated an inverse correlation between age in the PMC group and the GI, BI, PLI, and FDI indices (P<0.01), rather than in the ZC group (P>0.05). Conclusions: PMC and ZC can be applied to restore deciduous molar caries. The periodontal status of deciduous teeth in ZC group was superior to that in the PMC group. The periodontal status of deciduous teeth in PMC group may be stable with increased age.
[An Experimental Study on Fracture Resistance of Metal-Ceramic Crowned Incisors with Different Post-Core Systems].

Abstract:OBJECTIVE:To evaluate fracture resistance of metal-ceramic crown restored incisors with different post-and-core systems.METHODS:Selected 40 intact maxillary central incisors were endodontically treated and then randomly assigned to four groups of 10 teeth each. Teeth in Group A were prepared to root canal with 10 mm in length, 1.6 mm in diameter and restored with fiber-reinforced posts and composite cores. Same final preparation but root canal with 1.5 mm in diameter was achieved for teeth in the other three groups. Teeth in Group B were restored with prefabricated titanium alloy posts and composite cores and teeth in Group C were restored with cast nickel-chromium post-cores. The posts were luted with a composite resin luting system, and metal-ceramic crowns were restored and cemented with the same luting system for all of the teeth in Group A, B and C. The other 10 teeth were restored with cast nickel-chromium post-cores and metal-ceramic crowns as a control, which were cemented with glass-ionomer cement. All restored teeth were thermo-cycled for 5000 cycles (5 degrees C/55 degrees C) as a fatigue test. The tooth was loaded in a universal testing machine at an angle of 135 degrees to the long-axis at the incisal edge with a cross-head speed of 1.5 mm/min until fracture. Fracture loads (N) and modes (repairable or catastrophic) were recorded. One-way ANOVA and SNK test were used to determine the significance of the failure loads between groups. Chi-square test was conducted for evaluation of the fracture mode.RESULTS:The fracture loads from Group A, B, C and control group were (534.4 +/- 145.7) N, (499.8 +/- 168.9) N, (412.6 +/- 99.3) N, (337.4 +/- 121.2) N, respectively. A significant difference was existed among four groups (P < 0.05). The fracture loads of Group A and Group B were significantly higher than control group (P < 0.05). The repairable mode of fracture observed from Group A to control group was 80%, 40%, 20% and 30%, Group A had a significantly higher number of repairable fractures than those of the other groups (P < 0.05).CONCLUSIONS:Within the limitations of this study, fiber-reinforced post has an excellent fracture resistance, and can be recommended as an alternative to cast post-cores, especially for incisor esthetic restoration.

Abstract:The restoration of endodontically treated teeth is one of the main challenges of restorative dentistry. The structure of the tooth is a complex assembly in which the materials that make it up, enamel and dentin, have very different mechanical behaviors. Therefore, finding alternative replacement materials for dental crowns in the area of restorative care isa highly significant challenge, since materials such as ceramic and zirconia have very different stress load resistance values. The aim of this study is to assess which material, either ceramic or zirconia, optimizes the behavior of a restored tooth under various typical clinical conditions and the masticatory load. A finite element analysis (FEA) framework is developed for this purpose. The 3D model of the restored tooth is input into the FEA software (Ansys Workbench R23)and meshed into tetrahedral elements. The presence of masticatory forces is considered: in particular, vertical, 45° inclined, and horizontal resultant forces of 280 N are applied on five contact points of the occlusal surface. The numerical results show that the maximum stress developed in the restored tooth including a ceramic crown and subject to axial load is about 39.381 MPa, which is rather close to the 62.32 MPa stress computed for the natural tooth; stresses of about 18 MPa are localized at the roots of both crown materials. In the case of the zirconia crown, the stresses are much higher than those in the ceramic crown, except for the 45° load direction, while, for the horizontal loads, the stress peak in the zirconia crown is almost three times as large as its counterpart in the ceramic crown (i.e., 163.24 MPa vs. 56.114 MPa, respectively). Therefore, the zirconia crown exhibits higher stresses than enamel and ceramic that could increase in the case of parafunctions, such as bruxism. The clinician's choice between the two materials should be evaluated based on the patient's medical condition.

