Abstract:Objective To analyse the clinical effects of microendoscopic discectomy(MED)with traditional discectomy by laminectomy fenestration,and explore the advantages of MED.Methods Retrospective analysis of the clinical data of 121 cases of lumbar disc herniation were made.The study group(52 cases)were treated with MED,the control group(69 cases)treated with traditional discectomy by laminectomy fenestration(open surgery).The incision length,hemorrhage in operation,operation time,ratio of improved raising degree by straight limb test,the time of out of bed,residence time after operation,recovery time,the rate of excellent and good outcomes according to Macnab scale after 3 months operation in study group were compared to the control group.The complications were recorded.Results The rate of excellent and good outcomes in study group treated with MED was 98.08% compared with 88.41% in the control group,and there was no statistical difference in two groups(χ 2=2.75,P>0.05).But there were statistical differences in incision length,hemorrhage in operation,ratio of improved raising degree by straight limb test,the time of out of bed,residence time after operation,and recovery time of the two groups(t=30.27,22.75,1.46,11.47,13.85,14.69,16.04,all P< 0.05)except in operation time(t=1.46,P>0.05).There was one case suffered more pain after MED,two cases occurred dura mater ruputure in control group.Conclusions To treat lumbar disc herniation,MED has the advantages of minimaltrauma to lumbar tissue and less recovery time compared with open surgery.
Comparative evaluation of posterior percutaneous endoscopy cervical discectomy using a 3.7 mm endoscope and a 6.9 mm endoscope for cervical disc herniation: a retrospective comparative cohort study

Abstract:Background: Posterior percutaneous endoscopy cervical discectomy (p-PECD) is an effective strategy for the treatment of cervical diseases, with a working cannula ranging from 3.7 mm to 6.9 mm in diameter. However, to date, no studies have been performed to compare the clinical outcomes of the use of endoscopes with different diameters in cervical disc herniation (CDH) patients. The purpose of this study was to compare the clinical outcomes of patients with unilateral CDH treated with p-PECD using a 3.7 mm endoscope and a 6.9 mm endoscope. Methods: From January 2016 to June 2018, a total of 28 consecutive patients with single-level CDH who received p-PECD using either the 3.7 mm or the 6.9 mm endoscope were enrolled. The clinical results, including the surgical duration, hospitalization, visual analog scale (VAS) score and modified MacNab criteria, were evaluated. Cervical fluoroscopy, CT, and MRI were also performed during follow-up. Results: Tthere was a significant difference in regard to the average identification time of the "V" point (18.608 ± 3.7607 min vs. 11.256 ± 2.7161 min, p < 0.001) and the mean removal time of the overlying tissue (16.650 ± 4.1730 min vs. 12.712 ± 3.3079 min, p < 0.05) for the use of the 3.7 mm endoscope and the 6.9 mm endoscope, respectively. The postoperative VAS and MacNab scores of the two endoscopes were significantly improved compared with those the preoperative scores (p < 0.05). Conclusion: The application of both the 3.7 mm endoscope and 6.9 mm endoscope represent an effective method for the treatment of CDH in selected patients, and no significant difference can be observed in the clinical outcomes of the endoscopes. The 6.9 mm endoscope shows superiority to the 3.7 mm endoscope in terms of the efficiency of "V" point identification, the removal of overlying soft tissue and the prevention of spinal cord injury. However, the 6.9 mm endoscope may be inferior to the 3.7 mm endoscope in regards to anterior foraminal decompression due to its large diameter; this result needs to be further evaluated with the support of a large number of randomized controlled trials.