Pitch diversity in alpha-helical coiled coils.

Abstract:Two complementary methods for measuring local pitch based on heptad position in alpha-helical coiled coils are described and applied to six crystal structures. The results reveal a diversity of pitch values: two-stranded coiled coils appear to have pitch values near 150 A; the values for three- and four-stranded coiled coils range closer to 200 A. The methods also provide a rapid and sensitive gauge of local coiled-coil conformation. Polar or charged residues in the apolar interface between coiled-coil helices markedly affect local pitch values, suggesting a connection between pitch uniformity and coiled-coil stability. Moreover, the identification of a skip residue (heptad frame shift) in the hemagglutinin glycoprotein of influenza virus (HA) allows interpretation of local pitch changes. These results on relatively short coiled-coil structures have relevance for the much longer fibrous proteins (many of which have skip residues) whose detailed structures are not yet established. We also show that local pitch values from molecular dynamics predictions of the GCN4 leucine zipper are in striking agreement with the high-resolution crystal structure--a result not readily discerned by direct comparison of atomic coordinates. Taken together, these methods reveal specific aspects of coiled-coil structure which may escape detection by global analyses of pitch.
Gingival health, plaque accumulation, and restoration outcomes in deciduous anterior teeth: evaluating zirconia crowns versus composite resin strip crowns

Abstract:Background: Zirconia crowns, resembling crystals and with enamel-like aesthetics, are recent entrants in pediatric restorative dentistry. Despite growing use, their empirical efficacy remains under-explored. The aim of this study is to compare resin composite strip crowns and premanufactured zirconia crowns on primary maxillary incisors. Methods: In the Pediatric Dental Clinics, 240 primary maxillary incisors (69 patients) were treated with either zirconia (120 teeth) or composite strip crowns (120 teeth). Evaluations of gingival health, plaque, secondary caries, restoration failure, and opposing teeth wear occurred at 3, 6, and 12-month intervals. Results: At 3 months, zirconia crowns showed less gum bleeding (P=0.006) and plaque. Through 6 and 12 months, zirconia crowns exhibited fewer failures but higher opposing teeth wear. Strip crowns indicated greater tooth material loss. Conclusions: Zirconia crowns demonstrated superior gingival health and durability over composite resin strip crowns but caused increased wear on opposing teeth.

Abstract:Background Paediatric‐preformed zirconia crowns have been associated with several issues, primarily their inability to be crimped and the need for extensive tooth preparation. Additionally, the capacity to adjust the size, shape, and fit of these crowns is very limited. Comparison of Compressive Strength Between Traditional and Novel Open-Faced Stainless Steel Crowns Filled With Different Restorative Materials

Abstract:Purpose: To compare the compressive strength of traditional open-faced stainless-steel crown (OFSSC) to novel Preformed OFSSC (POFSSC) filled with different restorative materials. Methods: Seventy-five uniform 3D-printed models were divided into five groups (n=15): Group one (G1) traditional OFSSC; Group two (G2) POFSSC with restorative composite; Group three (G3) POFSSC with bulk-fill composite; Group four (G4) POFSSC with flowable resin restorative; and Group 5 (G5) POFSSC with resin-reinforced glass-ionomer. Samples were subjected to 1,000 cycles of thermocycling, at five degrees to 55 degrees Celsius, followed by a tactile exam. Instron was used for compressive strength test, with the maximum force at fracture initiation recorded in Newtons (N). Failure of the facial-surface was categorized as <50% facial-surface chipped, ≥50% to <100% facial-surface chipped, or the entire facial-surface came-off. One-way analysis of variance and the Kruskal-Wallis test was used (P<0.05). Results: All samples were intact during the tactile exam following thermocycling. The compressive strengths are ranked as: G2???784.66±86.29 N; G3???730.46±159.52 N; G4???726.33±150.47 N; G1???650.59±116.05 N; G5???556.60±137.65 N. The compressive strength of G2 was significantly higher than G1 (P=0.007). G5 showed significantly lower compressive strength than G2 (P=0.0), G3 (P=0.001), and G4 (P=0.001). The entire facial-surface cameoff as follows: G2???6.67%; G4???13.33%; G5???26.67%; G3???53.33%; G1???66.67%. Conclusion: Compressive strength of G2 was significantly higher than control; and G2 had the lowest percentage of the entire facial-surface coming-off.
Comparison of the biomechanical effects of the post-core crown, endocrown and inlay crown after deep margin elevation and its clinical significance