Abstract:Abstract Objective Full endoscopic techniques are being gradually introduced from single-segment cervical disc herniation surgery to two-segment cervical disc herniation surgery. However, there is no suitable full endoscopic treatment for mixed-type two-segment cervical disc herniation (MTCDH) in which one segment herniates in front of the spinal cord and the other segment herniates behind the spinal cord. Therefore, we introduce a new full endoscopic technique by combining an anterior transcorporeal approach and a posterior translaminar approach. In addition, we provide a brief description of its safety, efficacy, feasibility, and surgical points. Methods Thirty patients with MTCDH were given full endoscopic surgical treatment by a combined transcorporeal and transforaminal approach and were followed up for at least 12 months. Results Clinical assessment scales showed that the patient’s symptoms and pain were significantly reduced postoperatively. Imaging results showed bony repair of the surgically induced bone defect and the cervical Cobb angle was increased. No serious complications occurred. Conclusion This technique enables minimally invasive surgery to relieve the compression of the spinal cord by MTCDH. It avoids the fusion of the vertebral body for internal fixation, preserves the vertebral motion segments, avoids medical destruction of the cervical disc to the greatest extent possible, and expands the scope of adaptation of full endoscopic technology in cervical surgery.

Anterior transcorporeal approach of percutaneous endoscopic cervical discectomy for disc herniation at the C4-C5 levels: a technical note

Abstract:Background context: With the continuous development of the spinal endoscopic technique in recent years, percutaneous endoscopic cervical discectomy (PECD) has emerged, which bridges the gap between conservative therapy and traditional surgery and has been mainly divided into the anterior transdiscal approach and the posterior interlaminar access. Because of the relatively greater violation to the anterior nucleus pulposus, there is a higher potential of postoperative intervertebral space decrease in the anterior transdiscal approach than in the posterior interlaminar access. In addition, when the herniated lesion is migrated upward or downward behind the vertebral body, both approaches, and even anterior cervical discectomy and fusion, are impractical, and corpectomy is commonly considered as the only efficacious treatment. Anterior transcorporeal approach under endoscopy could enable an individual and adjustable trajectory within the vertebral body under different conditions of disc herniation preserving the motion of adjacent segment, especially in a migrated or sequestered lesion. Purpose: This report aimed to first describe a novel anterior transcorporeal approach under endoscopy in which we addressed a migrated disc herniation at the C4-C5 levels. Study design: A technical report was carried out. Patient sample: A 37-year-old woman presented with posterior neck pain and weakness of extremities for 9 months. On neurologic examination, tingling sensation and numbness were not obvious. However, the power of extremities was dramatically decreased at a level of 3. Hoffmann sign was positive in the bilateral hand. Magnetic resonance imaging (MRI) showed a huge herniation of the C4-C5 disc compressing the median area of the spinal cord. Besides the C4-C5 disc herniation, preoperative computer tomography (CT) also detected that the herniated disc had partial calcification. A novel anterior transcorporeal approach of PECD, through the vertebral body of C5, was performed to address a migrated disc herniation at the C4-C5 levels. Outcome measures: The posterior neck pain was measured using the visual analog scale (VAS). Methods: A novel anterior transcorporeal approach under endoscopy was performed to address a migrated disc herniation at the C4-C5 levels. Results: This operation was accomplished in 75 minutes. Postoperatively, the drainage tube was retained into the drilling hole for 24 hours to avoid the possibility of hematoma. The patient was advised to wear a neck collar for 3 weeks. Immediately after the operation, the posterior neck pain improved from VAS 7/10 preoperatively to 3/10, and the myodynamia of extremities improved stepwise. At 12 hours postoperatively, the range of motion was also improved. In the further follow-up, the patient has completely recovered from the preoperative symptoms, whose myodynamia of extremities is normal. Besides the postoperative MRI, a total removal of the herniated disc and the transcorporeal drilling tunnel are observed in CT. At postoperative 3-month follow-up, neither disc space narrowing nor instability was observed on CT, in which the bone defect after drilling tunnelwas partially decreased, indicating bone healing. There were no surgery-related complications, such as dysphagia, Horner syndrome, recurrent laryngeal nerve palsy, vagus nerve injury, tracheoesophageal injury, or cervical hematocele. Conclusions: As a supplement to the described surgical approach of PECD, the transcorporeal approach is a novel access for the treatment of cervical intervertebral disc herniation. Among the advantages of this approach are providing a clear visual field during microendoscopic surgery and decreasing the intraoperative iatrogenic injury to, as well as avoiding violation to the discal tissue. Theoretically, the potential of secondary decline of intervertebral height is low. However, as the limitation of one case shows, whether this transcorporeal approach is efficacious and reliable should be verified in a further comparative cohort study with a large volume of patients.
Disc Height and Angle Changes on Radiographs and Magnetic Resonance Imaging after Anterior or Posterior Percutaneous Endoscopic Cervical Discectomy