Abstract:Background: The purpose of this in vitro study was to compare and evaluate the stress distribution of maxillary first premolar residual crowns restored with post-core crowns, endocrowns and inlay crowns after deep margin elevation, to explore the fitting restoration for residual crowns using finite element analysis. Methods: A healthy complete right maxillary first premolar from a male adult was scanned by cone beam computed tomography (CBCT). The finite element model of the tooth was established by reverse engineering software such as Mimics, Geomagic and Hypermesh. On this basis, the residual crown model after deep margin elevation was made, and the experimental group models were divided into three groups, those restored with post core crowns, endocrowns and inlay crowns. Vertical and oblique static loads were applied to the experimental models to simulate the force on the tooth during mastication (the loading position was located in the central fossa of the occipital surface, and the load was 100 N) using Abaqus software. Results: The peak value and distribution of von Mises stress in each part of the experimental model were observed. After deep margin elevation, the peak dentin von Mises stresses were lower than the tensile strength of normal dentin in the post-core crown, endocrown, and inlay crown groups; the lowest stress results were found in the post-core crown group for the dentin, restoration, enamel, and deep margin elevation (DME) layers under vertical and oblique loading. In terms of stress distribution clouds, the peak stresses in the dentin tissue were located in the apical 1/3 of the root after postcore crown restorations for both loads, while stress concentrations were evident in the cervical and root areas after endocrown and inlay crown restorations; regardless of the load and restoration method, the corresponding stress concentration areas appeared at the junction of the DME and dentin tissue at the loading site of the restorations; CONCLUSIONS: Post-core crowns, endocrowns and inlay crowns can be used to restore residual crowns after deep margin elevation, and post-core crowns can better protect the residual tooth tissue.

Abstract:Objectives The main purpose of this study is to evaluate the efficacy of polyetheretherketone (PEEK) as a material for prefabricated crown restorations in pediatric dentistry, particularly for restoring primary tooth structure defects. Materials and methods This study analyzed the effects of three surface treatment modalities on PEEK's surface morphology, wettability (as measured by contact angle), and shear bond strength. These treatments included alumina (Al 2 O 3 ) sandblasting alone, and in conjunction with a 98% sulfuric acid pretreatment. Subsequently, an evaluation was conducted to assess the compressive and fatigue properties of self-fabricated PEEK preformed crowns, preformed metal crowns, and preformed zirconia crowns. Results Al 2 O 3 sandblasting followed by sulfuric acid pretreatment significantly enhanced the bond strength of PEEK. PEEK crowns, when compared with stainless steel and zirconia crowns, showed similar compressive and fatigue strengths to zirconia crowns but lower than metal alternatives. Additionally, PEEK crowns required less extensive modification of dental abutments compared to zirconia crowns. Conclusions The study concludes that PEEK prefabricated crown, especially after alumina (Al 2 O 3 ) sandblasting in conjunction with a 98% sulfuric acid pretreatment, exhibits improved bond strength and favorable material characteristics for use in pediatric dental restorations. Clinical relevance The findings highlight the potential of PEEK prefabricated crowns in pediatric dentistry as a viable option for the aesthetic restoration of primary teeth, offering benefits over traditional materials like stainless steel and zirconia in terms of preferable compressive and fatigue strengths and ideal reduction of dental abutments.

Abstract:OBJECTIVE: This work analyzed and compared the mechanical properties of identical cracked tooth models treated with different materials and crown parameters. Thus, to provide dentists with a more structured way to select materials and geometric parameters and determine the strongest restoration model for cracked teeth.METHODS: This work used finite element analysis (FEA). We applied 25 restorative models, including five restorative materials, and three preparation parameters. Seven mechanical properties of the cracked tooth preparation were analyzed using correlation analysis.RESULTS: The highest lifetime of the cracked preparation was obtained for crowns with a 5° of polymerization, width = 0.8 mm, and a length offset of 0.2 mm. The highest lifetime was obtained with ZC crown material, but the least deformation of the cracked tip was obtained with LU material.SIGNIFICANCE: The results showed that the larger MOE material for the crown and a reasonable increase in the thickness and length of the crown is a favorable method to prevent further cracks to extend. This FEA study, thereby forming a novel basis for clinical guidance as to preparation of dental crowns applicable to cracked teeth.

Abstract:This study evaluated the fracture resistance of crown-restored incisors with different post-and-core systems and luting cements. Fifty intact maxillary central incisors were randomly assigned to five groups of 10 teeth each. Group 1 was restored with fibre posts (Snowpost) luted with an adhesive composite resin cement (Panavia F). Group 2 was restored with titanium alloy posts (Parapost) luted with the resin cement, and Group 3 with titanium alloy posts and a glass-ionomer cement (Fuji I). Composite cores (Clearfil Photocore) were built up in groups 1, 2 and 3. Group 4 was restored with cast post-and-cores luted with the resin cement, and group 5 with the cast post-and-cores and the glass-ionomer-cement served as a control group. All teeth were restored with metal-ceramic crowns. After thermal stressing, the specimen was then secured in a universal testing machine. Fracture loads and modes were recorded. One-way ANOVA and a Tukey test were used to determine significant differences between the failure loads of groups. Chi-square test was conducted for evaluation of the fracture modes. The fracture loads of groups 1 and 2 were significantly higher than that of the control group (P < 0.05). Group 1 had a significantly higher number of repairable fractures than the other four groups (P < 0.001). Within the limitations of this study, the results suggest that fibre posts can be recommended as an alternative to cast and prefabricated metallic posts. Composite resin cement cannot significantly improve fracture resistance of metallic post and crown-restored incisors.
[Three-Dimensional Finite Element Analysis of One-Piece Computer Aided Design and Computer Aided Manufacture Involved Zirconia Post and Core].

Abstract:OBJECTIVE To analyze the biomechanics trait of one-piece computer aided design and computer aided manufacture (CAD/CAM) zirconia post and core by the Three-dimensional finite element. METHODS The Three-dimensional finite element models of three upper central incisors restored with one-piece CAD/CAM zirconia post and core (group 1), refabricated zirconia post and hot-pressed porcelain core (group 2), and cast gold alloy post and core (group 3) were built by geometry method respectively. 100 N vertical loading through the central incisor models long axis and 100 N loading along directing at an angle of 45° with the models long axis were used to imitate the central incisor stress state in biting and mandible physiological protraction movement. RESULTS Under vertical loading, the restored teeth without dentin ferrule, the maximum Von-Mises stress value of the tooth root in group 1 was the least(11.02 N), which was the largest (13.17 N)in group 2. The stress became weaker from the upper to the lower of the tooth root. The maximum Von-Mises stress value of the tooth root, post and core became smaller while the restored teeth with the 2.0 mm high dentin ferrule. Under directing at an angle of 45° loading, without the design of dentin ferrule in the restored teeth, the maximum Von-Mises stress value of the post and core in group 1 was the greatest(20.45 N), while that stress of post and core in group 3 was the smallest(13.61 N). With 2.0 mm high dentin ferrule design in the restored teeth, the tooth root stress became weaker. The maximum Von-Mises stress value of the tooth root was the greatest (14.10 N) in group 3, but which was the lowest (13.38 N) in group 1. CONCLUSION The results of the Three-dimensional finite element analysis infers that one-piece zirconia post and core restoration is more beneficial to disperse the bite force than the prefabricated zirconia post and the cast gold alloy post and core. The one-piece of zirconia post and core is good to protect the teeth and keep the restoration intact.