Abstract:Objectives: Cervical disc herniation (CDH) leads to pain, numbness, and potential disability. Percutaneous endoscopic cervical discectomy (PECD) offers an anterior or posterior approach. This study aims to compare postoperative disc height and angle changes one year after PECD, considering both approaches. Methods: We retrospectively reviewed the data from patients with CDH who underwent PECD from October 2017 to July 2022. Cervical disc height was measured using the preoperative and one-year postoperative magnetic resonance imaging (MRI) examinations. Lordotic angle (LA), global alignment angle (GAA), segmental alignment angle (SAA), and slippage distance (SD) at the surgical level were measured on radiographs in the neutral, flexion, and extension positions. Results: Thirty-eight patients who underwent posterior PECD (PPECD) and five patients who underwent anterior PECD (APECD) were included in the evaluation. The mean age of the patients was 47.4 years (range: 29–69 years). There was a significant difference in the preoperative and one-year postoperative GAA and SAA in extension in the PPECD group (p = 0.003 and 0.031, respectively). The mean decreased disc height one-year postoperative was 1.30 mm in the APECD group and 0.3 mm in the PPECD group by MRI. A significant disc height decrease was observed in the APECD group (p < 0.001). Conclusions: Treating CDH with PPECD or APECD is feasible, as it can relieve symptoms and reduce disability. Stability remained unaffected during the first year after surgery, even though there was an increase in angulation during extension. Despite a significant decrease in disc space following APECD, patients reported significant symptom improvement and no new symptoms.

Abstract:Background: We aimed to analyze the clinical outcomes and effectiveness of cervical biportal endoscopic spine surgery (C-BESS) and anterior cervical discectomy and fusion (ACDF) in patients with symptomatic cervical disc herniation. Methods: This study was a retrospective chart review of four-year clinical data involving 318 cases of symptomatic cervical disc herniation, with 156 patients undergoing the ACDF and 162 patients receiving the C-BESS. Preoperative and postoperative one-year data were collected. Results: The numeric rating scale and neck disability index showed statistically significant improvement for both ACDF and C-BESS groups. While showing a longer operation time and more blood loss during surgery compared to the ACDF group, the C-BESS group demonstrated a learning effect as the surgeon's proficiency increased with more cases. There was no significant difference in the postoperative length of hospitalization between the two methods. The subgroup with predominant arm pain revealed the statistical difference in arm pain intensity changes between the two groups (p < 0.001). The rates of complication were 2.6% for the ACDF group and 1.9% for the C-BESS group. Conclusions: C-BESS and ACDF are effective surgical treatments for patients with symptomatic single-level cervical disc herniation in relieving relevant pain intensities and pain-related disabilities.

Clinical Study on Posterior Approach Microendoscopic Discectomy for Treatment of Lumbar Disc Herniation

Abstract:Objective To evaluate the posterior approach to lumbar vertebral disc herniation by microendoscopic discectomy. Methods Thirty-six patients with lumbar disc herniation were posterior approached with microendoscopic discectomy from January, 2002 through December, 2006 and the outcomes were evaluated according to the NaKai standards. Results All the patients were successfully completed the surgical procedure. The complications included 1 cerebrospinal fluid leak and 1 nerve root injury. Neither infection nor major vascular injury and other complications occurred. Followed-up from 12 to 36 months, average 24 months, demonstrated that the functional recovery excellent rate was 91.67%. Conclusions Posterior approach microendoscopic discectomy for lumbar disc herniation have the advantages of minimal invasive surgical procedures, quicker recovery, definite effectiveness, and fewer complications, which is valid in treatment of lumbar disc herniation. The predominant factors for successful operation are skilled surgical technique and indication. Posterior approach microendoscopic discectomy for lumbar disc herniation is worthy to be recommended clinically.

Abstract:Objective: To evaluate the preliminary outcomes and clinical efficacy of a novel posterior biportal endoscopic technique in the treatment of CCDH. Method: A total of eleven patients with symptomatic CCDH who met the inclusion criteria underwent posterior biportal endoscopic discectomy between December 2021 and May 2023. The surgical procedure involved flavectomy, foraminotomy, pediculoplasty, and discectomy using 30° and 45° arthroscopes and specialised minimally invasive tools. Functional outcomes were assessed using the Japanese Orthopedic Association (JOA) scoring system, Neck Disability Index (NDI), and visual analogue scale (VAS) for axial neck pain. Clinical efficacy was evaluated at the final follow-up using the modified Macnab criteria. Results: All eleven patients successfully underwent posterior biportal endoscopic discectomy with a mean operative time of 82.7±10.1 minutes and mean estimated blood loss of 31.8±9.8 ml. The mean hospital stay was 5.2±1.1 days, and the mean follow-up period was 13.8±2.4 months. Significant improvements were observed in NDI, JOA and VAS scores. Clinical efficacy was rated as excellent in three patients, good in six patients, and fair in two patients according to the modified Macnab criteria. No cases of cervical instability or kyphosis were observed during postoperative follow-up. Conclusion: The novel posterior biportal endoscopic technique demonstrated significant clinical efficacy and safety in treating CCDH, with marked improvements in clinical outcomes, rapid postoperative recovery, and a low incidence of complications.
Comparison of clinical and radiographic outcomes between transforaminal endoscopic lumbar discectomy and microdiscectomy: a follow-up exceeding five years

Abstract:OBJECTIVE: To compare the long-term clinical and radiographic outcomes of transforaminal endoscopic lumbar discectomy (TELD) versus microdiscectomy (MD).METHODS: The data of 154 patients with lumbar disc herniation (LDH) who underwent TELD (n = 89) or MD (n = 65) were retrospectively analyzed. The patients' clinical outcomes were evaluated using visual analogue scales for leg and low back pain, the Japanese Orthopaedic Association (JOA) score, and the Oswestry Disability Index (ODI). The evolution of radiographic manifestations was observed during follow-up. Potential risk factors for a poor clinical outcome were investigated.RESULTS: During a mean follow-up of 5.5 years (range, 5-7 years), the recurrence rate was 4.49% in the TELD group and 1.54% in the MD group. All scores significantly improved from preoperatively to postoperatively in both groups (p < 0.01). The improvement in the ODI and JOA scores was significantly greater in the TELD than MD group (p < 0.05). Forty-seven patients (52.8%) in the TELD group and 32 (49.2%) in the MD group had Modic changes before surgery, most of which showed no changes at the last follow-up. The degeneration grades of 292 discs (71.0%) were unchanged at the last follow-up, while 86 (20.9%) showed improvement, mostly at the upper adjacent segment. No significant difference was observed in the intervertebral height index or paraspinal muscle-disc ratio.CONCLUSION: Both TELD and MD provide generally satisfactory long-term clinical outcomes for patients with LDH. TELD can be used as a reliable alternative to MD with less surgical trauma. Modic type II changes, decreased preoperative intervertebral height, and a high body mass index are predictors of a poor prognosis.