Abstract:Objective To evaluate the reliability, maximum principal stress, shear stress, and crack initiation of a computer-aided design/computer-aided manufacturing (CAD/CAM) resin composite (RC) incorporating surface pre-reacted glass (S-PRG) filler for primary molar teeth. Methods Mandibular primary molar crowns fabricated by experimental (EB) or commercially available CAD/CAM RCs (HC) were prepared and cemented to a resinous abutment tooth using an adhesive resin cement (Cem) or a conventional glass-ionomer cement (CX). These specimens were subjected to a single compressive test (n = 5/each) and the step-stress accelerated life testing (SSALT) (n = 12/each). Data was evaluated using Weibull analyses and reliability was calculated. Afterwards, the maximum principal stress and crack initiation point of each crown was analyzed by finite element analysis. To evaluate bonding of EB and HC to dentin, microtensile bond strength (μTBS) testing was conducted using primary molar teeth (n = 10/each). Results There was no significant difference between the fracture loads of EB and HC for either cement ( p > 0.05). The fracture loads of EB-CX and HC-CX were significantly lower than EB-Cem and HC-Cem ( p < 0.05). The reliability at 600 N for EB-Cem was greater than that for EB-CX, HC-Cem, and HC-CX. The maximum principal stress concentrated on EB was lower than that on HC. The shear stress concentrated in the cement layer for EB-CX was higher than that for HC-CX. There was no significant difference among the μTBSs of EB-Cem, EB-CX, HC-Cem, and HC-CX ( p > 0.05). Significance The crowns fabricated with the experimental CAD/CAM RC incorporating S-PRG filler yielded greater fracture loads and reliability than the crowns manufactured with commercially available CAD/CAM RC regardless of the luting materials. These findings suggest that the experimental CAD/CAM RC crown may be clinically useful for the restoration of primary molars.

Abstract:Aim This study aimed to compare the fracture resistance of different materials used in composite core buildups, including conventional filler composite, nanofiller composite, and short fiber-reinforced composite (SFRC). Methods This in vitro study was conducted on 30 freshly extracted premolars. The teeth were treated using a uniform endodontic procedure, and Fiber Posts (REFORPOST, Angelus) were placed. The teeth were then divided into three groups and restored using different materials. Group 1 was restored using SFRC (everX Posterior, GC, Europe), Group 2 using microfiller composite (Te-Econom Flow, Ivoclar Vivadent), and Group 3 using nanofiller composite (Tetric N-Flow, Ivoclar Vivadent). The restoration materials were then light-cured for 40 seconds. The teeth were placed in a Universal Testing Machine (Instron) and a load was applied with a stainless-steel ball (4 mm diameter) until the tooth fractured. The fracture load for each tooth was recorded, and after the mechanical test, the experimental groups were examined for failure modes. Statistical analysis was performed using SPSS version 21.0 software. A one-way ANOVA test was conducted to compare more than two groups, followed by Tukey's test for post hoc pairwise comparison. Results The mean fracture resistance of the microfiller composite (346.94±44.63) was the lowest among the three groups. When analyzed using Tukey's test at p<0.05, fracture resistance was significantly higher in the SFRC, followed by nanofillers and microfiller composites. Conclusion Due to the increasing demand for aesthetic restorations in recent years, composites have become important in modern restorative dentistry. The development and implementation of composite dental restorative materials rely on a comprehensive understanding of each composite component and consideration of methods for modifying each component. As a result, the findings of this study will be beneficial in determining which material to use based on specific cases.
[Effect of composite restoration on the reinforcement of teeth]