Abstract:Velocardiofacial syndrome (VCFS) is a genetic syndrome with a range of psychiatric symptoms. Identification of the gene(s) involved in its expression should lead to an improved understanding of both normal CNS development and how specific mutations contribute to psychiatric disorders. VCFS was first defined by Robert Shprintzen more than 20 years ago. Only in recent years, with the increased interest in molecular biology in psychiatry, has it received wider recognition. VCFS has an estimated prevalence of 1 in 4,000, making it the second most common genetic syndrome after Down syndrome. It is caused by a microdeletion in the long arm of chromosome 22. Several disorders, such as DiGeorge syndrome, some cases of Pierre-Rubin syndrome, and other rarer syndromes, are caused by a seemingly identical microdeletion. Identification of the gene(s) that are affected by the deletion will be required before it is known whether the same or different genes contribute to the expression of these disorders. Together with VCFS, they are collectively referred to as the 22q11 deletion syndromes (22qDS). Ninety percent of patients with 22qDS have a deletion of approximately 3 million base pairs, while 7% have smaller deletions of 1.5 million bases. Similar to other syndromes caused by a microdeletion, the molecular diagnosis of 22qDS is usually made by fluorescence in situ hybridization (FISH) (Ward et al., 1999). In FISH, a fluorescently labeled sequence of a few thousand nucleotides is constructed in the laboratory. This sequence is used as a probe that will bind to the complementary sequence of bases on chromosome 22. As there are two copies of each chromosome, two hybridization signals will appear on the FISH examination in the normal individual. In those cases in which a deletion is present, fluorescence will be detected on only one chromosome (Fig. 1). Ninety-five percent of 22qDS can be detected with this technique. The rest fail to be detected because they are caused by unique deletions or unbalanced translocations. There are several major clinical characteristics of 22qDS. Congenital heart disease is present in 74% of patients with 22qDS. The most common are tetralogy of Fallot, interrupted aortic arch, and ventricular septal defect. Screening of patients with cardiac anomalies ascertained at cardiac clinics determined that the deletion is present in 3% to 15% of them. Abstract:Abstract Background Data: Recently, transforaminal endoscopic discectomy (TED) has become accepted as a safe alternative procedure for microdiscectomy (MID) in lumbar disc surgery. Numerous studies compared microdiscectomy with interlaminar endoscopic discectomy; however, the number of studies comparing MID with TED is relatively limited. Purpose: To compare TED and MID in treating lower lumbar disc prolapse (LDP) and associated unilateral sciatica in terms of overall outcome, complications, and rate of recurrence. Study Design: Retrospective clinical case series. Patients and Methods: This retrospective study included one hundred patients with low back pain and unilateral sciatica due to lower lumbar herniated discs. They were divided into 2 groups, each one consisted of 50 patients: Group A underwent MID and Group B TED. Clinical assessments of all patients were conducted using Visual Analogue Score (VAS) and Oswestry Disability Index (ODI) preoperatively and at one-year postoperative follow-up. Results: In this study, one hundred patients were surgically treated (50 for MID and 50 for TED) from June 2017 to December 2018. The mean age was 40.44 ± 11.31 and 41.14 ± 11.60 years for MID and TED, respectively. Males were most affected in both TED and MID groups (76% in MID and 66% in TED). The most affected disc level in both groups was the L4-L5 level, representing 60% and 68% for MID and TED, respectively. The mean operative time was 63.82 ± 17.37 and 72.60 ± 16.90 minutes for MID and TED, respectively, with significant difference (p < /em> = 0.02). Upon comparing the postoperative values, all patients in both groups showed a significant improvement in their preoperative back pain, leg pain, and ODI scores. According to Macnab’s Outcome Criteria, in our study, the results were as follows: for the MED group, overall good to excellent outcomes in 92% (N = 46), fair in 4% (N = 2), and poor in 4% (N = 2); for the TED group, overall good to excellent outcomes in 86% (N = 43), fair in 6% (N = 3), and poor in 8% (N = 4). The mean hospital stay was (29.80± 31.73&14.76± 11.20 hours for MID and TED respectively with significant relation P=0.02). Compared to the post-operative values, all patients in both groups showed a significant improvement of their preoperative back pain, leg pain and ODI scores. According to Mac Nab’s Outcome Criteria, in our study, overall good to excellent results for MED group (46pts, 92%), fair (2pts 4%) and poor outcome in (2 pts 4%).and overall good to excellent of TED group was (43 pts, 86%), fair (3pts, 6%), poor outcome (4pts, 8%). Abstract:Background Context Anterior cervical disc replacement (ACDR) and minimally invasive posterior cervical foraminotomy (MI-PCF) have emerged as two increasingly popular alternatives to anterior cervical discectomy and fusion (ACDF) for the management of cervical radiculopathy. Both techniques provide advantages of segmental motion preservation and lower rates of adjacent segment degeneration (ASD) compared to ACDF. Purpose The purpose of this study was to analyze the clinical and functional outcomes of patients undergoing ACDR or MI-PCF for the treatment of unilateral cervical radiculopathy. Study Design/Setting Retrospective Cohort Review Patient Sample 152 patients were included (86 ACDR and 66 MI-PCF). Outcome Measures 1) Patient demographics; 2) Perioperative data; 3) Rates of complications and revisions; 5) Visual analogue scale (VAS) and Neck Disability Index (NDI) scores Methods A retrospective cohort review was performed to identify all patients at a single institution between 2012-2020 who underwent 1- or 2- level ACDR or MI-PCF from C3-C7 with a minimum follow-up of 24 months. Patient demographics, perioperative data, postoperative complications, and revisions were analyzed. Patient reported outcome measures including Visual analogue scale (VAS) and Neck Disability Index (NDI) scores were compared. Results The ACDR group had a significantly greater mean operative time (99.8 minutes vs 79.2 minutes, p<0.001), but comparable estimated blood loss and length of stay following surgical intervention (p=0.899). The overall complication rate was significantly greater in the ACDR group than the MI-PCF group (24.4% vs 6.2%; p=0.003) but was largely driven by approach-related dysphagia in 20.9% of ACDR patients. The MI-PCF group had significantly greater revision rates (13.6% vs 1.2%; p=0.002) with an average time to revision of 20.7 months in the MI-PCF group compared to 40.3 months in the ACDR group. The ACDR cohort had significantly greater improvements in NDI scores at the final follow-up (25.0 vs 21.3, p<0.001). Conclusion Our results suggest that ACDR offer clinically relevant advantages over MI-PCF in terms of long-term revision rates despite an increased approach-related risk of transient postoperative dysphagia. Additionally, patients in the ACDR cohort achieved greater mean improvements in NDI scores but these results may have limited clinical significance due to inability to reach minimally clinically important difference (MCID) thresholds.

Abstract:Objective is to analyze the surgical strategy, safety, and clinical results of percutaneous full-endoscopic discectomy through interlaminar or extraforaminal puncture technique for LDH. Preoperative CT and MRI were analyzed, which were based on the main location of the herniated disc and its relationship with compressed nerve root. Sixty-two patients satisfied the inclusion criteria during the period from August 2012 to March 2014. We use percutaneous full-endoscopic discectomy through different puncture technique to remove the protrusive NP for LDH. Sixty patients completed the full-endoscopic operation successfully. Their removed disc tissue volume ranged from 1.5 mL to 3.8 mL each time. Postoperative ODI and VAS of low back and sciatica pain were significantly decreased in each time point compared to preoperative ones. No nerve root injury, infection, and other complications occurred. The other two patients were shifted to open surgery. No secondary surgery was required and 91.6% of excellent-to-good ratio was achieved on the basis of Macnab criteria at postoperative 12 months. Acquired benefits are fewer complications, rapid recovery, complete NP removal, effective nerve root decompression, and satisfactory cosmetic effect as well. This is a safe, effective, and rational minimally invasive spine-surgical technology with excellent clinical outcome.

Abstract:BACKGROUND: Noncontiguous 2-level cervical disc herniation (NCT-CDH) is a common condition that often requires surgical intervention. In this study, we developed a surgical approach for the treatment of NCT-CDH using anterior percutaneous full-endoscopic single incision through the vertebral body. We provide a brief overview of its safety, efficacy, and feasibility, along with a description of our relevant surgical experience.METHODS: A retrospective study was conducted, involving 30 patients who were followed up for at least 12 months. Preoperative and postoperative visual analog scale, Japanese Orthopedic Association scores, Nurick scores, intervertebral disc height, and modified Macnab criteria were recorded. Patients underwent regular radiological evaluations throughout the follow-up period.RESULTS: Postoperative computed tomography, magnetic resonance imaging, and X-ray examinations revealed bone tunnel healing, intact drilled vertebral bodies without collapse, adequate decompression of the spinal canal, and normal cervical mobility. There was a significant improvement in postoperative visual analog scale, Japanese Orthopedic Association scores, Nurick scores, and modified Macnab criteria compared to the preoperative values (P < 0.05).CONCLUSIONS: Our study revealed that the anterior percutaneous full-endoscopic transcorporeal with single-incision treatment for NCT-CDH is a safe and feasible surgical method. Therefore, it can be considered as a viable treatment option for patients with NCT-CDH.