Abstract:Objective: To determine the effect of composite restoration on reinforcement of weakened tooth structure and the possible mechanism. Methods: Sixty freshly extracted non-carious maxillary premolars were collected and divided into 6 groups with 10 specimen in each group. MOD cavities (buccolingual width: 2.8 to 3.2 mm; palatal cusp width: 2.0 mm; cusp height: 5.0 mm) were prepared individually. Group 1 was prepared and not restored (control). The other 5 groups were restored with silver amalgam alloy (group 2), Z250 without bonding (group 3), F2000 (group 4), Z250 (group 5) and Z350 nanocomposite (group 6) (3M ESPE) respectively. The fracture resistance of the tested teeth was determined by applying a vertical splitting load through a specially shaped steel rod at a crosshead speed of 1 mm/min. The data were analyzed by ANOVA. Results: The average fracture resistance of the 6 groups was: (245.29 +/- 39.49) N (group 1), (255.09 +/- 42.14) N (group 2), (267.34 +/- 31.56) N (group 3), (293.90 +/- 33.42) N (group 4), (337.81 +/- 32.63) N (group 5) and (349.08 +/- 32.93) N (group 6). There was no significant difference between the group 1, group 2 and group 3. The fracture resistance of group 4, group 5 and group 6 was higher than that of group 1 and group 2 (P < 0.05). Significant difference was noted between group 5 and group 3 (P < 0.01). The fracture resistance of group 4 was much lower than that of group 5 and group 6 (P < 0.01). No significant difference was found between group 5 and group 6. Conclusions: The use of composite increased the fracture resistance of the tooth with an MOD restoration. This effect was related to the adhesive force, polymerization shrinkage stress and the elastic modulus of the composite.
Fatigue Resistance of Resin-Bonded Post–Core–Crown Treated Teeth with Flared Root Canal

Abstract:Here the objective was to evaluate fatigue resistance of anterior teeth with flared root canal restored using resin-bonded individualized fiber-reinforced composite (FRC) posts, prefabricated FRC posts, or prefabricated stainless-steel posts.Twenty-four maxillary central incisors were endodontically treated and prepared as flared root canals. They were divided into three groups of 8 each. In Group 1, individual posts were made of FRC material. Prefabricated FRC posts were used for Group 2 and prefabricated stainless-steel posts used in Group 3. All posts were cemented with a self-adhesive resin into canals. The composite core and porcelain fused to metal crown were made for each. The specimens underwent thermocycling and dynamic mechanical loading and survived specimens were loaded to fracture. The data were statistically analyzed using ANOVA and independent sample t-test.In the fatigue test, Group 1 had no failure; all Group 2 specimens failed; and 7 specimens of Group 3 survived. There were significant differences in numbers of cyclically mechanical loading times between Groups 1 and 2, and Groups 2 and 3 (p < 0.01, p < 0.05). The average of residual load-bearing capacity was 317.5 (+/- 90.2) N for Group 1 and 403.8 (+/- 138.2) N for Group 3 and there was no significant difference between these two groups. The fracture mode of specimens was cervical root fracture with or without vertical crack in both Groups 1 and 3. In Group 2 the debonding mostly occurred at the cement-post interface.This in vitro study indicates that the flared canal teeth restored with individually formed FRC posts and prefabricated serrated stainless-steel posts provide good resistance against cyclic loading. The teeth restored with conventional prefabricated FRC posts may not be strong enough to resist masticatory forces without a ferrule. (C) Koninklijke Brill NV, Leiden, 2009