Abstract:Objective To evaluate clinical effects and relating problems of traditional opening lumbar discectomy(group A)and contemporary microendoscopy(MED)(group B)treated lumbar disc herniation.Methods According to clinical manifestation and image results,625 cases of lumbar disc herniation were operated upon with traditional open discectomy,546 cases with MED.Naikai's criteria was used to assess the clinical outcomes,with comparatively analyzing operating time、the volume of blood lose、complications and postoperative recovery time.Results The rate of exellent and good in group A was 94.2%,and group B was 98.3%.There was no significant differences between two groups(P0.05).The mean operating time in group A was 75.6 minutes,and 67.7 minutes in group B,which had no statistical significance(P0.05).There were 8 cases complication in group A,3 cases complication in group B,which had no statistical significance(P0.05).There was significance differences in the volumes of blood lose、postopereting recovery time between two groups(P0.05).Conclusion The clinical outcome of two groups are same satisfactory.The laminectomy discectomy is always main method to treat lumbar disc herniation.being a minimally invasive procedure in spinal surgery,MED has advantage of minimal incision、less blood lose、speed recovery and less damage to posterior column of lumbar spine,but it can't fully replace the convetional discectomy.However,under the condition of masterring the convetional discectomy skill、accurae operation indication and skilled operative procedure are the key to improve outcome and reduce the complication.

Abstract:OBJECTIVE: The most common used technique for posterolateral cervical disc herniations is anterior approach. However, posterior cervical laminotoforaminomy can provide excellent results in appropriately selected patients with foraminal stenosis in either soft disc prolapse or cervical spondylosis. The purpose of this study was to present the clinical outcomes following posterior laminoforaminotomy in patients with radiculopathy.MATERIALS AND METHODS: We retrospectively evaluated 35 patients diagnosed with posterolateral cervical disc herniation and cervical spondylosis with foraminal stenosis causing radiculopathy operated by the posterior cervical keyhole laminoforaminotomy between the years 2010 and 2015.RESULTS: The file records and the radiographic images of the 35 patients were assessed retrospectively. The mean age was 46.4 years (range: 34-66 years). Of the patients, 19 were males and 16 were females. In all of the patients, the neurologic deficit observed was radiculopathy. The posterolaterally localized disc herniations and the osteophytic structures were on the left side in 18 cases and on the right in 17 cases. In 10 of the patients, the disc level was at C5-6, in 18 at C6-7, in 2 at C3-4, in 2 at C4-5, in 1 at C7-T1, in 1 patient at both C5-6 and C6-7, and in 1 at both C4-5 and C5-6. In 14 of these 35 patients, both osteophytic structures and protruded disc herniation were present. Intervertebral foramen stenosis was present in all of the patients with osteophytes. Postoperatively, in 31 patients the complaints were relieved completely and four patients had complaints of neck pain and paresthesia radiating to the arm (the success of operation was 88.5%). On control examinations, there was no finding of instability or cervical kyphosis.CONCLUSION: Posterior cervical laminoforaminotomy is an alternative appropriate choice in both cervical soft disc herniations and cervical stenosis.
Comparison of the effectiveness and outcome of microendoscopic and open discectomy in patients suffering from lumbar disc herniation

Abstract:BACKGROUND: The purpose of our study is to compare the outcomes and effectiveness of MED vs OLD for lumbar disc herniation.OBJECTIVES: To identify the functional outcomes in terms of ODI score, VAS score complications in terms of intraoperative blood loss, use of general anesthesia, and morbidity in terms of total hospital stay between MED and OLD.METHODS: In our randomized prospective study we analyzed 60 patients with clinical signs and symptoms with 2 weeks of failed conservative treatment plus MRI or CT scan findings of lumbar disc herniation who underwent MED and OLD. The study was undertaken from November 2017 to January 2019 at Guangzhou Medical University of Second Affiliated Hospital, department of orthopedic surgery in spinal Unit, Guangzhou, China. Patients were divided into 2 groups i.e. who underwent MED group and the OLD group then we compared the preoperative and postoperative ODI and VAS score, duration of total hospital stay, intraoperative blood loss, and operation time.RESULTS: We evaluated 60 patients. Among them, 30 underwent MED (15 female and 15 male) and 30 underwent OLD 14 male 16 female. Surgical and anesthesia time was significantly shorter, blood loss and hospital stay were significantly reduced in patients having MED than OLD (<0.005). The improvement in the ODI in both groups was clinically significant and statistically (P < .005) at postoperative 1st day (with greater improvement in the MED group), at 6 weeks (P > .005), month 6 (>0.005) statistically no significant. The clinical improvement was similar in both groups. VAS and ODI scores improved significantly postoperatively in both groups. However, the MED group was superior to the OLD group with less time in bed, shorter operation time, less blood loss which is clinically and statistically significant (P < .05).CONCLUSIONS: The standard surgical treatment of lumbar disc herniation has been open discectomy but there has been a trend towards minimally invasive procedures. MED for lumbar spine disc herniation is a well-known but developing field, which is increasingly spreading in the last few years. The success rate of MED is about approximately 90%. Both methods are equally effective in relieving radicular pain. MED was superior in terms of total hospital stay, morbidity, and earlier return to work and anesthetic exposure, blood loss, intra-op time comparing to OLD. MED is a safe and effective alternative to conventional OLD for patients with lumbar disc herniation.

Abstract:Purpose Percutaneous endoscopic lumbar discectomy has been increasingly used in the treatment of lumbar disc herniation. However, there is no consensus on which method would be more effective between the transforaminal and interlaminar approach. Objective To compare clinical outcomes, patient satisfaction rate, reoperation rate, and residual symptoms between percutaneous endoscopic transforaminal discectomy (PETD) and percutaneous endoscopic interlaminar discectomy (PEID). Study design A comparative, retrospective, controlled trial. Setting The study was conducted at the Department of Orthopaedics at a local hospital. Methods From January 2015 to September 2020, consecutive patients who underwent PETD or PEID treatment for lumbar disk herniation (LDH) at the L5/S1 level in our department were retrospectively collected. Baseline data including age, gender, body mass index (BMI), smoking status, alcohol drinking, clinical symptoms, physical examination, and radiographic characteristics were documented. During the two to three year follow-up periods, patients were evaluated clinically, including clinical outcomes assessed by the visual analog score (VAS), patient satisfaction rates assessed by the North American Spine Society patient satisfaction index (PSI), recurrent rate, and residual symptoms. Results A total of 113 patients with PELD in our department were included in the current study, with 65 patients in the PETD group and 48 in the PEID group. Demographic characteristics including age, gender, height, BMI, cigarette smoking, alcohol consumption status, and diabetes did not show any significant difference between the PETD and PEID groups. The VAS scores of the two groups were similar preoperatively, but the postoperative VAS score of the PEID group was lower than that of the PETD group. There were 90.8% of patients in the PETD group who were satisfied with the operation compared to 97.9% in the PEID group. The recurrence rate did not differ between groups, with three patients in both groups. Regarding residual symptoms, there were more patients in the PETD group who reported low back pain during the follow-up periods. Limitation The main limitations are that all patients were operated by the same surgical team from the same site, and there was a lack of multicenter data. Conclusion Both PETD and PEID have satisfactory patient-reported outcomes for treating LDH. The PEID procedure results in fewer low back pain residual symptoms than the PELD procedure.
Early Endoscopic Discectomy in Preventing Degenerative Spinal Changes in Patients With Lumbar Disc Herniation

Abstract:In this study, we compare the outcomes of two patients with similar spinal pathologies who chose different treatment options. The first case involves a 38-year-old female with significant lumbar disc herniation and associated degenerative spinal changes, including type I Modic changes. She presented with sciatica and, after conservative treatments failed, underwent an endoscopic discectomy. This intervention led to a marked improvement in her pain and functional status, along with a partial resolution of the Modic changes and a decrease in multifidus atrophy and fatty infiltration, on her follow-up MRI. In the second case, we discuss a 33-year-old patient with a large disc herniation. Despite three years of conservative management, she developed Modic changes and sclerosis in the adjacent vertebral endplates and a worsening of multifidus atrophy and fatty infiltration. This report supports the consideration of early minimally invasive discectomy for young patients who do not benefit from conservative treatment, as a means to prevent the progression of degenerative spinal changes.
